OBJECTIVE To analyze the factors associated with stiffness of the great arteries in prepubertal children.METHODS This study with convenience sample of 231 schoolchildren aged 9-10 years enrolled in public and private schools in Vitória, ES, Southeastern Brazil, in 2010-2011. Anthropometric and hemodynamic data, blood pressure, and pulse wave velocity in the carotid-femoral segment were obtained. Data on current and previous health conditions were obtained by questionnaire and notes on the child’s health card. Multiple linear regression was applied to identify the partial and total contribution of the factors in determining the pulse wave velocity values.RESULTS Among the students, 50.2% were female and 55.4% were 10 years old. Among those classified in the last tertile of pulse wave velocity, 60.0% were overweight, with higher mean blood pressure, waist circumference, and waist-to-height ratio. Birth weight was not associated with pulse wave velocity. After multiple linear regression analysis, body mass index (BMI) and diastolic blood pressure remained in the model.CONCLUSIONS BMI was the most important factor in determining arterial stiffness in children aged 9-10 years.

OBJECTIVE To analyze if metabolic syndrome and its altered components are associated with demographic, socioeconomic and behavioral factors in fixed-shift workers.METHODS A cross-sectional study was conducted on a sample of 902 shift workers of both sexes in a poultry processing plant in Southern Brazil in 2010. The diagnosis of metabolic syndrome was determined according to the recommendations from Harmonizing the Metabolic Syndrome. Its frequency was evaluated according to the demographic (sex, skin color, age and marital status), socioeconomic (educational level, income and work shift), and behavioral characteristics (smoking, alcohol intake, leisure time physical activity, number of meals and sleep duration) of the sample. The multivariate analysis followed a theoretical framework for identifying metabolic syndrome in fixed-shift workers.RESULTS The prevalence of metabolic syndrome in the sample was 9.3% (95%CI 7.4;11.2). The most frequently altered component was waist circumference (PR 48.4%; 95%CI 45.5;51.2), followed by high-density lipoprotein. Work shift was not associated with metabolic syndrome and its altered components. After adjustment, the prevalence of metabolic syndrome was positively associated with women (PR 2.16; 95%CI 1.28;3.64), workers aged over 40 years (PR 3.90; 95%CI 1.78;8.93) and those who reported sleeping five hours or less per day (PR 1.70; 95%CI 1.09;2.24). On the other hand, metabolic syndrome was inversely associated with educational level and having more than three meals per day (PR 0.43; 95%CI 0.26;0.73).CONCLUSIONS Being female, older and deprived of sleep are probable risk factors for metabolic syndrome, whereas higher educational level and higher number of meals per day are protective factors for metabolic syndrome in fixed-shift workers.

The effects of artemisinin-based combination therapies (ACTs) on transmission of Plasmodium falciparum were evaluated after a policy change instituting the use of ACTs in an endemic area. P. falciparum gametocyte carriage, sex ratios and inbreeding rates were examined in 2,585 children at presentation with acute falciparum malaria during a 10-year period from 2001-2010. Asexual parasite rates were also evaluated from 2003-2010 in 10,615 children before and after the policy change. Gametocyte carriage declined significantly from 12.4% in 2001 to 3.6% in 2010 (@@χ² for trend = 44.3, p < 0.0001), but sex ratios and inbreeding rates remained unchanged. Additionally, overall parasite rates remained unchanged before and after the policy change (47.2% vs. 45.4%), but these rates declined significantly from 2003-2010 (@@χ² for trend 35.4, p < 0.0001). Chloroquine (CQ) and artemether-lumefantrine (AL) were used as prototype drugs before and after the policy change, respectively. AL significantly shortened the duration of male gametocyte carriage in individual patients after treatment began compared with CQ (log rank statistic = 7.92, p = 0.005). ACTs reduced the rate of gametocyte carriage in children with acute falciparum infections at presentation and shortened the duration of male gametocyte carriage after treatment. However, parasite population sex ratios, inbreeding rates and overall parasite rate were unaffected.

