Introdução: A tuberculose continua um problema de saúde pública emergente. A incidência da tuberculose genito-urinária tem vindo a aumentar sendo o segundo local mais comum de infecção a seguir aos pulmões. Caso clínico: Os autores apresentam dois casos de mulheres com hemorragia vaginal pós-menopausa. O exame clínico não revelou alterações. A ecografia pélvica endocavitária detectou a presença de uma lâmina líquida na cavidade endometrial em ambos os casos. A histeroscopia identificou espessamentos focais do endométrio que foram biopsados. O exame anatomopatológico revelou granulomas de células epitelióides sem atipia celular. O exame cultural do endométrio foi positivo para Mycobacterium tuberculosis. Não foi detectado envolvimento de outros órgãos tendo as doentes iniciado terapêutica antibacilar. Conclusão: A tuberculose genital é rara na mulher pós-menopausa sendo responsável por cerca de 1% da hemorragia vaginal pós-menopausa. No entanto, é uma doença curável cujo diagnóstico precoce é importante, prevenindo a utilização de procedimentos invasivos desnecessários.

Female genital mutilation, also named female genital cutting or female circumcision, refers to all practices involving total or partial removal of female external genitalia, or causing other lesions without an established health benefit. The World Health Organization estimates that 130 million women have been submitted to genital mutilation and 3 million are annually exposed to such risk in Africa. It has classified the practice of female genital mutation in four types. Portugal is considered a risk country for the practice of genital mutilation because of the high migration rates from the African continent, and women from Guinea-Bissau are at particular risk. A multidisciplinary commission published an Action Program in 2009, with measures directed at providing healthcare professionals with information on how to deal with this problem.

Portugal is considered by the World Health Organization (WHO) a risk country for the practice of Female Genital Mutilation (FGM). Objectives: To evaluate the knowledge that health professionals from Maternity Dr. Alfredo da Costa (MAC) have regarding FGM. Population and Methods: Analysis of surveys delivered to health professionals from MAC (a hospital dedicated to reproductive health), between April and June 2008, addressing issues related to the knowledge about FGM. Results: Authors collected 112 valid surveys involving 38 doctors, 48 nurses and 26 medical auxiliaries/administrative personnel. From the respondents, 106 (95%) had heard about FMG practice before, the media being the most reported source of information; 59 (53%) replied they could be able to recognize FGM cases in their clinical practice; however, only 31 (28%) claimed to know the FGM type classiication and 32 (29%) admitted to be prepared to recognize and manage these situations in their own clinical practice; 9 had been consulted explicitly by a FGM practice complication and 1 doctor had admitted having been asked to perform/execute FGM; 13 (12%) recognized that the Portuguese legislation its this practice. Regarding the practice of FGM, 100 (89%) of respondentes stated that it should not be maintained and 97 (87%) stated that it should not be tolerated. However, 42 (38%) considered that if these practices were a reality, then they should be medical assisted. Discussion: Health professionals can play an important role in eliminating the practice of FGM, not only by the proper clinical management of this situation, but also by preventing those communities at risk to resort to FGM. Most health professionals are not prepared to deal with FGM in their clinical practice. It is important to promote a better knowledge on the subject and to create protocols for proper clinical management.

BACKGROUND: Both primary and secondary gynaecological neuroendocrine (NE) tumours are uncommon, and the literature is scarce concerning their imaging features. METHODS: This article reviews the epidemiological, clinical and imaging features with pathological correlation of gynaecological NE tumours. RESULTS: The clinical features of gynaecological NE tumours are non-specific and depend on the organ of origin and on the extension and aggressiveness of the disease. The imaging approach to these tumours is similar to that for other histological types and the Revised International Federation of Gynecology and Obstetrics (FIGO) Staging System also applies to NE tumours. Neuroendocrine tumours were recently divided into two groups: poorly differentiated neuroendocrine carcinomas (NECs) and well-differentiated neuroendocrine tumours (NETs). NECs include small cell carcinoma and large cell neuroendocrine carcinoma, while NETs account for typical and atypical carcinoids. Cervical small cell carcinoma and ovarian carcinoid are the most common gynaecological NE tumours. The former typically behaves aggressively; the latter usually behaves in a benign fashion and tends to be confined to the organ. CONCLUSION: While dealing with ovarian carcinoids, extra-ovarian extension, bilaterality and multinodularity raise the suspicion of metastatic disease. NE tumours of the endometrium and other gynaecological locations are very rare. TEACHING POINTS: • Primary or secondary neurondocrine (NE) tumours of the female genital tract are rare. • Cervical small cell carcinoma and ovarian carcinoids are the most common gynaecological NE tumours. • Cervical small cell carcinomas usually behave aggressively. • Ovarian carcinoids tend to behave in a benign fashion. • The imaging approach to gynaecological NE tumours and other histological types is similar.

