Introduction: physical exercise has been recommended as a non-pharmacological, therapeutic strategy in the treatment of important cardiovascular risk factors. Objective: to analyze the impact of an exercise program, tailored to the reality of the Family Health Units (FHU), on body composition, cardiovascular risk factors and Framingham score in obese postmenopausal. Methods: 70 women between 50 and 79 years, sedentary, obese and without menstruating for at least twelve months, were randomly assigned to a trained group (TG) (n = 35) and an untrained (GnT) (n = 35). The GT took 20 weeks of a physical exercise program with three weekly sessions, consisting of monitoring activities and heating (10 minutes), 25 minutes of exercise flexibility and strength, 50 minute walk with intensity between 50-65% of VO2max and 5-minute cool-down. The GnT was instructed to maintain their normal activities. Results: TG showed significant reductions in body mass index (30,1+3,7 vs. 29,3+3,7; p=0,0001), waist circumference (93,3+10,3 vs. 89,1+10,4; p=0,0001), percentage of fat (54,2+2,9 vs. 53,2+3,3; p=0,0001), systolic blood pressure (128,0+14,6 vs. 119,2+10,3; p=0,0001), triglycerides (148,4+66,1 vs. 122,8+40,7; p=0,006), VLDL cholesterol (29,7+13,2 vs. 24,5+8,0; p=0,005) and Framingham score (13,08+4,0 vs. 11,77+4,1; p=0,010). In the untrained group were observed significant increases in the percentage of fat (55,0+4,0 vs. 57,0+3,8; p=0,0001), systolic blood pressure (128,6+10,5 vs. 133,7+12,0; p=0,001), fasting glucose (95,2+18,4 vs. 113,7+28,8; p=0,001) and Framingham score (12,82+3,2 vs. 13,91+4,0; p=0,043), but also decreases levels of HDL cholesterol (55,1+10,5 vs. 51,7+11,0; p=0,017). Conclusion: the exercise program, adapted to the conditions of FHU, was effective in reducing cardiovascular risk factors in obese postmenopausal women served by the SUS program.

Although the regular practice of physical exercise for the maintenance of the nutritional state and quality of life is important, it is not a common habit among the elderly, especially those of the lower income social bracket. The objective was to characterize and compare according to social-economic conditions and anthropometric indexes of the nutritional state of the elderly who regularly practice and those who do not practice exercises, offered at no cost, in the Northern zone of São José do Rio Preto-SP. In this study, 110 elderly women participated, of which 60 (group P) exercised regularly and 50 did not (group NP). Group P participated in a special gym program, free of charge, for at least 2 years, 2 to 3 times a week, one hour a day. Group NP was recruited from the same location as Group P. The comparison between the groups of elderly women was proportionately distributed according to marital status, level of education, means of locomotion, results of body mass index (IMC), waist to hip ratio (RCQ) and calf circumference, using the chi-square test. Comparisons were also made of average age, per capita family income, individual income, IMC, arm and triceps skin fold measured by t-Test. Differences were not found in all the comparisons made. The variables that stood out were the IMC (overweight) and RCQ (risk of chronic diseases) results, which were above what was expected, and the high frequency of walking and riding buses for both groups of elderly women (about 60%). We came to the conclusion that the low-intensity exercise program evaluated made no difference in the nutritional profi le of the elderly who also used walking as a means of getting around.

Model Study: An experimental study Introduction: Chronic Kidney Disease (CKD) refers to a syndromic diagnosis which leads to a progressive and irreversible loss of renal function. A hemodialysis patient may have limitations in functional capacity, pulmonary function and respiratory musclular strength impacting in quality of life. Objective: To evaluate the effects of an exercise program on pulmonary function, functional capacity, quality of life and pain in patients undergoing hemodialysis. Methodology: The study included 28 patients of both genders, women and men aged between 40 and 60 years undergoing dialysis at the Kidney Institute, Santa Casa de Misericordia in Presidente Prudente-SP. Primary outcomes included respiratory muscular strength measurements assessed by manovacuometry. The functional capacity was evaluated by a six minute walking test. A life quality questionnaire was applied to evaluate quality of life (SF36-KDQOL). Lung function was evaluated by spirometry. Pain was assessed by a visual analogue scale. The exercise program consisted of training 3 times a week for 40 minutes on hemodialysis during eight weeks. At the end of the program all patients were reassessed. Results: There was no significant difference in the values of FVC and FEV1 before and after the exercise program as well as the index Tiffenau. The value of post MIP was significantly higher than the value obtained in the pre program. For variable MEP no significant difference was found. Functional capacity evaluations showed that there were no significant differences (p> 0.05). The evaluation of quality of life, about the domains of specific areas of CKD showed statistical significance when comparing the list of symptoms and problems with overloading of renal disease and professional role. Indicators related to pain were significantly reduced after the program (P <0.05). Discussion: A chronic kidney patient faces complex situations of physical, social and financial aspects. Although no statistically significant results were found in all variables, the study corroborates to others found in the literature, which suggests that an exercise program can be positive for this population. Conclusion: Although lung capacity and functional capacity did not submit changes to the end of the study, reduced levels of pain, fatigue and dyspnea suggest improvement in functional performance after exercise programs.

