The purpose of this study was to determine if walking a dog would increase motivation to adhere to a walking program and result in an increase in walking endurance and mobility among institutionalized elderly. An experimental pre and post test two group randomly assigned study design was utilized. Thirty subjects, 20 females and 10 males with an average age of 72, were enrolled from three long-term care facilities. The walking program was 3 times a week for 6 weeks. The experimental group walked with a certified therapy dog and the handler. The control group walked with only the handler. The Outcome Expectations for Exercise Scale (OEES) was used to measure the perceived benefits of exercise. The 2-minute walk test and the 30 second chair stand test were administered before and after the walking program. The OEES scores did not significantly predict adherence to the program. The pre- and post-chair stand test and the 2-minute walk test did not show statistical significant differences between groups. All of the participants did show an increase (7 minutes) in walking time during the 6 week period (p=0.048). The mean pre and post walk test scores for participants with stroke/arthritis were significantly less than those without stroke/arthritis (p=0.013). The experimental group had 12 subjects with stroke/arthritis compared with 6 in the control group. The walk test means in feet walked were 362.44 ± 130.36 (control) vs. 201.27 ± 106.25 (experimental), p=0.001. The results indicate walking practice has the potential to increase walking time and endurance. Because residents of long-term care facilities were not allowed outside the facilities without accompaniment, the presence of the dog handler was key to their walking. Analysis of conversations during the walks indicated that for participants who walked with dogs, the dogs did serve as motivation for continuing in the program. ^

