Resultados tratamiento de las fracturas intertrocantéricas en el Hospital Universitario Mayor MEREDÍ

Las fracturas intertrocantericas son una importante causa de la morbimortalidad en los adultos mayores. Requieren la mayoria manejo quirurgico. No se ha logrado definir si es mejor el tratamiento con osteosintesis o artoplastia de cadera. Por esta razon decidimos realizar un estudio identificando los resultados en cada uno de los tratamientos con poblacion colombiana en el Hospital Universitario Mayor Mederi. Metodos: Estudio de serie de casos. Se analizó una cohorte retrospectiva de pacientes mayores de 59 años con fractura intertrocantérica en el Hospital Universitario Mayor Méderi. Resultados: Se reportaron un total de 179 pacientes con diagnóstico de fractura intertrocantérica. De los cuales se realizaron 100 osteosíntesis , 20 reemplazos totales de cadera y 59 hemiartroplastias. La mortalidad fueron 11 pacientes que corresponde al 6.1%, 3 fueron hombres y 8 mujeres, en cuanto al procedimiento realizado a 7 pacientes se les realizo osteosíntesis y a los 4 restantes se les realizo hemiartroplastia. En total 7 infecciones las cuales se presentaron respectivamente en 6 osteosíntesis y 1 hemiartroplastia. Discusión: La mortalidad fue mayor en la osteosíntesis con 7 pacientes que equivale al 63,6 % de la mortalidad total del estudio. Los porcentajes de infección postoperatoria fueron mayores en la osteosíntesis , encontrándose que del total de pacientes intervenidos 3,9% se infectaron y de estos el 85,7 % corresponden a osteosíntesis versus 14,3% de hemiartroplastia. El sangrado postoperatorio fue mayor a 500 cc en un 39% de las osteosíntesis y en un 44% en las hemiartroplastias. Conclusión: el tratamiento de las fracturas intertrocantéricas tuvo menor mortalidad y menor porcentaje de infección cuando los pacientes fueron tratados con hemiartroplastia y reemplazo total de cadera.

El mundo del software está cambiando. El desarrollo de Internet y las conexiones de datos hacen que las personas estén conectadas prácticamente en cualquier lugar. La madurez de determinadas tecnologías y el cambio del perfil de los usuarios de consumidor a generador de contenidos son algunos de los pilares de este cambio. Los Content Management Systems (CMS) son plataformas que proporcionan la base para poder generar webs colaborativas de forma sencilla y sin necesidad de tener excesivos conocimientos previos y son responsables de buena parte de este desarrollo. Una de las posibilidades que todavía no se han explotado suficientemente en estos sistemas es la georreferenciación de contenidos. De esta forma, aparece una nueva categoría de enlaces semánticos en base a las relaciones espaciales. En el actual estado de la técnica, se puede aprovechar la potencia de las bases de datos espaciales para manejar contenidos georreferenciados y sus relaciones espaciales, pero prácticamente ningún CMS lo aprovecha. Este proyecto se centra en desarrollar un módulo para el CMS Drupal que proporcione un soporte verdaderamente espacial y una interfaz gráfica en forma de mapa, mediante las que se puedan georreferenciar los contenidos. El módulo es independiente del proveedor de cartografía, ya que se utiliza la librería Open Source de abstracción de mapas IDELab Mapstraction Interactive. De esta forma se aúna la independencia tecnológica con la gestión verdaderamente espacial de los contenidos

Esta investigação teve como objeto de estudo o Ateliê Caderneta de Cromos, do projeto Geração Cool, fruto de uma parceria entre várias instituições do concelho de Almada. Trata--se de um projeto de desenvolvimento social comunitário, multicultural, associado a uma escola. Através de um estudo de caso, pretendeu analisar-se, criticamente, um modelo não formal de práticas ligadas às artes visuais, vocacionado para jovens em risco e mostrar como as aprendizagens desenvolvidas num ateliê de artes visuais contribuem para o processo de inclusão desses jovens. Desenvolveu-se um quadro teórico abrangente, no sentido de sustentar as perguntas iniciais, referenciando questões consideradas pertinentes tais como: visões contemporâneas das realidades multiculturais das periferias urbanas; perspetivas pós-modernistas de ensino artístico, defensoras de uma construção cognitiva; ação dos projetos artísticos de desenvolvimento social e ainda, a importância ética e social dos currículos artísticos atuais. Tendo como referência a hipótese de antagonismo e/ou complementaridade entre o ato pedagógico não formal e o institucional, o estudo procurou estabelecer uma relação entre essa prática e a inclusão, ao identificar e desmontar um roteiro estratégico de aprendizagens. O ato pedagógico no Ateliê mostrou potenciar uma aprendizagem construtiva nas respostas produzidas, sendo também significativa pelo caráter experiencial vivido e cognitiva no sentido em que determina a construção de um significado, assumido como a própria assunção identitária.

