Dr. Jules Cotard (1840-1889) was a Parisian neurologist who first described the delire des negations. Cotard's syndrome or Cotard's delusion comprises any one of a series of delusions ranging from the fixed and unshakable belief that one has lost organs, blood, or body parts to believing that one has lost one's soul or is dead. In its most profound form, the delusion takes the form of a professed belief that one does not exist. Encountered primarily in psychoses such as schizophrenia and bipolar disorder, Cotard's syndrome has also been described in organic lesions of the nondominant temporoparietal cortex as well as in migraine. Cotard's delusion is the only self-certifiable syndrome of delusional psychosis. Jules Cotard, a Parisian neurologist and psychiatrist and former military surgeon, was one of the first to induce cerebral atrophy by the experimental embolization of cerebral arteries in animals and a pioneer in studies of the clinicopathologic correlates of cerebral atrophy secondary to perinatal and postnatal pathologic changes. He was the first to record that unilateral cerebral atrophy in infancy does not necessarily lead to aphasia and was also the pioneer of studies of altered conscious states in diabetic hyperglycemia.

A detailed analysis procedure is described for evaluating rates of volumetric change in brain structures based on structural magnetic resonance (MR) images. In this procedure, a series of image processing tools have been employed to address the problems encountered in measuring rates of change based on structural MR images. These tools include an algorithm for intensity non-uniforniity correction, a robust algorithm for three-dimensional image registration with sub-voxel precision and an algorithm for brain tissue segmentation. However, a unique feature in the procedure is the use of a fractional volume model that has been developed to provide a quantitative measure for the partial volume effect. With this model, the fractional constituent tissue volumes are evaluated for voxels at the tissue boundary that manifest partial volume effect, thus allowing tissue boundaries be defined at a sub-voxel level and in an automated fashion. Validation studies are presented on key algorithms including segmentation and registration. An overall assessment of the method is provided through the evaluation of the rates of brain atrophy in a group of normal elderly subjects for which the rate of brain atrophy due to normal aging is predictably small. An application of the method is given in Part 11 where the rates of brain atrophy in various brain regions are studied in relation to normal aging and Alzheimer's disease. We present global and regional rates of brain atrophy measured on serially acquired T1-weighted brain MR images for a group of Alzheimer's disease (AD) patients and age-matched normal control (NC) subjects using the analysis procedure described in Part I. Three rates of brain atrophy: the rate of atrophy in the cerebrum, the rate of lateral ventricular enlargement and the rate of atrophy in the region of temporal lobes, were evaluated for 14 AD patients and 14 age-matched NC subjects. All three rates showed significant differences between the two groups, However, the greatest separation of the two groups was obtained when the regional rates were combined. This application has demonstrated that rates of brain atrophy, especially in specific regions of the brain, based on MR images can provide sensitive measures for evaluating the progression of AD. These measures will be useful for the evaluation of therapeutic effects of novel therapies for AD. (C) 2002 Elsevier Science Inc. All rights reserved.

Age-related macular degeneration (AMD) is the major cause of blindness in the developed world. its pathomechanism is unknown and its late onset, complex genetics and strong environmental components have all hampered investigations. Here we demonstrate the development of an animal model for AMD that reproduces features associated with geographic atrophy, a transgenic mouse line (mcd/mcd) expressing a mutated form of cathepsin D that is enzymatically inactive thus impairing processing of phagocytosed photoreceptor outer segments in the retinal pigment epithelial (RPE) cells. Pigmentary changes indicating RPE cell atrophy and a decreased response to flash electroretinograms were observed in 11- to 12-month-old mcd/mcd mice. Histological studies showed RPE cell proliferation, photoreceptor degeneration, shortening of photoreceptor outer segments, and accumulation of immunoreactive photoreceptor breakdown products in the RPE cells. An accelerated photoreceptor cell death was detected in 12-month-old mcd/mcd mice. Transmission electron microscopy demonstrated presence of basal laminar and linear deposits that are considered to be the hallmarks of AMD. Small hard drusen associated with human age-related maculopathy were absent in the mcd/mcd mouse model at the ages analyzed. in summary, this model presents several features of AMD, thus providing a valuable tool for investigating the underlying biological processes and pathomechanism of AMD.

In mammals, prolonged immobilization of the limbs can result in a loss of capillary tortuosity, resulting in skeletal muscle haemorrhaging if rapid remobilization is permitted. In this study, we examined the effect of 4 months' immobilization on semimembranosus capillary structure in the Green-striped burrowing frog, Cyclorana alboguttata. C alboguttata routinely aestivates as part of a physiological strategy to avoid desiccation in semi-arid environments and, in this capacity, the hindlimbs of C alboguttata are immobilized in a cocoon for months at a time. We found that 4 months' aestivation had no effect on three-dimensional capillary structure in the semimembranosus muscle and that capillary tortuosity is preserved in immobilized C. alboguttata. The preservation of capillary structure in the hindlimb muscles of C alboguttata in part accounts for their remarkable ability to emerge with a fully competent locomotor system after prolonged immobilization.

