This article will reveal the experiment and the subsequent investigation work carried out with patients having chronic and severe mental illness at the Creativity and Rehabilitation Workshop of the Mental Health Service at the Health Department no. 12 of the Hospital Francisco de Borja of Gandia, Spain. The course focuses on patient training in the use of photography and image as a means of expression and as a part of psychosocial therapies to improve patients’ quality of life in front of the society. In addition, it tries to enhance the analytic capacity of the patient not only with regards to the photography aesthetically, but also personally. There are many international scientific surveys backing up the knowledge and use of Artistic and Creative Therapy in chronic patients; however, these activities are seen as pioneering actions in Spain given the current health structure, which includes few national psychiatric centers developing them continuously and being temporally monitored. This experiment demonstrates that photography significantly improves the quality of life of chronic patients and motivates them socially to communicate themselves through art, just as it helps them using creativity as a tool for social action.

A 1-year-old child with clinical features of monosomy 14 is reported. She has dysmorphic facial features including ocular colobomata, dolichocephaly and microcephaly, retinal pigmentation, severe seizures, fair curly hair and tapering fingers. There was severe mental retardation. This is the first reported case of severe mosaic monosomy 14, with up to 30% mosaicism. A recognizable facial gestalt is present in children with 14q deletions or partial monosomy 14, as well as susceptibility to infection, feeding difficulties, seizures and retinal pigmentation. Schizophrenia is a common psychotic mental disorder that is believed to result from the effects of multiple genetic and environmental factors. In this study, we explored gene-gene interactions and main effects in both case-control (657 cases and 411 controls) and family-based (273 families, 1350 subjects) datasets of English or Irish ancestry. Fifty three markers in 8 genes were genotyped in the family sample and 44 markers in 7 genes were genotyped in the case-control sample. The Multifactor Dimensionality Reduction Pedigree Disequilibrium Test (MDR-PDT) was used to examine epistasis in the family dataset and a 3-locus model was identified (permuted p=0.003). The 3-locus model involved the IL3 (rs2069803), RGS4 (rs2661319), and DTNBP1 (rs21319539) genes. We used MDR to analyze the case-control dataset containing the same markers typed in the RGS4, IL3 and DTNBP1 genes and found evidence of a joint effect between IL3 (rs31400) and DTNBP1 (rs760761) (cross-validation consistency 4/5, balanced prediction accuracy=56.84%, p=0.019). While this is not a direct replication, the results obtained from both the family and case-control samples collectively suggest that IL3 and DTNBP1 are likely to interact and jointly contribute to increase risk for schizophrenia. We also observed a significant main effect in DTNBP1, which survived correction for multiple comparisons, and numerous nominally significant effects in several genes. (C) 2008 Elsevier B.V. Objectives. We investigated whether exposure to negative aspects of close relationships was associated with subsequent increase in body mass index (BMI) and waist circumference.
Methods. Data came from a prospective cohort study (Whitehall II) of 9425 civil servants aged 35 to 55 years at baseline (phase 1: 1985-1988). We assessed negative aspects of close relationships with the Close Persons Questionnaire (range 0-12) at phases 1 and 2 (1989-1990). We measured BMI and waist circumference at phases 3 (1991-1994) and 5 (1997-1999). Covariates at phase 1 included gender, age, marital status, ethnicity, BMI, employment grade, smoking, physical activity, fruit and vegetable consumption, and common mental disorder.
Results. After adjustment for sociodemographic characteristics and health behaviors, participants with higher exposure to negative aspects of close relationships had a higher likelihood of a 10% or greater increase in BMI and waist circumference (odds ratios per 1-unit increase 1.08 [95% confidence interval (CI)=1.02, 1.14; P=.007] and 1.09 [CI=1.04, 1.14; P <= .001], respectively) as well as a transition from the overweight (25 <= BMI <30) to the obese (BMI >= 30) category.
Conclusions. Adverse social relationships may contribute to weight gain.

