Medulloblastomas (MB) are the most common malignant brain tumors in childhood. Alkylator-based drugs are effective agents in the treatment of patients with MB. In several tumors, including malignant glioma, elevated O(6)-methylguanine-DNA methyltransferase (MGMT) expression levels or lack of MGMT promoter methylation have been found to be associated with resistance to alkylating chemotherapeutic agents such as temozolomide (TMZ). In this study, we examined the MGMT status of MB and central nervous system primitive neuroectodermal tumor (PNET) cells and two large sets of primary MB. In seven MB/PNET cell lines investigated, MGMT promoter methylation was detected only in D425 human MB cells as assayed by the qualitative methylation-specific PCR and the more quantitative pyrosequencing assay. In D425 human MB cells, MGMT mRNA and protein expression was clearly lower when compared with the MGMT expression in the other MB/PNET cell lines. In MB/PNET cells, sensitivity towards TMZ and 1-(2-chloroethyl)-3-cyclohexyl-1-nitrosourea (CCNU) correlated with MGMT methylation and MGMT mRNA expression. Pyrosequencing in 67 primary MB samples revealed a mean percentage of MGMT methylation of 3.7-92% (mean: 13.25%, median: 10.67%). Percentage of MGMT methylation and MGMT mRNA expression as determined by quantitative RT-PCR correlated inversely (n = 46; Pearson correlation r (2) = 0.14, P = 0.01). We then analyzed MGMT mRNA expression in a second set of 47 formalin-fixed paraffin-embedded primary MB samples from clinically well-documented patients treated within the prospective randomized multicenter trial HIT'91. No association was found between MGMT mRNA expression and progression-free or overall survival. Therefore, it is not currently recommended to use MGMT mRNA expression analysis to determine who should receive alkylating agents and who should not.

A genome-wide association study (GWAS) of educational attainment was conducted in a discovery sample of 101,069 individuals and a replication sample of 25,490. Three independent single-nucleotide polymorphisms (SNPs) are genome-wide significant (rs9320913, rs11584700, rs4851266), and all three replicate. Estimated effects sizes are small (coefficient of determination R(2) ≈ 0.02%), approximately 1 month of schooling per allele. A linear polygenic score from all measured SNPs accounts for ≈2% of the variance in both educational attainment and cognitive function. Genes in the region of the loci have previously been associated with health, cognitive, and central nervous system phenotypes, and bioinformatics analyses suggest the involvement of the anterior caudate nucleus. These findings provide promising candidate SNPs for follow-up work, and our effect size estimates can anchor power analyses in social-science genetics.

Les patients atteints de cancers reçoivent différents traitement, tels que la radiothérapie ou la chimiothérapie. Actuellement, environ 60% des enfants survivants du cancer développent des effets secondaires cognitifs, consécutifs aux traitements énoncés précédemment. Compte tenu de la perspective du développement psychomoteur de l’enfant et de l’immaturité du système nerveux central (SNC) chez ces patients, il s’avère particulièrement pertinent d’étudier les effets secondaires que provoquent les traitements anticancéreux sur le développement cognitif de cette population de malades. Des études ont démontrées l’existence de liens étroits entre ces effets secondaires et l’abolition de la neurogénèse provoquée principalement par l’irradiation. Ce projet de maîtrise porte sur les effets du facteur de croissance épidermique, l’EGF (un facteur de croissance impliqué dans la prolifération cellulaire) sur la neurogénèse de la souris. Nous avons également cherché un vecteur de sécrétion efficace pour permettre une diffusion continue d’EGF à long terme (2 à 4 semaines). Notre hypothèse est que l’EGF serait capable de stimuler la neurogénèse et protéger les cellules de l’apoptose dans le cerveau de la souris, suite à une irradiation. Nous avons montré un effet positif de l’EGF sur la formation et la prolifération des neuroblastes Dcx(+) dans la zone sous ventriculaire (ZSV) et non dans l’hippocampe (Hi), suite à l’injection de l’EGF, directement dans le cerveau à l’aide d’une pompe osmotique. Nous avons observé que cette augmentation de la quantité de jeunes neurones est indépendante de la capacité de l’EGF à les protéger de l’apoptose. L’EGF ne protège pas non plus les blastes leucémiques, issus de lignées de cellules humaines, des effets secondaires d’une irradiation. Les cellules souches mésenchymateuses (CSM) modifiées génétiquement et générées pour sécréter l’EGF ne montrent aucun effet sur la stimulation de la neurogénèse quand elles sont directement injectées dans le cerveau. Finalement, nos résultats indiquent que l’EGF pourrait être un bon candidat pour le développement de nouvelles thérapies pour traiter les effets secondaires que provoque une irradiation du cerveau. L’utilisation de pompes pour permettre l’administration d’EGF dans le cerveau devient alors très intéressante pour améliorer la qualité de vie des patients.

