RESUMO: A atividade física (AF) surge como uma estratégia constante no combate aos efeitos nefastos do envelhecimento e nesse sentido, surgem recomendações, mundialmente aceites, de que os idosos deverão realizar pelo menos 150 minutos de atividade moderada por semana, aumentar as atividades ligeiras e reduzir os comportamentos sedentários (ACSM, 2010). Contudo não sabemos se os idosos cumprem ou não estas recomendações e ao que corresponde objetivamente aumentar os níveis de atividade ligeira e diminuir os comportamentos sedentários: que proporção ocupam ou deverão ocupar na vida dos idosos? Os benefícios da AF são vastos e amplamente aceites, nomeadamente ao nível da melhoria da autoperceção de saúde (ApS) e redução da dor, no entanto, desconhece-se a relação existente entre o nível de AF e estas variáveis e o estudo desta relação revela-se de extrema importância tendo em conta o seu impacto na funcionalidade, bem-estar e qualidade de vida do idoso. Objetivo: Caracterizar os níveis de AF e os comportamentos sedentários de indivíduos com mais de 75 anos e analisar a sua relação com a auto-perceção de saúde e a dor. Metodologia: Trata-se de um estudo descritivo de correlação, com uma amostra constituída por 66 participantes, com média de idade de 80.1 (±3.83) anos. As variáveis em estudo foram o nível de AF, os comportamentos sedentários, a ApS e a dor. Foi aplicado um protocolo de avaliação, constituído por um questionário de caracterização sociodemográfica e do nível de AF, o Yale Física Activity Survey (YPAS), o MOS Short-Form Health Survey (SF-12) e a Escala Numérica de Dor. Resultados: Os resultados revelaram que os participantes despendiam em média 50% do seu tempo semanal em comportamentos sedentários; 38.5% em atividades ligeiras e 480.23 minutos, ou seja, 11%, em atividades moderadas. Verificou-se uma relação positiva e estatisticamente significativa entre a ApS geral e a quantidade de AF moderada (Rs=0.490,p=0.000), o gasto total energético semanal (Rs=0.231, p=0.031), a pontuação de caminhada (Rs=0.422, p=0.000) e a pontuação de movimento (Rs=0.313, p=0.005); uma associação negativa, estatisticamente significativa, entre a dor e a pontuação de posição de pé (Rs=-0.305,p=0.006); e entre a pontuação de posição de sentado do YPAS e a ApS geral (Rs=-0.342,p=0.003). Conclusões: Os resultados sugerem que os participantes ocupavam metade da sua semana em comportamentos sedentários, contudo em termos da quantidade de AF moderada vão de encontro aos mínimos propostos pelas guidelines internacionais para se obter benefícios de saúde. No entanto, a distribuição, quer em termos de intensidade como de frequência, destas atividades ao longo da semana poderá não ser a mais adequada. O presente estudo aponta ainda para a existência de uma relação positiva entre o nível de AF e a ApS, ou seja, na nossa amostra um maior nível de AF estava associado a uma melhor ApS; uma relação negativa entre o nível de AF e a dor, um maior nível de AF estava também associado a uma menor intensidade de dor; e uma relação negativa entre os comportamentos sedentários e a ApS, ou seja, na amostra de utentes, com mais de 75 anos, em estudo, um score mais elevado de comportamentos sedentários estava associado a uma pior ApS.---------ABSTRACT: Background: Physical activity (PA) has been widely pointed as an answer to overcome aging’s negative impact. In this sense, recommendations have arise supporting that older adults should perform, at least, 150 minutes of moderate intensity PA per week, increase their light intensity PA and decrease sedentary behaviours (ACSM, 2010). Nevertheless, it is unclear whether older adults reach these recommendations or not and, also, what exactly means to increase light intensity PA and to reduce sedentary behaviours: which proportion they fill or should fill in older adults life? PA’s benefits are extensive and widely accepted, namely improvements in self-related health (SRH) and pain reduction, however, the relation between these variables and PA level and sedentary behaviours is still unknown, and we find it extremely important to clarify the nature of this relation considering its impact on older adults functional level, wellbeing and quality of life. Purpose: Characterize older adults, over 75 years old, PA levels and sedentary behaviours and to investigate its relation to self-rated health and pain. Methods: We conducted a descriptive-correlational study, with a geographic convenience sample of 66 participants with a mean age of 80.1 (±3.83) years. Our study variables were PA level, sedentary behaviours, SRH and pain. We applied an assessment protocol, including a socio-demographic and PA level questionnaire, Yale Physical Activity Survey (YPAS), MOS Short-Form Health Survey (SF-12) and Numeric Pain Scale. Results: Revealed that participants spent an average of 50% of their total weekly time in sedentary behaviours; 38.5% in light intensity PA; and 480.23 minutes per week, meaning 11.04%, in moderate intensity PA. We encountered a positive relation, with statistical significance, between global SRH and moderate intensity PA amount (Rs=0.490, p=0.000), total energy expenditure (Rs=0.231, p=0.031), walking score (Rs=0.422, p=0.000) and movement score (Rs=0.313, p=0.005); a negative association, with statistical significance, between pain and standing score (Rs=-0.305, p=0.006); and between sitting score and global SRH (Rs=-0.342,p=0.003). Conclusions: Our results unveil that the subjects in our sample spent half of their week in sedentary behaviours, nonetheless they met moderate intensity PA recommendations to obtain health benefits. However, activities distribution, regarding both its intensity and frequency, throughout the week might not be the most appropriate. This study points towards the existence of a positive relation between PA level and SRH, meaning that, in our sample, a higher PA level was associated to a better SRH; a negative relation between PA level and pain, i.e. a higher PA level was associated to less pain; and a negative relation between sedentary behaviours and SRH, meaning that a higher sitting score was associated to a worse SRH, in the sample of older adults over 75 years in study.

