Although simian/human immunodeficiency virus (SHIV) strain DH12 replicates to high titers and causes immunodeficiency in pig-tailed macaques, virus loads measured in SHIVDH12-infected rhesus monkeys are consistently 100-fold lower and none of 22 inoculated animals have developed disease. We previously reported that the administration of anti-human CD8 mAb to rhesus macaques at the time of primary SHIVDH12 infection resulted in marked elevations of virus loads. One of the treated animals experienced rapid and profound depletions of circulating CD4+ T lymphocytes. Although the CD4+ T cell number partially recovered, this monkey subsequently suffered significant weight loss and was euthanized. A tissue culture virus stock derived from this animal, designated SHIVDH12R, induced marked and rapid CD4+ cell loss after i.v. inoculation of rhesus monkeys. Retrospective analyses of clinical specimens, collected during the emergence of SHIVDH12R indicated: (i) the input cloned SHIV remained the predominant virus during the first 5–7 months of infection; (ii) variants bearing only a few of the SHIVDH12R consensus changes first appeared 7 months after the administration of anti-CD8 mAb; (iii) high titers of neutralizing antibody directed against the input SHIV were detected by week 10 and persisted throughout the infection; and (iv) no neutralizing antibody against SHIVDH12R ever developed.

To identify cellular functions involved in the early phase of the retroviral life cycle, somatic cell mutants were isolated after selection for resistance to infection. Rat2 fibroblasts were treated with chemical mutagens, and individual virus-resistant clones were recovered after selection for resistance to infection. Two clones were characterized in detail. Both mutant lines were resistant to infection by both ecotropic and amphotropic murine viruses, as well as by human immunodeficiency virus type 1 pseudotypes. One clone showed a strong block to reverse transcription of the retroviral RNA, including formation of the earliest DNA products. The second clone showed normal levels of viral DNA synthesis but did not allow formation of the circular DNAs normally found in the nucleus. Cell fractionation showed that the viral preintegration complex was present in a form that could not be extracted under conditions that readily extracted the complex from wild-type cells. The results suggest that the DNA was trapped in a nonproductive state and excluded from the nucleus of the infected cell. The properties of these two mutant lines suggest that host gene products play important roles both before and after reverse transcription.

Binding and signaling proteins based on Escherichia coli alkaline phosphatase (AP; EC 3.1.3.1) were designed for the detection of antibodies. Hybrid proteins were constructed by using wild-type AP and point mutants of AP [Asp-101 --> Ser (D101S) and Asp-153 --> Gly (D153G)]. The binding function of the hybrid proteins is provided by a peptide epitope inserted between amino acids 407 and 408 in AP. Binding of anti-epitope antibodies to the hybrid proteins modulates the enzyme activity of the hybrids; upon antibody binding, enzyme activity can increase to as much as 300% of the level of activity in the absence of antibody or can decrease as much as 40%, depending on the presence or absence of the point mutations in AP. The fact that modulation is altered from inhibition to activation by single amino acid changes in the active site of AP suggests that the mechanism for modulation is due to structural alterations upon antibody binding. Modulation is a general phenomenon. The properties of the system are demonstrated by using two epitopes, one from the V3 loop of human immunodeficiency virus type 1 gp120 protein and one from hepatitis C virus core protein, and corresponding monoclonal antibodies. The trend of modulation is consistent for all hybrids; those in wild-type AP are inhibited by antibody, while those in the AP mutants are activated by antibody. This demonstrates that modulation of enzyme activity of the AP-epitope hybrid proteins is not specific to either a particular epitope sequence or a particular antibody-epitope combination.