Although most recent publications focus on Ventilator-associated Pneumonia, Non-Ventilator-associated Hospital-acquired pneumonia (NVHAP) is still worrisome. We studied risk factors for NVHAP among patients admitted to a small teaching hospital. Sixty-six NVHAP case patients and 66 controls admitted to the hospital from November 2005 through November 2006 were enrolled in a case-control study. Variables under investigation included: demographic characteristics, comorbidities, procedures, invasive devices and use of medications (Sedatives, Antacids, Steroids and Antimicrobials). Univariate and multivariable analysis (hierarchical models of logistic regression) were performed. The incidence of NVHAP in our hospital was 0.68% (1.02 per 1,000 patients-day). Results from multivariable analysis identified risk factors for NVHAP: age (Odds Ratio[OR]=1.03, 95% Confidence Interval[CI]=1.01-1.05, p=0.002), use of Antacids (OR=5.29, 95%CI=1.89-4.79, p=0.001) and Central Nervous System disease (OR=3.13, 95%CI=1.24-7.93, p=0.02). Although our findings are coherent with previous reports, the association of Antacids with NVHAP recalls a controversial issue in the physiopathology of Hospital-Acquired Pneumonia, with possible implications for preventive strategies.

Background: The use of multiple medicines is very frequent among the elderly, allowing them to perceive more often adverse side effects from drugs and present undesirable drug interactions.Methods: This article presents a cross-sectional survey about the use of medicines among 300 elderly Brazilians, equally divided into institutionalized and community-dwelling groups.Results: The average daily intake of medicines is 3.2 among institutionalized elderly, a higher (p < 0.001) number when compared with community-dwelling elderly, who takes an average of 1.8 medicines daily. The most commonly used medications are antihypertensives (58.0%), diuretics (23.0%), nonsteroidal anti-inflammatory drugs (22.7%), supplements (21.7%), antidiabetics (16.3%), and antiulcerants (14.0%). Antiulcerants, diuretics, supplements, and central nervous system drugs are more frequently used by institutionalized than by community-dwelling elderly.Conclusion: In this Brazilian elderly sample, the most widely used medicines were antihypertensives, diuretics, and nonsteroidal anti-inflammatory drugs, and institutionalized used more medications than community-dwelling elderly. Detecção de formas amastigotas do Trypanosoma cruziem enxerto renalA doença de Chagas é zoonose transmitida pelo Trypanosoma cruzi, o qual apresenta duas formas distintas no hospedeiro vertebrado, a tripomastigota circulante e a amastigota tecidual. Esta última parasita freqüentemente os tecidos musculares cardíaco, liso e estriado, e o tecido nervoso. Até o presente momento nunca foram detectados formas amastigotas em parênquima renal. O presente relato descreve, pela primeira vez, a detecção de formas amastigotas do T. cruzi em parênquima renal em receptor de enxerto de rim, com testes sorológicos negativos para a doença de Chagas e ausência de transfusões prévias, observado 1 mês após o transplante renal com doador cadáver proveniente de região endêmica. O paciente desenvolveu doença de Chagas aguda com detecção de formas tripomastigotas circulantes. Como a única forma de transmissão desta zoonose pelo enxerto é através de órgão parasitado com formas amastigotas, sugere-se fortemente que o rim transplantado foi o responsável pela transmissão da doença de Chagas, no presente caso. Esta é a via de infecção que deve ser levada em consideração em transplantes nas áreas endêmicas

