Radiologists have been confronted with multiple new challenges in recent years. While there has been a steady increase in the number of radiological examinations and imaging material per examination, examination protocols have become more complex and highly time-consuming whereas case-based remuneration is on the decline. The identification of inefficient components in examination processes and reporting is therefore essential. Where and why do time delays occur? How can they be avoided? The following article provides a brief overview and is designed to stimulate discussion.

BACKGROUND & AIMS Patients with chronic hepatitis C virus (HCV) infection may develop cirrhosis with portal hypertension, reflected by decreased platelet count and splenomegaly. This retrospective cohort study aimed to assess changes in platelet counts after antiviral therapy among chronic HCV-infected patients with advanced fibrosis. METHODS Platelet counts and spleen sizes were recorded in an international cohort of patients with Ishak 4-6 fibrosis who started antiviral therapy between 1990 and 2003. Last measured platelet counts and spleen sizes were compared to their pre-treatment values (within 6 six months prior to the start of therapy). All registered platelet count measurements from 24 week following cessation of antiviral therapy were included in repeated measurement analyses. RESULTS This study included 464 patients; 353 (76%) had cirrhosis and 187 (40%) attained sustained virological response (SVR). Among patients with SVR, median platelet count, increased by 35 x10(9) /L (IQR 7-62, p<0.001). In comparison, patients without SVR showed a median decline of 17 x10(9) /L (IQR -5-47, p<0.001). In a subgroup of 209 patients, median decrease in spleen size was 1.0 cm (IQR 0.3-2.0) for patients with SVR, while median spleen size increased with 0.6 cm (IQR -0.1-2.0, p<0.001) among those without SVR. The changes in spleen size and platelet count were significantly correlated (R=-0.41, p<0.001). CONCLUSIONS Among chronic HCV-infected patients with advanced hepatic fibrosis the platelet counts improved following SVR and the change in platelets correlated with the change in spleen size following antiviral therapy. The benefits of urban agriculture are many and well documented, ranging from health improvement to community betterment, more sustainable urban development and environment protection. On the negative side, urban soils are commonly enriched in toxic trace elements that have accumulated over time through the deposition of atmospheric particles (generated by automotive traffic, heating systems, historical industrial activities and resuspended street dust), and the uncontrolled disposal of domestic, commercial and industrial wastes. This in turn has given rise to concerns about the level of exposure of urban farmers to these elements and the potential health hazards associated with this exposure. Research efforts in this field have started relatively recently and have almost systematically omitted the influence of Sb and Se, and to a lesser extent of As, although all three have proven toxic effects.

Despite having been described by the then (2003) Chief Pharmaceutical Officer for England as ·probably the biggest untapped resource for health improvement", the development of the public health function of community pharmacists has been limited. However, devolution of healthcare budgets has led 10 differential rates of development of the public health function in each administration of the UK (England, Scotland, Wales and Northern Ireland). This is measured and reflected upon in this thesis. Two large-scale surveys were conducted, one of key strategic personnel (Directors of Public Health and Chief Pharmacists) in Primary Care Organisations (PCOs) and one of practicing community pharmacists. This research highlights the fact that community pharmacists have developed an individualistic, service-based approach to their engagement with public health that is contrary to the more collective approach adopted by the wider public health movement. The study measures the scope and level of health-improving services through community pharmacy across the UK and shows that the nature of the pharmacy contractor (independent, multiple etc.) may impact on the range and nature of services provided. Survey data also suggest that attitudes towards pharmacy involvement in the public health agenda vary markedly between Directors of Public Health, PCO Chief Pharmacists, and community pharmacists. Furthermore, within the community pharmacist population, attitudes are affected by a wide range of factors including the nature of employment (owner, employee, self-employed) and the type of employing pharmacy (independent, multiple etc.). Implications for policy and areas for further research aimed at maximising the public health function of community pharmacists are suggested.

