Esta tese é composta por três estudos ecológicos que incluíram as 27 capitais brasileiras. Esses três estudos foram os seguintes: 1- A associação entre a disponibilidade de cirurgiões-dentistas e a quantidade de procedimentos odontológicos nos serviços públicos de odontologia; 2- A associação entre a disponibilidade de cirurgiões-dentistas e a proporção de dentes restaurados (em relação ao total de dentes atacados pela cárie) em indivíduos de 15 a 19 anos ; 3- A associação da disponibilidade de cirurgiões-dentistas com a prevalência e severidade da cárie em indivíduos de 15 a 19 anos. As três investigações são apresentadas sob forma de artigos. Foram utilizados diversos bancos de dados secundários, disponíveis gratuitamente na internet. No primeiro estudo foi identificada associação do número de Equipes de Saúde Bucal do programa Saúde da Família (ESB) e de cirurgiões-dentistas no SUS de uma forma geral com o número de procedimentos odontológicos no serviço público; quanto mais ESB e cirurgiões-dentistas mais procedimentos odontológicos, tanto preventivos quanto restauradores. Mais dentistas no serviço público de odontologia significaram mais procedimentos preventivos e coletivos, porém um número relativamente pequeno a mais de restaurações. É preocupante a quantidade relativamente pequena de restaurações realizadas pelos dentistas do serviço público no Brasil diante do grande número de dentes com cárie não tratada, identificado pela pesquisa nacional de saúde bucal. O segundo estudo revelou que a quantidade de dentistas nas capitais brasileiras é muito grande e que, portanto, há capacidade instalada para atender todas as necessidades de tratamentos restauradores. Entretanto, o índice de cuidado odontológico em jovens de 15 a 19 anos revelou que menos da metade dos dentes atacados pela cárie tinham recebido o cuidado adequado, i.e., estavam restaurados. Este estudo concluiu que, o grande investimento da sociedade brasileira em odontologia, seja no setor público ou privado, não está tendo o retorno esperado, pelo menos para jovens de 15 a 19 anos. O terceiro estudo concluiu que fatores socioeconômicos amplos e flúor na água foram os principais determinantes da variação na prevalência e severidade da cárie em jovens de 15 a 19 anos e que a contribuição do dentista foi relativamente pequena. Diante do papel relativamente pequeno do dentista na prevenção da cárie, o esforço clínico do mesmo deveria, portanto, enfatizar tratamentos de maior complexidade, visando a restauração e reabilitação de danos relevantes para a função e bem estar (Serviço Pessoal de Saúde). Esforços efetivos para evitar a cárie dentária ocorrem principalmente no âmbito de estratégias preventivas populacionais (Serviço não Pessoal de Saúde), com uma contribuição relativamente pequena do trabalho clínico.

Dados demográficos confirmam o fenômeno do envelhecimento da população brasileira. Entretanto idosos que têm a ventura de desfrutar mais anos de vida, nem sempre conseguem fazê-lo com saúde. Isso é evidente, quando se analisa a condição de saúde bucal. Por várias razões, muitos idosos são institucionalizados para receberem a atenção e o auxílio de pessoal capacitado. Dentre os diversos serviços fornecidos, tanto hospitalares quanto de hotelaria, o cuidado odontológico não tem sido prioritário, e costumeiramente se observa flagrantes omissões. O objetivo deste trabalho foi identificar as características da provisão de cuidado odontológico proporcionado aos idosos residentes em instituições geriátricas de pequeno porte no município de Porto Alegre/RS, e estabelecer sua relação com as condições de higiene bucal deste grupo. Combinando métodos quantitativos e qualitativos de pesquisa, buscou-se coletar dados sobre a higiene bucal dos idosos institucionalizados e informações sobre cuidado odontológico com seus cuidadores e supervisores nas 12 casas geriátricas integrantes da amostra. A análise dos depoimentos dos supervisores e cuidadores demonstra que o cuidado odontológico não segue qualquer tipo de protocolo, padronização ou normatização, sendo realizado empiricamente e, em última análise, sob a responsabilidade e critério dos cuidadores. O índice de placa médio em dentes encontrado nos idosos resultou 2,24, segundo os parâmetros de Sillnes e Löe. No exame em próteses, segundo o índice de Ambjornsen, o resultado apurado atingiu 7,14. Embora os entrevistados considerem que o cuidado odontológico seja realizado com regularidade, questiona-se a sua eficácia diante dos altos níveis de placa observados. Os dados indicam que as características empíricas dos procedimentos de higiene bucal, desassociados de adequada orientação técnica, especialmente odontogeriátrica, ante o descompromisso organizacional privado, institucional público, e até familiar, implicam um cuidado odontológico bastante insatisfatório, sugerindo que os idosos institucionalizados estão sendo negligenciados permanentemente no âmbito odontológico.