The definitions of the base units of the international system of units have been revised many times since the idea of such an international system was first conceived at the time of the French revolution. The objective today is to define all our units in terms of 'invariants of nature', i.e. by referencing our units to the fundamental constants of physics, or the properties of atoms, rather than the characteristics of our planet or of artefacts. This situation is reviewed, particularly in regard to finding a new definition of the kilogram to replace its present definition in terms of a prototype material artefact.

A novel framework referred to as collaterally confirmed labelling (CCL) is proposed, aiming at localising the visual semantics to regions of interest in images with textual keywords. Both the primary image and collateral textual modalities are exploited in a mutually co-referencing and complementary fashion. The collateral content and context-based knowledge is used to bias the mapping from the low-level region-based visual primitives to the high-level visual concepts defined in a visual vocabulary. We introduce the notion of collateral context, which is represented as a co-occurrence matrix of the visual keywords. A collaborative mapping scheme is devised using statistical methods like Gaussian distribution or Euclidean distance together with collateral content and context-driven inference mechanism. We introduce a novel high-level visual content descriptor that is devised for performing semantic-based image classification and retrieval. The proposed image feature vector model is fundamentally underpinned by the CCL framework. Two different high-level image feature vector models are developed based on the CCL labelling of results for the purposes of image data clustering and retrieval, respectively. A subset of the Corel image collection has been used for evaluating our proposed method. The experimental results to-date already indicate that the proposed semantic-based visual content descriptors outperform both traditional visual and textual image feature models. Commisssioned by Frieze Art for the Frieze Sculpture Park The project presents the image of a sculpture as a sculpture, installed in the form of a large scale digital print on vinyl stretched over a 14 x 28ft (4.2 x 8.4m) stretcher supported by a scaffolding structure. The image itself depicts a futuristic public sculpture, an ‘impossible’ artwork, referencing Ballard’s descriptions in his book ‘Vermillion Sands’. The work also draws upon examples of rococo ornamentation and the compositional conventions of ‘images of sculpture’ (in art magazines, catalogues, publicity photos) including examples sited in Regents park in previous years. Technical details: The image is printed on vinyl, stretched over a 14 x 28ft (4.2 x 8.4m) wooden stretcher and fixed to a deep buttressed scaffold 8m long by 6.23 deep with IBC water tanks on the back edge as kentledge (4 x I tonne IVC water containers - 1 per bay). The structure is constructed from clean silver Layher system scaffold and wrapped by a dense black mesh netting.

Project includes: a large scale live performance and resulting performance video, at Curtain Razors, Regina Queen’s Square, Regina, 2008 Live Performance, 45 mins, incl. 1 actor, 23 extras, 2 live cameras, live video and sound mixing, stage set, video projection. Video 45 mins Video Trailer 7 mins The Extras is a video performance referencing the form of a large live film shoot. The Extras contextualises contemporary Westerns genres within an experimental live tableau. The live performance and resulting 45 mins video make reference 19th century Western Author German Karl May, the tradition of Eastern European Western, (Red Western), Uranium exploitation and entrepreneurial cultures in the Canadian Prairies. Funded by the Canada Council for the Arts, Saskatchewan Arts Board and Curtain Razors, the Extras Regina was staged and performed at Central Plaza in Regina, with a crew of 23 extras, 2 live cameras, live video and sound mixing ad video projection. It involved research in Saskatchewan film and photographic archives. The performance was edited live and mixed with video material which was shot on location, with a further group of extras at historical historical ‘Western’ locations, including Fort Qu' Appelle, Castle Butte and Big Muddy. It also involved a collaboration with a local theatre production company, which enacted a dramatised historical incident.

