Currently, there are limited published data for the population dynamics of antimicrobial-resistant commensal bacteria. This study was designed to evaluate both the proportions of the Escherichia coli populations that are resistant to ampicillin at the level of the individual chicken on commercial broiler farms and the feasibility of obtaining repeated measures of fecal E. coli concentrations. Short-term temporal variation in the concentration of fecal E. coli was investigated, and a preliminary assessment was made of potential factors involved in the shedding of high numbers of ampicillin-resistant E. coli by growing birds in the absence of the use of antimicrobial drugs. Multilevel linear regression modeling revealed that the largest component of random variation in log-transformed fecal E. coli concentrations was seen between sampling occasions for individual birds. The incorporation of fixed effects into the model demonstrated that the older, heavier birds in the study were significantly more likely (P = 0.0003) to shed higher numbers of ampicillin-resistant E. coli. This association between increasing weight and high shedding was not seen for the total fecal E. coli population (P = 0.71). This implies that, in the absence of the administration of antimicrobial drugs, the proportion of fecal E. coli that was resistant to ampicillin increased as the birds grew. This study has shown that it is possible to collect quantitative microbiological data on broiler farms and that such data could make valuable contributions to risk assessments concerning the transfer of resistant bacteria between animal and human populations.

Rationale: Opioid antagonism reduces the consumption of palatable foods in humans but the neural substrates implicated in these effects are less well understood. Objectives: The aim of the present study was to examine the effects of the opioid antagonist, naltrexone, on neural response to rewarding and aversive sight and taste stimuli. Methods: We used functional magnetic resonance imaging (fMRI) to examine the neural responses to the sight and taste of pleasant (chocolate) and aversive (mouldy strawberry) stimuli in 20 healthy volunteers who received a single oral dose of naltrexone (50 mg) and placebo in a double-blind, repeated-measures cross-over, design. Results: Relative to placebo, naltrexone decreased reward activation to chocolate in the dorsal anterior cingulate cortex and caudate, and increased aversive-related activation to unpleasant strawberry in the amygdala and anterior insula. Conclusions: These findings suggest that modulation of key brain areas involved in reward processing, cognitive control and habit formation such as the dorsal anterior cingulate cortex (dACC) and caudate might underlie reduction in food intake with opioid antagonism. Furthermore we show for the first time that naltrexone can increase activations related to aversive food stimuli. These results support further investigation of opioid treatments in obesity.

This study investigates the effects of a short-term pedagogic intervention on the development of L2 fluency among learners studying English for Academic purposes (EAP) at a university in the UK. It also examines the interaction between the development of fluency, and complexity and accuracy. Through a pre-test, post-test design, data were collected over a period of four weeks from learners performing monologic tasks. While the Control Group (CG) focused on developing general speaking and listening skills, the Experimental Group (EG) received awareness-raising activities and fluency strategy training in addition to general speaking and listening practice i.e following the syllabus. The data, coded in terms of a range of measures of fluency, accuracy and complexity, were subjected to repeated measures MANOVA, t-tests and correlations. The results indicate that after the intervention, while some fluency gains were achieved by the CG, the EG produced statistically more fluent language demonstrating a faster speech and articulation rate, longer runs and higher phonation time ratios. The significant correlations obtained between measures of accuracy and learners’ pauses in the CG suggest that pausing opportunities may have been linked to accuracy. The findings of the study have significant implications for L2 pedagogy, highlighting the effective impact of instruction on the development of fluency.

This case series compares patient experiences and therapeutic processes between two modalities of cognitive behaviour therapy (CBT) for depression: computerized CBT (cCBT) and therapist-delivered CBT (tCBT). In a mixed-methods repeated-measures case series, six participants were offered cCBT and tCBT in sequence, with the order of delivery randomized across participants. Questionnaires about patient experiences were administered after each session and a semi-structured interview was completed with each participant at the end of each therapy modality. Therapy expectations, patient experiences and session impact ratings in this study generally favoured tCBT. Participants typically experienced cCBT sessions as less meaningful, less positive and less helpful compared to tCBT sessions in terms of developing understanding, facilitating problem-solving and building a therapeutic relationship.

