The development of Australian forms of the Childhood Asthma Questionnaires (CAQs) is reported. Focus group methods and psychometric analyses were used to establish the conceptual, semantic and technical equivalence of these forms with the UK versions. Both versions also provide for data collection from non-asthmatic youngsters. The internal consistency was found to be acceptable (Cronbach's alpha 0.52-0.90) and the health-related quality of life (HRQoL) scores were found to vary with asthma severity (p < 0.05). Comparison with the UK data revealed that the non-asthmatic scores were higher for Australian than British children (p < 0.001) but that the scores for children with asthma did not differ between the two countries. It was only In the Australian sample that the group with asthma reported impaired HRQoL when compared to their healthy peers. These findings were interpreted in the context of cultural expectations of life quality and conclusions are presented regarding the importance of the gap between experience and expectations. One of the challenges in screening for dementia in developing countries is related to performance differences due to educational and cultural factors. This study evaluated the accuracy of single screening tests as well as combined protocols including the Mini-Mental State Examination (MMSE), Verbal Fluency animal category (VF), Clock Drawing test (CDT), and Pfeffer Functional Activities Questionnaire (PFAQ) to discriminate illiterate elderly with and without Alzheimer`s disease (AD) in a clinical sample. Cross-sectional study with 66 illiterate outpatients diagnosed with mild and moderate AD and 40 illiterate normal controls. Diagnosis of AD was based on NINCDS-ADRDA. All patients were submitted to a diagnostic protocol including a clinical interview based on the CAMDEX sections. ROC curves area analyses were carried out to compare sensitivity and specificity for the cognitive tests to differentiate the two groups (each test separately and in two by two combinations). Scores for all cognitive (MMSE, CDT, VF) and functional assessments (PFAQ) were significantly different between the two groups (p < 0.001). The best screening instruments for this sample of illiterate elderly were the MMSE and the PFAQ. The cut-off scores for the MMSE, VF, CDT, and PFAQ were 17.5, 7.5, 2.5, and 11.5, respectively. The most sensitive combination came from the MMSE and PFAQ (94.1%), and the best specificity was observed with the combination of the MMSE and CDT (89%). Illiterate patients can be successfully screened for AD using well-known screening instruments, especially in combined protocols.

Introduction: The aim of this study was to assess cyclic fatigue resistance in rotary nickel-titanium instruments submitted to nitrogen ion implantation by using a custom-made cyclic fatigue testing apparatus. Methods: Thirty K3 files, size #25, taper 0.04, were divided into 3 experimental groups as follows: group A, 12 files exposed to nitrogen ion implantation at a dose of 2.5 x 10(17) ions/cm(2), accelerating voltage of 200 kV, currents of 1 mu A/cm(2), 130 degrees C temperature, and vacuum conditions of 10 x 10(-6) torr for 6 hours; group B, 12 nonimplanted files; and group C, 6 files submitted to thermal annealing for 6 hours at 130 degrees C. One extra file was used for process control. All files were submitted to a cyclic fatigue test that was performed with an apparatus that allowed the instruments to rotate freely, simulating rotary instrumentation of a curved canal (40-degree, 5-mm radius curve). An electric motor handpiece was used with a contra-angle of 16:1 at an operating speed of 300 rpm and a torque of 2 N-cm. Time to failure was recorded with a stopwatch in seconds and subsequently converted to number of cycles to fracture. Data were analyzed with the Student t test (P < .05). Results: Ion-implanted instruments reached significantly higher cycle numbers before fracture (mean, 510 cycles) when compared with annealed (mean, 428 cycles) and nonimplanted files (mean, 381 cycles). Conclusions: Our results showed that nitrogen ion implantation improves cyclic fatigue resistance in rotary nickel-titanium instruments. Industrial implementation. of this surface modification technique would produce rotary nickel-titanium instruments with a longer working life. (J Endod 2010;36:1183-1186)

