“Closing the gap in curriculum development leadership” is a Carrick-funded University of Queensland project which is designed to address two related gaps in current knowledge and in existing professional development programs for academic staff. The first gap is in our knowledge of curriculum and pedagogical issues as they arise in relation to multi-year sequences of study, such as majors in generalist degrees, or core programs in more structured degrees. While there is considerable knowledge of curriculum and pedagogy at the course or individual unit of study level (e.g. Philosophy I), there is very little properly conceptualised, empirically informed knowledge about student learning (and teaching) over, say, a three-year major sequence in a traditional Arts or Sciences subject. The Carrick-funded project aims to (begin to) fill this gap through bottom-up curriculum development projects across the range of UQ’s offerings. The second gap is in our professional development programs and, indeed, in our recognition and support for the people who are in charge of such multi-year sequences of study. The major convener or program coordinator is not as well supported, in Australian and overseas professional development programs, as the lecturer in charge of a single course (or unit of study). Nor is her work likely to be taken account of in workload calculations or for the purposes of promotion and career advancement more generally. The Carrick-funded project aims to fill this gap by developing, in consultation with crucial stakeholders, amendments to existing university policies and practices. Background: The Helicobacter pylori reinfection seems to be higher in developing countries, than in developed ones. The aim of the study was to determine the annual recurrence rate of H. pylori, in Brazilian patients with peptic ulcer disease, in a 5-year follow-up. Methods: Patients, with peptic ulcer disease diagnosed by upper digestive endoscopy (UDE) and H. pylori infection verified by histological analysis, rapid urease test, polymerase chain reaction, and urea breath test (UBT), were treated for bacterial eradication. The cure of the infection was verified using the same tests, 3 months after. Clinical evaluation and UBT were performed after sixth and ninth month. After 1 year of follow-up, UBT and UDE were repeated. Up to the fifth year, patients were assessed twice a year and an UBT was performed annually. The patients included and all the reinfected were tested for 15 different genes of the H. pylori. Results: One hundred and forty-seven patients were followed: 19 for 1 year, eight for 2 years, four for 3 years, five for 4 years, and 98 for 5 years, totaling 557 patients/years. Recurrence did not occur in the first year. In the second year, two patients were reinfected; in the third, four patients; in the fourth, three patients; and in the fifth, one patient. The total of reinfected patients was 10. The annual reinfection rate was 1.8%. Conclusion: Brazil presents a low prevalence of H. pylori reinfection, similar to the developed countries.

Objective: To document outcome and to investigate patterns of physical and psychosocial recovery in the first year following severe traumatic brain injury (TBI) in an Australian patient sample. Design: A longitudinal prospective study of a cohort of patients, with data collection at 3, 6, 9, and 12 months post injury. Setting: A head injury rehabilitation unit in a large metropolitan public hospital. Patients: A sample of 55 patients selected from 120 consecutive admissions with severe TBI. Patients who were more than 3 months post injury on admission, who remained confused, or who had severe communication deficits or a previous neurologic disorder were excluded. Interventions: All subjects participated in a multidisciplinary inpatient rehabilitation program, followed by varied participation in outpatient rehabilitation and community-based sen ices. Main Outcome Measures: The Sickness impact Profile (SIP) provided physical, psychosocial, and total dysfunction scores at each follow-up. Outcome at 1 year was measured by the Disability Rating Scale. Results: Multivariate analysis of variance indicated that the linear trend of recovery over time was less for psychosocial dysfunction than for physical dysfunction (F(1,51) = 5.87, P < .02). One rear post injury, 22% of subjects had returned to their previous level of employability, and 42% were able to live independently. Conclusions: Recovery from TBI in this Australian sample followed a pattern similar to that observed in other countries, with psychosocial dysfunction being more persistent. Self-report measures such as the SIP in TBI research are limited by problems of diminished self-awareness.

