A Comissão Europeia irá lançar, num futuro próximo, uma nova directiva relativa à protecção radiológica - Basic Safety Standards – a qual irá substituir, num único documento, os actualmente existentes. Durante as últimas três décadas, em Portugal, a protecção radiológica contra radiações ionizantes tem sido regulada segundo cinco directivas, publicadas entre 1989 e 2003. Têm sido relatados vários incidentes ocorridos em ambiente clínico, nomeadamente naqueles que operam com radiação ionizante, pois na maioria dos casos, as regras, boas práticas e infra-estruturas não estão adequadas para esta finalidade. As questões ligadas com esta temática estão agora em análise e sob discussão pública. É, contudo, importante entender os desafios e requisitos da nova directiva para melhorar e assegurar a excelência dos resultados nas organizações de cuidados de saúde. Este novo documento reúne as abordagens da International Commission on Radiation Protection (ICRP), International Atomic Energy Agency (IAEA), Nuclear Energy Agency (NEA), International Radiation Protection Association (IRPA) e World Health Organization (WHO) e também de outros especialistas na matéria. O propósito da nova directiva aplicada a qualquer exposição previamente planeada baseia-se no princípio ALARA (optimização das práticas, justificação da exposição e limitação da dose). O objectivo deste estudo é, na opinião das partes interessadas, identificar as oportunidades que podem resultar da implementação da nova directiva.

This review aims to identify strategies to optimise radiography practice using digital technologies, for full spine studies on paediatrics focusing particularly on methods used to diagnose and measure severity of spinal curvatures. The literature search was performed on different databases (PubMed, Google Scholar and ScienceDirect) and relevant websites (e.g., American College of Radiology and International Commission on Radiological Protection) to identify guidelines and recent studies focused on dose optimisation in paediatrics using digital technologies. Plain radiography was identified as the most accurate method. The American College of Radiology (ACR) and European Commission (EC) provided two guidelines that were identified as the most relevant to the subject. The ACR guidelines were updated in 2014; however these guidelines do not provide detailed guidance on technical exposure parameters. The EC guidelines are more complete but are dedicated to screen film systems. Other studies provided reviews on the several exposure parameters that should be included for optimisation, such as tube current, tube voltage and source-to-image distance; however, only explored few of these parameters and not all of them together. One publication explored all parameters together but this was for adults only. Due to lack of literature on exposure parameters for paediatrics, more research is required to guide and harmonise practice.

This paper reviews the literature for lowering of dose to paediatric patients through use of exposure factors and additional filtration. Dose reference levels set by The International Commission on Radiological Protection (ICRP) will be considered. Guidance was put in place in 1996 requires updating to come into line with modern imaging equipment. There is a wide range of literature that specifies that grids should not be used on paediatric patients. Although much of the literature advocates additional filtration, contrasting views on the relative benefits of using aluminium or copper filtration, and their effects on dose reduction and image quality can vary. Changing kVp and mAs has an effect on the dose to the patient and image quality. Collimation protects adjacent structures whilst reducing scattered radiation.

