In August of 2012 the Iowa State Office of Rural Health (SORH) conducted a survey to determine the value of the Small Rural Hospital Improvement Program (SHIP) in Iowa. This survey was distributed to the 84 participating hospitals however, because some hospitals network their SHIP funds we only asked the contract administrators of the contracts to complete the survey. 58 of the 78 SHIP contract administrators completed the survey (74%). Background: SHIP brings in roughly $750,000.00 annually to Iowa to assist small Iowa hospitals. Average distribution of approximately $7,500 per hospital. Seventy three of ninety nine Iowa counties are represented.

BACKGROUND: Delirium and frailty - both potentially reversible geriatric syndromes - are seldom studied together, although they often occur jointly in older patients discharged from hospitals. This study aimed to explore the relationship between delirium and frailty in older adults discharged from hospitals. METHODS: Of the 221 patients aged >65 years, who were invited to participate, only 114 gave their consent to participate in this study. Delirium was assessed using the confusion assessment method, in which patients were classified dichotomously as delirious or nondelirious according to its algorithm. Frailty was assessed using the Edmonton Frailty Scale, which classifies patients dichotomously as frail or nonfrail. In addition to the sociodemographic characteristics, covariates such as scores from the Mini-Mental State Examination, Instrumental Activities of Daily Living scale, and Cumulative Illness Rating Scale for Geriatrics and details regarding polymedication were collected. A multidimensional linear regression model was used for analysis. RESULTS: Almost 20% of participants had delirium (n=22), and 76.3% were classified as frail (n=87); 31.5% of the variance in the delirium score was explained by frailty (R (2)=0.315). Age; polymedication; scores of the Confusion Assessment Method (CAM), instrumental activities of daily living, and Cumulative Illness Rating Scale for Geriatrics; and frailty increased the predictability of the variance of delirium by 32% to 64% (R (2)=0.64). CONCLUSION: Frailty is strongly related to delirium in older patients after discharge from the hospital.

OBJECTIVE: Show the steps of a Trauma Registry (TR) implementation in a Brazilian public hospital and evaluate the initial data from the database.METHODS: Descriptive study of the a TR implementation in João XXIII Hospital (Hospital Foundation of the state of Minas Gerais) and analysis of the initial results of the first 1,000 patients.RESULTS: The project was initiated in 2011 and from January 2013 we began collecting data for the TR. In January 2014 the registration of the first 1000 patients was completed. The greatest difficulties in the TR implementation were obtaining funds to finance the project and the lack of information within the medical records. The variables with the lowest completion percentage on the physiological conditions were: pulse, blood pressure, respiratory rate and Glasgow coma scale. Consequently, the Revised Trauma Score (RTS) could be calculated in only 31% of cases and the TRISS methodology applied to 30.3% of patients. The main epidemiological characteristics showed a predominance of young male victims (84.7%) and the importance of aggression as a cause of injuries in our environment (47.5%), surpassing traffic accidents. The average length of stay was 6 days, and mortality 13.7%.CONCLUSION: Trauma registries are invaluable tools in improving the care of trauma victims. It is necessary to improve the quality of data recorded in medical records. The involvement of public authorities is critical for the successful implementation and maintenance of trauma registries in Brazilian hospitals.

OBJECTIVE: to characterize the epidemiological profile of patients undergoing hip replacement, primary or revisional. METHODS: we conducted a retrospective, descriptive study, including hip arthroplasties performed from January 2009 to June 2012 in a Belo Horizonte teaching hospital, Minas Gerais State - MG, Brazil. Data were analyzed using descriptive statistics. RESULTS: orthopedic procedures represented 45% of the operations at the hospital in the period, 1.4% hip arthroplasties. There were 125 hip replacements, 85 total, 27 partial and 13 reviews. Among the patients, 40% were male and 60% were female. Age ranged between 20 and 102 years, mean and median of 73 and 76 years, respectively. The most frequent diagnosis (82%) was femoral neck fracture by low-energy trauma caused by falling form standing position. In 13 revision operations, 12 required removal of the prosthesis. The infectious complication led to revision in 54% of the time, followed by dislocation (15%), peri-prosthetic fracture (15%) and aseptic loosening (15%). The infection etiologic agent was identified in 43% of occasions. The average length of the prosthesis to a revision operation was eight months. CONCLUSION: patients undergoing hip arthroplasty are elderly, with femoral neck fracture caused by falling form standing position, affecting more women. The incidence of hip prosthesis loosening was 10%. The main cause of the infection was loosening. The incidence of revisional hip arthroplasty was 10% and the incidence of hospital mortality in patients undergoing hip arthroplasty was 7.2%.

