The risk-to-benefit ratio for the use of low dose of aspirin in primary cardiovascular (CV) prevention in patients with diabetes mellitus remains to be clarified. We assessed the effect of aspirin on risk of CV events in type 2 diabetic patients with nephropathy, in order to verify the usefulness of Guidelines in clinical practice. We carried out a prospective multicentric study in 564 patients with type 2 diabetic nephropathy free of CV disease attending outpatient diabetes clinics. A total of 242 patients received antiplatelet treatment with aspirin 100 mg/day (group A), and 322 were not treated with antiplatelet drugs (group B). Primary end point was the occurrence of total major adverse cardio-vascular events (MACE). Secondary end points were the relative occurrence of fatal MACE. The average follow-up was 8 years. Total MACE occurred in 49 patients from group A and in 52 patients from group B. Fatal MACE occurred in 22 patients from group A and in 20 from group B; nonfatal MACE occurred in 27 patients from group A and in 32 patients from group B. Kaplan-Meier analysis did not show a statistically significant difference of cumulative MACE between the two groups. A not statistically significant difference in the incidence of both fatal (p = 0.225) and nonfatal CV events (p = 0.573) between the two groups was observed. These results were confirmed after adjustment for confounders (HR for MACE 1.11, 95 % CI 0.91-1.35). These findings suggest that low dose of aspirin is ineffective in primary prevention for patients with nephropathy. © 2014 Springer-Verlag Italia.

We study the phase diagram of the double exchange model, with antiferromagnetic interactions, in a cubic lattice both at zero and finite temperature. There is a rich variety of magnetic phases, combined with regions where phase separation takes place. We identify phases, intrinsic to the cubic lattice, which are stable for realistic values of the interactions and dopings. Some of these phases break chiral symmetry, leading to unusual features.

Antonio Salieri’s La calamita de’ cuori (1774) warrants musicological attention for what it can tell us about Salieri’s compositional craft and what it reveals about the development of form in Viennese Italian-language comic opera of the mid- and late-eighteenth century. In Part I of this dissertation, I explore the performance history of La calamita, present the first plot synopsis and English translation of the libretto, and describe the variants between Carlo Goldoni’s 1752 libretto and the revised version created for Salieri’s opera. I have collated Salieri’s holograph score, Österreichische Nationalbibliothek, Vienna, Mus. Hs. 16.508, with four copies having different relationships to it, and I propose a stemma that represents the relationships between these five sources. The analyses in Part II contribute to our understanding of formal practices in eighteenth-century drammi giocosi. My study of Salieri’s La calamita reveals his reliance on a clearly defined binary structure, referred to in this dissertation as “operatic binary form,” in almost half of the arias, ensembles, and instrumental movements of this opera. Salieri’s consistent use of operatic binary form led me to explore its use in drammi giocosi by other prominent composers of this time, including Baldassare Galuppi’s La calamita de’ cuori (1752), Wolfgang Amadeus Mozart’s Il dissoluto punito, ossia Il Don Giovanni (1787), and selected arias by Pasquale Anfossi, Florian Leopold Gassmann, Giuseppe Gazzaniga, Franz Joseph Haydn, Giovanni Paisiello, and Niccolò Piccinni dating from 1760 to 1774. This study showed that Salieri and his peers adhered to a recognizable tonal plan and set of design elements in their operatic binary forms, and that their arias fall into three distinct categories defined by the tonality at the beginning of the second half of the binary structure. The analysis presented here adds to our present understanding of operatic form in mid- and late-century drammi giocosi and shows that in La calamita de’ cuori, Salieri was following the normative formal procedures of his time.