Background Serotonin is under-researched in attention deficit hyperactivity disorder (ADHD), despite accumulating evidence for its involvement in impulsiveness and the disorder. Serotonin further modulates temporal discounting (TD), which is typically abnormal in ADHD relative to healthy subjects, underpinned by reduced fronto-striato-limbic activation. This study tested whether a single acute dose of the selective serotonin reuptake inhibitor (SSRI) fluoxetine up-regulates and normalizes reduced fronto-striato-limbic neurofunctional activation in ADHD during TD. Method Twelve boys with ADHD were scanned twice in a placebo-controlled randomized design under either fluoxetine (between 8 and 15 mg, titrated to weight) or placebo while performing an individually adjusted functional magnetic resonance imaging TD task. Twenty healthy controls were scanned once. Brain activation was compared in patients under either drug condition and compared to controls to test for normalization effects. Results Repeated-measures whole-brain analysis in patients revealed significant up-regulation with fluoxetine in a large cluster comprising right inferior frontal cortex, insula, premotor cortex and basal ganglia, which further correlated trend-wise with TD performance, which was impaired relative to controls under placebo, but normalized under fluoxetine. Fluoxetine further down-regulated default mode areas of posterior cingulate and precuneus. Comparisons between controls and patients under either drug condition revealed normalization with fluoxetine in right premotor-insular-parietal activation, which was reduced in patients under placebo. Conclusions The findings show that a serotonin agonist up-regulates activation in typical ADHD dysfunctional areas in right inferior frontal cortex, insula and striatum as well as down-regulating default mode network regions in the context of impulsivity and TD.

Objective Underreporting of energy intake is prevalent in food surveys, but there is controversy about which dietary assessment method provides greater underreporting rates. Our objective is to compare validity of self-reported energy intake obtained by three dietary assessment methods with total energy expenditure (TEE) obtained by doubly labeled water (DLW) among Brazilian women. Design We used a cross-sectional study. Subjects/setting Sixty-five females aged 18 to 57 years (28 normal-weight, 10 over-weight, and 27 obese) were recruited from two universities to participate. Main outcome measures TEE determined by DLW, energy intake estimated by three 24-hour recalls, 3-day food record, and a food frequency questionnaire (FFQ). Statistical analyses performed Regression and analysis of variance with repeated measures compared TEE and energy intake values, and energy intake-to-TEE ratios and energy intake-TEE values between dietary assessment methods. Bland and Altman plots were provided for each method. chi(2) test compared proportion of underreporters between the methods. Results Mean TEE was 2,622 kcal (standard deviation [SD] =490 kcal), while mean energy intake was 2,078 kcal (SD=430 kcal) for the diet recalls; 2,044 kcal (SD=479 kcal) for the food record and 1,984 kcal (SD=832 kcal) for the FFQ (all energy intake values significantly differed from TEE; P<0.0001). Bland and Altman plots indicated great dispersion, negative mean differences between measurements, and wide limits of agreement. Obese subjects underreported more than normal-weight subjects in the diet recalls and in the food records, but not in the FFQ. Years of education, income and ethnicity were associated with reporting accuracy. Conclusions The FFQ produced greater under- and overestimation of energy intake. Underreporting of energy intake is a serious and prevalent error in dietary self-reports provided by Brazilian women, as has been described in studies conducted in developed countries.

The multivariate skew-t distribution (J Multivar Anal 79:93-113, 2001; J R Stat Soc, Ser B 65:367-389, 2003; Statistics 37:359-363, 2003) includes the Student t, skew-Cauchy and Cauchy distributions as special cases and the normal and skew-normal ones as limiting cases. In this paper, we explore the use of Markov Chain Monte Carlo (MCMC) methods to develop a Bayesian analysis of repeated measures, pretest/post-test data, under multivariate null intercept measurement error model (J Biopharm Stat 13(4):763-771, 2003) where the random errors and the unobserved value of the covariate (latent variable) follows a Student t and skew-t distribution, respectively. The results and methods are numerically illustrated with an example in the field of dentistry.

