Wireless technology based pervasive healthcare has been proposed in many applications such as disease management and accident prevention for cost saving and promoting citizen’s wellbeing. However, the emphasis so far is on the artefacts with limited attentions to guiding the development of an effective and efficient solution for pervasive healthcare. Therefore, this paper aims to propose a framework of multi-agent systems design for pervasive healthcare by adopting the concept of pervasive informatics and using the methods of organisational semiotics. The proposed multi-agent system for pervasive healthcare utilises sensory information to support healthcare professionals for providing appropriate care. The key contributions contain theoretical aspect and practical aspect. In theory, this paper articulates the information interactions between the pervasive healthcare environment and stakeholders by using the methods of organisational semiotics; in practice, the proposed framework improves the healthcare quality by providing appropriate medical attentions when and as needed. In this paper, both systems and functional architecture of the multi-agent system are elaborated with the use of wireless technologies such as RFID and wireless sensor networks. The future study will focus on the implementation of the proposed framework.

Objective Interruptions are known to have a negative impact on activity performance. Understanding how an interruption contributes to human error is limited because there is not a standard method for analyzing and classifying interruptions. Qualitative data are typically analyzed by either a deductive or an inductive method. Both methods have limitations. In this paper a hybrid method was developed that integrates deductive and inductive methods for the categorization of activities and interruptions recorded during an ethnographic study of physicians and registered nurses in a Level One Trauma Center. Understanding the effects of interruptions is important for designing and evaluating informatics tools in particular and for improving healthcare quality and patient safety in general. Method The hybrid method was developed using a deductive a priori classification framework with the provision of adding new categories discovered inductively in the data. The inductive process utilized line-by-line coding and constant comparison as stated in Grounded Theory. Results The categories of activities and interruptions were organized into a three-tiered hierarchy of activity. Validity and reliability of the categories were tested by categorizing a medical error case external to the study. No new categories of interruptions were identified during analysis of the medical error case. Conclusions Findings from this study provide evidence that the hybrid model of categorization is more complete than either a deductive or an inductive method alone. The hybrid method developed in this study provides the methodical support for understanding, analyzing, and managing interruptions and workflow.

Oncologic specialty societies and multidisciplinary collaborative groups have dedicated considerable effort to developing evidence-based quality indicators (QIs) to facilitate quality improvement, accreditation, benchmarking, reimbursement, maintenance of certification, and regulatory reporting. In particular, radiation oncology as a field has a long history of organized quality assessment efforts, and continues to work toward developing consensus quality standards in the face of continually evolving technologies and standards of care. The present report provides a comprehensive review of the current state of quality assessment in radiation oncology, with an emphasis on recent quality improvement efforts. Specifically, this report aims to highlight implications of the healthcare quality movement for radiation oncology and review existing efforts to define and measure quality in the field, with particular focus on dimensions of quality that are specific to radiation oncology within the "big picture" of oncologic quality improvement efforts.^

Although the various tools and techniques of quality management are routinely deployed in order to improve healthcare quality, an integrated approach is lacking, which combines the customer focus to identify quality issues, analytical techniques for prioritising improvement measures and a project management approach to plan, implement and evaluate the improvement projects. This study develops an innovative framework using Quality Function Deployment (QFD) and a logical framework in order to address this issue, and demonstrates its effectiveness using a case study on the intensive care unit of a hospital.

