Le système de santé est aujourd’hui marqué d’une complexité attribuable aux maladies chroniques, dont la hausse anticipée de la prévalence génère l’urgence d’une organisation et d’une approche différentes des services dispensés. Ce réaménagement des soins de santé fait appel à un changement de paradigme basé sur la collaboration interprofessionnelle et l’ouverture à l’incertitude, aptes à favoriser une mise en commun des connaissances. Dans cette perspective, les communautés de pratique (CdeP) semblent un moyen qui favorise le développement de la collaboration interprofessionnelle et le partage des connaissances. Elles apparaissent donc comme une formule qui mérite une investigation, à savoir si elles peuvent favoriser une telle collaboration et faciliter la co-construction de connaissances permettant une pratique évolutive, centrée sur le patient. Notre recherche s’inscrit dans cette optique et s’intéresse aux expériences, rapportées dans la littérature, de collaboration interprofessionnelle au sein de communautés de pratique centrées sur le patient atteint de maladie chronique. Pour ce faire, nous avons d’abord développé un cadre conceptuel visant à identifier les fondements théoriques et conceptuels de la notion de communauté de pratique. Ceci nous a amené à : 1) analyser l’évolution du concept de CdeP dans les principales œuvres de Wenger et de ses collaborateurs; 2) positionner les CdeP eu égard à d’autres formules de travail collaboratif (communauté, communauté d’apprentissage, communauté apprenante, organisation apprenante, communauté épistémique); 3) comparer le concept de collaboration avec d’autres s’en rapprochant (coopération notamment), de même que les notions d’interdisciplinarité et d’interprofessionnalité souvent associées à celle de CdeP; 4) étayer les dimensions cognitive et collaborative inhérentes aux expériences de CdeP; et 5) identifier les avancées et les limites attribuées à la mise en application du concept de CdeP dans le domaine de la santé. La métasynthèse (Beaucher et Jutras, 2007) dans une approche « compréhensive interprétative », s’est imposée comme méthode appropriée à notre intention de mieux saisir les expériences de collaboration interprofessionnelle menées dans le cadre des CdeP dans le domaine de la santé. La métasynthèse nous a ainsi permis d’obtenir une vue d’ensemble de la littérature portant sur les CdeP dans le domaine de la santé et de catégoriser les 194 articles rassemblés, pour ensuite retenir 13 articles traitant d’une réelle expérience de CdeP. Ainsi, dans une perspective contributoire, basée sur notre cadre conceptuel, l’analyse des principaux résultats de la métasynthèse, nous a permis 1) d’identifier les flous conceptuels liés aux expériences de CdeP rapportées, ainsi que 2) de dégager les retombées et les difficultés de leur mise en application dans le domaine de la santé et, enfin, 3) de démontrer la nécessité de mener des recherches visant l’étude du processus de développement de CdeP sur le terrain de la pratique de soins, notamment mais non exclusivement, lorsqu’il s’agit de maladies chroniques.

Profundizar en la dinámica del proceso educativo, con especial referencia a la salud mental del docente y a los distintos factores que sobre él gravitan. Profesorado en sus tres niveles: EGB, bachillerato y universitario. Fueron entrevistados 283 profesores, de los cuales 61 eran de EGB, 58 de bachillerato y 164 de la universidad. Sexo, edad, nivel docente, estado civil y los ítems de cada uno de los tests. General Health Questionnaire (GHQ) de Goldberg. Test de Langner. Escala de Zung para la depresión. Distribución de frecuencias, análisis de significación. Casi la mitad del profesorado estudiado presenta malestar psicológico importante, sin que sean significativas las diferencias entre varones y hembras. En cuanto a las tasas de afectación psicológica en los tres niveles del profesorado estudiado los profesores de EGB son los más sanos, los de bachillerato son los más afectados, siguiendo en afectación psicológica los de universidad. No existen diferencias estadísticamente significativas entre la salud psicosomática de los tres grupos de profesores, si bien los de EGB presentan una menor proporción de personas afectadas. La tensión y malestar psicosomático que se deja sentir entre docentes, los convierte en un grupo de alto riesgo en el campo de la higiene mental.

