BACKGROUND: Exclusive liver metastases occur in up to 40% of patients with uveal melanoma associated with a median survival of 2-7 months. Single agent response rates with commonly available chemotherapy are below 10%. We have investigated the use of fotemustine via direct intra-arterial hepatic (i.a.h.) administration in patients with uveal melanoma metastases. PATIENTS AND METHODS: A total of 101 patients from seven centers were treated with i.a.h. fotemustine, administered intra-arterially weekly for a 4-week induction period, and then as a maintenance treatment every 3 weeks until disease progression, unacceptable toxicity or patient refusal. RESULTS: A median of eight fotemustine infusions per patient were delivered (range 1-26). Catheter related complications occurred in 23% of patients; however, this required treatment discontinuation in only 10% of the patients. The overall response rate was 36% with a median overall survival of 15 months and a 2-year survival rate of 29%. LDH, time between diagnosis and treatment start and gender were significant predictors of survival. CONCLUSIONS: Locoregional treatment with fotemustine is well tolerated and seems to improve outcome of this poor prognosis patient population. Median survival rates are among the longest reported and one-third of the patients are still alive at 2 years.

Peroxisome proliferator-activated receptor gamma (PPAR-gamma) plays a key role in adipocyte differentiation and insulin sensitivity. Its synthetic ligands, the thiazolidinediones (TZD), are used as insulin sensitizers in the treatment of type 2 diabetes. These compounds induce both adipocyte differentiation in cell culture models and promote weight gain in rodents and humans. Here, we report on the identification of a new synthetic PPARgamma antagonist, the phosphonophosphate SR-202, which inhibits both TZD-stimulated recruitment of the coactivator steroid receptor coactivator-1 and TZD-induced transcriptional activity of the receptor. In cell culture, SR-202 efficiently antagonizes hormone- and TZD-induced adipocyte differentiation. In vivo, decreasing PPARgamma activity, either by treatment with SR-202 or by invalidation of one allele of the PPARgamma gene, leads to a reduction of both high fat diet-induced adipocyte hypertrophy and insulin resistance. These effects are accompanied by a smaller size of the adipocytes and a reduction of TNFalpha and leptin secretion. Treatment with SR-202 also dramatically improves insulin sensitivity in the diabetic ob/ob mice. Thus, although we cannot exclude that its actions involve additional signaling mechanisms, SR-202 represents a new selective PPARgamma antagonist that is effective both in vitro and in vivo. Because it yields both antiobesity and antidiabetic effects, SR-202 may be a lead for new compounds to be used in the treatment of obesity and type 2 diabetes.

PURPOSE: Effective cancer treatment generally requires combination therapy. The combination of external beam therapy (XRT) with radiopharmaceutical therapy (RPT) requires accurate three-dimensional dose calculations to avoid toxicity and evaluate efficacy. We have developed and tested a treatment planning method, using the patient-specific three-dimensional dosimetry package 3D-RD, for sequentially combined RPT/XRT therapy designed to limit toxicity to organs at risk. METHODS AND MATERIALS: The biologic effective dose (BED) was used to translate voxelized RPT absorbed dose (D(RPT)) values into a normalized total dose (or equivalent 2-Gy-fraction XRT absorbed dose), NTD(RPT) map. The BED was calculated numerically using an algorithmic approach, which enabled a more accurate calculation of BED and NTD(RPT). A treatment plan from the combined Samarium-153 and external beam was designed that would deliver a tumoricidal dose while delivering no more than 50 Gy of NTD(sum) to the spinal cord of a patient with a paraspinal tumor. RESULTS: The average voxel NTD(RPT) to tumor from RPT was 22.6 Gy (range, 1-85 Gy); the maximum spinal cord voxel NTD(RPT) from RPT was 6.8 Gy. The combined therapy NTD(sum) to tumor was 71.5 Gy (range, 40-135 Gy) for a maximum voxel spinal cord NTD(sum) equal to the maximum tolerated dose of 50 Gy. CONCLUSIONS: A method that enables real-time treatment planning of combined RPT-XRT has been developed. By implementing a more generalized conversion between the dose values from the two modalities and an activity-based treatment of partial volume effects, the reliability of combination therapy treatment planning has been expanded.

