Estudi retrospectiu que analitza les característiques de l’apertura palpebral en 22 pacients amb regeneració aberrant secundària a paràlisi adquirida del III nervi cranial. S’ha estudiat el percentatge d’apertura palpebral de l’ull parètic respecte a la posició primària i respecte a l’ull sà en 5 posicions de la mirada i la relació entre l’apertura palpebral en adducció i en depressió respecte a la posició primària mitjançant regressió lineal. S’ha comprovat que, rere un quadre de regeneració aberrant, no només hi ha una recuperació parcial o total de la parpella en posició primària, sinó una retracció palpebral en adducció i en depressió.

A case of meibomian carcinoma of the left eyelid is reported in a 72-year-old female patient. The tumor had been present on the left eyelid for months. Clinically, the tumor appeared as a reddish mass implanted on the external part of the free margin of the left superior eyelid. An excisional biopsy disclosed meibomian carcinoma. A total resection of the left superior eyelid was followed by plastic surgery. Results after a one-month follow-up were very satisfactory. This case is emphasizes the importance of an early diagnosis which enabled us to perform a rather conservative treatment limited to the removal of the affected eyelid. The diagnosis of meibomian carcinoma is infrequent but it must be kept in mind in cases of tumor without the typical clinical characteristics of a basal cell or squamous cell carcinoma. Complete removal surgery may bring a curative effect and histopathology has a key role in the diagnosis of meibomian carcinoma.

Friedreich's ataxia (FRDA), the most common autosomal recessive ataxia, is characterised by progressive ataxia with dysarthria of speech, loss of deep-tendon reflexes, impaired vibratory and proprioceptive sensations and corticospinal weakness with a Babinski's sign. Patients eventually also develop kyphoscoliosis, cardiomyopathy and diabetes mellitus. The disease is a GAA repeat disorder resulting in severely reduced levels of frataxin, with secondary increased sensitivity to oxidative stress. The anti-oxidative drug, idebenone, is effective against FRDA-associated cardiomyopathy. We provide detailed clinical, electrophysiological and biochemical data from 20 genetically confirmed FRDA patients and have analysed the relationship between phenotype, genotype and malondialdehyde (MDA), which is a marker of superoxide formation. We assessed the effects of idebenone biochemically by measuring blood MDA and clinically by serial measurements of the International Cooperative Ataxia Rating Scale (ICARS). The GAA repeat length influenced the age at onset (p <0.001), the severity of ataxia (p = 0.02), the presence of cardiomyopathy (p = 0.04) and of low-frequency hearing loss (p = 0.009). Multilinear regression analysis showed (p = 0.006) that ICARS was dependent on the two variables of disease duration (p = 0.01) and size of the GAA expansion (p = 0.02). We found no correlation to bilateral palpebral ptosis, visual impairment, diabetes mellitus or skeletal deformities, all of which appear to be signs of disease progression rather than severity. We discuss more thoroughly two underrecognised clinical findings: palpebral ptosis and GAA length-dependent low-frequency hearing loss. The average ICARS remained unchanged in 10 patients for whom follow-up on treatment was available (mean 2.9 years), whereas most patients treated with idebenone reported an improvement in dysarthria (63%), hand dexterity (58%) and fatigue (47%) after taking the drug for several weeks or months. Oxidative stress analysis showed an unexpected increase in blood MDA levels in patients on idebenone (p = 0.04), and we discuss the putative underlying mechanism for this result, which could then explain the unique efficacy of idebenone in treating the FRDA-associated cardiomyopathy, as opposed to other antioxidative drugs. Indeed, idebenone is not only a powerful stimulator of complexes II and III of the respiratory chain, but also an inhibitor of complex I activity, then promoting superoxide formation. Our preliminary clinical observations are the first to date supporting an effect of idebenone in delaying neurological worsening. Our MDA results point to the dual effect of idebenone on oxidative stress and to the need for controlled studies to assess its potential toxicity at high doses on the one hand, and to revisit the exact mechanisms underlying the physiopathology of Friedreich's ataxia on the other hand, while recent reports suggest non-oxidative pathophysiology of the disease.

