La malaltia pulmonar obstructiva crónica (MPOC) té una prevalença elevada al nostra medi i un gran impacte econòmic. La sobreinfecció respiratoria en els pacients MPOC avançats, es bàsicament, per Pseudomonas. L´antibioticoteràpia domiciliaria endovenosa en aquest tipus de sobreinfecció es de gran importància. La forma d´administració de l´antibiòtic es mitjançant bombes d´infusió continues i bombes d´infusió intermitents. Es tracta d´un estudi comparatiu dels dos tipus de bombes d´infusió per l´erradicació de Pseudomonas aeruginosa en el pacient MPOC.
Estudio descriptivo del Síndrome de Distrés Respiratorio del Adulto en el Paciente Politraumatizado.

La síndrome de distrés respiratori agut es caracteritza per hipoxemia, infiltrats bilaterals en la radiografia de tórax en absència de signes d'insuficiència cardíaca. Les teràpies destinades al tractament no han tingut resultats esperançadors per la impossibilitat de detectar precoçment la població en risc. El nostre objectiu és fer una anàlisi descriptiva i prospectiu dels pacients politraumatizats que van ingressar en la Fe de València durant 2009. Analitzant diverses variables: APACHE, pH, Làctic, etc; mesurant-les a l'ingrés, a les 12, 24 i 48h. Concloent que els únics factors amb significació estadística van ser l'A. Làctic i el pH a les 12h

iv. La infiltració subfascial contínua d'anestèsic local a la ferida quirúrgica és una tècnica d'analgèsia postoperatòria. Objectius: Intentem valorar l'eficàcia analgèsica d'aquesta tècnica després d'una cesària. Material i Mètodes: Estudiem 44 pacients en les 48 hores següents a la cesària infiltrant bupivacaína 0'2% amb un catèter subfascial i una bomba d'infusió contínua. Resultats: Hi ha una correcta analgèsia amb un pic de dolor inicial. Discussió i Conclusions: La infiltració contínua d'un anestèsic local a nivell subfascial a la ferida quirúrgica és eficaç en la reducció del dolor postcesària però seria convenient reforçar-la inicialment.

Objectiu: Avaluar una intervenció dirigida a la reducció d‘infeccions relacionades amb els catèters venosos centrals (IRC). Mètodes: Estudi pre-postintervenció, 2004-2006. Població d'estudi: pacients portadors de catèter venós central (CVC). La intervenció va consistir en conèixer la situació bassal, identificar factors de risc, i en el segon període realitzar diverses intervencions. Resultats: Es van analitzar 175 i 200 CVC en el període pre i postintervenció, respectivament. Es va observar una incidència de IRC de 20% durant el període preintervenció i de 5,5% en el període post-intervenció (p&0,001). Conclusió: La aplicació de mesures de prevenció ha aconseguit una disminució d’aquestes del 72,5%.

La manometría convencional es el “gold standard” para diagnosticar trastornos motores del esófago, pero la información que da sobre la repercusión funcional de estas alteraciones es escasa. La manometría de alta resolución permite estudiar con detalle la motilidad esofagogástrica mediante la generación de mapas topográficos de presiones desde la orofaringe hasta el estómago. Hipótesis: Si la generación de mapas topográficos de presiones en el esófago mediante manometría de alta resolución demuestra la resistencia al flujo esofagogástrico. Objetivo: Comprobar si un test con sobrecarga de agua puede mostrar la presencia de resistencia al flujo esofagogástrico en pacientes con acalasia. Método: Estudiamos 2 grupos de pacientes con alteración de la motilidad esofágica. Un grupo de 8 pacientes que cumplen criterios manométricos de acalasia y, como grupo control, 8 pacientes con disfunción del peristaltismo esofágico ( DPE). A cada paciente se le realizó un test de sobrecarga que consistía en la ingesta rápida de 200 ml de agua mientras se registraban las presiones esofágicas. Resultados: Los pacientes con acalasia ingirieron el agua más lentamente que los pacientes con DPE (82± 14 seg vs 34± 6 seg, p&0,05). Mientras la unión gastroesofágica (UGE) permaneció contraída en el grupo de pacientes con acalasia (46,6± 6 mm Hg; p&0,05), permaneció relajada durante el tiempo del ingesta en pacientes con DPE ( 16,6 ± 4,7 mm Hg). La unión esófago-gástrica (UGE) experimentó una migración proximal en pacientes con acalasia de 1,3 ± 0 cm mientras que en el grupo control no migró (0 cm; p&0,05). La ingesta de agua se asoció a un incremento de la presión del esófago distal (2 cm por encima de la UGE) significativamente mayor en los pacientes con acalasia que en los pacientes con DPE (42,2 ± 20 vs 9,5± 7,9 mm Hg respectivamente, p&0,05) lo que produjo un incremento del gradiente de presión esófago-gástrico en pacientes con acalasia (16± 0,9 mmHg) que no se observó en los pacientes con DPE (0,1 ± 0,4 mmHg ; p&0,05). Conclusión: Un test con sobrecarga de agua durante la medición de la topografía y de las presiones esofágicas demuestra obstrucción al flujo esofagogástrico en los pacientes con acalasia. Este test podría contribuir a valorar la repercusión funcional en pacientes con acalasia y podría servir para el seguimiento de pacientes con acalasia tratados.

