La Hepatitis C y B, junto al alcoholismo, continúan siendo un verdadero problema de Salud Pública. Sin embargo, actualmente no existen datos locales que nos permitan estimar la prevalencia de infección por virus hepatotropos en pacientes alcoholistas, sus genotipos, distrubución geográfica, ni su asociación con determinado tipo de alcoholismo. Además, la co-infección del virus de la inmunodeficiencia humana (VIH) con los virus de hepatitis supone un impacto muy importante desde el punto de vista sanitario y estadÃstico en nuestro paÃs, ya que alrededor del 50 por ciento de los pacientes VIH positivos presentan dicha coinfección. Es asà que, por ser de interés sanitario y por compartir vÃas de transmisión con el virus de hepatitis B y C, nos parece adecuado estudiar también la presencia de VIH-1 en esta población. Nuestro centro de atención pública (IPAD), atiende a pacientes con trastornos por el consumo de sustancias; deshabitúa y rehabilita alcoholistas y a otros trastornos por consumo. Dichos pacientes, que viven en la ciudad capital, serán evaluados serológicamente para la detección de virus hepatotropos C-B y el VIH. Considerando que en nuestra institución se atiende a un 70 % de Alcoholistas Puros (con un promedio de 7 pacientes nuevos por dÃa), nos resulta importante pesquisar la prevalencia de Virus C, B y VIH en nuestra población de alcoholistas. Toda esta problemática, es la propuesta de mi tesis doctoral. Hipótesis: estimamos encontrar en nuestra población de estudio cifras superiores a la prevalencia de estos virus publicada en bancos de sangre, lo cual se toma como referencia. Objetivos: -Conocer la prevalencia de infección por Virus de Hepatitis C, B y VIH-1 en pacientes alcoholistas de la ciudad de Córdoba, determinar si existe asociación de estos virus con algún tipo de alcoholismo, e identificar genotipos prevalentes y su distribución geográfica en Córdoba. Se incluirán en forma prospectiva y aleatorea, pacientes que concurren por primera vez, de ambos sexos, mayores de 21 años, alcoholistas puros (Gama-Delta-Epsilon de Jellinek). Se confeccionará una ficha, previo consentimiento informado, que permitirá categorizar al "tipo de bebedor". Se les realizará SerologÃa para HCV, Ag HBs (en caso de reactividad se adicionará el Anti HBcore) y VIH. En caso de la positividad serológica, se procederá al frisado de los mismos, para la Genotipificación correspondiente. La recolección, captura y procesamiento de los datos se realizarán en una planilla o ficha. Luego se reubicarán en una base electrónica de datos y se harán los análisis estadÃsticos de los mismos. Resultados esperados: estimamos encontrar, coincidiendo con la bibliografÃa, un aumento en la prevalencia de estos virus. Creemos que pueden existir diferencias en los distintos tipos de alcoholismo debido a las diversas situaciones de riesgo a las que se exponen (más exposición en el Gama de Jellineck). Por esto esperamos encontrar un aumento en la prevalencia del Virus C, especialmente en el tipo consuetudinario (delta de Jellineck) por los trastornos nutritivos derivados del modo de consumo. Posiblemente esto pueda ser la llave de otros estudios que puedan esclarecer una vÃa de transmisión desconocida para este virus. De la misma manera, identificar los distintos genotipos existentes en nuestra ciudad y su distribución, y que como sabemos tiene implicancia en la evolución, y en los costos por el tiempo de tratamiento. Esta información será un aporte para programar medidas de vigilancia epidemiológica adecuada, elaborar estrategias preventivas además de aplicar el tratamiento correspondiente a los pacientes infectados que se detecten como tal durante el desarrollo del proyecto.

The writers report results on the application of four fumigants (D. D., E. D. B.-40, C. B. P. and Vapam) for control of root-knot and meadow nematodes attacking potato in beds filled with soil artificially inoculated. The data obtained were as follows: a) as reported by previous authors, potato is sensitive to C.B.P., the toxical effects of which disapearing only about 6 and half months after application. On the other hand, C.B.P. proved to have a significative residual nematicidal value, protecting the seeds from root-knot nematodes for a period of two years; b) D. D., E. D. B., and Vapam were effective for controling root-knot but with no residual value, having to be used prior to each planting; c) at the rates used, no nematicide was effective to control meadow nematodes; d) in the conditions of the experiments, all nematicides incited attacks bv Streptomyces scabies. Actually, in some cases scab did not affect any tuber from the check while the entire production from the treated beds was heavily desfigured. The writers assume that as the nematicides killed protozoa and too many bacteria-eating nematodes, they destroyed the biological equilibrium existing in the soil, thus allowing the S. scabies population to reach a high level.