Schizophrenia is an idiopathic mental disorder with a heritable component and a substantial public health impact. We conducted a multi-stage genome-wide association study (GWAS) for schizophrenia beginning with a Swedish national sample (5,001 cases and 6,243 controls) followed by meta-analysis with previous schizophrenia GWAS (8,832 cases and 12,067 controls) and finally by replication of SNPs in 168 genomic regions in independent samples (7,413 cases, 19,762 controls and 581 parent-offspring trios). We identified 22 loci associated at genome-wide significance; 13 of these are new, and 1 was previously implicated in bipolar disorder. Examination of candidate genes at these loci suggests the involvement of neuronal calcium signaling. We estimate that 8,300 independent, mostly common SNPs (95% credible interval of 6,300-10,200 SNPs) contribute to risk for schizophrenia and that these collectively account for at least 32% of the variance in liability. Common genetic variation has an important role in the etiology of schizophrenia, and larger studies will allow more detailed understanding of this disorder.

All suicides and related prior attempts occurring in Northern Ireland over two years were analyzed, focusing on number and timing of attempts, method, and mental health diagnoses. Cases were derived from coroner's records, with 90% subsequently linked to associated general practice records. Of those included, 45% recorded at least one prior attempt (with 59% switching from less to more lethal methods between attempt and suicide). Compared with those recording one attempt, those with 2+ attempts were more likely to have used less lethal methods at the suicide (OR = 2.77: 95% CI = 1.06, 7.23); and those using less lethal methods at the attempts were more likely to persist with these into the suicide (OR = 3.21: 0.79, 13.07). Finally, those with preexisting mental problems were more likely to use less lethal methods in the suicide: severe mental illness (OR = 7.88: 1.58, 39.43); common mental problems (OR = 3.68: 0.83, 16.30); and alcohol/drugs related (OR = 2.02: 0.41, 9.95). This analysis uses readily available data to highlight the persisting use of less lethal methods by visible and vulnerable attempters who eventually complete their suicide. Further analysis of such conditions could allow more effective prevention strategies to be developed.

This experiment (N 5 49) is the first to show that imagined contact can bufferanticipatory physiological responses to future interactions, and improve the qualityof these interactions. Participants imagined a positive interaction with a person withschizophrenia, or in a control condition, a person who did not have schizophrenia. They then interacted with a confederate whom they believed had schizophrenia. Participants in the imagined contact condition reported more positive attitudes andless avoidance of people with schizophrenia, displayed smaller anticipatory physio-logical responses, specifically smaller changes in interbeat interval and skin conduct-ance responses, and had a more positive interaction according to the confederate.These findings support applying imagined contact to improve interactions with people with severe mental illnesses.

Background: Contact with primary care and psychiatric services prior to suicide may be considerable, presenting
opportunities for intervention. However, there is scant knowledge on the frequency, nature and determinants of
contact.
Method: Retrospective cohort study-an analysis of deaths recorded as suicide by the Northern Ireland Coroner’s
Office linked with data from General Practice patient records over a 2 year period
Results: Eighty-seven per cent of suicides were in contact with General Practice services in the 12 months before
suicide. The frequency of contact with services was considerable, particularly among patients with a common
mental disorder or substance misuse problems. A diagnosis of psychiatric problems was absent in 40 % of suicides.
Excluding suicide attempts, the main predictors of a noted general practitioner concern for patient suicidality are
male gender, frequency of consultations, diagnosis of mental illness and substance misuse.
Conclusions: Despite widespread and frequent contact, a substantial proportion of suicidal people were
undiagnosed and untreated for mental health problems. General Practitioner alertness to suicidality may be too
narrowly focused.