Background: Post-surgical management of stage I seminoma includes: surveillance with repeated CT-scans and treatment reserved for those who relapse, or adjuvant treatment with either immediate radiation therapy (RT) or carboplatin. The cancer specific survival is close to 100%. Cure without long-term sequelae of treatment is the aim. Our goal is to estimate the risk of radiation-induced secondary cancers (SC) death from for patients undergoing S, adjuvant RT or adjuvant carboplatin (AC).Materials and Methods: We measured organ doses from CT scans (3 phases each one) of a seminoma patient who was part of the active surveillance strategy and from a man undergoing adjuvant RT 20-Gy and a 30-Gy salvage RT treatment to the para-aortic area using helical Intensity Modulated RT (Tomotherapy®) with accurate delineation of organs at risk and a CTV to PTV expansion of 1 cm. Effective doses to organs in mSv were estimated according to the tissue-weighting factors recommendations of the International Commission on Radiological Protection 103 (Ann ICRP 2007). We estimated SC incidence and mortality for a 10,000 people population based on the excess absolute risk model from the Biological Effects of Ionizing Radiation (BEIR) VII (Health Risk of Exposure to Low Levels of Ionizing Radiation, NCR, The National Academies Press Washington, DC, 2006) assuming a seminoma diagnosis at age 30, a total life expectancy of 80 years.Results: The nominal risk for a fatal secondary cancers was calculated 1.5% for 15 abdominal CT scans, 14.8% for adjuvant RT (20 Gy paraaortic field) and 22.2% for salvage RT (30 Gy). The calculation assumed that the risk of relapse on surveillance and adjuvant AC was 15% and 4% respectively and that all patients were salvaged at relapse with RT. n CT abdomen/Pelvis = secondary cancer % RT Dose and % receiving treatment = secondary cancer % Total secondary cancer risk in % Active surveillance 15 = 1.5% 30 Gy in 15% of pts = 3.3% 4.8 Adjuvant carboplatin 7 = 0.7% 30 Gy in 4% of pts = 0.88% 1.58 Adjuvant radiotherapy 7 = 0.7% 20 Gy in 100% of pts = 14.8% 15.5Conclusions: These data suggest that: 1) Adjuvant radiotherapy is harmful and should not anymore be regarded as a standard option for seminoma stage I. 2) AC seems to be an option to reduce radiation induced cancers. Limitations: the study does not consider secondary cancers due to chemotherapy with AC (unknown). The use of BEIR VII for risk modeling with higher doses of RT needs to be validated.

The comparison of radiotherapy techniques regarding secondary cancer risk has yielded contradictory results possibly stemming from the many different approaches used to estimate risk. The purpose of this study was to make a comprehensive evaluation of different available risk models applied to detailed whole-body dose distributions computed by Monte Carlo for various breast radiotherapy techniques including conventional open tangents, 3D conformal wedged tangents and hybrid intensity modulated radiation therapy (IMRT). First, organ-specific linear risk models developed by the International Commission on Radiological Protection (ICRP) and the Biological Effects of Ionizing Radiation (BEIR) VII committee were applied to mean doses for remote organs only and all solid organs. Then, different general non-linear risk models were applied to the whole body dose distribution. Finally, organ-specific non-linear risk models for the lung and breast were used to assess the secondary cancer risk for these two specific organs. A total of 32 different calculated absolute risks resulted in a broad range of values (between 0.1% and 48.5%) underlying the large uncertainties in absolute risk calculation. The ratio of risk between two techniques has often been proposed as a more robust assessment of risk than the absolute risk. We found that the ratio of risk between two techniques could also vary substantially considering the different approaches to risk estimation. Sometimes the ratio of risk between two techniques would range between values smaller and larger than one, which then translates into inconsistent results on the potential higher risk of one technique compared to another. We found however that the hybrid IMRT technique resulted in a systematic reduction of risk compared to the other techniques investigated even though the magnitude of this reduction varied substantially with the different approaches investigated. Based on the epidemiological data available, a reasonable approach to risk estimation would be to use organ-specific non-linear risk models applied to the dose distributions of organs within or near the treatment fields (lungs and contralateral breast in the case of breast radiotherapy) as the majority of radiation-induced secondary cancers are found in the beam-bordering regions.

Purpose: Diagnostic radiology involving ionizing radiation often leads to crucial information but also involves risk. Estimated cancer risks associated with CT range between 1 in 1000 to 1 in 10 000, depending on age and exposure settings. The aim of this contribution is to provide radiologists a way to inform a patient about these risks on a collective and individual base. Materials and methods: After a brief review of the effects of ionizing radiations, conversion from dose indicators into effective dose will be presented for radiography, fluoroscopy and CT. The Diagnostic Reference Level (DRL) concept will be then introduced to enable the reader to compare the level of exposure of various examinations. Finally, the limit of effective dose will be explained and risk projections after various radiological procedures for adults and children will be presented. Results: From an individual standpoint the benefit of a well justified and optimized CT examination clearly outweigh its risk of inducing a fatal cancer. The uncertainties associated with the effective dose concept should be kept in mind in order to avoid cancer risk projections after an examination on an individual basis. Conclusion: Risk factors or effective dose are not the simplest tools to communicate when dealing with radiological risks. Thus, a set of categories should be preferred as proposed in the ICRP (International Commission on Radiation Protection) report 99.