Foram examinadas prospectivamente189 amostras cervicais de mulheres sintomáticas e assintomáticas. Foram colhidas 2 amostras do canal endocervical, das quais uma foi examinada pela reação de imunofluorescência direta (IFD) com anticorpo monoclonal (MicroTrak), para verificação da adequação das amostras. A segunda amostra foi inoculada em cultura de células McCoy. Uma terceira amostra foi coletada para pesquisa de anticorpos das classes IgG e IgA. A Chlamydia trachomatis foi isolada de 14/166 (8,4%) das mulheres com sintomas e de 3/23 (13%) daquelas sem sintomas. Observamos que as 152 mulheres do grupo sintomático, com cultura negativa, possuiam sintomas equivalentes. Em relação ao número de células epiteliais, verificou-se que 13 das 17 (76,5%) amostras endocervicais positivas pela cultura e pela IFD, todas apresentavam mais de 5 células. Tomando-se como critério de positividade títulos ³ 1:8, foram detectados anticorpos das classes IgG e/ou IgA específicos para C. trachomatis em 11/189 (64,7%) das 17 mulheres com cultura positiva. Conclusões: a) não existe sintoma que seja específico de infecção por clamídia (p > 0,05); b) a quantidade de células epiteliais representariam fator de interferência na positividade da cultura, sendo, portanto, variáveis dependentes (p < 0,001); c) a pesquisa de anticorpos na cérvice não poderia ser utilizada como diagnóstico alternativo, pois a sua detecção depende da fase evolutiva da infecção e da resposta imunitária individual.

RESUMO - A infeção por Vírus do Papiloma Humano (HPV) constitui uma das infeções de transmissão sexual mais comuns em mulheres adolescentes e jovens sexualmente ativas, sendo responsável por 99,7% dos casos de cancro do colo do útero (CCU). O desenvolvimento de duas vacinas de segurança e eficácia comprovadas e a introdução da vacinação contra os HPV 6, 11, 16 e 18 nos programas de imunização de diversos países constituiu um marco muito importante para a investigação no âmbito do conhecimento das populações nesta temática. O presente trabalho de projeto teve como objetivo geral avaliar o conhecimento dos adolescentes/jovens entre os 15 e os 19 anos acerca da infeção genital por HPV. Foi efetuado um estudo piloto cuja amostra foi constituída por 20 jovens, género feminino, que acorreram à Consulta de Obstetrícia e Ginecologia do Adolescente dos Serviços de Assistência Médico-Social do Sindicato dos Bancários do Sul e Ilhas, acompanhadas dos respetivos encarregados de educação. Foi-lhes solicitado o consentimento informado e entregue um questionário de caráter anónimo e confidencial com questões para avaliação do conhecimento geral acerca da infeção por HPV, formas de transmissão, manifestações clínicas, prevenção, relação entre HPV e CCU e necessidades de informação. Este estudo piloto poder-se-á revelar útil na aferição de resultados quando aplicado em larga escala, para posterior avaliação comparativa do conhecimento no âmbito do HPV. Pode igualmente ser útil no desenvolvimento de mensagens apropriadas que acompanham os programas de rastreio nesta coorte, no futuro, com vista ao aumento da literacianos jovens.

Female genital schistosomiasis is not uncommon in endemic areas for schistosomiasis, but there are few reports in the Brazilian medical literature. Here, we describe the case of a 31-year-old woman with lower abdominal pain who was diagnosed as presenting a fallopian tube tumor caused by Manson's schistosomiasis. The diagnosis was delayed because her symptoms were considered nonspecific. Involvement of the parietal peritoneum of the ovarian fossa was observed during laparoscopy and confirmed by histological analysis. The left tube and the tumor were excised and schistosomiasis was treated with praziquantel. She presented a full recovery and options for future reproduction are under evaluation.