Resumen: Desde un punto de vista general el agua es definida en la actualidad como un recurso natural no renovable en cantidad, recuperable en calidad y con posibilidad de nuevo uso (re-uso) en continua mutación dentro del ciclo hidrológico. A la vez posee las categorías de bien común como elemento esencial para la vida humana y que como tal no tiene precio, y de recurso hídrico con dimensión económica como insumo, que supone un valor mensurable. Los recursos hídricos y su gestión forman parte de la administración pública, como pueden serlo la energía o la salud. La organización adecuada de un servicio, y las formas institucionales a través de las que se presta, varían de acuerdo a las épocas, los costes tecnológicos, los de distribución, organización y regulación, resultando un aspecto importante del análisis la implementación y funcionamiento del servicio público. Atendiendo a estos aspectos teóricos, este trabajo continua con la problemática del abastecimiento de agua corriente en la ciudad de Córdoba, ahora en la primera década del siglo XX, manteniendo el supuesto de que las dificultades para el abastecimiento de agua obedecen a razones de orden natural, a dificultades económicas y a enfrentamientos políticos, en el marco de un Estado que aún posee limitaciones de índole institucional y administrativo, por lo cual intervendrán el gobierno provincial y Nacional para la concreción de las obras de salubridad, en una ciudad y un país enrolados en el proceso de modernización impulsado por el deseo de adecuación al modelo de vida europeo. Seguimos analizando las fuentes inéditas del Archivo Histórico Municipal de Córdoba, fuentes oficiales editas, publicaciones periódicas y bibliografía tanto general como específica.

Este estudo tem como objetivo identificar a estrutura representacional do cuidado em saúde ao paciente soropositivo ao HIV/Aids entre os profissionais da equipe de saúde, descrever a representação do cuidado em saúde através das abordagens estrutural e processual, analisar o cuidado em saúde a partir das representações construídas por essa equipe e discutir a mudança de conhecimentos e práticas de cuidado em saúde a partir das representações. Pesquisa qualitativa, descritiva e exploratória, orientada pela Teoria das Representações Sociais. Os sujeitos do estudo foram profissionais de saúde atuantes em instituições públicas no Rio de Janeiro, sendo na primeira etapa, 148 profissionais de 20 instituições e, na segunda, 23 de duas instituições. A coleta de dados deu-se através de questionários de caracterização, evocações livres e entrevista semiestruturada. A análise dos dados utilizou o quadro de quatro casas a partir do software EVOC 2003, a análise de similitude e a análise lexical pelo software Alceste 4.10. Com relação à estrutura da Representação, destaca-se que o Núcleo Central foi composto por: amor, acolhimento, informação, respeito, adesão-tratamento e não-discriminação, que indicam uma dimensão relacionada aos aspectos afetivos, práticos e funcionais do cuidado. Na zona de contraste, ressalta-se a presença da proteção-profissional. A partir da análise de similitude foi possível perceber a tendência à centralidade do léxico amor. Ele organiza a representação e possui forte ligação com o termo solidariedade. Na análise realizada pelo Alceste, foram definidas seis classes: A constituição da equipe de saúde e o processo de cuidar: definições, práticas e dificuldades; As práticas de proteção adotadas pelos profissionais frente ao HIV; As formas de enfrentamento diante do diagnóstico positivo ao HIV; Atitudes profissionais: avaliações e julgamentos; As memórias sociais sobre a aids: origem, circulação, informação e imagens e, por fim, A epidemia de aids nos dias atuais. Conclui-se que a representação social do cuidado entre profissionais de saúde tem, em sua estrutura e organização, a interação sustentada por uma arqueologia que se liga aos considerados sentimentos nobres e às ações de abnegação e de superação humanas, tendo como elemento central o amor que caracteriza suas ações e influencia sua práxis. A importância da oferta desse cuidado amoroso está em sua relação com a humanização das ações assistenciais, atendendo à demanda do indivíduo durante a manifestação da doença, respeitando sua singularidade como ser humano e facilitando seu processo de adoecimento e busca pela melhoria da saúde.

