A series of studies in the field of Epidemiological Psychiatry have been performed over the last two decades, and these have focused on the ability of primary care physicians to detect emotional disorders in the patients that attend their practices. The scientific methodology utilized in these studies is the subject of this review, which contains a discussion concerning: a) interviewer awareness bias; b) accuracy of the instruments and c) medical and psychological concepts involved in defining minor emotional disorders. Suggestions for change in the methodology are made in each of the sections of the review.

A cohort study on acute respiratory infections, involving 270 children observed by pediatricians in their homes every 10 days over a period of 32 months, gave the opportunity to experience logistic and methodological problems seldom described in the literature. The purpose of this article is to alert researchers as to the difficulties faced when performing community-based studies in developing countries. Although a carefully planned project was undertaken, problem areas included the establishment of the target population, population dynamics, field related problems, laboratory aspects and data management. It is hoped that other investigators may benefit from the extensive experience gained from our program in foreseeing and coping with the difficulties involved.

The increasing endemicity of tuberculosis resulting from causes such as immigration, poverty, a declining public health infrastructure and co-infection by HIV/Mycobacterium tuberculosis, is leading to a change in tuberculosis control programmes. One of the main reasons for the resurgence of tuberculosis is HIV infection - the risk of tuberculosis is greater in HIV patients than in the majority of the population as can be seen from numerous research projects. The need for systematic testing for HIV infection in all tuberculosis patients by undertaking confidential HIV tests on admission to a tuberculosis programme is brought out. This measure would increase the number of cases diagnosed and provide data for better surveillance of the co-infection.

INTRODUCTION: In Mexico, breast cancer (BC) is one of the main causes of cancer deaths in women, with increasing incidence and mortality in recent years. Therefore, the aim of the study is identify possible risk factors related to BC. METHODS: An epidemiological study of hospital cases of BC and controls with cervical uterine cancer (CUCA) was carried out at eight third level concentration hospitals in Mexico City. The total of 353 incident cases of BC and 630 controls with CUCA were identified among women younger than 75 years who had been residents of the metropolitan area of Mexico City for at least one year. Diagnosis was confirmed histologically in both groups. Variables were analyzed according to biological and statistical plausibility criteria. Univariate, bivariate and multivariate analyses were carried out. Cases and controls were stratified according to the menopausal hormonal status (pre and post menopause). RESULTS: The factors associated with BC were: higher socioeconomic level (OR= 2.77; 95%CI = 1.77 - 4.35); early menarche (OR= 1.32; 95%CI= 0.88 - 2.00); old age at first pregnancy (>31 years: OR= 5.49; 95%CI= 2.16 - 13.98) and a family history of BC (OR= 4.76; 95% CI= 2.10 - 10.79). In contrast, an increase in the duration of the breastfeeding period was a protective factor (>25 months: OR= 0.38; 95%CI= 0.20 - 0.70). CONCLUSIONS: This study contributes to the identification of risk factors for BC described in the international literature, in the population of Mexican women. Breastfeeding appears to play an important role in protecting women from BC. Because of changes in women`s lifestyles, lactation is decreasing in Mexico, and young women tend not to breastfeed or to shorten the duration of lactation.

OBJECTIVE: Describe the overall transmission of malaria through a compartmental model, considering the human host and mosquito vector. METHODS: A mathematical model was developed based on the following parameters: human host immunity, assuming the existence of acquired immunity and immunological memory, which boosts the protective response upon reinfection; mosquito vector, taking into account that the average period of development from egg to adult mosquito and the extrinsic incubation period of parasites (transformation of infected but non-infectious mosquitoes into infectious mosquitoes) are dependent on the ambient temperature. RESULTS: The steady state equilibrium values obtained with the model allowed the calculation of the basic reproduction ratio in terms of the model's parameters. CONCLUSIONS: The model allowed the calculation of the basic reproduction ratio, one of the most important epidemiological variables.

