Objective: To investigate the influence of a regular oral use of acetylsalicylic acid in the prevalence of actinic keratosis.Methods: A case-control study with dermatologic outpatients above 50 years of age assessed between 2009 and 2011. Cases were defined as those who had been under regular use of oral acetylsalicylic acid for more than six consecutive months. The assessment focused on: age, sex, skin-type, tobacco smoking, use of medication, occurrence of individual or family skin cancer, and sunscreen and sun exposure habits. Actinic keratoses were counted in the medial region of the face and upper limbs. Counts were adjusted by co-variables based on a generalized linear model.Results: A total of 74 cases and 216 controls were assessed. The median time of acetylsalicylic acid use was 36 months. Cases differed from controls as to the highest age, highest prevalence of use of angiotensin-converting enzyme inhibitors and fewer keratosis on the face and on the upper limbs (p < 0.05). The multivariate model showed that the use of acetylsalicylic acid was associated to lower counts of face actinic keratosis and upper-limb erythematous actinic keratosis (p < 0.05), regardless of other risk factors.Conclusion: The regular use of oral acetylsalicylic acid for more than six months was associated to a lower prevalence of actinic keratosis, especially facial and erythematous ones.

Bacterial cellulose is a highly hydrated pellicle made up of a random assembly of ribbon shaped fibers less than 5 nm wide. The unique properties provided by the nanometric structure have led to a number of diagnostic biological probes, display devices due to their unique size-dependent medical applications. Bacterial cellulose matrix extracellular is a novel biotechnology and unique medicine indicated for ultimate chronic wound treatment management, drug delivery, tissue engineering, skin cancer and offers an actual and effective solution to a serious medical and social problem and to promote rapid healing in lesions caused by Diabetic burns, ulcers of the lower limbs or any other circumstance in which there's epidermal or dermal loss. In this work, it is reported novel antimicrobial peptides (AMPs) bacterial cellulose/polyhexanide biguanide (PHMB) which are produced by symbioses culture between polyhexanide biguanide and green tea culture medium resulting in the pure 3-D structure consisting of an ultra-fine network of novel biocellulose/PHMB nanofibres matrix (2-8 nm), highly hydrated (99% in weight), and with higher molecular weight, full biocompatibility.

BackgroundMelanoma of the skin is characterised by a high metastatic potential, but reports of metastasis to the tongue are rare. We report a case of skin melanoma with metastasis to the lymph nodes, tongue and brain.ObjectivesThis report highlights the clinical and histological features of oral metastatic melanoma.Case reportA 72-year-old man was seen with a nodule on the tongue. The differential diagnosis included salivary gland tumour, lymphoma and metastatic melanoma. His medical history revealed treatment for melanoma in the periumbilical region and micrometastases in the inguinal lymph nodes. An incisional biopsy was obtained and histological analysis showed the presence of a solid, epithelioid malignant tumour of monotonous appearance infiltrating the skeletal musculature. Immunohistochemistry showed reactivity of neoplastic cells to anti-HMB45, anti-melan A and anti-S100 antibodies and negativity for anti-PAN cytokeratin, confirming the diagnosis of metastatic melanoma.ConclusionThe present findings highlight the importance of a complete medical evaluation of the patient by anamnesis to identify possible oral repercussions of primary diseases in other organs and/or systems.

Photodynamic therapy is a method of healing skin lesions , in addition to radiotherapy and brachytherapy are commonly used in the treatment of skin cancers , but it is still little known in Brazil , consisting of the application to the skin of a drug in the form of cream or lotion ( photosensitizer ) . It is absorbed by tumor cells and subsequently activated by light device , that wavelength of light in the order of 630 nanometers , promoting the selective destruction of cancer cells while preserving healthy cells around the lesion . It can be considered highly effective for treatment of skin cancer types of non-melanoma ( basal cell carcinoma , 85-100 % success in curing [ 3 ] , and Bowen's disease), actinic keratoses , which are precancerous lesions and still no studies that prove also action in the treatment of acne and aging caused by the sun . And the advantage of this treatment is that , when indicated , replace the surgical procedure , selectively treating the diseased cells . Thus, it offers better cosmetic results, reducing the risk of scarring [ 4 ]

