Entre 2005 e 2008, registaram-se 8413 nascimentos na maternidade do Hospital Dona Estefânia (HDE), aproximadamente 8% do total nacional no mesmo período. A mortalidade fetal (0,20%) atingiu os objectivos do Plano Nacional de Saúde (PNS) para 2010 e está aos melhores niveis da União Europeia. Contudo, a percentagem média de nascimentos prétermo (8,1%) e de cesarianas (31,9%) situam-se ainda acima das metas estabelecidas pelo PNS de, respectivamente, 4,9% e 24,8%. O odds ratio de ocorrência de um índice Apgar baixo aos 5 minutos por cada 100 g de peso a menos à nascença foi de 1,35 e por cada semana a menos de gestação foi de 1,33. As parturientes tiveram em média 30,4 anos de idade, sendo 3,8% adolescentes. Cerca de 22% eram estrangeiras, valor superior à média nacional de 9%. Registou-se variabilidade entre as principais nacionalidades quanto a percentagem de partos pré-termo e de cesarianas, sendo menor nas mães chinesas. Encontrou-se significado estatístico na relação do peso à nascença com a idade de gestação,tipo de gravidez (simples/gemelar), sexo, paridade e idade da mãe. Fixando as covariáveis,uma semana de gestação a mais correspondeu, em média, a mais 176 g; um recém-nascido(RN) gémeo teve, em média, menos 381 g que um não-gémeo e um RN do sexo feminino pesou, em média, menos 48 g que um masculino. Apresentam-se tabelas de percentis de peso por sexo e idade gestacional (36-41 semanas) para os RN do HDE.

Nesta revisão é actualizado o primeiro Consenso Nacional sobre Nutrição Parentérica no Recém-Nascido, publicado em 2004, com especial relevância para as indicações, contra-indicações e limitações, formulação das misturas e armazenamento, suprimento recomendado de líquidos, energia, macronutrientes e micronutrientes, nutrição parentérica em situações particulares, suas principais complicações, estimativa da osmolaridade das soluções, vias de administração, controlo clínico e laboratorial.

TICAI 2008 é o terceiro volume de uma série que recolhe as contribuições mais significativas dos melhores congressos de língua espanhola e Portuguesa no âmbito da Sociedade de Educação do IEEE. Este volume corresponde aos eventos realizados no ano de 2008, é promovido pelos Capítulos Português e Espanhol desta Sociedade. Como os volumes anteriores, o âmbito centra-se na investigação e aplicações da tecnologia na educação e engloba o desenho e investigação de novas ferramentas, materiais e técnicas que facilitem o ensino/ aprendizagem e aplicações, métodos pedagógicos e experiências concretas de uso destas novas técnicas e ferramentas. Tudo com um enfoque particular no ensino/ aprendizagem das disciplinas próprias do IEEE, fundamentalmente nas áreas da Engenharia Electrotécnica, Tecnologia Electrónica, Engenharia de Telecomunicações e Engenharia Informática. Assim, os capítulos deste livro passaram pelo crivo apertado de duas revisões: primeiro do próprio congresso e depois uma segunda selecção de entre os artigos aceites para o congresso. Para esse processo, contamos com membros destacados de cada um dos Comités de Organização ou de programa dos respectivos congressos.

Com as dificuldades económicas que se fazem sentir nos últimos anos, tanto em Portugal como no resto do mundo, em qualquer área de negócio, a qualidade tornou-se um dos fatores de sucesso. Com um mercado cada vez com mais concorrência, os clientes procuram frequentemente a melhor qualidade do produto ou do serviço da empresa. Para isso as empresas cada vez mais apostam em fornecer um produto de qualidade através de um sistema da qualidade bem delineado, bem gerido e certificado. O presente trabalho que foi desenvolvido nesta dissertação de Mestrado consistiu na conceção e acompanhamento da Implementação do Sistema de Gestão da Qualidade (SGQ) segundo a norma NP EN ISO 9001:2008 realizada numa indústria de componentes de calçado, designada NEWCOMP, Componentes para Calçado, LDA. (NCP), localizada em S. João da Madeira. Será uma ferramenta bastante útil e estratégica para a empresa a certificação, com inúmeras vantagens e benefícios para a sua imagem. As principais razões que levaram a empresa a interessar-se pela certificação do SGQ foi, principalmente, a melhoria da qualidade dos seus produtos, satisfação dos clientes e alcançar novos mercados. A documentação elaborada do SGQ inclui a política e objetivos da qualidade, esquemas dos principais processos, manual de funções entre outros documentos considerados necessários, para utilização de todas as partes envolvidas.

Botulism is a rare and potentially lethal illness caused by Clostridium botulinum neurotoxin. We describe the findings of a laboratorial investigation of 117 suspected cases of botulism reported to the surveillance system in Brazil from January 2000 to October 2008. Data on the number and type of samples analyzed, type of toxins identified, reporting of the number of botulism cases and transmission sources are discussed. A total of 193 clinical samples and 81 food samples were analyzed for detection and identification of the botulism neurotoxin. Among the clinical samples, 22 (11.4%) presented the toxin (nine type A, five type AB and eight with an unidentified type); in food samples, eight (9.9%) were positive for the toxin (five type A, one type AB and two with an unidentified type). Of the 38 cases of suspected botulism in Brazil, 27 were confirmed by a mouse bioassay. Laboratorial botulism diagnosis is an important procedure to elucidate cases, especially food-borne botulism, to confirm clinical diagnosis and to identify toxins in food, helping sanitary control measures.