The prevalence, virological and epidemilogical aspects of the hepatitis C virus (HCV) and the hepatitis B virus (HBV) infections vary among hemodialysis patients in different countries. Aiming at analyzing these aspects of HCV and HBV infections in hemodialysis patients in Belo Horizonte, MG, Brazil, we studied three hemodialysis units including 434 patients. Serology was used to detect anti-HCV and HBsAg. Reverse trancriptase nested polymerase chain reaction (RT-nested-PCR) of the 5'-noncoding region was used to detect circulating HCV RNA and restriction fragment length polymorphism analysis for genotyping. Seroprevalence varied from 26.5% to 11.1% for hepatitis C and from 5.9% to 0% for hepatitis B. Risk factors observed for HBV and/or HCV infections were the number of patients per dialysis unit, duration of treatment, number of clinics attended, number of blood units transfused, and lower level scholarity. Alanine aminotransferase levels were altered with a higher frequency in HBV or HCV seropositive patients. Half of ten patients, negative for anti-HCV, had detectable viremia by RT-nested-PCR, indicating that this technique should be used to confirm infections in this group of patients. The HCV genotype 1 was the most frequently observed, followed by the genotype 2, but no correlation was detected between genotype and clinical or epidemiological data.

We set out to determine the seroprevalence of hepatitis B and C among human immunodeficiency virus type-1 (HIV-1) infected individuals in North-Central Nigeria to define the influence of these infections on CD4+ lymphocytes cells among our patients as access to antiretroviral therapy improves across the Nigerian nation. The CD4+ values of 180 confirmed HIV-1 infected individuals were enumerated using a superior fluorescence-activated cell sorter system. These patients were tested for the presence of hepatitis B surface antigen and anti-hepatitis C virus (HCV) using third generation enzyme-linked immunosorbent assays. Fifty (27.8%) patients had active hepatitis B virus (HBV) infection while 33 (18.3%) tested positive for anti-HCV antibody. Of these infections, 110 (61.1%), 37 (20.6%), and 20 (11.1%) had HIV only, HBV/HIV-only, and HCV/HIV-only respectively. A HBV/HCV/HIV coinfection prevalence of 7.2% (13 patients) was recorded. Patients coinfected with HIV/HBV/HCV appeared to have lower CD4+ counts (mean = 107 cells/µl; AIDS defining) when compared to HBV/HIV-only (mean = 377 cells/µl), HCV/HIV-only (mean = 373 cells/µl) and patients with mono HIV infection (mean = 478 cells/µl). Coinfection with HBV or HCV is relatively common among HIV-infected patients in Nigeria and should be a big consideration in the initiation and choice of therapy.

In this study, we analysed the frequency of micronuclei (MN), nucleoplasmic bridges (NPBs) and nuclear buds (NBUDs) and evaluated mutagen-induced sensitivity in the lymphocytes of patients chronically infected with hepatitis B virus (HBV) or hepatitis C virus (HCV). In total, 49 patients with chronic viral hepatitis (28 HBV-infected and 21 HCV-infected patients) and 33 healthy, non-infected blood donor controls were investigated. The frequencies (8240;) of MN, NPBs and NBUDs in the controls were 4.41 ± 2.15, 1.15 ± 0.97 and 2.98 ± 1.31, respectively. The frequencies of MN and NPBs were significantly increased (p < 0.0001) in the patient group (7.01 ± 3.23 and 2.76 ± 2.08, respectively) compared with the control group. When considered separately, the HBV-infected patients (7.18 ± 3.57) and HCV-infected patients (3.27 ± 2.40) each had greater numbers of MN than did the controls (p < 0.0001). The HCV-infected patients displayed high numbers of NPBs (2.09 ± 1.33) and NBUDs (4.38 ± 3.28), but only the HBV-infected patients exhibited a significant difference (NPBs = 3.27 ± 2.40, p < 0.0001 and NBUDs = 4.71 ± 2.79, p = 0.03) in comparison with the controls. Similar results were obtained for males, but not for females, when all patients or the HBV-infected group was compared with the controls. The lymphocytes of the infected patients did not exhibit sensitivity to mutagen in comparison with the lymphocytes of the controls (p = 0.06). These results showed that the lymphocytes of patients who were chronically infected with HBV or HCV presented greater chromosomal instability.

