It is recognised that patients with chronic disease are unable to remembercorrectly information provided by health care profesionals. The teach-back method is acknowledgedas a technique to improve patients’ understanding. Yet it is not used in nursing practice in Vietnam. Objectives This study sought to examine knowledge background of heart failure among cardiac nurses, introduce a education about heart failure self-management and the teach-back method to assist teaching patients on self-care. The study also wanted to explore if a short education could benefit nurses’ knowledge so they would be qualified to deliver education to patients. Methods A pre/post-test design was employed. Cardiac nurses from 3 hospitals (Vietnam National Heart Institute, E Hospital, Huu Nghi Hospital) were invited to attend a six-hour educational session which covered both the teach-back method and heart failure self-management. Role-play with scenarios were used to reinforce educational contents. The Dutch Heart Failure Knowledge Scale was used to assess nurses’ knowledge of heart failure at baseline and after the educational session. Results 20 nurses from3 selected hospitals participated. Average age was 34.5±7.9 years and years of nursing experience was 11.6±8.3. Heart failure knowledge score at the baseline was 12.7±1.2 and post education was 13.8±1.0. There was deficiency of nurses knowledge regarding fluid restriction among heart failure people, causes of worsening heart failure. Heart failure knowledge improved significantly following the workshop (p < 0.001). All nurses achieved an overall adequate knowledge score (≥11 of the maximum 15) at the end. 100% of nurses agreed that the teach-back method was effective and could be used to educate patients about heart failure self-management. Conclusions The results of this study have shown the effectiveness of the piloteducaiton in increasing nurses’ knowledge of heart failure. The teach-back method is accepted for Vietnamese nurses to use in routine cardiac practice.

- BACKGROUND Chronic diseases are increasing worldwide and have become a significant burden to those affected by those diseases. Disease-specific education programs have demonstrated improved outcomes, although people do forget information quickly or memorize it incorrectly. The teach-back method was introduced in an attempt to reinforce education to patients. To date, the evidence regarding the effectiveness of health education employing the teach-back method in improved care has not yet been reviewed systematically. - OBJECTIVES This systematic review examined the evidence on using the teach-back method in health education programs for improving adherence and self-management of people with chronic disease. - INCLUSION CRITERIA Types of participants: Adults aged 18 years and over with one or more than one chronic disease. Types of intervention: All types of interventions which included the teach-back method in an education program for people with chronic diseases. The comparator was chronic disease education programs that did not involve the teach-back method. Types of studies: Randomized and non-randomized controlled trials, cohort studies, before-after studies and case-control studies. Types of outcomes: The outcomes of interest were adherence, self-management, disease-specific knowledge, readmission, knowledge retention, self-efficacy and quality of life. - SEARCH STRATEGY Searches were conducted in CINAHL, MEDLINE, EMBASE, Cochrane CENTRAL, Web of Science, ProQuest Nursing and Allied Health Source, and Google Scholar databases. Search terms were combined by AND or OR in search strings. Reference lists of included articles were also searched for further potential references. - METHODOLOGICAL QUALITY Two reviewers conducted quality appraisal of papers using the Joanna Briggs Institute Meta-Analysis of Statistics Assessment and Review Instrument. - DATA EXTRACTION Data were extracted using the Joanna Briggs Institute Meta-Analysis of Statistics Assessment and Review Instrument data extraction instruments. - DATA SYNTHESIS There was significant heterogeneity in selected studies, hence a meta-analysis was not possible and the results were presented in narrative form. - RESULTS Of the 21 articles retrieved in full, 12 on the use of the teach-back method met the inclusion criteria and were selected for analysis. Four studies confirmed improved disease-specific knowledge in intervention participants. One study showed a statistically significant improvement in adherence to medication and diet among type 2 diabetics patients in the intervention group compared to the control group (p < 0.001). Two studies found statistically significant improvements in self-efficacy (p = 0.0026 and p < 0.001) in the intervention groups. One study examined quality of life in heart failure patients but the results did not improve from the intervention (p = 0.59). Five studies found a reduction in readmission rates and hospitalization but these were not always statistically significant. Two studies showed improvement in daily weighing among heart failure participants, and in adherence to diet, exercise and foot care among those with type 2 diabetes. - CONCLUSION Overall, the teach-back method showed positive effects in a wide range of health care outcomes although these were not always statistically significant. Studies in this systematic review revealed improved outcomes in disease-specific knowledge, adherence, self-efficacy and the inhaler technique. There was a positive but inconsistent trend also seen in improved self-care and reduction of hospital readmission rates. There was limited evidence on improvement in quality of life or disease related knowledge retention.