Introducción. La prevalencia de la Diabetes Gestacional (DG) varía en todo el mundo, así como entre los grupos raciales y étnicos del mismo país. Hasta la fecha actual, no se ha llegado a un consenso en el criterio diagnóstico, y eso dificulta una estimación veraz de prevalencia entre países. A pesar de ello, es ineludible obviar el incremento en la incidencia de esta complicación en todo el mundo, y la trascendencia de sus riesgos a la salud pública. En España, según los criterios clásicos –del National Diabetes Data Group- existe una alta prevalencia en un 8,8 % de DG en gestantes. Es importante encontrar la mejor vía para la prevención de la DG y, uno de los factores de riesgo parece ser el aumento excesivo de peso durante el embarazo. El ejercicio es un elemento fundamental para el control del metabolismo de la glucosa y para reducir los niveles de hiperlipidemia. Sin embargo, existe controversia para definir el tipo de sesiones, duración e intensidad que puedan contribuir a su prevención. Objetivo. Conocer en qué medida el ejercicio físico programado durante el embarazo, combinado en agua y tierra, con ejercicios aeróbicos y de tonificación muscular, puede actuar como un factor de prevención de la DG. Al mismo tiempo, valorar si exceder las recomendaciones de peso puede influir el diagnóstico de la DG. Material y Métodos. Este estudio se desarrolló mediante una colaboración entre la Universidad Politécnica de Madrid y los Servicios de Ginecología y Obstetricia del Hospital Universitario de Puerta de Hierro, el Hospital Universitario de Torrelodones y el Centro de Salud de Torrelodones. Se obtuvo la aprobación del Comité Ético de Investigación Clínica (CEIC). Se realizó un ensayo clínico, aleatorizado, controlado, no enmascarado. 272 mujeres gestantes sanas dieron su consentimiento informado para la inclusión en el estudio. De las cuáles, finalmente 257 (edad= 33,2±4,4 años) fueron analizadas, 101 de ellas correspondientes al grupo intervención (GI, n=101) y 156 al grupo control (GC, n=156). El inicio del programa correspondió a la semana 10-14 del embarazo hasta el final, la 38-40. Con una frecuencia de 3 sesiones semanales y una duración de 60 y 50 minutos, en tierra y agua, respectivamente. Resultados. Se halló diferencias significativas en los valores en los 180 min del test de tolerancia oral a la glucosa [GI: 98,00±29,48 mg/dl vs. GC: 116,25±29,90 mg/dl (t64= 2,37; p= 0,021)] y, de igual modo, el GI mostró menor prevalencia de la DG [GI: 1 %, Ejercicio y DG n= 1 vs. GC: 8,8 %, n= 13 (2 1= 6,84; p= 0,009)] y una estimación de riesgo significativa (OR= 9,604; 95 % CI: 1,23-74,66). La excesiva ganancia de peso fue menor en el GI [GI: 22,8 %, n= 23 vs. GC: 34,9 %, n= 53 (2 1= 4,22; p= 0,040)], pero no existió una correlación con la incidencia de DG (ϕ= -0,007; p= 0,910). Conclusiones. El programa de ejercicio desarrollado durante el embarazo mostró efectividad en la reducción de la prevalencia de la DG, preservó la tolerancia a la glucosa y redujo la excesiva ganancia de peso materno. Background. The prevalence of Gestational Diabetes (GD) varies around the world, as well as between racial and ethnic groups within the same country. Currently, there is not a consensus about the diagnostic criteria, and that makes it difficult to obtain accurate estimates of prevalence between countries. The increased trend in the prevalence across the globe and the risks for public health cannot be ignored. In Spain, according to the diagnostic criteria of National Diabetes Data Group, there is a prevalence of 8.8 % for GD in pregnant women. It is important to look for the best way to prevent GD and one of the risk factors seems to be excessive weight gained during pregnancy. Exercise is an essential element for glucose metabolic control and reducing hyperlipidemia levels. However, there is controversy to define the type of activity, duration and intensity to prevent GD. Objective. To assess the effectiveness of an exercise programme carried out during pregnancy (land/aquatic activities), both aerobic and muscular conditioning can help to the prevent GD. Also, to assess if excessive maternal weight gain influences the GD diagnosis. Material and methods. Collaboration between the Technical University of Madrid and the Gynecology and Obstetrics Department of Puerta de Hierro University Hospital, Torrelodones University Hospital and Health Center of Torrelodones supported the study. It was approved by the Clinical Research Ethics Committee (CEIC). A clinical, randomized controlled trial recruited 272 pregnant women without obstetric contraindications and gave informed consent for inclusion in the study. Of these women, 257 were studied (age= 33,2±4,4 years), 101 in intervention group (IG, n= 101) and 156 in control group (CG, n= 156). A physical exercise program three times per week during pregnancy was developed. The duration of the sessions was 60 minutes and 50 minutes in land and water, respectively. Results. The IG showed lower maternal values in the Oral Glucose Tolerance Test (OGTT) at 180 minutes [IG: 98,00±29,48 mg/dl vs. CG: 116,25±29,90 mg/dl (t64= 2,37; p= 0,021)] and the IG reduced the prevalence of GD [IG: 1%, n= 1 vs. CG: 8,8 %, n= 13 (2 1= 6,84; p= 0,009)] with a significance risk estimate (OR= 9,604; 95 % CI: 1,23- 74,66). Excessive maternal weight gain was less in the IG [IG: 22,8 %, n= 23 vs. CG: Exercise and GD 34,9 %, n= 53 (2 1= 4,22; p= 0,040)] but there was no correlation with the prevalence of GD (ϕ= -0,007; p= 0,910). Conclusions. The exercise programme performed during pregnancy reduced the prevalence of GD, preserved glucose tolerance and reduced excessive maternal weight gain.

Objetivo: Analizar el grado de relación entre cuatro pruebas que valoran la funcionalidad de la marcha en sujetos jóvenes con daño cerebral adquirido (DCA) en fase subaguda y conocer el grado de relación entre estas pruebas y la percepción subjetiva de seguridad en actividades de la vida diaria. Metodología: 67 participantes jóvenes con DCA en fase subaguda (43 hombres y 24 mujeres) con una edad media 35,09 años. Se realizó estadística descriptiva de todas las variables demográficas: género, edad, IMC, meses desde que se produjo la lesión y etiología lesional. Para analizar si existe correlación entre las variables se utilizó el coeficiente de Pearson. Resultados: El Timed 10-Meter Walk presenta una correlación muy alta con Timed Up and Go (TUG) (r=093), alta con el 6-Minute Walk Test (r=0,77) y moderada con el Step Test (r=0,56). El 6-Minute Walk Test presenta una correlación alta con el TUG (r=0,82) y una correlación moderada con el Step Test (r=0,69). El Step Test presenta una correlación moderada con el TUG (r= -0,68). The Activities-specific Balance Confidence Scale (ABC) presenta una correlación moderada con el Timed 10-Meter Walk (r=0,42), TUG (R=0,40), 6-Minute Walk Test (r=0,40) y Step Test (r=0,44). Conclusiones: Las pruebas de funcionalidad de la marcha presentan una correlación significativa entre moderada y muy alta en personas jóvenes con DCA. El ABC presenta una correlación significativa moderada con las cuatro variables de funcionalidad de la marcha analizadas en esta población