Introduction: The aim of this study was to assess the occurrence of apical root transportation after the use of Pro Taper Universal rotary files sizes 3 (F3) and 4 (F4). Methods: Instruments were worked to the apex of the original canal, always by the same operator. Digital subtraction radiography images were produced in buccolingual and mesiodistal projections. A total of 25 radiographs were taken from root canals of human maxillary first molars with curvatures varying from 23-31 degrees. Quantitative data were analyzed by intraclass correlation coefficient and Wilcoxon nonparametric test (P = .05). Results: Buccolingual images revealed a significantly higher degree of apical transportation associated with F4 instruments when compared with F3 instruments in relation to the original canal (Wilcoxon test, P = .007). No significant difference was observed in mesiodistal images (P = .492). Conclusions: F3 instruments should be used with care in curved canals, and F4 instruments should be avoided in apical third preparation of curved canals. (J Endod 2010;36:1052-1055)

Background: Newly formed biofilm after implant debridement may challenge the long-term stability of peri-implant therapy. This in vitro study aimed to assess the roughness and adherence of Streptococcus sanguinis after treatment of smooth and rough titanium surfaces with an erbium-doped: yttrium, aluminum, and garnet (Er:YAG) laser, metal and plastic curets, and an air-powder abrasive system. Methods: Forty titanium disks with smooth-machined surfaces and 40 with sand-blasted and acid-etched surfaces were divided into the following treatment groups: Er:YAG laser; plastic curet; metal curet, and air-powder abrasive system. The surface roughness (roughness average [Raj) before and after treatments was determined using a profilometer. S. sanguinis (American Type Culture Collection 10556) was grown on treated and untreated specimens, and the amounts of retained bacteria on the surfaces were measured by the culture method. Rough and smooth surfaces with and without a suspension of S. sanguinis were also analyzed using scanning electron microscopy (SEM). Results: For smooth surfaces, the roughest surfaces were produced by metal curets (repeated - measures analysis of variance [ANOVA] and Tukey test; P<0.05). The rough-surface profile was not altered by any of the treatments (repeated-measures ANOVA; P>0.05). Rough surfaces treated with metal curets and air-powder abrasion showed the lowest level of bacteria] adhesion (two-way ANOVA and Tukey test; P<0.05). SEM analysis revealed distinct surface profiles produced by all devices. Conclusions: Metal curets are not recommended for smooth titanium surface debridement due to severe texture alteration. Rough surfaces treated with a metal curet and the air-powder abrasive system were less susceptible to bacterial adhesion, probably due to texture modification and the presence of abrasive deposits. The aim of this study was to compare the concentration of mast cells (MCs) in the healing process of incisions. Thirty rats were submitted to six linear incisions each, performed in the dorsal skin by carbon dioxide (CO(2)) and diode lasers, electrocautery and conventional scalpel. The animals were euthanized at intervals of 0 h, 24 h, 48 h, 72 h, 7 days and 14 days after the incisions had been made. Histological sections were obtained and stained with toluidine blue for identification of MCs, which were manually counted by conventional microscopy in 20 microscopic fields in the border of the incision, near the granulation tissue, or in the area of new collagen formation, depending on intervals. The concentration of MCs was significantly higher in the wounds made by scalpel than in those made by other techniques at 48 h and 72 h. After 72 h the number of MCs was also significantly higher after electrocautery than after incisions made by 4 W CO(2) laser. On days 7 and 14, there was no significant difference in the MC count among the different types of incisions. In summary, the MC concentration varied after different surgical incisions at early phases of wound healing. At the end of the healing process, however, there were similar MC concentrations around the incisions, suggesting that, in standard incisions in the surgical techniques studied, the wound healing process ultimately occurred in a similar pattern.

The paper reports the findings of an experimental survey conducted to determine the public's willingness to pay (WTP) for the protection and conservation of the golden-shouldered parrot in Australia. This parrot is endemic to Australia and is one of Australia's most endangered birds. The paper examines the public's knowledge of this parrot and compares it with other endangered birds as well as common birds and the public's WTP for conservation from a hypothetical allocation of money based on their current knowledge. We then examine how this allocation changes with increased knowledge about all species.

