BACKGROUND AND PURPOSE: Obesity and physical inactivity are independently associated with physical and functional limitations in older adults. The current study examines the impact of physical activity on odds of physical and functional limitations in older adults with central and general obesity. METHODS: Data from 6279 community-dwelling adults aged 60 years or more from the Health and Retirement Study 2006 and 2008 waves were used to calculate prevalence and odds of physical and functional limitation among obese older adults with high waist circumference (waist circumference ≥88 cm in females and ≥102 cm in males) who were physically active versus inactive (engaging in moderate/vigorous activity less than once per week). Logistic regression models were adjusted for age, sex, race/ethnicity, education, smoking status, body mass index, and number of comorbidities. RESULTS: Physical activity was associated with lower odds of physical and functional limitations among older adults with high waist circumference (odds ratio [OR], 0.59; confidence interval [CI], 0.52-0.68, for physical limitations; OR, 0.52; CI, 0.44-0.62, for activities of daily living; and OR, 0.44; CI, 0.39-0.50, for instrumental activities of daily living). CONCLUSIONS: Physical activity is associated with significantly lower odds of physical and functional limitations in obese older adults regardless of how obesity is classified. Additional research is needed to determine whether physical activity moderates long-term physical and functional limitations.

There is compelling evidence for the effectiveness of home-based occupational therapy and physiotherapy rehabilitation for community dwelling elderly who may struggle with basic activities and the functions of daily living and mobility. Nonetheless, an estimated 2% of home care’s elderly clients receive these therapies. Ontario’s home care data indicates that 78% of clients that could benefit from these specific therapies are not receiving them. The study examined a subset of elderly clients receiving home care following a hospital discharge during 2009-2010. The aim of this study was to: understand the difference between those home care clients who received occupational therapy or physiotherapy and those who did not; and determine if receiving these therapies impacted the utilization of hospital emergency departments and inpatient admissions. A retrospective cohort design and multivariate and survival analysis of hospital and home care administrative data structured the study. Results suggest that home-based rehabilitation is offered to a minority of the home care population. Distinct client characteristics and process variables significantly associated with the increased likelihood of receiving home-based occupational and physical therapies included: clients who were older, females, admitted to home care from hospital inpatient units, assessed as non-acute for clinical and service needs and required more home making support and assistance with activities of daily living. Almost one quarter of the total sample returned to hospital. Visits to emergency departments accounted for the greater part of hospital utilization and primarily for sub-acute general symptoms and signs, post-procedural complications, infections or acute episodes from chronic obstructive pulmonary disease and renal failure. Slightly over half of the clients returning to hospital did not receive home-based rehabilitation. Clients who received occupational therapy returned to the hospital sooner following their home care admission whereas clients receiving physiotherapy spent the longest time before rehospitalizing. The majority of the clients receiving occupational therapy were admitted to home care having just resolved sub-acute conditions or symptoms, many of which are known to influence functional and physical decline. Moreover, analysis of process variables indicated that the wait time for a referral to occupational therapy was two times longer compared to physiotherapy. These same clients also waited, on average, over one month before an occupational therapist’s first visit. The need to discriminate who receives home-based rehabilitation is essential to understanding how specific therapies contribute to improving systems outcomes. This study is the first examination that focuses specifically on home-based occupational therapy and physiotherapy rehabilitation and the client characteristics and process variables associated with receiving/not receiving these therapies and the impact these factors have on the time-to-rehospitalization.

BACKGROUND: Current evidence supports the use of exercise-based treatment for chronic low back pain that encourages the patient to assume an active role in their recovery. Walking has been shown it to be an acceptable type of exercise with a low risk of injury. However, it is not known whether structured physical activity programmes are any more effective than giving advice to remain active.

