Posttransplant lymphoproliferative disorders (PTLDs) are associated with significant morbidity and mortality among solid-organ transplant patients, but approaches to diagnosis and management vary considerably. An international multidisciplinary panel evaluated current understanding of risk factors and classification systems and developed recommendations to aid in PTLD prevention. We considered evidence on PTLD risk factors including Epstein- Barr virus serostatus and immunosuppression and identified knowledge gaps for future research. Recommendations address prophylactic and preemptive strategies to minimize PTLD development, including modulation of immunosuppression and antiviral drug regimens. Finally, new classification criteria were outlined that may help facilitate standardized reporting and improve our understanding of PTLD.

Context. The ESO public survey VISTA variables in the Via Lactea (VVV) started in 2010. VVV targets 562 sq. deg in the Galactic bulge and an adjacent plane region and is expected to run for about five years. Aims. We describe the progress of the survey observations in the first observing season, the observing strategy, and quality of the data obtained. Methods. The observations are carried out on the 4-m VISTA telescope in the ZYJHK(s) filters. In addition to the multi-band imaging the variability monitoring campaign in the K-s filter has started. Data reduction is carried out using the pipeline at the Cambridge Astronomical Survey Unit. The photometric and astrometric calibration is performed via the numerous 2MASS sources observed in each pointing. Results. The first data release contains the aperture photometry and astrometric catalogues for 348 individual pointings in the ZYJHK(s) filters taken in the 2010 observing season. The typical image quality is similar to 0 ''.9-1 ''.0. The stringent photometric and image quality requirements of the survey are satisfied in 100% of the JHK(s) images in the disk area and 90% of the JHK(s) images in the bulge area. The completeness in the Z and Y images is 84% in the disk, and 40% in the bulge. The first season catalogues contain 1.28 x 10(8) stellar sources in the bulge and 1.68 x 10(8) in the disk area detected in at least one of the photometric bands. The combined, multi-band catalogues contain more than 1.63 x 10(8) stellar sources. About 10% of these are double detections because of overlapping adjacent pointings. These overlapping multiple detections are used to characterise the quality of the data. The images in the JHK(s) bands extend typically similar to 4 mag deeper than 2MASS. The magnitude limit and photometric quality depend strongly on crowding in the inner Galactic regions. The astrometry for K-s = 15-18 mag has rms similar to 35-175 mas. Conclusions. The VVV Survey data products offer a unique dataset to map the stellar populations in the Galactic bulge and the adjacent plane and provide an exciting new tool for the study of the structure, content, and star-formation history of our Galaxy, as well as for investigations of the newly discovered star clusters, star-forming regions in the disk, high proper motion stars, asteroids, planetary nebulae, and other interesting objects.

Diffuse large B-Cell lymphoma is the most common subtype of non-Hodgkin lymphoma in the West. In Brazil, it is the fifth cause of cancer, with more than 55,000 cases and 26,000 deaths per year. At Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo - HCFMUSP, diffuse large B-Cell lymphoma represents 49.7% of all non-Hodgkin lymphoma cases. Initially, the classification of non-Hodgkin lymphoma was based on morphology, but advances in immunology and molecular medicine allowed the introduction of a biological classification for these diseases. As for other cancers, non-Hodgkin lymphoma involves patterns of multi factorial pathogenesis with environmental factors, as well as genetic, occupational and dietary factors, contributing to its development. Multiple lesions involving molecular pathways of B-cell proliferation and differentiation may result in the activation of oncogenes such as the BCL2, BCL6,and MYC genes and the inactivation of tumor suppressor genes such as p53 and INK4, as well as other important transcription factors such as OCT-1 and OCT-2. A dramatic improvement in survival was seen after the recent introduction of the anti-CD20 monoclonal antibody. The association of this antibody to the cyclophosphamide, hydroxydaunorubicin, oncovin and prednisolone (CHOP) regimen has increased overall survival of diffuse large B-Cell lymphoma and follicular lymphoma patients by 20%. However, 50% of all diffuse large B-Cell lymphoma patients remain incurable, creating a demand for more research with new advances in treatment. Thus, it is important to know and understand the key factors and molecular pathways involved in the pathogenesis of diffuse large B-Cell lymphoma.

