The prevalence of obesity is increasing even in older patients. Bariatric surgery is often considered more risky in this group, and not necessarily associated with the same benefits as in younger patients. In France, guidelines recommend to assess indication for surgery based on comorbidities and physiological age.

The year 2014 was rich in significant advances in all areas of internal medicine. Many of them have an impact on our daily practice and on the way we manage one problem or another. From the use of the ultrasound for the diagnosis of pneumonia to the choice of the site of venous access and the type of line, and the increasing complexity of choosing an oral anticoagulant agent, this selection offers to the readers a brief overview of the major advances. The chief residents in the Service of internal medicine of the Lausanne University hospital are pleased to share their readings.

Many studies indicate a crucial role for the vitamin B12 and folate-dependent enzyme methionine synthase (MS) in brain development and function, but vitamin B12 status in the brain across the lifespan has not been previously investigated. Vitamin B12 (cobalamin, Cbl) exists in multiple forms, including methylcobalamin (MeCbl) and adenosylcobalamin (AdoCbl), serving as cofactors for MS and methylmalonylCoA mutase, respectively. We measured levels of five Cbl species in postmortem human frontal cortex of 43 control subjects, from 19 weeks of fetal development through 80 years of age, and 12 autistic and 9 schizophrenic subjects. Total Cbl was significantly lower in older control subjects (> 60 yrs of age), primarily reflecting a >10-fold age-dependent decline in the level of MeCbl. Levels of inactive cyanocobalamin (CNCbl) were remarkably higher in fetal brain samples. In both autistic and schizophrenic subjects MeCbl and AdoCbl levels were more than 3-fold lower than age-matched controls. In autistic subjects lower MeCbl was associated with decreased MS activity and elevated levels of its substrate homocysteine (HCY). Low levels of the antioxidant glutathione (GSH) have been linked to both autism and schizophrenia, and both total Cbl and MeCbl levels were decreased in glutamate-cysteine ligase modulatory subunit knockout (GCLM-KO) mice, which exhibit low GSH levels. Thus our findings reveal a previously unrecognized decrease in brain vitamin B12 status across the lifespan that may reflect an adaptation to increasing antioxidant demand, while accelerated deficits due to GSH deficiency may contribute to neurodevelopmental and neuropsychiatric disorders.

We systematically reviewed 25 randomised controlled trials of ultrasound-guided brachial plexus blockade that recruited 1948 participants: either one approach vs another (axillary, infraclavicular or supraclavicular); or one injection vs multiple injections. There were no differences in the rates of successful blockade with approach, relative risk (95% CI): axillary vs infraclavicular, 1.0 (1.0-1.1), p = 0.97; axillary vs supraclavicular, 1.0 (1.0-1.1), p = 0.68; and infraclavicular vs supraclavicular, 1.0 (1.0-1.1), p = 0.32. There was no difference in the rate of successful blockade with the number of injections, relative risk (95% CI) 1.0 (1.0-1.0), p = 0.69, for one vs multiple injections. The rate of procedural paraesthesia was less with one injection than multiple injections, relative risk (95% CI) 0.6 (0.4-0.9), p = 0.004.

INTRODUCTION: This article is part of a research study on the organization of primary health care (PHC) for mental health in two of Quebec's remote regions. It introduces a methodological approach based on information found in health records, for assessing the quality of PHC offered to people suffering from depression or anxiety disorders. METHODS: Quality indicators were identified from evidence and case studies were reconstructed using data collected in health records over a 2-year observation period. Data collection was developed using a three-step iterative process: (1) feasibility analysis, (2) development of a data collection tool, and (3) application of the data collection method. The adaptation of quality-of-care indicators to remote regions was appraised according to their relevance, measurability and construct validity in this context. RESULTS: As a result of this process, 18 quality indicators were shown to be relevant, measurable and valid for establishing a critical quality appraisal of four recommended dimensions of PHC clinical processes: recognition, assessment, treatment and follow-up. CONCLUSIONS: There is not only an interest in the use of health records to assess the quality of PHC for mental health in remote regions but also a scientific value for the rigorous and meticulous methodological approach developed in this study. From the perspective of stakeholders in the PHC system of care in remote areas, quality indicators are credible and provide potential for transferability to other contexts. This study brings information that has the potential to identify gaps in and implement solutions adapted to the context.