La prótesis maxilofacial comprende aquellos dispositivos artificiales que restauran una parte ausente de la cara, debido a tumores, traumatismos, o como resultado de una anomalía congénita. El objetivo de este artículo es la exposición de un caso clínico que precisaba la confección de una prótesis óculo-palpebral tras sufrir una pérdida de sustancia por causa oncológica. Este tipo de prótesis son de las más difíciles de elaborar con buenos resultados pues intentan reconstruir un órgano móvil con una prótesis estática. La prótesis maxilofacial tiene la finalidad de aumentar la calidad de vida del paciente y ayudar a la seguridad emocional de los mismos, quedando así rehabilitados para poder integrarse nuevamente en la sociedad.

Breast hypertrophy, combined with massive ptosis with a suprasternal notch-to-nipple distance of more than 40 cm, remains an endeavour. Different refinements of the initial technique with free nipple grafts have been described to circumvent the problems of nipple underprojection, areolar hypopigmentation and loss of sensibility secondary to nipple grafting, as well as lacking breast projection due to scarce glandular tissue. Techniques relying on nipple areola complex transposition, rather than grafting, have been described with inferior, superomedial and medial pedicles. The aim of this study is to present the results obtained in a series of 10 patients suffering from bilateral breast hypertrophy with massive ptosis, which was defined as a distance >40 cm from the suprasternal notch-to the nipple. All breasts were managed with a superior pedicle and inverted T technique. The mean preoperative suprasternal notch-to-nipple distance was 44 ± 2 cm, and the resection weight ranged from 800 to 2490 g per breast with an average of about 1450 g in this patient population presenting with overweight or obesity. With a mean nipple areola complex (NAC) lift of 20 ± 3 cm, neither nipple nor areola necrosis was observed. One partial epidermolysis of the areola and two cases of delayed wound healing at the trifurcation point of the inverted T were conservatively managed. Only one re-operation was necessary for an important wound dehiscence of the lateral part of the horizontal scar. These results underscore the safety of the superior pedicle technique in cases of massive ptosis with transposition of the NAC of approximately 20 cm, that is, a pedicle length of about 25 cm.

BACKGROUND: Bilateral ptosis is a very interesting clinical challenge for doctors because of the multiple possible localizations of a lesion which can lead to this neurological sign. OBJECTIVES: Through this case report, we aim to determine the difference between an apraxia of lid opening (ALO) with difficulty in initiating the act of lid elevation, in spite of adequate understanding, motor control and cranial nerve pathways, and a bilateral ptosis with a lesion in the oculomotor nucleus or blepharospasm. METHODS: The case report of a 50-year-old patient presenting bilateral ptosis and multiple ischemic lesions in the brainstem and bilateral frontal lobe lesions after the emergency removal of a large frontal tumor. RESULTS: Our patient had an ALO according to the neurological follow-up and showed the ability, after a few weeks, of initiating the act of opening her eyes with her hand. The ophthalmic evaluation confirmed that in her case the ALO was associated with a nuclear lesion of the oculomotor nerve secondary to a midbrain lesion. CONCLUSION: Our case report confirms multiple differential diagnoses in bilateral ptosis and the importance of clinical examination in spite of good neurological imaging.

OBJETIVO: avaliar as complicações, vantagens e desvantagens da abordagem subtarsal quando utilizada para exposição da borda infra-orbital e do assoalho da órbita em fraturas zigomático-orbitais. MÉTODOS: Estudo retrospectivo de 41 incisões subtarsais empregadas em 39 pacientes com fraturas do complexo zigomático e/ou do assoalho da órbita do tipo blow-out, no período de janeiro de 2002 a dezembro de 2006. RESULTADOS: As complicações observadas foram conjuntivite, epífora e cicatriz aparente em seis (14,6%), cinco (12,2%) e dois (4,9%) dos casos, respectivamente. Não houve presença de ectrópio, entrópio ou esclera aparente nos pacientes reavaliados. CONCLUSÃO: As principais vantagens dessa abordagem são os resultados estéticos e funcionais associados aos benefícios trans-operatórios.

