Project Management involves onetime endeavors that demand for getting it right the first time. On the other hand, project scheduling, being one of the most modeled project management process stages, still faces a wide gap from theory to practice. Demanding computational models and their consequent call for simplification, divert the implementation of such models in project management tools from the actual day to day project management process. Special focus is being made to the robustness of the generated project schedules facing the omnipresence of uncertainty. An "easy" way out is to add, more or less cleverly calculated, time buffers that always result in project duration increase and correspondingly, in cost. A better approach to deal with uncertainty seems to be to explore slack that might be present in a given project schedule, a fortiori when a non-optimal schedule is used. The combination of such approach to recent advances in modeling resource allocation and scheduling techniques to cope with the increasing flexibility in resources, as can be expressed in "Flexible Resource Constraint Project Scheduling Problem" (FRCPSP) formulations, should be a promising line of research to generate more adequate project management tools. In reality, this approach has been frequently used, by project managers in an ad-hoc way.

Introduction: It is a big challenge that devolopes in a family with the birth ofdisabled or handycapped child. The siblings of the disabled or handicapped child aredealt with issues like sufferings, sickness, weakness and even death. Furthermorethey have to take part in assisting and care of the disabled or challenged child. Theexperience they make as they grow up with a disabled or handycapped sibling is aburden more than a challenge lies on the support service options they are entitled.The choices are from the representatives of social organisations to professional help.Aim of this work is to analyse the necessity of support services for the siblings of thedisabled or handycapped child. To achieve this, a retrospective analysis of the peoplewho were affected by such a situation is necessary.Material and Methods: Structured, guidelined, problem centered personnalinterviews with affected siblings. The method was chosen against the background ofadequate affected siblings with necessary informations, knowledge and experience.Nine interviewees were selected and requested for cooperation per e-mail. FourInterviews were done personnally and five through telephone.Result: All the interviewees had disabled or handicapped or diseased siblings. Someof the interviewees knew there was something not in order with thier sibling beforethier preschool age. Some of them noticed thier siblings as "different" through theinteractions with other schoolmates. All the interviewees except the ones who werementally challenged siblings were well informed by their parents about the situation.The limitations of the siblings were very complex. The interviewees had a lot oflimitations and obligations for themselves because of the disability of their siblings.All but one of the interviewees could atleast think of one nice experience with thiermentally or physically challenged sibling. BACKGROUND: Psychomental stress is a major source of illness and reduced productivity. Data objectifying physiological stress responses are scarce. We studied salivary cortisol levels in a highly stressful environment, the pediatric critical care unit. The aim was to identify targets for organizational changes, to implement these changes and to assess their impact on cortisol levels. DESIGN: Repeated measurements observational cohort study (before and after intervention). SUBJECTS: 84 nurses working in two independent teams (A and B) in a 19 bed pediatric intensive care unit. Between study periods team A experienced a major exchange of experienced staff while the turnover rate in team B remained average. MEASUREMENTS AND INTERVENTIONS: Salivary cortisol samples were collected every 2 h and after stressful events. Nurses in study period I showed elevated cortisol levels at the beginning of the late shift, interpreted as an anticipatory stress reaction. To ease conditions during the early part of the late shift (conflicting tasks, noise and crowding), we postponed the afternoon ward round, limited non-urgent procedures and introduced a change in visiting hours. The early shift, which was not affected by the intervention, served as control. MAIN RESULTS: Both crude and adjusted analysis revealed a decrease of cortisol levels at the beginning of the late shift in team B (p = 0.0009), but not in team A (p = 0.464). The control situation showed no difference between teams and study periods. INTERPRETATION: We demonstrated reduced cortisol secretions in one team following organizational changes, which was probably overridden by the disruption of social coherence in the second team.

We analyzed data from historical controls treated with meglumine antimoniate to compare the frequency of adverse events observed in patients with cutaneous leishmaniasis treated with the same dose of meglumine antimoniate contaminated with heavy metals in an endemic area of the State of Bahia, Brazil. Group A patients were treated in 2000 with the drug produced by Eurofarma Laboratórios Ltda., São Paulo, Brazil (lot A) and group B patients were treated in 1996 with the reference drug produced by Rhodia Farma Ltda., São Paulo, Brazil (lot B). We observed an unusual higher frequency of skin reactions in group A patients. However, all type of adverse events observed in group A were also observed in group B. The physico-chemical analysis of these lots revealed that lot A had lower pH and higher concentration of total and trivalent antimony, lead, cadmium, and arsenic. Our findings suggest that the skin reactions could be attributed to heavy metal contamination of lot A.