Background: Increased hospital readmission and longer stays in the hospital for patients with type 2 diabetes and cardiac disease can result in higher healthcare costs and heavier individual burden. Thus, knowledge of the characteristics and predictive factors for Vietnamese patients with type 2 diabetes and cardiac disease, at high risk of hospital readmission and longer stays in the hospital, could provide a better understanding on how to develop an effective care plan aimed at improving patient outcomes. However, information about factors influencing hospital readmission and length of stay of patients with type 2 diabetes and cardiac disease in Vietnam is limited. Aim: This study examined factors influencing hospital readmission and length of stay of Vietnamese patients with both type 2 diabetes and cardiac disease. Methods: An exploratory prospective study design was conducted on 209 patients with type 2 diabetes and cardiac disease in Vietnam. Data were collected from patient charts and patients' responses to self-administered questionnaires. Descriptive statistics, bivariate correlation, logistic and multiple regression were used to analyse the data. Results: The hospital readmission rate was 12.0% among patients with both type 2 diabetes and cardiac disease. The average length of stay in the hospital was 9.37 days. Older age (OR= 1.11, p< .05), increased duration of type 2 diabetes (OR= 1.22, p< .05), less engagement in stretching/strengthening exercise behaviours (OR= .93, p< .001) and in communication with physician (OR= .21, p< .001) were significant predictors of 30-dayhospital readmission. Increased number of additional co-morbidities (β= .33, p< .001) was a significant predictor of longer stays in the hospital. High levels of cognitive symptom management (β= .40, p< .001) significantly predicted longer stays in the hospital, indicating that the more patients practiced cognitive symptom management, the longer the stay in hospital. Conclusions: This study provides some evidence of factors influencing hospital readmission and length of stay and argues that this information may have significant implications for clinical practice in order to improve patients' health outcomes. However, the findings of this study related to the targeted hospital only. Additionally, the investigation of environmental factors is recommended for future research as these factors are important components contributing to the research model.

OBJECTIVE We aimed to 1) describe the peripartum management of type 1 diabetes at an Australian teaching hospital and 2) discuss factors influencing the apparent transient insulin independence postpartum. RESEARCH DESIGN AND METHODS We conducted a retrospective review of women with type 1 diabetes delivering singleton pregnancies from 2005 to 2010. Information was collected regarding demographics, medical history, peripartum management and outcome, and breast-feeding. To detect a difference in time to first postpartum blood glucose level (BGL) >8 mmol/L between women with an early (<4 h) and late (>12 h) requirement for insulin postpartum, with a power of 80% and a type 1 error of 0.05, at least 24 patients were required. RESULTS An intravenous insulin infusion was commenced in almost 95% of women. Univariate analysis showed that increased BMI at term, lower creatinine at term, longer duration from last dose of long- or intermediate-acting insulin, and discontinuation of an insulin infusion postpartum were associated with a shorter time to first requirement of insulin postpartum (P = 0.005, 0.026, 0.026, and <0.001, respectively). There was a correlation between higher doses of insulin commenced postpartum and number of out-of-range BGLs (r[36] = 0.358, P = 0.030) and hypoglycemia (r[36] = 0.434, P = 0.007). Almost 60% had at least one BGL <3.5 mmol/L between delivery and discharge. CONCLUSIONS Changes in the pharmacodynamic profile of insulin may contribute to the transient insulin independence sometimes observed postpartum in type 1 diabetes. A dose of 50–60% of the prepregnancy insulin requirement resulted in the lowest rate of hypoglycemia and glucose excursions. These results require validation in a larger, prospective study.