Background Recent physiological knowledge allows the design of bariatric procedures that aim at neuroendocrine changes instead of at restriction and malabsorption. Digestive adaptation is a surgical technique for obesity based in this rationale. Methods The technique includes a sleeve gastrectomy, an omentectomy and a jejunectomy that leaves initial jejunum and small bowel totaling at least 3 m (still within normal variation of adult human bowel length). Fasting ghrelin and resistin and fasting and postprandial GLP-1 and PYY were measured pre- and postoperatively. Results Patients: 228 patients with initial body mass index (BMI) varying from 35 to 51 kg/m(2); follow-up: I to 5 years; average EBMIL% was 79.7% in the first year; 77.7% in the second year; 71.6% in the third year; 68.9% in the fourth year. Patients present early satiety and major improvement in presurgical comorbidities, especially diabetes. Fasting ghrelin and resistin were significantly reduced (P<0.05); GLP-1 and PYY response to food ingestion was enhanced (P<0.05). Surgical complications (4.4%) were resolved without sequela and without mortality. There was neither diarrhea nor detected malabsorption. Conclusions Based on physiological and supported by evolutionary data, this procedure creates a proportionally reduced gastrointestinal (GI) tract that amplifies postprandial neuroendocrine responses. It leaves basic GI functions unharmed. It reduces production of ghrelin and resistin and takes more nutrients to be absorbed distally enhancing GLP-1 and PYY secretion. Diabetes was improved significantly without duodenal exclusion. The patients do not present symptoms nor need nutritional support or drug medication because of the procedure, which is safe to perform.

Este trabalho tem como objeto de estudo os impressos do Projeto Trilhas, material pedagógico produzido em parceria entre o Instituto Natura, a Comunidade Educativa CEDAC e o Ministério da Educação. Trata-se de uma análise documental com as quais se procurou envolver as noções de enunciado, texto, gênero e suporte que possibilitaram fundamentar a proposta metodológica, pautada pelo diálogo, que teve como escopo problematizar como esse conjunto de materiais pode contribuir no processo do ensino e da aprendizagem das crianças matriculadas nas turmas do primeiro ano do Ensino Fundamental, com foco na análise das concepções de alfabetização, leitura e escrita, engendradas nos materiais. Para isso, o referencial teórico que balizou as reflexões se fundamentou nas contribuições da perspectiva bakhtiniana de linguagem e lançou ancoragens no conceito de alfabetização proposto criticamente por Gontijo (2008). As análises se constituíram como uma arena, isto é, um palco de alteridade. Logo, buscaram compreender como o conceito e as concepções se materializaram nas atividades produzidas pelos sujeitos-autores e problematizaram como os impressos do Projeto Trilhas podem contribuir para a melhoria do ensino e da aprendizagem das crianças matriculadas no primeiro ano do Ensino Fundamental. Com as análises, sustenta-se que o conceito que solidifica a constituição dos impressos deste projeto se aproxima das contribuições de Ferreiro e Teberosky (1999), isto é, a alfabetização é o processo pelo qual as crianças assimilam o código escrito e compreende os usos que são dados a ele nas culturas do escrito. A leitura se configurou como decodificação dos signos linguísticos e compreensão de significados, e a escrita como codificação.

RESUMO: A presente investigação teve como objectivo, conhecer as experiências emocionais que caracterizam os estudantes dos PALOP ao saberem que vão entrar no Ensino Superior em Portugal e durante o primeiro semestre; verificar se as emoções sentidas durante a frequência do primeiro semestre estão relacionadas com a sua integração social e académica. Participaram 100 alunos oriundos dos PALOP no primeiro ano do ensino superior português, de diferentes estabelecimentos de Ensino Superior, com idades entre os 20 e os 50 anos. Nesta investigação foram utilizados como instrumentos de medidas, as seguintes medidas: um Questionário sócio-demográfico, um questionário de experiências emocionais (Russell & Carroll); Escala de Emoções Mistas (AES, Carrera & Oceja, 2007); Teste de Positividade (Fredrickson, 2009); Escala de Integração Social e Académica (ISA, Silva; Abrantes, & Duarte (2009) e a Escala de Integração Social no Ensino Superior (EISES, Diniz & Almeida 2005). Concluímos assim que as emoções mistas mais sentidas são alegria e medo, assim como alegria e tristeza. Foi ainda possível concluir que o processo de integração está relacionado com um maior rácio da positividade na frequência do primeiro semestre. ABSTRACT: The objective of the present investigation is to know the emotional experiences that characterize the students of the African Countries with Portuguese as the Official Language while knowing that they are going to enter in the Superior Teaching in Portugal and during the first semester; to check if the emotions felt during the frequency of the first semester are connected with his social and academic integration. The participants were 100 pupils originating from the African Countries with Portuguese as the Official Language in the first year of the superior Portuguese teaching, of different establishments of Superior Teaching, with ages between the 20 and 50 years. In this investigation we used the following measures: a Questionnaire demographic-partner, a questionnaire of emotional experiences (Russell & Carroll); Analogical Emotional Scale (AES, Carrera & Oceja, 2007); Positivity Self-Test (Fredrickson, 2009); Scale of Social and Academic Integration (ISA, Silva, Abrantes, & Duarte, 2009) and the Scale of Social Integration in the Superior Teaching (EISES, Diniz & Almeida 2005). We concluded that the most felt mixed emotions are joy and fear, as well as joy and sadness. It was also possible to conclude that the process of integration is connected with a bigger ratio of the positivity during the frequency of the first semester.

