OBJETIVO: Avaliar tecnicamente a monitorização ambulatorial de pressão arterial (MAPA) em adolescentes normais. MÉTODOS: Quarenta e cinco adolescentes eutróficos, entre 10-18 anos de idade, sendo 27 do sexo feminino. RESULTADOS: Verificaram-se, em média, 90% de medidas bem sucedidas: incômodo relacionado ao funcionamento do monitor em 30% dos casos; valores médios de descenso sistólico, diastólico e de diminuição de freqüência cardíaca, no sono noturno, respectivamente iguais a 13%, 23% e 24%; carga pressórica na vigília, no sexo masculino, de 25,4±27,7% e 11,8±14,6% e, no feminino, de 17,5±18,7% e 11,8±11,4% para pressão arterial sistólica (PAS) e diastólica (PAD), respectivamente; carga pressórica no sono, no sexo masculino, de 15,4±22,9% e 2,8±4,9% e, no feminino, de 10,5±18,2% e 1,8±2,7% para PAS e PAD; medidas diastólicas mais elevadas nas duas primeiras horas de monitorização; diferenças entre sono noturno e vespertino quanto aos parâmetros cardiovasculares estudados. CONCLUSÃO: A MAPA mostrou-se bem tolerada pela população adolescente. Os registros obtidos apresentaram-se tecnicamente adequados para análise.

OBJETIVO - Avaliar o efeito dos ésteres de fitoesteróis (FE) da dieta, em função dos diferentes genótipos de apo E, sobre os lípides plasmáticos em indivíduos moderadamente hipercolesterolêmicos. MÉTODOS - Pacientes moderadamente hipercolesterolêmicos (20 a 60 anos de idade; 50 mulheres e 10 homens), mantidos em dieta habitual, receberam margarina (20g/dia) enriquecida com fitoesteróis (2,8g/dia=1,68g de FE), ou margarina normal (placebo), por dois períodos de quatro semanas cada, em estudo duplo cego e cruzado. RESULTADOS - Fitoesteróis reduziram significativamente o colesterol total e o LDL-colesterol, respectivamente, em 10% e 12% quando comparado à fase basal de admissão ao programa, e 6% e 8% quando comparado à fase placebo. O HDL-colesterol e a trigliceridemia não se modificaram. CONCLUSÃO - Ésteres de fitoesteróis da dieta reduziram a colesterolemia e a redução do LDL-colesterol foi mais pronunciada nos indivíduos que apresentavam valores maiores de LDL-colesterol, quando admitidos no estudo. Em relação ao sinótipo de apo E não houve diferença significativa entre apo E 3/3 e apo E 3/4.

OBJECTIVE: To assess the Dixtal DX2710 automated oscillometric device used for blood pressure measurement according to the protocols of the BHS and the AAMI. METHODS: Three blood pressure measurements were taken in 94 patients (53 females 15 to 80 years). The measurements were taken randomly by 2 observers trained to measure blood pressure with a mercury column device connected with an automated device. The device was classified according to the protocols of the BHS and AAMI. RESULT: The mean of blood pressure levels obtained by the observers was 148±38/93±25 mmHg and that obtained with the device was 148±37/89±26 mmHg. Considering the differences between the measurements obtained by the observer and those obtained with the automated device according to the criteria of the BHS, the following classification was adopted: "A" for systolic pressure (69% of the differences < 5; 90% < 10; and 97% < 15 mmHg); and "B" for diastolic pressure (63% of the differences < 5; 83% < 10; and 93% < 15 mmHg). The mean and standard deviation of the differences were 0±6.27 mmHg for systolic pressure and 3.82±6.21 mmHg for diastolic pressure. CONCLUSION: The Dixtal DX2710 device was approved according to the international recommendations.

Descreve-se o caso de um recém-nascido prematuro de peso muito baixo, gemelar, com trombose intracardíaca. O recém-nascido apresentou quadro compatível com sepse neonatal, sendo submetido a suporte avançado de vida, terapia com antibióticos, nutrição parenteral, uso de hemoderivados e cateterismo venoso profundo. Evoluiu com suspeita de endocardite infecciosa, sendo realizada ecocardiografia bidimensional com Doppler, quando foi evidenciado volumoso trombo intracavitário. Pela alta letalidade e pela dificuldade técnica da cirurgia, que, em alguns casos, é contra-indicada, optou-se pelo uso do trombolítico ativador de plasminogênio tecidual recombinante humano (rTPA) associado a aspirina, obtendo-se dissolução total do trombo sem efeitos adversos.