Ler proporciona ao indivíduo novas descobertas. Com a leitura o indivíduo passa a adquirir práticas e conhecimentos, modificando a sua percepção do mundo. A leitura é uma porta que abre passagem para diversos universos diferentes. Além da informação inicial a visão pessoal do leitor irá determinar novos pensamentos, irá criar diferentes interpretações, recriará ideias, ela dá significado à vida e ao papel de cada um diante a sociedade em que está inserido. A capacidade de ler do aluno, quando ele é ainda criança, está veiculada com a biblioteca e o bibliotecário escolar, fortalecendo o ensino do professor em sala de aula oferecendo ao estudante novas informações de forma dinâmica e criativa. O bibliotecário escolar tem um papel fundamental na formação intelectual do aluno. Para que se tenham resultados satisfatórios, quando se trata da formação de leitores na escola, é preciso que se tenha uma biblioteca escolar bem estruturada e um profissional bibliotecário capacitado. A biblioteca precisa sempre esta com o seu acervo atualizado para oferecer condições de trabalho para o profissional bibliotecário, fazendo com que ele cative e estimule os alunos a irem mais vezes na biblioteca, pois lá é o espaço de aquisição dos conhecimentos. O presente artigo apresentou a relação e o hábito de leitura desses estudantes do 1° e 3° ano do ensino médio da escola pública estadual CPDAC.

The Hybrid Monte Carlo algorithm is adapted to the simulation of a system of classical degrees of freedom coupled to non self-interacting lattices fermions. The diagonalization of the Hamiltonian matrix is avoided by introducing a path-integral formulation of the problem, in d + 1 Euclidean space–time. A perfect action formulation allows to work on the continuum Euclidean time, without need for a Trotter–Suzuki extrapolation. To demonstrate the feasibility of the method we study the Double Exchange Model in three dimensions. The complexity of the algorithm grows only as the system volume, allowing to simulate in lattices as large as 163 on a personal computer. We conclude that the second order paramagnetic–ferromagnetic phase transition of Double Exchange Materials close to half-filling belongs to the Universality Class of the three-dimensional classical Heisenberg model.

A história da enfermagem em Portugal, necessita de estudos de aprofundamento, rigorosamente alicerçados em fontes. Em particular os que podem contribuir para a compreensão do processo de invisibilidade/visibilidade da enfermagem portuguesa. Objetivo: Interpretar as fontes históricas disponíveis nos diários das sessões das câmaras de representantes de Portugal, no período 1821-1910. Métodos: Utilizando-se descritores sensíveis à temática enfermagem, foi possível acessar, identificar a sua localização e organizar os diários das sessões disponíveis em texto integral, no endereço digital da Assembleia da República Portuguesa. Resultados: Encontrou-se 1317 páginas de 903 diários de sessões correspondentes a seis câmaras que funcionaram nesse período. Organizou-se cronologicamente a informação e procedeu-se à caracterização das tendências,lacunas e aumento de referência à enfermagem- enfermeiro nestes documentos. Conclusão: É nítida a referência aos descritores enfermeiro, enfermagem e enfermaria em determinados períodos políticos de Portugal, com destaques para momentos de maior investimento nas colônias, exigindo outros estudos.

Nel cuore della Sicilia, a mille metri d’altezza, svetta, la città di Enna. Dalla storia millenaria, questa roccaforte naturale, ha dato protezione alle differenti dominazioni che si sono susseguite. In questa cittadina, dalla delicata armonia, si è attivato un processo di modernizzazione iniziato con il regime fascista, quando divenne da comune a provincia. Uno sviluppo controllato, anche se con relazioni urbane non proporzionate alla città, a completamento dell’acrocoro del monte. Con il dopoguerra, questa questa crescita futto della speculazione edilizia, che ha portato alla costruzione di grandi e alti edifici residenziali, andando a sgretolare la delicata bellezza della città, che piano piano è stata soppiantata da quella contemporanea. Negli anni ‘70, per un adeguamento funzionale della viabilità, è stato costruito un viadotto, nello storico accesso alla città: questo ha generato una ferita in essa, negando alla città tutto lo spazio al di sotto. La sfida progettuale è stata quindi, da una parte, quella di ridare un volto, un’aspetto urbano a questa parte di città che adesso vuol sembrare uno svincolo autostradale, dall’altra, quella di riappropriarsi dello spazio pubblico. L’intervento trasforma l’infrastruttura, in un’architettura generata dalla città e dalle sue giaciture, frammentando e delimitando lo spazio sotto il ponte, che si configura come una sequenza di spazi concatenati da un percorso. Osservando il viadotto come una struttura incompleta fatta di solo scheletro di pilastri e copertura, il progetto ne costituisce il tamponamento composto dai servizi: l’intervento rafforza e aumenta le funzioni di collegamento pedonale, connettendo le parti di città che in quest’area si trovano a quote differenti, portando il visitatore verso il centro storico.