Um número significativo de mortes no trauma ocorre dias a semanas após a injúria inicial, sendo causado por infecções e insuficiência orgânica, relacionadas a hipercatabolismo e consequente desnutrição proteica aguda. A terapia nutricional deve ser planejada e incluÃda com as demais condutas de reanimação para pacientes politraumatizados e grandes queimados. A rápida aquisição de uma via para suporte nutricional é importante para inicio da terapia nutricional precoce em até 48 horas do atendimento. A via enteral é a opção preferencial no pós-operatório de pacientes traumatizado mas a via parenteral deve ser prescrita quando a enteral está contraindicada ou insuficiente. Após as medidas iniciais ditadas pelo ATLS, sintetizadas em A (air), B (breath), C (circulation), D (disability) e E (exposure), nós incluÃmos a letra F (feed) para enfatizar a importância do atendimento nutricional precoce no trauma.

Maculalactones A, B and C from the marine cyanobacterium Kyrtuthrix maculans are amongst the only compounds based on the tribenzylbutyrolactone skeleton known in nature and (+) maculalactone A from the natural source possesses significant biological activity against various marine herbivores and marine settlers. We now report a concise synthesis of racemic maculalactone A in five steps from inexpensive starting materials. Maculalactones B and C were synthesized by a minor modification to this procedure, and the synthetic design also permitted an asymmetric synthesis of maculalactone A to be achieved in around 85% ee. The (+) and (-) enantiomers of maculalactone A were assigned, respectively, to the S and R configurations on the basis of the chiral selectivity expected for catecholborane reduction of an unsymmetrical ketone in the presence of Corey's oxazoborolidine catalyst. Surprisingly, it appeared that natural (+) maculalactone A was biosynthesized in K. maculans in a partially racemic form, comprising ca. 90-95% of the (S) enantiomer and 5-10% of its (R) enantiomer. Coincidentally therefore, the percentage enantiomeric excess of the product obtained from asymmetric synthesis almost exactly matched that found in nature. (C) 2004 Elsevier Ltd. All rights reserved.

First measurements of the differential cross sections d(3)sigma/(dp(T)(gamma)dy(gamma)dy(jet)) for the inclusive production of a photon in association with a heavy quark (b, c) jet are presented, covering photon transverse momenta 30 < p(T)(gamma)< 150 GeV, photon rapidities |y(gamma)|< 1.0, jet rapidities |y(jet)|< 0.8, and jet transverse momenta p(T)(jet)> 15 GeV. The results are based on an integrated luminosity of 1 fb(-1) in pp collisions at s=1.96 TeV recorded with the D0 detector at the Fermilab Tevatron Collider. The results are compared with next-to-leading order perturbative QCD predictions.

As hepatites virais representam um importante problema de saúde pública no Brasil e no mundo. A hepatite B e a hepatite C são as de principal interesse para os profissionais da área de saúde em função do modo de transmissão e da possibilidade de aquisição ocupacional. O presente trabalho teve por principal objetivo verificar a soroprevalência da infecção pelo VÃrus da Hepatite B (VHB) e pelo VÃrus da Hepatite C (VHC) em cirurgiões-dentistas e relacioná-la com os fatores de risco. Participaram do estudo 97 cirurgiões-dentistas, sendo 39 do gênero masculino e 58 do gênero feminino, no perÃodo de junho a dezembro de 2005, que atuam no municÃpio de Belém, Pará, Brasil. Os dados epidemiológicos foram obtidos por meio de inquéritos e os sorológicos por um ensaio imunoenzimático para a pesquisa de antÃgeno e anticorpo tanto para o VHB quanto para o VHC. O teste de Tendência foi utilizado para a análise estatÃstica dos resultados. A prevalência do VHB foi de 6,2%, enquanto do VHC foi de 3,1% entre a população estudada. Das amostras sororreativas para os marcadores da hepatite B, a prevalência foi de 1,03% (1/97) para o anti-HBc total, 5,16% (5/97) para a presença simultânea do anti-HBc total e anti-HBs e 54,61% (53/97) para o anti-HBs. Quando comparado ao encontrado na população de doadores de sangue no Estado do Pará a prevalência do VHC na população estudada foi significativamente maior, enquanto a do VHB foi semelhante. Além disso, 37,7% (36/97) relataram terem tido algum tipo de exposição ocupacional, estando o acidente com objetos pérfuro-cortante como o mais relatado (86,1%). A medida adotada após a exposição foi sempre a lavagem com água e sabão e apenas 2,8% (1/36) dos acidentados relataram à realização de testes sorológicos. O conhecimento das normas de biossegurança e a utilização de pelo menos uma barreira de proteção individual foram relatados por todos, sendo o uso de luvas e máscara a resposta mais mencionada (96,9%). Encontravam-se imunizados contra o VHB por meio de vacina 54,61% (53/97) dos cirurgiões-dentistas, enquanto que imunes por infecção natural 5,16% (5/97). O elevado percentual de ocorrência de acidentes ocupacionais, aliado a baixa soroconversão pós-exposição dentre os cirurgiões-dentistas participantes demonstra a necessidade de se conhecer a prevalência de infecções de risco ocupacional em profissionais da área de saúde para que se adotem medidas de prevenção e controle mais eficazes contra os agentes causadores.