The aim of this study was the assessment of exposure to ultrafine in the urban environment of Lisbon, Portugal, due to automobile traffic, and consisted of the determination of deposited alveolar surface area in an avenue leading to the town center during late spring. This study revealed differentiated patterns for weekdays and weekends, which could be related with the fluxes of automobile traffic. During a typical week, ultrafine particles alveolar deposited surface area varied between 35.0 and 89.2 μm2/cm3, which is comparable with levels reported for other towns such in Germany and the United States. These measurements were also complemented by measuring the electrical mobility diameter (varying from 18.3 to 128.3 nm) and number of particles that showed higher values than those previously reported for Madrid and Brisbane. Also, electron microscopy showed that the collected particles were composed of carbonaceous agglomerates, typical of particles emitted by the exhaustion of diesel vehicles. Implications: The approach of this study considers the measurement of surface deposited alveolar area of particles in the outdoor urban environment of Lisbon, Portugal. This type of measurements has not been done so far. Only particulate matter with aerodynamic diameters <2.5 (PM2.5) and >10 (PM10) μm have been measured in outdoor environments and the levels found cannot be found responsible for all the observed health effects. Therefore, the exposure to nano- and ultrafine particles has not been assessed systematically, and several authors consider this as a real knowledge gap and claim for data such as these that will allow for deriving better and more comprehensive epidemiologic studies. Nanoparticle surface area monitor (NSAM) equipments are recent ones and their use has been limited to indoor atmospheres. However, as this study shows, NSAM is a very powerful tool for outdoor environments also. As most lung diseases are, in fact, related to deposition of the alveolar region of the lung, the metric used in this study is the ideal one.

Objetivo: avaliar as modificações ocorridas no âmbito da sequência de ativação o da sinergia extensora do MS e na sua influência sobre a capacidade de placing do polegar face à aplicação de um programa de intervenção em fisioterapia. Pretendeu-se também verificar as repercussões ao nível da participação dos indivíduos nas AVD`s. Metodologia: esta série de estudos de casos foi constituída por dois indivíduos com modificações nas componentes neuromotoras de movimento após AVE predominantes no MS. A avaliação dois indivíduos foi realizada em dois momentos PRE e POST separados por um período de intervenção de cerca de quinze semanas. Em cada momento de avaliação foi aplicada a F-MAMR, foi recolhido sinal electromiográfico dos músculos GD, DP, TLONG, TLAT e ABDPOL durante o movimento de alcance e usaram-se seis itens da CIF selecionadas de acordo com as necessidades sentidas. No momento PRE aplicou-se a MMSE para detetar défices cognitivos que interferissem com o processo de avaliação e de intervenção. O programa de intervenção foi individualizado e devidamente ajustado aos indivíduos, procurando que estes mantivessem um papel ativo durante a reabilitação. Esta foi realizada com a frequência de três vezes por semana e cada sessão teve cerca de quarenta e cinco minutos de trabalho ativo. Resultados: encontraram-se modificações na sequência de ativação muscular nos dois indivíduos e entre os indivíduos nos dois momentos de avaliação, nomeadamente no tempo de ativação muscular. O ABDPOL modificou o tempo em que iniciou atividade muscular e passou a recrutar atividade quase simultaneamente ao início do movimento da mão, verificando-se uma influência positiva da sinergia extensora do MS na capacidade de placing do polegar. Verificaram-se resultados positivos no estado sensório-motor do MS e na capacidade do indivíduo em participar em diversas AVD`s selecionadas de acordo com as necessidades. Conclusão: este estudo parece apontar para uma influência positiva nas modificações do nível de atividade da sinergia extensora do MS e na influência desta sobre a capacidade de placing do polegar durante o movimento de alcance, face a um programa de intervenção. Este estudo parece apontar também para uma influência positiva sobre a capacidade do indivíduo em participar em AVD`s selecionadas para a avaliação de acordo com necessidades sentidas.