RESUMO: Objectivo: O objectivo geral do estudo é analisar a relação entre intensidade da actividade física (AF) realizada pela população idosa com 75 anos ou mais, com a respectiva sensação subjectiva de esforço (SSE). Introdução: Em Portugal, e de acordo o Instituto Nacional de Estatística, em 2011 cerca de 19% da população tem mais de 65 anos. Em 2050, as estimativas apontam que esta percentagem seja de 25%, ou seja, um quarto do país. O envelhecimento propicía um declínio físico e funcional, e para o retardar estão recomendados 150 minutos por semana de AF moderada. A forma como tem sido avaliada a AF tem por base um pressuposto de comparação com valores de gasto energético de uma população com uma média de 40 anos. É fundamental a realização de estudos que analisem a associação entre o nível de intensidade de AF com a SSE, para aferir se esta comparação é transponível para esta faixa etária. Metodologia: Trata-se de um estudo observacional analítico transversal, com uma amostra constituída por 39 participantes, com uma mediana 77,0 anos de idade (percentil 25: 76,0; percentil 75: 80,0), sendo a mínima 75 e a máxima 86 anos. As variáveis são a SSE e a intensidade de cada actividade realizada. O protocolo de avaliação foi constituído pelo questionário de caracterização sóciodemográfica, Diário de AF e a Escala de Borg RPE 6-20. Resultados: Existe uma associação negativa significativa entre a idade e o gasto energético total (rs = -0,365, p=0,022) e uma associação negativa, moderada e significativa entre a idade e a SSE reportada média (rs = -0,521, p=0,001) e máxima (rs = -0,465, p=0,003). Observou-se uma associação positiva e significativa entre a SSE máxima diária e o tempo total dedicado a actividades físicas (r=0,367 e p=0,022) e o gasto energético total (r=0,569, p<0,001). Verificou-se igualmente que quanto maior o gasto energético total diário, maior a média da SSE (r=0,411; p=0,009). Existe uma concordância moderada e significativa (Kappa=0,475; p<0,001) entre a sensação reportada pelo participante e a classificação atribuída em cada actividade pelo Compêndio de Actividades Físicas (Ainsworth et al, 1993). No entanto, com uma tendência para a subestimação dos MET’s atribuídos a cada actividade Conclusão: Os resultados do estudo sugerem que existe uma associação positiva e significativa entre intensidade da AF realizada e a SSE, aquando da realização da mesma actividade. Apesar desta associação ser moderada, este estudo sugere que a classificação que o Compêndio utiliza não é directamente aplicável à população idosa com 75 anos ou mais.-------------ABSTRACT: Purpose: The overall objective of the study is to analyze the relationship between intensity of physical activity performed by the elderly population with 75 years or more with their subjective feeling of effort. Background: In Portugal, according with data from Instituto Português de Estatística, in 2011 aproximately 19% of the population has more than 65 years old. In 2050, estimates suggest that this percentage will be 25%, which means a quarter of the country. The way in which physical activity has been evaluated is based on a comparing assumption with values of energy expenditure in a population with an average of 40 years. It is critical to conduct studies to examine the association between the level of intensity of physical activity with the subjective feeling of effort, to assess whether this comparison is applied to this age group. Methods: This is a cross sectional observational study with a sample of 39 participants, with a median age of 77.0 years (25th percentile: 76.0; 75th percentile: 80.0), with the minimum age 75 years and maximum age 86 years. The variables are the subjective feeling of physical effort and intensity of each activity. It was applied one protocol evaluation consisting of a questionnaire of sociodemographic characteristics, A journal of physical activities and Borg RPE 6-20 scale. Results: There is a significant negative association between age and total energy expenditure (rs = -0.365, p = 0.022) and a negative, moderate and significant association between age and average (rs = -0.521, p = 0.001) and maximum subjective feeling of exertion reported (rs = -0.465, p = 0.003). There was a significant positive association between the subjective feeling of daily maximum effort and total time spent on physical activity (r = 0.367 and p = 0.022) and total energy expenditure (r = 0.569, p <0.001). It was also found that the higher the total daily energy expenditure, the higher the average of the subjective sense of effort (r = 0.411, p = 0.009). There is a moderate and significant correlation (kappa = 0.475, p <0.001) between the sensation reported by the participant and the rank assigned to each activity by Compendium of Physical Activities (Ainsworth et al, 1993). However, there is a tendency to over-estimate the subjective feeling of effort, or underestimation of METs assigned to each activity Conclusion: Study results suggest a positive and significant association between the level of intensity of physical activity and the subjective feeling of exertion, within the same activity. Even though this association is moderated, this study suggests that the Compendium of Physical Activities rating doesn’t suit for individuals with 75 years old or more.

