Brazilian public policy entered in the so-called new social federalism through its conditional cash transfers. States and municipalities can operate together through the nationwide platform of the Bolsa Familia Program (BFP), complementing federal actions with local innovations. The state and the city of Rio de Janeiro have created programs named, respectively, Renda Melhor (RM) and Família Carioca (FC). These programs make use of the operational structure of the BFP, which facilitates locating beneficiaries, issuing cards, synchronizing payment dates and access passwords and introducing new conditionalities. The payment system of the two programs complements the estimated permanent household income up to the poverty line established, giving more to those who have less. Similar income complementation system was subsequently adopted in the BFP and the Chilean Ingreso Ético Familiar, which also follow the principle of estimation of income used in the FC and in the RM. Instead of using the declared income, the value of the Rio cash transfers are set using the extensive collection of information obtained from the Single Registry of Social Programs (Cadastro Único): physical configuration of housing, access to public services, education and work conditions for all family members, presence of vulnerable groups, disabilities, pregnant or lactating women, children and benefits from other official transfers such as the BFP. With this multitude of assets and limitations, the permanent income of each individual is estimated. The basic benefit is defined by the poverty gap and priority is given to the poorest. These subnational programs use international benchmarks as a neutral ground between different government levels and mandates. Their poverty line is the highest of the first millennium goal of the United Nations (UN): US$ 2 per person per day adjusted for the cost of living. The other poverty line of the UN, US$ 1.25, was implicitly adopted as the national extreme poverty line in 2011. The exchange of methodologies between federal entities has happened both ways. The FC began with the 575,000 individuals living in the city of Rio de Janeiro who were on the payroll of the BFP. Its system of impact evaluation benefited from bi-monthly standardized examinations. In the educational conditionalities, the two programs reward students' progress, a potential advantage for those who most need to advance. The municipal program requires greater school attendance than that of the BFP and the presence of students’ parents at the bimonthly meetings held on Saturdays. Students must achieve a grade of 8 or improve at least 20% in each exam to receive a bi-monthly premium of R$50. In early childhood, priority is given to the poor children in the program Single Administrative Register (CadÚnico) to enroll in kindergarten, preschools and complementary activities. The state program reaches more than one million people with a payment system similar to the municipal one. Moreover, it innovates in that it transfers awards given to high school students to savings accounts. The prize increases and is paid to the student, who can withdraw up to 30% annually. The total can reach R$3,800 per low-income student. The State and the city rewarded already education professionals according to student performance, now completing the chain of demand incentives on poor students and their parents. Increased performance is higher among beneficiaries and the presence of their guardians at meetings is twice compared to non beneficiaries; The Houston program, also focuses on aligning the incentives to teachers, parents and students. In general, the plan is to explore strategic complementarities, where the whole is greater than the sum of its parts. The objective is to stimulate, through targets and incentives, synergies between social actors (teachers, parents, students), between areas (education, assistance, work) and different levels of government. The cited programs sum their efforts and divide labor so as to multiply interactions and make a difference in the lives of the poor.

Plagioscion magdalenae (pescada-curuca) é um sciaenídeo bentopelágico, de importância comercial, amplamente distribuído na bacia do rio Amazonas. O objetivo deste trabalho foi descrever a biologia reprodutiva dessa espécie na baía do Marajó, estuário Amazônico, Brasil. Neste estudo foi determinado o estádio do desenvolvimento gonadal, tamanho de primeira maturação gonadal (L50), proporção entre sexos, época e o tipo de desova. A coleta foi realizada bimestralmente no período de dezembro de 2005 a outubro de 2006. Foi examinado um total de 251 exemplares, variando entre 220 e 590 mm de comprimento total (CT). A relação peso-comprimento para fêmeas, machos e sexos agrupados foi altamente significativa, com alometria positiva. O L50 foi de 279 mm considerando sexos agrupados, 305 e 269 mm CT para fêmeas e machos respectivamente. A proporção entre sexos para o total de indivíduos foi favorável aos machos (2,02 machos: 1 fêmea). Macroscopicamente, as gônadas foram classificadas em imatura, em maturação, matura e desovada. Considerando-se as avaliações macro e microscópicas das gônadas, foi registrado um período prolongado de desova, principalmente entre agosto e fevereiro.

Apesar da sua localização perto de Belém, Pará, a Ilha de Cotijuba tem sido assolada por surtos de malária durante os últimos anos, principalmente nos meses de abril a maio. A ilha faz parte de um arquipélago situado às margens da baía do Marajó, a 29 km de Belém, constituindo uma parte insular dessa cidade, e possui uma área de cerca de 60 km² e uma costa com 20 km de praias, que corresponde a 66% da área total da ilha. Por causa da epidemia, tomou-se necessário conhecer as espécies de anofelinos vetores de malária na ilha. Nos anos de 2002 a 2004 foram realizadas coletas periódicas de larvas e adultos de mosquitos e os seus criatórios foram localizados e caracterizados. Os imaturos foram coletados com auxílio de conchas e em bandejas plásticas. Para os adultos utilizou-se o método de captura manual com isca humana. Duas coletas de adultos na floresta foram realizadas, uma no período seco e outra no período chuvoso, com duração de 24 horas ininterruptas. Seis coletas bimensais no peridomicílio foram feitas, com duração de 6 horas. Tanto na floresta como no peridomicilio foram registradas apenas duas espécies em atividade atacando o homem, Anopheles (Nyssorhynchus) aquasalis e Anopheles (Anopheles) intermedius. A. aquasalis foi mais freqüente no peridomicílio, enquanto A. intermedius teve maior freqüência na floresta. Verificou-se que o maior número de casos de malária na ilha ocorre dois meses depois do início das chuvas, no mês de maio. Na i, como um todo, existem quatro lagos que são os possíveis criatórios de anofelinos da ilha e o lago da Gabriela é o principal criatório da ilha de Cotijuba, sendo responslha de Cotijubaável por 42% dos casos de malária na ilha no ano de 2003. As condições climáticas, o comportamento dos residentes e a falta de recursos para o efetivo controle dos vetores da doença, entre outros fatores, favorecem a persistência de malária na Ilha de Cotijuba.

