OBJECTIVES: Toll-like receptors (TLRs) are innate immune sensors that are integral to resisting chronic and opportunistic infections. Mounting evidence implicates TLR polymorphisms in susceptibilities to various infectious diseases, including HIV-1. We investigated the impact of TLR single nucleotide polymorphisms (SNPs) on clinical outcome in a seroincident cohort of HIV-1-infected volunteers. DESIGN: We analyzed TLR SNPs in 201 antiretroviral treatment-naive HIV-1-infected volunteers from a longitudinal seroincident cohort with regular follow-up intervals (median follow-up 4.2 years, interquartile range 4.4). Participants were stratified into two groups according to either disease progression, defined as peripheral blood CD4(+) T-cell decline over time, or peak and setpoint viral load. METHODS: Haplotype tagging SNPs from TLR2, TLR3, TLR4, and TLR9 were detected by mass array genotyping, and CD4(+) T-cell counts and viral load measurements were determined prior to antiretroviral therapy initiation. The association of TLR haplotypes with viral load and rapid progression was assessed by multivariate regression models using age and sex as covariates. RESULTS: Two TLR4 SNPs in strong linkage disequilibrium [1063 A/G (D299G) and 1363 C/T (T399I)] were more frequent among individuals with high peak viral load compared with low/moderate peak viral load (odds ratio 6.65, 95% confidence interval 2.19-20.46, P < 0.001; adjusted P = 0.002 for 1063 A/G). In addition, a TLR9 SNP previously associated with slow progression was found less frequently among individuals with high viral setpoint compared with low/moderate setpoint (odds ratio 0.29, 95% confidence interval 0.13-0.65, P = 0.003, adjusted P = 0.04). CONCLUSION: This study suggests a potentially new role for TLR4 polymorphisms in HIV-1 peak viral load and confirms a role for TLR9 polymorphisms in disease progression.

The 24-item Brief Psychiatric Rating Scale (BPRS, version 4.0) enables the rater to measure psychopathology severity. Still, little is known about the BPRS's reliability and validity outside of the psychosis spectrum. The aim of this study was to examine the factorial structure and sensitivity to change of the BPRS in patients with unipolar depression. Two hundred and forty outpatients with unipolar depression were administered the 24-item BPRS. Assessments were conducted at intake and at post-treatment in a Crisis Intervention Centre. An exploratory factor analysis of the 24-item BPRS produced a six-factor solution labelled "Mood disturbance", "Reality distortion", "Activation", "Apathy", "Disorganization", and "Somatization". The reduction of the total BPRS score and dimensional scores, except for "Activation", indicates that the 24-item BPRS is sensitive to change as shown in patients that appeared to have benefited from crisis treatment. The findings suggest that the 24-item BPRS could be a useful instrument to measure symptom severity and change in symptom status in outpatients presenting with unipolar depression.

The Summit Lake Watershed Improvement Project is a watershed-based sediment control project designed to greatly reduce to nearly eliminate sedimentation of an existing lake that is being renovated for use as a water source in southern Iowa. Summit Lake is owned by the City of Creston and was once a water source lake until around 1984. The watershed improvements will include lakeshore stabilization and erosion control practices as a precursor for related improvements to the lake and overall 4,900-acre watershed. Best practices included in this phase are the implementation of riprap, a rain garden, grade stabilization structures, grassed waterways, terraces, basins, water use and access ordinances, education and outreach, water monitoring, and other stream bank improvements. These improvements, along with leveraged work to be done by strategic partners, will enable the lake to be used for local and regional water supplies by sustaining the lake for many years to come. Without the lake rehabilitation, the lake will likely be filled with sedimentation to the point that it will have no recreational value. Key partners are the City of Creston, IDNR, Southern Iowa Rural Water Association, Union County, the Union County NRCS office, Southwestern Community College, and the Summit Lake Association, which is a non-profit group of landowners working to protect the lake. The project will address WIRB targets: a) streambank stabilization, b) livestock runoff, c) agricultural runoff and drainage, d) stormwater runoff, and e) a section of inadequately sewered community.

Electron energy-loss spectroscopy is used to map composition and electronic states in epitaxial La2/3Ca1/3MnO3 films grown on SrTiO3 001 and 110 substrates. It is found that in partially relaxed 110 films cationic composition and valence state of Mn3+/4+ ions are preserved across the film thickness. In contrast, in fully strained 001 films, the Ca/La ratio gradually changes across the film, being La rich at film/substrate interface and La depleted at free surface; Mn valence state changes accordingly. These observations suggest that a strongly orientation-dependent adaptative composition mechanism dominates stress accommodation in manganite films and provides microscopic understanding of their dissimilar magnetic properties.