Juvenile idiopathic arthritis (JIA) is associated with growth disturbances, especially leg length discrepancy (LLD) and knee valgus deformity (KVD). Studies have demonstrated growth plate stimulation with chronic arthritis. In the context of surgical treatment of LLD or KVD of a growing knee, the less invasive procedures, which allow immediate mobilisation, are preferred. Establishment of the skeletal age and the correction potential in the knees of rheumatic children is difficult due to rheumatic changes. In this present work, an analysis of the efficacy, safety and long-term results of temporary epiphyseal arrests performed in Rheumatism Foundation Hospital (Heinola, Finland). The distribution of diagnoses among children (n=71) with JIA and LLD (68 knees) was consistent with the normal oligoarthritis-predominated population of children with JIA. A higher male:female ratio (1:1.7 vs. 1:2.4 in population-based studies (PBS)) and earlier mean onset age (4 vs. 7 years in PBSs) were, however, distinct features in the study population. In most cases the correction was reliable and temporary arrest produced a mean correction of 1mm per month. The time of arrest required, however, varied significantly, probably due to the effect of underlying diseases and medication, and the age of the child. All complications encountered (10%) were minor. The correction achieved persisted in long-term follow-up. KVD (n=112, 177 knees) was associated with a high proportion of polyarthritic disease subtype (45% vs. 12-31% in PBSs), and the male:female distribution was grossly female-dominated (1:4.9 vs. 1:2.4 in PBSs). The early mean onset age (3 vs. 7 years in PBSs) was also notable in this cohort. Successful correction was achieved in 2/3 cases and the mean angular correction was 0.7 degrees per month. The required time of arrest, however, varied considerably. In 13% of knees the paucity of follow-up visits resulted in over-correction to varus. The complication rate (3%) in the knees operated for KVD was considerably lower compared to ten per cent in the management of LLD. Most of the complications related to epiphyseal stapling were reversible. However, the risk of premature closure of growth plates does exist. The number of over-corrections was notably high, with 13% knees turning to varus. The correction achieved persisted in long-term follow-up.

Technological development of fast multi-sectional, helical computed tomography (CT) scanners has allowed computed tomography perfusion (CTp) and angiography (CTA) in evaluating acute ischemic stroke. This study focuses on new multidetector computed tomography techniques, namely whole-brain and first-pass CT perfusion plus CTA of carotid arteries. Whole-brain CTp data is acquired during slow infusion of contrast material to achieve constant contrast concentration in the cerebral vasculature. From these data quantitative maps are constructed of perfused cerebral blood volume (pCBV). The probability curve of cerebral infarction as a function of normalized pCBV was determined in patients with acute ischemic stroke. Normalized pCBV, expressed as a percentage of contralateral normal brain pCBV, was determined in the infarction core and in regions just inside and outside the boundary between infarcted and noninfarcted brain. Corresponding probabilities of infarction were 0.99, 0.96, and 0.11, R² was 0.73, and differences in perfusion between core and inner and outer bands were highly significant. Thus a probability of infarction curve can help predict the likelihood of infarction as a function of percentage normalized pCBV. First-pass CT perfusion is based on continuous cine imaging over a selected brain area during a bolus injection of contrast. During its first passage, contrast material compartmentalizes in the intravascular space, resulting in transient tissue enhancement. Functional maps such as cerebral blood flow (CBF), and volume (CBV), and mean transit time (MTT) are then constructed. We compared the effects of three different iodine concentrations (300, 350, or 400 mg/mL) on peak enhancement of normal brain tissue and artery and vein, stratified by region-of-interest (ROI) location, in 102 patients within 3 hours of stroke onset. A monotonic increasing peak opacification was evident at all ROI locations, suggesting that CTp evaluation of patients with acute stroke is best performed with the highest available concentration of contrast agent. In another study we investigated whether lesion volumes on CBV, CBF, and MTT maps within 3 hours of stroke onset predict final infarct volume, and whether all these parameters are needed for triage to intravenous recombinant tissue plasminogen activator (IV-rtPA). The effect of IV-rtPA on the affected brain by measuring salvaged tissue volume in patients receiving IV-rtPA and in controls was investigated also. CBV lesion volume did not necessarily represent dead tissue. MTT lesion volume alone can serve to identify the upper size limit of the abnormally perfused brain, and those with IV-rtPA salvaged more brain than did controls. Carotid CTA was compared with carotid DSA in grading of stenosis in patients with stroke symptoms. In CTA, the grade of stenosis was determined by means of axial source and maximum intensity projection (MIP) images as well as a semiautomatic vessel analysis. CTA provides an adequate, less invasive alternative to conventional DSA, although tending to underestimate clinically relevant grades of stenosis.