Background: Walking speed seems to be related to aerobic capacity, lower limb strength, and functional mobility, however it is not clear whether there is a direct relationship between improvement in muscle strength and gait performance in early postmenopausal women. Objective: To evaluate the effect of muscle strengthening exercises on the performance of the 6-minute walk test in women within 5 years of menopause. Methods: The women were randomized into control group (n=31), which performed no exercise, and exercise group (n=27), which performed muscle strengthening exercises. The exercises were performed twice a week for 3 months. The exercise protocol consisted of warm-up, stretching, and strengthening of the quadriceps, hamstring, calf, tibialis anterior, gluteus maximus, and abdominal muscles, followed by relaxation. Muscular strength training started with 60% of 1MR (2 series of 10-15 repetitions), reaching 85% until the end of the 3-month period (4 series of 6 repetitions each). Results: The between-group comparisons pre- and post-intervention did not show any difference in distance walked, heart rate or blood pressure (p>0.05), but showed differences in muscle strength post-intervention, with the exercise group showing greater strength (p<0.05). In the within-group comparison, there were differences in final heart rate and quadriceps and hamstring strength pre- and post-intervention in the exercise group (p<0.05). Conclusion: The results suggest that muscle strengthening of the lower limbs did not improve performance in the 6-minute walk test in this population of postmenopausal women. Context: Liposuction is suggested to result in long-term body fat regain that could lead to increased cardiometabolic risk. We hypothesized that physical activity could prevent this effect. Objective: Our objective was to investigate the effects of liposuction on body fat distribution and cardiometabolic risk factors in women who were either exercise trained or not after surgery. Design, Setting, and Participants: Thirty-six healthy normal-weight women participated in this 6-month randomized controlled trial at the University of Sao Paulo, Sao Paulo, Brazil. Interventions: Patients underwent a small-volume abdominal liposuction. Two months after surgery, the subjects were randomly allocated into two groups: trained (TR, n = 18, 4-month exercise program) and nontrained (NT, n = 18). Main Outcome Measures: Body fat distribution (assessed by computed tomography) was assessed before the intervention (PRE) and 2 months (POST2), and 6 months (POST6) after surgery. Secondary outcome measures included body composition, metabolic parameters and dietary intake, assessed at PRE, POST2, and POST6, and total energy expenditure, physical capacity, and sc adipocyte size and lipid metabolism-related gene expression, assessed at PRE and POST6. Results: Liposuction was effective in reducing sc abdominal fat (PRE vs. POST2, P = 0.0001). Despite the sustained sc abdominal fat decrement at POST6 (P = 0.0001), the NT group showed a significant 10% increase in visceral fat from PRE to POST6 (P = 0.04; effect size = -0.72) and decreased energy expenditure (P = 0.01; effect size = 0.95) when compared with TR. Dietary intake, adipocyte size, and gene expression were unchanged over time. Conclusion: Abdominal liposuction does not induce regrowth of fat, but it does trigger a compensatory increase of visceral fat, which is effectively counteracted by physical activity. En un ejercicio no extenuante la frecuencia cardíaca (FC) guarda una relación lineal con el consumo máximo de oxígeno (V O2max) y se suele usar como uno de los parámetros de referencia para cuantificar la capacidad del sistema cardiovascular. Normalmente la frecuencia cardíaca puede remplazar el porcentaje de V O2max en las prescripciones básicas de ejercicio para la mejora de la resistencia aeróbica. Para obtener los mejores resultados en la mejora de la resistencia aeróbica, el entrenamiento de los individuos se debe hacer a una frecuencia cardíaca suficientemente alta, para que el trabajo sea de predominio dinámico con la fosforilación oxidativa como fuente energética primaria, pero no tan elevada que pueda suponer un riesgo de infarto de miocardio para el sujeto que se está entrenando. Los programas de entrenamiento de base mínima y de base óptima, con ejercicios de estiramientos para prevenir lesiones, son algunos de los programas más adecuados para el entrenamiento de la resistencia aeróbica porque maximizan los beneficios y minimizan los riesgos para el sistema cardiovascular durante las sesiones de entrenamiento. En esta tesis, se ha definido un modelo funcional para sistemas de inteligencia ambiental capaz de monitorizar, evaluar y entrenar las cualidades físicas que ha sido validado cuando la cualidad física es la resistencia