AbstractIron deficiency is a highly prevalent nutritional problem worldwide and it impacts on the cognitive development of children. Therefore, the aim of this research was to develop meatloaf with high iron content by using in their formulations pork liver. Meatloaves were prepared with additions of 9.98% and 13.31% (formulations A and B) of pork liver in order to meet 15% and 20% of the daily requirement of iron (10 mg/day) for children. Samples were evaluated regarding their physicochemical, microbiological and sensory characteristics. The results were subjected to Analysis of Variance (ANOVA) followed by Tukey test. Results of physicochemical analyses showed an increase in protein and mineral contents and a decrease in fat content. The iron and zinc contents were respectively 100.0% and 70.83% (formulation A) and 152.73% and 97.92% (formulation B) higher than that of the standard formulation. Regarding fat content, the reduction of 31.5% in formulation B makes it a light product. As for the microbiological aspect, all meatloaves were adequate for consumption. Regarding sensory analysis, all the attributes considered were not statistically different, but for purchase intention test formulation B was better accepted. Therefore, formulations A and B are good sources of iron and zinc.

Chemotherapy-induced anemia is often an important problem for cancer patients, and this complication can be treated with erythropoiesis-stimulating agents (ESAs). This commentary discusses the findings of a study by Bastit et al., in which 396 patients with nonmyeloid malignancies and chemotherapy-induced anemia were treated with darbepoetin alfa with or without intravenous iron. This phase III trial showed that intravenous iron supplementation increases the hematopoietic response rates to ESAs in cancer patients; however, this study provides no information as to whether all cancer patients with anemia should receive intravenous iron as well as treatment with ESAs. Further data are needed to identify those patients who might benefit from intravenous iron supplementation in addition to ESAs, in order to avoid overtreatment of patients who are unlikely to benefit from the additional iron. As both ESAs and intravenous iron have known short-term and long-term risks, identification of reliable predictors of response that can guide these treatments is necessary before this strategy can be implemented into practice.

The puzzling linkage between genetic hemochromatosis and histocompatibility loci became even more so when the gene involved, HFE, was identified. Indeed, within the well defined, mainly peptide-binding, MHC class I family of molecules, HFE seems to perform an unusual yet essential function. As yet, our understanding of HFE function in iron homeostasis is only partial; an even more open question is its possible role in the immune system. To advance on both of these avenues, we report the deletion of HFE α1 and α2 putative ligand binding domains in vivo. HFE-deficient animals were analyzed for a comprehensive set of metabolic and immune parameters. Faithfully mimicking human hemochromatosis, mice homozygous for this deletion develop iron overload, characterized by a higher plasma iron content and a raised transferrin saturation as well as an elevated hepatic iron load. The primary defect could, indeed, be traced to an augmented duodenal iron absorption. In parallel, measurement of the gut mucosal iron content as well as iron regulatory proteins allows a more informed evaluation of various hypotheses regarding the precise role of HFE in iron homeostasis. Finally, an extensive phenotyping of primary and secondary lymphoid organs including the gut provides no compelling evidence for an obvious immune-linked function for HFE.

O trabalho objetivou comparar a prevalência de anemia de crianças, antes e após 6 meses de consumo de leite fortificado com ferro, do Projeto Vivaleite, acompanhados de orientação nutricional. O estudo foi realizado em Itapeva, Piracicaba, São José dos Campos, Taubaté e São Paulo, locais selecionados devido ao aumento representativo do número de crianças beneficiadas pelo Projeto. A amostra foi constituída de 399 crianças, de 6 a 36 meses de idade. Os dados da criança e da família foram obtidos mediante aplicação de formulário. O diagnóstico da anemia foi efetuado por meio da dosagem de hemoglobina, utilizando-se o aparelho HemoCue® e o valor crítico proposto pela OMS, de 11,0g/dL. Os níveis médios de concentração de hemoglobina foram comparados pelo Teste T Pareado e Qui-quadrado de Pearson, com nível de significância de 5 por cento. Foi encontrada diferença significativa (p<0,05) entre as concentrações de hemoglobina antes (10,18g/dL) e após o consumo do leite do Vivaleite (10,99 g/dL), verificando-se um incremento de 0,81g/dL nos índices de hemoglobina. Ainda, houve uma diminuição significativa (p<0,05) na prevalência de crianças com anemia, ou seja, 38,8 por cento das crianças que apresentavam anemia no início do estudo não estavam anêmicas no final. Está bem documentada a efetividade da intervenção proposta pelo Vivaleite no controle da deficiência marcial, sendo que a proposta deve ser acompanhada por atividades de orientação nutricional constante, como as aplicadas neste trabalho o que, provavelmente, motivou o sucesso do programa