Polyamide ("peptide") nucleic acids (PNAs) are molecules with antigene and antisense effects that may prove to be effective neuropharmaceuticals if these molecules are enabled to undergo transport through the brain capillary endothelial wall, which makes up the blood-brain barrier in vivo. The model PNA used in the present studies is an 18-mer that is antisense to the rev gene of human immunodeficiency virus type 1 and is biotinylated at the amino terminus and iodinated at a tyrosine residue near the carboxyl terminus. The biotinylated PNA was linked to a conjugate of streptavidin (SA) and the OX26 murine monoclonal antibody to the rat transferrin receptor. The blood-brain barrier is endowed with high transferrin receptor concentrations, enabling the OX26-SA conjugate to deliver the biotinylated PNA to the brain. Although the brain uptake of the free PNA was negligible following intravenous administration, the brain uptake of the PNA was increased at least 28-fold when the PNA was bound to the OX26-SA vector. The brain uptake of the PNA bound to the OX26-SA vector was 0.1% of the injected dose per gram of brain at 60 min after an intravenous injection, approximating the brain uptake of intravenously injected morphine. The PNA bound to the OX26-SA vector retained the ability to bind to synthetic rev mRNA as shown by RNase protection assays. In summary, the present studies show that while the transport of PNAs across the blood-brain barrier is negligible, delivery of these potential neuropharmaceutical drugs to the brain may be achieved by coupling them to vector-mediated peptide-drug delivery systems.

The acyclic nucleoside phosphonate analog 9-(2-phosphonylmethoxyethyl)adenine (PMEA) was recently found to be effective as an inhibitor of visna virus replication and cytopathic effect in sheep choroid plexus cultures. To study whether PMEA also affects visna virus infection in sheep, two groups of four lambs each were inoculated intracerebrally with 10(6.3) TCID50 of visna virus strain KV1772 and treated subcutaneously three times a week with PMEA at 10 and 25 mg/kg, respectively. The treatment was begun on the day of virus inoculation and continued for 6 weeks. A group of four lambs were infected in the same way but were not treated. The lambs were bled weekly or biweekly and the leukocytes were tested for virus. At 7 weeks after infection, the animals were sacrificed, and cerebrospinal fluid (CSF) and samples of tissue from various areas of the brain and from lungs, spleen, and lymph nodes were collected for isolation of virus and for histopathologic examination. The PMEA treatment had a striking effect on visna virus infection, which was similar for both doses of the drug. Thus, the frequency of virus isolations was much lower in PMEA-treated than in untreated lambs. The difference was particularly pronounced in the blood, CSF, and brain tissue. Furthermore, CSF cell counts were much lower and inflammatory lesions in the brain were much less severe in the treated lambs than in the untreated controls. The results indicate that PMEA inhibits the propagation and spread of visna virus in infected lambs and prevents brain lesions, at least during early infection. The drug caused no noticeable side effects during the 6 weeks of treatment.

A class of potent nonpeptidic inhibitors of human immunodeficiency virus protease has been designed by using the three-dimensional structure of the enzyme as a guide. By employing iterative protein cocrystal structure analysis, design, and synthesis the binding affinity of the lead compound was incrementally improved by over four orders of magnitude. An inversion in inhibitor binding mode was observed crystallographically, providing information critical for subsequent design and highlighting the utility of structural feedback in inhibitor optimization. These inhibitors are selective for the viral protease enzyme, possess good antiviral activity, and are orally available in three species.

Les chimiokines et leurs récepteurs respectifs jouent un rôle important dans l’immunité innée et adaptative. Les récepteurs de chimiokines identifient des cellules T CD4+ avec potentiel de migration dans des tissus spécifiques et à fonctionnalité distincte du point de vue de la spécificité antigénique et de la production de cytokines. L’identité de la population des cellules T CD4+ susceptibles versus résistantes à l’infection par le virus de l’immunodéficience humaine (VIH) reste mal définie. Le recrutement dans les muqueuses intestinales d’un excès de cellules T effectrices (CD8+) comparé aux cellules cibles (CD4+) représente un bon pronostic de l’infection par le virus de l’immunodéficience simienne (VIS), tandis que la déplétion des cellules Th17 dans les tissus lymphoïdes associés au tractus gastro-intestinal (GALT) est un marqueur de la progression de l’infection à VIH. L’effet régulateur des chimiokines sur l’activation de la réplication virale dans différentes sous-populations cellulaires T CD4+ reste peu étudié. Ce projet de maîtrise est divisé en 3 parties: (1) l’identification des récepteurs de chimiokines CCR4, CXCR3 et CCR6 comme marqueurs de surfaces des sous populations T CD4+ avec susceptibilité distincte à l’infection par le VIH; (2) la caractérisation phénotypique et fonctionnelle des cellules T CD4+ et T CD8+ spécifiques au VIH de sujets à progression lente vers le stade sida (LTNP); et (3) les effets des chimiokines ligands de CCR4, CXCR3 et CCR6 sur l’activation cellulaire et la réplication virale in vitro. Nos résultats démontrent que les cellules T CD4+ CCR4+CCR6+ (profile cytokinique Th17) et CXCR3+CCR6+ (profile cytokinique Th1/Th17) sont hautement permissives à l’infection par le VIH. Nous proposons également de nouveaux corrélats de protection immunitaire contre le VIH chez les sujets LTNP: (i) le potentiel de co-localisation dans les muqueuses intestinales des cellules T CD4+ et CD8+ spécifiques au VIH via l’intégrine β7, (ii) le ratio élevé entre les cellules T effectrices (CD8+) versus les cellules cibles (CD4+) spécifiques au VIH, (iii) le profil cytokinique Th17 et (iv) la capacité des cellules T CD4+ et CD8+ spécifiques au VIH à produire des ligands de CCR5 bloquant l’entrée virale. Finalement, nos résultats sur l’effet co-stimulateur des chimiokines sur les cellules T et leurs effets opposés sur la réplication virale démontrent l’implication du réseau des chimiokines dans la régulation de la pathogenèse de l’infection à VIH.

