Foi feito um estudo do timo durante a evolução de dois tumores exprimentais, transplantáveis em ratos. Verificou-se uma queda de peso do órgão, associada a um desaparecimento gradual dos linfócitos. Foi discutida a hipótese da migração destas células para o baço e gânglios linfáticos.

Estudou-se a atividade da transaminase glutâmico oxaloacética (TGO) em diferentes tecidos (fígado, músculo, rim, pulmão, baço e soro sanguíneo) de ratos e de soro humano. verificou-se que a atividade da enzima proveniente de qualquer um destes tecidos é aumentada cerca de tr~es vezes quando a incubação se faz a 60ºC, ao invés de 37ºC. São feitas considerações acerca da importância deste fato.

Os autores estudaram 253 ratos domésticos (Rattus norvegicus e Rattus r. frugivorus), infectados experimentalmente (via subcutânea) com duas cepas de Yersinia pestis (cepas "PEXU-19" e "RANGEL"), isoladas de roedores silvestres capturados no nordeste brasileiro (Exú - Pernambuco) e na Venezuela, respectivamente, as quais foram mantidas em meio de cultura e subinoculadas periodicamente. A sobrevida dos animais infectados variou de dois a sete dias, sendo os sobreviventes sacrificados no 20º dia após a data da inoculação. Para cada uma das duas espécies de rato, foram empregadas cargas infectantes de intensidades diferentes (750 - 4.250; 7.500 - 41.250 e 75.000 - 750.000 bacilos), correspondentes a cada uma das duas cepas de Y. pestis utilizadas. A patologia foi estudada em relação ao fígado, baço, pulmão e linfonodo inguinal, órgãos onde as lesões se mostraram mais exuberantes. As lesões hepáticas foram essencialmente representadas por congestão, necrose coagulativa multifocal e infiltração inflamatória aguda portal e sinusoidal, com formação de microabscessos. No baço ocorreu atrofia acentuada dos folículos linfóides de Malpighi, congestão, hemorragia e esplemite aguda abscedada. No pulmão, foi achado freqüente a pneumonite aguda, ás vezes acompanhada de abscessos. De um modo geral, a cepa PEXU-19 revelou maior poder patogênico que a cepa RANGEL, embora, em relação à espécie de roedor infectado não tenha sido possível surpreender diferenças quanto á sua susceptibilidade frente ás duas cepas Y. pestis utilizadas no presente trabalho.

O presente trabalho mostra a histopatologia da infecção pela Yersinia pestis, entre as diferentes espécies de roedores silvestres e comensais (cricetídeos, equimídeos, murídeos e cavídeos) que ocorrem na zona endêmica de peste do Nordeste do Brasil. Estes roedores foram encontrados naturalmente infectados nos campos ou inoculados experimentalmente no laboratório (vias percutânea, subcutânea ou picada de pulgas) com cepas locais e/ou estrangeiras de Yersiniapestis. Quase todos os animais, exceto alguns dos cavídeos, desenvolveram a forma bubosepticêmica da peste. Entre as lesões encontradas, a necrose coagulativa multifocal do fígado, a pneumonite intersticial aguda difusa e a atrofia linfoide do baço, podem, por sua constância, ser consideradas como os principais indicadores histológicos da infecção pestosa, embora estas lesões não sejam exclusivas da peste. A diversidade e a intensidade das lesões entre os Zygodontomys lasiurus pixuna, podem explicar a mortalidade elevada desta espécie e a disseminação da peste nos focos naturais do Nordeste brasileiro. Cricetídeos e murídeos mostraram alterações histopatológicas qualitativamente semelhantes. A resistência dos cavídeos à infecção pestosa foi evidenciada pela sobrevida desses roedores à fase aguda da infecção e pelo desenvolvimento de uma reação histiocitária interna, delimitando as áreas abscedadas. è possível que estas lesões crônicas abriguem bacilos virulentos, que permitirão a reinfecção periódica das pulgas e conseqüente reativação do processo epizoótico.

Estudou-se o efeito da dieta láctea, por um período de 150 dias em camundongos infectados com diferentes números das formas sangüíneas de Plasmodium berghei, e observou-se o desenvolvimento da imunidade humoral nestes animais pela dosagem das imunoglobulinas das classes IgG e IgM no soro, usando o teste de imunofluorescência indireta. Os resultados indicam que a administração do leite, como único alimento em camundongos, protege-os cotnra infecção malárica fatal, independentemente do número de parasitas inoculados. Os animais desenvolveram altos níveis de anticorpos IgG, os quais persistiram no soro por longo período de tempo. Contudo, os anticorpos IgM somente foram detectáveis no soro durante as primeiras duas semanas de infecção. O P. berghei continua presente na circulação periférica, após dois meses de infecção, uma vez que o sangue destes animasi inoculados em camundongos mantidos em dieta norma, produziu infecção fatal nos recipientes. No entanto, ao exame microscópico não foi possível detectar o parasita da malária no sangue periférico destes animais. O protozoário esteve presente no baço e fígado dos camundongos durante todo o tempo de duração da pesquisa. A presença contínua do P. berghei nestes animais, em nível de infecção subclínica, ofereceu ao hospedeiro o desenvolvimento de uma imunidade sólida contra subseqüente infecção. Esta imunidade adquirida esteve presente, nestes animais, até cinco meses após a infecção.

