BACKGROUND: Heerfordt syndrome is rare and is characterized by fever, uveitis, parotid gland enlargement, and facial nerve palsy. We hereby present a case of Heerfordt syndrome with unilateral facial nerve palsy as a presentation of sarcoidosis. HISTORY AND SIGNS: A 29-year-old male patient from Sri Lanka presented with eye redness OU, blurred vision OD, fever, headache, night sweat, fatigue, and weight loss (5 kg over 1 month). Examination revealed mild anterior uveitis OU, mild vitritis OD, fundus whitish lesions OU, left otalgia, taste disorders, bilateral parotid gland enlargement, and left facial nerve palsy. Work-up for infection or tumour was negative. Chest computed tomography and transbronchial lymph node biopsy set the diagnosis of sarcoidosis. THERAPY AND OUTCOME: The patient recovered completely within 2 months under therapy with prednisone and azathioprine. One year after onset of treatment, no recurrence was noted. CONCLUSIONS: Heerfordt syndrome is a rare manifestation of neurosarcoidosis and has to be included in the differential diagnosis of facial nerve palsy.

CODEX SEARCH és un motor de recuperació d'informació especialitzat en dret d'estrangeria que està basat en eines i coneixement lingüístics. Per a desenvolupar un SRI (sistema de recuperació d'informació) eficient en el domini indicat no n'hi ha prou amb emprar un model tradicional de RI (recuperació d'informació), és a dir, comparar els termes de la pregunta amb els de la resposta, bàsicament perquè no expressen implicacions. En aquest sentit, la solució lingüística proposada es basa a incorporar el coneixement dels especialistes mitjançant la integració en el sistema d'una llibreria de casos. Els casos són exemples de procediments aplicats per experts/ertes en la solució de problemes que han ocorregut en la realitat i que han acabat en èxit o fracàs. Els resultats obtinguts en aquesta primera fase són molt encoratjadors, però és necessari continuar la investigació en aquest camp per millorar el rendiment del prototip.

The purpose of this study is to analyze the influence of geometric parameters on the fatigue strength of the welded joint. The thesis includes the analytical study pertaining to variation of undercut and the weld toe radius. The theoretical part includes the concepts of fracture mechanics, different stages of the crack propagation and finally the results. The numerical results are obtained from FRANC2D/L. The results show that the fatigue strength of the welded structure depends on the parameters of the welds such as the weld toe radius, weld angle, height of the undercut and plate thickness. The results show that the fatigue life can be improved by having the undercut as minimum as possible. However there has been significant variation on the fatigue life of the structure when the weld toe radius is increased.

NlmCategory="UNASSIGNED">Preserving β cell function during the development of obesity and insulin resistance would limit the worldwide epidemic of type 2 diabetes (T2DM). Endoplasmic reticulum (ER) calcium (Ca(2+)) depletion induced by saturated free fatty acids and cytokines causes β cell ER stress and apoptosis, but the molecular mechanisms behind these phenomena are still poorly understood. Here, we demonstrate that palmitate-induced sorcin (SRI) down-regulation, and subsequent increases in glucose-6-phosphatase catalytic subunit-2 (G6PC2) levels contribute to lipotoxicity. SRI is a calcium sensor protein involved in maintaining ER Ca(2+) by inhibiting ryanodine receptor activity and playing a role in terminating Ca(2+)-induced Ca(2+) release. G6PC2, a GWAS gene associated with fasting blood glucose, is a negative regulator of glucose-stimulated insulin secretion (GSIS). High fat feeding in mice and chronic exposure of human islets to palmitate decreases endogenous SRI expression while levels of G6PC2 mRNA increase. Sorcin null mice are glucose intolerant, with markedly impaired GSIS and increased expression of G6pc2. Under high fat diet, mice overexpressing SRI in the β cell display improved glucose tolerance, fasting blood glucose and GSIS, whereas G6PC2 levels are decreased and cytosolic and ER Ca(2+) are increased in transgenic islets. SRI may thus provide a target for intervention in T2DM.

Tutkimus eettiset sijoitusrahastot Suomessa – nykytilan arviointi, on kvalitatiivinen, teorialähtöinen kirjallisuustutkimus, jossa selvitetään eettisten sijoitusrahastojen nykytilannetta Suomessa. Päätavoitteena on selvittää, mitä ja minkälaisia eettiset sijoitusrahastot ovat Suomessa. Alatavoitteena tutkitaan myös eettisten rahastojen kehitystä niin Suomessa kuin kansainvälisestikin. Tutkimuksessa on kartoitettu Suomessa toimivat eettiset sijoitusrahastot ja vertailtu niitä keskenään sekä selvitetty niiden sijoituspolitiikkaa ja toimintaa. Aineistona on käytetty tieteellisen kirjallisuuden lisäksi pankkien ja pankkiiriliikkeiden aineistoja. Tutkimus on tehty kirjallisuuden pohjalta ja tutkimus perustuu kirjallisuuden ja tutkimusaineiston analysointiin sekä siitä tehtäviin johtopäätöksiin. Tutkimuksessa havaittiin, että suomalaiset eettisiksi luokiteltavat rahastot ovat hyvin identtisiä ja niissä näkyy vahva painotus ympäristöön ja ilmastoon. Eettisten rahastojen määrä ja koko on kasvanut nopeasti koko 2000 – luvun ja tutkimuksen tekovaiheessa Suomessa toimii 22 eri eettistä rahastoa ja näiden yhteenlaskettu rahastopääoma on noin 5,8 Mrd. euroa, joka on noin 8 % koko rahastopääomasta (66 Mrd. euroa, (2007)) Suomessa eettinen sijoittaminen on vielä suhteellisen uusi ilmiö ja kansainvälisesti vertailtuna eettinen sijoittaminen on toistaiseksi pientä.

