The main feature of the so called multiproblem families is the persistence along time of a set of problems in various areas of the individual’s functioning in several family members.This research study aims: a) To identify and characterise the major health problems faced by the members of these families; b) To explore the perceived relevance of these problems; c) To explore the perceived effectiveness of health care interventions received by respondents; d) To explore the level of control perceived over these problems.

PURPOSE AND METHOD: This questionnaire survey of 190 university music students assessed negative feelings of music performance anxiety (MPA) before performing, the experience of stage fright as a problem, and how closely they are associated with each other. The study further investigated whether the experience of stage fright as a problem and negative feelings of MPA predict the coping behavior of the music students. Rarely addressed coping issues were assessed, i.e., self-perceived effectiveness of different coping strategies, knowledge of possible risks and acceptance of substance-based coping strategies, and need for more support.RESULTS: The results show that one-third of the students experienced stage fright as a problem and that this was only moderately correlated with negative feelings of MPA. The experience of stage fright as a problem significantly predicted the frequency of use and the acceptance of medication as a coping strategy. Breathing exercises and self-control techniques were rated as effective as medication. Finally, students expressed a strong need to receive more support (65%) and more information (84%) concerning stage fright.CONCLUSION: Stage fright was experienced as a problem and perceived as having negative career consequences by a considerable percentage of the surveyed students. In addition to a desire for more help and support, the students expressed an openness and willingness to seriously discuss and address the topic of stage fright. This provides a necessary and promising basis for optimal career preparation and, hence, an opportunity to prevent occupational problems in professional musicians. [Authors]

Contexte : Les enfants atteints de maladies chroniques utilisent souvent des médecines complémentaires. Plusieurs études traitent de l’utilisation de ces traitements et des facteurs qui y sont associés chez les enfants atteints d’arthrite juvénile mais aucune étude n’est longitudinale. De plus, aucune n’a documenté l’utilisation de ces traitements chez les enfants ayant des incapacités physiques en attente de services publics de réadaptation. Objectifs : Les objectifs de cette étude étaient de déterminer la fréquence d’utilisation des médecines complémentaires chez les enfants atteints d’arthrite juvénile et d’incapacités physiques, d’évaluer leur efficacité telle que perçue par les parents et d’explorer les facteurs associés à leur utilisation. Méthodes : Une cohorte d’enfants atteints d’arthrite juvénile idiopathique (n=182, âge moyen : 10,2 ans) qui fréquentent des cliniques d’arthrite et une cohorte d’enfants ayant des incapacités physiques en attente de services de réadaptation publics (n=224, âge moyen : 2,6 ans) ont été suivis durant une période d’un an. L’utilisation des médecines complémentaires et la perception de leur efficacité d’après les parents ont été évaluées à l’aide de statistiques descriptives à chaque trois mois pour la cohorte d’enfants atteints d’arthrite et au début de l’étude pour la cohorte d’enfants ayant des incapacités physiques. Les facteurs associés à l’utilisation de ces traitements ont été explorés par des analyses de type GEE (« Generalized estimating equations ») et des régressions polytomique et logistique. Résultats : L’utilisation antérieure de ces médecines était de 51,1% pour les enfants atteints d’arthrite et de 15% pour les enfants ayant des incapacités physiques. Les médecines complémentaires étaient considérées comme étant efficaces dans 72% des cas par les parents d’enfants atteints d’arthrite et dans 83% des cas par les parents d’enfants ayant des incapacités physiques. Les facteurs associés à l’utilisation des médecines complémentaires chez les enfants atteints d’arthrite étaient l’utilisation antérieure des médecines complémentaires par les parents et la perception des parents que les médicaments prescrits ne sont pas utiles pour leur enfant. Chez les enfants ayant des incapacités physiques, les facteurs associés à l’utilisation des médecines complémentaires étaient l’origine culturelle canadienne, un niveau de scolarité plus élevé que le diplôme d’études secondaires et une moins bonne qualité de vie reliée à la santé. Finalement, l’utilisation des médecines complémentaires semblait associée à de moins bons résultats chez les enfants atteints d’arthrite. Conclusion: Une proportion non-négligeable des enfants participant à la présente étude ont utilisé des médecines complémentaires. Leur utilisation était plus fréquente chez les enfants atteints d’arthrite juvénile idiopathique, surtout chez ceux dont les parents avaient déjà utilisé les médecines complémentaires par le passé et chez ceux qui trouvaient la médication peu efficace. Chez les enfants ayant des incapacités physiques, l’utilisation des médecines complémentaires était associée à des facteurs socio-démographiques et à des besoins plus élevés en matière de santé. Les médecines complémentaires étaient considérées comme étant efficaces dans les deux cohortes mais leur utilisation était associée à de faibles résultats chez les enfants atteints d’arthrite. Ces résultats démontrent l’importance d’évaluer l’utilisation des médecines complémentaires afin de mieux renseigner les parents et de les aider à prendre les meilleures décisions possibles concernant le traitement de leur enfant.