Purpose – The purpose of this paper is to develop an integrated patient-focused analytical framework to improve quality of care in accident and emergency (A&E) unit of a Maltese hospital. Design/methodology/approach – The study adopts a case study approach. First, a thorough literature review has been undertaken to study the various methods of healthcare quality management. Second, a healthcare quality management framework is developed using combined quality function deployment (QFD) and logical framework approach (LFA). Third, the proposed framework is applied to a Maltese hospital to demonstrate its effectiveness. The proposed framework has six steps, commencing with identifying patients’ requirements and concluding with implementing improvement projects. All the steps have been undertaken with the involvement of the concerned stakeholders in the A&E unit of the hospital. Findings – The major and related problems being faced by the hospital under study were overcrowding at A&E and shortage of beds, respectively. The combined framework ensures better A&E services and patient flow. QFD identifies and analyses the issues and challenges of A&E and LFA helps develop project plans for healthcare quality improvement. The important outcomes of implementing the proposed quality improvement programme are fewer hospital admissions, faster patient flow, expert triage and shorter waiting times at the A&E unit. Increased emergency consultant cover and faster first significant medical encounter were required to start addressing the problems effectively. Overall, the combined QFD and LFA method is effective to address quality of care in A&E unit. Practical/implications – The proposed framework can be easily integrated within any healthcare unit, as well as within entire healthcare systems, due to its flexible and user-friendly approach. It could be part of Six Sigma and other quality initiatives. Originality/value – Although QFD has been extensively deployed in healthcare setup to improve quality of care, very little has been researched on combining QFD and LFA in order to identify issues, prioritise them, derive improvement measures and implement improvement projects. Additionally, there is no research on QFD application in A&E. This paper bridges these gaps. Moreover, very little has been written on the Maltese health care system. Therefore, this study contributes demonstration of quality of emergency care in Malta.

The global population of people aged 60 years and older is growing rapidly [1]. Ongoing advances in mobile technologies have the potential to improve independence and quality of life of older adults by supporting the delivery of personalised and ubiquitous healthcare solutions. Suggested healthcare reforms reflect the need for a future model of healthcare delivery wherein older adults take more responsibility for their own healthcare in their own homes in an attempt to moderate healthcare costs without impairing healthcare quality. For such a paradigm shift to be realised, the supporting technology must address the needs of older patients efficiently and effectively to ensure technology acceptance and use. We argue this is not possible without employing participatory approaches for the informed and effective design and development of such technologies and outline recommendations for engaging in participatory design with older adults with impairments based on practical experience.

Abstract Introduction: Breast cancer is the most common form of cancer among women worldwide and, therefore, deserves the highest attention and assistance from medical services. Considering patients’ satisfaction as an indication of healthcare quality, women who have undergone a mastectomy will assess the medical care received. This assessment will be based on what is expected from that medical care and on the expected improvement of her health condition. . Objective: To determine the level of satisfaction of women who have undergone a mastectomy with the medical care provided by nurses, doctors and by the way hospital services are organized. Design: A descriptive and cross-sectional study, developed in Portugal. Participants: A non-probabilistic sample formed by 153 women who underwent a mastectomy with an average age of 55, married (67.3%), unemployed (56.2%), living in a rural area (71.2%) and living on minimum wage (54.9%). Measurement instrument: European Organization for Research and Treatment of Cancer (EORTC) IN-PATSAT32 questionnaire. Results: 113 (73.85%) of the 153 women are satisied with the medical care provided and 40 (26.14%) of them show their lack of satisfaction. A highly signiicant percentage of women (49.01%) feel fairly satisied with the medical care provided by nurses and with the way services are organized (37.9). On the other hand (37.9%) show their dissatisfaction towards doctors. The family network proved to be a predictor of the satisfaction with doctors (β = 0.163; P = .044) and the period of hospitalization predicts the satisfaction with the organization (β = 0.171; P = .011). Both predictors will be useful to explain the 3% variability in patients’ satisfaction. Conclusions: Monitoring the satisfaction with the medical care received is a fundamental strategy to promote the well-being of women who underwent a mastectomy.

The aim of this study was to identify what outcome measures or quality indicators are being used to evaluate advanced and new roles in nine allied health professions and whether the measures are evaluating outcomes of interest to the patient, the clinician, or the healthcare provider. A systematic search strategy was used. Medical and allied health databases were searched and relevant articles extracted. Relevant studies with at least 1 outcome measure were evaluated. A total of 106 articles were identified that described advanced roles, however, only 23 of these described an outcome measure in sufficient detail to be included for review. The majority of the reported measures fit into the economic and process categories. The most reported outcome related to patients was satisfaction surveys. Measures of patient health outcomes were infrequently reported. It is unclear from the studies evaluated whether new models of allied healthcare can be shown to be as safe and effective as traditional care for a given procedure. Outcome measures chosen to evaluate these services often reflect organizational need and not patient outcomes. Organizations need to ensure that high-quality performance measures are chosen to evaluate the success of new health service innovations. There needs to be a move away from in-house type surveys that add little or no valid evidence as to the effect of a new innovation. More importance needs to be placed on patient outcomes as a measure of the quality of allied health interventions.