Evaluate, through computerized tomography, canal transportation and centring ability of RaCe rotary instruments after preparation of mesiobuccal root canals in maxillary molar teeth. Twenty-seven teeth were submitted to three cone beam tomographic analyses, one preoperatively, and two after preparation with file size 35, .02 taper and size 50, .02 taper. Canal transportation and centring ability were measured with reference to the distance between the noninstrumented portion of the root canals and the mesial and distal periphery of the root, compared with images obtained after the preparation with size 35 and 50 instruments. Canal transportation after preparation with the size 35 file was 0.030 +/- 0.253 mm and after the size 50 file was 0.057 +/- 0.317 mm. The centring ratio values after preparation with the size 35 file was 0.42 +/- 0.32 and after the size 50 file was 0.54 +/- 0.29, with no significant statistical difference between the groups. RaCe instruments allowed the preparation of curved root canals with preparation diameters larger than those normally used with minimal canal transportation and adequate centring ability.

Riparian vegetation can be an effective measure for preventing degradation of streambanks and riparian areas. However, riparian revegetation imposes large costs on landholders associated with tree establishment and removal of land from cropping, while providing benefits to downstream landholders, fishers, the local community and environmentalists. Appropriate policy instruments are required to promote sustainable and balanced use of riparian zones. This article analyses the capacity of existing legislation and other instruments to promote restoration of degraded riparian zones on private land. The role of legislation. economic instruments, community engagement and extension programs, in persuading landholders to revegetate riparian areas and improve riparian vegetation cover; is examined in the context ofa small degraded catchment in an intensive farming area in tropical north Queensland. It is found that while legislation and regulations can control undesirable modification of riparian areas, in general they are unable to make a useful contribution to restoration of these areas; incentives and assistance measures appear to offer greater potential.

The introduction of new asset/income tested charges for high care residents was the 1997-98 Commonwealth government policy response to concerns about financing residential aged care. This in-depth study of residents, families, staff and managers in three aged care facilities explores issues of equity, access and empowerment arising when some residents pay more for the same level of care and amenity. The study reports little evidence of financial contributions affecting access to high care places and the delivery of care, the potential for differential access to amenities such as single rooms linked to the extra payments, and no evidence of a sense of empowerment linked to payment of the new charges. The complexity of current financial arrangements, access to appropriate financial advice at the time of entry, and the potential for an informal two tier system in relation to the allocation Of amenities are identified as developing policy issues.