The objective of this study was to verify the association between some mobility items of the International Classification Functionality (ICF), with the evaluations Gross Motor Function Measure (GMFM-88), 1-minute walk test (1MWT) and if the motor impairment influences the quality of life in children with Cerebral Palsy (PC), by using the Paediatric Quality of Life Inventory (PedsQL 4.0 versions for children and parents). The study included 22 children with cerebral palsy spastic, classified in levels I, II, and III on the Gross Motor Function Classification System (GMFCS), with age group of 9.9 years old. Among those who have participated, seven of them were level I, eight of them were level II and seven of them were level III. All of the children and teenagers were rated by using check list ICF (mobility item), GMFM-88, 1-minute walk test and PedsQL 4.0 questionnaires for children and parents. It was observed a strong correlation between GMFM-88 with check list ICF (mobility item), but moderate correlation between GMFM-88 and 1-minute walk test (1MWT). It was also moderate the correlation between the walking test and the check list ICF (mobility item). The correlation between PedsQl 4.0 questionnaires for children and parents was weak, as well as the correlation of both with GMFM, ICF (mobility item) and the walking test. The lack of interrelation between physical function tests and quality of life, indicates that, regardless of the severity of the motor impairment and the difficulty with mobility, children and teenagers suffering of PC spastic, functional level I, II and III GMFCS and their parents have a varied opinion regarding the perception of well being and life satisfaction.

Objective To estimate the long-term effect of intensive, 6-week physiotherapy programs, with and without deep abdominal muscle (TrA) training, on persistent postpartum stress urinary incontinence (SUI). Methods The study was a single-blind randomized controlled trial. Fifty-seven postnatal women with clinically demonstrated persistent SUI 3 months after delivery participated in 8 weeks of either pelvic floor muscle training (PFMT) (28) or PFMT with deep abdominal muscle training (PFMT + TrA) (29). Seven years post-treatment, 35 (61.4%) participants agreed to the follow-up; they were asked to complete a 20-min pad test and three incontinence-specific questionnaires with an assessor blinded to each participant's group assignment. Results: Of the 35 (61.4%) who agreed to the follow-up: 26 (45.6%) took the 20-min pad test (12 PFMT and 14 PFMT + TrA) and 35 (61.4%) completed the questionnaires (18 PFMT and 17 PFMT + TrA). The baseline clinical characteristics of the follow-up and non-follow-up participants were not significantly different; nor did they differ between PFMT and PFMT + TrA participants enrolled in the follow-up study. At 7 years, the pad test scores for the PFMT group did not differ statistically from those of the PFMT + TrA group. When combining both treatment groups, a total of 14/26 (53%) follow-up participants were still continent according to the pad test. Conclusion The addition of deep abdominal training does not appear to further improve the outcome of PFM training in the long term. However, benefits of physiotherapy for postpartum SUI, although not as pronounced as immediately after the initial intervention, is still present 7 years post-treatment.

