Malignant Pleural Mesothelioma (MPM) is an aggressive cancer that is often diagnosed at an advanced stage and is characterized by a long latency period (20-40 years between initial exposure and diagnosis) and prior exposure to asbestos. Currently accurate diagnosis of MPM is difficult due to the lack of sensitive biomarkers and despite minor improvements in treatment, median survival rates do not exceed 12 months. Accumulating evidence suggests that aberrant expression of long non-coding RNAs (lncRNAs) play an important functional role in cancer biology. LncRNAs are a class of recently discovered non-protein coding RNAs >200 nucleotides in length with a role in regulating transcription. Here we used NCode long noncoding microarrays to identify differentially expressed lncRNAs potentially involved in MPM pathogenesis. High priority candidate lncRNAs were selected on the basis of statistical (P<0.05) and biological significance (>3-fold difference). Expression levels of 9 candidate lncRNAs were technically validated using RT-qPCR, and biologically validated in three independent test sets: (1) 57 archived MPM tissues obtained from extrapleural pneumonectomy patients, (2) 15 cryopreserved MPM and 3 benign pleura, and (3) an extended panel of 10 MPM cell lines. RT-qPCR analysis demonstrated consistent up-regulation of these lncRNAs in independent datasets. ROC curve analysis showed that two candidates were able to separate benign pleura and MPM with high sensitivity and specificity, and were associated with nodal metastases and survival following induction chemotherapy. These results suggest that lncRNAs have potential to serve as biomarkers in MPM.

Latent transforming growth factor-beta (TGF-beta) binding proteins (LTBPs) -1, -3 and -4 are ECM components whose major function is to augment the secretion and matrix targeting of TGF-beta, a multipotent cytokine. LTBP-2 does not bind small latent TGF-beta but has suggested functions as a structural protein in ECM microfibrils. In the current work we focused on analyzing possible adhesive functions of LTBP-2 as well as on characterizing the kinetics and regulation of LTBP-2 secretion and ECM deposition. We also explored the role of TGF-beta binding LTBPs in endothelial cells activated to mimic angiogenesis as well as in malignant mesothelioma. We found that, unlike most adherent cells, several melanoma cell lines efficiently adhered to purified recombinant LTBP-2. Further characterization revealed that the adhesion was mediated by alpha3beta1 and alpha6beta1 integrins. Heparin also inhibited the melanoma cell adhesion suggesting a role for heparan sulphate proteoglycans. LTBP-2 was also identified as a haptotactic substrate for melanoma cell migration. We used cultured human embryonic lung fibroblasts to analyze the temporal and spatial association of LTBP-2 into ECM. By We found that LTBP-2 was efficiently assembled to the ECM only in confluent cultures following the deposition of fibronectin (FN) and fibrillin-1. In early, subconfluent cultures it remained primarily in soluble form after secretion. LTBP-2 colocalized transiently with FN and fibrillin-1. Silencing of fibrillin-1 expression by lentiviral shRNAs profoundly disrupted the deposition of LTBP-2 indicating that the ECM association of LTBP-2 depends on a pre-formed fibrillin-1 network. Considering the established role of TGF-beta as a regulator of angiogenesis we induced morphological activation of endothelial cells by phorbol 12-myristate 13-acetate (PMA) and followed the fate of LTBP-1 in the endothelial ECM. This resulted in profound proteolytic processing of LTBP-1 and release of latent TGF-beta complexes from the ECM. The processing was coupled with increased activation of MT-MMPs and specific upregulation of MT1-MMP. The major role of MT1-MMP in the proteolysis of LTBP-1 was confirmed by suppressing the expression with lentivirally induced short-hairpin RNAs as well as by various metalloproteinases inhibitors. TGF-beta can promote tumorigenesis of malignant mesothelioma (MM), which is an aggressive tumor of the pleura with poor prognosis. TGF-beta activity was analyzed in a panel of MM tumors by immunohistochemical staining of phosphorylated Smad-2 (P-Smad2). The tumor cells were strongly positive for P-Smad2 whereas LTBP-1 immunoreactivity was abundant in the stroma, and there was a negative correlation between LTBP-1 and P-Smad2 staining. In addition, the high P-Smad2 immunoreactivity correlated with shorter survival of patients. mRNA analysis revealed that TGF-beta1 was the most highly expressed isoform in both normal human pleura and MM tissue. LTBP-1 and LTBP-3 were both abundantly expressed. LTBP-1 was the predominant isoform in established MM cell lines whereas the expression of LTBP-3 was high in control cells. Suppression of LTBP-3 expression by siRNAs resulted in increased TGF-beta activity in MM cell lines accompanied by decreased proliferation. Our results suggest that decreased expression of LTBP-3 in MM could alter the targeting of TGF-beta to the ECM and lead to its increased activation. The current work emphasizes the coordinated process of the assembly and appropriate targeting of LTBPs with distinct adhesive or cytokine harboring properties into the ECM. The hierarchical assembly may have implications in the modulation of signaling events during morphogenesis and tissue remodeling.

