This paper makes use of a short, sharp, unexpected health shock in the form of the 2010 Colombian Dengue outbreak to examine the direct and indirect impact of negative health shocks on behaviour of households in affected areas. Our analysis combines data from several sources in order to obtain a comprehensive picture of the influence of the outbreak, and furthermore to understand the underlying mechanisms driving the effects. Our initial analysis indicates that the outbreak had a substantial negative effect on the health status of adults and adversely affected their ability to function as usual in their daily lives. In our aggregated school data, in areas with high levels of haemorrhagic Dengue we observe a reduction innational exam attendance (last year of secondary school) and on enrolment rates in primary education. Further analysis aims to exploit detailed individual level data to gain a more in depth understanding of the precise channels through which this disease influenced the behaviour and outcomes of the poor in Colombia.

Dengue and Chikungunya viruses cause the most important arthropod-borne viral infections for humans. These viruses are predominant in tropical and subtropical regions. In addition, these viruses are predominant in tropical and subtropical regions. Dengue mortality rate is around 1.2 to 3.5% and deaths due to chikungunya fever are around 1 in 1000; however, half of chikungunya-infected patients evolve into a chronic state that can persist for months up to years. There are no antiviral drugs available for DENV and CHIKV treatment and prevention. Moreover, vector control strategies have failed so far. Thus, the development of potent inhibitors for a broad spectrum of RNA viruses is urgently needed. We established and characterized a new embryonic insect cell line from Culex quinquefasciatus mosquito. Also we established the flaviviruses and alphavirus replication, both in C6/36 and Lulo insect cell lines, as well as in Vero cell line. In addition we carried out a reference compound library and reference panel of assays and data for DENV, which provides a benchmark for further studies. During this study, a panel of 9 antiviral molecules, with proven in vitro anti-dengue virus activity and that act at different stages of the DENV life cycle, was selected. Finally, Favipiravir or T-705, was identified as inhibitor in vitro and in vivo of alphaviruses and the mutation K291R in nsP4, which is responsible of the polymerase activity, was found as the mode of action in CHIKV. Interestingly, lysine in motif F1 is also highly conserved in positive-stranded RNA viruses and this might explain the broad spectrum of T-705 antiviral activity.

Em 2007-2008, o Rio de Janeiro viveu uma epidemia de dengue com 80.404 notificações, com 109 óbitos, 42% em < 15 anos. Objetivos: 1. Descrever a distribuição espacial e o perfil clínico-epidemiológico da epidemia de dengue por faixa etária no município do Rio de Janeiro. 2. Avaliar os fatores clínicos e laboratoriais preditivos de dengue grave em uma coorte de crianças internadas. Métodos: Esta tese foi formatada com dois artigos: 1) Distribuição espacial e caracterização clínico-epidemiológico do dengue no município do Rio de Janeiro; 2) Sinais clínicos e laboratoriais associados ao dengue grave em crianças hospitalizadas. Artigo 1: estudo ecológico com base no Sistema de Informação de Agravos de Notificação do município do Rio de Janeiro (2007-2008). Foram incluídos todos os casos de dengue confirmados pelo critério clínico-laboratorial dos residentes do município do Rio de Janeiro. Foram descritas as variáveis sócio demográficas e clínicas (classificação do caso, hospitalização e evolução) segundo a idade. Foi utilizado o critério de classificação de caso do Ministério da Saúde de 2005 (febre do dengue, dengue com complicações e febre hemorrágica do dengue - FHD). Autocorrelação espacial das taxas de incidência, mortalidade e letalidade foi verificada pelos índices de Moran e de Geary Artigo 2: coorte retrospectiva das hospitalizações por dengue (até 18 anos) em uma unidade de referência (2007-2008). Dengue grave foi definido pelo uso de terapia de suporte avançado de vida ou óbito. Diferenças de medianas e de proporções foram avaliadas, respectivamente, pelo teste de Mann-Whitney e pelo exato de Fisher (p<0,05). Foram calculados os parâmetros de acurácia dos sinais e sintomas. Resultados: Artigo 1: De 159887 notificações, 151527 eram residentes do município do Rio de Janeiro, 38.808 preencheram os critérios de inclusão. Destes, 8897 (22,9%) tiveram complicações e 1051 (2,7%) FHD; 16.436 (42,4%) eram < 18 anos e 15.288 (39,4%) foram confirmados laboratorialmente. A letalidade foi maior em lactentes (1,4%) e nos casos de FHD (7,7%). Extravasamento plasmático foi mais comum de 2 a 11 anos. Houve autocorrelação espacial global quanto à mortalidade e letalidade e autocorrelação local para os três índices (p<0,05). Artigo 2: De 145 pacientes, 53,1% eram do sexo feminino, com mediana de idade de 8,7 anos (IIQ=7,2-11,5); 15,9% evoluíram para gravidade A duração da febre em dias foi menor no grupo grave (G: Mediana Md=3,0, IIQ= 2,0\20134,5; NG: Md=4,0, IIQ=3,0\20135,0; p=0.062). Letargia, irritabilidade, sangramento, dispneia, ausculta pulmonar alterada, derrame pleural, distensão abdominal e edema foram associados à evolução para gravidade (p<0,05). Níveis de hematócrito (p=008) e de albumina sérica (p<0,001) foram menores nos graves. Letargia apresentou Valor Preditivo Negativo (VPN) > 90%, com Valor Preditivo Positivo (VPP) de 80%. As demais variáveis com p-valor <0,05 apresentaram VPN próximo ou > 90%, com baixo VPP. Conclusão: Apesar da incidência de FHD ter sido maior entre escolares, os lactentes tiveram maior letalidade. Extravasamento plasmático foi mais frequente em escolares e pré-escolares. Sangramento, sonolência e irritabilidade na admissão hospitalar associaram-se à gravidade e o derrame cavitário obteve maior acurácia do que a hemoconcentração como sinal de extravasamento plasmático