The role of the hospital-employed nurse educator is evolving. Factors influencing this change include the introduction of standards for nurse educators by the College of Nurses of Ontario (CNO), a change in the way nurses are educated, the emergence of nursing as a profession, and hospital restructuring as a result of budgetary constraints. Two of these influencing factors: the introduction of the updated Standards of Practice for Registered Nurses and Registered Practical Nurses (1996) and hospital restructuring occurred over the last 7 years at several hospitals in southern Ontario. Current literature as well as the Standards of Practice (1996) were utilized to examine the current roles and responsibilities of nurse educators and subsequently develop a questionnaire to study the impact of these influencing factors on the role of the nurse educator. This questionnaire was piloted and revised before its distribution at 4 hospitals in southern Ontario. Twenty-five of the 41 surveys (61%) distributed were returned for analysis. The data reflected that the Standards of Practice had a positive influence on the role of the nurse educator, while hospital restructuring had a negative impact. In addition, many of the roles and responsibilities identified in the literature were indeed part of the current role of nurse educators, as well as several responsibilities not captured in the literature. The predictions for the future of this role in its current state were not positive given the financial status of the health care system as well as the lack of clarity for the role and the current level ofjob satisfaction among practicing nurse educators. However, a list of recommendations were generated which, if implemented, could add clarity to the role and improve job satisfaction. This could enhance the retention of current nurse educators and the possibility of recruiting competent nurse educators to the role in the future.

El objetivo de este trabajo es presentar el diseño de un proyecto de mejora a través de la filosofía TOC para la gestión Administrativa del Hospital El Tunal del Nivel III de Bogotá DC, durante el período 2013. Para lograr el objetivo de la propuesta de mejoramiento basada en la filosofía TOC (Inglés siglas Theory of Constraints), se centra en crear un proceso de mejora continua en la gestión administrativa del Hospital El Tunal. Esta entidad es una Empresa Social del Estado y tiene por objeto la prestación de servicios de salud a usuarios que presentan algún tipo de enfermedad o trastorno orgánico que afecta su salud; se centra en mejorar, corregir y proporcionar una mejor calidad de vida al usuario que viene a cubrir sus necesidades de salud. Este producto de investigación terminará con una propuesta centrada en la mejora de la gestión administrativa, basada en la filosofía TOC aplicado a la cartera morosa, tratando de ofrecer una serie de soluciones a los problemas actuales que impactan estas organizaciones centrándose en la mejora continua, la velocidad de flujo de los recursos administrativos, verificando si es eficaz en beneficio de los usuarios y los profesionales de la salud.

Se realizó un estudio descriptivo de los accidentes por agresión animal atendidos en el Hospital Centro Oriente E.S.E. de Bogotá, D.C. durante los años 2002 y 2003, con el objetivo de caracterizar clínica y epidemiológicamente los pacientes que consultaron por este evento. La población estudio fue 338 pacientes atendidos en las diferentes Unidades Asistenciales por accidentes causados por animales 46% (n=154) en el 2002 y 54% (n=184) en el 2003, encontrando el mayor porcentaje de ocurrencia en el grupo etáreo de 5 – 9 años, (11%, n =17 y 13%, n =23), predominando el género masculino con 64% (n = 99) en el 2002 y 55% (n =102) en el 2003.

Objetivo: Evaluar la percepción que tienen los trabajadores acerca del sistema de seguridad y salud en el trabajo en la población asistencial y administrativa en un Hospital de III nivel de atención., Bogotá-Colombia. Materiales y métodos: Estudio de corte transversal en población de trabajadores asistenciales y administrativos. Se aplicó el “Cuestionario Nórdico Sobre Seguridad en el Trabajo NOSACQ 50 Spanish” validado. La muestra fue probabilística estratificada aleatoria, en 308 trabajadores (230 asistenciales y 78 administrativos). Resultados: El promedio de edad fue 39.5± 12 años, con mayor frecuencia de género femenino (74.68%), estado civil soltero (38.96%) y nivel educativo técnico (34.40%). La percepción que tienen los trabajadores acerca del sistema de seguridad y salud en el trabajo fue independiente de su tipo de actividad laboral administrativa y asistencial (p>0.05), la mayor percepción en ambos grupos fue la confianza de los trabajadores en la eficacia del sistema de seguridad (2.71 y 2.77), y las de menor percepción presentaron el empoderamiento de seguridad de gestión (2.35 y 2.46) y la seguridad como prioridad de los empleados y rechazo del riesgo (2.35 y 2.40). Conclusiones: Los trabajadores del Hospital tienen un nivel adecuado de buena percepción acerca de los aspectos de seguridad y salud en el trabajo donde se evidenció que la fortaleza es la confianza de los trabajadores en la eficacia del sistema y la debilidad del sistema se encuentra en la falta de empoderamiento y rechazo al riesgo.

