Background: TIDratio indirectly reflects myocardial ischemia and is correlated with cardiacprognosis. We aimed at comparing the influence of three different softwarepackages for the assessment of TID using Rb-82 cardiac PET/CT. Methods: Intotal, data of 30 patients were used based on normal myocardial perfusion(SSS<3 and SRS<3) and stress myocardial blood flow 2mL/min/g)assessed by Rb-82 cardiac PET/CT. After reconstruction using 2D OSEM (2Iterations, 28 subsets), 3-D filtering (Butterworth, order=10, ωc=0.5), data were automatically processed, and then manually processed fordefining identical basal and apical limits on both stress and rest images.TIDratio were determined with Myometrix®, ECToolbox® and QGS®software packages. Comparisons used ANOVA, Student t-tests and Lin concordancetest (ρc). Results: All of the 90 processings were successfullyperformed. TID ratio were not statistically different between software packageswhen data were processed automatically (P=0.2) or manually (P=0.17). There was a slight, butsignificant relative overestimation of TID with automatic processing incomparison to manual processing using ECToolbox® (1.07 ± 0.13 vs 1.0± 0.13, P=0.001)and Myometrix® (1.07 ± 0.15 vs 1.01 ± 0.11, P=0.003) but not using QGS®(1.02 ±0.12 vs 1.05 ± 0.11, P=0.16). The best concordance was achieved between ECToolbox®and Myometrix® manual (ρc=0.67) processing.Conclusion: Using automatic or manual mode TID estimation was not significantlyinfluenced by software type. Using Myometrix® or ECToolbox®TID was significantly different between automatic and manual processing, butnot using QGS®. Software package should be account for when definingTID normal reference limits, as well as when used in multicenter studies. QGS®software seemed to be the most operator-independent software package, whileECToolbox® and Myometrix® produced the closest results.

BACKGROUND: No prior studies have identified which patients with deep vein thrombosis in the lower limbs are at a low risk for adverse events within the first week of therapy. METHODS: We used data from the Registro Informatizado de la Enfermedad TromboEmbólica (RIETE) to identify patients at low risk for the composite outcome of pulmonary embolism, major bleeding, or death within the first week. We built a prognostic score and compared it with the decision to treat patients at home. RESULTS: As of December 2013, 15,280 outpatients with deep vein thrombosis had been enrolled. Overall, 5164 patients (34%) were treated at home. Of these, 12 (0.23%) had pulmonary embolism, 8 (0.15%) bled, and 4 (0.08%) died. On multivariable analysis, chronic heart failure, recent immobility, recent bleeding, cancer, renal insufficiency, and abnormal platelet count independently predicted the risk for the composite outcome. Among 11,430 patients (75%) considered to be at low risk, 15 (0.13%) suffered pulmonary embolism, 22 (0.19%) bled, and 8 (0.07%) died. The C-statistic was 0.61 (95% confidence interval [CI], 0.57-0.65) for the decision to treat patients at home and 0.76 (95% CI, 0.72-0.79) for the score (P = .003). Net reclassification improvement was 41% (P < .001). Integrated discrimination improvement was 0.034 for the score and 0.015 for the clinical decision (P < .001). CONCLUSIONS: Using 6 easily available variables, we identified outpatients with deep vein thrombosis at low risk for adverse events within the first week. These data may help to safely treat more patients at home. This score, however, should be validated.

PURPOSE: To compare clinical benefit response (CBR) and quality of life (QOL) in patients receiving gemcitabine (Gem) plus capecitabine (Cap) versus single-agent Gem for advanced/metastatic pancreatic cancer. PATIENTS AND METHODS: Patients were randomly assigned to receive GemCap (oral Cap 650 mg/m(2) twice daily on days 1 through 14 plus Gem 1,000 mg/m(2) in a 30-minute infusion on days 1 and 8 every 3 weeks) or Gem (1,000 mg/m(2) in a 30-minute infusion weekly for 7 weeks, followed by a 1-week break, and then weekly for 3 weeks every 4 weeks) for 24 weeks or until progression. CBR criteria and QOL indicators were assessed over this period. CBR was defined as improvement from baseline for >or= 4 consecutive weeks in pain (pain intensity or analgesic consumption) and Karnofsky performance status, stability in one but improvement in the other, or stability in pain and performance status but improvement in weight. RESULTS: Of 319 patients, 19% treated with GemCap and 20% treated with Gem experienced a CBR, with a median duration of 9.5 and 6.5 weeks, respectively (P < .02); 54% of patients treated with GemCap and 60% treated with Gem had no CBR (remaining patients were not assessable). There was no treatment difference in QOL (n = 311). QOL indicators were improving under chemotherapy (P < .05). These changes differed by the time to failure, with a worsening 1 to 2 months before treatment failure (all P < .05). CONCLUSION: There is no indication of a difference in CBR or QOL between GemCap and Gem. Regardless of their initial condition, some patients experience an improvement in QOL on chemotherapy, followed by a worsening before treatment failure.