There are complex and diverse methodological problems involved in the clinical and epidemiological study of respiratory diseases and their etiological factors. The association of urban growth, industrialization and environmental deterioration with respiratory diseases makes it necessary to pay more attention to this research area with a multidisciplinary approach. Appropriate study designs and statistical techniques to analyze and improve our understanding of the pathological events and their causes must be implemented to reduce the growing morbidity and mortality through better preventive actions and health programs. The objective of the article is to review the most common methodological problems in this research area and to present the most available statistical tools used.

OBJETIVO: Os transtornos depressivos constituem um problema de saúde pública devido a sua alta prevalência e impacto psicossocial. Pacientes deprimidos são freqüentadores assíduos de serviços de atendimento primário, porém, muitas vezes, não são diagnosticados como tais. O objetivo do estudo é avaliar a associação entre sintomas depressivos e funcionamento social numa amostra de pacientes que procuraram um serviço de cuidados primários em uma capital brasileira. MÉTODOS: Foram avaliados 2.201 usuários de serviços de cuidados de saúde primários de Porto Alegre quanto à saúde física e emocional. Foi aplicado questionário em entrevista única, com duas questões genéricas de avaliação de qualidade de vida do World Health Organization Quality of Life (WHOQOL-Breve), mais itens do Medical Outcomes Study Short-Forms 12 (SF-12) e MHI 5 (MHI-5), do Centers for Epidemiologic Studies -- Depression (CES-D), além de outras questões referentes a busca de atendimento médico e faltas ao trabalho. Foram realizados testes de Kruska-Wallis e de comparações múltiplas de Tambane. RESULTADOS: Dos indivíduos estudados, 79,5% eram do sexo feminino, com média de idade de 40 anos. A intensidade da sintomatologia depressiva medida pelo CES-D foi de 20,2 para as mulheres e de 16,2 para os homens. Todos os parâmetros avaliados tiveram relação inversa com a intensidade dos sintomas depressivos. CONCLUSÕES: Os resultados reforçam a afirmativa de que a sintomatologia depressiva tem uma alta associação com pior funcionamento social e qualidade de vida e maior utilização de recursos de saúde em pacientes de cuidados primários.

Recent epidemiologic studies clearly outline the link between fungal sensibilization and exarcebations of asthma, leading to increased morbidity and mortality. Amongst the filamentous fungi, Aspergillus scpecies have been strongly linked with exarcebations of asthma and other respiratory allergic diseases. Particles of approximately 1 to 4 pm are deposited in the lower respiratory tract. Therefore, conidia of A. fumigatus are small enough to traverse the terminal respiratory airways and reach the pulmonary alveoli, whereas the larger conidia of some other Aspergillus species, such as A. flavus and A. niger, tend to be deposited in the paranasal sinuses and upper airways. Exposute to environmental fungal spores has been associated with worsening asthma symptoms, lung function, hospital admissions and asthma-related deaths.

Os sistemas fotovoltaicos produzem energia eléctrica limpa, e inesgotável na nossa escala temporal. A Agência Internacional de Energia encara a tecnologia fotovoltaica como uma das mais promissoras, esperando nas suas previsões mais optimistas, que em 2050 possa representar 20% da produção eléctrica mundial, o equivalente a 18000 TWh. No entanto, e apesar do desenvolvimento notável nas últimas décadas, a principal condicionante a uma maior proliferação destes sistemas é o ainda elevado custo, aliado ao seu fraco desempenho global. Apesar do custo e ineficiência dos módulos fotovoltaicos ter vindo a diminuir, o rendimento dos sistemas contínua dependente de factores externos sujeitos a grande variabilidade, como a temperatura e a irradiância, e às limitações tecnológicas e falta de sinergia dos seus equipamentos constituintes. Neste sentido procurou-se como objectivo na elaboração desta dissertação, avaliar o potencial de optimização dos sistemas fotovoltaicos recorrendo a técnicas de modelação e simulação. Para o efeito, em primeiro lugar foram identificados os principais factores que condicionam o desempenho destes sistemas. Em segundo lugar, e como caso prático de estudo, procedeu-se à modelação de algumas configurações de sistemas fotovoltaicos, e respectivos componentes em ambiente MatlabTM/SimulinkTM. Em seguida procedeu-se à análise das principais vantagens e desvantagens da utilização de diversas ferramentas de modelação na optimização destes sistemas, assim como da incorporação de técnicas de inteligência artificial para responder aos novos desafios que esta tecnologia enfrentará no futuro. Através deste estudo, conclui-se que a modelação é não só um instrumento útil para a optimização dos actuais sistemas PV, como será, certamente uma ferramenta imprescindível para responder aos desafios das novas aplicações desta tecnologia. Neste último ponto as técnicas de modelação com recurso a inteligência artificial (IA) terão seguramente um papel preponderante. O caso prático de modelação realizado permitiu concluir que esta é igualmente uma ferramenta útil no apoio ao ensino e investigação. Contudo, convém não esquecer que um modelo é apenas uma aproximação à realidade, devendo recorrer-se sempre ao sentido crítico na interpretação dos seus resultados.