El Trauma Craneoencefálico (TCE) infantil constituye un motivo frecuente de consulta en Urgencias y supone la primera causa de muerte en niños, llegando a ser hasta del 50% en trauma severo. En Colombia se conocen cifras de TCE por algunos estudios descriptivos, pero no existen reportes en Bogotá ni específicamente en TCE severo. Objetivo: Caracterizar el trauma craneoencefálico severo pediátrico en la Unidad de Cuidado intensivo del Hospital de la Misericordia entre los años 2010 y 2013. Materiales y métodos: Un estudio descriptivo retrospectivo fue realizado en el Hospital de la Misericordia mediante revisión de las historias clínicas de los pacientes que ingresaron a la Unidad de Cuidado Intensivo pediátrico con diagnóstico de trauma craneoencefálico severo entre el año 2010 al 2013. Resultados: Se incluyeron 63 pacientes (71,4% hombres) con una edad mediana de 4 años (RIQ 2-8). La mayoría de los traumas fueron originados por caída o accidente de tránsito (79,4%). La principal lesión fue fractura de cráneo (79%). Casi la mitad de los pacientes sufrieron algún tipo de secuela neurológica al egreso (47,1%). La mayoría de los pacientes que murieron (19%) sufrieron choque hipovolémico (83,3%) y presentaron trauma asociado (66,7%). Conclusión: Las características y epidemiología del trauma craneoencefálico en nuestra población muestran claras similitudes con lo reportados en otras series de la literatura mundial y de Colombia, excepto por la mortalidad, que se esperaba más alta al estudiar solo pacientes con TCE severo.

A motivação organizacional é assunto que vem sendo estudado mais profundamente com o passar do tempo, buscando entender o que realmente faz com que o colaborador se sinta motivado para desenvolver suas atividades laborais. A fim de atingir o objetivo proposto nesta pesquisa foram estudados os aspectos motivacionais no ambiente de trabalho da área administrativa do hospital filantrópico Divina Providência, localizado no município de Marituba integrante da área metropolitana do Estado do Pará e, qual a percepção dos clientes em relação aos serviços prestados por estes profissionais. Para tanto, a primeira parte do estudo, apresenta uma pesquisa bibliográfica, com o objetivo de coletar contribuições de diversos autores sobre o tema motivação Estes aspectos foram estudados sob a égide de estudiosos como Maslow, Herzberg, McGregor, Aldefer e Adams. O estudo justifica-se pela necessidade de serem obtidos dados que dizem respeito a aspectos objetivos e subjetivos que podem ser captados ou não na correlação entre os fatores motivacionais e o êxito dos serviços realizados. Face à natureza do problema, este estudo de caso tem como procedimento metodológico a aplicação e a análise de três questionários em três amostragens: colaboradores da área administrativa sem cargo de chefia, colaboradores que possuem cargo de chefia na área administrativa em dados sobre motivação laboral, clientes sobre a qualidade de serviços prestados pelo hospital. Por um lado, os dados evidenciaram uma visão realista em relação a motivação dos colaboradores. Mostrando ainda uma visão positiva em relação à qualidade dos serviços prestados. Os resultados expostos conseguiram demonstrar quais os principais fatores que representam a motivação dos colaboradores no trabalho além de visualizar a satisfação dos usuários com os serviços prestados.

O presente estudo teve objetivo identificar qual a percepção e expectativas que os Gestores da Sociedade Hospitalar Maria Vitória localizada no Estado de Pernambuco tem acerca da contribuição do Prontuário Eletrônico do Paciente – PEP – para a gestão estratégica do Hospital. Este trabalho está focado em três importantes pilares, a saber: Administração Hospitalar e suas principais características, a Tecnologia da Informação – TI como ferramenta estratégica para a gestão e a utilização do Prontuário Eletrônico do Paciente e suas peculiaridades. No que se refere à metodologia aplicada trata-se de um estudo de caso, o tipo de pesquisa utilizado, quanto aos meios, foram bibliográfica, documental e de campo. As conclusões suscitadas servirão no sentido de contribuir para o avanço em matéria de administração pública, mais especificamente, a percepção dos gestores da potencial contribuição do PEP como ferramenta estratégica na gestão do Hospital Maria Vitória.
