Anabolic androgenic steroids (AAS) are testosterone derivatives used either clinically, in elite sports, or for body shaping with the goal to increase muscle size and strength. Clinically developed compounds and nonclinically tested designer steroids often marketed as food supplements are widely used. Despite the considerable evidence for various adverse effects of AAS use, the underlying molecular mechanisms are insufficiently understood. Here, we investigated whether some AAS, as a result of a lack of target selectivity, might inhibit 11β-hydroxysteroid dehydrogenase 2 (11β-HSD2)-dependent inactivation of glucocorticoids. Using recombinant human 11β-HSD2, we observed inhibitory effects for several AAS. Whereas oxymetholone, oxymesterone, danazol, and testosterone showed medium inhibitory potential, fluoxymesterone was a potent inhibitor of human 11β-HSD2 (half-maximal inhibitory concentration [IC(50)] of 60-100nM in cell lysates; IC(50) of 160nM in intact SW-620, and 530nM in MCF-7 cells). Measurements with rat kidney microsomes and lysates of cells expressing recombinant mouse 11β-HSD2 revealed much weaker inhibition by the AAS tested, indicating that the adverse effects of AAS-dependent 11β-HSD2 inhibition cannot be investigated in rats and mice. Furthermore, we provide evidence that fluoxymesterone is metabolized to 11-oxofluoxymesterone by human 11β-HSD2. Structural modeling revealed similar binding modes for fluoxymesterone and cortisol, supporting a competitive mode of inhibition of 11β-HSD2-dependent cortisol oxidation by this AAS. No direct modulation of mineralocorticoid receptor (MR) function was observed. Thus, 11β-HSD2 inhibition by fluoxymesterone may cause cortisol-induced MR activation, thereby leading to electrolyte disturbances and contributing to the development of hypertension and cardiovascular disease.

BACKGROUND: To estimate the prevalence of undernutrition among children with profound intellectual and multiple disabilities (PIMD) and to explore its influence on quality of life. METHODS: Seventy-two children with PIMD (47 male; 25 female; age range 2 to 15 years 4 months; mean age 8.6, SD 3.6) underwent an anthropometric assessment, including body weight, triceps skinfold thickness, segmental measures and recumbent length. Undernutrition was determined using tricipital skinfold percentile and z-scores of weight-for-height and height-for-age. The quality of life of each child was evaluated using the QUALIN questionnaire adapted for profoundly disabled children. RESULTS: Twenty-five children (34.7%) were undernourished and seven (9.7%) were obese. Among undernourished children only eight (32 %) were receiving food supplements and two (8%) had a gastrostomy, of which one was still on a refeeding programme. On multivariate analysis, undernutrition was one of the independent predictors of lower quality of life. CONCLUSION: Undernutrition remains a matter of concern in children with PIMD. There is a need to better train professionals in systematically assessing the nutritional status of profoundly disabled children in order to start nutritional management when necessary.

Stimulants are banned in-competition for all categories of sports by the World Anti-Doping Agency. A simple liquid chromatography-tandem mass spectrometry (LC-MS/MS) assay employing electrospray ionisation in positive mode was developed in that work for the quantification in urine specimens of 4-methyl-2-hexaneamine, a primary amine exhibiting sympathomimetic properties. Following a simple pretreatment procedure, the analyte was separated using a gradient mobile phase on reverse phase C8 column. Selected reaction monitoring m/z 116.2-->57.3 was specific for detection of 4-methyl-2-hexaneamine and the assay exhibited a linear dynamic range of 50-700 ng/mL. The validated method has been successfully applied to analyze the target compound in food supplements as well as in urine specimens. The administered drug (40 mg) was detected at the level of 350 ng/mL in the urine up to 4 days.

The present study reports arsenic speciation analysis in edible Shiitake (Lentinula edodes) products. The study focused on the extraction, and accurate quantification of inorganic arsenic (iAs), the most toxic form of arsenic, which was selectively separated and determined using anion exchange LC-ICPMS. A wide variety of edible Shiitake products (fresh mushrooms, food supplements, canned and dehydrated) were purchased and analysed. A cultivated Shiitake grown under controlled conditions was also analysed. The extraction method showed satisfactory extraction efficiencies (>90%) and column recoveries (>85%) for all samples. Arsenic speciation revealed that iAs was the major As compound up to 1.38 mg As per kg dm (with a mean percentage of 84% of the total arsenic) and other organoarsenicals were found as minor species. Shiitake products had high proportions of iAs and therefore should not be ignored as potential contributors to dietary iAs exposure in populations with a high intake of Shiitake products.