OBJECTIVE: Studies on the aspects of HIV infection in small Brazilian municipalities are invaluable to appropriately design control strategies, better allocate resources, and improve health care services. The objective of the study was to assess the clinical and epidemiological aspects of HIV infection in a small municipality. METHODS: A descriptive study was carried out in Miracema, a small municipality in the northwestern area of the state of Rio de Janeiro, Brazil, between July 1999 and December 2003. All HIV-infected adult patients followed up at the local HIV/AIDS Program were included. Clinical and epidemiologic characteristics were prospectively assessed through standardized questionnaires. RESULTS: A total of 65 adult patients who attended the local HIV/AIDS Program were analyzed. Most (34) were women (male to female ratio: 0.9). An absolute predominance of patients who were born in Miracema or neighboring municipalities (94%), lived in Miracema (90.7%), were single (70.8%), attributed the acquisition of HIV infection to unprotected heterosexual intercourse (72.3%) and had a past history of snorting cocaine (27.7) was found Central nervous system disorders (including five cases of cryptococcal meningitis) and acute pulmonary pneumocystosis-like respiratory failure were major causes of morbidity. Most patients (56.9%) were at presented in advanced stages of HIV infection. CONCLUSIONS: The predominance of patients on advanced stages of HIV infection suggest the existence of a large pool of undiagnosed cases in the community. A major feature of the cohort was an inverted male to female ratio. Further investigations over a broader geographic area are urgently needed for better understanding the clinical and epidemiological characteristics of HIV infection in small Brazilian municipalities and rural areas.

This paper describes the main methodological aspects of a cohort study, with emphasis on its recent phases, which may be relevant to investigators planning to carry out similar studies. In 1993, a population based study was launched in Pelotas, Southern Brazil. All 5,249 newborns delivered in the city's hospitals were enrolled, and sub-samples were visited at the ages of one, three and six months and of one and four years. In 2004-5 it was possible to trace 87.5% of the cohort at the age of 10-12 years. Sub-studies are addressing issues related to oral health, psychological development and mental health, body composition, and ethnography. Birth cohort studies are essential for investigating the early determinants of adult disease and nutritional status, yet few such studies are available from low and middle-income countries where these determinants may differ from those documented in more developed settings.

Human immunodeficiency virus (HIV) is the worldwide disseminated causative agent of acquired immunodeficiency syndrome (AIDS). HIV is a member of the Lentivirus genus of Retroviridae family and is grouped in two types named HIV-1 and HIV-2. These viruses have a notable ability to mutate and adapt to the new conditions of human environment. A large incidence of errors at the transcriptional level results in changes on the genetic bases during the reproductive cycle. The elevated genomic variability of HIV has carried important implications for the diagnosis, treatment and prevention as well as epidemiologic investigations. The present review describes important definitions and geographical distribution of subtypes, circulating recombinant forms and other genomic variations of HIV. The present study aimed at leading students of Biomedical Sciences and public health laboratory staff guidance to general and specific knowledge about the genomic variability of the HIV.