Lower extremity peripheral arterial disease (PAD) is associated with decreased functional status, diminished quality of life (QoL), amputation, myocardial infarction, stroke, and death. Nevertheless, public awareness of PAD as a morbid and mortal disease is low. The aim of this study was to assess the incidence of major lower extremity amputation due to PAD, the extent of reamputations, and survival after major lower extremity amputation (LEA) in a population based PAD patient cohort. Furthermore, the aim was to assess the functional capacity in patients with LEA, and the QoL after lower extremity revascularization and major amputation. All 210 amputees due to PAD in 1998–2002 and all 519 revascularized patients in 1998–2003 were explored. 59 amputees alive in 2004 were interviewed using a structured questionnaire of QoL. Two of each amputee age-, gender- and domicile-matched controls filled in and returned postal self-administered QoL questionnaire as well as 231 revascularized PAD patients (the amount of these patients who engaged themselves to the study), and one control person for each patient completed postal self-administered QoL questionnaire. The incidence rate of major LEA was 24.1/100 000 person-years and it was considerably high during the years studied. The one-month mortality rate was 21%, 52% at one-year, and the overall mortality rate was 80%. When comparing the one-year mortality risk of amputees, LEAs were associated with a 7.4-fold annual mortality risk compared with the reference population in Turku. Twenty-two patients (10%) had ipsilateral transversions from BK to AK amputation. Fifty patients (24%) ended up with a contralateral major LEA within two to four amputation operations. Three bilateral amputations were performed at the first major LEA operation. Of the 51 survivors returning home after their first major LEA, 36 (71%) received a prosthesis; (16/36, 44%) and were able to walk both in- and outdoors. Of the 68 patients who were discharged to institutional care, three (4%) had a prosthesis one year after LEA. Both amputees and revascularized patients had poor physical functioning and significantly more depressive symptoms than their controls. Depressive symptoms were more common in the institutionalized amputees than the home-dwelling amputees. The surviving amputees and their controls had similar life satisfaction. The amputees felt themselves satisfied and contented, whether or not they lived in long-term care or at home. PAD patients who had undergone revascularizations had poorer QoL than their controls. The revascularized patients’ responses on their perceived physical functioning gave an impression that these patients are in a declining life cycle and that revascularizations, even when successful, may not be sufficient to improve the overall function. It is possible that addressing rehabilitation issues earlier in the care may produce a more positive functional outcome. Depressive symptoms should be recognized and thoroughly considered at the same time the patients are recovering from their revascularization operation. Also primary care should develop proper follow-up, and community organizations should have exercise groups for those who are able to return home, since they very often live alone. In rehabilitation programs we should consider not only physical disability assessment but also QoL.

Tutkielmassa pyrittiin arvioimaan SRI-rahastojen riskikorjattua suorituskykyä tavallisiin rahastoihin verrattuna tavanomaisin mittarein. Lisäksi tutkittiin salkunhoitajan ajoituskykyä kummallakin rahastoryhmällä kahdella eri mallilla, Treynor-Mazuy mallilla ja Henriksson-Merton mallilla.

The system of rice intensification has emerged as a promising rice production package but weed infestation could lead to incomplete benefits from the system. A two-year field study was performed to determine an appropriate method of weed management in SRI. Weed management treatments were manual hoeing 20, 40 and 60 days after transplanting (DAT), hoeing with rotary hoe at 20, 40 and 60 DAT, hoeing with rotary hoe at 20 DAT + spray with sorghum and sunflower water extracts at 15 L ha-1 40 DAT, manual hoeing 20 DAT + spray with sorghum and sunflower water extracts, both in equal amount, at 15 L ha-1 40 DAT, orthosulfamuron at 145 g a.i. ha-1 7 DAT, weedy check and weed free. Manual hoeing at 20, 40 and 60 DAT was the treatment that exhibited the maximum kernel yield i.e. 5.34 and 4.99 t ha-1., which was 8.4 and 7.2% higher than orthosulfamuron and 61.0 and 64.9% higher than weedy check, during both years of study, respectively. The highest weed suppression was also achieved by manual hoeing at 20, 40 and 60 DAT with weed control efficiency of 87.89 and 82.32% during 2010 and 2011, respectively. Manual hoeing at 20, 40 and 60 DAT is an eco-friendly, non-chemical weed control method to increase kernel yield of fine rice under SRI.