RESUMO - Objectivo: descrever o estado nutricional e o padrão alimentar de grupos da população de Lisboa. Delineamento, locais e participantes: estudo transversal do estado nutricional e do padrão alimentar de homens com 38 anos e mulheres e homens com 50-65 anos que vivem na freguesia do Lumiar, do concelho de Lisboa; estudo transversal dos hábitos alimentares e do estado nutricional de adolescentes com 12 a 19 anos estudantes de escolas secundárias de Lisboa; estudo transversal da excreção de sódio em urina de 24 horas de rapazes de 7 a 8 anos de escolas primárias do Lumiar. Métodos: o estado nutricional foi avaliado com metodologias padronizadas tal como propostas pela OMS. A ingestão alimentar foi medida pelo método das 24 horas anteriores no primeiro estudo e pelo método da história alimentar no segundo estudo. Resultados: nas crianças, a excreção urinária de sódio foi uma das mais altas da Europa. Nos adolescentes, a prevalência de excesso de peso/obesidade foi de 19% nos rapazes e 16% nas raparigas. 16% dos adolescentes não tomam pequeno-almoço, mas mais de 30% comem bolos a meio da manhã. A proporção de adolescentes com uma ingestão diária de certos alimentos é de 44% para guloseimas, 43% para bolos, 29% para refrigerantes e 16% para chocolates e gelados. Por outro lado, a frequência de ingestão diária de fruta e produtos hortícolas é baixa, principalmente nos adolescentes de menor nível sócio-económico. Nos adultos, 57% dos homens com 38 anos e 80% e 74% dos homens e mulheres com 50-65 anos, respectivamente, têm excesso de peso ou obesidade. A hipertensão arterial foi detectada em mais de 20% dos homens jovens e a sua prevalência aumenta com a idade. Foi observado um colesterol sérico superior a 200 mg/dL em perto de metade dos adultos e maior do que 240 mg/dL em 28% dos homens com 38 anos, em 24% dos homens com 50-65 anos e 44% das mulheres do mesmo grupo etário. 20% dos adultos jovens têm colesterol HDL inferior a 35 mg/dL, mais do que um terço são fumadores e a ingestão de álcool representa 13% da sua ingestão calórica total. Conclusões: foram observados estilos de vida não saudáveis, nomeadamente padrões alimentares não equilibrados, uma elevada prevalência de obesidade, hipertensão e dislipidemias, e, por isso, uma percentagem relativamente elevada da população de Lisboa tem um risco elevado de doenças cárdio-vasculares e outras doenças crónicas, bem como de morte prematura.