A saliva, secreção produzida pelas glândulas salivares, é de vital importância à manutenção da saúde bucal. Com o avançar da idade, as glândulas salivares sofrem mudanças estruturais e funcionais. A exposição das mesmas, durante tomadas radiográficas de interesse odontológico, também pode gerar algum tipo de alteração. Assim sendo, o objetivo deste estudo foi avaliar as alterações morfológicas e morfométricas das glândulas parótidas de camundongos submetidos à radiação X ao longo do envelhecimento. Para tanto, selecionou-se 20 animais que posteriormente foram divididos aleatoriamente em dois grupos, sendo um teste (10 animais) e outro controle (10 animais). Os camundongos do grupo teste foram expostos a 0,12Gy semanais durante 30 semanas, enquanto que os camundongos do grupo controle foram submetidos às mesmas condições, excetuando-se a exposição à radiação. Transcorridas as 30 semanas, os animais foram mortos, após a remoção das glândulas parótidas. Essas foram processadas, cortadas e coradas pela técnica histoquímica de H/E. Análises qualitativa e quantitativa foram realizadas para verificar a existência de possíveis diferenças entre as parótidas pertencentes aos animais do grupo teste em relação às mesmas do grupo controle. Através dos resultados obtidos, por meio da metodologia empregada neste estudo, concluiu-se que não houve alterações morfológicas e morfométricas substanciais nas glândulas parótidas de camundongos expostos à radiação X durante o envelhecimento.

Uma apreciação de um programa de prestação de serviços odontológicos para crianças de 4 a 6 anos, desenvolvido pela Seção de Odontologia da Secretaria de Saúde e Promoção Social da Prefeitura do Município de são Bernardo do Campo, a partir de agosto de 1979. Ele se propunha a priorizar o atendimento da faixa etária acima mencionada, para integrar os recursos odontológicos municipais com os do Serviço Dentário Escolar do Estado e, com isto, obter uso mais racional dos recursos disponíveis, de forma a propiciar cobertura mais ampla do atendimento de saúde bucal á população infantil, do Município. Procura-se aqui fazer a análise dos aspectos administrativos do Programa, desde sua formulação até as etapas de implementação; estuda-se os resultados obtidos em uma amostra da população alvo do Programa depois de um ano e meio de vigência do mesmo. Para tanto são utilizadas, como referencial teórico, a evolução da organização dos serviços odontológicos, a teoria dos sistemas e as técnicas de planejamento e avaliação. Neste trabalho procuramos analisar e avaliar as fases de planejamento e implementação do Programa, buscando principalmente os aspectos administrativos envolvidos no problema.

O objetivo desta pesquisa foi avaliar o impacto das condições bucais no desempenho diário em uma amostra representativa de 276 funcionários públicos com idades entre 35 e 44 anos do Departamento Municipal de Limpeza Urbana da Prefeitura de Porto Alegre, sul do Brasil. As informações sobre os impactos odontológicos e características socioeconômicas foram coletadas por meio de entrevistas estruturadas. O índice Oral Impact on Daily Performances (OIDP), desenvolvido por Adulyanon, Vourapukjaru e Sheiham (1996) foi utilizado para medir os impactos odontológicos. O índice CPOD (Número de Dentes Cariados, Perdidos e Obturados) foi utilizado para avaliar as condições de saúde bucal. Três diferentes níveis de condição bucais foram utilizados: CPOD Alto, CPOD Médio e CPOD Baixo. As informações foram analisadas através da regressão logística multinomial. Os resultados mostraram que, do total da amostra, 73,6% tiveram pelo menos um desempenho diário afetado por problemas odontológicos nos últimos seis meses. O desempenho diário mais afetado foi comer e apreciar a comida (48,6%). A falta de dentes (21,7%) e a dor de dente (20,7%) foram as principais causas percebidas de impacto no desempenho diário. Pessoas com CPOD Alto apresentaram 5,8 vezes (95% IC=2,1-16,1) mais chances de apresentar impacto odontológico alto no desempenho diário do que as pessoas com CPOD Baixo e sem impacto odontológico. Sujeitos com cáries de coroa apresentaram 4,3 vezes (95% IC=1,9-9,8) mais chances de apresentar impacto odontológico alto no desempenho diário do que pessoas sem cárie e sem impacto odontológico. Além disso, pessoas com oito ou mais dentes faltantes apresentaram 2,4 vezes (95% IC=1,1-5,3) mais chances de apresentar impactos odontológicos no desempenho diário do que pessoas com até sete dentes faltantes e sem impactos odontológicos Além disso, pessoas com oito ou mais dentes faltantes apresentaram 2,4 vezes (95% IC=1,1-5,3) mais chances de apresentar impactos odontológicos no desempenho diário do que pessoas com até sete dentes faltantes e sem impactos odontológicos. O índice OIDP combinado com a avaliação clínica é útil para analisar como os diferentes níveis de saúde bucal afetam as dimensões físicas, psicológicas e sociais do desempenho diário.