OBJETIVO: Utilizar o código PENELOPE e desenvolver geometrias onde estão presentes heterogeneidades para simular o comportamento do feixe de fótons nessas condições. MATERIAIS E MÉTODOS: Foram feitas simulações do comportamento da radiação ionizante para o caso homogêneo, apenas água, e para os casos heterogêneos, com diferentes materiais. Consideraram-se geometrias cúbicas para os fantomas e geometrias em forma de paralelepípedos para as heterogeneidades com a seguinte composição: tecido simulador de osso e pulmão, seguindo recomendações da International Commission on Radiological Protection, e titânio, alumínio e prata. Definiram-se, como parâmetros de entrada: a energia e o tipo de partícula da fonte, 6 MV de fótons; a distância fonte-superfície de 100 cm; e o campo de radiação de 10x 10 cm². RESULTADOS: Obtiveram-se curvas de percentual de dose em profundidade para todos os casos. Observou-se que em materiais com densidade eletrônica alta, como a prata, a dose absorvida é maior em relação à dose absorvida no fantoma homogêneo, enquanto no tecido simulador de pulmão a dose é menor. CONCLUSÃO: Os resultados obtidos demonstram a importância de se considerar heterogeneidades nos algoritmos dos sistemas de planejamento usados no cálculo da distribuição de dose nos pacientes, evitando-se sub ou superdosagem dos tecidos próximos às heterogeneidades.

Pienien värierojen mittaamiseen käytetään CIE:n (The International Commission on Illumination) kehittämää ja standardoitua CIELAB – väriavaruutta ja sille suunniteltua CIEDE2000 – värieromittaria. CIELAB ei kuitenkaan ole täysin yhtenäinen ja siksi mittareita joudutaan virittämään sopivammiksi tähän väriavaruuteen. Työssä tutkitaan millä tavoin CIELAB - väriavaruutta voitaisiin korjata, jotta siitä saataisiin yhtenäisempi ja yksinkertainen ja virittämättömämpi värieromittari toimisi siinä paremmin. Työn tuloksena saatu muokattu väriavaruus paransi yhtenäisyyttä keskimäärin kymmenen prosenttia, mikä on liian vähän kun tavoitteena oli 30 - 50 prosenttia.