Sleep problems among detainees are common. Appropriate evaluation and treatment remain challenging in correctional settings. However, this is not primarily a problem of resources; rather, it is, to a great extent, an issue of adequate training. Correctional health professionals need appropriate education regarding insomnia evaluation and management. Guidelines should be based on the principle of equivalence of care and should take into account all evidence from research in the community and in correctional settings. Educational material from outside prisons exists and should be made available to detainees and health professionals (Falloon et al., 2011; Sateia & Nowell, 2004). Priority should be given to changes in prison conditions and to nonpharmacological treatment. There is no evidence-based justification to replace BZD prescriptions with antipsychotics or antidepressants. In correctional settings, prescriptions of antipsychotics and antidepressants for sleep problems can increase risk due to polypharmacy and higher suicide risks. Correctional physicians should monitor and document the evaluation and treatment practice concerning insomnia complaints to improve safe, evidence-based treatment.

Capsaicinoids (CAPS) are substances responsible for pungency in Capsicum. It is important to quantify these types of compounds owing to their broad application in food, pharmaceuticals, cosmetics and chemical weapons. In this work, we developed an indirect spectrophotometric method based on the colorimetric reaction between CAPS, Co(II) 3.10×10-5 mol L-1 and 4-(2-pyridylazo) resorcinol (PAR) 6.23×10-5 and, in cachaça:water 92:8v/v solutions, for quantification of total CAPS in Capsicum peppers. The product of the reaction is CoPAR2CAPS2 and its absorption in aquo-ethanolic solution at 510 nm is proportional to the total CAPS concentration from 0.60 to 17.94 mg L-1. The values of limit of detection and limit of quantification were 0.0004 and 0.001 mg of CAPS/g of pepper, respectively, with 4% relative standard deviation. The developed method yielded similar results to those obtained from high performance liquid chromatography, with 95% of confidence.

The pharmacological effects of 4-phenyl-2-trichloromethyl-3H-1,5-benzodiazepine hydrogen sulfate (PTMB), a novel synthetic benzodiazepine, were examined in mice. In the elevated plus-maze test of anxiety, 0.3-1 mg/kg diazepam ip (F(3,53) = 3.78; P<0.05) and 1-10 mg/kg PTMB ip increased (F(5,98) = 3.26; P<0.01), whereas 2 mg/kg picrotoxin ip decreased (F(3,59) = 8.32; P<0.001) the proportion of time spent in the open arms, consistent with an anxiolytic action of both benzodiazepines, and an anxiogenic role for picrotoxin. In the holeboard, 1.0 mg/kg diazepam ip increased (F(3,54) = 2.78; P<0.05) and 2 mg/kg picrotoxin ip decreased (F(3,59) = 4.69; P<0.01) locomotor activity. Rotarod assessment revealed that 1 mg/kg diazepam ip and 3, 10 and 30 mg/kg PTMB ip produced significant motor incoordination compared to vehicle control (F(4,70) = 7.6; P<0.001). These data suggest that the recently synthesized PTMB compound possesses anxiolytic activity and produces motor incoordination similar to those observed with diazepam.

This study aims to explore the effect of microRNA-21 (miR-21) on the proliferation of human degenerated nucleus pulposus (NP) by targeting programmed cell death 4 (PDCD4) tumor suppressor. NP tissues were collected from 20 intervertebral disc degeneration (IDD) patients, and from 5 patients with traumatic spine fracture. MiR-21 expressions were tested. NP cells from IDD patients were collected and divided into blank control group, negative control group (transfected with miR-21 negative sequences), miR-21 inhibitor group (transfected with miR-21 inhibitors), miR-21 mimics group (transfected with miR-21 mimics) and PDCD4 siRNA group (transfected with PDCD4 siRNAs). Cell growth was estimated by Cell Counting Kit-8; PDCD4, MMP-2,MMP-9 mRNA expressions were evaluated by qRT-PCR; PDCD4, c-Jun and p-c-Jun expressions were tested using western blot. In IDD patients, the expressions of miR-21 and PDCD4 mRNA were respectively elevated and decreased (both P<0.05). The miR-21 expressions were positively correlated with Pfirrmann grades, but negatively correlated with PDCD4 mRNA (both P<0.001). In miR-21 inhibitor group, cell growth, MMP-2 and MMP-9 mRNA expressions, and p-c-Jun protein expressions were significantly lower, while PDCD4 mRNA and protein expressions were higher than the other groups (all P<0.05). These expressions in the PDCD4 siRNA and miR-21 mimics groups was inverted compared to that in the miR-21 inhibitor group (all P<0.05). MiR-21 could promote the proliferation of human degenerated NP cells by targeting PDCD4, increasing phosphorylation of c-Jun protein, and activating AP-1-dependent transcription of MMPs, indicating that miR-21 may be a crucial biomarker in the pathogenesis of IDD.