Työssä mitattiin työilman TBEP- ja TEHP-pitoisuuksia siivousalalla (3 kohdetta) ja muovituoteteollisuudessa (2 kohdetta). Pitoisuutta ilmassa verrattiin työntekijöidenbiomonitorointinäytteistä analysoitujen 2-butoksietikkahapon (2-BAA) ja 2-etyyliheksaanihapon (2-EHA) pitoisuuteen. Tavoitteena oli biomonitoroin-timenetelmän soveltuvuuden selvittäminen altistumisen arvioinnissa sekä tarvittavien lisäsuojaus- ja torjunta-tarpeiden selvittäminen. Biomonitorointia sekoittavien tekijöiden (2-butoksietanoli ja 2-etyyli-1-heksanoli) pitoisuus mitattiin työntekijöidenhengitysilmasta kerätystä VOC-näytteestä. OVS-keräimiin kerätyt TBEP- ja TEHP-näytteet uutettiin ultraäänellä sopivalla liuottimella ja analysoitiin kaasukromatografisesti (GC). VOC-näytteet kerättiin Tenax GR ¿adsorbenttiin, irrotettiin keräimestä termodesorptiolla ja analysoitiin GC:lla. Biomonitorointinäytteiden (virtsa) 2-BAA- ja 2-EHA -pitoisuus analysoitiin myös GC:lla. Lattianvahauksen aikana siivoojien hengitysilmasta mitatuissa TBEP-näytteissä pitoisuudet vaihtelivat välillä 70 - 860 ng/m3 sekä VOC-näytteistä 88 %:sta löytyi2-butoksietanolia, jonka pitoisuus vaihteli välillä 3 - 1800 µg/m3. Virtsan 2-BAA:n pitoisuudet vaihtelivat välillä <0,3 - 26 mmol/mol kreatiniinia ollen korkeimmillaan 43 % toimenpiderajasta. Raskaana olevien toimenpideraja ylitettiin 30 % näytteistä. Muovituotetehtaissa TEHP-pitoisuus työntekijöiden hengitysilmassa oli alle määritysrajan (eli < 1 - < 3 ng/m3). Muovituotetehtaissa kerätyistä VOC-näytteistä 16 %:sta löydettiin 2-etyyli-1-heksanolia, jonka pitoisuus vaihteli välillä 90 - 100 µg/m3. Virtsan 2-EHA-pitoisuudet olivat välillä <0,1 - 0,2 mmol/mol kreatiniinia. Tulosten perusteella siivoojat altistuvat TBEP:lle ja 2-butoksietanolille lattianvahauksen yhteydessä, muttaaltistuminen ei ilman pitoisuuksien ja biomonitoroinnin tulosten mukaan aiheutaterveyshaittaa ainakaan yhden mahdollisen metaboliitin 2-BAA:n kautta. Pois lukien raskaana olevat työntekijät, joille 2-butoksietanolia sisältävän vahanpoistotuotteen käsitteleminen aiheuttaa mahdollista terveyshaittaa. Käytetyillä biomonitorointimenetelmillä ei voitu osoittaa altistumista tapahtuneen mitatuilla TBEP:n ja TEHP:n pitoi-suustasoilla. Metaboliareittien varmistamisen jälkeen on mahdollista tutkia toimivampaa menetelmää altistumisen arviointiin biomonitoroinnin avulla. Työntekijöiden suojautuminen niin muovituotetehtaissa kuin siivoustyössäkin mitattujen yhdisteiden osalta oli pääosin riittävää. Ainoastaan raskaana olevien työntekijöiden, jotka siivoustyössään altistuvat 2-butoksietanolille, suojaukseen tulisi kiinnittää huomiota.

This paper is a review of the history, synthesis and application of organophosphorus compounds, especially of those of pentavalent phosphorus, such as phosphoramidates, phosphorothioates, phosphonates and phosphonic acids with insecticide and anticancer activities. The organophosphorus compounds with agrochemical applications show great structural variety, They include not only insecticides, but also fungicides, herbicides, and others. The large variety of commercially available organophosphorus pesticides is remarkable. Even more interesting is the high efficiency of some organophosphorus compounds as anticancer agents such as cyclophosphamide and its derivatives.

This study aims to reveal the ability of bacteria isolated from Cochin estuary and the Arabian Sea to produce phosphatases, important characters of the enzymes, its ability to utilize organophosphorus compounds as source of phosphate and also their role in degradation of organophosphorus pesticides. It deals with isolation, identification and screening of bacteria for phosphatase production, and it describes the effect of cultural conditions on growth and phosphatase production. The effect of various factors like pH, NaCl concentration, temperature of incubation, carbon source, period of incubation etc. on growth and phosphatase production by the two selected species were studied to establish suitable environment for phosphatase production by these bacteria. In this study regulation of phosphatase synthesis, characteristics of acid and alkaline phosphatases are discussed.

Man uses a variety of synthetic material for his comfortable materialistic life. Thus human interactions may become harmful for various terrestrial and aquatic lives. This is by contaminating their habitat and by becoming a threat to organisms itself. Thus the application and dispersal of several organic pollutants can lead to the development of several mutated forms of the species when exposed to sublethal concentrations of the pollutants. Otherwise, a decrease in number or extinction of these exposed species from earth's face may happen. Pesticides, we use for the benefit of crop yield, but its persistence may become havoc to non-target organism. Pesticides reaching a reservoir can subsequently enter the higher trophic levels. Organophosphorus compounds have replaced all other pesticides, due to its acute toxicity and non-persistent nature.Hence the present study has concentrated on the toxicity of the largest market-selling and multipurpose pesticide, chlorpyrifos on the commonly edible aquatic organism, fish. The euryhaline cichlid Oreochromis mossambicus was selected as animal model. The study has concentrated on investigating biochemical parameters like tissue-specific enzymes, antioxidant and lipid-peroxidation parameters, haematological and histological observations and pesticide residue analysis.Major findings of this work have indicated the possibility of aquatic toxicity to the fish on exposure to the insecticide chlorpyrifos. The insecticide was found as effective to induce structural alteration, depletion in protein content, decrease in different metabolic enzyme levels and to progress lipid peroxidation on a prolonged exposure of 21 days. The ion-transport mechanism was found to be adversely affected. Electrophoretic analysis revealed the disappearance of several protein bands after 21days of exposure to chlorpyrifos. Residue, analysis by gas chromatography explored the levels of chlorpyrifos retaining on the edible tissue portions during exposure period of 21days and also on a recovery period of 10 days.

