Objetivo: analisar os conceitos e percepções que adolescentes e seus cuidadores possuem sobre saúde mental e serviços de saúde em seu contexto ecológico e investigar as barreiras de acesso à assistência à saúde mental vivenciadas. Método: trata-se de estudo exploratório e analítico em amostra de conveniência obtida no período de outubro de 2009 a junho de 2010, com 100 adolescentes e 100 cuidadores, no município de Belém-PA, em dois contextos clínicos públicos, sendo um ambulatório especializado em saúde mental e um geral e dois contextos escolares, sendo um público e um privado. Utilizou-se questionários estruturados, para investigar diferentes dimensões envolvidas nas temáticas saúde, família, bem-estar e condições de vida, seguidos de análise estatística, com técnicas de análise da variância e correlacional. Resultados: a média das idades dos adolescentes foi de 14,47 (DP 1,90) anos, sendo 58% feminino; o tipo de problema de saúde mental relatado pela maioria foram problemas na escola (21,9%); o profissional mais frequentemente procurado foi o psicólogo (59,4%). No que tange as concepções de saúde mental, adolescentes e cuidadores deram importância ao comportamento de abster-se de drogas; quanto às concepções de doença mental, ambos, conceberam como algo a ser considerado com seriedade; ambos concordaram que a religião contribui para a saúde/doença mental e revelaram a primazia da mãe na busca de ajuda; no que tange as estratégias de coping os adolescentes lidavam de forma semelhante com os problemas de saúde mental em suas vidas; adolescentes e cuidadores possuíam uma visão estigmatizada do profissional de saúde e temores de discriminação principalmente pelos pares; quanto ao tratamento real ou imaginado ambos revelaram concepções favoráveis das terapias como fonte de ajuda e espaço privilegiado para expressar a própria opinião e em qualquer dos casos, a mãe revelou-se como a principal pessoa a contribuir na busca de ajuda especializada. As variáveis que revelaram a procedência das concepções sobre saúde/doença mental e as estratégias empregadas na manutenção da saúde mental da família mostraram diferenças entre os contextos investigados; no que tange ao auto conceito, os adolescentes da escola privada mostraram maior auto-congruência entre o self real e o ideal comparativamente os demais contextos; os cuidadores revelaram auto-congruência maior na escola pública. Quanto às perspectivas que o adolescente tem sobre a família revelaram identificações reais mais frequentes nos quatro contextos com a mãe, seguidas da avó/avô; quanto aos modelos de identificação familiar nos contextos clínicos e escola privada é maior com a mãe; na escola pública é maior com o pai; foi observado discrepância da perspectiva do cuidador acerca do conceito sobre o adolescente. Para a maioria dos adolescentes e cuidadores as condições de saúde foram classificadas de "boas" a "excelentes". A auto-avaliação do bem-estar dos adolescentes na amostra geral mostrou que, em sua maioria, sentiam-se muito satisfeitos, totalmente cheios de energia, divertiam-se e tiveram boa relação com os professores; na visão dos cuidadores, a maioria de seus adolescentes sentiam-se muito satisfeitos com a vida, utilizavam seu tempo livre divertindo-se com amigos e deram maior importância aos sentimentos de bem-estar com relação ao desempenho físico. Conclusões: são evidenciadas as semelhanças e diferenças entre adolescentes e cuidadores nas amostras clínicos e escolares que podem subsidiar ações preventivas de saúde contextualizadas para a cidade de Belém.