Pulmonary hypertension (PH) is a rare but serious condition that causes progressive right ventricular (RV) failure and death. PH may be idiopathic, associated with underlying connective-tissue disease or hypoxic lung disease, and is also increasingly being observed in the setting of heart failure with preserved ejection fraction (HFpEF). The management of PH has been revolutionised by the recent development of new disease-targeted therapies which are beneficial in pulmonary arterial hypertension (PAH), but can be potentially harmful in PH due to left heart disease, so accurate diagnosis and classification of patients is essential. These PAH therapies improve exercise capacity and pulmonary haemodynamics, but their overall effect on the right ventricle remains unclear. Current practice in the UK is to assess treatment response with 6-minute walk test and NYHA functional class, neither of which truly reflects RV function. Cardiac magnetic resonance (CMR) imaging has been established as the gold standard for the evaluation of right ventricular structure and function, but it also allows a non-invasive and accurate study of the left heart. The aims of this thesis were to investigate the use of CMR in the diagnosis of PH, in the assessment of treatment response, and in predicting survival in idiopathic and connective-tissue disease associated PAH. In Chapter 3, a left atrial volume (LAV) threshold of 43 ml/m2 measured with CMR was able to distinguish idiopathic PAH from PH due to HFpEF (sensitivity 97%, specificity 100%). In Chapter 4, disease-targeted PAH therapy resulted in significant improvements in RV and left ventricular ejection fraction (p<0.001 and p=0.0007, respectively), RV stroke volume index (p<0.0001), and left ventricular end-diastolic volume index (p=0.0015). These corresponded to observed improvements in functional class and exercise capacity, although correlation coefficients between Δ 6MWD and Δ RVEF or Δ LVEDV were low. Finally, in Chapter 5, one-year and three-year survival was worse in CTD-PAH (75% and 53%) than in IPAH (83% and 74%), despite similar baseline clinical characteristics, lung function, pulmonary haemodynamics and treatment. Baseline right ventricular stroke volume index was an independent predictor of survival in both conditions. The presence of LV systolic dysfunction was of prognostic significance in CTD-PAH but not IPAH, and a higher LAV was observed in CTD-PAH suggesting a potential contribution from LV diastolic dysfunction in this group.

La vérification de la résistance aux attaques des implémentations embarquées des vérifieurs de code intermédiaire Java Card est une tâche complexe. Les méthodes actuelles n'étant pas suffisamment efficaces, seule la génération de tests manuelle est possible. Pour automatiser ce processus, nous proposons une méthode appelée VTG (Vulnerability Test Generation, génération de tests de vulnérabilité). En se basant sur une représentation formelle des comportements fonctionnels du système sous test, un ensemble de tests d'intrusions est généré. Cette méthode s'inspire des techniques de mutation et de test à base de modèle. Dans un premier temps, le modèle est muté selon des règles que nous avons définies afin de représenter les potentielles attaques. Les tests sont ensuite extraits à partir des modèles mutants. Deux modèles Event-B ont été proposés. Le premier représente les contraintes structurelles des fichiers d'application Java Card. Le VTG permet en quelques secondes de générer des centaines de tests abstraits. Le second modèle est composé de 66 événements permettant de représenter 61 instructions Java Card. La mutation est effectuée en quelques secondes. L'extraction des tests permet de générer 223 tests en 45 min. Chaque test permet de vérifier une précondition ou une combinaison de préconditions d'une instruction. Cette méthode nous a permis de tester différents mécanismes d'implémentations de vérifieur de code intermédiaire Java Card. Bien que développée pour notre cas d'étude, la méthode proposée est générique et a été appliquée à d'autres cas d'études.

Los estudios acerca del concepto actividad física (AF) son diversos, presentando diferentes concepciones; su relación con calidad de vida suele estar generada dentro del discurso médico, que propende por la ejecución de la Actividad Física desde una mirada netamente biológica. Si bien esta disertación es importante, se debe tener en cuenta que los estudios relacionados con calidad de vida y la AF se basan en la condición de bienestar y percepción frente al estado de salud; dichos estudios no se han realizado desde las condiciones de vida y del contexto social. Si bien es cierto que la mirada médica y lo estudios objetivos son relevantes, ya que arrojan estadísticas que permiten abordar recomendaciones en cuanto a la actividad física, en este documento se elaboró una investigación de tipo cualitativo por medio de la revisión documental del concepto de actividad física, sus prácticas y su relación con calidad de vida, que abordan diferentes autores. Para ello se elige la base de datos PubMed por su énfasis en las publicaciones de salud; se seleccionan artículos publicados del 2004 y 2014, que estudien el concepto de actividad física, sus prácticas y relaciones con calidad de vida, para finalmente hacer un análisis desde los modelos de determinación y determinantes sociales. De esta forma se analiza la posición de los autores con respecto al concepto, sus prácticas y las relaciones que puede llegar a surgir con la calidad de vida. En esta investigación se obtuvo como resultados tendencias biológicas, psicológicas, sociales y culturales, en los cuales los autores dejan clara la posición médica ya que en la mayoría de investigaciones centran sus relaciones en la funcionalidad, y es a través de la visión terapéutica donde buscan el bienestar, la satisfacción de los pacientes que padecen cualquier enfermedad. Además, aparecen categorías emergentes como: cuerpo como medio de publicidad, cibernética que avanza vertiginosamente y el papel del poder en la actividad física que pueden ser contempladas para otros estudios.

