A Qualidade de Vida (QV) em pacientes com câncer de cabeça e pescoço é afetada por fatores relacionados à doença e seu tratamento. Este estudo teve por objetivo avaliar a QV em pacientes com câncer de localização na cabeça e no pescoço, relacionado-a as condições de saúde bucal. Foram examinados 30 pacientes com diagnóstico de câncer de cabeça e pescoço. Foram pesquisadas variáveis socioeconômicas, hábitos e aspectos associados ao tumor, além de variáveis relacionadas à saúde bucal — CPOD, Uso e Necessidades Protéticas, Xerostomia e Consulta ao Dentista nos últimos seis meses, Mucosite e Mutilação. O questionário utilizado para avaliação da QV foi o EORTC QLQ-C30, e o módulo específico para câncer de cabeça e pescoço H&N35. A QV, no primeiro momento do estudo, foi considerada mediana, o nível funcional moderado e baixa sintomatologia. Não houve correlação estatisticamente significativa entre os componentes perdido e cariado do CPOD com a QV. O grupo de pacientes que necessitava de próteses relatou Desempenho Funcional e Função Social menores do que aqueles que não necessitavam. A Fadiga, a Dor e a Insônia foram maiores para o primeiro grupo. O grupo de pacientes com xerostomia demonstrou menor QV, menor nível funcional e maior sintomatologia quando comparado com aqueles sem queixa de xerostomia. O grupo dos fumantes apresentou maior sintomatologia e pior nível funcional do que os não-fumantes. Não foram encontradas diferenças significativas de QV entre os indivíduos que ingeriam ou não chimarrão. A QV dos pacientes no acompanhamento foi semelhante à primeira parte do estudo, porém houve aumento dos sintomas e diminuição do nível funcional. O CPOD aumentou, e o uso de próteses decresceu, enquanto a necessidade das mesmas aumentou Não foi possível verificar a associação entre a QV e a Mucosite, pois poucos pacientes foram afetados por esta condição. A Mutilação foi correlacionada negativamente com o Desempenho Funcional, Náusea e Vômito e Perda de Apetite. Apenas a avaliação clínica das variáveis de saúde bucal foi insuficiente para predizer a correlação entre a condição de saúde bucal e a QV, porém é essencial a atuação de uma equipe multidisciplinar de cuidado oncológico, na qual o cirurgião-dentista deve estar inserido de modo a minimizar os danos à saúde bucal desses pacientes, auxiliando na construção de uma melhor QV.

Objectives: To investigate the occurrence and severity of lymphoedema of the lower extremities (LLE), quality of life (QoL), and urinary and sexual dysfunction in women with vulvar cancer submitted to surgical treatment.Study design: Twenty-eight patients with vulvar cancer submitted to vulvectomy and inguinofemoral lymphadenectomy and 28 healthy, age-matched women (control group) were evaluated. The occurrence and severity of LLE were determined by Miller's Clinical Evaluation. QoL, urinary function and sexual function were assessed by the EORTC QLQ-C30, SF-ICIQ and FSFI questionnaires, respectively. The differences between groups and correlations were assessed using Student's r-test, Chi-squared test, Mann-Whitney U-test and Spearman's rho test.Results: The groups were similar in terms of marital status, educational status, menopausal status, hormone therapy and height. The occurrence and severity of LLE were higher in women with vulvar cancer compared with the control group (p < 0.001 and p = 0.003, respectively). A significant association was found between the severity of LLE and advanced age (p = 0.04), and the severity of LLE and higher body mass index (BMI; p = 0.04) in patients with vulvar cancer. In the patients with vulvar cancer, there was a significant correlation between the severity of LLE and worse QoL in the following domains: physical, cognitive, emotional, social, fatigue, pain, sleep and financial questions (p < 0.05). There was no difference in urinary function between the two groups (p = 0.113). Age and number of deliveries were the only variables associated with the occurrence of urinary incontinence (p = 0.01). Urinary incontinence was present in women with a mean age of 74.9 +/- 4.6 years and a mean of 7.3 +/- 1.3 normal deliveries. There was no difference between the groups in terms of the sexual function. Multivariate analysis showed an association between sexual function and age (p = 0.01), and sexual function and being in a stable relationship (p = 0.02).Conclusion: Patients submitted to vulvectomy or inguinofemoral lymphadenectomy for vulvar cancer are at higher risk of developing LLE compared with healthy, age-matched women. This has a negative effect on QoL, but does not interfere with urinary or sexual function. (C) 2012 Elsevier B.V. All rights reserved.