Abstract Genetic studies have shown an association between schizophrenia and a GAG trinucleotide repeat (TNR) polymorphism in the catalytic subunit (GCLC) of the glutamate cysteine ligase (GCL), the key enzyme for glutathione (GSH) synthesis. The present study was aimed at analyzing the influence of a GSH dysregulation of genetic origin on plasma thiols (total cysteine, homocysteine, and cysteine-glycine) and other free amino acid levels as well as fibroblast cultures GSH levels. Plasma thiols levels were also compared between patients and controls. As compared with patients with a low-risk GCLC GAG TNR genotype, patients with a high-risk genotype, having an impaired GSH synthesis, displayed a decrease of fibroblast GSH and plasma total cysteine levels, and an increase of the oxidized form of cysteine (cystine) content. Increased levels of plasma free serine, glutamine, citrulline, and arginine were also observed in the high-risk genotype. Taken together, the high-risk genotypes were associated with a subgroup of schizophrenia characterized by altered plasma thiols and free amino acid levels that reflect a dysregulation of redox control and an increased susceptibility to oxidative stress. This altered pattern potentially contributes to the development of a biomarker profile useful for early diagnosis and monitoring the effectiveness of novel drugs targeting redox dysregulation in schizophrenia. El propòsit d’aquest Treball de Fi de Carrera (TFC) és analitzar la imatge de la dona en la publicitat. Per a realitzar-lo hem estudiat primerament el marc regulador per tal de saber els límits legals i ètics dins els quals ens movem. Posteriorment hem analitzat les queixes presentades a Autocontrol, FACUA i l’Observatori de la Imatge de les Dones durant el període 2003- 2010 tot classificant-les en funció de l’any, la marca, el sector i el suport publicitari utilitzat. També s’ha establert una categorització pròpia dels diferents tipus de rols de dona dins els quals es poden englobar el conjunt de queixes estudiades. Hem arribat a la conclusió de que gran part de les queixes van dirigides a l’Observatori de la Dona, i que la televisió i la dona objecte aglutinen el major nombre de denúncies. No obstant, durant el període de temps estudiat no s’ha observat cap tendència pel que fa al nombre de queixes presentades.

After the World Health Organization officially declared the end of the first pandemic of the XXI century in August 2010, the influenza A(H1N1)pdm09 virus has been disseminated in the human population. In spite of its sustained circulation, very little on phylogenetic data or oseltamivir (OST) resistance is available for the virus in equatorial regions of South America. In order to shed more light on this topic, we analysed the haemagglutinin (HA) and neuraminidase (NA) genes of influenza A(H1N1)pdm09 positive samples collected during the pandemic period in the Pernambuco (PE), a northeastern Brazilian state. Complete HA sequences were compared and amino acid changes were related to clinical outcome. In addition, the H275Y substitution in NA, associated with OST resistance, was investigated by pyrosequencing. Samples from PE were grouped in phylogenetic clades 6 and 7, being clustered together with sequences from South and Southeast Brazil. The D222N/G HA gene mutation, associated with severity, was found in one deceased patient that was pregnant. Additionally, the HA mutation K308E, which appeared in Brazil in 2010 and was only detected worldwide the following year, was identified in samples from hospitalised cases. The resistance marker H275Y was not identified in samples tested. However, broader studies are needed to establish the real frequency of resistance in this Brazilian region.

This cross-sectional study aimed to analyze the adherence to drug and non-drug treatments in 17 Family Health Strategy units. A total of 423 patients with type 2 diabetes mellitus were selected through stratified random sampling in Family Health Strategy units of a city in the state of Minas Gerais, Brazil, in 2010. The results showed that the prevalence rate of adherence to drug therapy was higher than 60% in the 17 units investigated; in relation to physical activity, adherence was higher than 60% in 58.8% units; and for the diet plan, there was no adherence in 52.9% units. Therefore, we concluded that adherence to drug therapy in most units was high and the practice of physical activity was heterogeneous, and in relation to diet adherence, it was low in all units. We recommend strengthening of institutional guidelines and educational strategies, in line with SUS guidelines, so that, professionals may face the challenges imposed by the lack of adherence.