The care processes of healthcare providers are typically considered as human-centric, flexible, evolving, complex and multi-disciplinary. Consequently, acquiring an insight in the dynamics of these care processes can be an arduous task. A novel event log based approach for extracting valuable medical and organizational information on past executions of the care processes is presented in this study. Care processes are analyzed with the help of a preferential set of process mining techniques in order to discover recurring patterns, analyze and characterize process variants and identify adverse medical events.

O objetivo desta tese foi avaliar a influências da exposição a diversos fatores de risco e ocorrência de infecção hospitalar (IH) e examinar fatores de risco relacionados às pneumonias associadas ao uso de ventilador mecânico (PAV) em crianças críticas. Usamos os métodos de estudo prospectivo envolvendo uma coorte de 268 crianças menores de três anos, realizado em unidade de pacientes graves, de janeiro de 1997 e setembro de 2000. Aplicou-se técnica de regressão de Poisson para estimar razões de risco e estratégia de abordagem hierárquica para identificar fatores de risco associados à IH. Apenas para 179 crianças críticas que usaram ventilador mecânico, aplicou-se a regressão de Cox para estimar razões de risco e identificar fatores de risco associados à PAV. Os resultados apresentaram 74 infecções hospitalares diagnosticadas no total, com taxa de incidência de 48,1 IH por 1000 pacientes-dia. Foi determinante para ocorrência de infecção hospitalar, idade superior a dois anos (Razão de Risco) [RR]: 2,66; intervalo de confiança [IC]: 95%: 1,46-4,58), uso de sonda vesical (RR: 2,92; IC 95%: 1,47-5,80), uso de nutrição parenteral (RR: 1,90; IC 95%: 1,15-3,13), realização de broncoscopia (RR: 1,84; IC 95%: 1,03-3,27), tempo de exposição ao cateter vascular central (RR: 2,36; IC 95%: 1,18-4,71) e ao ventilador mecânico (RR: 1,72; IC 95%: 0,94-3,15). Observou-se PAV em 29 crianças (16,3%), com taxa de incidência de 29,3 casos por 1000 dias de ventilação mecânica (IC 95%: 20,34-42,11), dos quais 50% dos eventos ocorreram até o quinto dia de ventilação. A taxa de risco diária aumentou até um máximo de 2,3%, observada no 7 dia de ventilação, e reduziu a partir daí. Foram fatores de risco para PAV na análise multivariada hierarquizada, idade acima de 1 ano (RR: 3,49; IC 95%; 1,64-7,45), cirurgia do aparelho digestivo (RR: 5,05; IC 95%; 2,17-11,78) e nutrição parenteral (RR: 2,68; IC 95%: 1,24-5,8). /exposição a antibióticos antes da internação conferiu proteção (RR: 0,29; IC 95%: 0,13-0,66). Concluímos que os resultados encontrados neste estudo indicam que a influência do tempo de exposição é determinante para a ocorrência de infecções hospitalares e está associado aos processos de cuidados do paciente crítico. Políticas institucionais direcionadas ao controles e prevenção das IH devem fazer de estratégias fundamentais para a qualidade da assistência e segurança do paciente internado. Vigilância de Saúde Pública e componentes longitudinais de estudo de fatores de risco para infecções hospitalares e para pneumonias associadas ao ventilador podem ajudar avaliar prognósticos e planejar e testar medidas preventivas, concentrando-se esforços na primeira semana de ventilação.