Aim: To use published literature and experts' opinion to investigate the clinical meaning and magnitude of changes in the Quality of Life (QOL) of groups of patients measured with the European Organisation for the Research and Treatment of Cancer Quality of Life Questionnaire Core 30 (EORTC QLQ-C30). Methods: An innovative method combining systematic review of published studies, expert opinions and meta-analysis was used to estimate large, medium, and small mean changes over time for QLQ-C30 scores. Results: Nine hundred and eleven papers were identified, leading to 118 relevant papers. One thousand two hundred and thirty two mean changes in QOL over time were combined in the meta-analysis, with timescales ranging from four days to five years. Guidelines were produced for trivial, small, and medium size classes, for each subscale and for improving and declining scores separately. Estimates for improvements were smaller than respective estimates for declines. Conclusions: These guidelines can be used to aid sample size calculations and interpretation of mean changes over time from groups of patients. Observed mean changes in the QLQ-C30 scores are generally small in most clinical situations, possibly due to response shift. Careful consideration is needed when planning studies where QOL changes over time are of primary interest; the timing of follow up, sample attrition, direction of QOL changes, and subscales of primary interest are key considerations. Durch die ansteigende Inzidenz und niedrige Mortalität steigt die Anzahl der überlebenden Männer nach Prostatakarzinom. Mit einer 5-Jahresprävalenz von 279.000 Männern stellte das Prostatakarzinom im Jahr 2010 den größten Anteil der Krebspatienten. Die absolute 5-Jahres-Überlebensrate liegt bei 78 %. Studien zur Lebensqualität dieser Langzeitüberlebenden (> 5 Jahre nach Diagnosestellung) beschränken sich meist auf bestimmte Therapien, schließen höhere Tumorstadien aus oder untersuchen nur die Wirkung von klinischen Einflussfaktoren. In Schleswig-Holstein wurde im Rahmen der populationsbezogenen OVIS- und CAESAR-Studie die Lebensqualität bei Männern mit bzw. nach Prostatakrebs zu drei Zeitpunkten erhoben (15 Monate, 3 ½ und 7 Jahre nach initialer Diagnose). Für die allgemeine krebsspezifische Lebensqualität (EORTC QLQ-C30) erfolgt eine Beschreibung des Verlaufs sowie ein Vergleich mit Referenzdaten aus der deutschen Allgemeinbevölkerung. Aus der dritten Befragung liegen auch Daten zur prostataspezifischen Lebensqualität (EORTC QLQ-PR25) vor. Mittels multipler linearer Regressionen werden für elf ausgewählte Lebensqualitätsskalen (mögliche Werte 0 bis 100) potenzielle Einflussfaktoren (klinisch, soziodemographisch, Lifestyle) untersucht. Die Lebensqualität der 911 Männer (medianes Alter bei Drittbefragung: 72 Jahre) nimmt im zeitlichen Verlauf nur gering, aber nicht klinisch relevant ab. Es zeigen sich nur geringe Unterschiede zur Lebensqualität der Referenzbevölkerung. Im absoluten Vergleich aller Skalen werden zum Zeitpunkt der Drittbefragung auf den prostataspezifischen Skalen die größten Einschränkungen berichtet. In den berechneten multiplen Regressionen war sieben Jahre nach Diagnose eine Krankheitsprogression auf allen untersuchten Skalen signifikant mit einer geringeren Lebensqualität assoziiert (niedrigster Regressionskoeffizient βadj -13,8, 95 %-CI -18,8; -8,8). Eine Strahlentherapie zeigte auf zehn, eine Hormontherapie auf fünf Skalen einen negativen Einfluss. Ebenfalls auf fünf Skalen war ein höherer Body-Mass-Index ein Prädiktor für eine geringere Lebensqualität. Auf allen Funktionsskalen war ein höherer Sozialstatus mit einer besseren Lebensqualität assoziiert und zeigte tendenziell einen größeren Einfluss als die initiale Therapie. Alleinstehende Männer berichteten eine geringere sexuelle Aktivität (βadj -7,5, 95 %-CI -13,8; -1,2) als Männer in einer Partnerschaft. Neben klinischen Faktoren beeinflussen auch soziodemographische Variablen die Lebensqualität von langzeitüberlebenden Männern nach bzw. mit Prostatakarzinom signifikant. Daher sollten in nicht-randomisierten Studien zum Adjustieren die entsprechenden Variablen (wie z. B. Body-Mass-Index, Sozialstatus, Partnerschaft) mit erhoben werden. Klinisch relevante Veränderungen der allgemeinen krebsspezifischen Lebensqualität finden – wenn überhaupt – innerhalb der ersten 15 Monate nach Diagnosestellung statt. Referenzdaten für die prostataspezifische Lebensqualität der Allgemeinbevölkerung liegen nicht vor. Eine Erhebung dieser scheint sinnvoll, da hier größere Unterschiede im Vergleich beider Gruppen erwartet werden.