The authors identify key issues that researchers, funding bodies, ethics committees and ethicists might consider in contemplating research subject payment ethics. They argue that what is missing from the broader debate is due consideration of ethics committee decision processes; research subject reasons for participation; and current research practices. The authors explore these issues and how they relate to existing guidelines on voluntary consent, and arguments that have been proposed for and against research subject payments. A rápida evolução dos dispositivos móveis e das tecnologias de comunicação sem fios transformou o telemóvel num poderoso dispositivo de computação móvel. A necessidade de estar sempre contactável, comum à civilização moderna, tem aumentado a dependência deste dispositivo, sendo transportado pela maioria das pessoas num ambiente urbano e assumindo um papel talvez mais importante que a própria carteira. A ubiquidade e capacidade de computação dos telemóveis aumentam o interesse no desenvolvimento de serviços móveis, além de tradicionais serviços de voz. Um telemóvel pode em breve tornar-se um elemento activo nas nossas tarefas diárias, servindo como um instrumento de pagamento e controlo de acessos, proporcionando assim novas interfaces para serviços existentes. A unificação de vários serviços num único dispositivo é um desafio que pode simplificar a nossa rotina diária e aumentar o conforto, no limite deixaremos de necessitar de dinheiro físico, cartões de crédito ou débito, chaves de residência e de veículos automóveis, ou inclusive documentos de identificação como bilhetes de identidade ou passaportes. O interesse demonstrado pelos intervenientes, desde os fabricantes de telemóveis e operadores de rede móvel até às instituições financeiras, levaram ao aparecimento de múltiplas soluções de serviços móveis. Porém estas soluções respondem geralmente a problemas específicos, apenas contemplando um fornecedor de serviços ou uma determinada operação de pagamento, como seja a compra de bilhetes ou pagamento de estacionamento. Estas soluções emergentes consistem também tipicamente em especificações fechadas e protocolos proprietários. A definição de uma arquitectura genérica, aberta interoperável e extensível é necessária para que os serviços móveis possam ser adoptados de uma forma generalizada por diferentes fornecedores de serviços e para diversos tipos de pagamento. A maior parte das soluções actuais de pagamento móvel depende de comunicações através da rede móvel, algumas utilizam o telemóvel apenas como uma interface de acesso à internet enquanto outras possibilitam o envio de um SMS (Short Message Service) para autorizar uma transacção, o que implica custos de comunicação em todas as operações de pagamento. Este custo de operação torna essas soluções inadequadas para a realização de micropagamentos e podem por isso ter uma aceitação limitada por parte dos clientes. As soluções existentes focam-se maioritariamente em pagamentos à distância, não tirando partido das características do pagamento presencial e não oferecendo por isso uma verdadeira alternativa ao modelo actual de pagamento com cartões de crédito/débito. As capacidades computacionais dos telemóveis e suporte de diversos protocolos de comunicação sem fio local não têm sido aproveitadas, vendo o telemóvel apenas como um terminal GSM (Global System for Mobile Communications) e não oferecendo serviços adicionais como seja a avaliação dinâmica de risco ou controlo de despesas. Esta dissertação propõe e valida, através de um demonstrador, uma aquitectua aberta para o pagamento e controlo de acesso baseado em dispositivos móveis, intitulada WPAC (Wireless Payment and Access Control). Para chegar à solução apresentada foram estudadas outras soluções de pagamento, desde o aparecimento dos cartões de débito até a era de pagamentos electrónicos móveis, passando pelas soluções de pagamento através da internet. As capacidades dos dispositivos móveis, designadamente os telemóveis, e tecnologias de comunicação sem fios foram também analisadas a fim de determinar o estado tecnológico actual. A arquitectura WPAC utiliza padrões de desenho utilizados pela indústria em soluções de sucesso, a utilização de padrões testados e a reutilização de soluções com provas dadas permite aumentar a confiança nesta solução, um destes exemplos é a utilização de uma infra-estrutura de chave pública para o estabelecimento de um canal de comunicação seguro. Esta especificação é uma arquitectura orientada aos serviços que utiliza os Web Services para a definição do contracto do serviço de pagamento. A viabilidade da solução na orquestração de um conjunto de tecnologias e a prova de conceito de novas abordagens é alcançada com a construção de um protótipo e a realização de testes. A arquitectura WPAC possibilita a realização de pagamentos móveis presenciais, isto é, junto do fornecedor de bens ou serviços, seguindo o modelo de pagamento com cartões de crédito/débito no que diz respeito aos intervenientes e relações entre eles. Esta especificação inclui como aspecto inovador a avaliação dinâmica de risco, que utiliza o valor do pagamento, a existência de pagamentos frequentes num período