The aims of this study were to analyze the access of dental services by child population, to determine the prevalence of dental caries, gingivitis and malocclusion in resident children from the municipal district of Sobral Ceará and to evaluate the incidence of the dental decay in adolescents associated with the factors related to socioeconomic condition, access to health services and self-perception. This study had as main factor the multidisciplinary represented by the participation of health professional (doctors, dentists, nurses) in the development of the survey's initial reference; student from Human Sciences area to apply the structured questionnaire in domiciliary visits; statistics professionals in the orientation of the analysis to be held and family health team (community health agents, dentists and dental clinic assistants) in the scheduling of domiciliary visits and the accomplishment of oral exam. The sample was determined from the domicile record that included children born between 1990 and 1994 to develop the research Children health conditions in the municipal district of Sobral Ceará . The first sample comprised 3425 parents of children from 5 to 9 years old, living in the urban area at the municipal district of Sobral Ceará, aiming at identifying the most important factors associated to the access to dental service. From this sample, 1021 children were selected in a systematic way, for the accomplishment to the epidemiological study of decay, gingivitis and malocclusion. In the study's third phase, in order to arrange the group to be followed, 688 adolescents were examined and interviewed, by means of the active search from the 1021 individuals that had been previously examined. It was observed that 50.9% of the children had access to dental service at least once in a lifetime. Of this total, 65.3% accomplished it during the last year, and 85.4% of these did in public services, what allows to identify the importance of this sector in the access to dental services. It was observed that the factors that most affected the access to dental 129 services were related to socioeconomic condition, such as the access to health plan, the possession of toothbrush, garbage collecting, mother s schooling, sewerage treatment and malnutrition. In relation to oral diseases, an increase in the DMF-T index according the age was observed, from 0.10 in five years old to 1.66 in the nine years old, while with the dmf-t index, the inverse happened, since the index decreased from 3.59 in five years old to 2.69 in nine years old. In relation to gingivitis, an average 32.7% of the children presented gum bleeding. In what concerns malocclusion, it was observed that 60.3% of the children didn't present any problem, 30.17% had light malocclusion and 9.5% severe malocclusion. The average incidence of dental caries was 1.86 teeth per youngster. Among the studied variables, tooth pain in the last six months, mother's income and school snack, adjusted by the perception about the need of treatment, the mother's schooling and the dentist's appointment at least once in a lifetime, were the variables that presented positive relationship with the high incidence of dental caries on this population by logistic regression. Variables of socioeconomic nature, related to the access to health services and behavior and biological variables presented a relationship with the high caries incidence. The study point out to the need of developing health actions in a humanized way, by an oral health team effectively bound to the population's interest, with the great objective to provide, with the public health services managers, adequate conditions to improve oral health

The present study seeks to understand tooth loss by investigating the social representations in the daily life of elderly individuals, thus characterizing itself as a comparative and analytic research. It is known that tooth loss is a common occurrence in elderly individuals, interfering at the psychosocial and biological levels, through its functional, esthetical and social implications. The area of oral health in Brazil is lacking studies on this topic, especially with respect to the psychosocial aspects of the elderly. The Theory of Social Representations and the Central Nucleus Theory were selected for theoretical-methodological support. The Free Association of Words Test was used, whose inducing stimulus were the words tooth loss , in which each subject was asked to associate 3 words, to respond to a questionnaire related to socio-economic conditions and containing an investigation of tooth loss, access to odontological services and the need for dental treatment, and to undergo a focus group interview. The study sample consisted of 120 individuals 60 years of age or over, resident in Natal, RN, Brazil and participants of the Live Together to Live Better group of the Basic Health Unit of Felipe Camarão Residential District and Unati (Open University of the Elderly); an interview was performed with 36 subjects. Data analysis was performed by Evoc 2000, SPSS/99, Graph Pad and Alceste softwares. The results demonstrate that the central nucleus of the social representations of tooth loss for the Live Together group emerged from the difficulty in eating categories, showing a relation between physiologic necessity, desire and pleasure from eating, not to mention the pain that resulted from justifying the tooth loss. Besides the central discourses, the following peripheral elements were gathered: difficulty in adapting to the prosthesis, treatment and difficulty in speaking. All of these categories, except the last, also comprised the class themes of the group interview. For the Unati group the central nucleus emerged from the socio-economic difficulties categories, demonstrating a narrow relation between poverty, access to health and education and esthetics, confirming in the discourse of common sense, the association between tooth loss and aging. At the margin of the central discourses was collected the peripheral element difficulty in adapting to the prosthesis, found both in the Live Together and Unati groups, which expresses the resistance of the subject to this new situation and the failure of the rehabilitation treatment in the sense of reviving the memory of their natural teeth. All of these categories also constitute the class themes of the of the group interview. Thus, through the study of the social representations, we can reveal a reality in the perspective of the social subjects, contemplating the multiple facets of the social-cultural reality experienced by these individuals