METHODS/DESIGN: The proposed study will test the feasibility of using a pedometer-driven walking programme, as an adjunct to a standard education and advice session in participants with chronic low back pain. Fifty adult participants will be recruited via a number of different sources. Baseline outcome measures including self reported function; objective physical activity levels; fear-avoidance beliefs and health-related quality of life will be recorded. Eligible participants will be randomly allocated under strict, double blind conditions to one of two treatments groups. Participants in group A will receive a single education and advice session with a physiotherapist based on the content of the 'Back Book'. Participants in group B will receive the same education and advice session. In addition, they will also receive a graded pedometer-driven walking programme prescribed by the physiotherapist. Follow up outcomes will be recorded by the same researcher, who will remain blinded to group allocation, at eight weeks and six months post randomisation. A qualitative exploration of participants' perception of walking will also be examined by use of focus groups at the end of the intervention. As a feasibility study, treatment effects will be represented by point estimates and confidence intervals. The assessment of participant satisfaction will be tabulated, as will adherence levels and any recorded difficulties or adverse events experienced by the participants or therapists. This information will be used to modify the planned interventions to be used in a larger randomised controlled trial.

DISCUSSION: This paper describes the rationale and design of a study which will test the feasibility of using a structured, pedometer-driven walking programme in participants with chronic low back pain.

Background: Clinical trials have shown the benefits of cholinesterase inhibitors for the treatment of mild-to-moderate Alzheimer's disease. It is not known whether treatment benefits continue after the progression to moderate-to-severe disease. Methods: We assigned 295 community-dwelling patients who had been treated with donepezil for at least 3 months and who had moderate or severe Alzheimer's disease (a score of 5 to 13 on the Standardized Mini-Mental State Examination [SMMSE, on which scores range from 0 to 30, with higher scores indicating better cognitive function]) to continue donepezil, discontinue donepezil, discontinue donepezil and start memantine, or continue donepezil and start memantine. Patients received the study treatment for 52 weeks. The coprimary outcomes were scores on the SMMSE and on the Bristol Activities of Daily Living Scale (BADLS, on which scores range from 0 to 60, with higher scores indicating greater impairment). The minimum clinically important differences were 1.4 points on the SMMSE and 3.5 points on the BADLS.
Results: Patients assigned to continue donepezil, as compared with those assigned to discontinue donepezil, had a score on the SMMSE that was higher by an average of 1.9 points (95% confidence interval [CI], 1.3 to 2.5) and a score on the BADLS that was lower (indicating less impairment) by 3.0 points (95% CI, 1.8 to 4.3) (P<0.001 for both comparisons). Patients assigned to receive memantine, as compared with those assigned to receive memantine placebo, had a score on the SMMSE that was an average of 1.2 points higher (95% CI, 0.6 to 1.8; P<0.001) and a score on the BADLS that was 1.5 points lower (95% CI, 0.3 to 2.8; P = 0.02). The efficacy of donepezil and of memantine did not differ significantly in the presence or absence of the other. There were no significant benefits of the combination of donepezil and memantine over donepezil alone.
Conclusions: In patients with moderate or severe Alzheimer's disease, continued treatment with donepezil was associated with cognitive benefits that exceeded the minimum clinically important difference and with significant functional benefits over the course of 12 months. Developmental coordination disorder (DCD) is defined as an impairment in the development of motor coordination that interferes with academic achievement or activities of daily living (DSM-IV). DCD has been reported to affect 5% to 9% of children in the normal population. This study describes the prevalence of DCD in a cohort of extremely low birth weight children (ELBW, <or = l800 g) at 8.9 years of age, from which were excluded children with major impairments. Seventy-three children were included in the study group, along with 18 term-born, socially matched controls. Of the 73 ELBW children, 37 (51%) were classified as having DCD. ELBW children with DCD also had significantly lower Performance IQ (PIQ) scores and were more likely (43%) to have a learning difficulty in arithmetic than ELBW children who did not have DCD. This study found that DCD is a common problem in school-aged ELBW children.