Which treatments are used for dysmenorrhea and with what reported outcome? A questionnaire was sent to 2400 students and apprentices, following the "retrospective treatment-outcome" method. The response rate was 22%. Most frequent treatments used are ibuprofene (53%), paracetamol (51%), hormonal contraception (40%), hot-water bottle (or hot pad) (35%), food supplements or medicinal plants (23%). Physicians only discuss a tiny proportion of dysmenorrhea treatment in their consultation, because it is mostly a matter of self-treatment, with the family as the source of information in 80% of the cases. Rather surprising because not mentioned in most official guidelines, hot-water bottle (or hot pad) appears as the treatment followed by the best reported outcome (satisfactory in 92% of users).

Este trabalho teve como objectivo investigar os hábitos e conhecimentos gerais de uma amostra da população de Lisboa e Vale doTejo relacionados com o consumo dos suplementos e medicamentos à base de plantas medicinais. Realizou-se um estudo não experimental descritivo, transversal, através de um questionário anónimo, confidencial e voluntário a 367 indivíduos, preenchido pelas entrevistadoras, mediante as respostas obtidas dos entrevistados, sendo a amostra obtida por conveniência. Os indivíduos pertenciam maioritariamente ao sexo feminino 74,7% (274/367) e à classe etária dos 20 aos 29 anos, 52,0% (191/367). Dos inquiridos, 48,8% (179/367) consomem medicamentos e/ou suplementos à base de plantas e destes, 25,7% (46/179) afirmam consumir medicamentos à base de plantas medicinais enquanto 20,1% (36/179) afirmam consumir suplementos alimentares à base de plantas. Do total dos inquiridos que consomem medicamentos e/ou suplementos à base de plantas, 2,8% (5/179) responderam que já sentiram reacção adversa, 93,9% (168/179) disseram não ter sentido qualquer reacção adversa e 3,4% (6/179) não sabe ou não se lembra. Inquiridos sobre a diferença entre medicamentos à base de plantas e suplementos alimentares à base de plantas, 67,6% (121/179) afirmam conhecer a diferença e 32,4% (58/179) não conhecem a diferença entre medicamentos e suplementos alimentares.

A perda de memória é uma das principais queixas entre a população idosa. Reivindicada para melhorar a função cognitiva e de fácil obtenção em farmácias e dietéticas, a suplementação com fosfolípidos produziu resultados mistos em vários estudos. Um grupo de adultos e idosos saudáveis (n = 522) com idade 65,84 ± 10,74 anos, e com um nível de escolaridade de 7,90 ± 4,90 anos participaram do estudo. Os participantes foram submetidos a Avaliação Breve do Estado Mental (ABEM), Escala de Queixas de Memória (EQM) e Escala de Depressão Geriátrica (EDG). Três indivíduos apresentaram disfunção cognitiva, 505 queixas de memória apresentadas e 257 estavam acima do ponto de corte para a EDG. Subsequentemente, um grupo de mulheres (n = 17) utilizaram a suplementação com fosfolípidos durante 4 semanas. Após o período de suplementação, foi observada uma diminuição nos valores médios de EQM e EDG. A suplementação de fosfolípidos, ao longo de 4 semanas reduziu significativamente os valores EQM (p <0,05) neste grupo de mulheres.

Para avaliar o efeito da suplementação de fosfatidilserina nos resultados obtidos na Escala de Queixas de Memória (EQM) e na Escala de Depressão Geriátrica (EDG), um grupo de adultos e idosos saudáveis (n= 915) com idade 65,65 ± 11,10 anos, e com um nível de scolaridade de 7,56 ± 4,56 anos participaram do estudo. Os participantes foram submetidos a Avaliação Breve do Estado Mental ABEM), Escala de Queixas de Memória (EQM) e Escala de Depressão Geriátrica (EDG). Quatro indivíduos apresentaram disfunção cognitiva, 874 queixas de memória apresentadas e 436 estavam acima do ponto de corte para a EDG. Subsequentemente, um grupo de mulheres (n= 48) utilizaram a suplementação de fosfatidilserina durante 4 semanas. A suplementação de fosfatidilserina, ao longo de 4 semanas reduziu significativamente os valores EQM (p=0,000) e de EDG (p=0,009) neste grupo de mulheres.