Como já descrevemos em publicação anterior (Conceição, 1985), foram isoladas 20 amostras de S. mansoni de pacientes do sexo masculino com idades entre 13 e 30 anos, autóctones do distrito de Capitão Andrade, município de Itanhomi, no Vale do Rio Doce, em Minas Gerais. Das amostras, seis eram portadores de esquistossomose-infecção (tipo I), seis da forma hepatointestinal (tipo II) e oito da forma hepatoesplênica (tipo III), adaptadas inicialmente, à B. glabrata da mesma área. Cada uma das amostras foi inoculada em 48 camundongos em lotes de 16, respectivamente com 25,50 e 100 cercárias, mantendo-se 12 animais não infectados, com controles. Após 90 dias perfundiu-se o sistema porta de 12 animais (quatro de cada lote). Os animais mortos naturalmente em diversos períodos e a metade de cada lote sacrificada aos 90 e 180 dias foram estudados através dos seguintes parâmetros: 1§) determinação dos pesos de fígado, baço, pulmão e intestino; 2§) contagem de ovos em intestinos (proximal e mediano) e grosso (distal). O número de vermes obtidos pela perfusão nos três grupos em média de 21,9%, 22% e 17,8%% para os tipos I, II e III. A mortalidde natural média dos camundongos submetidos à infecção com 25, 50 e 100 cercárias, foi respectivamente, 12,4%, 23,2% para o grupo I; de 4,7% 19,3% e 22,2% para o grupo II e 11,4%, 29,5% e 41,6% para o grupo III, apresentando-se, portanto, proporcional aos inóculos. O peso dos órgãos dos animais infectados bem como o número de ovos de S. mansoni foi sempre proporcional ao inóculo e a contagem mais elevada nas partes mediana e proximal do intestino nos três grupos. Concluiu-se que não houve correlação entre as formas clínicas da esquistossomose e o comportamento das amostras de S. mansoni em camundongo, ressaltando-se que as alterações parasitológicas encontradas foram proporcionais ao inóculo empregado e ao tempo de infecção, evidenciando os aspectos quantitativos na determinação da doença.

We analyse the Heston stochastic volatility model under an inversion of spot. The result is that under the appropriate measure changes the resulting process is again a Heston type process whose parameters can be explicitly determined from those of the original process. This behaviour can be interpreted as some measure of sanity of the Heston model but does not seem to be a general feature of stochastic volatility processes.

A new expression for the characteristic function of log-spot in Heston model is presented. This expression more clearly exhibits its properties as an analytic characteristic function and allows us to compute the exact domain of the moment generating function. This result is then applied to the volatility smile at extreme strikes and to the control of the moments of spot. We also give a factorization of the moment generating function as product of Bessel type factors, and an approximating sequence to the law of log-spot is deduced.