Children's judgements about pain at age 8-10 years were examined comparing two groups of children who had experienced different exposure to nociceptive procedures in the neonatal period: extremely low birthweight (ELBW) <or = 1000 g (N = 47) and full birthweight (FBW) > or = 2500 g (N = 37). The 24 pictures that comprise the Pediatric Pain Inventory, depicting events in four settings: medical, recreational, daily living, and psychosocial, were used as the pain stimuli. The subjects rated pain intensity using the Color Analog Scale and pain affect using the Facial Affective Scale. Child IQ and maternal education were statistically adjusted in group comparisons. Pain intensity and pain affect related to activities of daily living and recreation were significantly higher than psychosocial and medically related pain on both scales in both groups of children. Although the two groups of children did not differ overall in their perceptions of pain intensity or affect, the ELBW children rated medical pain intensity significantly higher than psychosocial pain, unlike the FBW group. Also, duration of neonatal intensive care unit stay for the ELBW children was related to increased pain affect ratings in recreational and daily living settings. Despite altered response to pain in the early years reported by parents, on the whole at 8-10 years of age ELBW children judged pain in pictures similarly to their term peers. However, differences were evident, which suggests that studies are needed of biobehavioural reactivity to pain beyond infancy, as well as research into beliefs, attitudes, and perceptions about pain during the course of childhood in formerly ELBW children.

The purpose of this article is to determine whether middle-aged and older adults would identify community support services (CSSs) as a source of assistance for difficulties with the instrumental activities of daily living (IADLs). Furthermore, we determine factors related to the identification of home health and CSSs. Telephone interviews were conducted with 768 adults aged 50 and older. Respondents were presented with a vignette describing a situation where loss of independence is threatened. They were asked what they would do in that situation. Although less than 20% mentioned CSSs, nearly 50% mentioned either a home health or CSS. Findings suggest those less likely to mention a home health or CSS include men, older adults, and the foreign born. In addition, those with less education, functional health limitations, no social support, and a lack of knowledge of where to find information about CSSs mentioned home health or CSSs less often. © The Author(s) 2010.

This paper examines the relationship between stature and later life health in 6 emerging economies, each of which are expected to experience significant increases in the mean age of their populations over the coming decades. Using data from the WHO Study on Global Ageing and Adult Health (SAGE) and pilot data from the Longitudinal Ageing Study in India (LASI), I show that various measures of health are associated with height, a commonly used proxy for childhood environment. In the pooled sample, a 10 cm increase in height is associated with between a 2 and 3 percentage point increase in the probability of being in very good or good self-reported health, a 3 percentage point increase in the probability of reporting no difficulties with activities of daily living or instrumental activities of daily living, and between a fifth and a quarter of a standard deviation increase in grip strength and lung function. Adopting a methodology previously used in the research on inequality, I also summarise the height-grip strength gradient for each country using the concentration index, and provide a decomposition analysis.

PURPOSE: Presbyopia limits activities of daily living, but population-based data from rural China are scarce. METHODS: A population-based, cross-sectional study was conducted in 2009 among all persons aged 40+ years in a rural area near Shenyang, China. Distance and near VA were measured using logMAR E charts. Individuals with pinhole-corrected distance vision ≥20/63 underwent detailed eye examination and near refraction. RESULTS: A total of 1008 (91.5%) respondents were examined (mean age, 58.4 ± 10.7 years for men, 56.8 ± 9.89 years for women). Women and older subjects were more likely to participate. The prevalence of functional presbyopia (near vision <20/50 [N8] improved by ≥1 line with correction) was 67.3% (95% confidence interval [CI], 64.30%-70.09%), increasing from 27.6% at 40 to 49 years of age to 81.8% at 60 to 69 years. Multivariate analysis showed that older age (P < 0.001), but not gender or education, was significantly associated with a higher risk of presbyopia. Self-reported presbyopic spectacle correction coverage was 51.5%. In multivariate logistic regression models, worse presenting near vision (P = 0.013) and higher required spherical equivalent power (P < 0.001) were associated with having correction, while age, gender, education, and distance vision were unassociated. Major barriers reported by persons without near correction included poor quality of available glasses (33.1%) and lack of awareness of the condition and its treatment (28.8%). CONCLUSIONS: Presbyopia is highly prevalent in rural China, and nearly half of affected persons have no access to correction. Interventions should focus on education and improvement in the quality of refractive services.