La recrudescence des conflits internes dans le contexte post-guerre froide a permis de propulser à l’avant-plan la préoccupation pour les individus. Alors que la paix et la sécurité internationales ont historiquement constitué les piliers du système institutionnel international, une porte s’ouvrait pour rendre effectif un régime de protection des droits de l’homme par-delà les frontières. Pour les humanistes, l’intervention humanitaire représentait un mal nécessaire pour pallier aux souffrances humaines souvent causées par des divergences ethniques et religieuses. Pourtant, cette pratique est encore souvent perçue comme une forme de néo-colonialisme et entre en contradiction avec les plus hautes normes régissant les relations internationales, soit les principes de souveraineté des États et de non-intervention. La problématique du présent mémoire s’inscrit précisément dans cette polémique entre la préséance des droits de l’État et la prédilection pour les droits humains universels, deux fins antinomiques que la Commission internationales pour l’intervention et la souveraineté des États (CIISE) a tenté de concilier en élaborant son concept de responsabilité de protéger. Notre mémoire s’inscrit dans le champ de la science politique en études internationales mais s’articule surtout autour des notions et enjeux propres à la philosophie politique, plus précisément à l’éthique des relations internationales. Le travail se veut une réflexion critique et théorique des conclusions du rapport La responsabilité de protéger, particulièrement en ce qui concerne le critère de la juste cause et, dans une moindre mesure, celui d’autorité appropriée. Notre lecture des conditions de la CIISE à la justification morale du déclenchement d’une intervention humanitaire – critères issues de la doctrine de la guerre juste relativement au jus ad bellum – révèle une position mitoyenne entre une conception progressiste cosmopolitique et une vision conservatrice d’un ordre international composé d’États souverains. D’une part, la commission se dissocie du droit international en faisant valoir un devoir éthique d’outrepasser les frontières dans le but de mettre un terme aux violations massives des droits de l’homme et, d’autre part, elle craint les ingérences à outrance, comme en font foi l’établissement d’un seuil de la juste cause relativement élevé et la désignation d’une autorité multilatérale à titre de légitimateur de l’intervention. Ce travail dialectique vise premièrement à présenter et situer les recommandations de la CIISE dans la tradition de la guerre juste. Ensuite, il s’agit de relever les prémisses philosophiques tacites dans le rapport de la CIISE qui sous-tendent le choix de préserver une règle de non-intervention ferme de laquelle la dérogation n’est exigée qu’en des circonstances exceptionnelles. Nous identifions trois arguments allant en ce sens : la reconnaissance du relativisme moral et culturel; la nécessité de respecter l’autonomie et l’indépendance des communautés politiques en raison d’une conception communautarienne de la légitimité de l’État, des réquisits de la tolérance et des avantages d’une responsabilité assignée; enfin, l’appréhension d’un bouleversement de l’ordre international sur la base de postulats du réalisme classique. Pour finir, nous nuançons chacune de ces thèses en souscrivant à un mode de raisonnement cosmopolitique et conséquentialiste. Notre adhésion au discours individualiste normatif nous amène à inclure dans la juste cause de la CIISE les violations systématiques des droits individuels fondamentaux et à cautionner l’intervention conduite par une coalition ou un État individuel, pourvu qu’elle produise les effets bénéfiques désirés en termes humanitaires.

When discussing the traditional and new missions of higher education (1996 Report to UNESCO of the International Commission on Education for the 21st Century) Jacques Delors stated that "Excessive attraction to social sciences has broken equilibrium of available graduates for workforce, thus causing doubts of graduates and employers on the quality of knowledge provided by higher education". Likewise, when discussing the progress of science and technology, the 1998 UNESCO World Conference on Higher Education concluded that "Another challenge concerts the latest advancements of Science, the sine qua non of sustainable development"; and that “with Information Technology, the unavoidable invasion of virtual reality has increased the distance between industrial and developing countries". Recreational Science has a long tradition all over the Educational World; it aims to show the basic aspects of Science, aims to entertain, and aims to induce thinking. Until a few years ago, this field of knowledge consisted of a few books, a few kits and other classical (yet innovative) ways to popularize the knowledge of Nature and the laws governing it. In Spain, the interest for recreational science has increased in the last years. First, new recreational books are being published and found in bookstores. Second the number of Science-related museums and exhibits is increasing. And third, new television shows are produced and new short science-based, superficial sketches are found in variety programs. However, actual programs in Spanish television dealing seriously with Science are scarce. Recreational Science, especially that related to physical phenomena like light or motion, is generally found at Science Museums because special equipment is required. On the contrary, Science related mathematics, quizzes and puzzles use to gather into books, e.g. the extensive collections by Martin Gardner. However, lately Science podcasts have entered the field of science communication. Not only traditional science journals and television channels are providing audio and video podcasts, but new websites deal exclusively with science podcasts, in particular on Recreational Science. In this communication we discuss the above mentioned trends and show our experience in the last two years in participating at Science Fairs and university-sponsored events to attract students to science and technology careers. We show a combination of real examples (e.g., mathemagic), imagination, use of information technology, and use of social networks. We present as well an experience on designing a computational, interactive tool to promote chemistry among high school, prospective students using computers ("Dancing with Bionanomolecules"). Like the concepts related to Web 2.0, it has been already proposed that a new framework for communication of science is emerging, i.e., Science Communication 2.0, where people and institutions develop new innovative ways to explain science topics to diverse publics – and where Recreational Science is likely to play a leading role

