Process management in healthcare.Sant Camil Hospital case study

Nowadays due to the crisis, some government measures are aimed at reducing healthcare spending, affecting in some level or another the quality offered. Process management is said to be a useful tool for reducing healthcare costs by improving management without any additional economic investment. That is doing more with the same resources and without reducing the quality offered. In this study an empirical case of a Catalan hospital is presented. Overall, the usefulness of process management in the healthcare sector is shown and some tips are provided for those managers that want to implement this management system in their hospitals. This work is also interesting for those managers responsible for the National Healthcare System due to a big question is stated: what would happen if process management was implemented in the whole healthcare system?

Jardueran oinarritutako komunitateak enpresa txiki eta ertainetan

Lan honetan, gure artean gutxi ezagutzen diren “Jardueran Oinarritutako Komunitateek” (JOK) enpresa txiki eta ertainetan izan dezaketen eragina aztertzen da. Zehazki, ikergaia hauxe da: organizazio barruan komunikazioa eta ezagutzaren jarioa areagotzea helburu duen tresna hori Gipuzkoako bederatzi enpresa eta erakundetan nola txertatu den eta txertaketa horren ondorioak zeintzuk izan diren erakustea. Bereziki, puntu hauei begiratu zaie: JOKetan aritu diren pertsonen espektatiben asebetetze-mailari, lan taldeen emaitzei, lan taldeen jarduera ahalbideratzen duten faktoreei eta egon diren oztopoei. Efectos de la eutrofizacion sobre la pesca en el lago Pellegrini.

Por los problemas ocasionados en el deterioro del lago, en la pesca comercial y deportiva, se planteó como objetivo el estudio de las características limnológicas del ambiente relacionando la situación actual con la de años anteriores y además con los aportes exógenos que recibe el ecosistema. Se realizaron monitoreos de calidad físico-química del agua, nutrientes y plancton del lago y su influencia sobre el recurso pesquero, relacionando las capturas reales con los altos rendimientos pesqueros teóricos determinados en base a parámetros químicos, biológicos y morfoedáficos. Los resultados comprueban una disminución de la productividad secundaria y la población de peces, entre otros problemas de eutrofización. Se estudian distintas posibilidades para revertir esta tendencia del lago y recuperar e impulsar la pesquería comercial y deportiva.

Evidence for classification of c.1852_1853AA > GC in MLH1 as a neutral variant for Lynch syndrome

Background: Lynch syndrome (LS) is an autosomal dominant inherited cancer syndrome characterized by early onset cancers of the colorectum, endometrium and other tumours. A significant proportion of DNA variants in LS patients are unclassified. Reports on the pathogenicity of the c.1852_1853AA>GC (p.Lys618Ala) variant of the MLH1 gene are conflicting. In this study, we provide new evidence indicating that this variant has no significant implications for LS. Methods: The following approach was used to assess the clinical significance of the p.Lys618Ala variant: frequency in a control population, case-control comparison, co-occurrence of the p.Lys618Ala variant with a pathogenic mutation, co-segregation with the disease and microsatellite instability in tumours from carriers of the variant. We genotyped p.Lys618Ala in 1034 individuals (373 sporadic colorectal cancer [CRC] patients, 250 index subjects from families suspected of having LS [revised Bethesda guidelines] and 411 controls). Three well-characterized LS families that fulfilled the Amsterdam II Criteria and consisted of members with the p.Lys618Ala variant were included to assess co-occurrence and co-segregation. A subset of colorectal tumour DNA samples from 17 patients carrying the p.Lys618Ala variant was screened for microsatellite instability using five mononucleotide markers. Results: Twenty-seven individuals were heterozygous for the p.Lys618Ala variant; nine had sporadic CRC (2.41%), seven were suspected of having hereditary CRC (2.8%) and 11 were controls (2.68%). There were no significant associations in the case-control and case-case studies. The p.Lys618Ala variant was co-existent with pathogenic mutations in two unrelated LS families. In one family, the allele distribution of the pathogenic and unclassified variant was in trans, in the other family the pathogenic variant was detected in the MSH6 gene and only the deleterious variant co-segregated with the disease in both families. Only two positive cases of microsatellite instability (2/17, 11.8%) were detected in tumours from p.Lys618Ala carriers, indicating that this variant does not play a role in functional inactivation of MLH1 in CRC patients. Conclusions: The p.Lys618Ala variant should be considered a neutral variant for LS. These findings have implications for the clinical management of CRC probands and their relatives.