Taking into account the fact that the sun’s radiation is estimated to be enough to cover 10.000 times the world’s total energy needs (BRAKMANN & ARINGHOFF, 2003), it is difficult to understand how solar photovoltaic systems (PV) are still such a small part of the energy source matrix across the globe. Though there is an ongoing debate as to whether energy consumption leads to economic growth or whether it is the other way around, the two variables appear correlated and it is clear that ensuring the availability of energy to match a country’s growth targets is one of the prime concerns for any government. The topic of centralized vs distributed electricity generation is also approached, especially in what regards the latter fit to developing countries needs, namely the lack of investment capabilities and infrastructure, scattered population, and other factors. Finally, Brazil’s case is reviewed, showing that the current cost of electricity from the grid versus the cost from PV solutions still places an investment of this nature with 9 to 16 years to reach breakeven (from a 25 year panel lifespan), which is too high compared to the required 4 years for most Brazilians. Still, recently passed legislation opened the door, even if unknowingly, to the development of co-owned solar farms, which could reduce the implementation costs by as much as 20% and hence reduce the number of years to breakeven by 3 years.

Debris flow susceptibility mapping at a regional scale has been the subject of various studies. The complexity of the phenomenon and the variability of local controlling factors limit the use of process-based models for a first assessment. GISbased approaches associating an automatic detection of the source areas and a simple assessment of the debris flow spreading may provide a substantial basis for a preliminary susceptibility assessment at the regional scale. The use of a digital elevation model, with a 10 m resolution, for the Canton de Vaud territory (Switzerland), a lithological map and a land use map, has allowed automatic identification of the potential source areas. The spreading estimates are based on basic probabilistic and energy calculations that allow to define the maximal runout distance of a debris flow.

BACKGROUND & AIMS: Nutrition therapy is a cornerstone of burn care from the early resuscitation phase until the end of rehabilitation. While several aspects of nutrition therapy are similar in major burns and other critical care conditions, the patho-physiology of burn injury with its major endocrine, inflammatory, metabolic and immune alterations requires some specific nutritional interventions. The present text developed by the French speaking societies, is updated to provide evidenced-based recommendations for clinical practice. METHODS: A group of burn specialists used the GRADE methodology (Grade of Recommendation, Assessment, Development and Evaluation) to evaluate human burn clinical trials between 1979 and 2011. The resulting recommendations, strong suggestions or suggestions were then rated by the non-burn specialized experts according to their agreement (strong, moderate or weak). RESULTS: Eight major recommendations were made. Strong recommendations were made regarding, 1) early enteral feeding, 2) the elevated protein requirements (1.5-2 g/kg in adults, 3 g/kg in children), 3) the limitation of glucose delivery to a maximum of 55% of energy and 5 mg/kg/h associated with moderate blood glucose (target ≤ 8 mmol/l) control by means of continuous infusion, 4) to associated trace element and vitamin substitution early on, and 5) to use non-nutritional strategies to attenuate hypermetabolism by pharmacological (propranolol, oxandrolone) and physical tools (early surgery and thermo-neutral room) during the first weeks after injury. Suggestion were made in absence of indirect calorimetry, to use of the Toronto equation (Schoffield in children) for energy requirement determination (risk of overfeeding), and to maintain fat administration ≤ 30% of total energy delivery. CONCLUSION: The nutritional therapy in major burns has evidence-based specificities that contribute to improve clinical outcome.