With the aim of identifying the etiology of acute febrile illness in patients suspected of having dengue, yet with non reagent serum, a descriptive study was conducted with 144 people using secondary serum samples collected during convalescence. The study was conducted between January and May of 2008. All the exams were re-tested for dengue, which was confirmed in 11.8% (n = 17); the samples that remained negative for dengue (n = 127) were tested for rubella, with 3.9% (n = 5) positive results. Among those non reactive for rubella (n = 122), tests were made for leptospirosis and hantavirus. Positive tests for leptospirosis were 13.9% (n = 17) and none for hantavirus. Non reactive results (70.8%) were considered as Indefinite Febrile Illness (IFI). Low schooling was statistically associated with dengue, rubella and leptospirosis (p = 0.009), dyspnea was statistically associated with dengue and leptospirosis (p = 0.012), and exanthem/petechia with dengue and rubella (p = 0.001). Among those with leptospirosis, activities in empty or vacant lots showed statistical association with the disease (p = 0.013). Syndromic surveillance was shown to be an important tool in the etiologic identification of IFI in the Federal District of Brazil.

This study describes the epidemiological profile of malaria in the State of Tocantins, in the period 2003-2008, investigates the association between the frequency of malaria and population growth, classifies the cases by 'autochthonous' and 'imported', reports the indices of the disease and analyses the distribution of the cases by Plasmodium species, age and gender. The retrospective study was based on secondary data, stored in SIVEP-malaria and analyzed using the software Epi-Info 3.5.1. and Bioestat 5.0. 19,004 samples were investigated for malaria, 19% of them were positive, 73.32% with Plasmodium vivax, 21.80% with Plasmodium falciparum, 4.79% with mixed infections and only 0.08% with Plasmodium malariae. Male individuals accounted for 76.95% and predominated in all years and age groups, especially in the 15 to 49 years old group. From the overall cases, 34.27% were autochthonous and 65.73% were imported (χ2 = 356.8, p = 0.0001). The frequency of malaria decreased significantly during the entire series (rp = 0.96, p = 0.002) and the number of municipalities with autochthonous transmission also diminished. It was found that malaria is predominantly imported, related to land activities, which confirms the need for effective measures to maintain vigilance throughout the state and enhance educational activities in order to guide the population towards early treatment-seeking.

Considering that there are some studies with autopsies from AIDS describing only malignant neoplasias and that changes can occur after the introduction of Highly Active Antiretroviral Therapy (HAART), our objectives were to analyze the frequency of benign and malignant neoplasms in AIDS patients in the periods of both pre- and post-HAART. This is a retrospective study with 261 autopsies of HIV-positive patients between 1989 and 2008 in Uberaba, Brazil. Sixty-six neoplasms were found (39 benign, 21 malignant and six premalignant) in 58 patients. The most frequent malignant neoplasms were lymphoid, in 2.7% (four Non-Hodgkin lymphoma, one Hodgkin, one multiple myeloma and one plasmablastic plasmacytoma), and Kaposi's Sarcoma, in 2.3% (six cases). The most frequent benign neoplasms were hepatic hemangiomas in 11 (4.2%) of 261 cases and uterine leiomyoma in 11 (15.7%) of 70 woman. In the pre-HAART period eight (9.8%) benign neoplasias and four (4.9%) malignant occurred in 82 patients; in the post-HAART period, 29 (16.2%) benign and 17 (9.5%) malignant were present; however, the differences were not significant. We conclude that the introduction of HAART in our region doesn't look to have modified the frequency of neoplasms occurring in patients with HIV.

Since 2000, the expansion of Sylvatic Yellow Fever (YF) has been observed in the southeast of Brazil, being detected in areas considered silent for decades. Epizootics in non-human primates (NHPs) are considered sentinel events for the detection of human cases. It is important to report epizootic events that could have impact on the conservation status of susceptible species. We describe the epizootics in NHPs, notified in state of São Paulo, Brazil, between September 2008 to August 2009. Ninety-one epizootic events, involving 147 animals, were reported in 36 counties. Samples were obtained from 65 animals (44.2%). Most of the epizootics (46.6%) were reported between March and April, the same period during which human cases of YF occurred in the state. Biological samples were collected from animals found dead and were sent to Instituto Adolfo Lutz, in São Paulo. Two samples, collected in two counties without an indication for YF vaccination, were positive for the virus. Another 48 animals were associated with YF by clinical-epidemiological linkage with laboratory confirmed cases. Because the disease in human and NHPs occurred in the same period, the detection of the virus in NHPs did not work as sentinel, but aided in the delineation of new areas of risk.

Hand-foot-and-mouth disease (HFMD) is becoming one of the extremely common airborne and contact transmission diseases in Guangzhou, southern China, leading public health authorities to be concerned about its increased incidence. In this study, it was used an ecological study plus the negative binomial regression to identify the epidemic status of HFMD and its relationship with meteorological variables. During 2008-2012, a total of 173,524 HFMD confirmed cases were reported, 12 cases of death, yielding a fatality rate of 0.69 per 10,000. The annual incidence rates from 2008 to 2012 were 60.56, 132.44, 311.40, 402.76, and 468.59 (per 100,000), respectively, showing a rapid increasing trend. Each 1 °C rise in temperature corresponded to an increase of 9.47% (95% CI 9.36% to 9.58%) in the weekly number of HFMD cases, while a one hPa rise in atmospheric pressure corresponded to a decrease in the number of cases by 7.53% (95% CI -7.60% to -7.45%). Similarly, each one percent rise in relative humidity corresponded to an increase of 1.48% or 3.3%, and a one meter per hour rise in wind speed corresponded to an increase of 2.18% or 4.57%, in the weekly number of HFMD cases, depending on the variables considered in the model. These findings revealed that epidemic status of HFMD in Guangzhou is characterized by high morbidity but low fatality. Weather factors had a significant influence on the incidence of HFMD.