Background: Otitis media (OM) is one of the most common childhood diseases. Approximately every third child suffers from recurrent acute otitis media (RAOM), and 5% of all children have persistent middle ear effusion for months during their childhood. Despite numerous studies on the prevention and treatment of OM during the past decades, its management remains challenging and controversial. In this study, the effect of adenoidectomy on the risk for OM, the potential risk factors influencing the development of OM and the frequency of asthma among otitis-prone children were investigated. Subjects and methods: One prospective randomized trial and two retrospective studies were conducted. In the prospective trial, 217 children with RAOM or chronic otitis media with effusion (COME) were randomized to have tympanostomy with or without adenoidectomy. The age of the children at recruitment was between 1 and 4 years. RAOM was defined as having at least 3 episodes of AOM during the last 6 months or at least 5 episodes of AOM during the last 12 months. COME was defined as having persistent middle ear effusion for 2-3 months. The children were followed up for one year. In the first retrospective study, the frequency of childhood infections and allergy was evaluated by a questionnaire among 819 individuals. In the second retrospective study, data of asthma diagnosis were analysed from hospital discharge records of 1616 children who underwent adenoidectomy or had probing of the nasolacrimal duct. Results: In the prospective randomized study, adenoidectomy had no beneficial effect on the prevention of subsequent episodes of AOM. Parental smoking was found to be a significant risk factor for OM even after the insertion of tympanostomy tubes. The frequencies of exposure to tobacco smoke and day-care attendance at the time of randomization were similar among children with RAOM and COME. However, the frequencies of allergy to animal dust and pollen and parental asthma were lower among children with COME than those with RAOM. The questionnaire survey and the hospital discharge data revealed that children who had frequent episodes of OM had an increased risk for asthma. Conclusions: The first surgical intervention to treat an otitis-prone child younger than 4 years should not include adenoidectomy. Interventions to stop parental smoking could significantly reduce the risk for childhood RAOM. Whether an otitis-prone child develops COME or RAOM, seems to be influenced by genetic predisposition more strongly than by environmental risk factors. Children who suffer from repeated upper respiratory tract infections, like OM, may be at increased risk for developing asthma.

Persistent pain is a commonly experienced symptom. It affects 25% of community-dwelling older adults and up to 80% of nursing home residents, and can have a major impact on quality of life and functional capacity. Unfortunately pain in older patients is often undertreated and misunderstood. Assessment of pain type and severity is important. Most older people, even with moderately impaired cognition, are able to self-report pain. Validated assessment tools using non-verbal pain cues are available for people with more advanced cognitive impairment. Management of pain in older people can be challenging. Physiological changes may impact on pain perception and the pharmacodynamics and pharmacokinetics of medications. Older people are often more sensitive to the adverse effects of analgesic medications and are at risk of drug–drug interactions due to the presence of co-morbidities and polypharmacy. In general, analgesic medications should be commenced at low doses, titrated based on effect and tolerability, and regularly reviewed. Contemporary pain management often utilises multiple analgesics in lower doses to optimise efficacy and avoid dose-related toxicity. A bio-psycho-social approach to the management of persistent pain, utilising a multidisciplinary team and including non-drug strategies, may produce the best results. The goal of pain management is not always to eliminate pain, since this may not be attainable, but rather to enhance function and improve quality of life. This article discusses persistent non-cancer pain in older people, its assessment and management, and the risks and benefits of pharmacological treatment in this population.