Purpose: Describe the initial clinical management and therapeutic monitoring dispensed for newborns diagnosed with congenital syphilis at birth and as of the high public hospitals in the Federal District, Brazil. Methods: Collected informations from the initial clinical management and continuity of care accorded to 81 newborns diagnosed with congenital syphilis reported to the National System of Diseases of Notification in 2008. Results: The hemogram occurred in 46.9%, and radiological long bone in 81.4%. Four children (4.9%) followed up according to the protocol of the Ministry of Health in relation to bi-monthly follow- -up until 12 months of age was 93.8% of cases ignored and found no record of complete follow-up at 18 months old of age. Conclusions: The initial clinical management and therapeutic follow-up of newborns with congenital syphilis in the Federal District is not carried out adequately and does not accordance with the guidelines set by the Ministry of Health.

Increased agricultural activity in watershed areas has been causing concern over contamination by herbicides in agricultural areas. The problem becomes more important when contamination can affect water for human consumption, as happens with water from the Poxim river, which supplies the city of Aracaju, capital of the State of Sergipe. The aim of this study was to evaluate the risk of contamination by herbicides to both surface and groundwater in the upper sub-basin of the Poxim River, and to detect the presence of the active ingredients Diuron and Ametrine up-river from the sugar-cane plantations. Risk analysis was carried out using criteria from the Environmental Protection Agency (EPA), the GUS index, and the GOSS method. It was observed that several active ingredients are at risk of leaching, demonstrating the importance of monitoring the river to control both the quality of water and the frequency and volume of herbicides used in the region. Based on the results, monitoring was carried out bi-monthly from July 2009 to July 2010 at two sampling points. Water samples were analyzed in the laboratory, where the presence of Diuron and Ametrine was noted. Water quality in the Sub-basin of the Rio Poxim is being influenced by the use of herbicides in the region. There was an increase in herbicide concentration in the surface water during the rainy season, possibly caused by soil runoff.

The purpose of this study was to determine the role of saliva-derived biomarkers and periodontal pathogens during periodontal disease progression (PDP). One hundred human participants were recruited into a 12-month investigation. They were seen bi-monthly for saliva and clinical measures and bi-annually for subtraction radiography, serum and plaque biofilm assessments. Saliva and serum were analyzed with protein arrays for 14 pro-inflammatory and bone turnover markers, while qPCR was used for detection of biofilm. A hierarchical clustering algorithm was used to group study participants based on clinical, microbiological, salivary/serum biomarkers, and PDP. Eighty-three individuals completed the six-month monitoring phase, with 39 [corrected] exhibiting PDP, while 44 [corrected] demonstrated stability. Participants assembled into three clusters based on periodontal pathogens, serum and salivary biomarkers. Cluster 1 members displayed high salivary biomarkers and biofilm; 71% [corrected] of these individuals were undergoing PDP. Cluster 2 members displayed low biofilm and biomarker levels; 76% [corrected] of these individuals were stable. Cluster 3 members were not discriminated by PDP status; however, cluster stratification followed groups 1 and 2 based on thresholds of salivary biomarkers and biofilm pathogens. The association of cluster membership to PDP was highly significant (p < 0.0007). AIM Assess the ability of a panel of gingival crevicular fluid (GCF) biomarkers as predictors of periodontal disease progression (PDP). MATERIALS AND METHODS In this study, 100 individuals participated in a 12-month longitudinal investigation and were categorized into four groups according to their periodontal status. GCF, clinical parameters and saliva were collected bi-monthly. Subgingival plaque and serum were collected bi-annually. For 6 months, no periodontal treatment was provided. At 6 months, patients received periodontal therapy and continued participation from 6 to 12 months. GCF samples were analysed by ELISA for MMP-8, MMP-9, Osteoprotegerin, C-reactive Protein and IL-1β. Differences in median levels of GCF biomarkers were compared between stable and progressing participants using Wilcoxon Rank Sum test (p = 0.05). Clustering algorithm was used to evaluate the ability of oral biomarkers to classify patients as either stable or progressing. RESULTS Eighty-three individuals completed the 6-month monitoring phase. With the exception of GCF C-reactive protein, all biomarkers were significantly higher in the PDP group compared to stable patients. Clustering analysis showed highest sensitivity levels when biofilm pathogens and GCF biomarkers were combined with clinical measures, 74% (95% CI = 61, 86). CONCLUSIONS Signature of GCF fluid-derived biomarkers combined with pathogens and clinical measures provides a sensitive measure for discrimination of PDP (ClinicalTrials.gov NCT00277745).