RESUMO - Introdução As quedas ocorridas em ambiente hospitalar são um dos principais eventos reportados e são responsáveis por consequências físicas, psicológicas e sociais nos doentes e seus familiares, mas também nos profissionais e nas organizações de saúde. Paralelamente, tais ocorrências representam significativo impacto clínico, económico e social. Objetivo, material e métodos O propósito desta investigação é caracterizar o impacte clínico e económico da ocorrência de quedas em ambiente hospitalar. De forma a alcançar este objetivo, optou-se por um estudo assente no paradigma quantitativo, observacional, transversal, com recolha de informação de forma retrospetiva. Resultados e discussão No ano de 2013, ocorreram 118 quedas, num total de 108 indivíduos hospitalizados no CHMT. A incidência de quedas foi 0,84 quedas por mil dias de internamento. Do total de doentes em que ocorreu a queda, verificou-se que 59% tiveram consequências físicas e estimou-se um aumento de dias de internamento num total de 1114 dias, com consequente aumento dos custos totais em cerca de 1.027.591,2€. Conclusões A incidência de quedas verificada neste estudo, quando comparada com outros estudos internacionais, é menor. No que se refere ao impacto clínico e económico, constatou-se que a maioria das quedas teve consequências físicas e que, os custos adicionais relacionados com o aumento da demora média de internamento são de uma magnitude importante. Tal circunstância, justifica assim, a necessidade de desenvolvimento de estudos nesta área, no sentido de implementar estratégias efetivas que atuem ao nível do sistema e do individuo, com vista à redução deste tipo de ocorrências.

RESUMO - Prestar cuidados a um doente que além da patologia de base apresenta úlceras por pressão (UPP) implica um esforço acrescido por parte da Instituição de saúde e equipa multidisciplinar, custos acrescidos no tratamento e reactualizações frequentes, para que se prestem cuidados que primem pela excelência. OBJETIVOS: caracterizar o impacto clinico e económico das UPP e relacioná-los com as temáticas da Segurança do Doente e Qualidade em Saúde. METODOLOGIA: estudo observacional retrospetivo longitudinal, com uma população do estudo constituída por 164 adultos que estiveram internados entre 1 de janeiro a 31 de dezembro de 2012 no CHMT e que desenvolveram UPP durante o internamento. RESULTADOS: com uma população do estudo maioritariamente masculina (57,3%), e uma média de idades de 81 anos, a taxa de incidência de UPP foi de 1,4%. A maior parte de UPP desenvolvidas durante o internamento foram as de categoria II e a localização anatómica mais frequente os membros inferiores. Em termos de impacto económico, 842.609 euros foi o montante estimado correspondente aos custos totais associados às UPP. CONCLUSÃO: o contínuo investimento na Segurança do Doente e na Qualidade em Saúde e, em especial, na prevenção das UPP devem ser mantidos para que se continuem a mobilizar esforços que minimizem a frequência da ocorrência das últimas. Mais do que tratar pretende-se prevenir tendo em conta que as consequências, quer para a díade doente-família como para as Instituições de saúde, são significativas.