A new concept for in vitro visual evaluation of sun protection factor (SPF) of cosmetic formulations based on a supramolecular ultraviolet (UV) dosimeter was clearly demonstrated. The method closely parallels the method validated for in vivo evaluation and relies on the determination of the slightest perceptible bleaching of an iron-complex dye/nanocrystallinetitanium dioxide interface (UV dosimeter) in combination with an artificial skin substrate simulating the actual human skin in the presence and absence of a cosmetic formulation. The successful evaluation of SPF was ensured by the similarity of the erythema response of our dosimeter and human skin to UV light irradiation. A good linear correlation of in vitro and in vivo data up to SPF 40 confirmed the effectiveness of such a simple, cheap, and fast method. In short, here we unravel a convenient and accessible visual FPS evaluation method that can help improving the control on cosmetic products contributing to the reduction of skin cancer, one of the critical public health issues nowadays. Topical chemotherapy using doxorubicin, a powerful anticancer drug, can be used as an alternative with reduced systemic toxicity when treating skin cancer. The aim of the present work was to use factorial design-based studies to develop cationic solid lipid nanoparticles containing doxorubicin; further investigations into the influence of these particles on the drug's cytotoxicity and cellular uptake in B16F10 murine melanoma cells were performed. A 3(2) full factorial design was applied for two different lipid phases; one phase used stearic acid and the other used a 1:2 mixture of stearic acid and glyceryl behenate. The two factors investigated included the ratio between the lipid and the water phase and the ratio between the surfactant (poloxamer) and the co-surfactant (cetylpyridinium chloride). It was observed that the studied factors did not affect the mean diameter or the polydispersity of the obtained nanoparticles; however, they did significantly affect the zeta potential values. Optimised formulations with particle sizes ranging from 251 to 306 nm and positive zeta potentials were selected for doxorubicin incorporation. High entrapment efficiencies were achieved (97%) in formulations with higher amounts of stearic acid, suggesting that cationic charges on doxorubicin molecules may interact with the negative charges in stearic acid. Melanoma culture cell experiments showed that cationic solid lipid nanoparticles without drug were not cytotoxic to melanoma cells. The encapsulation of doxorubicin significantly increased cytotoxicity, indicating the potential of these nanoparticles for the treatment of skin cancer.

OBJETIVO: A exposição à luz solar na infância ocorre, frequentemente, de forma mais intensa do que em muitos adultos. Dados da literatura comprovam de maneira inequívoca a associação desse comportamento social com o risco de desenvolvimento do melanoma maligno e do câncer cutâneo não melanoma mesmo na vida adulta. Além disso, o fotoenvelhecimento cutâneo é semeado já na infância com a exposição solar inadequada. Esta revisão tem como objetivo orientar os pediatras nas medidas adequadas de fotoproteção tópica nas crianças e adolescentes, o que irá alterar de maneira positiva o futuro desses pacientes. FONTES DOS DADOS: Realizou-se uma revisão da literatura indexada na base de dados MEDLINE/PubMed entre os anos de 1999 e 2012 sobre fotoproteção na infância, selecionando-se como fonte os artigos de revisão mais relevantes, do ponto de vista de abrangência do tema fotoproteção em crianças e adolescentes, fotoproteção e vitamina D, fototerapia na neonatologia e impacto no câncer cutâneo, bronzeamento artificial e câncer cutâneo. SÍNTESE DOS DADOS: Crianças e adolescentes devem adotar medidas adequadas de fotoproteção para diminuir o risco de câncer cutâneo melanoma e não melanoma. CONCLUSÕES: Há dados na literatura que suportam a associação de hábitos de exposição solar segura e uso de fotoprotetores tópicos em crianças e adolescentes com a redução da ocorrência do câncer cutâneo.