An outbreak of cryptococcosis occurred in a breeding aviary in São Paulo, Brazil. Seven psittacine birds (of species Charmosyna papou, Lorius lory, Trichoglossus goldiei, Psittacula krameri and Psittacus erithacus) died of disseminated cryptococcosis. Incoordination, progressive paralysis and difficulty in flying were seen in five birds, whereas superficial lesions coincident with respiratory alterations were seen in two birds. Encapsulated yeasts suggestive of Cryptococcus sp. were seen in faecal smears stained with India ink in two cases. Histological examination of the birds showed cryptococcal cells in various tissues, including the beak, choana, sinus, lungs, air sacs, heart, liver, spleen, kidneys, intestines and central nervous system. High titres of cryptococcal antigen were observed in the serum of an affected bird. In this case, titres increased during treatment and the bird eventually died. Yeasts were isolated from the nasal mass, faeces and liver of one bird. Cryptococcus neoformans var. gattii serovar B was identified based on biochemical, physiological and serological tests. These strains were resistant (minimum inhibitory concentration 64 μ g/ml) to fluconazole. This is the first report of C. neoformans var. gattii occurring in psittacine birds in Brazil. © 2004 ISHAM.

Paracoccidioidomycosis is caused by Paracoccidioides brasiliensis, a dimorphic fungus, prevalent in tropical and subtropical America. It is rare in the United States of America, Canada, Asia and Europe and in these countries it is related to immigrants from endemic areas. Paracoccidioidomycosis associated with immunosuppression runs a course with rapid progression and dissemination of the disease, with many cutaneous lesions. The mortality rate is up to 35% when associated with HIV infection or AIDS. The diagnosis depends on visualization of the agent through direct examination, histopathology, or culture. First choice treatment is done with Amphotericin B deoxycholate. Itraconazole is an option for long term treatment. Sporotrichosis is caused by Sporothrix schenckii, the species of reference. Other species have been considered such as: Sporothrix brasiliensis, S.globosa and S.mexicana and the S.schenckii var. lurei. It is a ubiquitous disease although more prevalent in tropical and subtropical areas. Currently, it has been reported as a zoonotic disease of cats and dogs, with transmission to their owners in the city of Rio de Janeiro (Brazil). Sporotrichosis associated to immunosuppression is uncommon or underreported. There were 34 cases in association with HIV infection or AIDS reported so far. Presenting with disseminated disease and non cutaneous lesions including joints, lungs and central nervous system. Amphotericin B deoxycholate is the first choice for treatment and itraconazol considered an alternative.

Os flebovírus (família Bunyaviridae; gênero Phlebovirus), possuem importância considerável em saúde publica, pois podem causar uma variedade de síndromes clínicas. Na região amazônica brasileira até o momento já foram isolados 23 flebovírus sendo que quatro se destacam por terem sido isolados de humanos: Virus Alenquer, Virus Candiru, Virus Morumbi e Virus Serra Norte. Estes mais o Virus Itaituba, isolado de Didelphis marsupialis, fazem parte do Complexo Candiru (grupo Candiru) e foram utilizados para estudos de caracterização genética e de infecção experimental em hamsters dourados (Mesocricetus auratus). Os Hamsters mostraram-se suscetíveis a infecção por esses flebovírus, apesar de não terem apresentado sinais de doença. A análise histopatológica demonstrou aspectos lesionais no fígado, nos rins, no baço, nos pulmões e no sistema nervoso central, sendo as lesões mais intensas no fígado comprovadas pela presença dos antígenos virais empregando a técnica de imunohistoquímica. A análise das seqüências nucleotídicas parciais dos segmentos PRNA e MRNA obtidas para os cinco flebovírus estudados mostraram maior similaridade genética entre si do que com outros membros do gênero Phlebovirus. Sendo as mesmas geneticamente mais relacionadas ao Virus Punta Toro. Filogeneticamente, independentemente do segmento genômico analisado, os cinco flebovírus constituem um grupo monofilético, sendo que a análise pelo método de máxima verossimilhança demonstrou diferentes origens evolutivas para os segmentos de RNA o que sugere a ocorrência de rearranjo genético em natureza entre estes cinco flebovírus amazônicos.