Undoubtedly the most important result of the investigations in physiology and biophysics was the discovery of the electrochemical mechanism of propagation of the action potential in nerves that was made by Hodgkin and Huxley during the first half of the past century. Since some decades ago diverse experiments about the electro optical properties of the axon membrane there was published using the most diverse optical experimental ‘procedures POT 6-10’. In this paper some results of a dynamical speckle technique applied for obtaining microscopic images of a section of a squid giant axon membrane during the activation by electrical impulses and his digital process are presented.

Molecular profiling of Peripheral T-cell lymphomas not otherwise specified Peripheral T-cell lymphomas (PTCLs) are a heterogeneous group of tumors that the WHO classification basically subdivides into specified and not otherwise specified (NOS). In Western countries, they represent around 12% of all non-Hodgkin's lymphomas. In particular, PTCL/NOS is the commonest subtype, corresponding to about 60-70% of all T-cell lymphomas. However, it remains a complex entity showing great variety regarding either morphology, immunophenotype or clinical behavior. Specially, the molecular pathology of these tumors is still poorly known. In fact, many alteration were found, but no single genes were demonstrated to have a pathogenetic role. Recently, gene expression profiling (GEP) allowed the identification of PTCL/NOS-associated molecular signatures, leading to better understanding of their histogenesis, pathogenesis and prognostication. Interestingly, proliferation pathways are commonly altered in PTCLs, being highly proliferative cases characterized by poorer prognosis. In this study, we aimed to investigate the possible role in PTCL/NOS pathogenesis of selected molecules, known to be relevant for proliferation control. In particular, we analyzed the cell cycle regulators PTEN and CDKN1B/p27, the NF-kB pathway, and the tyrosin kinase PDGFR. First, we found that PTEN and p27 seem to be regulated in PTCL/NOS as in normal T-lymphocytes, as to what expression and cellular localization are concerned, and do not present structural abnormalities in the vast majority of PTCL/NOS. Secondly, NF-kB pathway appeared to be variably activated in PTCL/NOS. In particular, according to NF-kB gene expression levels, the tumors could be divided into two clusters (C1 and C2). Specially, C1 corresponded to cases presenting with a global down-regulation of the entire pathway, while C2 showed over-expression of genes involved in TNF signaling. Notably, by immunohistochemistry, we showed that either the canonical or the alternative NK-kB pathway were activated in around 40% of cases. Finally, we found PGDFRA to be consistently over-expressed (at mRNA and protein level) and activated in almost all PTCLs/NOS. Noteworthy, when investigating possible causes for PDGFRA deregulation, we had evidences that PDGFR over-expression is due to the absence of miR-152, which appeared to be responsible for PDGFRA silencing in normal T-cells. Furthermore, we could demonstrate that its aberrant activation is sustained by an autocrine loop. Importantly, this is the first case, to the best of our knowledge, of hematological tumor in which tyrosin kinase aberrant activity is determined by deregulated miRNA expression and autocrine loop activation. Taken together, our results provide novel insight in PTCL/NOS pathogenesis by opening new intriguing scenarios for innovative therapeutic interventions.

Sudden cardiac death due to ventricular arrhythmia is one of the leading causes of mortality in the world. In the last decades, it has proven that anti-arrhythmic drugs, which prolong the refractory period by means of prolongation of the cardiac action potential duration (APD), play a good role in preventing of relevant human arrhythmias. However, it has long been observed that the “class III antiarrhythmic effect” diminish at faster heart rates and that this phenomenon represent a big weakness, since it is the precise situation when arrhythmias are most prone to occur. It is well known that mathematical modeling is a useful tool for investigating cardiac cell behavior. In the last 60 years, a multitude of cardiac models has been created; from the pioneering work of Hodgkin and Huxley (1952), who first described the ionic currents of the squid giant axon quantitatively, mathematical modeling has made great strides. The O’Hara model, that I employed in this research work, is one of the modern computational models of ventricular myocyte, a new generation began in 1991 with ventricular cell model by Noble et al. Successful of these models is that you can generate novel predictions, suggest experiments and provide a quantitative understanding of underlying mechanism. Obviously, the drawback is that they remain simple models, they don’t represent the real system. The overall goal of this research is to give an additional tool, through mathematical modeling, to understand the behavior of the main ionic currents involved during the action potential (AP), especially underlining the differences between slower and faster heart rates. In particular to evaluate the rate-dependence role on the action potential duration, to implement a new method for interpreting ionic currents behavior after a perturbation effect and to verify the validity of the work proposed by Antonio Zaza using an injected current as a perturbing effect.