Calidad en salud y Satisfacción en salud son dos fenómenos de naturaleza distinta, sin embargo la literatura reporta que en las mediciones de ambas parecen evaluarse factores similares en la prestación de los servicios de salud. La calidad en salud hace referencia a las ¨creencias que como forma de la percepción forman parte de la cognición mientras que la satisfacción concebida como un juicio de valor positivo o negativo que construye el sujeto que ha recibido el servicio de acuerdo al cumplimiento de sus expectativas, es considerada uno de los principales objetivos de la calidad de atención en salud y un indicador del óptimo funcionamiento de las instituciones hospitalarias, así entonces, el objetivo de este estudio fue determinar cuáles son las características asistenciales y administrativas que potencialmente influirían en la valoración de la satisfacción del usuario y funcionario frente a la atención en los servicios de salud del Hospital Municipal del Guamo-Tolima, mediante la identificación, medición y descripción de las mismas .Los resultados del estudio obtenidos a partir de la comparación de medias de cada uno de los ítems evaluados, y de la diferencia significativa de medias, mostraron que existen diferencias significativas entre la importancia asignada por los usuarios frente a la asignada por los funcionarios.

Aim: Examine the relationship between the functional capacity and the quality of life related to health in university workers. Methodology: Cross-sectional study in 146 subjects, divided in two groups: Low functional Capacity (< 9 METs) and High functional Capacity (> 9.1 METs). We evaluated quality of life related to health (HRQOL-Health Questionnaire SF-12) and functional capacity (Questionnaire PAR/PAF) as indicators of health status. Results: 47.3% (69 men) and 52.7% (78 women). The average age of the groups was 35.0 ± 9.7 years (range 19,0-60,0 years). For HRQOL, the average found in the population assessed was 45.2 ± 4.42 (range 33,0-58,1) and 43.8 ± 6.87 (range 19,8-43,8) in components Physical Component Summary (PCS-12) and Mental Component Summary (MCS-12), respectively p = NS. Significant differences were found when comparing functional ability and sex, p<0,001 in both groups. Similarly, sex and mental component MCS-12 (group of Lower Functional) p = 0,049 as well as women and the physical component PCS-12, p = 0,05 between groups. Finally, a better score in HRQL observed in the group of High Capacity and functional components in both sex OR 0.59 (0.25-1.38). Conclusions: The results of this study demonstrate the relationship between High functional Capacity and a better HRQOL in this population.

ResumenEl abuso físico y el abuso psicológico representan un problema de salud pública frecuente en mujeres colombianas. No obstante, pocas investigaciones exploran las variables asociadas en mujeres adultas. Objetivo: establecer variables psicosociales asociadas a abuso físico y psicológico en mujeres de Bogotá (Colombia). Materiales y métodos: se diseñó un estudio analítico transversal. Se solicitó la participación de mujeres adultas con pareja de estrato socioeconómico bajo. Se aplicaron las escalas: Apgar familiar (disfunción familiar), Escala de Rosenberg (autoestima), Cuestionario General de Salud (GHQ-12, trastornos mentales comunes) e Índice de Abuso a la Pareja (Index of Spouse Abuse, ISA-8, ISA-F e ISA-P). Mediante regresión logística se ajustaron la variables psicosociales asociadas al abuso físico y al abuso psicológico. Resultados: participaron 292 mujeres. La media para la edad fue 33,0 años (DE = 9,23). La prevalencia de baja autoestima fue del 29,1%; de disfunción familiar, del 82,5%; de trastornos mentales comunes, del 73,6%; de abuso psicológico, del 68,5% y de abuso físico, del 70,2%. Los trastornos mentales comunes (OR = 4,0; IC 95% 2,2-7,5), baja autoestima (OR = 2,4; IC 95% 1,2-4,7) y disfunción familiar (OR = 2,3; IC 95% 1,1-4,8) se asociaron a abuso psicológico; y baja autoestima (OR = 2,6; IC 95% 1,4-5,0) y trastornos mentales comunes (OR = 2,4; IC 95% 1,4-4,3) a abuso físico. Conclusiones: es alta la frecuencia de maltrato conyugal en mujeres de estrato socioeconómico bajo de Bogotá. El trastorno mental común y la baja autoestima se asocian a abuso psicológico y a abuso físico.