Monte Carlo track structures (MCTS) simulations have been recognized as useful tools for radiobiological modeling. However, the authors noticed several issues regarding the consistency of reported data. Therefore, in this work, they analyze the impact of various user defined parameters on simulated direct DNA damage yields. In addition, they draw attention to discrepancies in published literature in DNA strand break (SB) yields and selected methodologies. The MCTS code Geant4-DNA was used to compare radial dose profiles in a nanometer-scale region of interest (ROI) for photon sources of varying sizes and energies. Then, electron tracks of 0.28 keV-220 keV were superimposed on a geometric DNA model composed of 2.7 × 10(6) nucleosomes, and SBs were simulated according to four definitions based on energy deposits or energy transfers in DNA strand targets compared to a threshold energy ETH. The SB frequencies and complexities in nucleosomes as a function of incident electron energies were obtained. SBs were classified into higher order clusters such as single and double strand breaks (SSBs and DSBs) based on inter-SB distances and on the number of affected strands. Comparisons of different nonuniform dose distributions lacking charged particle equilibrium may lead to erroneous conclusions regarding the effect of energy on relative biological effectiveness. The energy transfer-based SB definitions give similar SB yields as the one based on energy deposit when ETH ≈ 10.79 eV, but deviate significantly for higher ETH values. Between 30 and 40 nucleosomes/Gy show at least one SB in the ROI. The number of nucleosomes that present a complex damage pattern of more than 2 SBs and the degree of complexity of the damage in these nucleosomes diminish as the incident electron energy increases. DNA damage classification into SSB and DSB is highly dependent on the definitions of these higher order structures and their implementations. The authors' show that, for the four studied models, different yields are expected by up to 54% for SSBs and by up to 32% for DSBs, as a function of the incident electrons energy and of the models being compared. MCTS simulations allow to compare direct DNA damage types and complexities induced by ionizing radiation. However, simulation results depend to a large degree on user-defined parameters, definitions, and algorithms such as: DNA model, dose distribution, SB definition, and the DNA damage clustering algorithm. These interdependencies should be well controlled during the simulations and explicitly reported when comparing results to experiments or calculations.

In this work, the energy response functions of a CdTe detector were obtained by Monte Carlo (MC) simulation in the energy range from 5 to 160keV, using the PENELOPE code. In the response calculations the carrier transport features and the detector resolution were included. The computed energy response function was validated through comparison with experimental results obtained with (241)Am and (152)Eu sources. In order to investigate the influence of the correction by the detector response at diagnostic energy range, x-ray spectra were measured using a CdTe detector (model XR-100T, Amptek), and then corrected by the energy response of the detector using the stripping procedure. Results showed that the CdTe exhibits good energy response at low energies (below 40keV), showing only small distortions on the measured spectra. For energies below about 80keV, the contribution of the escape of Cd- and Te-K x-rays produce significant distortions on the measured x-ray spectra. For higher energies, the most important correction is the detector efficiency and the carrier trapping effects. The results showed that, after correction by the energy response, the measured spectra are in good agreement with those provided by a theoretical model of the literature. Finally, our results showed that the detailed knowledge of the response function and a proper correction procedure are fundamental for achieving more accurate spectra from which quality parameters (i.e., half-value layer and homogeneity coefficient) can be determined.

The n→π* absorption transition of formaldehyde in water is analyzed using combined and sequential classical Monte Carlo (MC) simulations and quantum mechanics (QM) calculations. MC simulations generate the liquid solute-solvent structures for subsequent QM calculations. Using time-dependent density functional theory in a localized set of gaussian basis functions (TD-DFT/6-311++G(d,p)) calculations are made on statistically relevant configurations to obtain the average solvatochromic shift. All results presented here use the electrostatic embedding of the solvent. The statistically converged average result obtained of 2300 cm-1 is compared to previous theoretical results available. Analysis is made of the effective dipole moment of the hydrogen-bonded shell and how it could be held responsible for the polarization of the solvent molecules in the outer solvation shells.