Two simple methods were developed to determine, 11 pesticides in coconut water, a natural isotonic drink rich in salts, sugars and vitamins consumed by the people and athletes. The first procedure involves solid-phase extraction using Sep-Pak Vac C-18 disposable cartridges with methanol for elution. Isocratic analysis was carried out by means of high-performance liquid chromatography with ultraviolet detection at 254 nm to analyse captan, chlorothalonil, carbendazim, lufenuron and diafenthiuron. The other procedure is based on liquid-liquid extraction with hexane-dichloromethane (1:1, v/v), followed by gas chromatographic analysis with effluent splitting to electron-capture detection for determination of endosulfan, captan, tetradifon and trichlorfon and thermionic specific detection for determination of malathion, parathion-methyl and monocrotophos. The methods were validated with fortified samples at different concentration levels (0.01-12.0 mg/kg). Average recoveries ranged from 75 to 104% with relative standard deviations between 1.4 and 11.5%. Each recovery analysis was repeated at least five times. Limits of detection ranged from 0.002 to 2.0 mg/kg. The analytical procedures were applied to 15 samples and no detectable amounts of the pesticides were found in any samples under the conditions described. (C) 2002 Elsevier B.V. B.V. The effects of organophosphorus compounds which form a rapidly-ageing complex with acetylcholinesterase (AChE) (e.g. pinacolyl S-(2- trimethylaminoethyl)methylphosphonothioate (BOS)) and hence exert a persistent anticholinesterase (anti-ChE) action have been compared with other compounds with a shorter time course of inhibition (e.g. ecothiopate iodide (ECO)). Although the inhibition of AChE produced by BOS lasted longer than that seen with ECO, the time course of the myopathy appeared very similar. BOS also possessed a number of properties which have been seen with other anti-ChEs. BOS and ECO produced significant increases in neuromuscular "jitter" 5 days after injection, not only in the diaphragm but also in the soleus and extensor digitorum longus muscles. Increases in "jitter" produced by ECO could be prevented by pyridostigmine prophylaxis or rapid treatment with pyridine-2- aldoxime methiodide. Some protection from the BOS-induced increases in "jitter" could be gained by repeated treatment with pyridine-2-aldoxime methiodide, an effect which could not be accounted for simply by enzyme reactivation. From experiments performed in Rej 129 mice it was determined that increases in "jitter", although demonstrated in some dystrophic muscles, could not be used as an early diagnostic tool. Because sequalae of inhibition were present some time after intoxication, by which time AChE appeared biochemically normal, experiments were performed to investigate inactivation of physiologically important AChE. The time course of extracellular MEPPs was utilised as an indicator of physiologically important AChE and compared with the AChE activity measured by the technique of Ellman et al. (1961). It was concluded that the degree of persistence of anti-ChE action was unimportant for the induction of myopathy with a time course of 3-24 hours, but had some importance in events of longer duration.

Poisoning by pesticides from agricultural fields is a serious water pollution problem and its environmental long-term effect may result in the incidence of poisoning of fish and other aquatic life forms (jyothi and Narayan, 1999). Fishes like Heteropneustesfbssilis and C/arius batrac/nus are especially prone to serious pesticide pollution as their habitat is mostly the agriculture area. Though only few studies are conducted in this area, it can be assessed from the local information that, population of such fish is on the verge of vulnerability due to extensive use of pesticides. The knowledge of sublethal effects of xenobiotic compounds on hematological parameters, enzyme activities and metabolite concentrations is very important to delineate the fish health status and provide a future understanding of ecological impacts. These pesticides act by causing inhibition of cholinesterase enzymes (ChE) by formation of enzyme inhibitor complex (O'Brien, 1976) and damaging the nervous system. These effects may result in metabolic disorders. Associated to cholinesterase activities, a study of other enzymes such as phosphatases and aminotransferases close to intermediary metabolite determination provides a wider view of metabolism. Interest in toxicological aspects has grown in recent years and research is now increasingly focused on mechanistic aspects of oxidative damage and cellular responses in biological system. The term ‘biomarker’ is generally used in a broad sense to include almost any measurement reflecting an interaction between a biological system and a potential hazard, which may be chemical, physical or biological (WHO, 1993). As biomarker stands for immediate responses, they are used as early warning signals of biological effects caused by environmental pollutants. The present work attempts to assess the toxicity of organophosphorus insecticide monocrotophos on the experimental organism selected for this study namely stinging catfish (Heteropneustesfossi/is) (Bloch), and to probe into the stress responses of the organism