Pretende-se perceber se existe uma relação entre o nível regular de actividade física praticado pelas pessoas idosas e a sua autonomia instrumental. Desenvolveu-se um estudo descritivo, correlacional e transversal, no ano 2010, com amostra intencional constituída por 100 indivíduos com idade igual ou superior a 60 anos, de ambos sexos. A colheita de dados foi efectuada em dois ginásios e dois centros de dia localizados na cidade de Lisboa, e foram utilizados como instrumentos a Escala de Actividades Instrumentais de Vida Diária de Lawton e Brody e o International Physical Activity Questionnaire. Obteve-se uma média de idades de 76 anos, sendo 78% dos inquiridos do sexo feminino. Inseriram-se 30% na categoria de actividade física “ligeira”, 45% na “moderada” e 25% na “vigorosa”. Relativamente à autonomia instrumental, a maioria era moderadamente dependente. Verificou-se que o grau de autonomia instrumental dos idosos aumenta com o incremento da actividade física praticada (ρS=0.815; p-value<0.001); Abstract: This work seeks to realize if there is a relationship between the level of regular physical activity practiced by elderly persons and its instrumental autonomy. A descriptive, related and transversal study was developed in the year 2010, with an intentional sample consisting of 100 individuals, aged 60 years or above, of both genders. The data collection was carried out in two gymnasiums and two day care centers located in the city of Lisbon, and were used the Lawton and Brody’s Instrumental Activities of Daily Living Scale and the International Physical Activity Questionnaire. A mean age of 76 years was obtained and 78% of the respondents were female. 30% were classified as having a "low" physical activity level, 45% as "moderate" and 25% as "high". Regarding instrumental autonomy, most were moderately dependent. It was verified that the degree of instrumental autonomy of the elderly increases with increased physical activity (ρS=0.815; p-value<0.001).

O envelhecimento da população será um dos fenómenos mais importantes das próximas décadas, afectando as estruturas de apoio aos idosos assim como às suas famílias. É um fenómeno importante porque pessoas de diferentes idades, com diferentes capacidades, interesses e necessidades apresentam diferentes problemas de saúde e necessitam diferentes soluções e abordagens. Assim um cenário denominado de demografia da idade, ou gray power, emergiu no último século. Da literatura emerge a importância da família na resposta às necessidades e dependências de um ente querido idoso. Porém os cuidadores informais envolvem-se totalmente no cuidado sem possuírem o adestramento técnico, o que potencia situações de stress para o cuidador e mesmo para o idoso receptor de cuidados. Na recolha da amostra, foi utilizada a entrevista e o exame físico na aplicação de inquérito. (81 idosos e 86 cuidadores). Foi utilizada a análise de conteúdo nas respostas abertas e análise quantitativa de carácter estatístico, descritivo e inferencial, para o tratamento dos dados. Os resultados sugerem que: a) ambas as amostras manifestam problemas de saúde, porém os idosos apresentam situações mais graves; b) os familiares dos idosos, pelo acto de cuidar possuem elevados padrões de stress; c) os cuidadores informais necessitam de apoio técnico para desempenharem as suas funções. Como conclusão refere-se que o stress vivido pelo cuidador interfere na sua saúde e qualidade de vida, necessitando em consequência de apoio emocional e institucional; para muitos idosos dependentes existe a real necessidade dos Cuidados Continuados de proximidade e de Cuidados Paliativos.