Malignant pleural mesothelioma (MPM) is a rare aggressive cancer of the pleura. Asbestos exposure (through inhalation) is the most well established risk factor for mesothelioma. The current standard of care for patients suffering from MPM is a combination of cisplatin and pemetrexed (or alternatively cisplatin and raltitrexed). Most patients, however, die within 24 months of diagnosis. New therapies are therefore urgently required for this disease. Lysine acetyltransferases (KATs) including KAT5 have been linked with the development of cisplatin resistance. This gene may therefore be altered in MPM and could represent a novel candidate target for intervention. Using RT-PCR screening the expression of all known KAT5 variants was found to be markedly increased in malignant tumors compared to benign pleura. When separated according to histological subtype, KAT5 was significantly overexpressed in both the sarcomatoid and biphasic subgroups for all transcript variants. A panel of MPM cell lines including the normal pleural cells LP9 and Met5A was screened for expression of KAT5 variants. Treatment of cells with a small molecule inhibitor of KAT5 (MG-149) caused significant inhibition of cellular proliferation (p<0.0001), induction of apoptosis and was accompanied by significant induction of pro-inflammatory cytokines/chemokines.

A comparison of Mycobacterium tuberculosis complex isolates from seals (pinnipeds) in Australia, Argentina, Uruguay, Great Britain and New Zealand was undertaken to determine their relationships to each other and their taxonomic position within the complex. Isolates from 30 cases of tuberculosis in six species of pinniped and seven related isolates were compared to representative and standard strains of the M. tuberculosis complex. The seal isolates could be distinguished from other members of the M. tuberculosis complex, including the recently defined ‘Mycobacterium canettii’ and ‘Mycobacterium caprae’, on the basis of host preference and phenotypic and genetic tests. Pinnipeds appear to be the natural host for this ‘seal bacillus’, although the organism is also pathogenic in guinea pigs, rabbits, humans, Brazilian tapir (Tapirus terrestris) and, possibly, cattle. Infection caused by the seal bacillus is predominantly associated with granulomatous lesions in the peripheral lymph nodes, lungs, pleura, spleen and peritoneum. Cases of disseminated disease have been found. As with other members of the M. tuberculosis complex, aerosols are the most likely route of transmission. Malignant pleural mesothelioma is an asbestos-related neoplasm with poor prognosis, refractory to current therapies, the incidence of which is expected to increase in the next decades. Female gender was identified as a positive prognostic factor among other clinical and biological prognostic markers for malignant mesothelioma, yet a role of estrogen receptors (ERs) has not been studied. Our goal was to investigate ERs expression in malignant mesothelioma and to assess whether their expression correlates with prognosis. Immunohistochemical analysis revealed intense nuclear ER beta staining in normal pleura that was reduced in tumor tissues. Conversely, neither tumors nor normal pleura stained positive for ER alpha. Multivariate analysis of 78 malignant mesothelioma patients with pathologic stage, histologic type, therapy, sex, and age at diagnosis indicated that FRO expression is an independent prognostic factor of better survival. Moreover, studies in vitro confirmed that treatment with 17 beta-estradiol led to an ER beta-mediated inhibition of malignant mesothelioma cell proliferation as well as p21(CIP1) and p27(KIP1) up-regulation. Consistently cell growth was suppressed by ER beta overexpression, causing a G(2)-M-phase cell cycle arrest, paralleled by cyclin B1 and survivin down-regulation. Our data support the notion that ER beta acting as a tumor suppressor is of high potential relevance to prediction of disease progression and to therapeutic response of malignant mesothelioma patients. Patients who develop a severe stenosis in biological pulmonary conduits previously implanted for pulmonary outflow trunk reconstructions are treated either by surgical re-replacement, or by transcatheter stent-valve implantation through a femoral vein access. A catheter-based sub-xyphoidian access through the right ventricle for stent-valve positioning in a pulmonary conduit has rarely been proposed. We describe the case of a 20-year-old man who underwent a pulmonary trunk reconstruction for a congenital pulmonary valve dysplasia and a few years later developed a stenosis in the pulmonary conduit. He was successfully treated with a 23 mm Edwards Sapien stent-valve implantation in pulmonary position, through an unusual right ventricular, sub-xyphoidian access and without contrast medium injections and pleura opening.