aeróbica. El modelo se ha implementado en una aplicación Android utilizando la camiseta inteligente “GOW running” de la empresa Weartech. El sistema se ha comparado en el Laboratorio de Fisiología del Esfuerzo (LFE) de la Universidad Politécnica de Madrid (UPM) durante la realización de pruebas de esfuerzo. Además se ha evaluado un sistema de guiado con voz para los entrenamientos de base mínima y de base óptima. También el desarrollo del software ha sido validado. Con el uso de cuestionarios sobre las experiencias de los usuarios utilizando la aplicación se ha evaluado el atractivo de la misma. Por otro lado se ha definido una nueva metodología y nuevos tipos de cuestionarios diseñados para evaluar la utilidad que los usuarios asignan al uso de un sistema de guiado por voz. Los resultados obtenidos confirman la validez del modelo. Se ha obtenido una alta concordancia entre las medidas de FC hecha por la aplicación Android y el LFE. También ha resultado que los métodos de estimación del VO2max de los dos sistemas pueden ser intercambiables. Todos los usuarios que utilizaron el sistema de guiado por voz para entrenamientos de 10 base mínima y de base óptimas de la resistencia aeróbica consiguieron llevar a cabo las sesiones de entrenamientos con un 95% de éxito considerando unos márgenes de error de un 10% de la frecuencia cardíaca máxima teórica. La aplicación fue atractiva para los usuarios y hubo también una aceptación del sistema de guiado por voz. Se ha obtenido una evaluación psicológica positiva de la satisfacción de los usuarios que interactuaron con el sistema. En conclusión, se ha demostrado que es posible desarrollar sistemas de Inteligencia Ambiental en dispositivos móviles para la mejora de la salud. El modelo definido en la tesis es el primero modelo funcional teórico de referencia para el desarrollo de este tipo de aplicaciones. Posteriores estudios se realizarán con el objetivo de extender dicho modelo para las demás cualidades físicas que suponen modelos fisiológicos más complejos como por ejemplo la flexibilidad. Abstract In a non-strenuous exercise, the heart rate (HR) shows a linear relationship with the maximum volume of oxygen consumption (V O2max) and serves as an indicator of performance of the cardiovascular system. The heart rate replaces the %V O2max in exercise program prescription to improve aerobic endurance. In order to achieve an optimal effect during endurance training, the athlete needs to work out at a heart rate high enough to trigger the aerobic metabolism, while avoiding the high heart rates that bring along significant risks of myocardial infarction. The minimal and optimal base training programs, followed by stretching exercises to prevent injuries, are adequate programs to maximize benefits and minimize health risks for the cardiovascular system during single session training. In this thesis, we have defined an ambient intelligence system functional model that monitors, evaluates and trains physical qualities, and it has been validated for aerobic endurance. It is based on the Android System and the “GOW Running” smart shirt. The system has been evaluated during functional assessment stress testing of aerobic endurance in the Stress Physiology Laboratory (SPL) of the Technical University of Madrid (UPM). Furthermore, a voice system, designed to guide the user through minimal and optimal base training programs, has been evaluated. Also the software development has been evaluated. By means of user experience questionnaires, we have rated the attractiveness of the android application. Moreover, we have defined a methodology and a new kind of questionnaires in order to assess the user experience with the audio exercise guide system. The results obtained confirm the model. We have a high similarity between HR measurements made of our system and the one used by SPL. We have also a high correlation between the VO2max estimations of our system and the SPL system. All users, that tried the voice guidance system for minimal and optimal base training programs, were able to perform the 95% of the training session with an error lower than the 10% of theoretical maximum heart rate. The application appeared attractive to the users, and it has also been proven that the voice guidance system was useful. As result we obtained a positive evaluation of the users' satisfaction while they interacted with the system. In conclusion, it has been demonstrated that is possible to develop mobile Ambient Intelligence applications for the improvement of healthy lifestyle. AmIRTEM model is the first theoretical reference functional model for the design of this kind of applications. Further studies will be realized in order to extend the AmIRTEM model to other physical qualities whose physiological models are more complex than the aerobic endurance.