A cardiopatia induzida por estresse (precipitada por estresse emocional), também chamada de balonamento apical transitório do ventrículo esquerdo, síndrome do coração partido e, no Japão, síndrome de takotsubo, é caracterizada pela presença de movimento discinético transitório da parede anterior do ventrículo esquerdo, com acentuação da cinética da base ventricular. O curso clínico da cardiomiopatia de takotsubo pode se assemelhar ao do infarto agudo do miocárdio, com dor torácica típica e alterações eletrocardiográficas, sendo a cineangiocoronariografia realizada para distinguir as duas condições na fase aguda.

Background: The incidence of obesity in children is increasing worldwide, primarily in urbanized, high-income countries, and hypertension development is a detrimental effect of this phenomenon. Objective: In this cross-sectional study, we evaluated the prevalence of excess weight and its association with high blood pressure (BP) in schoolchildren. Methods: Here 4,609 male and female children, aged 6 to 11 years, from 24 public and private schools in Maringa, Brazil, were evaluated. Nutritional status was assessed by body mass index (BMI) according to cutoff points adjusted for sex and age. Blood pressure (BP) levels above 90th percentile for gender, age and height percentile were considered elevated. Results: The prevalence of excess weight among the schoolchildren was 24.5%; 16.9% were overweight, and 7.6% were obese. Sex and socioeconomic characteristics were not associated with elevated BP. In all age groups, systolic and diastolic BP correlated with BMI and waist and hip measurements, but not with waist-hip ratio. The prevalence of elevated BP was 11.2% in eutrophic children, 20.6% in overweight children [odds ratio (OR), 1.99; 95% confidence interval (CI), 1.61-2.45], and 39.7% in obese children (OR, 5.4; 95% CI, 4.23-6.89). Conclusion: Obese and overweight children had a higher prevalence of elevated BP than normal-weight children. Our data confirm that the growing worldwide epidemic of excess weight and elevated BP in schoolchildren may also be ongoing in Brazil.

Background:Left ventricular (LV) diastolic dysfunction is associated with new-onset atrial fibrillation (AF), and the estimation of elevated LV filling pressures by E/e' ratio is related to worse outcomes in patients with AF. However, it is unknown if restoring sinus rhythm reverses this process.Objective:To evaluate the impact of AF ablation on estimated LV filling pressure.Methods:A total of 141 patients underwent radiofrequency (RF) ablation to treat drug-refractory AF. Transthoracic echocardiography was performed 30 days before and 12 months after ablation. LV functional parameters, left atrial volume index (LAVind), and transmitral pulsed and mitral annulus tissue Doppler (e' and E/e') were assessed. Paroxysmal AF was present in 18 patients, persistent AF was present in 102 patients, and long-standing persistent AF in 21 patients. Follow-up included electrocardiographic examination and 24-h Holter monitoring at 3, 6, and 12 months after ablation.Results:One hundred seventeen patients (82.9%) were free of AF during the follow-up (average, 18 ± 5 months). LAVind reduced in the successful group (30.2 mL/m2 ± 10.6 mL/m2 to 22.6 mL/m2 ± 1.1 mL/m2, p < 0.001) compared to the non-successful group (37.7 mL/m2 ± 14.3 mL/m2 to 37.5 mL/m2 ± 14.5 mL/m2, p = ns). Improvement of LV filling pressure assessed by a reduction in the E/e' ratio was observed only after successful ablation (11.5 ± 4.5 vs. 7.1 ± 3.7, p < 0.001) but not in patients with recurrent AF (12.7 ± 4.4 vs. 12 ± 3.3, p = ns). The success rate was lower in the long-standing persistent AF patient group (57% vs. 87%, p = 0.001).Conclusion:Successful AF ablation is associated with LA reverse remodeling and an improvement in LV filling pressure.