In this work we propose and analyze nonlinear elliptical models for longitudinal data, which represent an alternative to gaussian models in the cases of heavy tails, for instance. The elliptical distributions may help to control the influence of the observations in the parameter estimates by naturally attributing different weights for each case. We consider random effects to introduce the within-group correlation and work with the marginal model without requiring numerical integration. An iterative algorithm to obtain maximum likelihood estimates for the parameters is presented, as well as diagnostic results based on residual distances and local influence [Cook, D., 1986. Local influence diagnostics based on estimating equations as the role of a gradient vector derived from any fit function are developed for repeated measures regression analysis. Our proposal generalizes tools used in other studies (Cook, 1986: Cadigan and Farrell, 2002), considering herein local influence diagnostics for a statistical model where estimation involves an estimating equation in which all observations are not necessarily independent of each other. Moreover, the measures of local influence are illustrated with some simulated data sets to assess influential observations. Applications using real data are presented. (C) 2010 Elsevier B.V. We consider a generalized leverage matrix useful for the identification of influential units and observations in linear mixed models and show how a decomposition of this matrix may be employed to identify high leverage points for both the marginal fitted values and the random effect component of the conditional fitted values. We illustrate the different uses of the two components of the decomposition with a simulated example as well as with a real data set.

Mixed linear models are commonly used in repeated measures studies. They account for the dependence amongst observations obtained from the same experimental unit. Often, the number of observations is small, and it is thus important to use inference strategies that incorporate small sample corrections. In this paper, we develop modified versions of the likelihood ratio test for fixed effects inference in mixed linear models. In particular, we derive a Bartlett correction to such a test, and also to a test obtained from a modified profile likelihood function. Our results generalize those in [Zucker, D.M., Lieberman, O., Manor, O., 2000. Improved small sample inference in the mixed linear model: Bartlett correction and adjusted likelihood. Journal of the Royal Statistical Society B, 62,827-838] by allowing the parameter of interest to be vector-valued. Additionally, our Bartlett corrections allow for random effects nonlinear covariance matrix structure. We report simulation results which show that the proposed tests display superior finite sample behavior relative to the standard likelihood ratio test. An application is also presented and discussed. A challenge for the clinical management of Parkinson's disease (PD) is the large within- and between-patient variability in symptom profiles as well as the emergence of motor complications which represent a significant source of disability in patients. This thesis deals with the development and evaluation of methods and systems for supporting the management of PD by using repeated measures, consisting of subjective assessments of symptoms and objective assessments of motor function through fine motor tests (spirography and tapping), collected by means of a telemetry touch screen device. One aim of the thesis was to develop methods for objective quantification and analysis of the severity of motor impairments being represented in spiral drawings and tapping results. This was accomplished by first quantifying the digitized movement data with time series analysis and then using them in data-driven modelling for automating the process of assessment of symptom severity. The objective measures were then analysed with respect to subjective assessments of motor conditions. Another aim was to develop a method for providing comparable information content as clinical rating scales by combining subjective and objective measures into composite scores, using time series analysis and data-driven methods. The scores represent six symptom dimensions and an overall test score for reflecting the global health condition of the patient. In addition, the thesis presents the development of a web-based system for providing a visual representation of symptoms over time allowing clinicians to remotely monitor the symptom profiles of their patients. The quality of the methods was assessed by reporting different metrics of validity, reliability and sensitivity to treatment interventions and natural PD progression over time. Results from two studies demonstrated that the methods developed for the fine motor tests had good metrics indicating that they are appropriate to quantitatively and objectively assess the severity of motor impairments of PD patients. The fine motor tests captured different symptoms; spiral drawing impairment and tapping accuracy related to dyskinesias (involuntary movements) whereas tapping speed related to bradykinesia (slowness of movements). A longitudinal data analysis indicated that the six symptom dimensions and the overall test score contained important elements of information of the clinical scales and can be used to measure effects of PD treatment interventions and disease progression. A usability evaluation of the web-based system showed that the information presented in the system was comparable to qualitative clinical observations and the system was recognized as a tool that will assist in the management of patients.