En el presente estudio se han analizado las posibilidades de reducción del impacto ambiental que genera el consumo de energía en el Monasterio budista Sakya Tashi Ling del Parque del Garraf, mediante la mejora de la eficiencia energética de los edificios y la producción de la energía consumida a partir de fuentes de energía renovables. Se ha realizado un inventario exhaustivo de los flujos energéticos de entrada del Monasterio: electricidad y combustibles fósiles, y el análisis de estos datos ha permitido observar que la mayor parte del consumo energético del Monasterio tiene como uso final la iluminación. El consumo total de energía se ha cuantificado en 138 kWh/m2/año y la emisiones de CO2 en 177 kg CO2/m2/año. A partir de estos datos se ha estudiado la posibilidad de reducir el consumo y abastecer la demanda energética del Monasterio a través de fuentes de energía renovable como las placas solares fotovoltaicas o las calderas de biomasa. Para alcanzar este objetivo se han propuesto tres escenarios posibles con costes económicos y resultados muy distintos. A diferencia de los escenarios “Edificio Energía Plus” y “Edificio Energía 0”, el tercer escenario propuesto, que trataba de alcanzar el triple objetivo del Plan 20/20/20 (producir a partir de fuentes renovables el 20% de la energía consumida, aumentar en una 20% la eficiencia energética y reducir en un 20% las emisiones de CO2 derivadas del uso de la energía) ha resultado ser muy viable económicamente: la inversión necesaria se amortizaría en tan sólo 5 años.

We introduce and analyze two new semi-discrete numerical methods for the multi-dimensional Vlasov-Poisson system. The schemes are constructed by combing a discontinuous Galerkin approximation to the Vlasov equation together with a mixed finite element method for the Poisson problem. We show optimal error estimates in the case of smooth compactly supported initial data. We propose a scheme that preserves the total energy of the system.

BACKGROUND: The pattern of substrate utilization with diets containing a high or a low proportion of unavailable and slowly digestible carbohydrates may constitute an important factor in the control, time course, and onset of hunger in humans. OBJECTIVE: We tested the hypothesis that isoenergetic diets differing only in their content of unavailable carbohydrates would result in different time courses of total, endogenous, and exogenous carbohydrate oxidation rates. DESIGN: Two diets with either a high (H diet) or a low (L diet) content of unavailable carbohydrates were fed to 14 healthy subjects studied during two 24-h periods in a metabolic chamber. Substrate utilization was assessed by whole-body indirect calorimetry. In a subgroup of 8 subjects, endogenous and exogenous carbohydrate oxidation were assessed by prelabeling the body glycogen stores with [(13)C]carbohydrate. Subjective feelings of hunger were estimated with use of visual analogue scales. RESULTS: Total energy expenditure and substrate oxidation did not differ significantly between the 2 diets. However, there was a significant effect of diet (P: = 0.03) on the carbohydrate oxidation pattern: the H diet elicited a lower and delayed rise of postprandial carbohydrate oxidation and was associated with lower hunger feelings than was the L diet. The differences in hunger scores between the 2 diets were significantly associated with the differences in the pattern of carbohydrate oxidation among diets (r = -0.67, P: = 0. 006). Exogenous and endogenous carbohydrate oxidation were not significantly influenced by diet. CONCLUSIONS: The pattern of carbohydrate utilization is involved in the modulation of hunger feelings. The greater suppression of hunger after the H diet than after the L diet may be helpful, at least over the short term, in individuals attempting to better control their food intake.