Estudos sobre saúde mental na adolescência destacam este tema como questão relevante, pois essa faixa etária, além de constituir-se como uma grande parcela da população que precisa e não procura atendimento, é identificada como um grupo etário vulnerável e de risco. A família e a escola têm sido consideradas como fatores de proteção à saúde mental de adolescentes. Sendo assim, precisa-se pensar em formas de intervenção mais eficazes, considerando o contexto familiar, cultural e social destes indivíduos. O objetivo do estudo foi investigar percepções sobre saúde e doença mental de adolescentes de escola pública e de escola privada na cidade de Belém-PA, bem como as principais redes de apoio e estratégias de cuidado utilizadas pelos adolescentes. Realizou-se um estudo transversal, do tipo quantitativo, no qual participaram 60 adolescentes, de ambos os sexos, e seus cuidadores. Os adolescentes tinham idades entre 12 a 17 anos, sendo 30 alunos de escola pública, localizada em um bairro periférico, e 30 de escola privada, localizada em um bairro central, na cidade de Belém-PA. Os cuidadores eram do sexo feminino, com idade entre 25 a 57 anos. Como instrumentos foram utilizados: roteiro de entrevista familiar, roteiro de entrevista com os coordenadores das escolas e questionário sobre saúde e doença mental e sobre serviços de saúde (versão para adolescente). Os resultados dos questionários foram analisados preferencialmente pelo teste do Qui-quadrado e o teste G para amostras independentes. Todo o processamento estatístico foi realizado no software BioEstat versão 5.2. Os resultados obtidos nas entrevistas permitiram a análise de aspectos socioeconômicos e de fatores de risco e de proteção na família dos adolescentes. Os resultados obtidos com os questionários revelaram que as percepções dos adolescentes da escola pública acerca da saúde mental estavam associadas a não ser tão sensível/frágil e a pensar positivo, ser otimista. Na escola privada, estavam associadas a sentir-se equilibrado e ser algo muito importante. Quanto às percepções de doença mental, na escola pública estavam relacionadas ao momento em que o corpo não está bem e a quando profissionais aconselham um tratamento; na escola privada, a ter sentimentos feridos e ser algo que não se percebe logo. Com relação à origem das ideias sobre saúde/doença mental, não houve real diferença entre os grupos. No que tange à religião, houve discordância apenas em relação a cura da doença mental. Como estratégia de enfrentamento, na escola pública esta esteve relacionada a falar com alguém sobre o problema enquanto na escola privada os adolescentes relataram que não procuravam ajuda. A mãe foi apontada como principal na busca de ajuda pelos adolescentes da escola pública; na escola particular, a principal referência foi o médico da família. A principal barreira para os adolescentes da escola pública no acesso ao serviço de saúde mental foi não saber o que o psicólogo/psiquiatra vai fazer com ele, e na escola privada foi não querer ser gozado/caçoado. Nos dois grupos, os principais problemas em saúde mental relatados foram problemas na escola e de comportamento. Os adolescentes de escola privada responderam que somente às vezes sentem-se sozinhos e felizes, enquanto na escola pública, os adolescentes afirmaram que sempre estiveram de bom humor e satisfeitos com a vida. Discute-se a necessidade de promover fatores de proteção à saúde mental de adolescentes.

This study focuses on the importance of the professional nurse in the exercise of extended care under the Mental Health of assistance provided by ESF. Aims to identify the difficulties experienced by nursing staff for assistance to people affected by mental illness. Inserts on the assumptions of qualitative research, and data collection used the semi-structured interviews and analyzed the material obtained through the thematic analysis. From the interviews the subjects were apprehended training/capacity building, intersectoral and little bias. These professionals have demonstrated that lack of skills to deal with the subjectivity of care, there is a failure in coordination among sectors of the area and that prejudice is still present in professional practice

The aim of this study was to evaluate maternal stress and anxiety and the quality of interaction of 10 mothers and their 0-3-month-old babies with Pierre Robin Sequence hospitalized in a university hospital. We used the Inventory of Stress Symptoms for Adults to evaluate stress and the State-Trait Anxiety Inventory to assess anxiety of the mothers, and a protocol to assess the recording of the mother-infant interaction. The results showed high levels of maternal stress and anxiety. A statistically significant inverse relationship was found: the higher the maternal anxiety and stress, the lower was the mother-infant interaction. The results indicate that work needs to be done to minimize the anxiety and stress of mothers with inpatient babies in order to encourage interaction.

