Valtatien 13 osuus Lappeenrannasta Nuijamaalle kuuluu Euroopan komission päättämään Suomen kattavaan liikenneverkkoon TEN-T. Tieosuus on maan toiseksi tärkein kansainvälisen liikenteen yhteys kuljetuksille ja henkilöliikenteelle. Valtatie 13 kulkee maan poikki länsirannikolta Kokkolasta Lappeenrantaan ja siitä edelleen Nuijamaan rajanylityspaikan kautta Venäjän puolelle Viipuriin. Valtatie 13 palvelee osaltaan myös paikallista liikkumista Nuijamaan kylätaajaman ja Lappeenrannan välillä. Erityisesti Mustolan alueelle ja myös osittain Nuijamaan raja-aseman läheisyyteen suunnitellut ja jo osittain toteutuneet maankäytön kaupalliset palvelut lisäävät voimakkaasti kasvaessaan myös seudullista liikennettä suunnitteluosuudella. Nuijamaan raja-asema on ollut Suomen itärajan toiseksi vilkkain tieliikenteen rajanylityspaikka. Rajanylityspaikan kautta kulki vuonna 2015 noin miljoona ajoneuvoa ja 2,4 miljoonaa matkustajaa. Venäjän ja Suomen valtioiden välisen liikenteen kasvu on ollut voimakkainta Kaakkois-Suomessa ja sille on edelleen perusteltuja kasvuodotuksia, vaikka viime aikoina liikenne on merkittävästi vähentynyt. Myös tavaraliikenteellä on potentiaalia lisääntyä viimeaikaisesta. Suunnittelualueeseen kuuluu valtatien 13 (16,6 km) lisäksi valtatien 6 länsipuolinen valtatien 13 jatke Karjalantie (mt 3821) (1,2 km). Yleissuunnitelmassa on selvitetty valtatien 13 puutteet ja ongelmat, palvelutasotavoitteet, valtatien 13 ja muiden väylien periaateratkaisut tilantarpeineen, suhde ympäröivään maankäyttöön, vaikutukset sekä mahdollisuudet vaiheittain toteuttamiseksi. Päätavoitteena on ollut selvittää valtatien 13 ja muiden väylien kehittämisen periaatteet niin, että palvelutasopuutteet saadaan poistettua ja valtatie 13 vastaa sille asetettuja vaatimuksia liikenteen sujuvuuden ja turvallisuuden kannalta. Osana suunnitelmaa on esitetty toimenpiteet meluhaittojen torjumiseksi ja ympäristövaikutusten lieventämiseksi. Valtatie 13 parannetaan nykyisellä paikallaan korkealuokkaiseksi nelikaistaiseksi valtatieksi tarvittavine tie-, katu- ja liittymäjärjestelyineen. Vastakkaiset ajosuunnat on erotettu toisistaan rakenteellisesti ja kaikki valtatien liittymät ovat eritasoliittymiä.

Esta revisión de la literatura tuvo como objetivo describir las actitudes hacia el VIH/SIDA, el cáncer y la Enfermedad de Alzheimer desde el modelo tripartito. Se revisaron 109 artículos publicados entre 2005 y 2015 en algunas bases de datos especializadas y herramientas de análisis de impacto. También se incluyeron fuentes secundarias ampliándose la búsqueda a los últimos 20 años (1995-2015). Los resultados mostraron que la mayoría de los estudios realizados sobre las actitudes hacia estas tres enfermedades son de tipo cuantitativo y la información se analizó con base en los componentes del modelo tripartito. Algunos aspectos sociodemográficos como el sexo y la edad están asociados con las actitudes hacia las tres enfermedades y predominan las creencias erróneas sobre ellas respecto a sus causas, curso y tratamiento. También predominan actitudes negativas hacia las tres enfermedades y las conductas e intenciones conductuales son diversas hacia cada una de ellas. No se hallaron antecedentes empíricos del estudio de la estructura de las actitudes propuesta por el modelo tripartito hacia las tres enfermedades. La Salud Pública ha liderado la investigación con base en el modelo de conocimientos, actitudes y prácticas propuesto por la OMS.

AIMS: To identify the rates and reasons for plate removal (PR) among patients treated for facial fractures. MATERIALS AND METHODS: A retrospective review of files of 238 patients. RESULTS: Forty-eight patients (20.2%) had plates removed. The reason for removal was objective in 33.3% and subjective in 29.2%. The most common subjective reason was cold sensitivity, and the most common objective reason was wound dehiscence/infection. Women had PR for subjective reasons more often than men (p=0.018). Removal was performed more often for subjective reasons after zygomatico-orbital fractures than after mandibular fractures (p=0.002). Plates inserted in the mandible from an intraoral approach were removed more frequently than extraorally inserted mandibular plates, intraorally inserted maxillary plates, and extraorally inserted plates in other locations (p<0.001). Orbital rim plates had a higher risk of being removed than maxillary or frontal bone plates (p=0.02). CONCLUSIONS: Subjective discomfort is a notable reason for PR among Finnish patients, suggesting that the cold climate has an influence on the need for removal. Patients receiving mandibular osteosynthesis with miniplates from an intraoral approach are at risk of hardware removal because of wound dehiscence/infection and loose/broken hardware, reminding us that more rigid fixation devices should not be forgotten despite the widespread use of miniplates.