Introducción: la osteogénesis es una patología de origen genético caracterizada por fragilidad ósea, en su curso natural los pacientes que la padecen se enfrentan a múltiples fracturas y múltiples intervenciones quirúrgicas, este tipo de pacientes por ser de alto riesgo necesitan técnicas quirúrgicas que aumenten el tiempo entre cada intervención y que demuestren un mayor impacto en el estado funcional. Objetivo: Determinar el impacto en el estado funcional de los pacientes con osteogénesis imperfecta llevados a tratamiento quirúrgico con clavos telescopados tipo Fassier Duval. Diseño: Estudio descriptivo prospectivo en el que se incluyeron 8 pacientes con diagnóstico de osteogénesis imperfecta, llevados a tratamiento quirúrgico con clavos telescopados tipo Fassier Duval desde el 2009 al 2013 a los cuales se les realizó seguimiento menor de 1 año del post operatorio. Resultados: La respuesta encontrada fue satisfactoria en la mayoría de los pacientes analizados 6 de 8, con cercanía a un estado funcional normal; un riesgo de caída bajo, incorporación y deambulación adecuada y una valoración funcional motora gruesa con valores cercanos al 100% identificando un buen nivel de independencia funcional. Se pudo demostrar que existieron cambios en los valores de la escala y que estos fueron estadísticamente significativos con p=0,028 indicando que el aumento dichos valores en el posoperatorio están relacionados con el procedimiento quirúrgico al utilizado en este grupo de pacientes. Conclusión: El tratamiento quirúrgico con el clavo telescopado de Fassier Duval en nuestra experiencia demostró tener una mejoría en el estado funcional de los pacientes del presente estudio, por lo tanto se sugiere la posibilidad de implementar su uso según este indicado con el fin de obtener un mejor resultado quirúrgico y funcional. Se exponen los problemas de salud mental y trastornos alimentarios con el propósito de aportar nuevos datos a la tarea de los educadores en una etapa importante de la educación como es la adolescencia. La investigación doctoral asume tres objetivos fundamentales: a) Comprobar la incidencia de los trastornos objeto de estudio o el riesgo de padecerlos. b) Analizar la prevalencia de alexitimia, los problemas de salud mental y el riesgo de padecer un trastorno alimentario en los adolescentes gallegos estudiantes de la ESO c) Comparar con otros estudios si los resultados conseguidos en los adolescentes gallegos presentan mayor o menor riesgo de padecer TCA, alexitimia o riesgos de salud. El estudio se realiza a una muestra representativa de 3389 sujetos, chicos y chicas, de primero a cuarto de ESO de las cuatro provincias de Galicia, de centros públicos y privados concertados y de zonas urbanas y rurales, se le aplican las pruebas de la TAS-20 (Bagby, Taylor y Parker, 1994) utilizando la adaptación española de Martínez y Sánchez (1996) para medir la Alexitimia, el 'General Health Questionnaire' (GHQ-28) (Cuestionario General de Salud) Goldberg y Hillier, 1979, para la Salud Mental y el Cuestionario de Actitudes ante la Alimentación (Eating Attitudes Test) (EAT) para los TCA. Las principales conclusiones ponen de manifiesto que los problemas que percibimos de TCA cada día se adelantan a edades más tempranas (11-13 años, primero de ESO) y aumentan a medida que pasa el tiempo. Aunque las cifras no son todavía alarmantes, sin embargo resulta un poco preocupante esta evolución que percibimos. Por otro lado constatamos que los adolescentes gallegos gozan de buena salud mental y no presentan problemas de alexitimia en general. Observamos que los alumnos con padres de mayor edad están más predispuestos a padecer problemas de salud mental. Los de centros públicos sufren más los problemas estudiados que los de los centros privados concertados, y los de zona rural más que los de la zona urbana. Las alumnas presentan más dificultad en las escalas de alexitimia a la hora de identificar y describir los sentimientos que los alumnos, pero estos tienen más problemas que ellas a la hora de exteriorizarlos. Donde se perciben más problemas de alexitimia es en los alumnos de primero y segundo de ESO y donde menos en los de cuarto. Presentan más problemas de salud mental los de tercero y también tienen mayores preocupaciones somáticas. Los que más sufren de ansiedad e insomnio son los alumnos de cuarto, y los que tienen más tendencia a la depresión severa son los alumnos de segundo. Y, por último, los alumnos que viven en las ciudades con mayor número de habitantes exteriorizan peor sus sentimientos que los de las poblaciones pequeñas, notándose también diferencias entre éstos, dependiendo de las provincias en las que residan.