Fabry's disease corresponds to an inherited disorder transmitted by an X-linked recessive gene. It generates a dysfunction of glycosphingolipid metabolism due to an enzymatic deficiency of alpha-galactosidase activity, resulting in glycosphingolipid deposits in all areas of the body. The clinical (heart, kidney, and central nervous system) manifestations are more severe in hemizygous boys than in heterozygous girls. They appear during childhood or adolescence: acroparesthesia, joint pain, angiokeratoma, corneal dystrophy, hypohydrosis or anhydrosis, and renal failure. The otoneurologic symptoms consist of hearing fluctuation, progressive unilateral or bilateral hearing loss, and episodes of vertigo or dizziness. Otoneurologic findings in 12 of 26 members of the same family are presented: the mother and 9 of her 12 children, as well as 2 of her 14 grandchildren: 4 healthy persons, 4 heterozygous female carriers, and 4 hemizygous male patients. Three of the male patients had fluctuation of hearing, sudden hearing loss, and episodes of vertigo and dizziness. The otoneurologic examinations showed a bilateral cochleovestibular deficit (n = 1), a right cochleovestibular deficit (n = 1), and a bilateral hearing loss combined with a right vestibular deficit (n = 1). Histopathologic evidence of glycosphingolipid accumulation in vascular endothelial and ganglion cells, as well as atrophy of the stria and spiral ligament, might explain the otoneurologic symptoms and findings.

When a child is not following the normal, predicted growth curve, an evaluation for underlying illness and central nervous system abnormalities is required and appropriate consideration should be given to genetic defects causing growth hormone (GH) deficiency. This article focuses on the GH gene, the various gene alterations, and their possible impact on the pituitary gland. Transcription factors regulating pituitary gland development may cause multiple pituitary hormone deficiency but may present initially as GH deficiency. The role of two most important transcription factors, POU1F1 (Pit-1) and PROP 1, is discussed.

Von Hippel-Lindau (VHL) disease is an autosomal dominant disorder characterized by the development of retinal and central nervous system hemangioblastoma, renal cell carcinoma (RCC), pheochromocytoma and pancreatic islet cell tumors (PICT). The VHL gene maps to chromosome 3p25 and has been shown to be mutated in 57% of sporadic cases of RCC, implicating VHL in the genesis of RCC. We report a multigeneration VHL kindred in which four affected female siblings developed PICT at early ages. Analysis of the three coding exons of the VHL gene in this family revealed a single, missense mutation in codon 238. Inheritance of the 238 mutation has been reported to correlate with a 62% risk of pheochromocytoma development. In this kindred, all affected individuals carried the mutation as well as one additional sibling who showed no evidence of disease. Clinical screening of this individual indicated small ($<$1 cm) pancreatic and kidney tumors. Results suggest that inheritance of the codon 238 mutation does not correlate with early onset pheochromocytoma. Rather, the only individual in the pedigree with pheochromocytoma was the proband's mother who developed bilateral pheochromocytoma at the age of 62. Thus, the VHL codon 238 mutation may predispose to late onset pheochromocytoma in this family; however, it does not explain the preponderance of PICT in the third generation since this mutation has not been reported to increase the risk of developing pancreatic lesions. This suggests that inheritance of the codon 238 mutation and subsequent somatic inactivation of the wild type allele of the VHL gene may not be sufficient to explain the initiation and subsequent progression to malignancy in VHL-associated neoplasms. Since the two tumor types that most frequently progress to malignancy are RCC and PICT, we asked whether loss of heterozygosity (LOH) could be detected proximal to the VHL gene on chromosome 3 in distinct regions of 3p previously implicated by LOH and cytogenetic studies to contain tumor suppressor loci for RCC. LOH was performed on high molecular weight DNA isolated from peripheral blood and frozen tumor tissue of family members using microsatellite markers spanning 3p. Results indicated LOH for all informative 3p loci in tumor tissue from affected individuals with PICT. LOH was detected along the entire length of the chromosome arm and included the proximal region of 3p13-14.2 implicated in the hereditary form of renal cell carcinoma.^ If 3p LOH were a critical event in pancreatic islet cell tumorigenesis, then it should be expected that LOH in sporadic islet cell tumors would also be observed. We expanded LOH studies to include sporadic cases of PICT. Consistent LOH was observed on 3p with a highest frequency LOH in the region 3p21.2. This is the first evidence for an association between chromosome 3 loci and pancreatic islet cell tumorigenesis. (Abstract shortened by UMI.) ^