BACKGROUND: There is an increasing demand for comprehensive forms of palliative cancer care, meeting physical as well as emotional, cognitive, spiritual and social needs. Therapy programs of anthroposophic hospitals are aimed at improving health and quality of life (QoL) at these levels. However, data on the influence of these programs on QoL of patients with advanced cancer are scarce. PATIENTS AND METHODS: 144 in-patients with advanced epithelial cancers were treated at the anthroposophic Lukas Klinik, Arlesheim, Switzerland. QoL was assessed upon admission, discharge and after 4 months, using 20 functional scales from the questionnaires EORTC QLQ-C30, HADS and SELT-M. Statistical testing was performed with the Wilcoxon signed rank test. At month 4, subjectively perceived benefits from anthroposophic medicine (AM) and conventional cancer therapy (CCT) were assessed by telephone. OBJECTIVE: The aim was to provide an account of global, physical, emotional, cognitive-spiritual and social QoL developments in advanced cancer patients, during and after in-patient AM treatment, and to investigate subjective benefits from AM and CCT. RESULTS: QoL improvements were observed in all 20 dimensions (12 significant). Compared to related studies, improvements were fairly high. At month 4, QoL scores had decreased but were still above baseline in all 20 dimensions. Both AM and CCT were perceived as beneficial. CONCLUSION: Our data provide evidence that in-patient therapy at an anthroposophic hospital can lead to significant QoL improvements, especially in emotional, but also global, physical, cognitive-spiritual and social aspects. Benefits of AM were experienced on the physical, emotional, cognitive- spiritual and social level. Benefits of CCT were tumor-focused.

Background Patients' health related quality of life (HRQoL) has rarely been systematically monitored in general practice. Electronic tools and practice training might facilitate the routine application of HRQoL questionnaires. Thorough piloting of innovative procedures is strongly recommended before the conduction of large-scale studies. Therefore, we aimed to assess i) the feasibility and acceptance of HRQoL assessment using tablet computers in general practice, ii) the perceived practical utility of HRQoL results and iii) to identify possible barriers hindering wider application of this approach. Methods Two HRQoL questionnaires (St. George's Respiratory Questionnaire SGRQ and EORTC QLQ-C30) were electronically presented on portable tablet computers. Wireless network (WLAN) integration into practice computer systems of 14 German general practices with varying infrastructure allowed automatic data exchange and the generation of a printout or a PDF file. General practitioners (GPs) and practice assistants were trained in a 1-hour course, after which they could invite patients with chronic diseases to fill in the electronic questionnaire during their waiting time. We surveyed patients, practice assistants and GPs regarding their acceptance of this tool in semi-structured telephone interviews. The number of assessments, HRQoL results and interview responses were analysed using quantitative and qualitative methods. Results Over the course of 1 year, 523 patients filled in the electronic questionnaires (1–5 times; 664 total assessments). On average, results showed specific HRQoL impairments, e.g. with respect to fatigue, pain and sleep disturbances. The number of electronic assessments varied substantially between practices. A total of 280 patients, 27 practice assistants and 17 GPs participated in the telephone interviews. Almost all GPs (16/17 = 94%; 95% CI = 73–99%), most practice assistants (19/27 = 70%; 95% CI = 50–86%) and the majority of patients (240/280 = 86%; 95% CI = 82–91%) indicated that they would welcome the use of electronic HRQoL questionnaires in the future. GPs mentioned availability of local health services (e.g. supportive, physiotherapy) (mean: 9.4 ± 1.0 SD; scale: 1 – 10), sufficient extra