BACKGROUND: Changing Directions, Changing Lives, the Mental Health Strategy for Canada, prioritizes the development of coordinated continuums of care in mental health that will bridge the gap in services for Inuit populations. OBJECTIVE: In order to target ways of improving the services provided in these contexts to individuals in Nunavik with depression or anxiety disorders, this research examines delays and disruptions in the continuum of care and clinical, individual and organizational characteristics possibly associated with their occurrences. DESIGN: A total of 155 episodes of care involving a common mental disorder (CMD), incident or recurring, were documented using the clinical records of 79 frontline health and social services (FHSSs) users, aged 14 years and older, living in a community in Nunavik. Each episode of care was divided into 7 stages: (a) detection; (b) assessment; (c) intervention; (d) planning the first follow-up visit; (e) implementation of the first follow-up visit; (f) planning a second follow-up visit; (g) implementation of the second follow-up visit. Sequential analysis of these stages established delays for each one and helped identify when breaks occurred in the continuum of care. Logistic and linear regression analysis determined whether clinical, individual or organizational characteristics influenced the breaks and delays. RESULTS: More than half (62%) the episodes of care were interrupted before the second follow-up. These breaks mostly occurred when planning and completing the first follow-up visit. Episodes of care were more likely to end early when they involved anxiety disorders or symptoms, limited FHSS teams and individuals over 21 years of age. The median delay for the first follow-up visit (30 days) exceeded guideline recommendations significantly (1-2 weeks). CONCLUSION: Clinical primary care approaches for CMDs in Nunavik are currently more reactive than preventive. This suggests that recovery services for those affected are suboptimal.

BACKGROUND AND AIMS: Inflammatory bowel diseases (IBDs) may impair quality of life (QoL) in paediatric patients. We aimed to evaluate in a nationwide cohort whether patients experience QoL in a different way when compared with their parents. METHODS: Sociodemographic and psychosocial characteristics were prospectively acquired from paediatric patients and their parents included in the Swiss IBD Cohort Study. Disease activity was evaluated by the Paediatric Crohn's Disease Activity Index (PCDAI) and the Paediatric Ulcerative Colitis Activity Index (PUCAI). We assessed QoL using the KIDSCREEN questionnaire. The QoL domains were analysed and compared between children and parents according to type of disease, parents' age, origin, education and marital status. RESULTS: We included 110 children and parents (59 Crohn's disease [CD], 45 ulcerative colitis [UC], 6 IBD unclassified [IBDU]). There was no significant difference in QoL between CD and UC/IBDU, whether the disease was active or in remission. Parents perceived overall QoL, as well as 'mood', 'family' and 'friends' domains, lower than the children themselves, independently of their place of birth and education. However, better concordance was found on 'school performance' and 'physical activity' domains. Marital status and age of parents significantly influenced the evaluation of QoL. Mothers and fathers being married or cohabiting perceived significantly lower mood, family and friends domains than their children, whereas mothers living alone had a lower perception of the friends domain; fathers living alone had a lower perception of family and mood subscores. CONCLUSION: Parents of Swiss paediatric IBD patients significantly underestimate overall QoL and domains of QoL of their children independently of origin and education.

BACKGROUND: Since its first detection, characterization of R. felis has been a matter of debate, mostly due to the contamination of an initial R. felis culture by R. typhi. However, the first stable culture of R. felis allowed its precise phenotypic and genotypic characterization, and demonstrated that this species belonged to the spotted fever group rickettsiae. Later, its genome sequence revealed the presence of two forms of the same plasmid, physically confirmed by biological data. In a recent article, Gillespie et al. (PLoS One. 2007;2(3):e266.) used a bioinformatic approach to refute the presence of the second plasmid form, and proposed the creation of a specific phylogenetic group for R. felis. METHODOLOGY/PRINCIPAL FINDINGS: In the present report, we, and five independent international laboratories confirmed unambiguously by PCR the presence of two plasmid forms in R. felis strain URRWXCal(2) (T), but observed that the plasmid content of this species, from none to 2 plasmid forms, may depend on the culture passage history of the studied strain. We also demonstrated that R. felis does not cultivate in Vero cells at 37 degrees C but generates plaques at 30 degrees C. Finally, using a phylogenetic study based on 667 concatenated core genes, we demonstrated the position of R. felis within the spotted fever group. SIGNIFICANCE: We demonstrated that R. felis, which unambiguously belongs to the spotted fever group rickettsiae, may contain up to two plasmid forms but this plasmid content is unstable.