Com este trabalho pretende-se realçar que os adolescentes passam por determinadas fases ou estádios da sua vida, em que se deparam com determinadas dificuldades ou até mesmo, incapacidades para levar a cabo a realização de determinadas necessidades; e que por isso mesmo, estes momentos podem vir a representar a oportunidade de crescimento da personalidade, mas ao mesmo tempo, o perigo de crescente vulnerabilidade ao distúrbio mental. Este relatório, expressa a preocupação com a problemática, analisa reflexivamente e descreve o trabalho desenvolvido na área da saúde mental e psiquiátrica por um enfermeiro, na Clínica da Juventude ao longo de seis meses, através da implementação do que se preconiza ser um Processo de Cuidados em Enfermagem (i.e., avaliação, diagnóstico, planeamento, intervenção e avaliação final), no acompanhamento de adolescentes com alterações do comportamento. O instrumento usado para avaliação diagnóstica foi a entrevista semiestrutura e semidireccionada, o planeamento e as intervenções variaram de acordo com a individualidade de cada adolescente e as suas necessidades particulares. Os resultados de uma forma geral demonstraram que o acompanhamento dos jovens com alterações do comportamento deve iniciar-se o mais precocemente possível, deve incidir na identificação das principais dificuldades ou necessidades e não nos diagnósticos médicos. O adolescente, deve também, ser abordado o mais holísticamente possível e nos diferentes setores onde se mobiliza. Conclui-se que a intencionalidade terapêutica deve recair no acompanhamento do adolescente, no sentido, de o tornar mais autónomo, capaz de assumir responsabilidades, tomar decisões, que cresça e se transforme num adulto autoconfiante e realizado; ABSTRACT: This work aims to emphasize that teenagers go through certain phases or stages of its life, when faced with certain difficulties and even disabilities to carry out the execution of particular needs, and that therefore, these moments may come to represent the opportunity for growth of personality, but at the same time, the danger of increasing vulnerability to mental disorder. This report expresses concern with the issue, reflective analysis and describes the work in the area of mental health and psychiatric care by a nurse in the Youth Clinic for six months, by implementing what it calls a procedure to be Care Nursing (ie, assessment, diagnosis, planning, intervention and final assessment), monitoring of adolescents with behavioral changes. The results generally showed that the monitoring of young people with behavioral changes should be initiated as early as possible, should focus on identifying the main difficulties or needs and not on medical diagnostics. The teen should also be approached holistically as possible and in different sectors where it is mobilized. We conclude that the intention must lie in therapeutic monitoring of the adolescent, in order, making it more autonomous, able to take responsibility, make decisions, to grow and become an adult self-confident and accomplished.

A psychotherapeutic approach for schizophrenia is now recommended as an adjuvant for psychopharmacology, since antipsychotic medications only have a partial impact especially as regards positive symptoms and insight. In addition, cognitive distortions and the lack of metacognitive skills might increase positive symptoms leading to poor social functioning. This underlines the need for specific approaches which target cognitive processes relevant for insight, and abilities in metacognition. Metacognitive training (MCT) is a structured group intervention, which enhances a patient's reflection on cognitive biases and improves problem-solving. The aim of our study was to assess MCTs' short term impact on insight, symptoms and quality of life. Fifty patients with schizophrenia or schizoaffective disorders and persistent positive symptoms (delusions or hallucinations) were enrolled in the study. After baseline assessment participants were randomised either to supportive therapy or MCT. Both groups used the same design (1h-session twice a week during 8weeks) although the basic knowledge given to participants was different between interventions. Participants were assessed at eight weeks based on the Scale to Assess Unawareness of Mental Disorder, Positive and Negative Syndrome Scale (PANSS), Psychotic Symptom Rating Scales, the Calgary Depression Scale for Schizophrenia and the Quality of Life Scale. Between-group differences were significant in favour of MCT on the PANSS positive scale. Between-group differences in post- and pre-test values showed a trend in favour of MCT for insight on hallucinations. Results of our study indicate that the MCT has an effect on reducing positive symptomatology, and a trend impact on insight and social functioning.

Au Québec, face à la prévalence élevée des problèmes de santé mentale et à la pénurie de médecins psychiatres, le médecin omnipraticien (MO) occupe une place primordiale dans la prise en charge et le suivi des soins de santé mentale. Dans le contexte de réforme du système de santé mentale axée sur un renforcement de la collaboration entre les MO, les psychiatres et les équipes de santé mentale, notre étude vise à mieux comprendre la pratique clinique et la pratique collaborative développée par les MO, leur appréciation des outils de travail et de la qualité des services de santé mentale, dans le but d’améliorer la complémentarité des soins au niveau primaire. Cette étude transversale impliquait 1415 MO de neuf territoires de centre de santé et de services sociaux (CSSS) du Québec. L’échantillon final était constitué de 398 MO représentatifs de lieux de pratique diversifiés et le taux de réponse était de 41%. Nos résultats mettent en évidence que la pratique clinique et la pratique collaborative des MO diffère selon le degré de gravité des problèmes de santé mentale des patients rencontrés, c’est à dire, trouble transitoire/modéré de santé mentale (TTM.SM) ou trouble grave de santé mentale (TG.SM), et que les MO sont favorables au fait de travailler en collaboration avec les autres professionnels de la santé mentale. Ainsi, il apparaît important de renforcer l’accessibilité des MO aux professionnels de la santé mentale, particulièrement les psychiatres, et de les informer de l’existence des autres acteurs en santé mentale sur leur territoire, pour renforcer la collaboration et la qualité des soins primaires de santé mentale.