Backgroung - Bariatric surgery is indicated as the most effective treatment for morbid obesity; the Roux-en-Y gastric bypass (RYGB) is considered the procedure of choice. However, nutritional deficiency may occur in the postoperative period as a result of reduced gastric capacity and change in nutrients absorption in the gastrointestinal tract. The prescription of vitamin and mineral supplementation is a common practice after RYGB; however, it may not be sufficient to prevent micronutrient deficiencies. The aim of this study was to quantify the micronutrient intake in patients undergoing RYGB and verify if the intake of supplementation would be enough to prevent nutritional deficiencies. Methods - The study was conducted on 60 patients submitted to RYGB. Anthropometric, analytical, and nutritional intake data were assessed preoperatively and 1 and 2 years postoperatively. The dietary intake was assessed using 24-h food recall; the values of micronutrients evaluated (vitamin B12, folic acid, iron, and calcium) were compared to the dietary reference intakes (DRI). Results - There were significant differences (p < 0.05) between excess weight loss at the first and second year (69.9 ± 15.3 vs 9.6 ± 62.9 %). In the first and second year after surgery, 93.3 and 94.1 % of the patients, respectively, took the supplements as prescribed. Micronutrient deficiencies were detected in the three evaluation periods. At the first year, there was a significant reduction (p < 0.05) of B12, folic acid, and iron intake. Conclusions - Despite taking vitamin and mineral supplementation, micronutrient deficiencies are common after RYGB. In the second year after surgery, micronutrient intake remains below the DRI.

Nous présentons le suivi d’une cohorte d’élèves, de la grande section (troisième année de maternelle en France -5 ans) à la fin du CE1 (deuxième année de primaire –7/8 ans). La population est constituée d’un groupe contrôle et d’un groupe expérimental pratiquant des séances hebdomadaires d’écriture approchée, observées et filmées à différentes périodes de l’année (octobre, mars et juin). Dans notre contribution, nous analysons les séances filmées en fin de CP (première primaire), dans une perspective essentiellement comparatiste. L’objectif est de décrire la démarche didactique mise en œuvre par les enseignantes lors de séances d’enseignement de l’écriture. L'analyse des productions d'écrit et des verbalisations des élèves nous permet également d'évaluer ces démarches d’enseignement et de caractériser celle qui semble le plus propice au développement de compétences métalinguistiques des apprentis scripteurs

Em acordo com o Dec. Lei nº 3/2008 de 7 de janeiro e para alunos com necessidades educativas especiais a medida currículo específico individual é considerada a mais restritiva de todas as medidas educativas. A área disciplinar da matemática, pela sua aplicabilidade no quotidiano, assume primordial importância no Programa Educativo Individual (PEI) destes alunos. Assim, o presente estudo visa analisar a área curricular de matemática dos PEI de alunos a frequentar o 2º e 3º ciclo de ensino básico ao abrigo da medida educativa currículo específico individual (CEI); visa igualmente constatar que seleção de conteúdos programáticos são percecionados como prioritários para a equipa que elabora o PEI. Em suma, o estudo visa compreender alguns aspetos que, de forma direta ou indireta, interagem com a elaboração do currículo. Tem, ainda, um caráter exploratório e está apoiado numa metodologia de natureza qualitativa e quantitativa (numa dimensão descritiva) que procede à análise documental de excertos (área curricular de matemática) dos Programas Educativos Individuais (PEI). Para o efeito foram analisados 50 PEI que identificaram regularidades relativas aos diferentes conteúdos e à extensão de cada conteúdo. Os resultados evidenciam uma escolha maioritária de conteúdos matemáticos associados ao programa do 1º ano do 1º ciclo do ensino básico e, simultaneamente, de descritores associados aos números e operações. Os resultados permitem extrapolar acerca da interação entre níveis de programação e de funcionalidade dos alunos em CEI e requerem mais estudos que sustentem aquelas evidências e clarifiquem variáveis que interagem na elaboração do currículo.