In der vorliegenden Studie wurde überprüft, ob Broschüren, die negative Konsequenzen unzureichender Hautselbstuntersuchung (HSU) betonen (Verlustrahmung), besser geeignet sind, die Früherkennung von Hautkrebs zu fördern als Broschüren, die positive Konsequenzen bei richtig durchgeführter HSU schildern (Gewinnrahmung). Geschlecht und die Bewältigungsdispositionen Vigilanz und kognitive Vermeidung wurden als mögliche Moderatorvariablen untersucht. Nach Erfassung von Baseline- und Hintergrundvariablen lasen 180 Teilnehmer jeweils eine der Broschüren, die ihnen per Zufall zugeteilt wurde. Diese variierten in Rahmung (Gewinn/Verlust) und Bedrohungsgrad (gering/hoch), so dass vier unterschiedliche Versionen vorlagen. Rahmung und Bedrohungsgrad wirkten sich in Abhängigkeit von Vigilanz, d.h. der Disposition, bedrohungsbezogene Informationen systematisch zu verarbeiten, auf die Intention aus. Deutlicher war die Wirkung der Rahmung auf die berichtete Häufigkeit von HSU, die zwei Monate nach dem Lesen erneut erfragt worden war. In der Verlustbedingung stieg die berichtete Häufigkeit von HSU bei Personen mit hoher Vigilanz, bei Personen mit niedriger Vigilanz dagegen sank sie. In der Gewinnbedingung profitierten Personen mit niedriger Vigilanz, während Personen mit hoher Vigilanz weniger HSU berichteten als zuvor. Diese Effekte korrespondierten zwar mit Änderungen der eigenen Risikowahrnehmung, der Erinnerungsleistung und der Beschäftigung mit dem Studienthema, jedoch ließ sich für keine dieser Variablen eine vermittelnde Wirkung nachweisen. Einstellung und Anforderung von Informationsmaterialien blieben unbeeinflusst.

Gegenstand dieser Arbeit war es, das Zusammenspiel zwischen DNA-Reparatur und zellulärem anitoxidativen Abwehrsystem in Melanomzellen und gesunden Hautfibroblasten näher zu untersuchen. Dabei konnte gezeigt werden, dass die dominierenden DNA-Läsionen im Falle einer Bestrahlung mit sichtbarem Licht (400 – 800 nm) Fpg-sensitive Läsionen, zu denen die Basenmodifikation 7,8-Dihydro-8-oxoguanin (8-oxoG) gehört, und im Falle der UVA-Bestrahlung Cyclobutan-Pyrimidindimere (CPDs) sind. Sowohl Melanomzellen als auch Hautfibroblasten waren problemlos in der Lage, die durch sichtbares Licht und UVA-Strahlung induzierten oxidativen DNA-Modifikationen zu reparieren. Jedoch reagierten Melanomzellen in einer adaptiven Antwort mit einer Erhöhung ihres Glutathion-Gehalts auf ein Maximum (nach circa 10 - 14 h) nach Bestrahlung mit sichtbarem Licht, wohingegen die Hautfibroblasten einen massiven Einbruch direkt nach Bestrahlung und eine extrem lange Erholungsphase über 48 h aufzuweisen hatten. Die darauffolgende Untersuchung der DNA-Reparaturkapazität der Zellen unter Bedingungen von oxidativem Stress mit vorangegangener Depletion intrazellulären Glutathions zeigten eine dramatische, nahezu vollständige Hemmung der Reparatur durch UVA- bzw. Sonnenlicht-induzierter Fpg-sensitiver DNA-Modifikationen (8-oxoG) - sowohl in Melanomzellen als auch in Hautfibroblasten. Dieser Effekt ließ sich durch den Zusatz von Dithiothreitol (DTT), nach erfolgter Bestrahlung der Glutathion-depletierten Zellen, wieder komplett revertieren. Diese Ergebnisse weisen darauf hin, dass an der Reparatur ein redoxempfindliches Protein oder zellulärer Cofaktor beteiligt sein muß. Zudem konnte durch Untersuchungen der Nukleotidexzisionsreparatur (NER) und der Einzelstrangbruchreparatur nach dem gleichen Versuchsdesign gezeigt werden, dass es sich hierbei sehr wahrscheinlich um einen für die Basenexzisionsreparatur (BER) von 7,8-dihydro-8-oxo-guanine (8-oxoG) exklusiven Effekt handelte. Zwei der wichtigsten Reparaturproteine der BER, nämlich hOGG1 und APE1, wurden anschließend auf ihre Funktionsfähigkeit hin untersucht, da es naheliegend war, dass der Reparaturhemmung ein Funktionsverlust eines dieser beiden Enzyme zugrunde liegen könnte. Im Falle des APE1-Proteins konnte dies ausgeschlossen werden, da mit Hilfe der Alkalischen Elution die volle Funktionsfähigkeit für die Reparatur von AP-Läsionen nachgewiesen werden konnte. Interessanterweise zeigte aber das hOGG1-Protein eine zwischen der dritten und vierten Stunde nach Bestrahlung Glutathion-depletierter Zellen stark abfallende Aktivität der 8-oxoG-Glykosylasefunktion. Die Western-Blot-Analyse ergab allerdings keinen Hinweis auf eine Proteinoxidation von hOGG1. Möglicherweise wird nicht hOGG1 selbst, wohl aber ein anderes, für eine konzertierte Abfolge der einzelnen Reparaturschritte entscheidend notwendiges Protein innerhalb der Zelle durch ROS leicht oxidiert. In jedem Fall bleibt festzustellen, dass Glutathion eine wichtige Aufgabe hinsichtlich einer voll funktionsfähigen Basenexzisionreparatur zuzukommen scheint. Die Ergebnisse unterstreichen die mögliche Bedeutung von oxidativem Stress für die Entstehung von Krebs durch Sonnenlicht, insbesondere durch UVA, da die durch die Strahlung (und eventuell auftretende Entzündung) gebildeten ROS nicht nur DNA-Schäden induzieren, sondern auch ihre Reparatur verhindern können.