Numerosi studi hanno mostrato come i meccanismi epigenetici di regolazione della cromatina svolgano un ruolo centrale nel controllare la trascrizione genica. E’ stato infatti dimostrato che complessi inibitori come SWI/SNF e gli enzimi ad esso associati quali istone deacetilasi (HDAC) e protein arginin-metiltrasferasi (PRMT), siano coinvolti nel controllo della crescita, differenziazione e proliferazione cellulare. Diversi studi hanno mostrato come i meccanismi epigenetici di controllo della trascrizione genica svolgano un ruolo di primo piano nel promuovere la sopravvivenza cellulare in leucemie/linfomi di derivazione dai linfociti B come la leucemia linfatica cronica, il linfoma mantellare ed i linfomi associati al virus di Epstein-Barr (EBV). Tuttavia, molto poco e’ conosciuto circa i meccanismi epigenetici di controllo della trascrizione che divengono operativi e che contribuiscono al processo di trasformazione dei linfociti B. PRMT5 e’ un enzima che di-metila specificamente residui argininici sugli istoni (H) 3 (H3R8) ed 4 (H4R3). PRMT5 ed HDAC lavorano in concerto per reprimere la trascrizione di specifici geni oncosoppressori. In questo progetto sono stati studiati i meccanismi e le conseguenze dell’iperespressione di PRMT5 durante il processo di trasformazione dei linfociti B indotto da EBV, e’ stata dimostrata l’importanza di questo enzima nel processo di trasformazione, e sono stati studiati nuovi metodi per inibirne l’espressione/attivita’. In particolare si e’ dimostrato che l’espressione di PRMT5 e’ ridotta o assente in linfociti B normali (o attivati da stimoli fisiologici) ed elevata in linee cellulari linfoblastoidi immortalizzate o completamente trasformate. Elevati livelli citosolici di PRMT5 sono detectabili dopo 4 giorni dall’infezione di linfociti B normali con EBV, PRMT5 e’ detectabile a livello nucleare, dove esercita la sua funzione repressoria la trascrizione, a partire dal giorno 8. L’utilizzo di specifici small interference RNAs (siRNA) in linee cellulari linfoblastoidi ha permesso di dimostrare la riduzione dell’espressione di PRMT5, la riduzione della metilazione degli istoni target di PRMT5, inibizione della proliferazione cellulare e abbassamento della soglia di sensibilita’ cellulare a stimoli pro-apoptotici. Esperimenti di co-immunoprecipitazione cromatinica hanno permesso di evidenziare che in queste cellule PRMT5 e’ parte di un complesso proteico a funzione inibitoria e che questo complesso si lega alla regione promotrice di specifici geni oncosoppressori quali ST7, GAS e NM23, inibendone la trascrizione. Si e’ inoltre provveduto a sviluppare una categoria di inibitori allosterici di PRMT5: l’attivita’ terapeutica/la specificita’ in vitro e la modalita’ di somministrazione ottimale in modelli murini di linfoma non-Hodgkin, sono in corso di valutazione.

La tesi tratta di come Noble giunse alla formulazione del modello matematico del cuore, a partire da quello degli impulsi nervosi ideato da Hodgkin e Huxley e di come in seguito arrivò a perfezionarlo grazie ad una sempre maggiore adesione alla biologia dei sistemi integrati, al punto che esso venne poi utilizzato per creare il primo "Cuore Virtuale". Si pone inoltre una particolare attenzione al modo in cui il pensiero di Noble cambiò nel corso dei suoi studi, così da permettergli la formulazione di spiegazioni corrette, relative ai fenomeni osservati.