Realizou-se um estudo fenológico de Tapirira guianensis Aubl. no Campus da Universidade Federal de Sergipe, Município de São Cristóvão (10º55'32"S e 37º06'08"W), com o intuito de obter informações sobre o comportamento dessa espécie. Foram observados 21 indivíduos em intervalos quinzenais, durante 30 meses, sendo analisados seus parâmetros fenológicos reprodutivos e vegetativos, por meio da porcentagem de indivíduos e percentual de intensidade de Fournier. T. guianensis apresentou floração na transição da estação seca para a chuvosa (outubro-maio), produção de frutos na época seca (dezembro-junho) com maturação no início das chuvas (maio-julho) e brotamento e queda de folhas contínuos ao longo do ano. Observou-se padrão sazonal anual de floração e frutificação na espécie.

Espécies florestais com potencial econômico, como é o caso da aroeira (Schinus terebinthifolius Raddi.), requerem a seleção de indivíduos com características superiores para uso em projetos de restauração florestal e para o estabelecimento de plantios comerciais. Estas plantações podem contribuir para a sustentabilidade de populações naturais de espécies nos fragmentos florestais remanescentes, em áreas de preservação permanente, reservas legais ou outras áreas de importância ecológica. Foi avaliada a produção de frutos, morfometria e viabilidade de sementes de 15 indivíduos de aroeira, em diferentes fragmentos, na região do Baixo São Francisco sergipano. A produção de frutos foi estimada pelo índice de intensidade de Fournier e as características morfométricas foram obtidas com paquímetro e balança analítica digital. A viabilidade e o vigor foram avaliados por meio da porcentagem de germinação e índice de velocidade de germinação (IVG), sob condições controladas. Os resultados da produção de frutos foram submetidos à análise de variância e as médias comparadas pelo teste de Tukey (p <0,05). Para as demais variáveis foi utilizado o teste de Scott-Knott (p <0,05). Os indivíduos diferiram em índices de Fournier (índices 1, 2, 3 e 4) e no tamanho de frutos e sementes. A germinação variou de 0 a 83% e o IVG de 0,00 a 0,98. As diferenças fenotípicas observadas entre os indivíduos para a produção de frutos e características morfofisiológicas podem ser exploradas em restauração florestal e estabelecimento de testes de procedências/progênies, visando à discriminação de material superior para futuros plantios comerciais.

Avaliaram-se retrospectivamente as cirurgias realizadas em aves no Serviço de Cirurgia de Pequenos Animais do Hospital Veterinário da Faculdade de Medicina Veterinária e Zootecnia, Universidade de São Paulo, durante período de oito anos. De um total de 90 intervenções cirúrgicas para diagnóstico e/ou tratamento de afecções, 27 foram ortopédicas e 63 de tecidos moles. Quanto ao percentual de cirurgias ortopédicas realizadas segundo as diferentes ordens, observou-se: Psittaciformes 85,19%, Piciformes 7,41%, Anseriformes 3,70% e Falconiformes 3,70%. Para as de tecidos moles os Psittaciformes representaram 92,06%, Columbiformes 3,17%, Passeriformes 3,17% e Anseriformes 1,60%. Entre os tipos de afecções ortopédicas encontradas as fraturas apresentaram a maior ocorrência (88,90%), seguidas de luxação (3,70%), avulsão traumática de extremidade (3,70%) e artrite/osteomielite (3,70%). Dentre as afecções cirúrgicas de tecidos moles as neoplasias apresentaram a maior ocorrência (30,15%), seguidas das neoformações cutâneas ou de anexos não neoplásicos (17,46%), neoformações cutâneas sem diagnóstico (7,94%), distocia (7,94%), fístula de papo (7,94%), hérnia abdominal (4,76%), sinusite (4,76%), gangrena de extremidade de membros (3,17%), perfuração de esôfago (3,17%), prolapso de cloaca (3,17%), "Necrose avascular de dígito" (1,59%), ferida na região da quilha (1,59%), perfuração de cavidade celomática (1,59%), neoformação em cavidade celomática sem diagnóstico (1,59%), corpo estranho em trato gastrointestinal (1,59%) e otite (1,59%). A distribuição das afecções cirúrgicas segundo as espécies acometidas mostrou o "grupo dos papagaios", representado em sua maioria por espécies do gênero Amazona, como prevalente. O conhecimento das afecções cirúrgicas e espécies de aves mais acometidas acrescentam informações para aqueles que já atuam nesta área e servem como indicador de estudo para futuros cirurgiões de aves.