Despite data favouring a role of dietary fat in colonic carcinogenesis, no study has focused on tissue n3 and n6 fatty acid (FA) status in human colon adenoma-carcinoma sequence. Thus, FA profile was measured in plasma phospholipids of patients with colorectal cancer (n = 22), sporadic adenoma (n = 27), and normal colon (n = 12) (control group). Additionally, mucosal FAs were assessed in both diseased and normal mucosa of cancer (n = 15) and adenoma (n = 21) patients, and from normal mucosa of controls (n = 8). There were no differences in FA profile of both plasma phospholipids and normal mucosa, between adenoma and control patients. There were considerable differences, however, in FAs between diseased and paired normal mucosa of adenoma patients, with increases of linoleic (p = 0.02), dihomogammalinolenic (p = 0.014), and eicosapentaenoic (p = 0.012) acids, and decreases of alpha linolenic (p = 0.001) and arachidonic (p = 0.02) acids in diseased mucosa. A stepwise reduction of eicosapentaenoic acid concentrations in diseased mucosa from benign adenoma to the most advanced colon cancer was seen (p = 0.009). Cancer patients showed lower alpha linolenate (p = 0.002) and higher dihomogammalinolenate (p = 0.003) in diseased than in paired normal mucosa. In conclusion changes in tissue n3 and n6 FA status might participate in the early phases of the human colorectal carcinogenesis.

La durée de psychose non traitée (Duration of Untreated Psychosis, DUP) est définie par le temps écoulé entre l'émergence d'un trouble psychotique et le début de son traitement. La réduction de la DUP est un des objectifs principaux des programmes spécialisés dans le traitement de la psychose émergente, de nombreux travaux de recherche suggérant qu'une DUP longue est associée à une évolution défavorable de la maladie. Ces résultats restent cependant controversés, certaines études ne démontrant pas une telle association. Cette contradiction dans les résultats pourrait être la conséquence d'un manque d'uniformité dans les définitions appliquées pour mesurer la DUP, plus particulièrement en ce qui concerne la définition de ce que l'on considère être « début » du traitement. En effet, si l'étude de la phase d'émergence de la pathologie psychotique a été le focus d'une attention considérable qui a conduit à un certain degré de consensus quant à sa définition, le concept de début du traitement n'est clairement pas défini de manière aussi homogène. Compte tenu de l'importance des enjeux relatifs à l'intervention précoce dans les troubles psychotiques, il nous a semblé utile d'explorer cette question de manière plus approfondie, considérant qu'un manque de consensus dans la définition de la DUP contribue certainement à troubler les résultats des études qui visent à évaluer son impact sur l'évolution de ces maladies. En conséquence, l'objectif premier de ce travail est d'explorer l'impact de l'application de diverses définitions de début de traitement sur l'estimation de la DUP. Dans un premier article, publié dans Acta Neuropsychiatrica en 2009 (Duration of untreated psychosis : What are we talking about ?), le focus a été placé sur une revue de littérature concernant les définitions utilisées pour caractériser la fin de la DUP ainsi que sur les conséquences possibles d'un manque de précision dans cette définition sur l'évaluation de l'impact d'un retard de traitement dans la psychose débutante. Ce travail nous a permis d'identifier trois groupes principaux de définition de fin de DUP (End of DUP ; E-DUP) parmi les multiples critères utilisés dans les études publiées. E-DUP-1 est définie par la mise en route d'un traitement antipsychotique, le plus souvent sans tenir compte ni du dosage prescrit, ni de l'adhérence au traitement. E-DUP-2 est définie par l'entrée dans un programme de traitement spécialisé, et E-DUP-3 enfin est définie par la conjonction de la prescription d'un traitement antipsychotique adapté, de l'adhérence à ce traitement, et de la mise en route d'une prise en charge dans un programme spécialisé. En conclusion, nous relevions que cette grande variété dans les définitions appliquées pour l'évaluation de la DUP avait probablement contribué à l'aspect contradictoire des résultats des études de son impact sur l'évolution des psychoses et qu'il était donc