For a complete comprehension of the effect of tooth loss is necessary to listen to the patients that have it. This study, of qualitative approach, investigate, in the dental history of users of SUS, listening to his/her experiences with the services of dental care, the reason that lead his/her to dental lost and the repercussion of this in his/her life. The collect of data was made by narrative interview, obeying to a pre-defined schema. The subjects interviewed were six (three of urban zone and three of rural zone), all of them were users of Family Health Units. The criterions of inclusion were the followings: the presence of tooth lost (total lost in both dental arch or in one of them, or partial lost in at least six elements in one of the arches); age between 25 and 59; male or female; to live in municipal district of São Tomé/RN or Natal/RN. Based on previous interviews was elaborated the odontological history of each patient. Such narratives, systemized in odontological history, were analyzed taking as base the studies of Souza71 and the proposal of Schutze, suggested for Jovchelovitch, Bauer34. The results show that toothache was the main reason for the search of odontological care. The patients confront the ache with home-made medicaments, allopathic ones, and searching for dental care. The searching for exodontics was stimulated for geographic access difficulties or for repressed demand, which as a result produced the aggravation of the lesions and the discredit in restoration s treatment. The self-care practice of tooth-brush with juá or toothpaste and the controlled ingestion of sugar was not sufficient to avoid dental lost. Guilty sentiments were identified in relation with lack of care with teeth. The acceptance of dental lost as a natural factor is an important motivation in lack of pain and in the belief that it was a simple part of life in old age. Life with dental prosthesis makes clear the difference between which was natural and which was unnatural, and difficulties with the prosthesis appeared. The limitation of the prosthesis in its functional aspect can be compensated by esthetic restitution, making possible smiling expression. Starting with this study and considering the high number of dental lost, mainly in low-rent population, which live with toothless limitations or bad-quality prosthesis which do not rehabilitate adequately, we suggested the realization of qualitative researches which include, also, another actors in heath care services such as professionals and administrators

The Specialized Dental Care Centers (CEO) were developed to provide specialized dental care to the population, given the accumulated needs of health since the past. They must operate as units of reference for the Oral Health Team of the Primary Care, complementing the dental procedures performed at this level of attention. This study aims to assess the performance of CEOs of the Grande Natal Health Region as a strategy of Secondary Care consolidation in oral health through users, dentists and managers. For this to try to identify factors about access, hosting and satisfaction with the service, the actions developed in these centers, integration between the CEOs and the Basic Health Units (UBS), considering the reference and counter reference. Data were collected through semi-structured interview, conducted in four CEOs, among 253 users, 31 dentists and 4 managers. It was submitted to descriptive statistical analysis and to content analysis by software ALCESTE 4.5. The results revealed that the specialties of prosthesis, endodontics and surgery were the most sought by 38.2%, 23.7% and 21.7% of respondents, respectively. It was noticed that among users aged 18 to 30 years-old the greatest demand is for the specialty of endodontics (44.4%) and over 50 years for prosthesis (76.4%). There is a weakness in the reference and counter reference between UBS and CEO, because part of users goes directly to the centers without going before to the Primary Care and the majority does not want to return to the dentist of Health Unit. Satisfaction with care was reported by 90.9% of users, because they resolved the problems needed and were welcomed by the team. But the delay in care was the main factor for not satisfaction. For most dentists, some users could solve their problems completely in Primary Care, which shows the existence of unnecessary referrals to the CEOs, however they consider the existence of limiting factors in UBS that compromise the service. Most dentists revealed that some users do not get to CEOs with the basic dental treatment done, and some of them do not counter reference users. It can be concluded that the studied CEOs are being resolutive for those who access them, offering necessary care for the population, and if they don t account with this service, will encounter obstacles to resolution of problems, ranging seek care service in particular, in another public sector, or even giving up treatment. However, it is perceived the need of professionals training to understand the importance of the reference and counter reference, to that they can better serve and guide users. It is also important that cities offer better conditions to UBS and CEOs, so they can work together, with complementary actions of oral health, seeking full care, aiming for better resolution to the users' health problems

