Abstract: Preferential flow and transport through macropores affect plant water use efficiency and enhance leaching of agrochemicals and the transport of colloids, thereby increasing the risk for contamination of groundwater resources. The effects of soil compaction, expressed in terms of bulk density (BD), and organic carbon (OC) content on preferential flow and transport were investigated using 150 undisturbed soil cores sampled from 15 × 15–m grids on two field sites. Both fields had loamy textures, but one site had significantly higher OC content. Leaching experiments were conducted in each core by applying a constant irrigation rate of 10 mm h−1 with a pulse application of tritium tracer. Five percent tritium mass arrival times and apparent dispersivities were derived from each of the tracer breakthrough curves and correlated with texture, OC content, and BD to assess the spatial distribution of preferential flow and transport across the investigated fields. Soils from both fields showed strong positive correlations between BD and preferential flow. Interestingly, the relationships between BD and tracer transport characteristics were markedly different for the two fields, although the relationship between BD and macroporosity was nearly identical. The difference was likely caused by the higher contents of fines and OC at one of the fields leading to stronger aggregation, smaller matrix permeability, and a more pronounced pipe-like pore system with well-aligned macropores.

A novel electrochemical sensor for ochratoxin A (OTA) detection was fabricated through the modification of a glassy carbon electrode (GCE) with multiwalled carbon nanotubes (MWCNTs) and a molecularly imprinted polymer (MIP). The MWCNTs dramatically promoted the sensitivity of the developed sensor, while polypyrrole (PPy) imprinted with OTA served as the selective recognition element. The imprinted PPy film was prepared by electropolymerization of pyrrole in the presence of OTA as a template molecule via cyclic voltammetry (CV). The electrochemical oxidation of OTA at the developed sensor was investigated by CV and differential pulse voltammetry (DPV). The developed MIP/MWCNT/GCE sensor showed a linear relationship, when using DPV, between peak current intensity and OTA concentration in the range between 0.050 and 1.0 μM, with limits of detection (LOD) and quantification of 0.0041 μM (1.7 μg/L) and 0.014 μM (5.7 μg/L) respectively. With the developed sensor precise results were obtained; relative standard deviations of 4.2% and 7.5% in the evaluation of the repeatability and reproducibility, respectively. The MIP/MWCNT/GCE sensor is simple to fabricate and easy to use and was successfully applied to the determination of OTA in spiked beer and wine samples, with recoveries between 84 and 104%, without the need of a sample pre-treatment step.

An 80 years old man suffered a cardiac arrest shortly after arrival to his local health department. Basic Life Support was started promptly and nine minutes later, on evaluation by an Advanced Life Support team, the victim was defibrillated with a 200J shock. When orotracheal intubation was attempted, masseter muscle contraction was noticed: on reevaluation, the victim had pulse and spontaneous breathing.Thirty minutes later, the patient had been transferred to an emergency department. As he complained of chest pain, the ECG showed a ST segment depression in leads V4 to V6 and laboratory tests showed cardiac troponine I slightly elevated. A coronary angiography was performed urgently: significant left main plus three vessel coronary artery disease was disclosed.Eighteen hours after the cardiac arrest, a quadruple coronary artery bypass grafting operation was undertaken. During surgery, a fresh thrombus was removed from the middle left anterior descendent artery. Post-operative course was uneventful and the patient was discharged seven days after the procedure. Twenty four months later, he remains asymptomatic.In this case, the immediate call for the Advanced Life Support team, prompt basic life support and the successful defibrillation, altogether, contributed for the full recovery. Furthermore, the swiftness in the detection and treatment of the acute reversible cause (myocardial ischemia in this case) was crucial for long-term prognosis.

É apresentada uma revisão teórica sobre Hérnia Diafragmática Congénita (HDC). Os autores realizam um estudo retrospectivo de 12 casos de HDC referenciados ou diagnosticados na Unidade de Ecografia da Maternidade Dr. Alfredo da Costa (MAC) entre os anos de 1996 a 1999. Após avaliação o aconselhamento foi efectuado, caso a caso, sendo proposta Interrupção Médica da Gravidez ou cirurgia pós-natal.