Lab tests are frequently used in primary care to guide patient care. This is particularly the case when a severe disorder, or one that will affect patients' initial care, needs to be excluded rapidly. At the PMU-FLON walk-in clinic the use of HIV testing as recommended by the Swiss Office of Public Health was hampered by the delay in obtaining test results. This led us to introduce rapid HIV testing which provides results within 30 minutes. Following the first 250 tests the authors discuss the results as well as the benefits of rapid HIV testing in an urban walk-in clinic.

OBJECTIVE: The purpose of the present study was to submit the same materials that were tested in the round robin wear test of 2002/2003 to the Alabama wear method. METHODS: Nine restorative materials, seven composites (belleGlass, Chromasit, Estenia, Heliomolar, SureFil, Targis, Tetric Ceram) an amalgam (Amalcap) and a ceramic (IPS Empress) have been submitted to the Alabama wear method for localized and generalized wear. The test centre did not know which brand they were testing. Both volumetric and vertical loss had been determined with an optical sensor. After completion of the wear test, the raw data were sent to IVOCLAR for further analysis. The statistical analysis of the data included logarithmic transformation of the data, the calculation of relative ranks of each material within each test centre, measures of agreement between methods, the discrimination power and coefficient of variation of each method as well as measures of the consistency and global performance for each material. RESULTS: Relative ranks of the materials varied tremendously between the test centres. When all materials were taken into account and the test methods compared with each other, only ACTA agreed reasonably well with two other methods, i.e. OHSU and ZURICH. On the other hand, MUNICH did not agree with the other methods at all. The ZURICH method showed the lowest discrimination power, ACTA, IVOCLAR and ALABAMA localized the highest. Material-wise, the best global performance was achieved by the leucite reinforced ceramic material Empress, which was clearly ahead of belleGlass, SureFil and Estenia. In contrast, Heliomolar, Tetric Ceram and especially Chromasit demonstrated a poor global performance. The best consistency was achieved by SureFil, Tetric Ceram and Chromasit, whereas the consistency of Amalcap and Heliomolar was poor. When comparing the laboratory data with clinical data, a significant agreement was found for the IVOCLAR and ALABAMA generalized wear method. SIGNIFICANCE: As the different wear simulator settings measure different wear mechanisms, it seems reasonable to combine at least two different wear settings to assess the wear resistance of a new material.

Major liver resection can be used in the treatment of liver cancer. The functional capacity of liver parenchyma needs to be evaluated preoperatively because it conditions the outcome. We assessed whether the whole body clearance of glycerol, a substrate essentially metabolized in liver cells, may be suitable as a simple test of liver function. Seven patients after major hepatectomy, six patients after colectomy and 12 healthy subjects were studied. Patients were investigated on the first day after surgery. All participants were studied during a 150-min basal period followed by a 120-min infusion of 16 mumol kg-1 min-1 13C-labelled glycerol. Whole body glycerol clearance was calculated from the change in plasma glycerol concentration. Whole body glucose production was measured with 6,6 2H2 glucose infused as a tracer in the basal state and during glycerol infusion. In addition, 13C glucose synthesis was monitored to quantitate gluconeogenesis from glycerol. Patients after liver resection had higher plasma glycerol concentrations and lower whole body glycerol clearance than healthy subjects and patients after colectomy. They also had higher plasma glucagon concentrations. Their fasting glucose production was mildly elevated in the fasting state and did not change after glycerol infusion, indicating a normal hepatic autoregulation of glucose production. These results indicate that whole body glycerol clearance can be simply determined from plasma glycerol concentrations during exogenous glycerol infusion. It is significantly reduced in patients after major hepatectomy, suggesting that it constitutes a sensitive test of hepatic function. Its use as a preoperative testing procedure remains to be evaluated.