To improve long-term survival, prompt revascularization of the infarct-related artery should be done in patients with acute myocardial infarction (AMI); therefore, a large proportion of these patients would be hospitalized during out of hours. The clinical effects of out-of-hours AMI management were already questioned, with conflicting results. The purpose of this investigation was to compare the in-hospital outcome of patients admitted for AMI during out of hours and working hours. All patients with AMI included in the AMIS Plus Registry from January 1, 1997, to March 30, 2006, were analyzed. The working-hours group included patients admitted from 7 a.m. to 7 p.m. on weekdays, and the out-of-hours group included patients admitted from 7 p.m. to 7 a.m. on weekdays or weekends. Major cardiac events were defined as cardiovascular death, reinfarction, and stroke. The study primary end points were in-hospital death and major adverse cardiac event (MACE) rates. A total of 12,480 patients met the inclusion criteria, with 52% admitted during normal working hours, and 48%, during out of hours. Patients admitted during weekdays included more women (28.1% vs 26%; p = 0.009), older patients (65.5 +/- 13 vs 64.1 +/- 13 years; p = 0.0011), less current smokers (40.1% vs 43.5%; p <0.001), and less patients with a history of ischemic heart disease (31.5% vs 34.5%; p = 0.001). A significantly higher proportion of patients admitted during out of hours had Killip's class III and IV. No differences in terms of in-hospital survival rates between the 2 groups (91.5% vs 91.2%; p = 0.633) or MACE-free survival rates (both 88.5%; p = 1.000) were noted. In conclusion, the outcome of patients with AMI admitted out of hours was the same compared with those with a weekday admission. Of predictors for in-hospital outcome, timing of admission had no significant influence on mortality and/or MACE incidence.

Dans les pays industrialisés, 20% des salariés sont concernés par le travail de nuit. Pourtant, les effets du travail de nuit sur la santé sont peu traités par les revues médicales générales, alors que le travail de nuit a des conséquences non négligeables sur les systèmes cardiovasculaires et digestifs entre autres, comme l'ont démontré de nombreuses études ces dernières décennies. Le travail de nuit a encore récemment attiré l'attention quand il a été déclaré cancérigène probable (catégorie 2A) par le Centre international de recherches sur le cancer. Ainsi dans cet article, seront passés en revue les troubles de la santé qui peuvent être générés ou aggravés par le travail de nuit. Deux cas pratiques illustreront la problématique et permettront d'aborder la conduite à tenir. 20% of employees in industrialized countries are concerned by shift work. Nevertheless, there is very little information in general medical journals about the effects of shift work on health. Shift work can have several major effects on health such as cardiovascular and digestive disorders among others, as demonstrated by several studies in recent decades. Shift work has attracted considerable attention recently when it was declared probable carcinogen by the International Agency for research on cancer. We review the health disorders that may be generated or aggravated by shift work and illustrate the problem by two case studies of occupational medicine and discuss the appropriate attitude to take

We address the performance optimization problem in a single-stationmulticlass queueing network with changeover times by means of theachievable region approach. This approach seeks to obtainperformance bounds and scheduling policies from the solution of amathematical program over a relaxation of the system's performanceregion. Relaxed formulations (including linear, convex, nonconvexand positive semidefinite constraints) of this region are developedby formulating equilibrium relations satisfied by the system, withthe help of Palm calculus. Our contributions include: (1) newconstraints formulating equilibrium relations on server dynamics;(2) a flow conservation interpretation of the constraintspreviously derived by the potential function method; (3) newpositive semidefinite constraints; (4) new work decomposition lawsfor single-station multiclass queueing networks, which yield newconvex constraints; (5) a unified buffer occupancy method ofperformance analysis obtained from the constraints; (6) heuristicscheduling policies from the solution of the relaxations.