Introduction. Le VIH-1 évolue en fonction de la réponse immunitaire spécifique de l’hôte. La pression sélective exercée par la réponse immunitaire VIH-spécifique de l’hôte entraine l’évolution des gènes viraux et à terme détermine l’évolution de la maladie. Cette évolution du virus à l’échelle d’un individu façonne également l’évolution du virus à l’échelle de la population et détermine le devenir de l’épidémie. Le VIH utilise les corécepteurs d’entrée CCR5 (virus R5) et CXCR4 (virus X4) afin d’infecter la cellule cible, et l’évolution du tropisme du virus de R5 vers X4, appelé switch du tropisme, est associé à la progression de la maladie. Les virus R5 sont rencontrés en début d’infection tandis que les virus X4 apparaissent en fin de maladie chez un certain de nombre de patients et sont considérés comme plus virulents. La pression sélective immunitaire exercée sur le gène de l’enveloppe (env) peut donc entrainer l’évolution du tropisme du VIH. La grossesse est un état immunitaire particulier considéré comme étant principalement caractérisé par un biais Th2 nécessaire à l’établissement de la tolérance materno-fétale. Le switch de tropisme de R5 vers X4 en grossesse n’a jamais été documenté, de même que l’évolution des déterminants du tropisme à l’échelle de la population. Hypothèses. Les changements immunitaires associés à l’initiation et la progression de la grossesse engendrent des changements dans la pression immunitaire exercée sur l’enveloppe et peuvent favoriser le switch du tropisme. L’évolution du tropisme du VIH-1 peut être observé à l’échelle de la population au même titre que l’évolution de l’enveloppe virale. Objectifs. Analyser l’évolution du tropisme et décrire la pression sélective sur l’enveloppe des femmes enceintes infectées par le VIH-1. Analyser l’évolution des déterminants du tropisme à l’échelle de la population. Méthodes. Nous avons dans un premier temps analysé l’évolution des déterminants du tropisme et déterminé le génotype et phénotype du VIH-1 chez 19 femmes enceintes issues de la cohorte du centre maternel et infantile sur le SIDA de l’hôpital Sainte-Justine (CMIS). Nous avons ensuite caractérisé et comparé la pression sélective exercée sur env, par une méthode bayésienne, chez 31 femmes enceinte et 29 femmes non-enceintes. Enfin, nous avons analysé et comparé des déterminants du tropisme entre des séquences d’enveloppe contemporaines et anciennes, issues des bases de données du NCBI. Résultats. Nos résultats montrent la présence de virus X4 chez la moitié de notre cohorte, et un switch de tropisme de R5 vers X4 chez 5/19 sujets. Les séquences des femmes enceintes présentaient des taux de substitutions plus élevées que celles des femmes non-enceintes. La pression sélective dans la région C2 était plus faible chez les femmes enceintes que chez les femmes non-enceintes, et différait dans 4 positions entre ces 2 groupes. Cette sélection diminuait au cours de la grossesse chez les patientes traitées. Enfin, une accumulation de mutations X4 a été observée dans les séquences R5 contemporaines par rapport aux séquences R5 anciennes. Conclusion. Les changements immunitaires associés à la grossesse semblent induire des modifications subtiles dans la pression sélective exercée sur env, suffisant à influencer l’évolution du tropisme de R5 vers X4. Un switch du tropisme à l’échelle de la population impliquerait une épidémie évoluant vers une plus grande virulence du virus. Nos résultats sont d’importance en ce qui concerne la prophylaxie antirétrovirale pour la santé de la mère et la prévention de la transmission mère-enfant du VIH-1. Ils sont aussi importants concernant l’avenir de la thérapie antirétrovirale dans le contexte d’une épidémie évoluant vers une plus grande virulence

