O artigo traz uma abordagem sobre as ações coletivas de participação nos conselhos municipais de saúde. A pesquisa que o fundamenta teve como objetivo verificar a atuação dos conselheiros nos conselhos municipais de saúde da Região Metropolitana de Belém (PA). Foi utilizado levantamento documental (período 2005 a 2010), entrevistas estruturadas com conselheiros e observações em sessões de reuniões dos conselhos de três municípios. Foi construído um indicador de três níveis de participação com base nas funções legais dos conselhos de saúde, tendo como fundamento analítico a teoria dos grupos sociais. Os resultados indicam que as ações dos conselhos são marcadas por cooptação de conselheiros, definição de agendas deliberativas por parte de representantes da gestão, predominando os interesses individuais dos conselheiros sobre os interesses coletivos nas deliberações. Concluiu que tais comportamentos contrariam os princípios do Sistema Único de Saúde - SUS e fragiliza a imagem social dos conselhos, além de abrir uma nova perspectiva de análise.

A expansão da obesidade em diversos países do mundo na última década tem resultado no aumento da morbidade e mortalidade por hipertensão arterial e suas complicações. O objetivo deste trabalho é analisar a distribuição espacial da obesidade e hipertensão arterial no estado de São Paulo no período de 2000 a 2010, a partir de registros hospitalares e internação do Sistema de Informações Hospitalares do Sistema Único de Saúde (SIH - SUS). Foram utilizados coeficientes de prevalência das doenças em cada município suavizadas pelo método bayesiano empírico, permitindo uma visualização do padrão espacial dessas morbidades no Estado. Foi explorada a dependência espacial destes padrões verificando a autocorrelação entre os indicadores por meio do cálculo do Índice de Autocorrelação Espacial de Moran. Além disso, estudou-se a correlação positiva (Pearson) entre obesidade e hipertensão. Os dados e os mapas mostraram clusters de 87 municípios onde há maior e menor prevalência de hipertensão e obesidade no espaço com forte autocorrelação entre os municípios vizinhos. O coeficiente correlação de Pearson encontrado para esses municípios foi de 0,404 e sugere associação entre as morbidades. As técnicas de análise espacial mostraram-se úteis para o planejamento de ações de saúde pública.

Brazilian biodiversity is a colossal source of secondary metabolites with remarkable structural features, which are valuable in further biodiscovery studies. In order to fully understand the relations and interactions of a living system with its surroundings, efforts in natural product chemistry are directed toward the challenge of detecting and identifying all the molecular components present in complex samples. It is plausible that this endeavor was born out of recent technological sophistication in secondary metabolite identification with sensitive spectroscopic instruments (MS and NMR) and higher resolving power of chromatographic systems, which allow a decrease in the amount of required sample and time to acquire data. Nevertheless, the escalation of data acquired in these analyses must be sorted with statistical and multi-way tools in order to select key information. Chromatography is also of paramount importance, more so when selected compounds need to be isolated for further investigation. However, in the course of pursuing a greener environment, new policies, with an aim to decrease the use of energy and solvents, are being developed and incorporated into analytical methods. Metabolomics could be an effective tool to answer questions on how living organisms in our huge biodiversity work and interact with their surroundings while also being strategic to the development of high value bio-derived products, such as phytotherapeutics and nutraceuticals. The incorporation of proper phytotherapeutics in the so-called Brazilian Unified Health System is considered an important factor for the urgent improvement and expansion of the Brazilian national health system. Furthermore, this approach could have a positive impact on the international interest toward scientific research developed in Brazil as well as the development of high value bio-derived products, which appear as an interesting economic opportunity in national and global markets. Thus, this study attempts to highlight the recent advances in analytical tools used in detection of secondary metabolites, which can be useful as bioproducts. It also emphasizes the potential avenues to be explored in Brazilian biodiversity, known for its rich chemical diversity.

This article offers theoretical and practical elements for an analysis of the Worker's Health Care Program developed by the Brazilian Unified Health System (SUS). From a determined locus, the region of Franca, State of São Paulo, it aims at identifying the challenges for implementation of the National Occupational Health Policy, due to the particularities of the work/health relationship within the sugarcane agro-industry. It is based on group interviews involving health workers, specifically sanitary inspectors and the worker's health interlocutors. The results showed that health care professionals must be better prepared, from the technical point of view, to be able to perform actions related to workers' health. These results also indicated the need for a greater integration between health services, particularly when damage notification and workers' health surveillance are concerned.