O presente trabalho pretende caracterizar a associação existente entre a função cognitiva executiva e a capacidade para o trabalho em profissionais de saúde (médicos e enfermeiros) e profissionais de educação (professores). A função cognitiva executiva é definida como uma série de processos cognitivos de ordem superior (capacidade de planeamento, raciocínio abstrato, flexibilidade cognitiva e resolução de problemas) determinantes no controlo e coordenação de operações cognitivas e fundamentais na organização e monitorização do comportamento humano. A integridade destas funções, são determinantes para a realização adequada de tarefas da vida diária, incluindo o contexto organizacional. A capacidade para trabalho é um forte preditor do desempenho laboral, sendo definida como a autoavaliação que o trabalhador faz do seu bem-estar no presente e no futuro próximo e da capacidade para assegurar o seu trabalho tendo em conta as exigências do mesmo, a saúde e os recursos psicológicos e cognitivos disponíveis. Assim, com o objetivo de compreender a relação entre estas duas variáveis em médicos, enfermeiros e professores, no presente trabalho utilizamos uma amostra composta por 218 sujeitos, sendo que 93 são enfermeiros, 100 professores (ensino secundário) e 25 médicos. Para avaliar as funções cognitivas executivas, nomeadamente a flexibilidade cognitiva e raciocínio abstrato não-verbal utilizamos o Halstead Category Test (HCT). Para avaliar a capacidade de planeamento e resolução de problemas, utilizamos a Torre de Hanoi (TH). Para determinamos o valor da capacidade para o trabalho, utilizamos o índice de capacidade para o trabalho. No sentido de controlar variáveis que poderiam influenciar esta relação, utilizamos Questionário Geral de Saúde (GHQ-12), escala de ansiedade-traço, Questionário de Personalidade de Eysenck, escala de satisfação no trabalho e uma questão dicotómica (Sim/Não) sobre o trabalho por turnos. Pela análise dos resultados, verificamos que alterações nas funções cognitivas executivas poderão prejudicar a capacidade para o trabalho. No entanto, verificamos que variáveis como a idade, trabalho por turnos, personalidade e saúde mental poderão exercer um efeito moderador desta relação. Por fim, em comparação com médicos, enfermeiros e professores, verificamos que os médicos e enfermeiros apresentam um maior prejuízo nas funções cognitivas executivas que os professores, mas não na capacidade para o trabalho. Como conclusão, o nosso trabalho contribuiu para uma melhor compreensão da ação das funções executivas em contexto laboral (em particular na área da saúde e educação), contribuindo para o desenvolvimento e implementação de programas de promoção de saúde laboral em contexto organizacional.

Osteoarthritis (OA) is one of the leading causes of pain and disability among older adults, particularly women. Pain and mobility disability are major consequences of knee OA which and can interfere with the functional autonomy of elderly and thus, making it difficult to perform activities of daily living. Evidence suggests that obesity is strongly linked to knee OA and that non­pharmacological therapy should be based on physical activity and weight loss in case of overweight and obesity. A positive relationship between adherence to the Mediterranean diet and health outcomes has been widely discussed in scientific literature, including its potential benefits in weight loss.

A Obesidade é considerada um grave problema de saúde pública, com consequências negativas para os indivíduos obesos, nomeadamente dificuldades no desempenho de atividades de vida diária, na locomoção e na prática de exercício físico, o que pode restringir a participação em atividades sociais e de lazer. Esta investigação consiste num estudo quantitativo descritivo e tem como objetivo principal descrever de que forma os indivíduos adultos obesos classificam a sua adaptação ocupacional, a partir dos conceitos de identidade e competência ocupacional. Com este estudo, pretende-se ainda verificar se os indivíduos obesos apresentam níveis de atividade física mais baixos e valores mais elevados de pressão plantar, em relação a indivíduos com peso normal. A amostra é constituída por dez indivíduos adultos, de ambos os sexos, com índice de massa corporal igual (IMC) ou superior a 30 Kg/m2, e os instrumentos de avaliação utilizados são o Questionário Ocupacional (adaptado por N. Riopel com assistência de G. Kielhofner e J. Hawkins Watts – 1986), o IPAQ – Versão Curta e o Sistema de Palmilhas Pedar. A partir dos resultados, pode-se verificar que os indivíduos obesos apresentam uma rotina diária em que a maioria das atividades realizadas está relacionada com a casa, o trabalho e o descanso e que a percentagem de atividades de lazer em que participam é reduzida. No entanto, parecem satisfeitos com o seu desempenho na maior parte das atividades, consideram que muitas delas são importantes para si e estão motivados para as realizar. Dos resultados obtidos, podemos sugerir que os indivíduos obesos apresentam boa adaptação ocupacional. Pode-se ainda dizer que os indivíduos obesos apresentam um baixo nível de intensidade de atividade física, não se observando diferenças significativas relativamente aos indivíduos com peso normativo e que os seus valores máximos de pressão plantar normalizados são inferiores quando comparados com a população de peso normal.