As hepatites B e C continuam sendo um importante problema de saúde pública no Brasil. Neste estudo, determinou-se a prevalência de marcadores sorológicos para as hepatites B e C em indivÃduos do Estado do Pará, atendidos no Laboratório Central de Saúde Pública do Pará, no perÃodo de janeiro de 2002 a dezembro de 2005. Foram realizados 11.282 exames para a pesquisa do HBsAg, 2.342 para o anti-HBc e 5.542 para o anti-vÃrus da hepatite C. A prevalência de HBsAg foi de 3,6% e predominou na faixa etária de 20 a 29 anos, enquanto que o anti-HBc foi observado em 37,7% dos indivÃduos. A prevalência do antivÃrus da hepatite C foi de 3,6% e predominou entre indivÃduos acima de 50 anos. Assim, as freqüências dos marcadores encontradas no Pará foram mais altas que em vários outros estados do paÃs, sugerindo a necessidade de medidas de saúde publica mais eficazes no combate a estes agravos na região.

BACKGROUND: The surfactant proteins B (SP-B) and C (SP-C) are important for the stability and function of the alveolar surfactant film. Their involvement and down-regulation in inflammatory processes has recently been proposed, but their level during neutrophilic human airway diseases are not yet known. METHODS: We used 1D-electrophoresis and Western blotting to determine the concentrations and molecular forms of SP-B and SP-C in bronchoalveolar lavage (BAL) fluid of children with different inflammatory airway diseases. 21 children with cystic fibrosis, 15 with chronic bronchitis and 14 with pneumonia were included and compared to 14 healthy control children. RESULTS: SP-B was detected in BAL of all 64 patients, whereas SP-C was found in BAL of all but 3 children; those three BAL fluids had more than 80% neutrophils, and in two patients, who were re-lavaged later, SP-C was then present and the neutrophil count was lower. SP-B was mainly present as a dimer, SP-C as a monomer. For both qualitative and quantitative measures of SP-C and SP-B, no significant differences were observed between the four evaluated patient groups. CONCLUSION: Concentration or molecular form of SP-B and SP-C is not altered in BAL of children with different acute and chronic inflammatory lung diseases. We conclude that there is no down-regulation of SP-B and SP-C at the protein level in inflammatory processes of neutrophilic airway disease.

Members of the NF-κB/Rel and inhibitor of apoptosis (IAP) protein families have been implicated in signal transduction programs that prevent cell death elicited by the cytokine tumor necrosis factor α (TNF). Although NF-κB appears to stimulate the expression of specific protective genes, neither the identities of these genes nor the precise role of IAP proteins in this anti-apoptotic process are known. We demonstrate here that NF-κB is required for TNF-mediated induction of the gene encoding human c-IAP2. When overexpressed in mammalian cells, c-IAP2 activates NF-κB and suppresses TNF cytotoxicity. Both of these c-IAP2 activities are blocked in vivo by coexpressing a dominant form of IκB that is resistant to TNF-induced degradation. In contrast to wild-type c-IAP2, a mutant lacking the C-terminal RING domain inhibits NF-κB induction by TNF and enhances TNF killing. These findings suggest that c-IAP2 is critically involved in TNF signaling and exerts positive feedback control on NF-κB via an IκB targeting mechanism. Functional coupling of NF-κB and c-IAP2 during the TNF response may provide a signal amplification loop that promotes cell survival rather than death.