Background: The posterior circulation Acute Stroke Prognosis Early CT Score (pc-ASPECTS) and the combined Pons-midbrain score quantify the extent of early ischemic changes in the posterior circulation. We compared the prognostic accuracy of both scores if applied to CT angiography (CTA) source images (CTA-SI) of patients in the Basilar Artery International Cooperation Study (BASICS).Methods: BASICS was a prospective, observational, multi-centre, registry of consecutive patients who presented with acute symptomatic basilar artery occlusion (BAO). Functional outcome was assessed at 1 month. We applied pc-ASPECTS and the combined Pons-midbrain score to CTA-SI by 3-reader-consensus. Readers were blinded to clinical data. We performed multivariable logistic regression analysis, adjusting for thrombolysis, baseline NIHSS score and age, and used the output to derive ROC curves to compare the ability of both scores to discriminate patients with favourable (modified Rankin Scale [mRS] scores 0-3) from patients with unfavourable (mRS scores 4-6) functional outcome.Results: We reviewed CTAs of 158 patients (64% men, mean age 65 _ 15 years, median NIHSS score 25 [0-38], median GCS score 7 [3-15], median onset-to-CTA time 234 minutes [11-7380]). At 1 month, 40 (25%) patients had a favourable outcome, 49 (31%) had an unfavourable outcome (mRS score 4-5) and 69 (44%) were deceased. Both techniques of assessing CTA-SI hypoattenuation in the posterior circulation showed equally good discriminative value in predicting final outcome (C-statistics; area under ROC curve 0.74 versus 0.75, respectively; p_0.37). Pc-ASPECTS dichotomized at _6 versus _6 was an independent predictor of favourable functional outcome (RR _ 2.2; CI95 1.1-4.7; p _ 0.034).Conclusion: Compared to the combined Pons-midbrain score, the pc-ASPECTS score has similar prognostic accuracy to identify patients with a favourable functional outcome in BASICS. OBJECTIVE: To develop a simple prognostic model to predict outcome at 1 month after acute basilar artery occlusion (BAO) with readily available predictors. METHODS: The Basilar Artery International Cooperation Study (BASICS) is a prospective, observational, international registry of consecutive patients who presented with an acute symptomatic and radiologically confirmed BAO. We considered predictors available at hospital admission in multivariable logistic regression models to predict poor outcome (modified Rankin Scale [mRS] score 4-5 or death) at 1 month. We used receiver operator characteristic curves to assess the discriminatory performance of the models. RESULTS: Of the 619 patients, 429 (69%) had a poor outcome at 1 month: 74 (12%) had a mRS score of 4, 115 (19%) had a mRS score of 5, and 240 (39%) had died. The main predictors of poor outcome were older age, absence of hyperlipidemia, presence of prodromal minor stroke, higher NIH Stroke Scale (NIHSS) score, and longer time to treatment. A prognostic model that combined demographic data and stroke risk factors had an area under the receiver operating characteristic curve (AUC) of 0.64. This performance improved by including findings from the neurologic examination (AUC 0.79) and CT imaging (AUC 0.80). A risk chart showed predictions of poor outcome at 1 month varying from 25 to 96%. CONCLUSION: Poor outcome after BAO can be reliably predicted by a simple model that includes older age, absence of hyperlipidemia, presence of prodromal minor stroke, higher NIHSS score, and longer time to treatment.

The members of the genus Acinetobacter are Gram-negative cocobacilli that are frequently found in the environment but also in the hospital setting where they have been associated with outbreaks of nosocomial infections. Among them, Acinetobacter baumannii has emerged as the most common pathogenic species involved in hospital-acquired infections. One reason for this emergence may be its persistence in the hospital wards, in particular in the intensive care unit; this persistence could be partially explained by the capacity of these microorganisms to form biofilm. Therefore, our main objective was to study the prevalence of the two main types of biofilm formed by the most relevant Acinetobacter species, comparing biofilm formation between the different species. Findings: Biofilm formation at the air-liquid and solid-liquid interfaces was investigated in different Acinetobacter spp. and it appeared to be generally more important at 25°C than at 37°C. The biofilm formation at the solid-liquid interface by the members of the ACB-complex was at least 3 times higher than the other species (80-91% versus 5-24%). In addition, only the isolates belonging to this complex were able to form biofilm at the air-liquid interface; between 9% and 36% of the tested isolates formed this type of pellicle. Finally, within the ACB-complex, the biofilm formed at the air-liquid interface was almost 4 times higher for A. baumannii and Acinetobacter G13TU than for Acinetobacter G3 (36%, 27% & 9% respectively). Conclusions: Overall, this study has shown the capacity of the Acinetobacter spp to form two different types of biofilm: solid-liquid and air-liquid interfaces. This ability was generally higher at 25°C which might contribute to their persistence in the inanimate hospital environment. Our work has also demonstrated for the first time the ability of the members of the ACB-complex to form biofilm at the air-liquid interface, a feature that was not observed in other Acinetobacter species.

Despite medical advances, mortality in infective endocarditis (IE) is still very high. Previous studies on prognosis in IE have observed conflicting results. The aim of this study was to identify predictors of in-hospital mortality in a large multicenter cohort of left-sided IE.Methods An observational multicenter study was conducted from January 1984 to December 2006 in seven hospitals in Andalusia, Spain. Seven hundred and five left-side IE patients were included. The main outcome measure was in-hospital mortality. Several prognostic factors were analysed by univariate tests and then by multilogistic regression model. Results.The overall mortality was 29.5% (25.5% from 1984 to 1995 and 31.9% from 1996 to 2006; Odds Ratio 1.25; 95% Confidence Interval: 0.97-1.60; p = 0.07). In univariate analysis, age, comorbidity, especially chronic liver disease, prosthetic valve, virulent microorganism such as Staphylococcus aureus, Streptococcus agalactiae and fungi, and complications (septic shock, severe heart failure, renal insufficiency, neurologic manifestations and perivalvular extension) were related with higher mortality. Independent factors for mortality in multivariate analysis were: Charlson comorbidity score (OR: 1.2; 95% CI: 1.1-1.3), prosthetic endocarditis (OR: 1.9; CI: 1.2-3.1), Staphylococcus aureus aetiology (OR: 2.1; CI: 1.3-3.5), severe heart failure (OR: 5.4; CI: 3.3-8.8), neurologic manifestations (OR: 1.9; CI: 1.2-2.9), septic shock (OR: 4.2; CI: 2.3-7.7), perivalvular extension (OR: 2.4; CI: 1.3-4.5) and acute renal failure (OR: 1.69; CI: 1.0-2.6). Conversely, Streptococcus viridans group etiology (OR: 0.4; CI: 0.2-0.7) and surgical treatment (OR: 0.5; CI: 0.3-0.8) were protective factors.Conclusions Several characteristics of left-sided endocarditis enable selection of a patient group at higher risk of mortality. This group may benefit from more specialised attention in referral centers and should help to identify those patients who might benefit from more aggressive diagnostic and/or therapeutic procedures.