Introdução: Sabe-se que a cirurgia de revascularização miocárdica está associada com alteração dos mediadores inflamatórios e da função imunitária, com ativação precoce dos linfócitos que poderia ser responsável pela linfopenia e diminuição da atividade dos linfócitos no pós-operatório. A elevação enzimática está diminuída na cirurgia sem circulação extracorpórea mas este achado não está associado a melhor evolução clínica. Nesta tese, testamos a hipótese de que a cirurgia de revascularização miocárdica realizada sem circulação extracorpórea pode levar a uma ativação linfocitária de menor intensidade do que a cirurgia com circulação extracorpórea. Métodos: A resposta da ativação linfocitária foi estudada durante o período trans e pósoperatório em 28 pacientes randomizados para cirurgia de coronária sem circulação extracorpórea (n=13) ou cirurgia convencional com circulação extracorpórea (n=15), utilizando citometria de fluxo para determinar a expressão de CD25, CD26, CD69 e DR em linfócitos T (CD3+) e B (CD19+), em sangue periférico. No mesmo período foram realizadas dosagens de troponina I por quimioluminescência e realizado ecocardiograma uni-bidimensional antes e após a cirurgia. Resultados: Não houve diferença estatisticamente significativa para nenhum dos marcadores de ativação linfocitária quando comparados os grupos operados sem ou com circulação extracorpórea (ANOVA bicaudal para medidas repetidas, p>0,05). Considerando todos os pacientes estudados, houve uma elevação da expressão proporcional de CD25 e CD69 em linfócitos T (CD3+) e B (CD19+). Nos linfócitos T, o valor proporcional médio mais elevado (+ EP) de CD69 foi observado 6 horas após terem sido completadas as anastomoses (+75 + 476%) e CD25 teve uma elevação mais gradual, com o pico de seu valor médio (+48 + 24 %) ocorrendo 24 horas após a revascularização. Em linfócitos B, o pico do valor médio de CD69 (+104 + 269 %) ocorreu também após o fim das anastomoses. CD25 teve seu pico de valor médio (+150 + 773 %) 112 horas após a revascularização e seu último valor medido ainda estava elevado. A expressão de CD26 em linfócitos T teve um aparente declínio nos seus valores proporcionais médios (-42 + 32 %) 12 horas após o fim das anastomoses. Não houve diferença significativa na elevação enzimática entre os dois grupos (teste estatístico >0,05). No ecocardiograma, o grupo operado sem circulação extracorpórea apresentou diminuição do volume diastólico (p=0,001) de da fração de ejeção (P=0,012), enquanto no grupo com circulação extracorpórea, diminuíram os volumes diastólico (p=0,006) e sistólico (p=0,01). Conclusões: 1) Comparando a cirurgia de revascularização miocárdica com circulação extracorpórea, a cirurgia sem circulação extracorpórea não reduz a ativação dos linfócitos. 2) A cirurgia de revascularização miocárdica produz uma ativação precoce dos linfócitos, com aumento da expressão de CD69 e CD25 em linfócitos T (CD3+) e B (CD19+), em sangue periférico. A elevação precoce de CD69, e elevação mais tardia de CD25, pode indicar duas partes de uma seqüência de ativação linfocitária. 3) O comportamento das enzimas cardíacas e dos achados ecocardiográficos não sugere benefício da cirurgia sem circulação extracorpórea sobre o dano miocárdio.