Background: Differently from HIV-1, HIV-2 disease progression usually takes decades without antiretroviral therapy and the majority of HIV-2 infected individuals survive as elite controllers with normal CD4+ T cell counts and low or undetectable plasma viral load. Neutralizing antibodies (Nabs) are thought to play a central role in HIV-2 evolution and pathogenesis. However, the dynamic of the Nab response and resulting HIV-2 escape during acute infection and their impact in HIV-2 evolution and disease progression remain largely unknown. Our objective was to characterize the Nab response and the molecular and phenotypic evolution of HIV-2 in association with Nab escape in the first years of infection in two children infected at birth. Results: CD4+ T cells decreased from about 50% to below 30% in both children in the first five years of infection and the infecting R5 viruses were replaced by X4 viruses within the same period. With antiretroviral therapy, viral load in child 1 decreased to undetectable levels and CD4+ T cells recovered to normal levels, which have been sustained at least until the age of 12. In contrast, viral load increased in child 2 and she progressed to AIDS and death at age 9. Beginning in the first year of life, child 1 raised high titers of antibodies that neutralized primary R5 isolates more effectively than X4 isolates, both autologous and heterologous. Child 2 raised a weak X4-specific Nab response that decreased sharply as disease progressed. Rate of evolution, nucleotide and amino acid diversity, and positive selection, were significantly higher in the envelope of child 1 compared to child 2. Rates of R5-to-X4 tropism switch, of V1 and V3 sequence diversification, and of convergence of V3 to a β-hairpin structure were related with rate of escape from the neutralizing antibodies. Conclusion: Our data suggests that the molecular and phenotypic evolution of the human immunodeficiency virus type 2 envelope are related with the dynamics of the neutralizing antibody response providing further support for a model in which Nabs play an important role in HIV-2 pathogenesis.

OBJECTIVE: The aim of this study is to evaluate the survival rate in a cohort of Parkinson's disease patients with and without depression. METHODS: A total of 53 Parkinson's disease subjects were followed up from 2003-2008 and 21 were diagnosed as depressed. Mean time of follow up was 3.8 (SD 95% = 1.5) years for all the sample and there was no significant difference in mean time of follow up between depressed and nondepressed Parkinson's disease patients. Survival curves rates were fitted using the Kaplan-Meier method. In order to compare survival probabilities according to the selected covariables the Log-Rank test was used. Multivariate analysis with Cox regression was performed aiming at estimating the effect of predictive covariables on the survival. RESULTS: The cumulative global survival of this sample was 83% with nine deaths at the end of the study - five in the depressed and four in the nondepressed group, and 55.6% died in the first year of observation, and none died at the fourth and fifth year of follow up. CONCLUSION: Our finding point toward incremental death risk in depressed Parkinson's disease patients.

OBJECTIVE - This analysis was undertaken to determine the composite incidence of cumulative adverse events (death, reinfarction, disabling stroke, and target vessel revascularization) at the end of the first year after acute myocardial infarction, in diabetic patients who underwent coronary stenting or primary coronary balloon angioplasty. METHODS - From the STENT PAMI trial, we analyzed the 6-month angiographic and 1-year clinical outcomes of 135 diabetic (112, noninsulin dependent) patients who underwent the randomization process of the trial and compared them with 758 nondiabetic patients. RESULTS - Coronary stenting did not significantly reduce the primary composite clinical end point when compared with PTCA (20 vs. 30%, p=0.2). A significant benefit from stenting was observed in patients with noninsulin dependent diabetes, with a trend toward a lesser need for new revascularization procedures (10 vs. 21%, p<.001), with a significant reduction in the primary composite clinical end point at 1 year (12 vs. 28%, p=. 04). At 6 months, the restenosis rate were significantly reduced only in nondiabetic patients (18 vs. 33%, p<. 001). Diabetic patients had the same restenosis rate (38%) either with stenting or balloon PTCA. CONCLUSIONS - Coronary Stenting in diabetics noninsulin dependent offered a significant reduction in the composite incidence of major clinical adverse events compared with balloon PTCA.