Introducción. La prevalencia de la Diabetes Gestacional (DG) varía en todo el mundo, así como entre los grupos raciales y étnicos del mismo país. Hasta la fecha actual, no se ha llegado a un consenso en el criterio diagnóstico, y eso dificulta una estimación veraz de prevalencia entre países. A pesar de ello, es ineludible obviar el incremento en la incidencia de esta complicación en todo el mundo, y la trascendencia de sus riesgos a la salud pública. En España, según los criterios clásicos –del National Diabetes Data Group- existe una alta prevalencia en un 8,8 % de DG en gestantes. Es importante encontrar la mejor vía para la prevención de la DG y, uno de los factores de riesgo parece ser el aumento excesivo de peso durante el embarazo. El ejercicio es un elemento fundamental para el control del metabolismo de la glucosa y para reducir los niveles de hiperlipidemia. Sin embargo, existe controversia para definir el tipo de sesiones, duración e intensidad que puedan contribuir a su prevención. Objetivo. Conocer en qué medida el ejercicio físico programado durante el embarazo, combinado en agua y tierra, con ejercicios aeróbicos y de tonificación muscular, puede actuar como un factor de prevención de la DG. Al mismo tiempo, valorar si exceder las recomendaciones de peso puede influir el diagnóstico de la DG. Material y Métodos. Este estudio se desarrolló mediante una colaboración entre la Universidad Politécnica de Madrid y los Servicios de Ginecología y Obstetricia del Hospital Universitario de Puerta de Hierro, el Hospital Universitario de Torrelodones y el Centro de Salud de Torrelodones. Se obtuvo la aprobación del Comité Ético de Investigación Clínica (CEIC). Se realizó un ensayo clínico, aleatorizado, controlado, no enmascarado. 272 mujeres gestantes sanas dieron su consentimiento informado para la inclusión en el estudio. De las cuáles, finalmente 257 (edad= 33,2±4,4 años) fueron analizadas, 101 de ellas correspondientes al grupo intervención (GI, n=101) y 156 al grupo control (GC, n=156). El inicio del programa correspondió a la semana 10-14 del embarazo hasta el final, la 38-40. Con una frecuencia de 3 sesiones semanales y una duración de 60 y 50 minutos, en tierra y agua, respectivamente. Resultados. Se halló diferencias significativas en los valores en los 180 min del test de tolerancia oral a la glucosa [GI: 98,00±29,48 mg/dl vs. GC: 116,25±29,90 mg/dl (t64= 2,37; p= 0,021)] y, de igual modo, el GI mostró menor prevalencia de la DG [GI: 1 %, Ejercicio y DG n= 1 vs. GC: 8,8 %, n= 13 (2 1= 6,84; p= 0,009)] y una estimación de riesgo significativa (OR= 9,604; 95 % CI: 1,23-74,66). La excesiva ganancia de peso fue menor en el GI [GI: 22,8 %, n= 23 vs. GC: 34,9 %, n= 53 (2 1= 4,22; p= 0,040)], pero no existió una correlación con la incidencia de DG (ϕ= -0,007; p= 0,910). Conclusiones. El programa de ejercicio desarrollado durante el embarazo mostró efectividad en la reducción de la prevalencia de la DG, preservó la tolerancia a la glucosa y redujo la excesiva ganancia de peso materno. Background. The prevalence of Gestational Diabetes (GD) varies around the world, as well as between racial and ethnic groups within the same country. Currently, there is not a consensus about the diagnostic criteria, and that makes it difficult to obtain accurate estimates of prevalence between countries. The increased trend in the prevalence across the globe and the risks for public health cannot be ignored. In Spain, according to the diagnostic criteria of National Diabetes Data Group, there is a prevalence of 8.8 % for GD in pregnant women. It is important to look for the best way to prevent GD and one of the risk factors seems to be excessive weight gained during pregnancy. Exercise is an essential element for glucose metabolic control and reducing hyperlipidemia levels. However, there is controversy to define the type of activity, duration and intensity to prevent GD. Objective. To assess the effectiveness of an exercise programme carried out during pregnancy (land/aquatic activities), both aerobic and muscular conditioning can help to the prevent GD. Also, to assess if excessive maternal weight gain influences the GD diagnosis. Material and methods. Collaboration between the Technical University of Madrid and the Gynecology and Obstetrics Department of Puerta de Hierro University Hospital, Torrelodones University Hospital and Health Center of Torrelodones supported the study. It was approved by the Clinical Research Ethics Committee (CEIC). A clinical, randomized controlled trial recruited 272 pregnant women without obstetric contraindications and gave informed consent for inclusion in the study. Of these women, 257 were studied (age= 33,2±4,4 years), 101 in intervention group (IG, n= 101) and 156 in control group (CG, n= 156). A physical exercise program three times per week during pregnancy was developed. The duration of the sessions was 60 minutes and 50 minutes in land and water, respectively. Results. The IG showed lower maternal values in the Oral Glucose Tolerance Test (OGTT) at 180 minutes [IG: 98,00±29,48 mg/dl vs. CG: 116,25±29,90 mg/dl (t64= 2,37; p= 0,021)] and the IG reduced the prevalence of GD [IG: 1%, n= 1 vs. CG: 8,8 %, n= 13 (2 1= 6,84; p= 0,009)] with a significance risk estimate (OR= 9,604; 95 % CI: 1,23- 74,66). Excessive maternal weight gain was less in the IG [IG: 22,8 %, n= 23 vs. CG: Exercise and GD 34,9 %, n= 53 (2 1= 4,22; p= 0,040)] but there was no correlation with the prevalence of GD (ϕ= -0,007; p= 0,910). Conclusions. The exercise programme performed during pregnancy reduced the prevalence of GD, preserved glucose tolerance and reduced excessive maternal weight gain.