Introduction: The last twenty years has witnessed important changes in the field of obstetric analgesia and anesthesia. In 2007, we conducted a survey to obtain information regarding the clinical practice of obstetric anesthesia in our country. The main objective was to ascertain whether recent developments in obstetric anesthesia had been adequately implemented into current clinical practice. Methodology: A confidential questionnaire was sent to 391 identified wiss obstetric anesthetists. The questionnaire included 58 questions on 5 main topics: activity and organization of the obstetric unit, practice of labor analgesia, practice of anesthesia for caesarean section, prevention of aspiration syndrome, and pain treatment after cesarean section. Results: The response rate was 80% (311/391). 66% of the surveyed anesthetists worked in intermediate size obstetric units (500-1500 deliveries per year). An anesthetist was on site 24/24 hours in only 53% of the obstetric units. Epidural labor analgesia with low dose local anesthetics combined with opioids was used by 87% but only 30% used patient controlled epidural analgesia (PCEA). Spinal anesthesia was the first choice for elective and urgent cesarean section for 95% of the responders. Adequate prevention of aspiration syndrome was prescribed by 78%. After cesarean section, a multimodal analgesic regimen was prescribed by 74%. Conclusion: When comparing these results with those of the two previous Swiss surveys [1, 2], it clearly appears that Swiss obstetric anesthetists have progressively adapted their practice to current clinical recommendations. But this survey also revealed some insufficiencies: 1. Of the public health system: a. Insufficient number of obstetric anesthetists on site 24 hours/24. b. Lack of budget in some hospitals to purchase PCEA pumps. 2. Of individual medical practice: a. Frequent excessive dosage of hyperbaric bupivacaine during spinal anesthesia for cesarean section. b. Frequent use of cristalloid preload before spinal anesthesia for cesarean section. c. Frequent systematic use of opioids when inducing general anesthesia for cesarean section. d. Fentanyl as the first choice opioid during induction of general anesthesia for severe preeclampsia. In the future, wider and more systematic information campaigns by the mean of the Swiss Association of Obstetric Anesthesia (SAOA) should be able to correct these points.

OBJECTIVES Primary intestinal lymphangiectasia is a lymphatic system's disorder, where lymphatic drainage is blockaged. Clinically it produces malabsorption, protein-losing enteropathy, hypogammaglobulin in blood, and several degrees of malnutrition. Its treatment is not easy and includes dietetic-therapy and drugs. MATERIAL AND METHOD A 35-year-old-woman case report is exposed. She has recurrent chylosa ascites, requiring several admissions and evacuatory paracentesis. After food-fat was replaced by medium-chain triacyl-glicerol-enriched diet, a clinical, analytical and anthropometric improvement was demonstrated. CONCLUSIONS The major way of treatment in intestinal lymphangiectasia in this case is the employement of specific-diet and adaptaded-basic-food. It's difficult and high collaboration of the patient is required, being necessary medical revisions during the whole life, due to the not well known evolution of this long-standing disease.

Hemodialysis patients present an increase in plasma homocysteine (Hcy) due to methylation impairment caused by uremia and the deficiency of the co-factors needed (vitamin B, folic acid). This correlates with a more common development of premature vascular disease. There is no consensus on the therapy, with a poor response to oral administration of conventional doses of folic acid. In this work, we assessed the response of hyperhomocysteinemia in 73 regular hemodialysis patients after the administration of 50 mg of parenteral folinic acid for 18 months. Plasma homocysteine of the patients at the time of the study beginning presented mean values of 22.67 (micromol/L). During the first year of supplementation the mean value was kept at 20 micromol/L. From the first year to the end of the 18-months observation period the mean homocysteine levels were 19.58 micromol/L. Although we found a clear trend towards a decrease in plasma homocysteine levels during the treatment period, there were no significant differences. Homocysteine levels did not come back to normal in none of the patients treated.

The purpose of the study was to know about the expectations had by chronic patients within th e Public Healthcare System in Andalusi a, concerning commu nication and making deci sions with the med ical professionals who take care of them duri ng the entire treatment process. A qualitative study was carried out in Andalusia, based on 19 i n-depth interviews and 17 focus groups with 15 4 chronic patients and their relatives, suffering from seven different types of illness, of which the content was later analyzed based on the type of disease (pathologies), and the level of care (Primary, Sp eci aliz ed or Em ergen cy Ser vices). The study shows that patients i ncl uded in all healthcare processes want information about their illness. In Primary Care, they value continued attention, closeness and trust as well as being referred to a proper specialist. In Specialized Care, they expect to receive trustworthy information, proper tests and a correct diagnosis, friendly treatment and to be listened to. In the Emergency Room, lack of tim e and overcrowding d i sru pt proper communic ation. The type of chronic illne ss i nfluenc es patients’ expectations in terms of communication and decisionmaking. Communic ation between patients and medical personnel and the decision-making process related to chronic illness are crucial for the improvement of the qualit y o f healthcare. The type of chr onic disease marks diffe rent expectation s about the communication and decision-making. The communication and the decision making in the chronic assistance is a priority to improve the quality of the assistance

The Andalusian Public Health System (SSPA) is considering the last time an attempt urgent process management through triage consultation, both hospital and primary care environment and tables situations in which the nurse responsible for these consultations can carry out a final statement of which only she is directly responsible through their independent intervention and referral (Triage Advanced). Pose, at once and consistently to the idea of teamwork, where they can be the limits to that intervention finalist and the circuits to follow. This paper proposes a definition line of one of those situations through triage concepts universally tested, and takes full advantage of advanced practice profile offered by nurses Device Critical Care (DCCU) of the SSPA and any the emerging legal and regulatory framework in terms of standardized collaborative prescription, us know legitimate receivers. This work stems from the vision of professionals and our contribution to that line of institutional work that must be consensus.