To evaluate the long-term impact of successive interventions on rates of methicillin-resistant Staphylococcus aureus (MRSA) colonization or infection and MRSA bacteremia in an endemic hospital-wide situation. DESIGN:Quasi-experimental, interrupted time-series analysis. The impact of the interventions was analyzed by use of segmented regression. Representative MRSA isolates were typed by use of pulsed-field gel electrophoresis. SETTING:A 950-bed teaching hospital in Seville, Spain. PATIENTS:All patients admitted to the hospital during the period from 1995 through 2008. METHODS:Three successive interventions were studied: (1) contact precautions, with no active surveillance for MRSA; (2) targeted active surveillance for MRSA in patients and healthcare workers in specific wards, prioritized according to clinical epidemiology data; and (3) targeted active surveillance for MRSA in patients admitted from other medical centers. RESULTS:Neither the preintervention rate of MRSA colonization or infection (0.56 cases per 1,000 patient-days [95% confidence interval {CI}, 0.49-0.62 cases per 1,000 patient-days]) nor the slope for the rate of MRSA colonization or infection changed significantly after the first intervention. The rate decreased significantly to 0.28 cases per 1,000 patient-days (95% CI, 0.17-0.40 cases per 1,000 patient-days) after the second intervention and to 0.07 cases per 1,000 patient-days (95% CI, 0.06-0.08 cases per 1,000 patient-days) after the third intervention, and the rate remained at a similar level for 8 years. The MRSA bacteremia rate decreased by 80%, whereas the rate of bacteremia due to methicillin-susceptible S. aureus did not change. Eighty-three percent of the MRSA isolates identified were clonally related. All MRSA isolates obtained from healthcare workers were clonally related to those recovered from patients who were in their care. CONCLUSION:Our data indicate that long-term control of endemic MRSA is feasible in tertiary care centers. The use of targeted active surveillance for MRSA in patients and healthcare workers in specific wards (identified by means of analysis of clinical epidemiology data) and the use of decolonization were key to the success of the program.

We describe a case of bacteremia due to an as yet unclassified Acinetobacter genomic species 17-like strain. The recognition of this microorganism as non-Acinetobacter baumannii may have important epidemiological implications, as it relieves the hospital of the implementation of barrier precautions for patients infected or colonized as may be necessary with a multiresistant A. baumannii epidemic.

Extended-spectrum β-lactamases (ESBLs) form a heterogeneous group that share the property of hydrolytic activity against the oxyimino-β-lactams while remaining susceptible to inhibition by β-lactamase inhibitors, such as clavulanic acid. From a clinical point of view, they are important because they confer resistance to penicillins, aztreonam, and cephalosporins, and ESBL-producing organisms are typically also resistant to aminoglycosides, trimethoprim-sulfamethoxazole, and quinolones [1]. Until recently, the main problem posed by ESBLs was related to nosocomial outbreaks caused by ESBL-producing Klebsiella species. These outbreaks are usually clonal, the strains are mainly spread through cross-transmission, and the risk factors are similar to those found for other multidrug-resistant nosocomial pathogens [2]. In Europe and the United States, most ESBL-producing Klebsiella isolates harbored enzymes belonging to the TEM and SHV families [3]. Detection of colonized patients by performing surveillance cultures within affected units, isolation precautions for colonized patients, and restriction of oxyimino-β-lactam use are frequently useful for the control of these outbreaks [1]. There is no evidence that hospital-acquired ESBL-producing klebsiellae are decreasing in importance—in fact, data from the Centers for Disease Control and Prevention show that 20.6% of Klebsiella pneumoniae isolates from United States intensive care units in 2003 were probable producers of ESBL [4]. This represented a 47% increase, compared with the preceding 5 years. However, during the last few years, an impressive increase in the number of ESBL-producing Escherichia coli (and, less frequently, other Enterobacteriaceae) is being described in several parts of the world [5–8].