Dengue virus type 4 (DENV-4) circulates in tropical and subtropical countries from Asia and the Americas. Despite the importance of dengue virus distribution, little is known about the worldwide viral spread. Following a Bayesian phylogenetic approach we inferred the evolutionary history of 310 isolates sampled from 37 countries during the time period 1956-2008 and the spreading dynamics for genotypes I and II. The region (tropical rainforest biome) comprised by Malaysia-Thailand was the most likely ancestral area from which the serotype has originated and spread. Interestingly, cross-correlation analysis on demographic time series with the Asian sequences showed a statistically significant negative correlation that could be suggestive of competition among genotypes within the same serotype. The dengue virus NS1 protein has been shown to be a protective antigen under different experimental conditions but the recombinant protein produced in bacterial expression systems is usually not soluble and loses structural and immunological features of the native viral protein In the present study, experimental conditions leading to purification and refolding of the recombinant dengue virus type 2 (DENV-2) NS1 protein expressed in Escherichia coil are described The refolded recombinant protein was recovered as heat-stable soluble dimers with preserved structural features, as demonstrated by spectroscopic methods In addition, antibodies against epitopes of the NS1 protein expressed in eukaryotic cells recognized the refolded protein expressed in E coli but not the denatured form or the same protein submitted to a different refolding condition Collectively, the results demonstrate that the recombinant NS1 protein preserved important conformation and antigenic determinants of the native virus protein and represents a valuable reagent either for the development of vaccines or for diagnostic methods. In this study, we revisited the phylogeography of the three of major DENV-3 genotypes and estimated its rate of evolution, based on the analysis of the envelope (E) gene of 200 strains isolated from 31 different countries around the world over a time period of 50 years (1956-2006). Our phylogenetic analysis revealed a geographical subdivision of DENV-3 population in several country-specific clades. Migration patterns of the main DENV-3 genotypes showed that genotype I was mainly circumspect to the maritime portion of Southeast-Asia and South Pacific, genotype 11 stayed within continental areas in South-East Asia, while genotype III spread across Asia, East Africa and into the Americas. No evidence for rampant co-circulation of distinct genotypes in a single locality was found, suggesting that some factors, other than geographic proximity, may limit the continual dispersion and reintroduction of new DENV-3 variants. Estimates of the evolutionary rate revealed no significant differences among major DENV-3 genotypes. The mean evolutionary rate of DENV-3 in areas with long-term endemic transmissions (i.e., Indonesia and Thailand) was similar to that observed in the Americas, which have been experiencing a more recent dengue spread. We estimated the origin of DENV-3 virus around 1890, and the emergence of current diversity of main DENV-3 genotypes between the middle 1960s and the middle 1970s, coinciding with human population growth, urbanization, and massive human movement, and with the description of the first cases of DENV-3 hemorrhagic fever in Asia. A comparison of dengue virus (DENV) antibody levels in paired serum samples collected from predominantly DENV-naive residents in an agricultural settlement in Brazilian Amazonia (baseline seroprevalence, 18.3%) showed a seroconversion rate of 3.67 episodes/100 person-years at risk during 12 months of follow-up. Multivariate analysis identified male sex, poverty, and migration from extra-Amazonian states as significant predictors of baseline DENY seropositivity, whereas male sex, a history of clinical diagnosis of dengue fever, and travel to an urban area predicted subsequent seroconversion. The laboratory surveillance of acute febrile illnesses implemented at the study site and in a nearby town between 2004 and 2006 confirmed 11. DENV infections among 102 episodes studied with DENV IgM detection, reverse transcriptase-polymerise chain reaction, and virus isolation; DENV-3 was isolated. Because DENV exposure is associated with migration or travel, personal protection measures when visiting high-risk urban areas may reduce the incidence of DENV infection in this rural population.