Introducción. Las enfermedades cardiovasculares (ECV) son la principal causa de muerte en nuestro país. Entre los factores independientes más importantes para el desarrollo de ECV se encuentran en primer lugar las alteraciones del perfil lipídico como el aumento del colesterol total (TC), las lipoproteínas de baja densidad (LDL) y los triglicéridos (TG) y/o la disminución de las lipoproteínas de alta densidad (HDL). Entre las diferentes formas de abordar el problema para prevenir y tratar estas patologías se encuentra la modificación de los hábitos de vida a través de un programa combinado de dieta y ejercicio. La evidencia confirma la efectividad de la primera variable, sin embargo, en el empleo del ejercicio se encuentran discrepancias acerca de cuál es el modo más eficaz para mejorar el perfil lipídico. Objetivo. Estudiar los cambios en las variables del perfil lipídico y los índices lipoproteicos comparando cuatro tipos de intervención que combinan dieta y diferentes modos de ejercicio, así como, analizar otras variables independientes (género, edad y genotipo ApoE) que pueden tener influencia sobre estos cambios. Diseño de la investigación. Los datos analizados en esta tesis forman parte del estudio “PROgramas de Nutrición y Actividad Física para el tratamiento de la obesidad” (PRONAF). Se trata de un estudio clínico desarrollado en España entre el 2008 y el 2011. La metodología del estudio nos permite comparar cuatro tipos de intervención para la pérdida de peso y evaluar su impacto sobre el perfil lipídico. El diseño fue experimental aleatorizado donde a todos los participantes se les sometió a un programa de dieta equilibrada hipocalórica junto a uno de los tres modos de ejercicio (grupo de entrenamiento de fuerza, grupo de entrenamiento de resistencia y grupo de entrenamiento combinado de los modos anteriores; los cuales fueron igualados en volumen e intensidad) o grupo de recomendaciones de actividad física. Las principales variables analizadas en los estudios que comprende esta tesis doctoral fueron: HDL, LDL, TG y TC, los índices derivados de estas y variables de la composición corporal y del entrenamiento. Conclusiones. Los cuatro tipos de intervención mostraron ser favorables para mejorar las variables del perfil lipídico y los índices lipoproteicos, sin diferencias significativas entre ellos. Tras la intervención, los varones mostraron una respuesta más favorable en los cambios del perfil lipídico. El genotipo ApoE2 obtuvo una reducción mayor en la concentración de TG y TC que el genotipo ApoE3 y ApoE4. Por último, los índices lipoproteicos mejoraron tras un programa de pérdida de peso, obteniéndose mayores cambios en el grupo de dieta más entrenamiento aeróbico para los índices ApoB/ApoA-1, TG/HDL y LDL/ApoB. ABSTRACT Introduction. The main cause of death in our country is cardiovascular disease (CVD). The most important independent factors for the development of CVD are the lipid profile alterations: increased total cholesterol (TC), low density lipoprotein (LDL) and triglycerides (TG) and/or decreased high-density lipoprotein (HDL). Among the different approaches to prevent and treat these diseases is modifying the lifestyle combining a diet and exercise program. The evidence confirms the effectiveness of the first variable, however, there is still controversy about the most effective mode of exercise combined with diet to achieve improvements. Objective. To study changes in lipoprotein profile comparing four types of intervention combining diet with different modes of exercise, and to analyze the independent variables (gender, age, and ApoE genotype) that can influence these changes. Research design. The data analized in this thesis are part of the study Nutrition and Physical Activity Programs for Obesity Treatments (the PRONAF study according to its Spanish initials). This is a clinical research carried out in Spain between 2008 and 2011. The aim of this study was to compare four types of intervention to weight loss with diet combining exercise. The design was experimental randomized where all participants were subjected to follow a hypocaloric balanced diet along one of the three modes of exercise (strength training group, resistance training group and combined training group of the above modes, which were matched by volume and intensity) or physical activity recommendations group. The main variables under investigation in this thesis were: HDL, LDL, TG and TC, the lipoprotein ratios, body composition and training variables. Main outcomes. The four types of interventions shown to be favorable to improve the lipid profile and lipoprotein level, with no significant differences between intervention groups. After the intervention, the men showed a more favorable respond in lipid profile changes. The genotype ApoE2 obtained more positive changes in the concentration of TG and TC than ApoE3 and ApoE4 genotype. Last, the lipoprotein ratios improve after weight loss treatment with diet combined different modes exercise. Our results reflected greater changes for E group in apoB/ApoA1, TG/HDL and LDL/ApoB compared within different intervention groups.