Objectivos: Estudar a prevalência, factores de risco, evolução clínica e abordagem terapêutica da sífilis congénita em recém-nascidos (RN) de risco, nascidos numa maternidade de referência com apoio perinatal diferenciado. Método: Realizou-se um estudo transversal para cálculo de prevalência à nascença de sífilis congénita, entre Janeiro de 1993 e Dezembro de 2004, através de recolha de dados registados nos processos clínicos das mães e respectivos RN. De acordo com os critérios definidos pelo Centers for Disease Control and Prevention (CDC) em 1989, os RN filhos de mãe com VDRL e/ou TPHA positivo foram divididos em três grupos de risco. Resultados: Foram identificados 467 recém-nascidos, verificando-se que a prevalência de risco de sífilis congénita à nascença se tem mantido ao longo dos anos (5,6‰). A maioria dos recém-nascidos (65%) enquadra-se no grupo de maior risco. Dezanove RN (4%) apresentaram sífilis congénita sintomática ao nascimento, a maioria pertencente ao grupo de maior risco. Outros factores de risco encontrados foram a gravidez não-vigiada, em 30% das mães, toxicodependência em 9%, coinfecção por vírus da hepatite B em 5%, por vírus da hepatite C em 4,7% e por vírus de imunodeficiência humana em 3,4% dos casos. Em alguns casos existia mais do que um factor de risco associado. Conclusões: Verificou-se que a prevalência de risco de sífilis congénita não sofreu grandes variações ao longo dos doze anos, pelo que a sífilis continua a constituir um problema de Saúde Pública em Portugal, com custos económicos e sociais.

Objectivo: revisão de casos de colesteatoma em idade pediátrica do HFF durante um período de 12 anos. Desenho do estudo: retrospectivo. Material e métodos: análise de 30 casos de cirurgia para colesteatoma em doentes dos 0 - 18 anos de Janeiro de 2000 a Dezembro de 2011. Resultados: o colesteatoma inicial foi extenso na maioria dos casos. Realizou-se mastoidectomia com técnica aberta (66,7%), mastoidectomia com técnica fechada (26,7%), aticomia transmeática (3,3%) e remoção de colesteatoma da membrana timpânica (3,3%). O GAP pós-operatório médio foi de 22,5 dB, com GAP 0-10dB em 16,7%, GAP 11-20dB em 10%, GAP 21-30 dB em 30%, mais de 30dB em 16,7% e desconhecido em 26,7%. A taxa de recorrência de colesteatoma foi de 23,3% Conclusões: Na nossa amostra o colesteatoma foi extenso e agressivo. Realizámos mastoidectomia com técnica aberta na maioria dos casos. Para obter bons resultados a abordagem deverá ser individualizada, atendendo a factores clínicos, anatómicos e capacidade de adesão à terapêutica.

Background: Approximately 5% of the population donates blood each year in developed countries. Recruiting and maintaining a pool of altruistic and healthy blood donors is a challenging task. Blood donation as a dynamic process must naturally co-exist with the arguably essential deferrals. Aims: To analyse a 11-year cohort of donors and blood donations in order to determine the profile of the average donor and the typical donation. Characterize the donor’s population in terms of gender, age, number of donations, most common causes for deferral and exclusion and the possible relationships between them. Establish the tendency flow of donations per year. Methods: Analysis of 95861 blood donations from 31550 donors collected between 2000 and 2010 (11 years) in the Immunohemotherapy Department of the ‘‘Centro Hospitalar Lisboa Central - Hospital de Sa˜o Jose´’’ (Lisboa, Portugal). Prior to blood donation, donors were required to fill out a form of informed consent, a questionnaire of 21 ‘‘yes or no’’ questions and were submitted to a clinical assessment and physical examination including: measurement of weight, blood pressure, pulse and capillary hemoglobin levels. Post-donation, the collected blood was tested for ALT elevation and blood-borne viral agents (HBV, HCV, HIV 1 and 2 and HTLV 1 and 2) and other infections (Treponema pallidum). Blood donors and donations were registered in a database and statistically studied in terms of: gender and age distribution, number of donations, most common causes for deferral and exclusion. The frequency of blood donations throughout the period of observation was analyzed and statistically significant relationships between the collected variables were investigated. Results: From the population of 31550 donors 61% were male and a mean age of 41.5 years (± 12.5 years) was found. From the total of 95682 blood donations collected 78% were successful while the most common causes for deferral were: donation incompatible hemoglobin levels (5% of the blood donations and 22% of deferrals), ALT elevation (3% and 14%), positive blood screening test for Treponema pallidum (1% and 6%), medication (1% and 4%), positive serological blood markers for HBV (1% and 4%), endoscopy in the previous 12 months (1% and 3%), arterial hypertension (1% and 3%), infectious conditions (1% and 3%), influenza or influenza-like symptoms (1% and 2%) and positive serological blood markers for HCV (1% and 2%). Summary/Conclusions: Several factors may have contributed to a limited number of new regular donors in the population, namely: ageing population, the alienation of the individual from the community induced by modern lifestyles and job precariousness. It is of the utmost importance to refine our blood donation campaigns according to the existing population of donors. The optimization of the blood donation potential of a population of donors must be achieved through the development of reliable and consistent screening methods. In order to appeal to new donors it is important to promote blood donations considering the profile of the regular and healthy blood donor of the existing population.