Il carcinoma a cellule basali (BCC) costituisce l'80 peercento dei tumori cutanei non-melanoma, rappresentando dunque il tumore maligno della cute più frequente nella popolazione generale. Tuttavia, non esistono ad oggi studi epidemiologici ampi ed approfonditi condotti su scala nazionale su questo tipo di neoplasia, poichè i tumori cutanei non-melanoma sono esclusi dal registro statistico dei tumori. A tale scopo presso la Dermatologia dell'Università di Bologna sono stati raccolti di tutti i casi di carcinoma basocellulare osservati dal 1 gennaio 1990 sino al 31 dicembre 2014, e sono stati rielaborati statisticamente. Il criterio di inclusione adottato è stato la positività per BCC all’esame istologico, sia in caso di biopsia semplice, sia in caso di asportazione radicale. Il progetto è stato svolto presso l’ambulatorio di chirurgia oncologica della nostra UO, così come il follow-up dei pazienti nei casi di recidività multiple o di comparsa di nuovi tumori cutanei. Ad oggi, tale neoplasia è risultata essere quella di più frequente osservazione nella Unità Operativa di Dermatologia dell’Università di Bologna. Non solo, la nostra casistica rimane quella più numerosa fino ad ora riportata in tutta Italia negli ultimi 24 anni.

Abstract: Basal cell carcinoma (BCC) is the most common form of skin cancer. Mutations of the PTCH hallmark gene are detected in about 50-60% of BCCs, which raises the question whether other mechanisms such as promoter methylation can inactivate PTCH. Therefore, we performed methylation analysis of the PTCH promoter in a total of 56 BCCs. The sensitivity of three different methods, including direct bisulphite sequencing PCR, MethyLight and high-resolution melting (HRM), was applied and compared. We found that HRM analysis of DNA from fresh tissue [rather than formalin-fixed and paraffin-embedded tissue (FFPE)] was the most sensitive method to detect methylation. Low-level methylation of the PTCH promoter was detected in five out of 16 analysed BCCs (31%) on DNA from fresh tissue but only in two (13%) samples on short-time stored FFPE DNA from the very same tumors. In contrast, we were unable to detect methylation by HRM on long-time stored DNA in any of the remaining 40 BCC samples. Our data suggest that (i) HRM on DNA extracted from fresh tissue is the most sensitive method to detect methylation and (ii) methylation of the PTCH promoter may only play a minor role in BCC carcinogenesis.

Skin cancer of the lip is frequent, and reconstruction after Mohs surgery might be challenging mostly when the postsurgical defect has a size of more than 1 cm(2) and is situated adjacent to the philtrum.

Transplantation is the treatment of choice for many different organ failures. Despite growing experience in surgery and immunosuppression protocols, the long-term mortality of the procedure remains much higher than in the general population. Second only to cardiovascular diseases as the cause of death in organ transplant recipients, cancer is now known to be at least partly related to the immunosuppression regimen. Nevertheless, if calcineurin inhibitors have a demonstrated pro-oncogenic effect, other classes, such as mTOR inhibitors, are antiproliferative, and even demonstrated as an efficient therapy in some advanced oncological situations. Therefore, the adaptation of the therapy protocol evolves now towards an individualized medicine based on the risk factors of each transplant recipient in terms of cardiovascular, infectious and oncological diseases. As the first organ involved by tumor is the skin, many different guidelines have been published to try and adapt the therapy to the occurrence of a new lesion. If, for example, limited actinic keratosis or the first episode of a non-melanoma skin cancer usually requires no change of the immunosuppressive therapy, but a local specialized care and frequent clinical controls, more advanced lesions imply the adaptation of the drug regimen. In any case, the collaboration between general practitioners, dermatologists and the transplantation team is mandatory.