A citologia conjuntival é um importante meio de diagnóstico de afecções da superfície ocular. Buscam-se técnicas que forneçam quantidade e qualidade celular, com uso de instrumentos que provoquem mínimo trauma e que diminuam as chances de danos iatrogênicos ao olho. Existem diversas técnicas de coleta de células, entre elas encontram-se: impressão, esfoliação e punção por agulha fina. Dentre os métodos utilizados para esfoliação, o uso da escova citológica fornece resultados superiores em vários parâmetros, incluindo a qualidade das células. Estudou-se a citologia conjuntival por esfoliação com escova citológica, utilizada para coleta de material da cérvix uterina, tendo como objetivos determinar se tal instrumento se adequaria à coleta de material da face palpebral da membrana nictitante e da conjuntiva palpebral de felinos sadios. Foram avaliados os seguintes parâmetros: facilidade de execução da técnica, possibilidade de ocorrência de danos iatrogênicos e quantidade e qualidade de células coletadas. Cinquenta gatos machos (58%) e fêmeas (42%), com ou sem raça definida, participaram do estudo. Apenas gatos isentos de alterações oculares no exame físico foram incluídos. A escova citológica se mostrou um instrumento de fácil utilização, que fornece células em quantidade satisfatória e com morfologia preservada. Comparada a conjuntiva palpebral, a face palpebral da membrana nictitante se mostrou um local mais adequado à realização da coleta de amostras citológicas, pela maior facilidade de execução da técnica e menor possibilidade de danos iatrogênicos. No que diz respeito à quantidade e qualidade celular, não houve diferença significativa entre os dois locais de coleta. Foi possível observar células provenientes das diferentes camadas do epitélio conjuntival com predomínio de células intermediárias e ausência de células caliciformes.

Tick paralysis (TP) is a rare disease with rapid progression and potential fatal evolution. Immediately after the diagnosis, removal of all ticks from the body of the patient is mandatory. The present study reports for the first time a human case of the disease in Brazil. The patient had loss of muscle strength, decreased reflexes and marked palpebral ptosis. Six hours after removal of the last tick, the ptosis improved and on the following day, the patient had near total regression of the symptoms. This report emphasizes the possible presence of similar cases that should be promptly diagnosed and quickly treated. A new induction pattern for TP in humans associated with immature stages of ticks is also presented.

O intuito deste é apresentar um caso raro de síndrome de Horner na infância. Trata-se de uma criança do sexo masculino, com idade de 2 anos e 1 mês, que apresentava desde o nascimento ptose palpebral, miose e anidrose da hemiface esquerda. A instilação de fenilefrina 2,5% provocou midríase, com pupilas isocóricas, confirmando o diagnóstico. A história e o exame clínico auxiliam a localizar o nível da lesão e a estabelecer a etiologia do quadro.

Apresenta-se relato de caso de paciente parda, com 47 anos de idade, havendo lesão pigmentada na margem palpebral inferior do olho direito, de aspecto irregular e heterogêneo, suspeitando-se clinicamente de lesão maligna. A paciente foi tratada com base no resultado da citologia de impressão e diagnosticada pelo exame histológico. Demonstra-se a importância da citologia para o planejamento da ressecção de lesões suspeitas como um método seguro e efetivo para assim se evitar cirurgias em áreas extensas da superfície ocular e de anexos.

OBJETIVO: Avaliar quantitativamente as mudanças da posição palpebral e as medidas da fenda palpebral de indivíduos acima dos 50 anos. MÉTODOS: Estudo observacional, tendo sido avaliados 325 indivíduos, com idade acima de 50 anos, segundo distância intercantal, largura e altura da fenda palpebral, ângulo palpebral externo e interno, distância entre o reflexo pupilar e a margem da pálpebra superior (distância reflexo-margem) e a área total da fenda palpebral. Utilizou-se filmadora Sony Lithium para obtenção das imagens digitais, com o indivíduo fixando um objeto a 1 metro de distância, sendo as imagens transferidas posteriormente para computador McIntosh G4 e processadas pelo programa NIH 1.58. Os dados foram submetidos à análise estatística. RESULTADOS: Os participantes apresentavam dermatocálase (96,5%), ptose do supercílio (60,8%), prolapso de gordura orbital (50,0%) ou ptose palpebral (39,1%). As alterações foram bilaterais em 68,8% dos indivíduos. A distância intercantal aumentou com a idade; a largura da fenda palpebral, a distância reflexo-margem e a medida do ângulo externo diminuíram nos mais idosos. As diferenças foram mais significativas quando os olhos foram estudados separadamente. CONCLUSÃO: A distância intercantal aumenta, ao passo que a largura da fenda palpebral, a distância reflexo-margem e a área total da fenda palpebral diminuem com o aumento da idade.