Les pratiques relationnelles de soin (PRS) sont au cœur même des normes et valeurs professionnelles qui définissent la qualité de l’exercice infirmier, mais elles sont souvent compromises par un milieu de travail défavorable. La difficulté pour les infirmières à actualiser ces PRS qui s’inscrivent dans les interactions infirmière-patient par un ensemble de comportements de caring, constitue une menace à la qualité des soins, tout en créant d’importantes frustrations pour les infirmières. En mettant l’accent sur l’aspect relationnel du processus infirmier, cette recherche, abordée sous l'angle du caring, renvoie à une vision novatrice de la qualité des soins et de l'organisation des services en visant à expliquer l’impact du climat organisationnel sur le façonnement des PRS et la satisfaction professionnelle d’infirmières soignantes en milieu hospitalier. Cette étude prend appui sur une adaptation du Quality-Caring Model© de Duffy et Hoskins (2003) qui combine le modèle d’évaluation de la qualité de Donabedian (1980, 1992) et la théorie du Human Caring de Watson (1979, 1988). Un devis mixte de type explicatif séquentiel, combinant une méthode quantitative de type corrélationnel prédictif et une méthode qualitative de type étude de cas unique avec niveaux d’analyse imbriqués, a été privilégié. Pour la section quantitative auprès d’infirmières soignantes (n = 292), différentes échelles de mesure validées, de type Likert ont permis de mesurer les variables suivantes : le climat organisationnel (global et cinq dimensions composites) ; les PRS privilégiées ; les PRS actuelles ; l’écart entre les PRS privilégiées et actuelles ; la satisfaction professionnelle. Des analyses de régression linéaire hiérarchique ont permis de répondre aux six hypothèses du volet quantitatif. Pour le volet qualitatif, les données issues des sources documentaires, des commentaires recueillis dans les questionnaires et des entrevues effectuées auprès de différents acteurs (n = 15) ont été traités de manière systématique, par analyse de contenu, afin d’expliquer les liens entre les notions d’intérêts. L’intégration des inférences quantitatives et qualitatives s’est faite selon une approche de complémentarité. Nous retenons du volet quantitatif qu’une fois les variables de contrôle prises en compte, seule une dimension composite du climat organisationnel, soit les caractéristiques de la tâche, expliquent 5 % de la variance des PRS privilégiées. Le climat organisationnel global et ses dimensions composites relatives aux caractéristiques du rôle, de l’organisation, du supérieur et de l’équipe sont de puissants facteurs explicatifs des PRS actuelles (5 % à 11 % de la variance), de l’écart entre les PRS privilégiées et actuelles (4 % à 9 %) ainsi que de la satisfaction professionnelle (13 % à 30 %) des infirmières soignantes. De plus, il a été démontré, qu’au-delà de l’important impact du climat organisationnel global et des variables de contrôle, la fréquence des PRS contribue à augmenter la satisfaction professionnelle des infirmières (ß = 0,31 ; p < 0,001), alors que l’écart entre les PRS privilégiées et actuelles contribue à la diminuer (ß = - 0,30 ; p < 0,001) dans des proportions fort similaires (respectivement 7 % et 8 %). Le volet qualitatif a permis de mettre en relief quatre ordres de facteurs qui expliquent comment le climat organisationnel façonne les PRS et la satisfaction professionnelle des infirmières. Ces facteurs sont: 1) l’intensité de la charge de travail; 2) l’approche d’équipe et la perception du rôle infirmier ; 3) la perception du supérieur et de l’organisation; 4) certaines caractéristiques propres aux patients/familles et à l’infirmière. L’analyse de ces facteurs a révélé d’intéressantes interactions dynamiques entre quatre des cinq dimensions composites du climat, suggérant ainsi qu’il soit possible d’influencer une dimension en agissant sur une autre. L’intégration des inférences quantitatives et qualitatives rend compte de l’impact prépondérant des caractéristiques du rôle sur la réalisation des PRS et la satisfaction professionnelle des infirmières, tout en suggérant d’adopter une approche systémique qui mise sur de multiples facteurs dans la mise en oeuvre d’interventions visant l’amélioration des environnements de travail infirmier en milieu hospitalier.