curto de tempo, e a data, hora e local do pagamento como factores de risco; solicitando ao cliente o conjunto de credenciais adequado ao risco avaliado, desde códigos pessoais a dados biométricos. É também apresentada uma alternativa ao processo normal de pagamento, que apesar de menos cómoda permite efectuar pagamentos quando não é possível estabelecer um canal de comunicação sem fios, aumentando assim a tolerância a falhas. Esta solução não implica custos de operação para o cliente na comunicação com o ponto de venda do comerciante, que é realizada através de tecnologias de comunicação local sem fios, pode ser necessária a comunicação através da rede móvel com o emissor do agente de pagamento para a actualização do agente de software ou de dados de segurança, mas essas transmissões são ocasionais. O modelo de segurança recorre a certificados para autenticação dos intervenientes e a uma infra-estrutura de chave pública para cifra e assinatura de mensagens. Os dados de segurança incluídos no agente de software móvel, para desabilitar a cópia ou corrupção da aplicação mas também para a comparação com as credenciais inseridas pelo cliente, devem igualmente ser encriptados e assinados de forma a garantir a sua confidencialidade e integridade. A arquitectura de pagamento utiliza o standard de Web Services, que é amplamente conhecido, aberto e interoperável, para definição do serviço de pagamento. Existem extensões à especificação de Web Services relativas à segurança que permitem trocar itens de segurança e definem o modo de cifra e assinatura de mensagens, possibilitando assim a sua utilização em aplicações que necessitem de segurança como é o caso de serviços de pagamento e controlo de acesso. O contracto de um Web Service define o modo de invocação dos serviços, transmissão de informação e representação de dados, sendo normalmente utilizado o protocolo SOAP que na prática não é mais que um protocolo de troca de mansagens XML (eXtensible Markup Language). O envio e recepção de mensagens XML; ou seja, a transmissão de simples sequências de caracteres, é suportado pela maioria dos protocolos de comunicação, sendo portanto uma solução abrangente que permite a adopção de diversas tecnologias de comunicação sem fios. O protótipo inclui um agente de software móvel, implementado sobre a forma de uma MIDlet, aplicação Java para dispositivos móveis, que implementa o protocolo de pagamento comunicando sobre uma ligação Bluetooth com o ponto de venda do comerciante, simulado por uma aplicação desenvolvida sobre a plataforma .NET e que por isso faz prova da heterogeneidade da solução. A comunicação entre o comerciante e o seu banco para autorização do pagamento e transferência monetária utiliza o protocolo existente para a autorização de pagamentos, com base em cartões de crédito/débito. A definição desta especificação aberta e genérica em conjunto com o forte interesse demonstrado pelos intervenientes, proporciona uma boa perspectiva em termos de adopção da solução, o que pode impulsionar a implementação de serviços móveis e dessa forma simplificar as rotinas diárias das pessoas. Soluções móveis de pagamento reduzem a necessidade de transportar vários cartões de crédito/débito na nossa carteira. A avaliação dinâmica de risco permite aumentar a segurança dos pagamentos, com a solicitação de mais credenciais ao cliente para pagamentos com um maior risco associado, sendo um ponto importante quer para os clientes quer para as instituições financeiras pois diminui o risco de fraude e aumenta a confiança no sistema. Esta solução de pagamento electrónico pode também facilitar a consulta de pagamentos efectuados e saldos, mantendo um histórico dos movimentos, o que não é possível nos cartões de crédito/débito sem uma visita a uma ATM (Automated Teller Machine) ou utilização de homebanking.

Purpose: To describe and compare the content of instruments that assess environmental factors using the International Classification of Functioning, Disability and Health (ICF). Methods: A systematic search of PubMed, CINAHL and PEDro databases was conducted using a pre-determined search strategy. The identified instruments were screened independently by two investigators, and meaningful concepts were linked to the most precise ICF category according to published linking rules. Results: Six instruments were included, containing 526 meaningful concepts. Instruments had between 20% and 98% of items linked to categories in Chapter 1. The highest percentage of items from one instrument linked to categories in Chapters 2–5 varied between 9% and 50%. The presence or absence of environmental factors in a specific context is assessed in 3 instruments, while the other 3 assess the intensity of the impact of environmental factors. Discussion: Instruments differ in their content, type of assessment, and have several items linked to the same ICF category. Most instruments primarily assess products and technology (Chapter 1), highlighting the need to deepen the discussion on the theory that supports the measurement of environmental factors. This discussion should be thorough and lead to the development of methodologies and new tools that capture the underlying concepts of the ICF.