Os autores analisam os processos clínicos dos doentes com carcinoma do pulmão, internados no Serviço 1 de Medicina do Hospital de Santo António dos Capuchos, num período de 12 anos (1978-1989). Dos 112 doentes considerados, 50% tinham confirmação histológica, os restantes tinham forte evidência clínico-radiológica, endoscópica e/ou necrópsica ou pesquisa positiva de células neoplásicas na expectoração, secreções brônquicas ou no líquido pleural. Apresentam-se os aspectos clínicos, meios complementares de diagnóstico utilizados, e realça-se o estado geralmente avançado da doença, a dificuldade do seu diagnóstico histológico e a elevada mortalidade (55,4%).

Objectivo — Conhecer a epidemiologia da meningite bacteriana em recém-nascidos admitidos na Unidade de Cuidados Intensivos Neonatais do Hospital de Dona Estefânia. Doentes e métodos — Foi feita a revisão dos processos de recém-nascidos admitidos na Unidade de Cuidados Intensivos Neonatais do Hospital de Dona Estefânia de Janeiro de 1985 a Dezembro de 1996 — 12 anos, provenientes da maternidade do Hospital ou do exterior. Foram excluídas as infecções congénitas e as crianças com idade superior a 28 dias. Definiu-se como precoce a infecção com início nas primeiras 72 horas de vida. Resultados — Houve 36 casos de meningite bacteriana correspondendo a 1,1% das admissões. A incidência de meningite bacteriana precoce na Maternidade do Hospital foi 0,13 por mil nados-vivos. Vinte e quatro crianças eram do sexo masculino (66,7%), 7 eram pré-termo, 4 de baixo peso e 1 de muito baixo peso. Dez recém-nascidos tiveram meningite precoce (27,8%) e 26 (72,2%) meningite tardia. Houve isolamento do agente bacteriano no líquido cefalorraquidiano em 27 crianças (77,1%): E. coli (n=7); Streptococcus do grupo B(SGB) (n=6); Klebsiella pneumoniae (n=3); Proteus mirabilis (n=2), Listeria monocytogenes (n=1), Streptococcus bovis (n=1), Staphylococcus aureus (n=1), Neisseria meningitidis (n=2) e Salmonella tiphy (n=1). Houve ainda o isolamento de 3 Gram negativos não identificados. A hemocultura foi positiva em 19 de 32 colheitas (59,4%). Na ausência de terapêutica antibiótica, em 6 casos a cultura do líquor foi positiva e a hemocultura negativa e noutros 2 ambas as culturas foram negativas. Durante o internamento faleceram 9 recém-nascidos — mortalidade de 25% e em 11 foram detectadas sequelas. Conclusão — Houve um predomínio de casos de meningite tardia, em recém-nascidos de termo e do sexo masculino. Os agentes mais frequentemente encontrados foram a E. coli e a Streptococcus do grupo B.

PURPOSE: To determine the correlation between ocular blood flow velocities and ocular pulse amplitude (OPA) in glaucoma patients using colour Doppler imaging (CDI) waveform analysis. METHOD: A prospective, observer-masked, case-control study was performed. OPA and blood flow variables from central retinal artery and vein (CRA, CRV), nasal and temporal short posterior ciliary arteries (NPCA, TPCA) and ophthalmic artery (OA) were obtained through dynamic contour tonometry and CDI, respectively. Univariate and multiple regression analyses were performed to explore the correlations between OPA and retrobulbar CDI waveform and systemic cardiovascular parameters (blood pressure, blood pressure amplitude, mean ocular perfusion pressure and peripheral pulse). RESULTS: One hundred and ninety-two patients were included [healthy controls: 55; primary open-angle glaucoma (POAG): 74; normal-tension glaucoma (NTG): 63]. OPA was statistically different between groups (Healthy: 3.17 ± 1.2 mmHg; NTG: 2.58 ± 1.2 mmHg; POAG: 2.60 ± 1.1 mmHg; p < 0.01), but not between the glaucoma groups (p = 0.60). Multiple regression models to explain OPA variance were made for each cohort (healthy: p < 0.001, r = 0.605; NTG: p = 0.003, r = 0.372; POAG: p < 0.001, r = 0.412). OPA was independently associated with retrobulbar CDI parameters in the healthy subjects and POAG patients (healthy CRV resistance index: β = 3.37, CI: 0.16-6.59; healthy NPCA mean systolic/diastolic velocity ratio: β = 1.34, CI: 0.52-2.15; POAG TPCA mean systolic velocity: β = 0.14, CI 0.05-0.23). OPA in the NTG group was associated with diastolic blood pressure and pulse rate (β = -0.04, CI: -0.06 to -0.01; β = -0.04, CI: -0.06 to -0.001, respectively). CONCLUSIONS: Vascular-related models provide a better explanation to OPA variance in healthy individuals than in glaucoma patients. The variables that influence OPA seem to be different in healthy, POAG and NTG patients.