Different aspects of the structure-magnetism and morphology-magnetism correlation in the ultrathin limit are studied in epitaxial Fe films grown on MgO(001). In the initial stages of growth the presence of substrate steps, intrinsically higher than an Fe atomic layer, prevent the connection between Fe islands and hence the formation of large volume magnetic regions. This is proposed as an explanation to the superparamagnetic nature of ultrathin Fe films grown on MgO in addition to the usually considered islanded, or Vollmer-Weber, growth. Using this model, we explain the observed transition from superparamagnetism to ferromagnetism for Fe coverages above 3 monolayers (ML). However, even though ferromagnetism and magnetocrystalline anisotropy are observed for 4 ML, complete coverage of the MgO substrate by the Fe ultrathin films only occurs around 6 ML as determined by polar Kerr spectra and simulations that consider different coverage situations. In annealed 3.5 ML Fe films, shape or configurational anisotropy dominates the intrinsic magnetocrystalline anisotropy, due to an annealing induced continuous to islanded morphological transition. A small interface anisotropy in thicker films is observed, probably due to dislocations observed at the Fe¿MgO(001) interface.

En la presente investigación se buscan cuatro objetivos principales: caracterizar a la población andaluza de menores de protección, comprendidos entre los cuatro y los doce años de edad, detectando los principales rasgos psico-socio-educativos de la misma; analizar las necesidades de la población andaluza de menores de protección, comprendidos entre los cuatro y los doce años de edad, para identificar los objetivos de los posibles y futuros programas de interevención socio-educativa a implementar con la referida población; constatar si los servicios sociales, que desde instancias públicas y/o privadas, se les está ofertando a la población andaluza de menores de protección, se adecuan a los rasgos y necesidades que la misma presenta; ofertar a la Administración Autonómica Andaluza, así como a las agencias de carácter social, un asesoramiento, lo más riguroso posible, que les puede ayudar a tomar decisiones adecuadas y eficaces para la resolución de los problemas que presenta la población de menores de protección, comprendida entre los cuatro y doce años de edad. Se seleccionó una muestra grande (el 32 por ciento del total de la población). Lo que quiere decir que la muestra la conformaban 426 menores (4-12 años) adaluces de ambos sexos. El procedimiento llevado a cabo en la investigación constó de tres etapas: 1. las necesidades expresadas por los menores a través de los cuestionarios, se clasificaron en primer lugar en torno a las cinco áreas o dimensiones aparecidas en el modelo jerárquico de necesidades expuesto por Maslow (1954). Se confeccionó con ellas un listado que los menores o en su lugar, los educadores, evaluaron del uno al cinco, jerarquizando las consideradas por ellos como más importantes. 2. Para conseguir la mayor objetividad posible en el proceso, se contrastó el valor dado a cada una de las necesidades del listado anterior, con el valor que diesen a las mismas y a otras que se pudieron agregar por su importancia. 3. Se aportó al estudio la literatura especializada para dar objetividad. Este requisito se alcanzó mediante un listado de necesidades siguiendo las directrices y aportaciones emanadas de dichas fuentes. El valor dado a cada una de las necesidades del listado, se contrastó con los valores que aparecen en los dos primeros listados: el listado evaluado por los menores y el evaluado por la muestra de educadores de centros de protección. Con el fin de recabar la información necesaria y suficiente para caracterizar a los menores, objeto de estudio y analizar las necesidades de los mismos, se utilizaron varios instrumentos y técnicas: un cuestionario aplicado a los menores o en su defecto a los educadores; un listado de necesidades, deducido de las respuestas dadas al anterior cuestionario; listado aplicado a los menores y/o educadores, así como a una muestra de educadores de centros de protección; un listado de necesidades, apoyándose en las directrices emanadas de la literatura especializada y la comunidad científica; y la técnica de 'Reunión en profundidad' aplicada a los educadores de centros de protección para recabar información. Entre las conclusiones emanadas del estudio destacamos: la mayoría de los centros de protección no reúnen los requisitos mínimos que faciliten la atención individualizada de los menores institucionalizados; debe cuidarse el enclave geográfico de los centros de protección; debe determinarse el centro escolar más adecuado para el menor; debe cuidarse que los menores de protección utilicen los recursos sociales normalizados; se debe favorecer el retorno de los menores a su entorno natural en el periodo de tiempo más breve posible; si no es posible el retorno del menor a la familia biológica, deben preverse medidas alternativas al internamiento; se debe cuidar la elección del equipo de educadores.

Se presentan las principales fases del proyecto de implantación de un sistema de gestión ambiental de acuerdo con al normativa española, en el IES Ramón y Cajal de Valladolid. En una primera fase se procede al diagnóstico ambiental inicial y a la formación por objetivos y áreas de trabajo de los grupos establecidos. A continuación, se procede a la elaboración de la documentación necesaria, entre ella, la política ambiental del IES, y la implantación del sistema. La fase de seguimiento del sistema se realiza a través de auditorías internas y revisiones por parte de la dirección. Finalmente, se obtiene la certificación de la norma por parte de SGS.