A exposição à radiação ultravioleta (UV) está associada a uma variedade de efeitos negativos causados no organismo humano. Este espectro de radiação UV divide-se em UVA (320-400nm), responsável pelo aparecimento dos sinais do envelhecimento cutâneo e danos indiretos no DNA, em UVB (290-320nm), responsável pelo eritema solar e danos diretos no DNA, e em UVC (100-290nm), cuja radiação fica retida na camada de ozono. A pele apresenta um mecanismo de defesa natural contra a radiação UV, mas este confere uma proteção limitada, tornando-se indispensável o recurso à fotoproteção. A utilização de vestuário adequado, chapéu e óculos de sol durante a exposição solar têm sido reconhecidas como medidas importantes de fotoproteção, em complementaridade com um protetor solar. Os filtros dos protetores solares são selecionados e aprovados por agências reguladoras da região em que o produto é comercializado. Na União Europeia existem atualmente 28 filtros para radiação UV aprovados. Esta dissertação aborda as últimas tendências na área da fotoproteção. Os últimos desenvolvimentos na área dos protetores solares sugerem filtros solares que compreendam ação sobre a radiação UVA e UVB e IV, em associação com outros compostos que demonstram propriedades reparadoras a nível tópico, como é o caso dos antioxidantes. Assim, os avanços nesta área incluem os suplementos alimentares, os agentes estimulantes da melanogénese e os novos filtros solares.

Bone metabolism involves a complex balance between the deposition of matrix and mineralization and resorption. There is now good evidence that dietary components and herbal products can influence these processes, particularly by inhibiting bone resorption, thus having beneficial effects on the skeleton. For example, it has been reported that a number of common vegetables, including onion, garlic and parsley, can inhibit bone resorption in ovariectomized rats. Essential oils derived from sage, rosemary, thyme and other herbs inhibit osteoclast activity in vitro and in vitro and leading to an increase in bone mineral density. Soya, a rich source of isoflavones, has shown promising results and epidemiological evidence to support a use in maintaining bone health, and various traditional herbal formulae in Chinese and Ayurvedic medicine also have demonstrable effects in pharmacological models of osteoporosis. Recently, cannabinoids have been described as having positive effects on osteoblast differentiation, and the presence of cannabinoid receptors in bone tissue indicates a more complex role in bone metabolism than previously thought. The first part of this review briefly discusses normal bone metabolism and disorders caused by its disruption, with particular reference to osteoporosis and current pharmacological treatments. The effects of natural products on bone and connective tissue are then discussed, to include items of diet, herbal extracts and food supplements, with evidence for their efficacy outlined. While providing physical and psychological benefits, excessive exercise could be or cause a compulsive behavior, making the individual dependent on it. In a parallel discussion, computerized psychological instruments, for a hand, reflects the development of information technology and your applicability to other areas, but also shows little advance for Psychological Assessment. In this perspective, this study aims to adapt the Exercise Dependence Scale (EDS-R) in two formats (paper-and-pencil and computerized) and evaluate evidence of factorial and convergent validity, and reliability of each version and compare them with each other. It is also proposed to observe the relationship of some bio-demographic (Sex, age, frequency, duration and intensity of practice exercise) and the exercise dependence (DEF). For this purpose, 709 regular physical activity practitioners, selected by procedures non-probabilistic sampling, responded a adapted version of EDS-R, Muscle Appearance Satisfaction Scale (MASS), Body Modification Scale (BMS) and a demographic questionnaire, analyzed through Exploratory Factor Analysis, Cronbach's Alpha and not parametric tests. Both the traditional version and the computer showed a seven factors structure, explaining 57 and 62% of the variance, respectively, and Cronbach's alphas of 0.83 and 0.89. Factors were: (1) intentionality, (2) continuity, (3) tolerance, (4) reduction of other activities, (5) lack of control, (6) abstinence and (7) time spent on exercise. Relationships were observed between the Exercise Dependence and the variables: age, diets, consumption of food supplements and medicines for weight change, desire to do plastic surgery and body satisfaction. We observed also a positive correlation between the DEF and the frequency, duration and intensity of exercise, and the factor "Dependence on exercising" from MASS, indicating convergent validity of the EDS-R. Finally, comparisons between the two formats were equivalent, with few changes: computerized version achieved higher DEF scores. Based on these results, it can be concluded that the EDS-R has factorial and convergent validity, reliability, to measure exerceise dependence on traditional e computerized formats. DEF is related to actions used to body modification and behaviors toward exercise. Finally, it was found equivalence between the formats, especially in psychometric parameters, thus suggesting feasibility of a computerized assessment. However, it was observed that the computerized data has sample recruiting strategies more limited