O tema do presente estudo é o gerenciamento de resíduos químicos em ambientes hospitalares. Os resíduos químicos são gerados nas atividades auxiliares dos estabelecimentos prestadores de serviços de saúde, tais como hospitais, laboratórios, serviços de diagnóstico e tratamento, centros de saúde, clínicas, institutos de medicina legal e outros. Dentre todos, os hospitais, por suas características de atendimento, são, sem dúvida, os maiores geradores deste tipo de resíduo. Controlar e diminuir os riscos inerentes a este tipo de resíduos, além de ser uma exigência legal, passa a ser uma necessidade ambiental e um desafio a ser enfrentado pelos administradores de estabelecimentos assistenciais à saúde. O objetivo geral desta pesquisa passa então por avaliar o gerenciamento dos resíduos químicos gerados nos hospitais públicos da região metropolitana do Estado do Rio de Janeiro, abordando às necessidades e dificuldades enfrentadas por estes geradores que incluí aspectos políticos, administrativos, econômicos e em alguns casos físico-estruturais. Para atingir tal objetivo o método empregado foi dividido em duas etapas: I) Pesquisa Aplicada, onde foram visitados alguns hospitais públicos do Estado e utilizou-se um questionário para avaliar as questões relacionadas com o gerenciamento de resíduos químicos e II) Estudo de Caso, onde se considerou, através de observações e entrevistas, o gerenciamento de resíduos químicos de um hospital universitário de grande porte de maneira efetiva. Foram utilizadas como fontes secundárias informações obtidas em seminários relacionados ao tema, reportagens de jornais e entrevistas publicadas em fontes especializadas. Os resultados obtidos mostram que as dificuldades enfrentadas no gerenciamento de RSS, inclusos neste grupo os resíduos químicos, é uma realidade para a maioria dos hospitais da rede pública. Conclusão: o maior problema a ser enfrentado por estas instituições está diretamente ligado à conscientização, ou melhor, a falta de conscientização, de funcionários, médicos e gerência dos hospitais, quanto à importância da correta segregação, armazenagem e manuseio destes resíduos. Além disso, a falta de recursos financeiros e, em alguns casos, até de espaço físico também dificulta o efetivo gerenciamento deste tipo de resíduos. Problemas secundários relacionam-se com a falta de fiscalização dos órgãos sanitário-ambientais competentes e o descaso da própria população que não atenta para os problemas ambientais e de saúde e segurança, decorrentes de um incorreto gerenciamento não só dos resíduos químicos, mas de todos os resíduos de serviço de saúde. Como recomendação, pode-se dizer que se faz necessária uma maior mobilização por parte dos estabelecimentos hospitalares para a discussão e atendimento das legislações aplicáveis e também, o desenvolvimento de uma estrutura gerencial com responsabilidades definidas e ações planejadas, compatíveis com a realidade do serviço público, e que possam levar a alcançar os objetivos e metas de um gerenciamento dos resíduos de serviço de saúde.

Esse trabalho aborda a elaboração de requisitos dos usuários como parte da adequação do sistema de informação-SISTEMA DE INFORMAÇÃO DE GESTÃO ACADÊMICA-Administrativo- SIGA-Adm, proposto para o Hospital Universitário da Universidade Federal de Juiz de Fora. SIGA-Adm está em utilização, desde 2007, na Universidade Federal de Juiz de Fora. No entanto, para a efetiva aplicabilidade no âmbito do Hospital Universitário, torna-se necessário uma investigação acerca dos processos de trabalho, dentro do HU e uma ação de melhoria desses processos, a fim de evitar a incorporação de um sistema de informação que não corresponda a realidade apresentada. A elaboração dos requisitos dos usuários para a adequação visa a ser um instrumento de intermediação para discussão acerca da elaboração do SIGA-Adm para o HU/UFJF. Os requisitos apresentados originaram-se no processo de gerenciamento de materiais, como resultado da utilização da Metodologia de Análise e Melhoria de Processos-MAMP, aplicada na cadeia de suprimentos do Hospital Universitário, no período de 2004 a 2005. O acompanhamento do gerenciamento de materiais é uma estratégia utilizada pela Rede Sentinela para o desenvolvimento de ações de pós-comercialização de produtos para a saúde tais como: investigação de uso, registro de produtos, retirada de produto do mercado e etc... O Hospital Universitário da UFJF é integrante da Rede Brasileira de Hospitais Sentinela, implementada pela Agência Nacional de Vigilância Sanitária, desde 2001. As ações Pós-Comercialização de Produtos para a Saúde, em Tecnovigilância, são compartilhadas pela ANVISA e pelos integrantes da Rede Sentinela, através de informações relativas a queixas técnicas e/ou eventos adversos que possam causar algum dano à população. A pesquisa indica a possibilidade do SIGA-Adm armazenar informações sobre os produtos que permitam o rastreamento em condições normais e/ou anormais de uso pela instituição, contribuindo para o exercício em Tecnovigilância. Além disso, espera-se que construção do SIGA-Adm/HU-UFJF permita integrar os sistemas existentes no HU.