Tämä opinnäytetyö on tehty SkinnAir Oy:lle, ja se käsittelee pientalojen LVI-teknistä suunnittelua pienen suunnitteluyrityksen näkökulmasta. Nykyinen kiivas rakentamistyyli, kasvaneet asumisviihtyvyydenvaatimukset, asennusammattilaisten puute sekä jatkuvasti kohoavat vaatimukset rakennusten energiatehokkuuden parantamiseksi vaativat yhä parempia ja laadukkaampia suunnitelmia pientalojen LVI-teknisten ratkaisujen toteuttamiseksi. Suunnittelusta saatavan heikon tuottavuuden johdosta monet suunnittelutoimistot eivät ole kiinnostuneita pientalojen LVI-suunnittelusta. Tässä työssä pyritään etsimään niitä menetelmiä, joilla erityisesti pieni suunnitteluyritys kykenee parantamaankannattavuuttaan ja nostamaan palvelun laatua kyseisellä suunnittelun osa-alueella. Työ on toteutettu perehtymällä yksityiskohtaisesti suunnitteluprosessin eri vaiheisiin sekä analysoimalla näistä vaiheista saadut tulokset Työssä tunnistetaan erilaiset asiakastyypit sekä heidän tarpeensa. Asiakastarpeiden perusteellasaadaan määritettyä oikeat lähtötiedot kohteen LVI-suunnittelua varten. Työn keskeinen osa on suunnitteluprosessin etenemisen sekä suunnittelutyöhön kuuluvien tehtävien tarkastelu. Tämän tarkastelun avulla pyritään löytämään keinot, joillakyetään tehostamaan suunnitteluprosessin eri vaiheisiin liittyviä toimintoja, parantamaan palvelun laatua sekä lisäksi minimoimaan suunnitteluprosessin aikaisia kustannuksia yrityksen kannattavuuden parantamiseksi. LVI-suunnittelutyön eteneminen painottuu asiakas- ja lähtötietojen keräämiseen sekä eri välivaiheiden hyväksyttämiseen tilaajalla. Suunnittelutyöhön vaikuttavat tekijät saadaan selville jatkuvassa, ennalta hyvin aikataulutetussa asiakaskontaktissa. Asiakkaan huomioiminen suunnitteluprosessin eri vaiheissa parantaa kokonaisvaltaista asiakaspalvelua. Näin asiakkaan saama vastine rahoilleen kasvaa. Suunnittelijan kannalta turhat muutostyöt vähenevät, koska asiakas on selvillä suunnittelutyön etenemisestä. Johtopäätöksenä voidaan todeta, että hyvän suunnitteluprosessin edellytyksenä on yksityiskohtaisten lähdetietojen käyttö. Työn tuloksena ilmeni, että valmiiden, ennalta hyvin laadittujen lomakkeiden käyttäminen parantaa selkeästi suunnittelutyön tehokkuutta. Lopputuloksena syntyi jatkokehittelyä vaativa lomakepohja asiakastarpeisiin perustuvalle lähtötietolomakkeelle. Lisäksi yrityksen käyttöön on tarkoitus laatia tarvittavat tietokannat sekä valmiit asennuspiirrokset. Lopullinen tavoite on rakentaa tässä työssä kuvatuista menetelmistä kattava tietotekninen järjestelmä parantamaan sekä yrityksen kannattavuutta että suunnittelutyön laatua. Lisäksi voidaan todeta, että rakennettava järjestelmä mahdollistaa myös vaativan suunnittelutyön tuotteistamisen.

The Thesis gives a decision support framework that has significant impact on the economic performance and viability of a hydropower company. The studyaddresses the short-term hydropower planning problem in the Nordic deregulated electricity market. The basics of the Nordic electricity market, trading mechanisms, hydropower system characteristics and production planning are presented in the Thesis. The related modelling theory and optimization methods are covered aswell. The Thesis provides a mixed integer linear programming model applied in asuccessive linearization method for optimal bidding and scheduling decisions inthe hydropower system operation within short-term horizon. A scenario based deterministic approach is exploited for modelling uncertainty in market price and inflow. The Thesis proposes a calibration framework to examine the physical accuracy and economic optimality of the decisions suggested by the model. A calibration example is provided with data from a real hydropower system using a commercial modelling application with the mixed integer linear programming solver CPLEX.

Tutkielmassa käsitellään Euroopan unionin kemikaaliasetusta 1907/2006, joka astui voimaan 1.6.2007 ja sen vaikutuksia maahantuontiyrityksen toimintaan. Kemikaaliasetusta toteutetaan soveltamalla REACH-järjestelmää. REACH tulee sanoista Registration (rekisteröinti), Evaluation (arviointi), Authorisation (lupamenettely) Chemicals (koskee kemikaaleja). Tutkielman kirjallisuusosassa käsitellään pääosin kemikaaliasetusta. Empiirisessä osassa käsitellään asetuksen tuomia haasteita ja peilataan niitä maahantuontiyrityksen tähänastisiin kokemuksiin ja vaikutelmiin. Kohdeyrityksenä on kemian alan yritys PCC SE. Tutkielmassa selvitetään myös, miten REACH on vaikuttanut tarkastus- ja testauslaitokseen. Lisäksi REACH-järjestelmää peilataan olemassa olevaan kasvinsuojelu-ainedirektiiviin ja käytännön kokemuksia saaneeseen kasvinsuojeluaine-teollisuuteen. Lyhyesti käsitellään myös joidenkin EU:n ulkopuolisten maiden kokemuksia. Johtopäätöksenä voitaneen todeta, että kemikaaliasetuksen tavoitteet, terveyttä ja ympäristöä suojelevat toimenpiteet, koetaan positiivisina asioina. Asioiden selvittely, valtava työmäärä ja korkeat kustannukset koettiin negatiivisiksi asioiksi. Markkinoiden uskotaan jakautuvan harvempien ja suurempien yritysten kesken, ja PK-yritysten toiminta-mahdollisuudet vähenevät oleellisesti. Tarkastus- ja testaus-asiantuntijoiden vähäinen määrä vaikuttaa jo nyt testausaikatauluihin ja tulee yhä enemmän vaikuttamaan tulevaisuudessa.