Most of the problems in modern structural design can be described with a set of equation; solutions of these mathematical models can lead the engineer and designer to get info during the design stage. The same holds true for physical-chemistry; this branch of chemistry uses mathematics and physics in order to explain real chemical phenomena. In this work two extremely different chemical processes will be studied; the dynamic of an artificial molecular motor and the generation and propagation of the nervous signals between excitable cells and tissues like neurons and axons. These two processes, in spite of their chemical and physical differences, can be both described successfully by partial differential equations, that are, respectively the Fokker-Planck equation and the Hodgkin and Huxley model. With the aid of an advanced engineering software these two processes have been modeled and simulated in order to extract a lot of physical informations about them and to predict a lot of properties that can be, in future, extremely useful during the design stage of both molecular motors and devices which rely their actions on the nervous communications between active fibres.

Backgrounds:Treatment of patients with relapsed/refractory (R/R) diffuse large B-cell lymphoma (DLBCL) not eligible to high dose therapy represents an unmet medical need. Panobinostat showed encouraging therapeutic activity in studies conducted in lymphoma cell lines and in vivo in patients with advanced hematologic malignancies.Purpose:FIL-PanAL10 (NCT01523834) is a phase II, prospective multicenter trial of the Fondazione Italiana Linfomi (FIL) to evaluate safety and efficacy of single agent Panobinostat as salvage therapy for R/R DLBCL patients and to evaluate a possible relationships between response and any biological features. Patients and Methods:Patients with R/R DLBCL were included. The treatment plan included 6 induction courses with Panobinostat monotherapy followed by other 6 courses of consolidation. The primary objective was to evaluate Panobinostat activity in terms of overall response (OR); secondary objectives were: CR rate, time to response (TTR), progression-free survival (PFS), safety and feasibility of Panobinostat. We included evaluation of the impact of pharmacogenetics, immunohistochemical patterns and patient’s specific gene expression and mutations as potential predictors of response to Panobinostat as explorative objectives. To this aim a pre-enrollment new tissue biopsy was mandatory. ResultsThirty-five patients, 21 males (60%), were enrolled between June 2011 and March 2014. At the end of induction phase, 7 responses (20%) were observed, including 4 CR (11%), while 28 patients (80%) discontinued treatment due to progressive disease (PD) in 21 (60%) or adverse events in 7 (20%). Median TTR in 9 responders was 2.6 months (range 1.8-12). With a median follow up of 6 months (range 1-34), the estimated 12 months PFS and OS were 27% and 30.5%, respectively. Grade 3-4 thrombocytopenia and neutropenia were the most common toxicities (in 29 (83%) and 12 (34%) patients, respectively. Conclusions The results of this study indicate that Panobinostat might be remarkably active in some patients with R/R DLBCL, showing durable CR