This study approached perceptions of mental illness by nineteen professionals who work in a Family Health Unit, by means of graphic representations (drawings). We used qualitative methods. Data was collected by means of a Presentative-Expressive Procedure. Four themes were identified using Thematic Analysis. The professionals associate mental disorders with: health care, medical-centered view, exclusion/inclusion, social environment. These perceptions are related to a biological paradigm, favoring the reproduction of prejudices about mental illness. The analysis also emphasizes the importance of a wide range of cares, while considering how difficult it may be for the professionals. We conclude that it is necessary to invest on professional preparation, in an attempt to transform ideoaffective contents presented by the subjects’ imaginary.

The role of social cognition in severe mental illness (SMI) has gained much attention, especially over the last decade. The impact of deficits in socio-cognitive functioning has been found to have detrimental effects on key areas of day-to-day functioning in individuals with SMI, such as gaining and maintaining employment and overall experienced quality of life. Treatment of individuals with SMI is challenging, as the presentation of individual signs and symptoms is rather heterogeneous. There are several treatment approaches addressing deficits ranging from broader social and interpersonal functioning to neurocognitive and more intrapersonal functioning. As research in the domain of social cognition continues to identify specific deficits and its functional detriments, treatment options need to evolve to better target identified functional deficits. Social Cognition and Interaction Training (SCIT) was recently developed to address specific socio-cognitive deficits in an inpatient population of individuals with schizophrenia-spectrum disorders. This study applied SCIT in an outpatient SMI population as many deficits remain after individuals’ symptoms are less severe and overall functioning is more stable than during the acute inpatient phase of their rehabilitation. Specifically, this study has two objectives. First, to demonstrate that deficits in social cognition persist after the acute phase of illness has abated. Second, to demonstrate that these deficits can be ameliorated via targeted treatment such as SCIT. Data was gathered in local outpatient treatment settings serving a heterogeneous SMI population. Community Dent Oral Epidemiol 2011. (C) 2011 John Wiley & Sons A/S Abstract Objectives: Previous studies have found an association between bruxism and emotional and behavioral problems in children, but reported data are inconsistent. The objective of this study was to estimate the prevalence of bruxism, and of its components clenching and grinding, and its associations with mental problems and depression. Methods: Data from two Brazilian birth cohorts were analyzed: one from 869 children in Ribeirao Preto RP (Sao Paulo), a more developed city, and the other from 805 children in Sao Luis SL (Maranhao). Current bruxism evaluated by means of a questionnaire applied to the parents/persons responsible for the children was defined when the habit of tooth clenching during daytime and/or tooth grinding at night still persisted until the time of the assessment. Additionally, the lifetime prevalence of clenching during daytime only and grinding at night only was also evaluated. Mental health problems were investigated using the Strength and Difficulties Questionnaire (SDQ) and depression using the Childrens Depression Inventory (CDI). Analyses were carried out for each city: with the SDQ subscales (emotional symptoms, conduct problems, peer problems, attention/hyperactivity disorder), with the total score (sum of the subscales), and with the CDI. These analyses were performed considering different response variables: bruxism, clenching only, and grinding only. The risks were estimated using a Poisson regression model. Statistical inferences were based on 95% confidence intervals (95% CI). Results: There was a high prevalence of current bruxism: 28.7% in RP and 30.0% in SL. The prevalence of clenching was 20.3% in RP and 18.8% in SL, and grinding was found in 35.7% of the children in RP and 39.1% in SL. Multivariable analysis showed a significant association of bruxism with emotional symptoms and total SDQ score in both cities. When analyzed separately, teeth clenching was associated with emotional symptoms, peer problems, and total SDQ score; grinding was significantly associated with emotional symptoms and total SDQ score in RP and SL. Female sex appeared as a protective factor for bruxism, and for clenching and grinding in RP. Furthermore, maternal employment outside the home and white skin color of children were associated with increased prevalence of teeth clenching in SL. Conclusions: Mental health problems were associated with bruxism, with teeth clenching only and grinding at night only. No association was detected between depression and bruxism, neither clenching nor grinding. But it is necessary to be cautious regarding the inferences from some of our results.