OBJETIVO: Evaluar la salud percibida en fumadores de cigarrillos según las etapas del cambio. METODOLOGIA: Se estudiaron 253 personas mediante el cuestionario de salud SF-36 cada 6 meses durante dos años. RESULTADOS: Después del abandono del tabaco mejora la Evolución Declarada de la Salud y las percepciones de salud del componente físico y mental. Los fumadores que siguen fumando pero progresan en las etapas del cambio experimentan un descenso en Función Física, Salud General y en el Componente Sumario Físico. La recaída en el tabaquismo se asocia a peor Vitalidad y Salud Mental, y a considerar el consumo de tabaco como causa del deterioro actual de la salud. CONCLUSIONES: Se observa una peor percepción de salud física al plantearse el abandono del consumo de tabaco a medio plazo, una mejoría global de la salud percibida al dejar de fumar y una peor salud emocional en la recaída.

This study examined whether individual differences in perception of the quality of professional support available at a time of stress may be associated with security of attachment. We developed a new measure of parents' perceptions of emotional and practical support provided by a wide range of professionals involved in the treatment of infants with cleft lip. it showed good internal reliability and stability over 4 months. Mothers of 102 infants with cleft lip, with or without cleft palate, completed the measure at 2 and 6 months, together with the Parental Bonding Instrument and the General Health Questionnaire. Mean scores reflecting how much they could trust or talk frankly, or share their worst fears, with professionals, and the extent to which they saw them as a source of useful information or practical help, were lower among mothers with recollections of low maternal care in childhood, or high control. This was the case at 2 and 6 months, and there were some indications of an increasing contribution of low maternal care from 2 to 6 months. The associations were not explained by current depression. Further research is needed to clarify the role of attachment processes in parents' responses to serious medical conditions in their children, and into the implications for the way professionals in paediatric services provide support.

Objective: Relatives play a vital role in caring for patients with severe mental illness but receive inadequate support from psychiatric services. Evidence suggests that although intensive case management is directed primarily at patients, relatives may benefit a's well. This study examined whether relatives of patients who were receiving intensive case management had more contact with mental health professionals than relatives of patients who were receiving standard case management. It also examined whether relatives of patients receiving intensive case management appraised caregiving less negatively and experienced less psychological distress than relatives of patients receiving standard case management. Methods: The sample was drawn from the pool of patients participating in the UK700 randomized controlled trial of intensive case management. Prospective data on contact between case managers and the relatives of 146 patients were collected over a two-year period. At a two-year follow-up assessment, relatives of 116 patients were. interviewed with the Experience of Caregiving Inventory and the 12-item General Health Questionnaire. Results: Considerably more relatives of patients receiving intensive case management had contact with a case manager during the study period than relatives of patients receiving standard case management (70 percent compared with 45 percent). However, relatives of patients receiving intensive case management did not-appraise caregiving less negatively or experience less psychological distress than relatives of patients who were receiving. standard case management. Conclusions: Reducing case managers' caseloads alone. will not guarantee adequate support for relatives.. Instead, providing more support will need to be an explicit aim, and staff will require specific additional training to achieve it.