Este estudio tiene como objetivo identificar cuáles son las variables que repercuten en la efectividad de las redes empresariales. Esto, con base en la búsqueda de literatura existente de la efectividad en equipos, en organizaciones y en las redes interorganizacionales, así como el análisis de modelos y estudios empíricos que permitieron el análisis. De acuerdo con la búsqueda, se encontró que variables como la estructura de la red, la estabilidad del sistema, el compromiso de los empleados en cada una de las organizaciones que hacen parte de la red, la confianza dentro de la red, la transferencia de conocimiento y la apertura del sistema son las variables que en conclusión, mostraron ser buenas predictoras de efectividad dentro de las redes empresariales.

As políticas públicas de saúde surgem em decorrência do reconhecimento de um cenário desumano e incoerente frente às condições sociais e de saúde da população, este cenário favoreceu a inclusão da família como foco de atenção nas políticas públicas. Neste contexto o profissional enfermeiro vem se destacando como agente dinamizador das ações dentro do Programa Saúde da Família(PSF). A partir desta premissa este estudo objetivou conhecer as atividades gerenciais desenvolvidas pelo enfermeiro gestor no Programa Saúde da Família, a percepção do enfermeiro gestor sobre a efetivação das metas á serem alcançadas pelo Programa Saúde da Família, e as dificuldades encontradas na prática cotidiana deste profissional para efetivação das metas a serem alcançadas no Programa Saúde da Família. Trata-se de um estudo descritivo com abordagem qualitativa, realizado em Belém do Pará. Os dados foram obtidos por meio de entrevistas semi-estruturadas junto a 45 enfermeiros gestores de 30 unidades saúde da família na região metropolitana de Belém. As informações coletadas foram organizadas conforme preceitos à análise de conteúdo segundo Bardin (1977). Da análise dos discursos emergiram cinco categorias. Os resultados deste estudo demonstraram que são inúmeras as atividades gerenciais do enfermeiro, que sua maior dificuldade na prática cotidiana deve-se ao fato da comunidade ainda estar fortemente arraigada ao atendimento hospitalocêntrico, além da insatisfação na função de gerência, ligadas as várias dificuldades como falta de perfil do agente comunitário de saúde, infraestrutura das unidades, falta do profissional médico em algumas unidades, não adesão do tratamento e ações educativas pelas famílias adstritas , além da impossibilidade em alcançar os objetivos propostos pelo Programa Saúde da Família, de acordo a maioria dos informantes. Essa realidade tem sido vivenciada pelo enfermeiro gestor, caracterizada como forma de tensão interna do sistema, gerando a construção peculiar para enfrentar tais dificuldades. Apesar das dificuldades e limitações dos enfermeiros gestores, estes profissionais realizam suas funções com responsabilidade, buscando cada vez mais autonomia, e consideram que este programa, é o marco nas políticas públicas de saúde.

This study compares relative and absolute forms of presenting risk information about influenza and the need for vaccination. It investigates whether differences in people's risk estimates and their evaluations of risk information, as a result of the different presentation formats, are still apparent when they are provided with information about the baseline level of risk. The results showed that, in the absence of baseline information, the relative risk format resulted in higher ratings of satisfaction, perceived effectiveness of vaccination, and likelihood of being vaccinated. However, these differences were not apparent when baseline information was presented. Overall, provision of baseline information resulted in more accurate risk estimates and more positive evaluations of the risk messages. It is recommended that, in order to facilitate shared and fully informed decision making, information about baseline level of risk should be included in all health communications specifying risk reductions, irrespective of the particular format adopted.

Objectives. To examine beliefs about medication risks and benefits in patients attending a specialist rheumatology clinic for pain-related conditions. Methods. Eighty-one patients (37 first attendees and 44 existing clinic patients) completed a written questionnaire which asked about current treatments, perceived effectiveness, main risks and benefits, and compliance. Results. Existing clinic patients perceived medications to be more effective and more risky than did the new patients, although both groups rated risks to be moderately low. The main perceived risks were adverse side-effects, although patients reported only moderately low levels of experiencing such effects. Conclusions. In contrast to some other studies, many of our patients were aware of medication risks and were prepared to accept them provided benefits were seen to be high. Existing clinic patients were more aware of risks and benefits, and reported higher compliance levels than new patients, possibly as a result of the hospital education programme. Future studies should evaluate the effects of the programme more systematically.