For its advocates, corporate social responsibility (CSR) represents a powerful tool through which business and particularly multinationals can play a more direct role in global sustainable development. For its critics, however, CSR rarely goes beyond business as usual, and is often a cover for business practices with negative implications for communities and the environment. This paper explores the relationship between CSR and sustainable development in the context of mining in Namibia. Drawing upon extant literatures on the geographies of responsibility, and referencing in-country empirical case-study research, a critical relational lens is applied to consider their interaction both historically and in the present.

Briefing phase interactions between clients and designers are recognized as social engagements, characterized by communicative sign use, where conceptual ideas are gradually transformed into potential design solutions. A semiotic analysis of briefing communications between client stakeholders and designers provides evidence of the significance and importance of stakeholder interpretation and understanding of design, empirical data being drawn from a qualitative study of NHS hospital construction projects in the UK. It is contended that stakeholders engage with a project through communicative signs and artefacts of design, referencing personal cognitive knowledge in acts of interpretation that may be different from those of designers and externally appointed client advisers. Such interpretations occur in addition to NHS client and design team efforts to ‘engage’ with and ‘understand’ stakeholders using a variety of methods. Social semiotic theorizations indicate how narrative strategies motivate the formulation of signs and artefacts in briefing work, the role of sign authors and sign readers being elucidated as a result. Findings are contextualized against current understandings of briefing communications and stakeholder management practices, a more socially attuned understanding of briefing countering some of the process-led improvement models that have characterized much of the post-Egan report literature. A stakeholder interpretation model is presented as one potential method to safeguard against unforeseen interpretations occurring, the model aligning with the proposal for a more measured recognition of how designs can trigger interpretations among client stakeholders.

Purpose When consumers buy online, they are often confronted with consumer reviews. A negative consumer review on an online shopping website may keep consumers from buying the product. Therefore, negative online consumer reviews are a serious problem for brands. This paper aims to investigate the effects of different response options to a negative consumer review. Design/methodology/approach In an online experiment of 446 participants different response options towards a negative consumer review on an online shopping website are examined. The experimental data is analysed with simple linear regression models using product purchase intentions as the outcome variable. Findings The results indicate that a positive customer review counteracts a negative consumer review more effectively than a positive brand response, whereas brand strength moderates this relationship. Including a reference to an independent, trusted source in a brand or a customer response is only a limited strategy for increasing the effectiveness of a response. Research limitations/implications Additional research in other product categories and with other subjects than students is suggested to validate the findings. In future research, multiple degrees of the phrasing’s strength of the reference could be used. Practical implications Assuming high quality products, brands should encourage their customers to write reviews. Strong brands can also reassure consumers by responding whereas weak brands cannot. Originality/value This research contributes to the online consumer reviews literature with new insights about the role of brand strength and referencing to an independent, trusted source.

Os efeitos da estimulação ultra-sônica sobre a consolidação óssea têm sido demonstrados por trabalhos experimentais e clínicos. Este estudo teve por objetivo investigar a aplicação clínica do ultra-som pulsado de baixa intensidade como tratamento adjuvante de fraturas diafisárias em cães. Foram utilizados 16 cães de raças variadas, com faixa etária entre sete meses e seis anos, peso corpóreo entre 2,5 e 43kg, portadores de fraturas diafisárias fechadas recentes localizadas no rádio e ulna, fêmur ou tíbia e fíbula, estabilizadas por procedimentos de osteossíntese (fixação esquelética externa, pinos intramedulares ou a associação desses métodos). Os cães foram divididos em dois grupos: fraturas estabilizadas tratadas por ultra-som de baixa intensidade (grupo tratado, n=8); fraturas estabilizadas, não tratadas por estimulação ultra-sônica, (grupo controle, n=8). Os animais foram avaliados por exames clínicos e radiográficos nos períodos pré-operatório, pós-operatório imediato e a cada 30 dias posteriores aos procedimentos cirúrgicos. Realizou-se tratamento com ultra-som pulsado (sinal senoidal com freqüência de 1,5MHz, largura de pulso de 200µs e freqüência de repetição de 1kHz) de baixa intensidade (30mW cm-2), aplicado de modo estacionário no foco de fratura. A terapia ultra-sônica foi realizada 20 minutos por dia, durante 21 dias consecutivos, a partir do período compreendido entre o 1° e o 9° dia pós-operatório. O teste t de Student, empregado na análise estatística, mostrou diferença significante (P<0,001 e alfa=0,05) entre as médias dos parâmetros de tempo para consolidação óssea observadas nos animais dos grupos tratado (média de 67,5 dias) e controle (média de 106 dias). Este protocolo de estimulação ultra-sônica promoveu sinais clínicos e radiográficos acelerados da consolidação óssea nas fraturas tratadas. Os resultados deste estudo sugerem que o ultra-som pulsado de baixa intensidade pode ser indicado como terapia adjuvante de fraturas diafisárias recentes em cães.