La schizophrénie est une maladie psychiatrique grave qui affecte approximativement 1 % de la population. Il est clairement établi que la maladie possède une composante génétique très importante, mais jusqu’à présent, les études ont été limitées au niveau de l’identification de facteurs génétiques spécifiquement liés à la maladie. Avec l’avènement des nouvelles avancées technologiques dans le domaine du séquençage de l’ADN, il est maintenant possible d’effectuer des études sur un type de variation génétique jusqu’à présent laissé pour compte : les mutations de novo, c.-à-d. les nouvelles mutations non transmises de manière mendélienne par les parents. Ces mutations peuvent avoir deux origines distinctes : une origine germinale au niveau des gamètes chez les parents ou une origine somatique, donc au niveau embryonnaire directement chez l’individu. L’objectif général de la présente recherche est de mieux caractériser les mutations de novo dans la schizophrénie. Comme le rôle de ces variations est peu connu, il sera également nécessaire de les étudier dans un contexte global au niveau de la population humaine. La première partie du projet consiste en une analyse exhaustive des mutations de novo dans la partie codante (exome) de patients atteints de schizophrénie. Nous avons pu constater que non seulement le taux de mutations était plus élevé qu’attendu, mais nous avons également été en mesure de relever un nombre anormalement élevé de mutations non-sens, ce qui suggère un profil pathogénique. Ainsi, nous avons pu fortement suggérer que les mutations de novo sont des actrices importantes dans le mécanisme génétique de la schizophrénie. La deuxième partie du projet porte directement sur les gènes identifiés lors de la première partie. Nous avons séquencé ces gènes dans une plus grande cohorte de cas et de contrôles afin d’établir le profil des variations rares pour ces gènes. Nous avons ainsi conclu que l’ensemble des gènes identifiés par les études de mutations de novo possède un profil pathogénique, ce qui permet d’établir que la plupart de ces gènes ont un rôle réel dans la maladie et ne sont pas des artéfacts expérimentaux. De plus, nous avons pu établir une association directe avec quelques gènes qui montrent un profil aberrant de variations rares. La troisième partie du projet se concentre sur l’effet de l’âge paternel sur le taux de mutations de novo. En effet, pour la schizophrénie, il est démontré que l’âge du père est un facteur de risque important. Ainsi, nous avons tenté de caractériser l’effet de l’âge du père chez des patients en santé. Nous avons observé une grande corrélation entre l’âge du père et le taux de mutations germinales et nous avons ainsi pu répertorier certaines zones avec un grand nombre de mutations de novo, ce qui suggère l’existence de zone chaude pour les mutations. Nos résultats ont été parmi les premiers impliquant directement les mutations de novo dans le mécanisme génétique de la schizophrénie. Ils permettent de jeter un nouveau regard sur les réseaux biologiques à l’origine de la schizophrénie en mettant sous les projecteurs un type de variations génétiques longtemps laissé pour compte.

Plusieurs travaux suggèrent que les personnes atteintes d’une maladie mentale grave représentent un risque plus élevé de passage à l’acte violent que les personnes de la population en générale. Les raisons pour lesquelles ces personnes seraient plus enclines à commettre des gestes violents restent toutefois souvent contradictoires. En 1994, Link et Stueve suggéraient qu’une personne atteinte de symptômes de type menace/atteintes aux mécanismes de contrôle (TCO), était significativement plus à risque de commettre un geste violent. Les résultats de ces travaux furent par la suite reproduits dans le cadre d’autres études. L’objectif de ce rapport de stage est de mieux comprendre la relation entre les symptômes TCO et le passage à l’acte agressif en cours d’hospitalisation. Pour ce faire, les informations concernant la symptomatologie et les passages à l’acte violents de deux résidents de l’Institut Philippe-Pinel de Montréal (IPPM) sont présentées sous forme d’études de cas. L’analyse du contenu clinique des études de cas suggère qu’une approche nomothétique ne permet de comprendre qu’en partie l’association entre le passage à l’acte agressif et les symptômes TCO. Une telle approche ne permet pas d’intégrer d’autres facteurs cliniques tels les hallucinations ou les idées de violence, à l’étude du passage à l’acte agressif. Une approche phénoménologique où l’on s’intéresse à la compréhension qu’a la personne de son environnement et aux moyens qu’elle envisage afin d’y répondre semble offrir davantage de pistes afin de comprendre le passage à l’acte violent chez la personne atteinte d’une maladie mentale grave.