La práctica de actividad física y del deporte ha dado muestras indiscutibles sobre su relación con un buen estado de salud, tanto mental como corporal. Sin embargo, también entraña riesgos que pueden mermar la calidad de vida del practicante, siendo uno de los más comunes e importantes el riesgo de sufrir una lesión deportiva. Actualmente, se han dado respuesta a los factores de riesgo internos y externos que pueden afectar a que un deportista, tanto a nivel profesional como recreativo, pueda sufrir una lesión, pero pocos estudios se han realizado para evaluar la incidencia, la prevalencia y la severidad de las lesiones, en personas sedentarias con sobrepeso y obesidad que deciden comenzar a entrenar y/o a realizar actividad física. Incluso en aquellos casos que por razones clínicas, deben incluir el ejercicio físico como una parte integral del tratamiento terapéutico. Por lo tanto, el objetivo del estudio fue analizar la incidencia, prevalencia y severidad de las lesiones deportivas registradas en los protocolos de entrenamiento desarrollados durante el proyecto de Nutrición y Actividad Física para el tratamiento del sobrepeso y la obesidad (PRONAF). Se realizó un análisis descriptivo y se evaluó el efecto y el grado de asociación entre la incidencia de lesión y la composición corporal, el sexo o el protocolo de entrenamiento. Por otro lado, se observó la localización anatómica más común de las lesiones en cada una de las fases. Por último, se propuso una ecuación para estimar el riesgo de lesión en sujetos en sobrepeso y obesidad, antes de comenzar un programa de ejercicio controlado. Un total de 167 adultos con sobrepeso y obesidad sanos (77 hombres, 90 mujeres) fueron evaluados durante el proyecto mediante un registro de lesiones previamente validado. Los principales resultados de este trabajo mostraron que un 47,3% de los participantes sufrió, al menos, una lesión o dolencia durante el programa de intervención, observando una incidencia de 1,22 lesiones por cada 100 horas de entrenamiento. No obstante, se observó que el 80,2% de las lesiones fueron menores (de 1 a 7 días). La rodilla fue la localización que mayor proporción de lesiones registró durante la intervención. Por otro lado, no se observaron asociaciones significativas entre la fase de sobrepeso y la fase de obesidad, ni entre ser hombre o mujer y sufrir más lesiones durante la intervención. Sin embargo, los protocolos de entrenamiento que incluyen cargas, mostraron hasta cinco veces más riesgo de sufrir una lesión que el protocolo que no las incluyó. Por último, se propuso una ecuación para predecir la influencia de las variables fase, lesión previa, protocolo de entrenamiento, sexo y edad para predecir el riesgo de lesión en una población con sobrepeso y obesidad que incluya los protocolos de entrenamiento propuestos en el proyecto PRONAF. ABSTRACT The practice of physical activity and sport has indisputable signs on his relationship with a good health, both mental and physical. However, it is true that also risks that may impair the quality of life of the practitioner, one of the most common and important risk of sports injury. However, although they have responded to factors internal and external risks that may affect an athlete, both professional and recreational may suffer an injury, few studies have been conducted to assess the incidence and prevalence of injuries in sedentary people who decide to start training and / or physical activity. Even in those cases for clinical reasons, such as overweight and obesity, should include exercise as an integral part of therapy. Therefore, the aim of the study was to analyze the incidence, prevalence and severity of sports injuries registered training protocols developed during the project's Nutrition and Physical Activity for the treatment of overweight and obesity (PRONAF). A descriptive analysis was performed and the effect and the degree of association between the incidence of injury and body composition, sex or training protocol was evaluated. Furthermore, the most common anatomical injury location in each of the phases were observed. Finally, an equation for estimating the risk of injury in overweight and obese subjects, before starting a controlled exercise program was proposed A total of 167 healthy adults with overweight and obesity (77 men, 90 women) were evaluated during the project using a previously validated record injuries. The main results of this study showed that 47.3% of participants experienced at least one injury or illness during the intervention program, having an incidence of 1.22 injuries per 100 hours of training. However, it was observed that 80.2% of injuries were lower (1 to 7 days). Knee location was higher proportion of lesions that recorded during the procedure. On the other hand, no significant associations between the phase of overweight and obesity were observed phase or between being male or female and further injury during project. However, training protocols that included loads, showed up to five times the risk of injury to the protocol did not include them. Finally, an equation was proposed to predict the influence of the phase, previous injury, training protocol, sex and age to observe the risk of injury in a population of overweight and obesity with includes the training protocols proposed in the PRONAF project.