Purpose of review: Overview on integrated care trials focusing on effectiveness and efficiency published from 2011 to 2013. Recent findings: Eight randomized controlled trials (RCTs) and 21 non-RCT studies were published from 2011 to 2013. Studies differed in several methodological aspects such as study population, psychotherapeutic approaches used, outcome parameters, follow-up times, fidelities, and implementation of the integrated care model and the nation-specific healthcare context with different control conditions. This makes it difficult to draw firm conclusions. Most studies demonstrated relevant improvements regarding symptoms (P = 0.001) and functioning (P = 0.01), quality of life (P = 0.01), adherence (P <0.05) and patient's satisfaction (P = 0.01), and reduction of caregiver's stress (P < 0.05). Mean total costs were favoring or at least equalizing costs but with positive effects found on subjective health favoring integrated care models. Summary: There is an increasing interest in the effectiveness and efficiency of integrated care models in patients with mental disorders, specifically in those with severe and persistent mental illness. To increase generalizability, future trials should exactly describe rationales and content of integrated care model and control conditions.

The Nursing Home Reform Act of 1987 requires nursing homes to provide basic mental health services for all residents and to give active mental health treatment, a set of specialized mental health services, to those residents who are admitted with a serious mental illness. This article examines the potential size of the nursing home population who will require mental health services, its demographic composition, and the facilities in which these individuals reside using the Institutional Population Component of the National Medical Expenditure Survey. Estimates of the potential costs of providing monthly psychotherapy and pharmacological management to this population in nursing homes indicate that the mandate will have significant financial effects on nursing facilities. Conclusions about how the requirements for maintaining the mental and psychosocial well-being of nursing home residents may affect the future of nursing home care and mental health care are considered.

This paper is part of a five-year research project funded by the National Institute on Disability and Rehabilitation Research (NIDRR) that addresses the health, disability and service needs of Native Americans in eastern tribes. The Participatory Action Research model with a community-based approach was used to facilitate collaboration among the participating tribes. Native American research technicians conducted individual interviews with members of their tribes. Demographics, prevalence of disabilities, and various factors associated with health and mental health are presented. Of the 858 tribal members who responded to survey questions, the third most prominent health problem reported was mental illness.

Background. Parkinson's disease is a chronic, progressive, age-related, neurodegenerative disorder with no known cause or promising cure. While substantial information is known about the pathophysiology of Parkinson's disease, little is known about the illness experience of persons living with the disease. The purpose of this study was to understand how persons with Parkinson's disease construct their illness experience and manage living with their illness on a daily basis. ^ Method. A qualitative study with an ethnographic approach employed the strategies of participant observations and fieldwork. Field data were generated from a two year exposure to two Parkinson's disease support groups in east Texas. Open-ended semi-structured interviews with seven men and seven women with Parkinson's disease were also conducted. These data were combined and analyzed using thematic analysis. ^ Findings. The illness experience is described through the metaphor "Sailing the Sea in The Eye of the Storm." This metaphor served as the overarching theme that covered the two interacting content themes of the voyage of Daily Negotiations in the Midst of Uncertainty and Reconstruction of the Self with Parkinson's Disease. Daily negotiations incorporated navigating daily activities with the uncertainty of both the progression and daily vicissitudes of the disease. Participants described their symptoms as progressive imprisonment that interfered with daily activities. The progressive nature of the disease required the participants to reconstruct their perceptions of themselves. Reconstructing the self involved the paradoxical balancing of preserving the self while simultaneously releasing aspects of the former self to reconstruct the self with Parkinson's disease. This process was reflected in four exemplars: I Know Me." "It's Still Me," "See Me." and "Remember Me." ^ Conclusions. This qualitative study illuminated the struggle of persons in dealing with the uncertainties and fluctuations of Parkinson's disease and the process of reconstructing their perceptions of themselves. The meaning and reconstruction of the illness experience expressed by participants will inform understanding beyond the disease itself to the illness experience that these participants must deal with on a daily basis. ^