Paciente de 39 años con antecedente de Síndrome de Tolosa Hunt, quien ingresa al servicio de urgencias por disminución de la agudeza visual y cefalea. Dado el antecedente se considera realizar estudios complementarios para descartar enfermedad granulomatosa, como causa del síndrome, encontrando los siguientes hallazgos incidentales en las imágenes del tórax. El secuestro pulmonar es una anomalía congénita caracterizada por tejido pulmonar displásico, sin conexión normal con la vía aérea, irrigado por la circulación arterial sístémica. Hay dos tipos de secuestros, el intralobar y el extralobar. El intralobar (75%), puede ser congénito o adquirido como secuela de infecciones recurrentes localizadas, está cubierto por pleura del pulmón normal adyacente y generalmente su drenaje venoso es a través del sistema venoso pulmonar ipsilateral. El secuestro extralobar (25%), es considerado una anomalía congénita, está revestido por su propia pleura y su drenaje venoso es sistémico. Tanto el secuestro intralobar como extralobar reciben suplencia arterial sistémica. En los estudios de imágenes, el secuestro se puede presentar como consolidación focal, nódulo o masa, de localización paravertebral, contiguo al hemidiafragma, en los lóbulos inferiores (segmento posterior), con mayor frecuencia en el lado izquierdo (98%) y con una suplencia arterial anómala dada por una o múltiples ramas (15%) generalmente de la aorta torácica y su drenaje puede ser al sistema venoso pulmonar y con menor frecuencia a la circulación sistémica.

Introdução: A dor é um importante fator de incremento da morbidade e mortalidade em pacientes submetidos a procedimentos cirúrgicos que incluem toracotomias. Diversos fatores contribuem para que esses pacientes apresentem um alto grau de dor no pós-operatório, entre os quais a secção da pele, músculos e pleura, retração dos músculos e ligamentos pelo afastador de Finochietto, irritação da pleura e nervos intercostais pelos drenos tubulares torácicos e fraturas ocasionais dos arcos costais. O aumento das taxas de morbidade e mortalidade é dado principalmente à respiração superficial decorrente da pouca mobilidade da parede torácica e conseqüente à dor e pela perda da efetividade do principal mecanismo de eliminação de secreções da árvore traqueobrônquica (tosse), resultando em atelectasias, inadequado gradiente ventilação / perfusão, hipoxemia e pneumonia. Uma vez caracterizada a necessidade de atenuação da dor como fator primordial na melhora dos índices de morbidade e mortalidade no período pós-operatório de cirurgia torácica, torna-se imperiosa uma análise das terapêuticas disponíveis na atualidade para tanto. Objetivos: Avaliar a utilização de três diferentes métodos de analgesia: 1. bloqueio peridural com morfina (BPM); 2. morfina parenteral (MP); e 3. bloqueio intercostal extrapleural contínuo com lidocaína” (BIC), em pacientes submetidos a procedimentos que incluíram toracotomias em sua execução, além de analisar o custo financeiro desses métodos. Materiais e métodos: Trata-se de um estudo prospectivo, randomizado, no qual foram analisados 79 pacientes, submetidos a toracotomias, subdivididos de forma aleatória em três grupos, de acordo com a modalidade terapêutica instituída: 25 pacientes no grupo BIC, 29 pacientes no grupo BPM e 25 pacientes no grupo MP. Cada paciente foi observado e analisado por profissionais de enfermagem previamente treinados. As variáveis analisadas foram a dor e a sedação. (quantificadas através de escores e analisadas através do método de Kruskal-Wallis com correção pelo teste de Dunn), além do custo financeiro de cada método e da necessidade de administração de opióides adicionais. Resultados: As variáveis dor e sedação foram obtidas através das seguintes medianas, respectivamente: grupos BIC (2,5 e 0); BPM (4 e 0) e MP (3,5 e 0). O custo financeiro foi de US$ 78,69 para o grupo BIC; US$ 28,61 para o grupo BPM e US$ 11,98 para o grupo MP. A necessidade adicional de opióide foi de 4,2 mg/dia para o grupo BIC; 5,7 mg/dia para o grupo BPM e 10,7 mg/dia para o grupo MP. Conclusões: A intensidade da dor foi significativamente menor no grupo BIC, quando comparado ao grupo MP. Não foram identificadas diferenças significativas de intensidade da dor quando comparados os grupos BIC versus BPM e BPM versus MP. A intensidade de sedação foi significativamente maior no grupo MP quando comparado aos grupos BIC e BPM. Não foram evidenciadas diferenças significativas quanto à sedação entre os grupos BIC e BPM. O custo financeiro do grupo MP foi sensivelmente menor quando comparado aos grupos BIC e BPM. A necessidade adicional de morfina foi significativamente maior no grupo MP, quando comparados aos grupos BIC e BPM.