A preocupação acrescida com a condição e aparência física, refletem-se em cuidados com o estado de saúde e bem-estar e são, nos últimos anos, responsáveis pela expansão dos ginásios e health clubs.Mas, atividade física e a alimentação estão intimamente ligadas, tanto no que respeita à estética como também, com maiores preocupações, no que respeita à saúde. Daí que as duas vertentes estejam ligadas à imagem de pessoas saudáveis e vigorosas. Na verdade, a capacidade de rendimento do organismo melhora com a nutrição adequada. A avaliação da composição corporal é um importante aspeto na determinação da condição física e permite-nos observar as alterações fisiológicas produzidas pelos programas de atividade física e/ou alimentares, oferecendo informações quanto a sua eficiência ou possíveis correções a serem efetuadas. Pretendeu-se com o presente estudo caracterizar os hábitos alimentares e a composição corporal dos indivíduos praticantes de exercício físico num ginásio do Concelho de Coimbra. A amostra foi constituída por 50 indivíduos, 22 do sexo masculino e 28 do sexo feminino, com idades compreendidas entre os 18 e os 57 anos. Estes indivíduos têm em comum o facto de praticarem exercício físico num mesmo ginásio, de uma forma sistemática há pelo menos 6 meses. Para a caracterização dos hábitos de AF dos participantes no estudo foi utilizada a versão curta do International Physical Activity Questionnaire (IPAQ). Para avaliação nutricional recorreu-se ao Questionário Semiquantitativo de Frequência Alimentar (QSFA), elaborado pelo Serviço de Higiene e Epidemiologia da Faculdade de Medicina da Universidade do Porto. A conversão dos alimentos em nutrientes foi realizada recorrendo ao programa informático Food Processor Plus ® versão 7.0. Foram ainda avaliados os seguintes parâmetros antropométricos: peso, altura, perímetro abdominal, IMC, massa gorda e massa magra. As análises estatísticas foram efetuadas com recurso ao Programa Estatístico SPSS (Statistical Package for the Sciences) para o Windows, versão 20.0. O nível de significância estatística foi mantida em 5% (p<0,05). A análise dos dados permitiu retirar as seguintes conclusões: Os frequentadores de ginásio são na sua maioria indivíduos do sexo feminino (56%), jovens com uma idade média de 27±7 anos. A maioria dos participantes do estudo praticava EF (exercício físico) há 39 meses, no entanto no grupo existe uma grande variabilidade, existindo indivíduos que praticam EF há 6 meses, e outros que o fazem há 180 meses. Praticam EF com uma frequência média de 3,5±1,3 sessões/semana a que corresponde uma prática de EF 6,40±3,2 h/semana. A maioria apresentava um Perímetro Abdominal (PA) de 85,2±6,3 cm, Índice de Massa Corporal (IMC) normal (23,1± 2,6 kg/m2), valores de Massa Gorda (MG) (23,9%) e de Massa Muscular (MM) de 34,6±7,9%. Observou-se uma diminuição da MG com o tempo de prática de exercício de 25,3% (praticantes entre 6-12 meses) vs 20,9% (praticantes> 48 meses). Esta influência temporal, embora sem significado estatístico, verifica-se também relativamente ao perímetro abdominal e IMC, embora neste último numa razão inversa, caracterizada pelo seu aumento. O tipo de exercício praticado influencia a composição corporal: praticantes de exercício aeróbio apresentam um perímetro abdominal inferior (82,3±6,0 cm) aos praticantes de outro tipo de exercícios porém acompanhado de uma percentagem de massa gorda superior (29,4±6,0%).Os praticantes de exercício de resistência apresentam valores de MG de 16,0±7,4%. No estudo por nós realizado, constatou-se inadequação na quantidade energética consumida, face às necessidades efetivas da amostra. Embora a ingestão de hidratos de carbono, de fibra e de lípidos se apresente dentro dos limites recomendados a ingestão de proteína ultrapassa os valores recomendados.Os resultados obtidos mostram que o tipo de exercício bem como o tempo de prática de exercício influenciam a composição corporal. Verificou-se ainda que a alimentação dos participantes no estudo não é adequada do ponto de vista nutricional e para além disso os hábitos nutricionais não parecem influir na composição corporal da amostra. Mais estudos serão necessários para que os hábitos alimentares dos praticantes de exercício em ginásios e health clubs sejam bem conhecidos e os profissionais da área tenham mais informações e possam orientar melhor os praticantes de exercício físico regular em ginásios e health clubs de modo a otimizar os resultados pretendidos através de um aconselhamento nutricional individualizado.