time (8.9 ± 1.5) and easy interpretation of HRQoL results (8.6 ± 1.6) as the most important prerequisites for their use. They believed HRQoL assessment facilitated both communication and follow up of patients' conditions. Practice assistants emphasised that this process demonstrated an extra commitment to patient centred care; patients viewed it as a tool, which contributed to the physicians' understanding of their personal condition and circumstances. Conclusion This pilot study indicates that electronic HRQoL assessment is technically feasible in general practices. It can provide clinically significant information, which can either be used in the consultation for routine care, or for research purposes. While GPs, practice assistants and patients were generally positive about the electronic procedure, several barriers (e.g. practices' lack of time and routine in HRQoL assessment) need to be overcome to enable broader application of electronic questionnaires in every day medical practice.

Long term quality of life data of adult patients harboring intracranial ependymomas have not been reported. The role of adjuvant radiation therapy in Grade II ependymomas is unclear and differs from study to study. We therefore sought to retrospectively analyze outcome and quality of life of adult patients that were operated on intracranial ependymomas at four different surgical centers in two countries. All patients were attempted to be contacted via telephone to assess quality of life (QoL) at the time of the telephone interview. The standard EORTC QoL Questionnaire C30 (EORTC QLQ-C30) and the EORTC QLQ-Brain Cancer Module (QLQ-BN20) were used. 64 adult patients with intracranial ependymomas were included in the study. The only factor that was associated with increased survival was age <55 years (p < 0.001). Supratentorial location was correlated with shorter progression free survival than infratentorial location (PFS; p = 0.048). In WHO Grade II tumors local irradiation did not lead to increased PFS (p = 0.888) or overall survival (p = 0.801). Even for incompletely resected Grade II tumors local irradiation did not lead to a benefit in PFS (p = 0.911). In a multivariate analysis of QoL, irradiated patients had significantly worse scores in the item "fatigue" (p = 0.037) than non-irradiated patients. Here we present QoL data of adult patients with intracranial ependymomas. Our data show that local radiation therapy may have long-term effects on patients' QoL. Since in the incompletely resected Grade II tumors local irradiation did not lead to a benefit in PFS in this retrospective study, prospective randomized studies are necessary. In addition to age, supratentorial tumor location is associated with a worse prognosis in adult ependymoma patients.

As relações sociais e de apoio constituem um fator importante para melhorar a qualidade de vida dos portadores de doenças crônico-degenerativas. Neste contexto, os grupos multidisciplinares de suporte representam uma prática de atenção à saúde que evidenciam um potencial terapêutico e de aprendizagem para os pacientes, familiares e profissionais de saúde. A finalidade deste estudo foi investigar se o grupo de suporte alterou a qualidade de vida 31 de pacientes portadores de câncer. Trata-se de um estudo descritivo-exploratório com abordagem quantitativa, no qual foi utilizado o questionário EORTC-QLQ-C30, específico para mensurar a qualidade de vida de pacientes oncológicos. O EORTC-QLQ-C30 foi utilizado em duas fases: antes da adesão do paciente ao grupo e após quatro meses de participação do paciente nas atividades grupais. A comparação dos resultados das duas fases mostrou aumento dos escores das Funções e da Medida Global de Saúde e Qualidade de Vida, diminuição dos escores das escalas de sintomas, além de alterações não significativas no item Dificuldades Financeiras. Conclui-se que após as atividades grupais, houve melhora da qualidade de vida dos participantes deste estudo, exceto no que se refere à questão financeira.(AU)