The Iota, Kappa and Lambda commercial carrageenans are rarely pure and normally contain varying amounts of the other types of carrageenans. The exact amount of impurity depends on the seaweed source and extraction procedure. Then, different analysis methods have been applied for determination of the main constituents of carrageenans because these three carrageenans are extensively used in food, cosmetic and pharmaceutical industry. The electrophoresis of these compounds proved that the carrageenans are constituted by sulfated polysaccharides. These compounds were characterized by colorimetric methods and was observed that the Lambda carrageenan shown the greater value (33.38%) of sulfate. These polymers were examined by means of 13C NMR spectroscopy and infrared spectra. The polysaccharides consisted mainly of units alternating of sulfated galactoses and anhydrogalactoses. The aim of the study was also to test the inflammatory action of these different polysaccharides. A suitable model of inflammation is acute sterile inflammation of the rat hind limb induced by carrageenan. Paw edema was induced by injecting carrageenans (κ, ι and λ) in saline into the hind paw of a male Wistar rats (175–200 g). The pathway to acute inflammation by carrageenan (kappa, iota and lambda) were expressed as time-edema dependence and measured by paw edema volume. For this purpose, was used an apparatus (pakymeter), which makes it possible to measure the inflammation (swelling of the rat foot) with sufficient accuracy. The results showed that κ-carrageenan (1%) have an edema of 3.7 mm and the paw edema increase was time and dose dependent; the ι-carrageenan (0.2%) caused an edema of 4 mm and the λ-carrageenan (1%) caused an edema of 3.6 mm. Other model was used in this study based in the inflammation of pleura for comparatives studies. Injection of carrageenans into the pleural cavity of rat induced an acute inflammatory response characterized by fluid accumulation in the pleural cavity, a large number of neutrophils and raised NO production. The levels of NO were measured by Griess reactive. The ι-carrageenan caused the greater inflammation, because it has high concentration of nitrite/nitrate (63.478 nmoles/rat), exudato volume (1.52 ml) and PMNs (4902 x 103 cells). Quantitative evaluation of inflammations of rats is a useful and important parameter for the evaluation of the efficacy of anti-inflammatory drugs

OBJETIVO: Avaliar, em cadáveres de cães, uma técnica para remoção do esôfago torácico sem toracotomia e dois métodos de substituição do esôfago torácico. MÉTODOS: Foram utilizados 27 cadáveres de cães. Estes foram aleatoriamente divididos em três grupos de nove animais, em que se estudou: G1 - esofagectomia torácica total pelo método de invaginação retrógrada; G2 - esofagectomia torácica total com substituição esofágica pelo estômago inteiro; G3 - esofagectomia torácica total com substituição esofágica por um gastrotubo confeccionado de acordo com a técnica de Büchler de gastroplastia por rotação do fundo. Após a ressecção esofágica no grupo 1, a integridade da rota intratorácica foi avaliada por endoscopia e solução de azul de metileno a 1%. RESULTADOS: A ruptura da pleura visceral ocorreu em todos os animais, especialmente no terço caudal. Entretanto, a rota transtorácica mediastinal permitiu a elevação de ambos os substitutos esofágicos (G2 e G3) para a realização da anastomose com a extremidade caudal do esôfago cervical. CONCLUSÕES: A substituição por estômago inteiro apresentou menor tensão na anastomose, maior facilidade e rapidez comparada à técnica de gastroplastia por rotação do fundo. Os resultados em cadáveres suportam a realização de estudos clínicos para validação da técnica.