Intravoxel incoherent motion (IVIM) MRI is a method to extract microvascular blood flow information out of diffusion-weighted images acquired at multiple b-values. We hypothesized that IVIM can identify the muscles selectively involved in a specific task, by measuring changes in activity-induced local muscular perfusion after exercise. We tested this hypothesis using a widely used clinical maneuver, the lift-off test, which is known to assess specifically the subscapularis muscle functional integrity. Twelve shoulders from six healthy male volunteers were imaged at 3 T, at rest, as well as after a lift-off test hold against resistance for 30 s, 1 and 2 min respectively, in three independent sessions. IVIM parameters, consisting of perfusion fraction (f), diffusion coefficient (D), pseudo-diffusion coefficient D* and blood flow-related fD*, were estimated within outlined muscles of the rotator cuff and the deltoid bundles. The mean values at rest and after the lift-off tests were compared in each muscle using a one-way ANOVA. A statistically significant increase in fD* was measured in the subscapularis, after a lift-off test of any duration, as well as in D. A fD* increase was the most marked (30 s, +103%; 1 min, +130%; 2 min, +156%) and was gradual with the duration of the test (in 10(-3) mm(2) /s: rest, 1.41 ± 0.50; 30 s, 2.86 ± 1.17; 1 min, 3.23 ± 1.22; 2 min, 3.60 ± 1.21). A significant increase in fD* and D was also visible in the posterior bundle of the deltoid. No significant change was consistently visible in the other investigated muscles of the rotator cuff and the other bundles of the deltoid. In conclusion, IVIM fD* allows the demonstration of a task-related microvascular perfusion increase after a specific task and suggests a direct relationship between microvascular perfusion and the duration of the effort. It is a promising method to investigate non-invasively skeletal muscle physiology and clinical perfusion-related muscular disorders.

The aim of this work is to develop and validate a dissolution test for glibenclamide tablets. Optimal conditions to carry out the dissolution test are 500 mL of phosphate buffer at pH 8.0, paddles at 75 rpm stirring speed, time test set to 60 min and using equipment with six vessels. The derivative UV spectrophotometric method for determination of glibenclamide released was developed, validated and compared with the HPLC method. The UVDS method presents linearity (r² = 0.9999) in the concentration range of 5-14 µg/mL. Precision and recoveries were 0.42% and 100.25%, respectively. The method was applied to three products commercially available on the Brazilian market.

The aim of this work was to compare the performance of isotope-selective non-dispersive infrared spectrometry (IRIS) for the 13C-urea breath test with the combination of the 14C-urea breath test (14C-UBT), urease test and histologic examination for the diagnosis of H. pylori (HP) infection. Fifty-three duodenal ulcer patients were studied. All patients were submitted to gastroscopy to detect HP by the urease test, histologic examination and 14C-UBT. To be included in the study the results of the 3 tests had to be concordant. Within one month after admission to the study the patients were submitted to IRIS with breath samples collected before and 30 min after the ingestion of 75 mg 13C-urea dissolved in 200 ml of orange juice. The samples were mailed and analyzed 11.5 (4-21) days after collection. Data were analyzed statistically by the chi-square and Mann-Whitney test and by the Spearman correlation coefficient. Twenty-six patients were HP positive and 27 negative. There was 100% agreement between the IRIS results and the HP status determined by the other three methods. Using a cutoff value of delta-over-baseline (DOB) above 4.0 the IRIS showed a mean value of 19.38 (minimum = 4.2, maximum = 41.3, SD = 10.9) for HP-positive patients and a mean value of 0.88 (minimum = 0.10, maximum = 2.5, SD = 0.71) for negative patients. Using a cutoff value corresponding to 0.800% CO2/weight (kg), the 14C-UBT showed a mean value of 2.78 (minimum = 0.89, maximum = 5.22, SD = 1.18) in HP-positive patients. HP-negative patients showed a mean value of 0.37 (minimum = 0.13, maximum = 0.77, SD = 0.17). IRIS is a low-cost, easy to manage, highly sensitive and specific test for H. pylori detection. Storing and mailing the samples did not interfere with the performance of the test.