Food insecurity, chronic hunger, starvation and malnutrition continue to affect millions of individuals throughout the developing world, especially Sub-Saharan Africa. Various initiatives by African governments and International Agencies such as the UN, the industrial nations, the International Monetary Fund, the World Bank and the World Trade Organisation to boost economic development, have failed to provide the much-needed solution to these challenges. The impact of these economic shifts and the failures of structural adjustment programmes on the nutritional well-being and health of the most vulnerable members of poor communities cannot be over-emphasised. The use of ad hoc measures as an adjunct to community-based rural integrated projects have provided little success and will be unsustainable unless they are linked to harnessing available local resources. The present paper therefore focuses on exploring alternative ways of harnessing the scant agricultural resources by employing a scientific approach to food-related problem-solving. The food multimix (FMM) concept offers a scientific contribution alongside other attempts currently in use by the World Food Programme, WHO and FAO to meet the food insecurity challenges that confront most of the developing world in the twenty-first century. It is an innovative approach that makes better use of traditional food sources as a tool for meeting community nutritional needs. The FMM concept employs a food-based approach using traditional methods of food preparation and locally-available, cheap and affordable staples (fruits, pulses, vegetables and legumes) in the formulation of nutrient-enriched multimixes. Developed recipes can provide >= 40% of the daily nutritional requirements of vulnerable groups, including patients with HIV/AIDS and children undergoing nutrition rehabilitation. The FMM approach can also be used as a medium- to long-term adjunct to community-based rural integration projects aimed at health improvement and economic empowerment in Sub-Saharan Africa.

RESUMO - Introdução: A inatividade física é um dos determinantes major das doenças crónicas não transmissíveis sendo a quarta maior causa de mortalidade no mundo, nomeadamente para as doenças vasculares. A prática regular de atividade física produz adaptações vasculares responsáveis por efeitos benéficos na prevenção e tratamento dos diferentes fatores de risco vascular, nomeadamente através do seu efeito no metabolismo das lipoproteínas. Objetivos: Analisar a interferência da atividade física no perfil lipídico de uma população residente em Portugal. Métodos: Estudo observacional descritivo transversal exploratório com 1027 indivíduos (idade: 18 aos 80 anos, 49% mulheres). Os dados foram analisados em SPSS (versão 20), tendo-se utilizado métodos de estatística descritiva e de análise bivariável entre os factores de risco vascular e as variáveis do perfil lipídico e ainda uma análise multivariável de regressão logística binária para medir a razão de riscos pelo odds ratio. O nível de significância foi estabelecido em 5%. Resultados: Na análise da relação entre atividade física e os biomarcadores do perfil lipídico verificou-se que existem benefícios no que diz respeito ao aumento dos níveis de HDL e de apoA1 e na diminuição dos níveis de TG com a prática regular de atividade física. Conclusões: A atividade física apresenta um papel importante na regulação do perfil lipídico evidenciando a necessidade de implementar estratégias multissectoriais de prevenção dos fatores de risco vascular, nomeadamente na área dos estilos de vida saudáveis que são fundamentais para a prevenção destas condições de saúde e para gerar ganhos em saúde.

The Chronic Obstructive Pulmonary Disease (COPD) has a progressive and irreversible character and it’s associated to the triad of dyspnea, exercise limitation and the evident deterioration of quality of life. In the United States the prevalence of COPD in adult population is approximately of 6% in men, and 1 to 3% in women and it’s the fourth cause of mortality by no transmissible chronic diseases. In 1993, the National Health Interview Surgery considered that 12 millions of Americans suffer from chronic bronchitis and 2 million had emphysema. These two affections are responsible for more than 13% of the hospitalizations. As this affection progresses, patients experience a diminution in quality of life related to health (CVRS), their capacity to work get worse and their participation in physical and social activities reduces. Nevertheless, it has been confirmed that the isolated evaluation of COPD seriousness, defined by the reduction of the Forced Expiratory Volume in the First Second (FEV1), does not provide enough information to know the health state perceived by the patients. The fact that the CVRS is the result of the interaction of multiple physical, psychological and social factors, unique for each individual, can explain this finding. This paper is a general and updated approach to the integral handling of patients with COPD, and it discusses the concept of quality of life, related to health improvement.