BACKGROUND: Ethanol can account for up to 10 percent of the energy intake of persons who consume moderate amounts of ethanol. Its effect on energy metabolism, however, is not known. METHODS: We studied the effect of ethanol on 24-hour substrate-oxidation rates in eight normal men during two 48-hour sessions in an indirect-calorimetry chamber. In each session, the first 24 hours served as the control period. On the second day of one session, an additional 25 percent of the total energy requirement was added as ethanol (mean [+/- SD], 96 +/- 4 g per day); during the other session, 25 percent of the total energy requirement was replaced by ethanol, which was isocalorically substituted for lipids and carbohydrates. RESULTS: Both the addition of ethanol and the isocaloric substitution of ethanol for other foods reduced 24-hour lipid oxidation. The respective mean (+/- SE) decreases were 49.4 +/- 6.7 and 44.1 +/- 9.3 g per day (i.e., reductions of 36 +/- 3 percent and 31 +/- 7 percent from the oxidation rate during the control day; P less than 0.001 and P less than 0.0025). This effect occurred only during the daytime period (8:30 a.m. to 11:30 p.m.), when ethanol was consumed and metabolized. Neither the addition of ethanol to the diet nor the isocaloric substitution of ethanol for other foods significantly altered the oxidation of carbohydrate or protein. Both regimens including ethanol produced an increase in 24-hour energy expenditure (7 +/- 1 percent with the addition of ethanol, P less than 0.001; 4 +/- 1 percent with the substitution of ethanol for other energy sources, P less than 0.025). CONCLUSIONS: Ethanol, either added to the diet or substituted for other foods, increases 24-hour energy expenditure and decreases lipid oxidation. Habitual consumption of ethanol in excess of energy needs probably favors lipid storage and weight gain.

Previous studies have demonstrated the difference between the basal metabolic rate (BMR) and the sleeping metabolic rate (SMR): however, the difference in the Japanese population has not yet been explored. This study examined the relationship between the BMR and SMR in ninety-four healthy Japanese subjects (37 males and 57 females, 39 +/- 12 y of age and 22.0 +/- 7.4% body fat) in a respiratory chamber. The SMR was significantly lower than the BMR (1416 +/- 245 vs. 1492 +/- 256 kcal/d): however, there was a highly significant correlation between the two (r = 0.867; p < 0.001). The ratio of SMR/BMR largely varied among individuals (0.95 +/-0.08, 8.4% of the coefficient of variation). The ratio was significantly lower in males than in females (0.93 +/- 0.10 vs. 0.97 +/- 0.06, p < 0.05). None of the anthropometric measures (age, weight, body mass index, body surface area or percent body fat) correlated with the ratio. These results showed that SMR was 95%, of BMR on average in a healthy Japanese group. However, when applied over a longer time period (24 h or more), the difference tends to become negligible for most analyses in a group. Although the difference between SMR and BMR will induce a 5% gap of physical activity level defined as the total energy expenditure divided by the BMR or SMR, this factor seems to have little practical importance in epidemiological research.

BACKGROUND & AIMS: n-3 fatty acids are expected to downregulate the inflammatory responses, and hence may decrease insulin resistance. On the other hand, n-3 fatty acid supplementation has been reported to increase glycemia in type 2 diabetes. We therefore assessed the effect of n-3 fatty acids delivered with parenteral nutrition on glucose metabolism in surgical intensive care patients. METHODS: Twenty-four surgical intensive care patients were randomized to receive parenteral nutrition providing 1.25 times their fasting energy expenditure, with 0.25 g of either an n-3 fatty acid enriched-or a soy bean-lipid emulsion. Energy metabolism, glucose production, gluconeogenesis and hepatic de novo lipogenesis were evaluated after 4 days. RESULTS: Total energy expenditure was significantly lower in patients receiving n-3 fatty acids (0.015+/-0.001 vs. 0.019+/-0.001 kcal/kg/min with soy bean lipids (P<0.05)). Glucose oxidation, lipid oxidation, glucose production, gluconeogenesis, hepatic de novo lipogenesis, plasma glucose, insulin and glucagon concentrations did not differ (all P>0.05) in the 2 groups. CONCLUSIONS: n-3 fatty acids were well tolerated in this group of severely ill patients. They decreased total energy expenditure without adverse metabolic effects.