RESUMO: Objectivo: Face à exiguidade de estudos em Portugal nesta temática, o objectivo do estudo foi a análise entre a aptidão cardiorrespiratória (ACR), e a prevalência da pré-obesidade e obesidade em crianças do 4º ano do 1º ciclo. Método: Foi efectuada uma revisão sistemática da literatura (RSL), evocando-se estudos tranversais e de RCT, cruzando-se com resultados do estudo observacional, do Agrupamento de Escolas Professor Armando Lucena, do concelho de Mafra, distrito de Lisboa. Do estudo de RSL, e estudo observacional, fez-se a análise da verificação da associação inversa, entre a “aptidão cardiorespiratória, a pré-obesidade e obesidade”. O estudo observacional, foi transversal, incidindo sobre 143 crianças, (73 raparigas) dos 9-12 anos de idade do concelho de Mafra. Foram utilizados os pontos de corte da International Obesity Task Force (IOTF), para definir a pré-obesidade e obesidade. Registou-se o IMC e a % Massa Gorda por bioimpedância. A avaliação da ACR foi efectuada através do teste Vaivém 20 metros Fitnessgram, utilizando-se equação de Fernhall et al., (1998). Os alunos foram avaliados por questionário sobre actividade física (AF) extra-curricular (QAD, Telama et al., 1997); os pais sobre os níveis AF (IPAQ, Bauman et al., 2009) e estatuto sócio-económico (ESE). Resultados: Os resultados do estudo observacional, corroboram os de outros estudos RSL. Não houve diferenças entre géneros na prevalência de pré-obesidade e obesidade: rapazes (20.55% e 8.21%) e raparigas (34.28% e 5.71%) (p<0.176). As crianças com maior ACR têm menor IMC (p<0.01) e MG (p<0.001) e a idade não esteve associada à ACR. Os pré-obesos são na sua maioria insuficientemente activos, e os normoponderais são insuficientemente activos (p=0.033). Não houve associação entre o ESE e AF dos pais e o IMC, ACR ou QAD dos alunos. Com base nos resultados encontrados na revisão, procurou-se situar diferentes abordagens teóricas sobre a actividade física, dando ênfase principal à importância “ da promoção (acesso) da actividade física através da educação física orientada pedagogicamente em todos os ciclos de ensino. Conclusão: As crianças que tiveram maior ACR registaram menor IMC e MG, independente da idade e do sexo. Verificou-se ainda que as variáveis de ESE e AF dos pais, não está associada aos resultados da ACR, IMC e MG das crianças. ABSTRACT: Objective: Given the paucity of studies in Portugal on this theme, the goal of the study was to analyse the of cardiorespiratory fitness (CRF), and the prevalence of overweight and obesity in children attending the fourth year of primary school. Method: A systematic literature review was conducted, in which cross-cutting studies and RCT were covered and, intersected with observational results obtained, from the group of Schools Professor Armando Lucena, located in, the municipality of Mafra, Lisbon district. From the systematic review of literature (SRL), plus the observational study, the verification analysis of the inverse association between 'cardiorespiratory fitness, pre-obesity and obesity' was performed.The observational study was cross-sectional, focusing on 143 children (73 girls) of 9-12 years old. To define overweight and obesity the IOTF cutoffs were used. BMI and percentage of fat mass were recorded by means of bioelectrical impedance. The assessment was made through the CRF test shuttle Fitnessgram 20 meters, using the equation proposed by Fernhall et al., (1998). Students were assessed by questionnaire on physical activity (PA), extra-curricular PA (PAF Telama et al., 1997); parents were questioned regarding their PA levels (IPAQ, Bauman et al., 2009) and socio-economic status (SES). Results: The results of the observational study, corroborate, other SRL studies. There were no gender differences in the prevalence of overweight and obesity: men (20. 55% and 8.21%) and girls (34.28% and 5.71%) (p <0.176). Children with higher BMI have a lower CRF (p <0.01) and MG (p <0.001). Age was not observed to be associated with CRF. The pre-obese are mostly insufficiently active, and the normalweight are insufficiently active (p = 0.033). There was no association between the PA, the ESS, the parents, and the BMI, the CRF or the PAF of the students. Based on the results found in the review, different theoretical approaches regarding physical activity were. Emphasis was given to the importance of promoting the access to physical activity through pedagogically oriented physical education in all cycles of education. Conclusion: Children who had higher ACR showed lower FAT and BMI, regardless of age and sex. Furthermore, it was found that parents’ SES and PA variables are, not associated to children’s CRF, BMI and FAT.