Removal of miniplates is a controversial topic in oral and maxillofacial surgery. Originally, miniplates were designed to be removed on completion of bone healing. The introduction of low profile titanium miniplates has led to the routine removal of miniplates becoming comparatively rare in many parts of the world. Few studies have investigated the reasons for non-routine removal of miniplates and the factors that affect osteosynthesis after osteotomy in large numbers of patients. The aim of the present study was to investigate complications related to osteosynthesis after bilateral sagittal split osteotomy (BSSO) in a large number (n=153) of patients. In addition to the rates of removal, emphasis was placed on investigating the reasons and risk factors associated with symptomatic miniplate removal. The rate of plate removal per patient was 18.6%, the corresponding rate per plate being 18.2%. Reasons for plate removal included plate-related complications in 16 patients and subjective discomfort in 13 patients. Half of the plates were removed during the first postoperative year. Smoking was the only significant predictor for plate removal. Patients undergoing orthognathic surgery should be screened with regard to smoking and encouraged and assisted to cease smoking, at least perioperatively.

PURPOSE: To identify the occurrence, types, and severity of associated injuries outside the facial region among patients diagnosed with facial fractures, and to analyze whether there are any factors related to associated injuries. MATERIALS AND METHODS: This was a cross-sectional study of 401 patients diagnosed with facial fractures during the 2-year period from 2003 to 2004. RESULTS: Associated injuries were observed in 101 patients (25.2%). The most common type of injury was a limb injury (13.5%), followed by brain (11.0%), chest (5.5%), spine (2.7%), and abdominal (0.8%) injuries. Multiple associated injuries were observed in 10% and polytrauma in 7.5%. The mortality rate was 0.2%. The occurrence of associated injury correlated significantly with trauma mechanism and fracture type; high-speed accidents and severe facial fractures were significant predictors of associated injury. CONCLUSIONS: Associated injuries are frequent among patients who have sustained facial fractures. The results underscore the importance of multiprofessional collaboration in diagnosis and sequencing of treatment, but also the importance of arranging appropriate clinical rotations for maxillofacial residents in training.

The utopian communities of Finns are linked to world history and the great ideological foundations behind numerous utopian endeavors. In the paper, Finnish utopian communities will be described, compared, and contrasted by their ideological backgrounds and in a global context. In addition, the reasons for the dissolution of these settlements are analyzed. Even though the Finnish utopian communities are not often mentioned together with More's Utopia, or with Fourier, Owen, Cabet, or Oneida, they have an interesting history reaching back to the 1792 “New Jerusalem” plan in Sierra Leone. While the best-known Finnish utopian ventures are Sointula in Canada (1901-1905) and Colonia Finlandesa (1906-1940) in Argentina there were, however, almost twenty similar Finnish ventures around the world based on nationalism, utopian socialism, cooperative movements, “tropic fevers,” and religious ideas.

The 20th Annual Biochemical Engineering Symposium was held at Kansas State University on April 21,1990. The objectives of the symposium were to provide: (i) a forum for informal discussion of biochemical engineering research being conducted at the participating institutions and (ii) an opportunity for students to present and publish their work. Recent Pan-Arctic shrub expansion has been interpreted as a response to a warmer climate. However, herbivores can also influence the abundance of shrubs in arctic ecosystems. We addressed these alternative explanations by following the changes in plant community composition during the last 10 years in permanent plots inside and outside exclosures with different mesh sizes that exclude either only reindeer or all mammalian herbivores including voles and lemmings. The exclosures were replicated at three forest and tundra sites at four different locations along a climatic gradient (oceanic to continental) in northern Fennoscandia. Since the last 10 years have been exceptionally warm, we could study how warming has influenced the vegetation in different grazing treatments. Our results show that the abundance of the dominant shrub, Betula nana, has increased during the last decade, but that the increase was more pronounced when herbivores were excluded. Reindeer have the largest effect on shrubs in tundra, while voles and lemmings have a larger effect in the forest. The positive relationship between annual mean temperature and shrub growth in the absence of herbivores and the lack of relationships in grazed controls is another indication that shrub abundance is controlled by an interaction between herbivores and climate. In addition to their effects on taller shrubs (> 0.3 m), reindeer reduced the abundance of lichens, whereas microtine rodents reduced the abundance of dwarf shrubs (< 0.3 m) and mosses. In contrast to short-term responses, competitive interactions between dwarf shrubs and lichens were evident in the long term. These results show that herbivores have to be considered in order to understand how a changing climate will influence tundra ecosystems.