Por muito tempo, os programas de qualidade e gestão vêm dominando o cenário de soluções para a melhoria das organizações. Nesse contexto, têm sido constantes os modismos e, ao mesmo tempo, o consumo de muitas soluções. E o mercado parece se nutrir desses pacotes. De pacote em pacote, a gestão nas companhias vem se desenvolvendo e se especializando e, junto como isso, os métodos e técnicas da boa gestão. A gestão de riscos, especificamente, vem nesse contexto. Apresenta-se como uma solução em gestão, mas parece se estabelecer como ponto de convergência e de influência na otimização e garantia de processos produtivos, de gestão e de suporte, abrangendo uma gama de possibilidades em diversas disciplinas do mundo empresarial, desde finanças até os aspectos de fraudes e distúrbios cotidianos. Na sequência da gestão de riscos, vem a gestão de crises. Esta, o hemisfério dos riscos transformados em impactos verdadeiros e com danos visíveis para a organização. A gestão de crises pressupõe a gestão dos riscos consumados e que, claramente, afetam a organização e seu contexto, tanto interno quanto externo. No ponto final dessa lógica, aparece a confiança como aquilo que sela o pacto da organização e seus stakeholders, como resultado da boa ou má gestão de riscos e crises. Este estudo é, então, sobre riscos, crises e confiança e sobre o entendimento de como a gestão sobrepõe esses elementos e como isso se aplica às organizações, especificamente a uma grande organização do mercado brasileiro. Após revisão bibliográfica, é feita uma pesquisa para se analisar como está sendo a aplicação dos conceitos e práticas de riscos e crises em uma grande empresa, entrevistando-se o principal executivo da companhia responsável pela gestão de riscos na área de segurança da informação e outro responsável pela gestão de crises. É feita uma pesquisa para se entender a percepção de empregados e gerentes da companhia estudada sobre os conceitos e práticas de gestão de riscos e crises e suas aplicações na companhia. Também é feita uma abordagem sobre confiança, extrapolando-se esse conceito para uma idéia de confiabilidade dessas práticas e propondo uma forma de medir essa confiabilidade, identificada como uma lacuna na gestão de crises. Ao final, é feita uma análise sobre o quanto a aplicação desses conceitos e práticas são sistemáticos ou não, de acordo com as hipóteses e suposições definidas, constatando-se o 9 caráter operacional e a aplicação recente das práticas, um tanto deslocada do modelo de referência proposto para o estudo e com pouca visibilidade por parte dos empregados, principalmente no que tange a percepção de efetividade das práticas adotadas.

Este estudo investiga qual a contribuição do Sistema Integrado de Administração Financeira do Governo Federal (SIAFI) na promoção da accountability da Administração Pública Federal no Brasil, com base na pesquisa documental e na percepção dos usuários do sistema. O modelo de análise da accountability ocorre a partir da investigação das suas definições e dimensões, segundo levantamento do referencial teórico abrangendo o campo da administração pública e da previsão na legislação brasileira. São definidas três dimensões para descrição da accountability: transparência, prestação de contas e responsabilização. Na condução da metodologia, inicialmente essas dimensões foram tratadas pela pesquisa documental, por meio da investigação no SIAFI – Manual Web da existência de mecanismos de accountability, com a aplicação de técnicas voltadas a levantar as expressões e instrumentos do sistema que possibilitam a promoção da accountability. O resultado dessa análise evidenciou funcionalidades, que foram posteriormente escritas na forma de questões de avaliação, segundo escala Likert de 4 pontos, a fim de identificar a eficácia percebida por quatro grupos de usuários do sistema: gestor, operador, agente de controle e agente de investigação. O questionário foi aplicado via internet aos usuários de 128 órgãos do Governo Federal, tendo retornado 287 formulários respondidos. O tratamento e análise dos resultados permitiram observar que o sistema é percebido como eficaz na promoção da accountability, apesar da necessita de melhorias e de treinamento de seus usuários. Além disso, foram identificadas divergências estatisticamente significativas nas respostas dos perfis de usuários, apontando uma não uniformidade nessa percepção segundo a origem do usuário e finalidade de sua relação com o sistema.

This study has as its object the municipality of Altinópolis (SP) and its potential for the practice of tourism and the combination of its segments, but it is not perceived effectiveness of the various ideas proposed in other studies. Since then, some work done in relation to this subject were analyzed and investigated the sectors involved, and so we sought to clarify the reason of not taking advantage of this potential for achieving economic, social, cultural and environmental development and consequently place in the city. It might be noted that the main weak points among the various apparent is the absence of significant investments of public power in this area and in all sectors engaging in this practice, or across the city