Cette thèse s’intéresse à l’amélioration des soins et des services de santé et touche aux relations entre 3 grands thèmes de l’analyse des organisations de santé : la gouvernance, le changement et les pratiques professionnelles. En nous appuyant sur l’analyse organisationnelle contemporaine, nous visons à mieux comprendre l’interface entre l’organisation et les pratiques cliniques. D’une part, nous souhaitons mieux comprendre comment l’organisation structure et potentialise les pratiques des acteurs. D’autre part, dans une perspective d’acteurs stratégiques, nous souhaitons mieux comprendre le rôle des pratiques des professionnels dans l’actualisation de leur profession et dans la transformation et l’évolution des organisations. Notre étude se fonde sur l’hypothèse qu’une synergie accrue entre l’organisation et les pratiques des professionnels favorisent l’amélioration de la qualité des soins et des services de santé. En 2004, le gouvernement ontarien entreprend une importante réforme des soins et services dans le domaine du cancer et revoit les rôles et mandats du Cancer Care Ontario, l’organisation responsable du développement des orientations stratégiques et du financement des services en cancer dans la province. Cette réforme appelle de nombreux changements organisationnels et cliniques et vise à améliorer la qualité des soins et des services dans le domaine de l’oncologie. C’est dans le cadre de cette réforme que nous avons analysé l’implantation d’un système de soins et de services pour améliorer la performance et la qualité et analysé le rôle des pratiques professionnelles, spécifiquement les pratiques infirmières, dans la transformation de ce système. La stratégie de recherche utilisée correspond à l’étude approfondie d’un cas correspondant à l’agence de soins et de services en oncologie en Ontario, le Cancer Care Ontario, et des pratiques professionnelles infirmières évoluant dans ce modèle. Le choix délibéré de ce cas repose sur les modalités organisationnelles spécifiques à l’Ontario en termes de soins en oncologie. La collecte de données repose sur 3 sources principales : les entrevues semi-structurées (n=25), l’analyse d’une abondante documentation et les observations non participatives. La thèse s’articule autour de trois articles. Le premier article vise à définir le concept de gouvernance clinique. Nous présentons l’origine du concept et définissons ses principales composantes. Concept aux frontières floues, la gouvernance clinique est axée sur le développement d’initiatives cliniques et organisationnelles visant à améliorer la qualité des soins de santé et la sécurité des patients. L’analyse de la littérature scientifique démontre la prédominance d’une vision statique de la gouvernance clinique et d’un contrôle accentué des pratiques professionnelles dans l’atteinte de l’efficience et de l’excellence dans les soins et les services. Notre article offre une conception plus dynamique de la gouvernance clinique qui tient compte de la synergie entre le contexte organisationnel et les pratiques des professionnels et soulève les enjeux reliés à son implantation. Le second article s’intéresse à l’ensemble des leviers mobilisés pour institutionnaliser les principes d’amélioration continue de la qualité dans les systèmes de santé. Nous avons analysé le rôle et la portée des leviers dans l’évolution du système de soins en oncologie en Ontario et dans la transformation des pratiques cliniques. Nos données empiriques révèlent 3 phases et de nombreuses étapes dans la transformation du système. Les acteurs en position d’autorité ont mobilisé un ensemble de leviers pour introduire des changements. Notre étude révèle que la transformation du Cancer Care Ontario est le reflet d’un changement radical de type évolutif où chacune des phases est une période charnière dans la transformation du système et l’implantation d’initiatives de qualité. Le troisième article pose un regard sur un levier spécifique de transformation, celui de la communauté de pratique, afin de mieux comprendre le rôle joué par les pratiques professionnelles dans la transformation de l’organisation des soins et ultimement dans le positionnement stratégique de la profession infirmière. Nous avons analysé les pratiques infirmières au sein de la communauté de pratique (CDP) des infirmières en pratique avancée en oncologie. En nous appuyant sur la théorie de la stratégie en tant que pratique sociale, nos résultats indiquent que l’investissement de la profession dans des domaines stratégiques augmente les capacités des infirmières à transformer leurs pratiques et à transformer l’organisation. Nos résultats soulignent le rôle déterminant du contexte dans le développement de capacités stratégiques chez les professionnels. Enfin, nos résultats révèlent 3 stratégies émergentes des pratiques des infirmières : une stratégie de développement de la pratique infirmière en oncologie, une stratégie d’institutionnalisation des politiques de la CDP dans le système en oncologie et une stratégie de positionnement de la profession infirmière. Les résultats de notre étude démontrent que l’amélioration de la qualité des soins et des services de santé est située. L’implantation de transformations dans l’ensemble d’un système, tel que celui du cancer en Ontario, est tributaire d’une part, des capacités d’action des acteurs en position d’autorité qui mobilisent un ensemble de leviers pour introduire des changements et d’autre part, de la capacité des acteurs à la base de l’organisation à s’approprier les leviers pour développer un projet professionnel, améliorer leurs pratiques professionnelles et transformer le système de soins.