Background: Persistent air leak after pulmonary resection is a difficult complication for thoracic surgeons to manage. Objectives: To show the results of our experience treating persistent pleuropulmonary air leak with autologous blood and review the literature on this specific method of treatment. Methods: Retrospective study of patients with persistent aerial pleuropulmonary fistula treated with autologous blood. The patient's own blood was collected from a peripheral vein and directly introduced through the pleural drain. An inverted siphon was located in the drainage system to avoid prolonged clamping of the drain. This siphon impeded blood return but not air escape. Results: Between January 2001 and August 2008, 27 patients were treated by the above method. Patient age ranged from 2 to 74 years, and 78% were male. Each procedure used a mean quantity of 92 ml blood. Mean persistent air leak time before pleurodesis was 10.6 days and mean time to fistula resolution after pleurodesis was 1.5 days. Twenty-three (85%) patients had persistent pleuropulmonary air leak closed with the above procedure. Conclusion: Treating persistent pleuropulmonary air leak with autologous blood is promising, but further studies are required to quantify its real effectiveness. Copyright (C) 2009 S. Entre 6.316 necropsias realizadas, no período de 1969 a 2000, no Departamento de Patologia da Faculdade de Medicina de Botucatu da Universidade Estadual Paulista (FMB/UNESP), foram encontradas 240 necropsias com diagnóstico de tuberculose, sendo 117 registradas como doença principal (grupo tuberculose/doença principal) e 123, como doença associada (grupo tuberculose/doença associada). No grupo tuberculose/doença principal, 100% apresentavam tuberculose ativa, com 80 ocorrências em homens e 37 em mulheres e média de idade de 47,7 anos. Caquexia (37,2%) e cor pulmonale (23,7%) foram as co-morbidades mais prevalentes nesse grupo. Os pulmões estavam comprometidos em 95,7% dos casos, seguido pelos linfonodos (38,9%) e pleuras (27,1%). Os padrões morfológicos das lesões pulmonares foram: 1. miliar, 58,4%; 2. cavernas, 56,6%; 3. fibrose, 41,5%; 4. bronquiectasias, 26,5%; 5. enfisema, 19,4%; e 6. cistos, 1,7%. Cavernas foram observadas em diferentes áreas, com discreto predomínio no pulmão direito, sendo a localização apical preferencial (100%) e quase sempre bilaterais (94%). Lesões miliares, bronquiectasias e fibrose estavam distribuídas, de forma difusa e aleatória, nos pulmões. No grupo tuberculose/doença associada também houve predomínio de homens (70,8%). Nesses casos, a AIDS e o alcoolismo crônico foram as doenças principais mais freqüentes, representando 22% e 16,3%, respectivamente. Os pulmões estavam acometidos em 85,2% dessas necropsias (100% nos casos de AIDS), seguidos pelos linfonodos (31,9%) e baço (28,6%). Nossos dados corroboram a importância da necropsia no estudo e no conhecimento da tuberculose, fornecendo subsídios para uma melhor abordagem clínica e epidemiológica dessa doença em nossa região.

CONTEXTO: A desnutrição protéico-energética constitui causa previsível para o desenvolvimento de complicações pós-operatórias e pior prognóstico de pacientes cirúrgicos. OBJETIVO: Estudar a associação de indicadores de estado nutricional com estádio da doença e as principais complicações e mortalidade pós-operatória de pacientes com câncer de esôfago. MÉTODO: Foram avaliados retrospectivamente 100 prontuários de pacientes com câncer de esôfago (38-81 anos) de ambos os sexos (85% masculino e 15% feminino) submetidos a esofagectomia (n = 25) e gastrojejunostomia (n = 75), no período de 1995 a 2004. Os dados coletados foram: história clínica, exame endoscópico, estádio (TNM-UICC), estado nutricional (índice de massa corporal, percentual de perda de peso - %PP, albuminemia e contagem de linfócitos total) e evolução clínica pós-operatória. Houve composição dos grupos de acordo com o porte da cirurgia (grande x pequeno). Foi realizada a associação entre as complicações pós-operatórias e a mortalidade (após pós-operatório de 30 dias). As comparações entre médias dos dois grupos foram feitas pelo teste t de Student e a existência de associações entre variáveis testadas pelos testes de χ2 ou exato de Fisher com P = 0,05. RESULTADOS: Houve predomínio dos tumores avançados (estádio III e IV), com a presença de disfagia em 95% dos pacientes e perda ponderal >10%, anterior ao diagnóstico, em 78%. A obstrução esofágica, presente em 77 pacientes, foi associada (P = 0,0021) com o baixo índice de massa corporal (desnutrição protéico-energética). A %PP e a hipoalbuminemia associaram-se estatisticamente (P<0,05) com o estádio avançado da doença. As complicações pós-operatórias precoces ocorreram em 69,2% e 30,7% dos pacientes submetidos a esofagectomia e ostomias, respectivamente, com predomínio das infecciosas nas ostomias (80%) e as pleuropulmonares nas esofagectomias (61%). A albuminemia foi menor nos pacientes submetidos as ostomias, tendo sido a hipoalbuminemia associada (P<0,05) com a ocorrência de complicações pós-operatórias e mortalidade. A %PP e a contagem de linfócitos total associaram-se com as complicações pós-operatórias precoces e infeccionas nas ostomias e a contagem de linfócitos total, com a mortalidade operatória nas esofagectomias. CONCLUSÕES: O estado de DPE esteve associado às complicações pós-operatórias apenas nos pacientes submetidos a ostomias, sem presença destas associações nas esofagectomias.