No primeiro dia para apresentação de emendas ao projeto de Constituição, 271 constituintes apresentaram suas propostas. O prazo corre até as 19 horas do dia 13 de janeiro. 22 funcionários da Secretaria Geral da Mesa trabalharão no recebimento das propostas. Pelo Regimento Interno, poderão ser apresentadas 2236 emendas individuais, 4 para cada constituinte. Além disso, 280 constituintes, juntos poderão assinar as emendas coletivas para mudar títulos inteiros do projeto. A primeira emenda apresentada ontem é contra o monopólio estatal na distribuição dos combustíveis. Muitos constituintes gostariam de apresentar mais de 4 emendas. O senador Marco Maciel está apresentando uma emenda coletiva pelo Presidencialismo. Ele espera conseguir as assinaturas necessárias. Os grupos estão preparando suas emendas coletivas. Entre os temas estão a estabilidade no emprego, a organização sindical, a previdência social. O Grupo do Entendimento, o Centrinho, quer consenso nos temas polêmicos da Constituinte. Ontem, o grupo terminou suas propostas na ordem social, discutindo temas como a jornada de trabalho de 8 horas diárias e 44 horas semanais.

Three-dimensional growth as multicellular spheroid activates the proangiogenic phenotype of colorectal carcinoma cells via LFA-1-dependent VEGF: implications on hepatic micrometastasis

Background: The recruitment of vascular stromal and endothelial cells is an early event occurring during cancer cell growth at premetastatic niches, but how the microenvironment created by the initial three-dimensional (3D) growth of cancer cells affects their angiogenesis-stimulating potential is unclear. Methods: The proangiogenic profile of CT26 murine colorectal carcinoma cells was studied in seven-day cultured 3D-spheroids of <300 mu m in diameter, produced by the hanging-drop method to mimic the microenvironment of avascular micrometastases prior to hypoxia occurrence. Results: Spheroid-derived CT26 cells increased vascular endothelial growth factor (VEGF) secretion by 70%, which in turn increased the in vitro migration of primary cultured hepatic sinusoidal endothelium (HSE) cells by 2-fold. More importantly, spheroid-derived CT26 cells increased lymphocyte function associated antigen (LFA)-1-expressing cell fraction by 3-fold; and soluble intercellular adhesion molecule (ICAM)-1, given to spheroid-cultured CT26 cells, further increased VEGF secretion by 90%, via cyclooxygenase (COX)-2-dependent mechanism. Consistent with these findings, CT26 cancer cells significantly increased LFA-1 expression in non-hypoxic avascular micrometastases at their earliest inception within hepatic lobules in vivo; and angiogenesis also markedly increased in both subcutaneous tumors and hepatic metastases produced by spheroid-derived CT26 cells. Conclusion: 3D-growth per se enriched the proangiogenic phenotype of cancer cells growing as multicellular spheroids or as subclinical hepatic micrometastases. The contribution of integrin LFA-1 to VEGF secretion via COX-2 was a micro environmental-related mechanism leading to the pro-angiogenic activation of soluble ICAM-1-activated colorectal carcinoma cells. This mechanism may represent a new target for specific therapeutic strategies designed to block colorectal cancer cell growth at a subclinical micrometastatic stage within the liver.

Language distance and non-native syntactic processing: Evidence from event-related potentials

[EN] In this study, we explore native and non-native syntactic processing, paying special attention to the language distance factor. To this end, we compared how native speakers of Basque and highly proficient non-native speakers of Basque who are native speakers of Spanish process certain core aspects of Basque syntax. Our results suggest that differences in native versus non-native language processing strongly correlate with language distance: native/non-native processing differences obtain if a syntactic parameter of the non-native grammar diverges from the native grammar. Otherwise, non-native processing will approximate native processing as levels of proficiency increase. We focus on three syntactic parameters: (i) the head parameter, (ii) argument alignment (ergative/accusative), and (iii) verb agreement. The first two diverge in Basque and Spanish, but the third is the same in both languages. Our results reveal that native and non-native processing differs for the diverging syntactic parameters, but not for the convergent one. These findings indicate that language distance has a significant impact in non-native language processing.

[EN] This paper is devoted to i, the Basque dative case marker. In part I, the paper aims at presenting a general approach the locus of syntactic variation within Universal Grammar (sections 1.1 to 1.3). After an excursus on the use of minorized languages in science (1.4), a discussion on parameters is provided. In (1.5), the notion of macroparameter from the 80’s is critically discussed mainly but not along the lines of Newmeyer (2005). The last section (1.6)supports to focus on microparameters, along the lines of Kayne (2000, 2005), as the right way to properly understand and explain variation. Part II provides an analysis of the main properties of Basque datives (sections 2.1 and 2.2). Sections 2.3 and 2.4 are devoted to the nature of Basque inflectional morpheme (k)i, interpreted asan applicative morpheme which ‘applies’ dative object arguments to the structure. Finally, microcomparative analyses on Basque Differential Object Marking and Dative Displacement are presented in sections (2.6) and (2.7) respectively.