Objective To evaluate the performance of diagnostic centers in the classification of mammography reports from an opportunistic screening undertaken by the Brazilian public health system (SUS) in the municipality of Goiânia, GO, Brazil in 2010. Materials and Methods The present ecological study analyzed data reported to the Sistema de Informação do Controle do Câncer de Mama (SISMAMA) (Breast Cancer Management Information System) by diagnostic centers involved in the mammographic screening developed by the SUS. Based on the frequency of mammograms per BI-RADS® category and on the limits established for the present study, the authors have calculated the rate of conformity for each diagnostic center. Diagnostic centers with equal rates of conformity were considered as having equal performance. Results Fifteen diagnostic centers performed mammographic studies for SUS and reported 31,198 screening mammograms. The performance of the diagnostic centers concerning BI-RADS classification has demonstrated that none of them was in conformity for all categories, one center presented conformity in five categories, two centers, in four categories, three centers, in three categories, two centers, in two categories, four centers, in one category, and three centers with no conformity. Conclusion The results of the present study demonstrate unevenness in the diagnostic centers performance in the classification of mammograms reported to SISMAMA from the opportunistic screening undertaken by SUS.

The definition of corporate social responsibility (CSR) has been developed since 1950s but even today there is no consensus what CSR includes. The main purpose of this thesis was to find out whether financial performance is better among first adopters of CSR standards in forest industry. To support the main purpose it was critical also investigate what kind of companies adopt CSR standards. The empirical part of the thesis based on a survey which was done in 2010 to forest industry companies and financial data that was gathered from different databases from years 2003-2010. According to the research results it seems the early CSR standards adopters benefits the position of the first adopter many times. Especially cash position and solvency of early adopter companies were better than later adopters or those who did not adopt CSR standards at all. Profitability seemed to be better among CSR standards adopters but early adopters did not have significantly better position compared to later adopters. CSR standards adopters were companies that considered themselves as environmental performance pioneers and had employee oriented management.

Corn is planted in the Center West region of Brazil as a second crop, following soybeans or beans. Intercropping of Brachiaria species with corn as a second crop increases the mulching in the cropping system. This study aimed to evaluate the weeds infestation in soybeans following corn/forages intercrop, as a function of corn plant structure, forage species and density. Experiments were conducted in a completely randomized blocks design with four replications, in Ponta Porã and Dourados municipalities, Mato Grosso do Sul state, Brazil, in 2010/2011. Treatments consisted of three corn hybrids with distinct plant architectures intercropped with three forage species: Brachiaria ruziziensis, B. brizantha and B.decumbens, at five densities, and the resulting dry mass was maintained throughout the winter. During the following cropping season, forages were desiccated prior to planting soybeans, and the dry mass of weeds, dry mass of the mulching, soil coverage by weeds, and the broadleaf/grass weed species index (WPI) were determined 15 days after soybean emergence, submitted to an F-test, and analyzed either by regression or by multiple mean comparison, according to the nature of the data. When intercropping corn with species of Brachiaria, a reduction in the overall weeds infestation may always be expected; among the studied forage species, more problems with weeds may be anticipated in areas with a less competitive species, e.g. B.ruziziensis. Under the conditions of the trials, B.brizantha and B.decumbens were more capable of inhibiting the emergence of weed species in the winter.

The impact of ventricular rate (VR) on the outcome of electrical cardioversion (ECV) of acute atrial fibrillation (AF) is currently unknown. We aimed to determine the effect of VR during acute AF on the success of ECV, recurrence of AF and occurrence of post-cardioversion complications in 30 days follow-up. All ECVs performed in patients with acute atrial fibrillation lasting <48 hours in 2 Finnish university hospitals during 2003-2010 and 1 central hospital during 2010 were retrospectively identified. A total of 6,624 ECVs were performed in 2,821 consecutive patients. VR≤60 BPM was defined low and VR≥160 BPM high. The median VR before ECV was 109 BPM. The success rate of ECV was 94.2%. Bradycardia occurred in 62 (0.9%) and thromboembolic complications in 39 (0.6%) ECVs. Low VR was observed before 75 (1.1%) ECVs and male sex was its only independent predictor. High VR was observed in 165 (2.5%) ECVs. The independent predictors of high VR were younger age, <12 h episode duration, no previous history of AF and alcohol abuse. Low or high VR were not related to the success of ECV, incidence of thromboembolic or bradycardic complications, or recurrence of AF, although VR was significantly (p<0.001) lower in the patients in whom AF recurred. In conclusion, ECV of acute AF is an effective procedure and VR during AF does not affect its efficacy, the maintenance of sinus rhythm or the incidence of bradycardic, thromboembolic or other complications during 30 days follow-up after ECV. Low VR is predominately observed in male patients, while high VR was a feature related to a shorter history of AF and high alcohol-intake.