Objective: To discuss the diagnosis and treatment of a patient with cubital tunnel syndrome and to illustrate novel treatment modalities for the ulnar nerve and its surrounding structures and target tissues. The rationale for the addition of nerve-gliding techniques will be highlighted. Clinical Features: Two months after onset, a 17-year-old female nursing student who had a traumatic onset of cubital tunnel syndrome still experienced pain around the elbow and paresthesia in the ulnar nerve distribution. Electrodiagnostic tests were negative. Segmental cervicothoracic motion dysfunctions were present which were regarded as contributing factors hindering natural recovery. Intervention and Outcomes: After 6 sessions consisting of nerve-gliding techniques and segmental joint manipulation and a home exercise program consisting of nerve gliding and light free-weight exercises, a substantial improvement was recorded on both the impairment and functional level (pain scales, clinical tests, and Northwick Park Questionnaire). Symptoms did not recur within a 10-month follow-up period, and pain and disability had completely resolved. Conclusions: Movement-based management may be beneficial in the conservative management of cubital tunnel syndrome. As this intervention is in contrast with the traditional recommendation of immobilization, comparing the effects of both interventions in a systematic way is an essential next step to determine the optimal treatment of patients with cubital tunnel syndrome.

Objective: The purpose of this study was to investigate whether an endurance-strength training program is effective in reducing myoelectric manifestations of sternocleidomastoid (SCM) and anterior scalene (AS) muscle fatigue which have been found to be greater in people with chronic neck pain. Methods: Fifty-eight female patients with chronic non-severe neck pain were randomized into one of two 6-week exercise intervention groups: an endurance-strength training regime for the cervical flexor muscles or a referent exercise intervention involving low load retraining of the cranio-cervical flexor muscles. The primary outcomes were a change in maximum voluntary contraction (MVC) force and change of the initial value and rate of change of the mean frequency, average rectified value and conduction velocity detected from the SCM and AS muscles during sub-maximal isometric cervical flexion contractions at 50, 25 and 10% MVC. Results: At the 7th week follow-up assessment, the endurance-strength training group revealed a significant increase in MVC force and a reduction in the estimates of the initial value and rate of change of the mean frequency for both the SCM and AS muscles (P < 0.05). Both exercise groups reported a reduced average intensity of neck pain and reduced neck disability index score (P < 0.05). Conclusions: An endurance-strength exercise regime for the cervical flexor muscles is effective in reducing myoelectric manifestations of superficial cervical flexor muscle fatigue as well as increasing cervical flexion strength in a group of patients with chronic non-severe neck pain. Significance: Provision of load to challenge the neck flexor muscles is required to reduce the fatigability of the SCM and AS muscles in people with neck pain. Improvements in cervical muscle strength and reduced fatigability may be responsible for the reported efficacy with this type of exercise program. Background: Currently 1 in 11 women over the age of 60 in Australia are diagnosed with breast cancer. Following treatment, most breast cancer patients are left with shoulder and arm impairments which can impact significantly on quality of life and interfere substantially with activities of daily living. The primary aim of the proposed study is to determine whether upper limb impairments can be prevented by undertaking an exercise program of prolonged stretching and resistance training, commencing soon after surgery. Methods/design: We will recruit 180 women who have had surgery for early stage breast cancer to a multicenter single-blind randomized controlled trial. At 4 weeks post surgery, women will be randomly assigned to either an exercise group or a usual care ( control) group. Women allocated to the exercise group will perform exercises daily, and will be supervised once a week for 8 weeks. At the end of the 8 weeks, women will be given a home-based training program to continue indefinitely. Women in the usual care group will receive the same care as is now typically provided, i.e. a visit by the physiotherapist and occupational therapist while an inpatient, and receipt of pamphlets. All subjects will be assessed at baseline, 8 weeks, and 6 months later. The primary measure is arm symptoms, derived from a breast cancer specific questionnaire (BR23). In addition, range of motion, strength, swelling, pain and quality of life will be assessed. Discussion: This study will determine whether exercise commencing soon after surgery can prevent secondary problems associated with treatment of breast cancer, and will thus provide the basis for successful rehabilitation and reduction in ongoing problems and health care use. Additionally, it will identify whether strengthening exercises reduce the incidence of arm swelling.