We detected and mapped a dynamically spreading wave of gray matter loss in the brains of patients with Alzheimer's disease (AD). The loss pattern was visualized in four dimensions as it spread over time from temporal and limbic cortices into frontal and occipital brain regions, sparing sensorimotor cortices. The shifting deficits were asymmetric (left hemisphere >right hemisphere) and correlated with progressively declining cognitive status ( p 15% loss). The maps distinguished different phases of AD and differentiated AD from normal aging. Local gray matter loss rates (5.3 +/- 2.3% per year in AD v 0.9 +/- 0.9% per year in controls) were faster in the left hemisphere ( p < 0.029) than the right. Transient barriers to disease progression appeared at limbic/frontal boundaries. This degenerative sequence, observed in vivo as it developed, provides the first quantitative, dynamic visualization of cortical atrophic rates in normal elderly populations and in those with dementia.

Diversas pesquisas apontam um contínuo desuso dos clíticos de 3ª pessoa (lhe, o, a, os, as e suas variantes) na modalidade oral do Português Brasileiro (PB), sendo substituídos pelo pronome reto, pelo objeto nulo ou pela repetição do sintagma a que eles fazem referência. Por outro lado, os clíticos de 3ª pessoa aparecem comumente em textos escritos veiculados pela mídia, bem como nos estilos mais formais da língua falada, o que faz com que esses elementos constem dos programas da disciplina Língua Portuguesa, nas escolas brasileiras. Dessa feita, esta pesquisa tem por objetivo descrever esse processo de aprendizagem do ponto de vista linguístico e social, procurando investigar como as crianças interpretam o uso dessas formas – e, por conseguinte, da variedade padrão da língua –, e se e/ou como a escola favorece a aprendizagem dessas formas. Para respondermos a essas perguntas, procedemos a uma pesquisa sociolinguística, que analisa os fatores linguísticos e sociais que poderiam influenciar essa aprendizagem. Para tanto, os clíticos de 3ª pessoa foram trabalhados gradualmente durante um ano letivo. Assim, o corpus desta pesquisa compõe-se de textos escritos livremente e de testes de compreensão – que visava verificar a compreensão dos clíticos presentes em textos infantis – e de desempenho – que consistia na substituição de expressões pelos clíticos adequados. Os dados foram colhidos em uma turma de 4º ano do Ensino Fundamental durante 10 meses letivos, numa escola pública de Belo Horizonte, Minas Gerais, cujos alunos pertencem a diferentes níveis socioeconômicos. Os resultados obtidos revelam que: i) o contexto em que o clítico é inserido contribui significativamente para a sua compreensão; ii) com relação aos fatores extralinguísticos, os alunos, independentemente de sua origem social e sexo/gênero, apresentam dificuldades em compreender os clíticos; entretanto, há uma leve tendência de um melhor aproveitamento quanto aos meninos da classe socioeconômica favorecida

A negligência é punível e não pode deixar de ser punível. É urgente a investigação, é urgente a acusação e o julgamento escorreito desse conjunto de energúmenos que, por puro dolo, ou puro atrofiamento mental, foram capazes de tais acções ou omissões. Caso não haja qualquer outra hipótese em termos legais internacionais, deverá ser criado – se for necessário, adequado e proporcional - um Tribunal had hoc internacional que julgue com especificidade este caso concreto. Basta de impunidade perante situações injustas como esta, onde tudo acaba por terminar no esquecimento. Abstract: Negligence is punishable and can only be punishable. There is an urgent investigation, it is urgent to prosecution and the trial this group of lunatics who, by pure malice, or pure mental atrophy, were capable of such acts or omissions. If there is no other choice in international law, should be created - if necessary, appropriate and proportionate - a Court had international hoc judges with this specific case. No more impunity unjust situations like this, where everything turns out to finish the wayside.

Este estudo refere-se a uma paciente de 33 anos com paralisia do nervo toráxico longo direito e consequente atrofia do grande dentado A paciente foi acompanhada durante 7 meses no serviço de Fisioterapia e tratada segundo as guidelines para o tratamento desta patologia. Após estabilização de resultados, por não apresentar mais melhoria, foi sujeita a análise electromigráfica no sentido de perceber quais as estratégias utilizadas pela paciente para compensar o défice do grande dentado, na manutenção de 4 movimentos: Elevação anterior a 90º, elevação lateral a 90º, elevação lateral no plano da omoplata a 90º e no push-up contra a parede. Foram avaliados o trapézio superior, médio e inferior, bem como o grande dentado. Verificou-se que não existe um padrão de activação consistente dos estabilizadores da omoplata em cadeia cinética aberta, sendo que o padrão é mantido em cadeia cinética fechada.