Within the framework of the Swiss governmental Program of Evaluation of Complementary Medicine (PEK) we assessed the prevalence, use, perceived effectiveness and appreciation of complementary medicine (CAM) in Switzerland, according to published surveys. Materials and Methods: Search was performed through electronic databases, by hand-searching and by contacting experts at universities, hospitals, health insurances, patient organizations and pharmaceutical companies. Results: Surveys were carried out among the general population (40%), physicians (20%), hospitalized patients (30%) and obstetric institutions (5%). The number of publications increased strongly between 1981 and 2004. The mean +/- SD prevalence (use) of CAM is 49 +/- 22% and varies depending on the survey's topic and the population group interviewed. The acceptance, appreciation or demand for CAM among individuals specifically interviewed on CAM is 91 +/- 6%. When asked about favored general improvements in healthcare, 6.5% of the individuals spontaneously mentioned CAM. CAM therapies are considered to be effective by the majority of CAM users and by about 40% of cancer patients using CAM. Approximately 50% of the population stated a preference for hospitals that also provide CAM. 85% of the population wishes the costs for CAM to be covered by the basic health insurance. Conclusion: Approximately half of the Swiss population has used CAM. CAM treatment is considered to be effective by the majority of CAM users. About 50% of the population would prefer hospitals that also provide CAM therapies and the majority of the population wishes the cost for CAM therapies to be covered by basic health insurance.

Background Though complementary and alternative medicine (CAM) are frequently used by children and adolescents with cancer, there is little information on how and why they use it. This study examined prevalence and methods of CAM, the therapists who applied it, reasons for and against using CAM and its perceived effectiveness. Parent-perceived communication was also evaluated. Parents were asked if medical staff provided information on CAM to patients, if parents reported use of CAM to physicians, and what attitude they thought physicians had toward CAM. Study Design All childhood cancer patients treated at the University Children’s Hospital Bern between 2002–2011 were retrospectively surveyed about their use of CAM. Results Data was collected from 133 patients (response rate: 52%). Of those, 53% had used CAM (mostly classical homeopathy) and 25% of patients received information about CAM from medical staff. Those diagnosed more recently were more likely to be informed about CAM options. The most frequent reason for choosing CAM was that parents thought it wouldimprove the patient’s general condition. The most frequent reason for not using CAM was lack of information. Of those who used CAM, 87% perceived positive effects. Conclusions Since many pediatric oncology patients use CAM, patients’ needs should be addressed by open communication between families, treating oncologists and CAM therapists, which will allow parents to make informed and safe choices about using CAM.

The European Union and Singapore are vastly different entities, each with its own regional and global priorities and policies. Both actors employ a range of tools and instruments to aid in their foreign policy objectives, including in the projection of their soft power. It is worth analysing and comparing the specific instruments of these two actors’ soft power strategies, including but not limited to their stated objectives and perceived effectiveness. This paper will compare the role of higher education and scholarships in diffusing soft power through a comparative case study of the Erasmus Mundus scholarship program and the Singapore Scholarship administered by the Ministry of Foreign Affairs. It will look at the ways in which these programs have shaped the standing of the actors in diffusing their norms and objectives in the regional and international arena. A comparative analysis of these programs will hopefully provide some insight into the proximity between foreign policy-making and higher education internationalisation. This paper will begin with an overview of the aforementioned programs and related schemes, before dissecting and comparing the intent and the policy-making processes behind these, and concludes with a discussion on the present and future role of higher education as a strategic soft power tool.

Wildlife feeding is a wide-spread and controversial practice that can pose serious threats to the safety of both wildlife and visitors. The design and effectiveness of warning signs in recreational areas varies considerably and is rarely the product of theoretical models or scientific research. This study uses front-end and formative evaluation to design and test the perceived effectiveness of warning signs relating to bird feeding. Stage One examined visitors' beliefs, attitudes and bird feeding behaviour and found significant differences between feeders and non-feeders. Stage Two involved designing and evaluating three signs that built on the beliefs, knowledge and mis/conceptions identified in Stage One. Respondents thought the sign that focused on the birds' health and safety would be the most persuasive, however, elements of the other two signs were also positively evaluated. During the last 15 years corporate governance has become increasingly prominent in the public sector. The Audit Commission's 1993 report on probity in local government recommended the establishment of audit committees. However, progress on this was slow, as demonstrated by a survey of Scottish local authorities by the authors in 1998. Recent major changes in government in Scotland at both a local and national level have prompted councils to improve the accountability, openness and integrity of their operations. One major aspect of this exercise was the formation of scrutiny committees to provide objective scrutiny of the process and audit committees were the most common vehicle for this. This article explores recent developments in Scottish local government and their impact on audit committees. The article also reports the results of a 2005 survey of Scottish local authorities and compares these with the 1998 survey. This indicates a significant growth in the number of audit committees in Scottish councils and although the level of their perceived effectiveness is patchy, they are a more important feature of local government than they were in 1998.