The study aims identify the existence of quality culture in Brazilian automotive dealerships with ISO 9001, motivated by this research problem: exist a quality culture in this dealerships, which facilitates the adoption of quality practices supported by ISO 9001? For referencing, the theoretical review was written in five themes: organizational culture, quality culture, total quality management, ISO 9001 quality management system and the Brazilian automobile industry. As regards the methodological aspects, the research has an applied nature, with a quantitative approach, being exploratory in their objectives, and bibliographic, documental and survey as technical procedures. The organizations participating in the study were all Brazilian automotive dealerships certified with ISO 9001. The research intended cover all the 80 active dealers with ISO 9001 certification identified by the Brazilian Committee for Quality (ABNT CB-25). The survey recorded participation of 32 companies (response rate 40%). The questionnaire was sent to seller managers, formatted into five sections: 1) introductory message 2) manager profile, 3) reasons for implementation and benefits generated by ISO 4) adoption levels of quality practices and 5) diagnosis of organizational culture. The questions contained in sections 2 and 3 were structured in multiple choice, and in the remaining sections were structured in Likert 5-point scale. The statistical method used (data analysis), was the descriptive statistics, for data representation in frequency percentage (FP) and standard level (SL). The results showed that the interviewed dealerships have an organizational culture with very high levels of prevalence in "outcome orientation" and "attention to detail" cultural dimensions. In addition, about the other two dimensions considered conducive to quality (innovation and teamwork/respect for people), both observed high prevalence. Based on the present results, concluded that the organizational culture of Brazilian dealerships with ISO 9001 are quality oriented, being conducive to adoption of quality practices supported by TQM Systems. However, it is important to mention that the quality culture identified is not sufficiently developed to adopt quality practices at optimal levels, which sets up an unfavorable scenario to deals with highly rigorous customer

The objective of this paper is to analyze how the female figure was portrayed in discursive narratives in the bi-weekly newspaper O PORVIR , which circulated in Currais Novos, in the state of Rio Grande do Norte, Brazil, between May 2, 1926 and January 20, 1929. Our analysis was based on the semantics of the text, by applying one of the primary concepts used in Textual Analysis of Speeches, Discursive Representation (ADAM, 2011; RODRIGUES; PASSEGGI; SILVA NETO, 2010). Textual Analysis of Speeches has its roots in Textual Linguistics and consists of a theoretical and descriptive approach (ADAM, 2011). This research is characterized as documentary (SEVERINO, 2007) and its corpus includes a review of 39 editions of the aforementioned newspaper, of which we extracted 292 articles that dealt with the female figure and 396 statements for analysis. We used the following semantic categories: referencing (CASTILHO, 2010; NEVES, 2007; KOCH; MARCUSCHI, 1998; MARCUSCHI, 2008; KOCH, 2009); predication (NEVES, 2007; RODRIGUES; PASSEGGI; SILVA NETO, 2010); modification (ADAM, 2011); and spacial and temporal localization (RODRIGUES; PASSEGGI; SILVA NETO, 2010). Our analysis revealed that lady, mother, wife and homemaker were most represented, and always combined with questions about motherhood, marriage and devotion to the home. Such representation does not diverge from pre-colonial models in the early twentieth century, which served as a standard for the female figure