Abstract Background: Right-sided heart failure has high morbidity and mortality, and may be caused by pulmonary arterial hypertension. Fractal dimension is a differentiated and innovative method used in histological evaluations that allows the characterization of irregular and complex structures and the quantification of structural tissue changes. Objective: To assess the use of fractal dimension in cardiomyocytes of rats with monocrotaline-induced pulmonary arterial hypertension, in addition to providing histological and functional analysis. Methods: Male Wistar rats were divided into 2 groups: control (C; n = 8) and monocrotaline-induced pulmonary arterial hypertension (M; n = 8). Five weeks after pulmonary arterial hypertension induction with monocrotaline, echocardiography was performed and the animals were euthanized. The heart was dissected, the ventricles weighed to assess anatomical parameters, and histological slides were prepared and stained with hematoxylin/eosin for fractal dimension analysis, performed using box-counting method. Data normality was tested (Shapiro-Wilk test), and the groups were compared with non-paired Student t test or Mann Whitney test (p < 0.05). Results: Higher fractal dimension values were observed in group M as compared to group C (1.39 ± 0.05 vs. 1.37 ± 0.04; p < 0.05). Echocardiography showed lower pulmonary artery flow velocity, pulmonary acceleration time and ejection time values in group M, suggesting function worsening in those animals. Conclusion: The changes observed confirm pulmonary-arterial-hypertension-induced cardiac dysfunction, and point to fractal dimension as an effective method to evaluate cardiac morphological changes induced by ventricular dysfunction.

This study compared tidepool fish assemblages within and among habitats at Iparana and Pecém beaches, State of Ceará, Northeast Brazil, using visual census techniques. A total of 8,914 fishes, representing 25 families and 43 species were recorded. The most abundant taxon was Sparisoma spp, followed by Haemulon parra (Desmarest, 1823), Acanthurus chirurgus (Bloch, 1787) and Abudefduf saxatilis (Linnaeus, 1758). Haemulidae was the most abundant family in number of individuals, followed by Scaridae, Acanthuridae and Pomacentridae. Within- and between- site differences in species assemblages probably reflected environmental discontinuities and more localized features, such as pool isolation episodes, or environmental complexity, both acting isolated or interactively. The locality of Iparana was probably subjected to a greater fishing pressure and tourism than Pecém, a potential cause for the observed lowest fish abundance and biodiversity. We conclude that tidepool ichthyofauna may be quite variable between and within reef sites. Thus, observations taken from or damages caused on one area may not be generalized to or mitigated by the protection of adjacent sites.

To study the action of molluscicide nine ponds were selected: 3 of them lying in Maruim municipality, 29 km far from north Aracaju, the State capital, and 6 ponds in Itabaianinha municipality, 118 km far from south Aracaju. This study was carried out for 16 months. Environmental parameters observed were those thought to have any influence on the planorbids and/or the molluscicide: water temperature, transparence, salinity, pH, dissolved oxygen, CO2, and the nutrients-phosphorus, nitrogen, potassium and calcium. Plancton microorganisms were also considered to observe Bayluscide action on them. SRB was used in a concentration of 6.25 kg per 1.000 [cubic metres] water, to achieve 1.0 ppm Bayluscide concentration according to the producer's instruction in Massachussett-USA.

We have observed that several plants used popularly as anti-snake venom show anti-inflammatory activity. From the list prepared by Rizzini, Mors and Pereira some species have been selected and tested for analgesic activity (number of contortions) and anti-inflammatory activity (Evans blue dye diffusion - 1% solution) according to Whittle's technique (intraperitoneal administration of 0.1 N-acetic acid 0.1 ml/10 g) in mice. Previous oral administration of a 10% infusion (dry plant) or 20% (fresh plant) corresponding to 1 or 2 g/Kg of Apuleia leiocarpa, Casearia sylvestris, Brunfelsia uniflora, Chiococca brachiata, Cynara scolymus, Dorstenia brasiliensis, Elephantopus scaber, Marsypianthes chamaedrys, Mikania glomerata and Trianosperma tayuya demonstrated analgesic and/or anti-inflammatory activities of varied intensity