Adhering to treatment can be a significant issue for many patients diagnosed with chronic health conditions and this has been reported to be greater during the adolescent years. However, little is known about treatment adherence in teenage and young adult (TYA) patients with cancer. To increase awareness of the adherence challenges faced by these patients, we have reviewed the published work. The available evidence suggests that a substantial proportion of TYA patients with cancer do have difficulties, with reports that up to 63% of patients do not adhere to their treatment regimens. However, with inconsistent findings across studies, the true extent of non-adherence for these young patients is still unclear. Furthermore, it is apparent that there are many components of the cancer treatment regimen that have yet to be assessed in relation to patient adherence. Factors that have been shown to affect treatment adherence in TYA patients include patient emotional functioning (depression and self-esteem), patient health beliefs (perceived illness severity and vulnerability), and family environment (parental support and parent–child concordance). Strategies that foster greater patient adherence are also identified. These strategies are multifactorial, targeting not only the patient, but the health professional, family, and treatment regimen. This review highlights the lack of interventional studies addressing treatment adherence in TYA patients with cancer, with only one such intervention being identified: a video game intervention focusing on behavioural issues related to cancer treatment and care. Methodological issues in measuring adherence are addressed and suggestions for improving the design of future adherence studies highlighted, of which there is a great need.

Objective: Psychological problems should be identified in breast cancer patients proactively if doctors and nurses are to help them cope with the challenges imposed by their illness. Screening is one possible way to identify emotional problems proactively. Self-report questionnaires can be useful alternatives to carrying out psychiatric interviews during screening, because interviewing a large number of patients can be impractical due to limited resources. Two such measures are the Hospital Anxiety and Depression Scale (HADS) and the General Health Questionnaire-12 (GHQ-12). Method: The present study aimed to compare the performance of the GHQ-12, and the HADS Unitary Scale and its subscales to that of the Schedule for Affective Disorders and Schizophrenia (SADS) in identifying patients with affective disorders, including DSM major depression and generalized anxiety disorder. The sample consisted of 296 female breast cancer patients who underwent surgery for breast cancer a year previously. Results: A small number of patients (11%) were identified as having DSM major depression or generalized anxiety disorder based on SADS score. The findings indicate that the optimal thresholds in detecting generalized anxiety disorder and DSM major depression with the HADS anxiety and depression subscales were ≥ 8 and ≥ 7, with 93.3% and 77.3% sensitivity, respectively, and 77.9% and 87.1% specificity, respectively. They also had a 21% and 36% positive predictive value, respectively. Using the HADS Unitary Scale the optimal threshold for detecting affective disorders was ≥ 12, with 88.9% sensitivity, 80.7% specificity, and a 35% positive predictive value. In detecting affective disorders, the optimal threshold on the GHQ-12 was ≥ 2, with 77.8% sensitivity and 70.2% specificity. This scale also had a 24% positive predictive value. In detecting generalized anxiety disorder and DSM major depression, the optimal thresholds on the GHQ-12 were ≥ 2 and ≥ 4 with 73.3% and 77.3% sensitivity, respectively, and 67.5% and 82% specificity, respectively. The scale also had 12% and 29% positive predictive values, respectively. Conclusion: The HADS Unitary Scale and its subscales were effective in identifying affective disorders. They can be used as screening measures in breast cancer patients. The GHQ-12 was less accurate in detecting affective disorders than the HADS, but it can also be used as a screening instrument to detect affective disorders, generalized anxiety disorder, and DSM major depression.