Trata-se de uma pesquisa documental, retrospectiva de fonte secundária,que adota uma abordagem quantitativa descritiva-exploratória. A partir da constatação de altos índices de absenteísmo nas unidades hospitalares, despertou-se o interesse em estudar os custos diretos das doenças ocupacionais que levam aos afastamentos e seu impacto econômico para o orçamento de recursos humanos de um hospital universitário do Rio de Janeiro. Neste contexto, definiu-se como objeto de estudo, o impacto econômico do absenteísmo por doença na equipe de enfermagem e, como objetivos: identificar as causas prevalentes de afastamentos no hospital universitário, de acordo com Classificação Internacional de Doenças e Problemas Relacionados a Saúde (CID-10); estimar os custos diretos mínimos das doenças que afastaram o trabalhador de enfermagem; estimar o custo real aproximado do absenteísmo relacionado a 1 (um) dia de trabalho prestado pelos trabalhadores de enfermagem, com projeção de 1 (um) mês e 1(um) ano numa visão operacional do Sistema Único de Saúde (SUS). Foi utilizada uma amostra estratificada de prontuários dos profissionais de saúde da equipe de enfermagem (enfermeiros e técnicos de enfermagem), a partir do seguinte critério de inclusão: profissionais de enfermagem concursados com afastamento no ano de 2010 e com diagnóstico médico determinante do afastamento, definido claramente. Para a coleta das informações foi feita a apreciação dos documentos arquivados no Serviço de Saúde do Trabalhador do hospital estudado e contou com a apreciação de especialistas médicos relativos aos grupos de diagnósticos estudados, orientados por roteiros criados pela pesquisadora. Os dados foram analisados e armazenados no programa Statistical Package for the Social Sciences (SPSS) versão 15 e no editor Microsoft excel 2003. Dentre os resultados obtidos tiveram destaque para as seguintes causas de afastamento, respectivamente, às doenças do sistema osteomuscular, os fatores que influenciam o estado de saúde e o contato com serviços de saúde, os transtornos mentais e comportamentais, as lesões, envenenamento e outras consequencias de causas externas e, as doenças do sistema circulatório, que representam um custo estimado aproximado de R$ 2,6 milhões. Pôde-se constatar que o impacto econômico do absenteísmo decorrentes dos agravos à saúde para o orçamento de recursos humanos do hospital universitário foi de aproximadamente 2,7%. O custo real aproximado do absenteísmo de enfermagem por dia, foi avaliado em R$ 92,50, tendo projeção mensal de R$ 2.775,00 e anual de R$ 33.300,00. Recomenda-se avaliar o absenteísmo dos profissionais regularmente para identificar as causas reais do absenteísmo por doença, a fim de definir metas para os programas de intervenção à saúde dos trabalhadores e promover uma Gestão participativa que favoreça uma análise do processo de trabalho no que concerne o atendimento das necessidades de saúde e operacionais da força de trabalho, determinantes do absenteísmo.