O objetivo desta pesquisa, de delineamento quase-experimental, foi verificar a influência de um Programa de Intervenção Motora Inclusiva (PIMI) no desenvolvimento motor (DM) e social (DS) de crianças, portadoras (PNEE) e não portadoras de necessidades educacionais especiais (n-PNEE), com atrasos motores. A amostra desta pesquisa foi não probabilística, intencional, composta por 76 crianças (43 meninos e 33 meninas), com idades de 4 a 10 anos (M=7,00, DP=1,44), sendo 24 (31,6%) crianças PNEE e 52 (68,4%) crianças n-PNEE, que apresentaram desempenho motores inferiores a seus pares, configurando atrasos motores, avaliados por meio do Test of Gross Motor Development- 2 (TGMD-2) (ULRICH, 2000). Trinta e cinco crianças constituíram o Grupo de Intervenção (GI) e quarenta e uma crianças constituíram o Grupo Controle (GC). Para a avaliação do DM das crianças dos grupos foi utilizado o TGMD-2 e para a avaliação do DS das crianças do GI foi utilizado a estrutura de Níveis de Responsabilidade Social e Pessoal (HELLISON, 2003). O PIMI foi desenvolvido em 14 semanas, implementando os princípios do Contexto Motivacional para a Maestria e os pressupostos da estrutura TARGET. General Linear Model com medidas repetidas no fator tempo foi conduzida para avaliar os efeitos do PIMI no DM das crianças. Para a análise do DS foi utilizado o teste de Friedman. Os resultados indicaram que (1) crianças, PNEE e n- PNEE, do GI demonstraram ganhos significantes em habilidades de locomoção e de controle de objeto do pré-teste para o pós-teste, enquanto que para as crianças, PNEE e n-PNEE, do GC mudanças significativas não foram encontradas, (2) crianças, PNEE e n-PNEE, do GI demonstraram desempenho significantemente superior em habilidades de locomoção e de controle de objeto comparadas as crianças, PNEE e n-PNEE, do GC no pós-teste, (3) crianças PNEE, do GI, demonstraram padrões de mudanças positivas e significativas do pré-teste para o pós-teste nas habilidades de locomoção e de controle de objeto semelhantes aos seus pares n-PNE do mesmo grupo, (4) crianças PNEE, do GI, demonstraram no pós-teste desempenho significantemente superior nas habilidades de locomoção e controle de objetos comparadas aos seus pares PNEE do GC, (5) crianças n-PNEE, do GI, demonstraram no pós-teste desempenho significantemente superior nas habilidades de locomoção e de controle de objeto comparadas aos seus pares n-PNEE do GC, (6) crianças, PNEE e n-PNEE, do GI, demonstraram mudanças positivas e significativas no DS no contexto de aprendizagem por meio da conquista de níveis de responsabilidade social e pessoal mais elevados, no decorrer do PIMI, (7) crianças PNEE, do GI, demonstraram padrões de mudanças positivas e significativas no DS semelhantes aos seus pares n-PNEE do mesmo grupo. E mais, a implementação do Contexto Motivacional para a Maestria possibilitou a participação cooperativa e efetiva de todas as crianças indiferentemente dos níveis de habilidade motora.

As digital systems move away from traditional desktop setups, new interaction paradigms are emerging that better integrate with users’ realworld surroundings, and better support users’ individual needs. While promising, these modern interaction paradigms also present new challenges, such as a lack of paradigm-specific tools to systematically evaluate and fully understand their use. This dissertation tackles this issue by framing empirical studies of three novel digital systems in embodied cognition – an exciting new perspective in cognitive science where the body and its interactions with the physical world take a central role in human cognition. This is achieved by first, focusing the design of all these systems on a contemporary interaction paradigm that emphasizes physical interaction on tangible interaction, a contemporary interaction paradigm; and second, by comprehensively studying user performance in these systems through a set of novel performance metrics grounded on epistemic actions, a relatively well established and studied construct in the literature on embodied cognition. The first system presented in this dissertation is an augmented Four-in-a-row board game. Three different versions of the game were developed, based on three different interaction paradigms (tangible, touch and mouse), and a repeated measures study involving 36 participants measured the occurrence of three simple epistemic actions across these three interfaces. The results highlight the relevance of epistemic actions in such a task and suggest that the different interaction paradigms afford instantiation of these actions in different ways. Additionally, the tangible version of the system supports the most rapid execution of these actions, providing novel quantitative insights into the real benefits of tangible systems. The second system presented in this dissertation is a tangible tabletop scheduling application. Two studies with single and paired users provide several insights into the impact of epistemic actions on the user experience when these are performed outside of a system’s sensing boundaries. These insights are clustered by the form, size and location of ideal interface areas for such offline epistemic actions to occur, as well as how can physical tokens be designed to better support them. Finally, and based on the results obtained to this point, the last study presented in this dissertation directly addresses the lack of empirical tools to formally evaluate tangible interaction. It presents a video-coding framework grounded on a systematic literature review of 78 papers, and evaluates its value as metric through a 60 participant study performed across three different research laboratories. The results highlight the usefulness and power of epistemic actions as a performance metric for tangible systems. In sum, through the use of such novel metrics in each of the three studies presented, this dissertation provides a better understanding of the real impact and benefits of designing and developing systems that feature tangible interaction.