Symbole de modernité pendant la majeure partie du XXe siècle, la cigarette est depuis une trentaine d’années la cible d’interventions croissantes visant à réduire sa consommation. La lutte contre le tabac bénéficie d’un dispositif sans précédent qui fait office de cas d’école en santé publique, par son ampleur et par l’adhésion qu’il suscite. L’intérêt d’étudier cet objet réside ainsi dans la propriété essentielle de la lutte anti-tabac de relier un dispositif institutionnel et des motivations subjectives. Le dispositif anti-tabac (DAT) a en effet vocation à faire converger les prescriptions normatives d’un ensemble d’institutions et les désirs individuels, et y parvient manifestement dans une certaine mesure. Il permet dès lors d’aborder à la fois un travail sur les sociétés et un travail sur soi. Cette thèse entreprend une analyse sociologique du dispositif anti-tabac au Québec et vise à interroger les modalités et les fins de ce contrôle public de la consommation de tabac, en mettant au jour ses dimensions culturelles, symboliques et politiques. La santé publique apparaissant de nos jours comme lieu central de l’espace politique et social dans les sociétés contemporaines (Fassin et Dozon, 2001 :7), l’utopie d’un « monde sans fumée » se révèle selon nous tout à fait typique des enjeux qui caractérisent la modernité avancée ou « société du risque » (Beck, 2001, [1986]). Après avoir présenté le rapport historiquement ambivalent des pays occidentaux au tabac et ses enjeux, puis problématisé la question de la consommation de substances psychotropes dans le cadre d’une production et d’une construction sociale et culturelle (Fassin, 2005a), nous inscrivons le DAT dans le cadre d’une biopolitique de la population (Foucault, 1976; 1997; 2004b). À l’aune des enseignements de Michel Foucault, cette thèse consiste ainsi en l’analyse de discours croisée du dispositif institutionnel anti-tabac et de témoignages d’individus désirant arrêter de fumer, au regard du contexte social et politique de la société moderne avancée. Le DAT illustre les transformations à l’œuvre dans le champ de la santé publique, elles-mêmes caractéristiques d’une reconfiguration des modes de gouvernement des sociétés modernes avancées. La nouvelle biopolitique s’adresse en effet à des sujets libres et entreprend de produire des citoyens responsables de leur devenir biologique, des sujets de l’optimisation de leurs conditions biologiques. Elle s’appuie sur une culpabilité de type « néo-chrétien » (Quéval, 2008) qui caractérise notamment un des leviers fondamentaux du DAT. Ce dernier se caractérise par une lutte contre les fumeurs plus que contre le tabac. Il construit la figure du non-fumeur comme celle d’un individu autonome, proactif et performant et fait simultanément de l’arrêt du tabac une obligation morale. Par ce biais, il engage son public à se subjectiver comme citoyen biologique, entrepreneur de sa santé. L’analyse du DAT au Québec révèle ainsi une (re)moralisation intensive des questions de santé, par le biais d’une biomédicalisation des risques (Clarke et al., 2003; 2010), particulièrement représentative d’un nouveau mode d’exercice de l’autorité et de régulation des conduites dans les sociétés contemporaines, assimilée à une gouvernementalité néolibérale. Enfin, l’analyse de témoignages d’individus engagés dans une démarche d’arrêt du tabac révèle la centralité de la santé dans le processus contemporain d’individuation. La santé publique apparait alors comme une institution socialisatrice produisant un certain « type d’homme » centré sur sa santé et adapté aux exigences de performance et d’autonomie prévalant, ces éléments constituant désormais de manière croissante des clés d’intégration et de reconnaissance sociale.

Esta investigación es la reconstrucción de 7 años de actividades (2003-2010) generadas en un centro cultural ubicado en la calle 32 con carrera 13 en la ciudad de Bogotá. Este proyecto contiene textos periodísticos, académicos, entrevistas, archivos de audio y fotografías. Además de reconstruir la historia de lo ocurrido en este edificio, la investigación estudia las relaciones entre colectivos artísticos, políticos y juveniles en el contexto social bogotano.