Com o objetivo de caracterizar os ábitos alimentares de nadadores competitivos de dois clubes das cidades do Rio de Janeiro e de São Paulo, foram estudados atletas, 30 homens e 37 mulheres pertencentes às equipes juvenil (15-17 anos) e seniors (18-25 anos). Concomitantemente à avaliação antropométrica, foi feito o inquérito alimentar mediante os métodos de registro alimentar, recordatório de 24 horas e freqüência de consumo alimentar. Os resultados mostraram semelhança antropométrica entre as duas faixas etárias para cada um dos sexos, semelhança dos alimentos prevalentemente ingeridos com aqueles referidos pela população da área metropolitana das duas cidades e semelhança entre os suplementos alimentares ingeridos pelos atletas dos dois clubes. A ingestão calórica observada foi quase o dobro da referida para a população daquelas localidades, mas equivalente à descrita para nadadores de outros países. Os lanches, entre refeições, contribuíram com 25-28% da ingestão calórica global e a maior contribuição energética alimentar foi dada pelo grupo de cereais. Apesar de contribuir com quase 50% do aporte calórico diário, a ingestão glicídica foi menor que a recomendada para atletas competitivos (55-60%). A ingestão protéica de 2,5-3,0 g/kg/dia superou em 100% o valor recomendado para atletas e ultrapassou os 15% na participação calórica diária. A ingestão lipídica foi considerada elevada, particularmente a de gordura saturada. O aporte de micronutrientes foi acima do referido para as populações locais em função da ingestão calórica elevada, mas mostrou-se relativamente inadequado em magnésio, ferro e vitaminas A e D. Conclui-se que o padrão alimentar dos nadadores estudados não difere, muito, do da população local, distinguindo-se pelo elevado e freqüente consumo de alimentos energéticos, maior no sexo masculino que no feminino.

Plants from Iryanthera genus have been traditionally used as food supplements by South American Indians. The MeOH extract of leaves of Iryanthera juruensis, one of the plants endemic to the Amazon region and consumed in Brazil, and the hexane extract from its seeds inhibited lipid peroxidation (LPO) and cyclooxygenase (COX-1 and -2)) enzymes in in vitro assays. Further analyses of these extracts yielded 5-deoxyflavones (1-5) from the leaf extract and sargachromenol (6), sargaquinoic acid (7), a novel juruenolic acid (8), omega-arylalkanoic acids (9a-c), and the lignan guaiacin (10) from the seed extract. Compounds 3-5 inhibited LPO by 86%, 77%, and 88% at 10 ppm, respectively, and compounds 6 and 9a-c showed inhibition at 76% and 78% at 100 ppm, respectively. However, compounds 7 and 8 were inactive and lignan 10 exhibited LPO inhibitory activity by 99% at 100 ppm compared to commercial antioxidants butylated hydroxyanisole (BHA), butylated hydroxytoluene (BHT), and vitamin E. The flavones 1-5 also inhibited COX-1 and -2 enzymes by 50-65% at 100 ppm. Compound 6 showed high but nonselective inhibition of COX-1 and COX-2 enzymes, when compared to aspirin and Celebrex, a nonsteroidal anti-inflammatory drug (NSAID). Compounds 7 and 10 inhibited COX-1 by 60% and 65% and COX-2 by 37% and 18%, respectively, whereas compounds 8 and 9a-c showed little or no activity against these enzymes.