Das VHL-Syndrom umfasst Erkrankungen, die mit einem Funktionsverlust von VHL einhergehen. Das Tumorspektrum umfasst retinale und zerebrale Hämangioblastome, Nierenzysten und klarzellige Nierenkarzinome, Zysten und Tumore des Pankreas, Phäochromocytome, Adenome der Hoden und Tumore des Mittelohrs. Obwohl aufgrund klinischer Studien bekannt ist, welche VHL-Mutation mit welchen Neoplasien assoziiert werden können, konnte bisher kein VHL-Mausmodell das Krankheitsbild des VHL-Syndroms widerspiegeln. Daher ist vermutlich eine zusätzliche Fehlregulation weiterer Gene nötig ist, um die Tumorgenese in den verschiedenen Geweben zu induzieren. In mehreren klarzelligen Nierenkarzinomen konnte bereits eine PTEN-Defizienz nachgewiesen werden, der Verlust von PTEN wird außerdem auch mit der Tumorgenese von Phäochromocytomen assoziiert. Möglicherweise wirken VHL und PTEN also in der Tumorsuppression in der Niere und der Nebenniere zusammen.rnIm Rahmen dieser Arbeit konnte erstmals eine VHL-vermittelte Stabilisierung der PTEN-Konzentration sowohl in embryonalen als auch in Tumor-Zellen der Niere nachgewiesen werden. Die Analyse des Regulationsmechanismus ergab erstens eine Hypoxie-abhängige Abnahme der Transkription von PTEN. Des Weiteren konnte eine VHL-vermittelte Ubiquitinylierung von NEDD4-1, welches als E3-Ligase von PTEN dessen Degradation und Kerntransport reguliert, ermittelt werden. rnIn Nierenkarzinom-Zellen wurde weiterhin eine VHL- bzw. PTEN-Restitution induziert, um die Auswirkungen der beiden Tumorsuppressoren auf das Zellverhalten in vitro und in vivo zu untersuchen. Sowohl VHL als auch PTEN hatten dieselben Effekte lediglich in unterschiedlicher Intensität auf das Verhalten der Zellen. So konnte VHL- und PTEN-abhängig eine Verstärkung der Adhäsion, eine Inhibierung der Migration und eine Verminderung der Überlebens- und Metastasierungsfähigkeit nachgewiesen werden. Des Weiteren wurden Mausmodelle mit einem ubiquitären, heterozygoten Pten-Verlust generiert, die teilweise eine zusätzliche Haploinsuffizienz von Vhl bzw. eine heterozygote VHL Typ II-Mutation (V2B oder V2C) trugen. Sporadisch entwickelten diese Mäuse Vhl-abhängig Lebertumore und Pten-abhängig Lymphome und Ovarialkarzinome. Einige Mäuse mit einer kombinierten Vhl- und Pten-Defizienz bildeten zusätzlich Nierenzysten aus, die teilweise das gesamte Volumen der Niere einnahmen. Besonders häufig entstanden in Pten-haploinsuffizienten Mäusen Phäochromocytome, die durch eine zusätzliche V2B- oder V2C-Mutation in gleichaltrigen Mäusen deutlich weiterentwickelt waren. Demnach induziert erst der gemeinsame Verlust von Vhl und Pten die Bildung von Nierenzysten und Phäochromocytomen, welche dem Krankheitsbild des VHL-Syndroms zugeordnet werden.rnDie Untersuchungen innerhalb dieser Arbeit zeigen erstmalig die Interaktion und Kooperation von VHL und PTEN in der Tumorsuppression. Die Resultate bieten außerdem die Grundlage für weitere Analysen der Auswirkung der VHL-vermittelten PTEN-Stabilisierung und für detailliertere Untersuchungen der durch die kombinierte Vhl- und Pten-Defizienz induzierten Neoplasien der Niere und der Nebennieren-Tumore in in vivo Mausmodellen.rn

After decades of confusion in lymphoma classification clearness was achieved with the publication of the REAL classification 1994 and of the WHO classification 2001. The revised 4th edition 2008 features some additional new categories. The WHO classification comprises B- and T-lymphoblastic neoplasms, mature B-cell lymphomas, mature T-cell and NK-cell lymphomas and Hodgkin lymphomas. A modern diagnostic work-up of lymphomas is based on morphology, immunohistochemistry and increasingly on molecular studies. Last but not least the evaluation of all these findings by an expert haematopathologist, who collaborates closely with the treating clinicians, is essential. The aim is to give an overview of the most frequent mature B-cell lymphomas and the most important classical Hodgkin lymphomas with focus on morphology and immunohistochemistry.

The literature on malignant lymphomas has grown to an extent which is hardly palatable for general practitioners and non-lymphoma aficionados. Nevertheless some lymphomas exhibit typical clinical and radiological features which permit to suspect the correct histopathological diagnosis even before a pathology report becomes available. The present article points out such "case vignettes" of particular types of malignant lymphoma.

The advent of highly active antiretroviral therapy (HAART) in 1996 led to a decrease in the incidence of Kaposi's sarcoma (KS) and non-Hodgkin's lymphoma (NHL), but not of other cancers, among people with HIV or AIDS (PWHA). It also led to marked increases in their life expectancy.