Introdução – O incremento do tempo de exposição à microgravidade origina um descondicionamento músculo-esquelético que precisa de ser prevenido através do treino. Objetivos – Identificar os padrões destas alterações e descrever os programas de treino em microgravidade e estratégias pós-exposição. Método – A pesquisa da revisão da literatura foi conduzida através da MEDLINE/PubMed e PEDro com as seguintes palavras--chave: “spaceflight rehabilitation”, “spaceflight muscle”, “microgravity muscle” e “bed rest muscle”, seguida de uma seleção dos artigos. Resultados – Os estudos encontrados apresentam uma resposta músculo-tendinosa diferencial sendo que o treino protege total ou parcialmente estas estruturas. Conclusão – O treino de resistance de intensidade elevada e baixas repetições associado a exercícios específicos é o mais adequado para responder ao descondicionamento. - ABSTRACT - Introduction – The increased microgravity exposition time raised the need for training programs to avoid muscle and tendinous deconditioning. Objectives – To identify the deconditioning patterns and to identify and describe the training programs used for its prevention during and after microgravity exposure. Methods – This literature review is based on a search conducted via MEDLINE/PubMed and PEDro using the following search words: “spaceflight rehabilitation”, “spaceflight muscle”, “microgravity muscle” and “bed rest muscle”. The search was followed by an article selection. Results – The studies reveal a differential exposure phenomenon for which the training programs reviewed are partly effective. Conclusion – According to the literature the high intensity low volume resistance programs with specific exercises are more appropriate to address the deconditioning problem.

Alzheimer Disease (AD) is characterized by progressive cognitive decline and dementia. Earlier diagnosis and classification of different stages of the disease are currently the main challenges and can be assessed by neuroimaging. With this work we aim to evaluate the quality of brain regions and neuroimaging metrics as biomarkers of AD. Multimodal Imaging Brain Connectivity Analysis (MIBCA) toolbox functionalities were used to study AD by T1weighted, Diffusion Tensor Imaging and 18FAV45 PET, with data obtained from the AD Neuroimaging Initiative database, specifically 12 healthy controls (CTRL) and 33 patients with early mild cognitive impairment (EMCI), late MCI (LMCI) and AD (11 patients/group). The metrics evaluated were gray-matter volume (GMV), cortical thickness (CThk), mean diffusivity (MD), fractional anisotropy (FA), fiber count (FiberConn), node degree (Deg), cluster coefficient (ClusC) and relative standard-uptake-values (rSUV). Receiver Operating Characteristic (ROC) curves were used to evaluate and compare the diagnostic accuracy of the most significant metrics and brain regions and expressed as area under the curve (AUC). Comparisons were performed between groups. The RH-Accumbens/Deg demonstrated the highest AUC when differentiating between CTRLEMCI (82%), whether rSUV presented it in several brain regions when distinguishing CTRL-LMCI (99%). Regarding CTRL-AD, highest AUC were found with LH-STG/FiberConn and RH-FP/FiberConn (~100%). A larger number of neuroimaging metrics related with cortical atrophy with AUC>70% was found in CTRL-AD in both hemispheres, while in earlier stages, cortical metrics showed in more confined areas of the temporal region and mainly in LH, indicating an increasing of the spread of cortical atrophy that is characteristic of disease progression. In CTRL-EMCI several brain regions and neuroimaging metrics presented AUC>70% with a worst result in later stages suggesting these indicators as biomarkers for an earlier stage of MCI, although further research is necessary.

The involvement of the gastrointestinal tract in the co-infection of HIV and Leishmania is rarely reported. We report the case of an HIV-infected adult man co-infected with a disseminated form of leishmaniasis involving the liver, lymph nodes, spleen and, as a feature reported for the first time in the English literature, the pancreas. Light microscopy showed amastigote forms of Leishmania in pancreatic macrophages and immunohistochemical staining revealed antigens for Leishmania and also for HIV p24. Microscopic and ultrastructural analysis revealed severe acinar atrophy, decreased zymogen granules in the acinar cytoplasm and also nuclear abnormalities such as pyknosis, hyperchromatism and thickened chromatin. These findings might correspond to the histologic pattern of protein-energy malnutrition in the pancreas as shown in our previous study in pancreas with AIDS and no Leishmania. In this particular case, the protein-energy malnutrition may be due to cirrhosis, or, Leishmania or HIV infection or all mixed. We believe that this case represents the morphologic substratum of the protein energy malnutrition in pancreas induced by the HIV infection. Further studies are needed to elucidate these issues.