temps de proposer une définition de consensus. La deuxième étude a été conduite dans le cadre d'un suivi de cohorte mis en place dans le programme de Traitement et Intervention Précoce dans les troubles Psychotiques (TIPP) établi dans le Département de Psychiatrie du CHUV à Lausanne depuis 2004. Les objectifs de cette seconde étude étaient au nombre de trois: (1) Exploration des variations de la DUP en fonction de l'application de trois principales définitions de fin de DUP (E-DUP) identifiées dans la littérature ; (2) Evaluation de la proportion de patients remplissant au moins une fois au cours des 18 mois de traitement la définition de E-DUP la plus compatible avec les directives de traitement proposées par l'International Early Psychosis Association (patient est à la fois engagé dans le traitement et se montre compliant à la médication, E-DUP-3); (3) Enfin, identification desfacteurs qui caractérisent les patients qui ne remplissent jamais les critères de cette dernière définition. L'exploration de différentes durées de DUP en utilisant les trois définitions d'E-DUP a donné les résultats suivants : La DUP1 médiane (2.2 mois) était significativement plus courte que la DUP2 (7.4 mois), et la DUP3 (13.6 mois) était significativement la plus longue des trois. De plus, 19.7% des patients n'avaient jamais rempli les critères de E-DUP-3 ; on peut donc considérer que près de 20% des patients traités dans ce programme spécialisé ne recevaient pas un traitement adéquat selon les directives intrernationales actuellement reconnues. Sur la base de ces chiffres, il apparaît clairement que, dans les études de l'impact de la DUP sur l'évolution de la psychose débutante, bon nombre des patients pour lesquels on considère que la DUP est terminée ne sont en fait pas adéquatement traités. Il est en conséquence très probable que ceci ait faussé les résultats de ces études, et qu'une définition plus restrictive permettrait de répondre de manière plus précise à cette question. Les patients qui ne remplissaient pas les critères E-DUP3 au cours des 18 premiers mois de traitement étaient caractérisés par un moins bon niveau de fonctionnement au cours de leur vie (« lower lifetime SOFAS » ; p=0.017) et ils étaient plus susceptibles de consommer du cannabis à l'entrée du programme ???? (?2 (1, n=49)=4.241, p=0.039). Pour ceux qui avaient rempli les critères E-DUP-3 au cours des 18 mois, une longue DUP3 était associée avec un jeune âge au début des symptômes psychotiques (rs =-0.573, p<0.001), et avec un faible niveau de fonctionnement pré-morbide (score de PAS élevés (rs =0.373, p=0.001), niveau maximal au cours de la vie bas pour le GAF(rs =-0.367, p<0.001) et pour le SOFAS (rs =-0.314, p=0.003)). En conclusion, ce travail a permis de mettre en évidence une grande variabilité dans la définition de la fin de la DUP parmi les études publiées jusque à ce jour, et l'impact important que le choix d'une ou l'autre de ces définitions peut avoir sur l'estimation de la DUP. De plus, nous avons observé que malgré la mise en place d'un programme spécialisé, près de 20% des patients ne remplissent pas les critères d'exposition à un traitement adéquat au cours des 18 premiers mois de prise en charge. Il est donc probable que l'estimation de l'impact de la DUP ait été faussé par cette variabilité, et il semble important que la communauté scientifique s'accorde sur une définition plus rigoureuse de cette variable. Enfin, certaines caractéristiques permettent d'identifier les patients qui sont à risque de ne pas remplir les critères de traitement adéquat a cours des 18 premiers mois de prise en charge ; il est possible qu'une identification précoce de ceux-ci permette la mise en place de stratégies mieux adaptées pour les aider à s'engager dans les soins. Le futur développement de ce travail sera d'évaluer l'impact de la DUP sur l'évolution des patients au cours des 36 mois de traitement proposés dans le programme TIPP, en appliquant les divers critères E-DUP, afin de voir si notre hypothèse que la variation des définitions a effectivement faussé les résultats de telles études. Nous devons pour cela attendre qu'un nombre suffisant de patients ait complété les 36 mois de traitement, de manière à avoir une puissance statistique suffisante pour répondre clairement à cette question.