We report for the first time the genetic and biological characterization of 10 HIV-1 primary isolates representing CRF28_BF and CRF29_BF together with additional unique BF recombinant forms (URFs) obtained by PBMC cocultivation. Recombination is an important factor promoting the increase in the genetic diversity of HIV-1. Notably, more than 20% of HIV-1 sequences worldwide were recombinants. Several recombinant viruses were reported in Brazil, and six circulating recombinant forms (CRFs) have been identified (CRF28_BF, CRF29_BF, CRF31_BC, CRF39_BF, CRF40_BF, and CRF46_BF). CRF28_BF and CRF29_BF were found to infect almost 30% of the patients in Sao Paulo State. The near full-length genomes of these 10 primary isolates were amplified by nested PCR in three overlapping segments, purified, and sequenced. Three samples were related to CRF28_BF, three to CRF29_BF, and four were unique recombinant forms (URFs), as determined by their breakpoint profile determined with the jpHMM program. Additionally, the coreceptor usage of these isolates was investigated in vitro using GHOST assays, which revealed three dual-tropic (X4/R5) viruses, four lymphotropic (X4) viruses, and three macrophage-tropic (R5) viruses with different V3-loop motifs, which challenges the notion that GWGR-carrying viruses are macrophage-tropic only. In sum, we report a much-anticipated well-characterized panel of viruses representing CRF28_BF, CRF29_BF, and URFs from Sao Paulo State, Brazil.

Most of the antiretroviral (ARV) studies in Brazil have been reported in treatment-experienced and naive patients rather than in the setting of treatment interruption (TI). In this study, we analysed reasons given for TI and resistance mutations occurring in 150 HIV-1-infected patients who underwent TI. Of the patients analysed, 110 (73.3%) experienced TI following medical advice, while the remaining patients stopped antiretroviral therapy (ART) of their own accord. The main justifications for TI were: ARV-related toxicities (38.7%), good laboratory parameters (30%) and poor adherence (20%). DNA sequencing of the partial pol gene was successful in 137 (91.3%) patients, of whom 38 (27.7%) presented mutations conferring ARV resistance. A higher viral load prior to TI correlated with drug resistance (P < 0.05). Our results demonstrate that there are diverse rationales for TI and that detection of resistant strains during TI most likely indicates a fitter virus than the wild type. High viral loads coupled with unprotected sex in this group could increase the likelihood of transmission of drug-resistant virus. Thus, treating physicians should be alerted to this problem when the use of ARVs is interrupted.

Objective: To evaluate cases of mother-to-child transmission of HIV-1 at multiple sites in Latin America and the Caribbean in terms of missed opportunities for prevention. Methods: Pregnant women infected with HIV-1 were eligible for inclusion if they were enrolled in either the NISDI Perinatal or LILAC protocols by October 20, 2009, and had delivered a live infant with known HIV-1 infection status after March 1, 2006. Results: Of 711 eligible mothers, 10 delivered infants infected with HIV-1. The transmission rate was 1.4% (95% CI, 0.7-2.6). Timing of transmission was in utero or intrapartum (n = 5), intrapartum (n = 2), intrapartum or early postnatal (n = 1), and unknown (n = 2). Possible missed opportunities for prevention included poor control of maternal viral load during pregnancy; late initiation of antiretrovirals during pregnancy; lack of cesarean delivery before labor and before rupture of membranes; late diagnosis of HIV-1 infection; lack of intrapartum antiretrovirals; and incomplete avoidance of breastfeeding. Conclusion: Early knowledge of HIV-1 infection status (ideally before or in early pregnancy) would aid timely initiation of antiretroviral treatment and strategies designed to prevent mother-to-child transmission. Use of antiretrovirals must be appropriately monitored in terms of adherence and drug resistance. If feasible, breastfeeding should be completely avoided.