La creación de un entorno seguro para otorgar la ayuda humanitaria a la población somalí, fue un desafío de proporciones internacionales, en el que la máxima potencia del mundo, Estados y organizaciones, vieron fracasar todos los proyectos tanto en forma de ayudas como en estrategias y operaciones. Las razones de este fracaso de carácter mundial, debido a que representó una amenaza para la paz y la seguridad internacional, todavía se pueden evidenciar en Somalia; este país continúa siendo uno de los más pobres de mundo, no cuenta con gobierno efectivo y no se logró hacer cesar las continuas violaciones de derechos humanos y derecho internacional humanitario en contra de los civiles. La importancia de este caso de estudio radica en el nuevo contexto en el cual se desenvuelve la actuación de la ONU y Estados Unidos en Somalia, en un escenario donde se hace crucial el respeto por los derechos humanos, y se puede evidenciar cómo en esta medida se da lugar a efectuar una intervención humanitaria que antes de la guerra fría hubiera sido inadmisible, partiendo de la importancia del concepto de soberanía estatal. El período fundamental en el cual el problema somalí asume magnitud internacional fue en 1991 con la caída de la dictadura militar y el inicio de un período de carestía que volvió la ya inestable situación del país en un escenario violento, caracterizado por una desastrosa situación de crisis humanitaria, culminado en la batalla de Mogadiscio, que obligó a las interrupciones de las intervenciones internacionales y al fracaso completo de estas últimas en 1995. La actuación de la ONU y de Estados Unidos en el intento de crear un entorno seguro para asistencia humanitaria en Somalia en un período de cuatro años, llevó a la creación de la UNOSOM I, UNITAF y UNOSOM II; durante este tiempo la ONU autorizó el uso de la fuerza si era necesario para asegurar la entrega de la ayuda humanitaria, por lo que los cascos azules hicieron una fuerte presencia. Finalizando 1995 los intentos fracasaron y la ONU y los Estados Unidos decidieron retirarse de Somalia indefinidamente. El siguiente trabajo se enfoca en cuatro años de un adverso desequilibrio en el territorio somalí, dando cuenta del papel que desempeñaron Estados Unidos y la ONU en el intento de crear un entorno seguro parar asistencia humanitaria, dada la precaria situación que atravesaban los civiles envueltos en la guerra civil que hasta hoy no ha culminado, y en un discurso moral que caracterizó a la comunidad internacional acerca la responsabilidad de proteger. A la luz de la carta de Naciones Unidas y de los hechos que transcurrieron antes y durante la actuación de la ONU y Estados Unidos, este caso de estudio pretende exponer los rasgos que definieron la intervención en el intento de crear un entorno seguro para asistencia humanitaria y que finalmente fracasaron.

El Lago Chad ha sido durante varias décadas, una fuente de supervivencia económica para millones de personas que habitan en cuatro Estados a saber; Nigeria, Níger, Chad y Camerún. No obstante, el cambio climático, el aumento acelerado de la población, la explotación insostenible y la mala regulación de los Estados ribereños han sido los principales factores que han dado lugar, en la última década, a la dramática reducción del nivel del Lago Chad. Teniendo en cuenta que los Estados aledaños al Lago, se encuentran inmersos en una Interdependencia Compleja, este nuevo contexto, ha tenido un impacto directo en la región, debido a que ha agravado otras variables económicas, sociales, ambientales y políticas, dejando un ambiente de inseguridad regional. De esta manera, la reducción de la Cuenca del Lago Chad representa una amenaza compartida que vincula estrechamente a Nigeria, Níger, Chad y Camerún, lo que permite vislumbrar la existencia de un Subcomplejo de Seguridad Regional.