Työn tavoitteena oli selvittää pintaenergian vaikutus painovärin tarpeeseen IGT-laboratoriopainokoneella. Lisäksi selvitettiin onko karheudella vaikutusta kontaktikulmamittaukseen. Työ koostui kirjallisuusosasta ja kokeellisesta osasta. Kirjallisuusosassa tarkasteltiin aihepiiriä koskevia teorioita ja aiempia tutkimuksia. Kokeellisessa osassa valmistettiin laboratoriomittakaavassa pintaenergialtaan ja karheudeltaan erilaisia papereita. Pintaenergiaa muuteltiin kolmen erilaisen massaliiman ja kahden erilaisen pintaliiman avulla. Paperin karheutta säädettiin kalanteroinnilla. Papereista mitattiin pintaominaisuuksia, nesteen ja paperin välinen kontaktikulma, sekä suoritettiin painatuskokeita. Pintaenergia laskettiin kirjallisuudesta löytyneiden teorioiden perusteella ja näiden pintaenergian laskemismenetelmien käyttökelpoisuutta pintaenergian määrittämiseen selvitettiin. Lisäksi tutkittiin pintaenergian vaikutusta painovärin tarpeeseen. Tämä työ selvästi osoittaa, että yhden, kahden ja kolmen komponentin menetelmällä ja nesteitä vaihtelemalla saadaan erilaisia pintaenergian tuloksia. Pintaenergian määrittämiseen vaikuttaa myös kontaktikulmamittausten hankaluus ja kontaktikulman suuri vaihtelu, joka vaikuttaa koko mittaussarjan luotettavuustarkasteluun. Lisäksi paperin karheudella ja huokoisuudella oli merkitystä kontaktikulmamittaukseen.

An improved defoamer dosage procedure and a more efficient dosing point to the approach system were studied in this thesis. Their influence on paper machine wet end operations was investigated. The improved defoamer dosing procedure was examined at UMP-Kymmene Tervasaari PM8. Air content and its controlling methods at the paper machine were studied in the literature survey. Also the influence of dissolved gases and entrained air in the papermaking furnish were introduced. Feeding methods – a TrumpJet chemical mixer and traditional feeding devices – were reviewed. The defoamer’s functioning methods were studied. The influence of the use of defoamers was estimated based on the main selected wet end operations. In the experimental part, defoamer mixing with a traditional feeding method and two improved mixing stages were compared based on the air content profiles in PM8’s approach system. The reference dosage procedure was PM8’s old dosing system. The first dosage procedure in the comparison involved two TrumpJet chemical mixers installed on the bottom wire trays. The second element of comparison involved the improvement brought by a third TrumpJet chemical mixer installed on the top wire tray. This second comparison of the efficient defoamer feeding concept was made at a higher production speed of PM8. The air content control situation was also studied at the higher production speed. In addition the connection between the defoamer and air content was observed and a mill-scale system was studied. The economical benefits of the new dosing procedure were also reviewed. Air content profiles of short circulation were measured in the reference situation and the two comparison points of the study. These air content measurements proved the main gas load is introduced to PM8's paper furnish from the white water tray. Thick stock air content was not essential when the air volume flow was considered. The improved defoamer dosing procedure made lower dosage amounts possible. Compared with the traditional feeding system, the new defoamer feeding concept made only few direct improvements to the wet end operations and the produced paper itself. The lower defoamer need was noticed to have a positive influence on hydrophobic sizing and paper defects. The surfaces of the white water tanks and the operation of pumps were assessed based on the density variations of the suspension. The temperature in the white water silo was stated to have a significant influence on the air content measured in the first centrifugal cleaning stage.
