Lo scompenso cardiaco è una sindrome clinica complessa di notevole prevalenza ed incidenza nella popolazione generale, con elevata mortalità e morbidità. Le numerose alterazioni strutturali e funzionali che lo caratterizzano sono in grado di generare contemporaneamente diversi tipi di alterazioni del ritmo: dalle tachiaritmie ventricolari/sopraventricolari alle turbe della conduzione dell’impulso con bradi-aritmie e dissincronie di contrazione. La cardioversione della fibrillazione atriale e la resincronizzazione cardiaca rappresentano due terapie elettriche molto importanti in tale contesto. Le modificazioni emodinamiche, funzionali e neuro-ormonali indotte da tali trattamenti, tanto in acuto che a medio/lungo termine, possono generare numerose informazioni sulla fisio-patologia di questa sindrome. Il progetto scientifico alla base del presente manoscritto è costituito da due studi volti ad affrontare separatamente le tematiche accennate. Il primo studio è stato focalizzato sulle modificazioni indotte in 38 pazienti dalla terapia di resincronizzazione cardiaca sui parametri di circolazione periferica. I risultati ottenuti evidenziano come il trattamento consenta un incremento del flusso muscolare, soprattutto in chi presenterà un rimodellamento ventricolare inverso. La diversa eziologia sottostante (ischemica vs. non ischemica) appare influenzare i parametri relativi alla circolazione periferica tanto in acuto, quanto in cronico. Il diverso comportamento in merito alle variabili della circolazione periferica nelle valutazioni seriate suggerisce che la terapia di resincronizzazione cardiaca abbia, principalmente nei pazienti responders, effetti non solo “centrali”, e che essi non siano puramente meccanici ma mediati da fattori probabilmente di natura neuro-ormonale. Il secondo studio si è occupato della cardioversione elettrica esterna di fibrillazione atriale, un’aritmia che presenta strette relazioni con l’insufficienza cardiaca. 242 pazienti sono stati sottoposti a cardioversione elettrica esterna con onda bifasica, utilizzando due 1 diverse configurazioni di erogazione dell’energia: antero-posteriore con patch adesivi e antero-apicale con piastre standard. Il ripristino del ritmo sinusale è stato ottenuto in oltre l’80% dei pazienti già col primo shock a 120J. Sebbene fra le due metodiche non si evidenzi una significatività in termini di efficacia, i risultati ottenuti suggeriscono che la scelta della specifica configurazione di shock dovrebbe prendere in considerazione anche alcune variabili biometriche: peso, altezza e superficie corporea del paziente. Il ripristino ed il mantenimento del ritmo sinusale inducono un’importante modificazione della concentrazione di NT-pro-BNP. Un’elevata attivazione neuro-ormonale pre-procedura predispone alle recidive a medio-lungo termine, mentre le recidive nel breve-medio periodo appaiono influenzate da tale fattore solo in corso di profilassi anti-aritmica per il mantenimento del ritmo sinusale. In conclusione i risultati del progetto di ricerca sottolineano come trattamenti mirati a parametri strettamente cardiaci (ritmo e conduzione dell’impulso) siano in grado di determinare importanti modificazioni sugli equilibri emodinamici e neuro-ormonali dell’intero organismo, confermando la stretta relazione tra questi parametri e l’evoluzione del quadro clinico.

Charakterisierung synapsenassoziierter Proteine des Haushuhns(Gallus gallus domesticus) Die Familie der synapsenassoziierten Proteine (SAP) umfaßt bei Säugern vier Proteine: SAP90 (=PSD-95), SAP97, SAP102 (=PSD-93) und Chapsyn110. Die Proteine enthalten charakteristischerweise drei PDZ-Domänen, eine SH3-Domäne und eine GK-Domäne über die sie mit anderen Proteinen interagieren können. SAP können so Verbindungen zwischen Neurotransmitterrezeptoren und Signaltransduktionsmolekülen sowie dem Zytoskelett herstellen.In dieser Arbeit wurden die synapsenassoziierten Proteine des Huhns charakterisiert. Die cDNAs von SAP90, SAP97 und Chapsyn110 wurden sequenziert. Die cDNA von SAP102 wurde teilweise sequenziert. Die Analyse genomischer DNA durch PCR ergab, daß die SAP90- und SAP97-mRNA von einem Gen transkribiert werden. Die mRNA-Verteilung von SAP90, SAP97 und Chapsyn110 im Gehirn einen Tag alter Küken wurde mit in situ Hybridisierung untersucht. Die Verteilung der SAP90-mRNA und von NMDA-Rezeptoren im Gehirn des Huhns ist sehr ähnlich. Weiterhin wurde bei Küken untersucht, inwieweit SAP bei der Prägung eine Rolle spielen. Der relative mRNA-Gehalt von SAP90, SAP97 und Chapsyn110 wurde 30 Minuten, 5 Stunden und 10 Stunden nach einer akustische Prägung der Küken gemessen. Fünf Stunden nach akustischer Prägung war der Gehalt der SAP90-mRNA, im anterioren lateralen Hyperstriatum ventrale um 13% erhöht. Der mRNA-Gehalt in anderen Regionen und der anderen SAP-Gene war unverändert.