O interesse acentuado por um estilo de vida saudável e uma alimentação equilibrada têm sido a base para melhoria na saúde e deste modo, na qualidade de vida. Esta motivação, cada vez mais presente na população, tem levado a um crescente interesse pelos suplementos nutricionais, onde se incluem os suplementos à base de plantas. As plantas medicinais têm sido usadas há milhares de anos devido às propriedades benéficas que possuem, sendo vistos como produtos de boa qualidade, seguros e com poucos efeitos secundários. Contudo, a baixa incidência de efeitos secundários nestes produtos nem sempre corresponde à realidade. Assim, compete ao profissional de saúde, em particular ao farmacêutico aconselhar o utente na aquisição destes produtos, uma vez que este tem as competências que lhe permitem prestar o aconselhamento adequado ao doente, que visa ponderar os benefícios versos os potenciais efeitos adversos destes suplementos. O elevado preço dos suplementos nutricionais em comparação com os medicamentos, que são comparticipados (situação que não se verifica, por exemplo nos Estados Unidos), e ainda a situação económica complicada que se vive em Portugal, tem feito com que a aquisição destes tenha sido severamente afetada. Não sendo considerados produtos essenciais são procurados principalmente quando os sintomas ocorrem ou quando recomendados pelo médico. No presente trabalho, será feita uma análise do mercado de todo o contexto económico que engloba os suplementos nutricionais à base de plantas assim como a previsão de venda dos mesmos de modo a entender se é uma mais-valia para as empresas apostarem na produção/venda desses produtos e da mesma maneira compreender se os consumidores têm a ganhar com a compra desses suplementos.

Fire-related accidents often result in injuries and sometimes death, which can be prevented through fire safety training. To estimate the extent to which fire safety training should be provided, it is essential to assess the current level of fire safety knowledge within the general community. Thus the objectives of the present study were to explore: (a) the level of fire safety knowledge among people of different age groups and investigate its relationship to the level of fire safety training, and (b) the manner in which people from different age groups would respond to a fire based on their fire safety training. Data from 158 participants aged between 18 and 80 years showed that fire safety training increases: (a) the level of fire safety knowledge and, (b) the accuracy of response to a fire. The results also show that middle-aged individuals would respond more accurately to a fire than younger and older adults. The findings demonstrate the importance of fire safety training in enhancing people's fire safety knowledge and their response in the event of a fire which could lead to a reduction in the rate of fire casualties. There are implications for incorporating fire safety training as part of health improvement programs to reduce the number of fire-related injuries and fatalities.

Background
There is a popular belief that out-of-home eating outlets, which typically serve energy dense food, may be more commonly found in more deprived areas and that this may contribute to higher rates of obesity and related diseases in such areas.

Methods
We obtained a list of all 1301 out-of-home eating outlets in Glasgow, UK, in 2003 and mapped these at unit postcode level. We categorised them into quintiles of area deprivation using the 2004 Scottish Index of Multiple Deprivation and computed mean density of types of outlet (restaurants, fast food restaurants, cafes and takeaways), and all types combined, per 1000 population. We also estimated odds ratios for the presence of any outlets in small areas within the quintiles.

Results
The density of outlets, and the likelihood of having any outlets, was highest in the second most affluent quintile (Q2) and lowest in the second most deprived quintile (Q4). Mean outlets per 1,000 were 4.02 in Q2, 1.20 in Q4 and 2.03 in Q5. With Q2 as the reference, Odds Ratios for having any outlets were 0.52 (CI 0.32–0.84) in Q1, 0.50 (CI 0.31 – 0.80) in Q4 and 0.61 (CI 0.38 – 0.98) in Q5. Outlets were located in the City Centre, West End, and along arterial roads.

Conclusion
In Glasgow those living in poorer areas are not more likely to be exposed to out-of-home eating outlets in their neighbourhoods. Health improvement policies need to be based on empirical evidence about the location of fast food outlets in specific national and local contexts, rather than on popular 'factoids'.

The Australian Government's current health reform agenda provides a timely opportunity to highlight the contribution of health psychology interventions in the prevention and management of chronic diseases associated with lifestyle risk factors. The World Health Organisation (2009) has identified the main risk factors responsible for deaths internationally as high blood pressure (responsible for 13% of deaths), tobacco use (9%), high blood sugar (6%), physical inactivity (6%), overweight and obesity (5%), high cholesterol (5%), unsafe sex (4%) and alcohol use (4%). A number of these factors also increase the risk of major chronic diseases - cardiovascular disease, diabetes and cancers. There is now a substantial evidence base for the effectiveness of health improvement interventions based on psychological theory, research and practice and hence they deserve a high level of recognition within systems for funding health. This article presents a summary of a systematic review of the evidence for the effectiveness of health psychology interventions in the prevention and treatment of chronic diseases associated with lifestyle risk factors.