Objective. To review the literature regarding the application of the notion of user embracement and to identify the contributions of this concept for primary health care practices in Brazil.Method. We carried out an integrative review of the literature regarding primary health care. The following databases were searched: LILACS, SciELO, and MEDLINE, covering the period from 2006 to 2010. The following search terms were used in LILACS and SciELO: acolhimento and programa saude da familia and saude. For MEDLINE, the terms user embracement and family health program and health were used. The review was performed in November 2010.Results. We identified 21 articles meeting the inclusion criteria, all of which described studies carried out in Brazil. The articles were divided into three empirical categories: integration and embracement; primary care work process; and evaluation of services. These are complementary categories that converge to two main views of embracement: the first sees embracement as a means of reorganizing the primary health care environment, and the second sees embracement as an attitude towards users. The review also shows that embracement may be a management tool that supports the Unified Health System and is associated with the principles of comprehensiveness and universality.Conclusions. Embracement is able to create a bond between health care workers and users. It promotes self-care, a better understanding of disease, as well as user co-responsibility for treatment. In addition, it facilitates universal access, strengthens multiprofessional and intersectoral work, qualifies care, humanizes practices, and encourages actions to combat prejudice. Nevertheless, the perspective of health care users regarding embracement deserves more attention and should be the focus of future studies.

Background: Birth weight reflects gestational conditions and development during the fetal period. Low birth weight (LBW) may be associated with antenatal care (ANC) adequacy and quality. The purpose of this study was to analyze ANC adequacy and its relationship with LBW in the Unified Health System in Brazil.Methods: A case-control study was conducted in Botucatu, Sao Paulo, Brazil, 2004 to 2008. Data were collected from secondary sources (the Live Birth Certificate), and primary sources (the official medical records of pregnant women). The study population consisted of two groups, each with 860 newborns. The case group comprised newborns weighing less than 2,500 grams, while the control group comprised live newborns weighing greater than or equal to 2,500 grams. Adequacy of ANC was evaluated according to three measurements: 1. Adequacy of the number of ANC visits adjusted to gestational age; 2. Modified Kessner Index; and 3. Adequacy of ANC laboratory studies and exams summary measure according to parameters defined by the Ministry of Health in the Program for Prenatal and Birth Care Humanization.Results: Analyses revealed that LBW was associated with the number of ANC visits adjusted to gestational age (OR = 1.78, 95% CI 1.32-2.34) and the ANC laboratory studies and exams summary measure (OR = 4.13, 95% CI 1.36-12.51). According to the modified Kessner Index, 64.4% of antenatal visits in the LBW group were adequate, with no differences between groups.Conclusions: Our data corroborate the association between inadequate number of ANC visits, laboratory studies and exams, and increased risk of LBW newborns. No association was found between the modified Kessner Index as a measure of adequacy of ANC and LBW. This finding reveals the low indices of coverage for basic actions already well regulated in the Health System in Brazil. Despite the association found in the study, we cannot conclude that LBW would be prevented only by an adequate ANC, as LBW is associated with factors of complex and multifactorial etiology. The results could be used to plan monitoring measures and evaluate programs of health care assistance during pregnancy, at delivery and to newborns, focusing on reduced LBW rates.

This study aimed to assess knowledge of nursing professionals (nurses, technicians and nursing assistants) on the Project Sentinel Hospitals in a state hospital in the interior of and identify the occurrence of under-reporting of hospital products and the reasons that prevented the team to notify them. This is a study of a descriptive and exploratory, held in a public hospital in the interior of which serves only patients of the Unified Health System (SUS). The study sample consisted of 245 nurses. Data analysis revealed that the majority of nurses and nursing assistants refers knowledge about Project Sentinel Hospitals. In relation to the Practical nurses less than half know Hospitals Project Sentry. Knowledge of the four spheres of Hospitals Project Sentinel is higher among nurses. Among the practical nurses and nursing assistants prevailing knowledge in the area of Pharmacovigilance. The nurses have more knowledge about the process of notification and are major notifiers. Technicians and nursing assistants are those who have greater interest in learning content as a whole Hospitals Project Sentry. The nurses were the professionals who witnessed the most under-and under-reporting associated with the fear, lack of knowledge on the subject, insecurity and lack of time. It can be concluded that the study identified the knowledge of nurses on Project Sentinel Hospitals and the procedure for notification of technical defects and adverse events related to health products as well as identify the presence of under-reporting among professionals. In addition, the study shows the importance and need for greater disclosure of the activities developed by Project Sentinel Hospitals among nursing professionals through meetings, lectures, brochures, among other outlets