It is noticeable that pressure, tension and overwork are frequent in health professionals routine. The work related to the ward area demands deep attention and surveillance. Because of that, it is essential to have a specific look at the humanization directed at health professionals, considering that taking care of other human beings is the essence of their job. This study has analyzed the psychic health levels, as well as the stress health professionals are submitted to, providing a debate about the humanization in 06 public hospitals (03 of them awarded by actions of humanization, and 03 not awarded) in Rio Grande do Norte state, Brazil. A study with 126 active health professionals (doctors, nurses, psychologists, nutritionists and social workers) in ward areas in their respective institutions was carried out. The thesis presented, with multi-disciplinary characteristic, counted on the support of statisticians (to calculate samples and data analysis), psychologists, social workers and administrators (linked to the human resources sector in each hospital). A cross-sectional study was performed, taking into consideration both quantitative and qualitative factors. The tools used for that were a semistructured questionnaire with socio-demographic characteristics, work and humanization; Lipp's Stress Symptoms Inventory for Adults (ISSL), and the Goldberg s General Health Questionnaire (QSG). The workers are predominantly women (84,9%), married (54,8%), between 46 and 55 years old (40,5%), working in the same institution for more than 20 years (22,2%), and between 16 and 20 years (20,6%), respectively. They work 40 hours a week (71,4%) and have multiple jobs (61,9%). Although most of these individuals global psychic health is in a good level, there are a significant number of people that is gradually getting worse concerning psychic stress (F1) showed by QSG (54,7%), and stress showed by ISSL(42,1%). Observing the categories, nurses (41,5%). Nutritionists (20,8%), doctors and social workers (18,9%), were among the most affected. About general health (F6), 63% of the awarded hospitals and 70% of the not awarded ones, presented good health levels (ranging from 5 to 50%). It was also noticed that, in the groups mentioned above, 25 and 20% respectively, were inserted in scores between 55 to 90%, what means that they are in worsening phase. The fact that the hospital is awarded or well recognized doesn t interfere in health professionals stress level and in their psychic health. Through what was heard from these individuals, it was possible to verify that they know little about humanization, once few of them identify or know that the service they offer is in an adoption process by Ministerial Policies. It was also detected the necessity of developing actions aimed at worker s health. Such results showed the importance of have more investments in programs that are directed to workers well-being, because they deal with other people s health and it is known that it is difficult for them to offer high-quality assistance if there are not suitable physical, psychological and material conditions to help them develop their jobs. As a warning, it is fair to say that investments in actions that provide humanized care to health professionals, mainly concerning preventive care for their health and life quality in their work

Institutional violence ranges from the most widespread lack of access to the poor quality of services provided. It includes abuses committed by virtue of the unequal power between patients and professionals within institutions. The aim of this study was to analyze the perception of women with regard to this type of violence, in the services offered at a reproductive health facility belonging to the National Health System (SUS) in Natal, Brazil. Interdisciplinary perspective is important, in that it provides interaction and complementarity between various disciplines, favoring, in an integrated way, a thematic approach in research activities, teaching and extension, involving professionals, students and researchers in medicine, social services, psychology, nursing, anthropology and physical therapy. A quantitative/qualitative approach was used, involving a sample of 401 women, as part of a transversal observational study. In the qualitative stage, which consisted of participatory observation and semi-structured interviews, we used an intentional sample of 10 individuals. The data were analyzed using logistic regression techniques, correspondence analysis and categorical thematic content analysis, showing that the 2 questions that investigated directly the perception of institutional violence obtained affirmative response frequencies of 28.2% and 31.8%, respectively. In regard to data collected in a field diary related to participatory observation, the main complaints referred to the health providerpatient relation, translated into dissatisfaction with the interpersonal relationship and with the resolution of the specific demand that required care. From content analysis, we classified 4 categories: Access; Information; Health professionalpatient relation; and Respect/dignity. We identified 6 subcategories: Impossibility of choice; Repressed demand; Communication difficulty; Asymmetric interpersonal relations; Privacy/confidentiality; Disrespect. We concluded, therefore, that the data presented show that in the reproductive health care programs, there are indicators of institutional violence. However, it is difficult to approach this phenomenon, mainly because of the power relations involved in the patient-health care provider interaction, resulting from unawareness that determinate situations violate sexual and reproductive rights. This can be explained by sociostructural questions that reveal marked inequalities, ratified by issues related to violation of the rights of National Health System (SUS) patients