Phlebotomine sand flies are insects of medical importance. Species in the Neotropical region are highly diverse. Some of these species are considered cryptic species because of their morphological similarity between adult females of different species make identification especially difficult. The aim of this study was to analyze and describe the armature in the genital atrium (AGA) of some adult female sand flies, in order to discover new taxonomic characters that make it possible to distinguish between species that would otherwise be treated as cryptic by analysis of the AGA. The AGA of 16 Phlebotomine sand fly species are described. Distinct differences were found in relation to the shape and size of the armature, the presence or absence of spines on the armature, and the shape, size, and grouping patterns of the spines. These characters made it possible to distinguish between the species studied.

OBJETIVO: Realizar uma revisão da literatura sobre o transtorno da excitação genital persistente (TEGP), um quadro clínico que acomete somente mulheres, recentemente descrito na literatura, caracterizado por sinais fisiológicos de excitação sexual sem a presença de desejos ou estímulos sexuais. MÉTODOS: Foi realizada revisão sistemática com busca nas bases científicas PubMed, ISI, SciELO e PsycInfo. Do total de artigos encontrados, 27 foram selecionados para integrar esta revisão. RESULTADOS: Os artigos, em sua maioria, são relatos de casos. Apesar de algumas hipóteses diagnósticas terem sido propostas, ainda não existe consenso sobre etiologia, fatores de risco e epidemiologia desse transtorno. A literatura aponta para uma correlação positiva entre a presença do transtorno e quadros depressivos, ansiosos e sintomas obsessivo-compulsivos. CONCLUSÃO: Há necessidade de estudos mais amplos para o melhor entendimento desse quadro clínico. É possível que muitos casos passem despercebidos pelos profissionais de saúde por desconhecimento dos critérios diagnósticos.

Zearalenone (ZEN) is a mycotoxin that has relatively low acute toxicity. However, it is a potent oestrogen, interfering with the reproductive tract of animals. Among other effects, ZEN decreases animals fertility, and induces fibrosis in the uterus, breast cancer and endometrial carcinoma (Zinedine et al., 2007). Anti-mycotoxin additives (AMA) are defined as a group of products that, when added to animal feed, are capable of adsorbing, inactivating, or neutralizing mycotoxins in the gastrointestinal tract of animals. One example of these products are adsorbents based on yeast cell walls, a safe and beneficial animal feed additive (Abreu et al., 2008). When based on active cells, yeast based products also act as a probiotic, contributing to improve the general animal health because it stimulates their immune system and promotes the integrity of intestinal mucosa (Albino et al., 2006). Strains of Saccharomyces cerevisiae isolated from silage were tested for their ZEN removal capability. Their effect on - and b-zearalenol (-ZOL and b-ZOL) was also tested. Strains were grown on YPD separately supplemented with ZEN, -ZOL and b-ZOL, and their elimination from culture media was quantified over time by HPLC-FL.

OBJECTIVE: To describe a surgical procedure utilizing a malleable bovine pericardium ring in mitral valve repair and clinical and echodopplercadiographic results. METHODS: Thirty-two (25 female and 7 male) patients, aged between 9 and 66 (M=36.4±17.2) years, were studied over a 16-month period, with 100% follow-up. In 23 (72%) of the patients, the mitral approach was the only one applied; 9 patients underwent associated operations. The technique applied consisted of measuring the perimeter of the anterior leaflet and implanting, according to this measurement, a flexible bovine pericardium prosthesis for reinforcement and conformation of the posterior mitral annulus, reducing it to the perimeter of the anterior leaflet with adjustment of the valve apparatus. RESULTS: The patient survival ratio was 93.8%, with 2 (6.2%) fatal outcomes, one from unknown causes, the other due to left ventricular failure. Only one reoperation was performed. On echodopplercardiography, 88% of the patients had functional recovery of the mitral valve (50% without and 38% with mild insufficiency and no hemodynamic repercussions). Of four (12%) of the remaining patients, 6% had moderate and 6% had seigre insufficiency. Twenty-eight percent of class II patients and 72% of class III patients passed into classes I (65%), II (32%), and III (3%), according to NYHA classification criteria. CONCLUSION: Being flexible, the bovine pericardium ring fit perfectly into the valve annulus, taking into account its geometry and contractility. Valve repair was shown to be reproducible, demonstrating significant advantages during patient evolution, which did not require anticoagulation measures.