Background: Limited information is available about predictors of short-term outcomes in patients with exacerbation of chronic obstructive pulmonary disease (eCOPD) attending an emergency department (ED). Such information could help stratify these patients and guide medical decision-making. The aim of this study was to develop a clinical prediction rule for short-term mortality during hospital admission or within a week after the index ED visit. Methods: This was a prospective cohort study of patients with eCOPD attending the EDs of 16 participating hospitals. Recruitment started in June 2008 and ended in September 2010. Information on possible predictor variables was recorded during the time the patient was evaluated in the ED, at the time a decision was made to admit the patient to the hospital or discharge home, and during follow-up. Main short-term outcomes were death during hospital admission or within 1 week of discharge to home from the ED, as well as at death within 1 month of the index ED visit. Multivariate logistic regression models were developed in a derivation sample and validated in a validation sample. The score was compared with other published prediction rules for patients with stable COPD. Results: In total, 2,487 patients were included in the study. Predictors of death during hospital admission, or within 1 week of discharge to home from the ED were patient age, baseline dyspnea, previous need for long-term home oxygen therapy or non-invasive mechanical ventilation, altered mental status, and use of inspiratory accessory muscles or paradoxical breathing upon ED arrival (area under the curve (AUC) = 0.85). Addition of arterial blood gas parameters (oxygen and carbon dioxide partial pressures (PO2 and PCO2)) and pH) did not improve the model. The same variables were predictors of death at 1 month (AUC = 0.85). Compared with other commonly used tools for predicting the severity of COPD in stable patients, our rule was significantly better. Conclusions: Five clinical predictors easily available in the ED, and also in the primary care setting, can be used to create a simple and easily obtained score that allows clinicians to stratify patients with eCOPD upon ED arrival and guide the medical decision-making process.

A Enfuvirtida(ENF), único inibidor de fusão disponível, representa uma opção interessante aos pacientes com infecção pelo HIV quando utilizada em combinação com outros antirretrovirais, principalmente no tratamento de multiexperimentados com falha virológica e poucas opções terapêuticas. Sua eficácia já comprovada em ensaios clínicos esbarra nas barreiras impostas por sua administração parenteral. Impulsionado por estes dados, avaliamos durante 48 semanas a resposta virológica, a evolução de células T CD4 a possível resistência primária a ENF e o impacto para a adesão do uso subcutâneo da droga em dez pacientes que fazem acompanhamento ambulatorial no Hospital Universitário Pedro Ernesto e que tinham história de mais de dez anos de infecção pelo HIV e uso de ENF no seu esquema terapêutico sugerido por teste de resistência. Todos os pacientes alcançaram ao final do seguimento sucesso terapêutico, mantendo carga viral não detectada, e um incremento médio significativo de linfócitos T CD4. Em relação a uma possível resistência primária, em nenhum dos testes, genotipagem da glicoproteína 41, foi visualizado mutações naturais que pudessem diminuir a ação da ENF. Sobre o manejo do medicamento, preparo e aplicação, observamos que é imprescindível um apoio multidisciplinar para que não haja descontinuação na sua utilização

Os Hospitais Universitários (HUs) enfrentam muitos problemas em sua gestão financeira. Assim, receitas insuficientes para suprir despesas culminam numa situação deficitária. O índice de inadimplência dos hospitais é alto; por conseqüência, o prazo de pagamento dos fornecedores prorrogado. No ano de 2006, foi estabelecida uma reestruturação financeira e orçamentária denominada Plano Operativo Anual (POA), juntamente com o Ministério da Saúde e o Ministério da Educação, no intuito de tentar minimizar o déficit dos HUs. Tendo em vista tal situação, este trabalho de pesquisa visa a relatar como um Sistema Integrado de Gestão (ERP) pode aprimorar, bem como monitorar, o desempenho do POA e sua eficácia servindo de instrumento de orçamento das receitas hospitalares baseados em metas. A partir deste contexto, teremos como ponto de partida o estudo do caso do aprimoramento quanto à inserção do Hospital Universitário Gaffrée e Guinle (HUGG) no Sistema Único de Saúde (SUS), através da contratação dos serviços de saúde ofertados pelo hospital e seu respectivo modelo de financiamento, bem como os mecanismos de acompanhamento e avaliação das atividades de atenção, gestão, ensino e pesquisa. Com isso, ocorre a agilização dos procedimentos diários, maior eficiência na gestão hospitalar. Evita-se assim; entre outros; problemas concernentes ao setor financeiro da instituição.