BACKGROUND: Interleukin-1 is pivotal in the pathogenesis of systemic juvenile idiopathic arthritis (JIA). We assessed the efficacy and safety of canakinumab, a selective, fully human, anti-interleukin-1β monoclonal antibody, in two trials. METHODS: In trial 1, we randomly assigned patients, 2 to 19 years of age, with systemic JIA and active systemic features (fever; ≥2 active joints; C-reactive protein, >30 mg per liter; and glucocorticoid dose, ≤1.0 mg per kilogram of body weight per day), in a double-blind fashion, to a single subcutaneous dose of canakinumab (4 mg per kilogram) or placebo. The primary outcome, termed adapted JIA ACR 30 response, was defined as improvement of 30% or more in at least three of the six core criteria for JIA, worsening of more than 30% in no more than one of the criteria, and resolution of fever. In trial 2, after 32 weeks of open-label treatment with canakinumab, patients who had a response and underwent glucocorticoid tapering were randomly assigned to continued treatment with canakinumab or to placebo. The primary outcome was time to flare of systemic JIA. RESULTS: At day 15 in trial 1, more patients in the canakinumab group had an adapted JIA ACR 30 response (36 of 43 [84%], vs. 4 of 41 [10%] in the placebo group; P<0.001). In trial 2, among the 100 patients (of 177 in the open-label phase) who underwent randomization in the withdrawal phase, the risk of flare was lower among patients who continued to receive canakinumab than among those who were switched to placebo (74% of patients in the canakinumab group had no flare, vs. 25% in the placebo group, according to Kaplan-Meier estimates; hazard ratio, 0.36; P=0.003). The average glucocorticoid dose was reduced from 0.34 to 0.05 mg per kilogram per day, and glucocorticoids were discontinued in 42 of 128 patients (33%). The macrophage activation syndrome occurred in 7 patients; infections were more frequent with canakinumab than with placebo. CONCLUSIONS: These two phase 3 studies show the efficacy of canakinumab in systemic JIA with active systemic features. OBJECTIVE: The purpose of this article is to assess the effect of the adaptive statistical iterative reconstruction (ASIR) technique on image quality in hip MDCT arthrography and to evaluate its potential for reducing radiation dose. SUBJECTS AND METHODS: Thirty-seven patients examined with hip MDCT arthrography were prospectively randomized into three different protocols: one with a regular dose (volume CT dose index [CTDIvol], 38.4 mGy) and two with a reduced dose (CTDIvol, 24.6 or 15.4 mGy). Images were reconstructed using filtered back projection (FBP) and four increasing percentages of ASIR (30%, 50%, 70%, and 90%). Image noise and contrast-to-noise ratio (CNR) were measured. Two musculoskeletal radiologists independently evaluated several anatomic structures and image quality parameters using a 4-point scale. They also jointly assessed acetabular labrum tears and articular cartilage lesions. RESULTS: With decreasing radiation dose level, image noise statistically significantly increased (p=0.0009) and CNR statistically significantly decreased (p=0.001). We also found a statistically significant reduction in noise (p=0.0001) and increase in CNR (p≤0.003) with increasing percentage of ASIR; in addition, we noted statistically significant increases in image quality scores for the labrum and cartilage, subchondral bone, overall diagnostic quality (up to 50% ASIR), and subjective noise (p≤0.04), and statistically significant reductions for the trabecular bone and muscles (p≤0.03). Regardless of the radiation dose level, there were no statistically significant differences in the detection and characterization of labral tears (n=24; p=1) and cartilage lesions (n=40; p≥0.89) depending on the ASIR percentage. CONCLUSION: The use of up to 50% ASIR in hip MDCT arthrography helps to reduce radiation dose by approximately 35-60%, while maintaining diagnostic image quality comparable to that of a regular-dose protocol using FBP.