In Brazil, 0-5 years old children just have an oral health care system since 1990 s. Innumerable experiences of implantation of the attendance to the babies in the cities had appeared throughout the years, but it hasn´t been evaluated the comparative effect between children displayed and not displayed to the program. In this regard, the main of this research was describe the Early Childhood Oral Health Care in public health service in Natal, Rio Grande do Norte, Brazil and evaluate the impact of this specific oral health care for babies by comparison of indicators between exposed and non-exposed children. It was created an experimental group, formed by children covered by program which was paired, based on sex, age and socioeconomic status, with a control group, formed by uncovered children. After filling ethical application, the parents of children were questioned about some risk factors to dental caries and, in sequence, it was accomplish an oral examination in the child. It was verified the Visible Plaque Index (VPI), Gingival Bleeding Index (GBI), dmf-s and verification of caries activity. The sample was 40 children in each group. The results showed, for VPI, a difference of 7 percentile points for the experimental group, however this difference had no statistical significance, obtained by Student s t test (p=0.314). In relation to GBI, the control group showed a low mean (0.8%) comparing with experimental group (2.77%) and this difference was statistically significant (p=0.003). The results for dmf-s and evaluation of caries activity showed no statistical difference between groups. Among the probable reasons for absence of impact of intervention, could be included: (a) the practice model was the same in two groups, or the difference was very weak and (b) the oral health care has intrinsic limitations for to impact on oral health in low income populations

Oral health measures directed to children are characterized by the lack of policies that consider the individual and his biopsychosocial aspects. As a consequence, it can be observed that a child´s dental experience has normally been prejudged as unpleasant and threatening. Thus, this study is aimed at investigating dental treatment from the child´s viewpoint using the theory of social representations for theoretical/methodological support. This theory was selected for being able to build practical knowledge surrounding the interrelations between social actors, the phenomenon and the world around them. The investigation was performed at the Professor José Fernandes Machado State School, located in the Ponta Negra district of Natal-RN-Brazil. The participants consisted of 30 children from the public elementary school system in the 6-10 year age group, who had undergone dental treatment within the previous year or were being treated at the time of data collection. An in-depth interview and story drawing within a theme were used as collection instruments. Thematic Content Analysis (Bardin, 2002) was used to analyze the material collected in the interviews, whereas for story drawing within a theme, in addition to content analysis, the data were also analyzed by the Coutinho model (2001). The two instruments demonstrated similarity of content as well as complementarity and contained categories associated to the model for dental treatment in children, the description of the dental office environment., the perception of the image of the dentist, as well as psychological and behavioral manifestations. The results enable us to understand determinate reactions of the children in relation to dental treatment, which has been characterized by its technical-curative approach, where the motive for seeking treatment, the manner in which it is performed and the relation of the dentist with the child are all in evidence. Thus, this study intends not only to reveal the viewpoint of children faced with dental treatment, but also to contribute to a dental practice directed towards them, in this way instilling in the professionals who attend them a more concrete awareness of the needs, anxieties and feelings of these young patients

The objective of this study was to evaluate a baby dentistry program which has been in the city of Natal for five years. The research was based on two analysis: a quantitative one, which tried to check caries prevalence in 52 children supported by the program and a qualitative one, which, through interviews carried out with the mothers, using the focus group technique, tried to realize the effect of the program their attitudes related to the care of their children´s oral health. For the quantitative analyses of caries prevalence there was the use of def-s and knutson rates and the values obtained were shown in a descritive and analytical way.The results showed that 4.2% of babies ranging 24 to 36 months old presented carie disease. Children from 36 to 48 months old and above 48 months old presented 18.2% and 16.7% carie prevalence respectively.The def-s averages found in ages 24 to 36, 36 to 48 and 48 to 60 months old were 0.08; 0.40 and 1.16 respectively, considered as being equally low, going along with Knutson rates. The qualitative analyses showed a high level of the mothers` satisfaction, considering the access,practitioner-patient relationship and opportunity to keep the oral health. Children supported by the program showed a low carie prevalence and the program turned out to be effective under the mothers` point of view