The alpha-1 adrenergic receptors (alpha(1)ARs) are critical in sympathetically mediated vasoconstriction. The specific role of each alpha(1)AR subtype in regulating vasoconstriction remains highly controversial. Limited pharmacological studies suggest that differential alpha(1)AR responses may be the result of differential activation of junctional versus extrajunctional receptors. We tested the hypothesis that the alpha(1B)AR subtype is critical in mediating sympathetic junctional neurotransmission. We measured in vivo integrated cardiovascular responses to a hypotensive stimulus (induced via transient bilateral carotid occlusion [TBCO]) in alpha(1B)AR knockout (KO) mice and their wild-type (WT) littermates. In WT mice, after dissection of the carotid arteries and denervation of aortic baroreceptor buffering nerves, TBCO produced significant pressor and positive inotropic effects. Both responses were markedly attenuated in alpha(1B)AR KO mice (change systolic blood pressure 46+/-8 versus 11+/-2 mm Hg; percentage change in the end-systolic pressure-volume relationship [ESPVR] 36+/-7% versus 12+/-2%; WT versus KO; P<0.003). In vitro alpha(1)AR mesenteric microvascular contractile responses to endogenous norepinephrine (NE; elicited by electrical field stimulation 10 Hz) was markedly depressed in alpha(1B)AR KO mice compared with WT (12.4+/-1.7% versus 21.5+/-1.2%; P<0.001). In contrast, responses to exogenous NE were similar in alpha(1B)AR KO and WT mice (22.4+/-7.3% versus 33.4+/-4.3%; NS). Collectively, these results demonstrate a critical role for the alpha(1B)AR in baroreceptor-mediated adrenergic signaling at the vascular neuroeffector junction. Moreover, alpha(1B)ARs modulate inotropic responses to baroreceptor activation. The critical role for alpha(1B)AR in neuroeffector regulation of vascular tone and myocardial contractility has profound clinical implications for designing therapies for orthostatic intolerance.

OBJECTIVES: To compare daily energy expenditure between RA patients and matched controls, and to explore the relationship between daily energy expenditure or sedentariness and disease-related scores. METHODS: One hundred and ten patients with RA and 440 age- and sex-matched controls were included in this study. Energy expenditure was assessed using the validated physical activity (PA) frequency questionnaire. Disease-related scores included disease activity (DAS-28), functional status (HAQ), pain visual analogue scale (VAS) and fatigue VAS. Total energy expenditure (TEE) and the amount of energy spent in low- (TEE-low), moderate- (TEE-mod) and high-intensity (TEE-high) PAs were calculated. Sedentariness was defined as expending <10% of TEE in TEE-mod or TEE-high activities. Between-group comparisons were computed using conditional logistic regression. The effect of disease-related scores on TEE was investigated using linear regression. RESULTS: TEE was significantly lower for RA patients compared with controls [2392 kcal/day (95% CI 2295, 2490) and 2494  kcal/day (2446, 2543), respectively, P = 0.003]. A significant difference was found between groups in TEE-mod (P = 0.015), but not TEE-low (P = 0.242) and TEE-high (P = 0.146). All disease-related scores were significantly poorer in sedentary compared with active patients. TEE was inversely associated with age (P < 0.001), DAS-28 (P = 0.032) and fatigue VAS (P = 0.029), but not with HAQ and pain VAS. CONCLUSION: Daily energy expenditure is significantly lower in RA patients compared with matched controls, mainly due to less moderate-intensity PAs performed. Disease activity and fatigue are important contributing factors. These points need to be addressed if promoting PA in RA patients is a health goal. Trial registration. OBJECTIVES: Toll-like receptors (TLRs) are innate immune sensors that are integral to resisting chronic and opportunistic infections. Mounting evidence implicates TLR polymorphisms in susceptibilities to various infectious diseases, including HIV-1. We investigated the impact of TLR single nucleotide polymorphisms (SNPs) on clinical outcome in a seroincident cohort of HIV-1-infected volunteers. DESIGN: We analyzed TLR SNPs in 201 antiretroviral treatment-naive HIV-1-infected volunteers from a longitudinal seroincident cohort with regular follow-up intervals (median follow-up 4.2 years, interquartile range 4.4). Participants were stratified into two groups according to either disease progression, defined as peripheral blood CD4(+) T-cell decline over time, or peak and setpoint viral load. METHODS: Haplotype tagging SNPs from TLR2, TLR3, TLR4, and TLR9 were detected by mass array genotyping, and CD4(+) T-cell counts and viral load measurements were determined prior to antiretroviral therapy initiation. The association of TLR haplotypes with viral load and rapid progression was assessed by multivariate regression models using age and sex as covariates. RESULTS: Two TLR4 SNPs in strong linkage disequilibrium [1063 A/G (D299G) and 1363 C/T (T399I)] were more frequent among individuals with high peak viral load compared with low/moderate peak viral load (odds ratio 6.65, 95% confidence interval 2.19-20.46, P < 0.001; adjusted P = 0.002 for 1063 A/G). In addition, a TLR9 SNP previously associated with slow progression was found less frequently among individuals with high viral setpoint compared with low/moderate setpoint (odds ratio 0.29, 95% confidence interval 0.13-0.65, P = 0.003, adjusted P = 0.04). CONCLUSION: This study suggests a potentially new role for TLR4 polymorphisms in HIV-1 peak viral load and confirms a role for TLR9 polymorphisms in disease progression.