Die Zellgenealogie des Polychaeten Platynereis dumerilii wurde durch Farbstoffinjektion in die Blastomeren des 2-, 4- und 8-Zellstadiums, sowie die Zellen 2d, 2d112, 4d und 4d1 untersucht. Injektionen gelangen durch Aufweichung der Vitellinhülle mittels Dithioerythritol und Trypsin. Die injizierten Keime wurden zur Trochophora bzw zum dreisegmentigen Jungwurm aufgezogen, fixiert und mit dem konfokalen Rasterlichtmikroskop dreidimensional aufgenommen. Die animal-vegetale Achse des Frühkeims entspricht der antero-posterioren Achse des Jungwurms. Die Mikromeren des ersten Quartetts sind radiär um die antero-posteriore Achse angeordnet und bilden den Kopf. Die Mikromere 2d proliferiert bilateralsymmetrisch von der dorsalen Mittellinie aus und liefert das gesamte Rumpfektoderm. Indirekt ließ sich ableiten, daß die Mikromeren 2a1 bis 2c1 schmale ektodermale Streifen zwischen Kopf und Rumpf bilden und aus 2a2 und 2c2 das ektodermale Stomodaeum hervorgeht. Die Mikromeren des dritten Quartetts sowie möglicherweise 2b2 bilden 'Ektomesoderm'. 4d proliferiert ebenfalls bilateralsymmetrisch von der dorsalen Mittellinie aus zum Rumpfmesoderm und liefert vielleicht noch kleine Beiträge zum Aufbau des Darmes. Der Mitteldarm stammt von den dotterreichen Makromeren 4A bis 4D.

Background: studi in letteratura mostrano come l'accelerometria stia diventando una pratica clinica (e non solo) molto utile sia in ambito diagnostico sia riabilitativo; molti di essi sfruttano i dati provenienti da più sensori inerziali posti su un soggetto e ne ricavano l'andamento accelerometrico dei vari segmenti corporei durante il movimento in esame. Obiettivo: caratterizzare il cammino di soggetti sani ed osservare quello di alcuni casi patologici, parametrizzando l'accelerazione Antero-Posteriore. Gli indici sono: il Range dell'accelerazione, la Simmetria sulle accelerazioni e sugli intervalli di tempo tra un appoggio al suolo ed il successivo, la Simmetria nel dominio delle frequenze. Strumenti e Metodi: 10 volontari clinicamente sani hanno svolto 5 prove a velocità Spontanea, Lenta e Lentissima con il sensore inerziale G-Walk (BTS Milano) all'altezza di L5. I dati, acquisiti mediante il software G-Studio, sono stati elaborati grazie a Matlab. Risultati: è definita una fascia di normalità per ogni velocità. Conclusioni: le differenze tra il cammino sano e quello patologico si manifestano nelle grandezze di Range ed Indice di Simmetria sulle accelerazioni. I due parametri che misurano la simmetria nel dominio dei tempi ed in quello delle frequenze sono invece sostanzialmente simili. Tale distinzione è importante in medicina, perchè permette di definire le cause di deficit motorio, il percorso chirurgico/riabilitativo da effettuare e l'entità del recupero funzionale.

I test clinici e i questionari generalmente non sono in grado di fornire una valutazione predittiva e quantitativa della stabilità motoria. Permettono al clinico di esaminare la forza muscolare del paziente, il grado di spasticità, la funzionalità motoria e l'autonomia nello svolgimento delle normali attività, ma non di capire quanto il soggetto sia stabile. Sono stati esaminati diciotto pazienti con esiti di stroke in fase post acuta ed il 38% ha affermato di essere caduto almeno una volta nell'arco degli ultimi dodici mesi. Adottando una politica di prevenzione delle cadute si potrebbero limitare questi eventi che vanno ad aggravare un quadro clinico già compromesso. Per tale motivo sono attualmente oggetto di studio misure biomeccaniche, eseguite in laboratorio, atte a definire metodi con alta sensibilità e specificità per la valutazione della stabilità del cammino. Nel presente lavoro le misure strumentali sono state ottenute partendo dal segnale di accelerazione del centro di massa corporeo. Servendosi di un'unità inerziale munita di accelerometro triassiale è stato possibile, durante il cammino, ricavare l'andamento delle accelerazioni antero-posteriore, medio-laterale e verticale. Grazie ad un algoritmo, messo a punto nel Laboratorio di Bioingegneria della Facoltà di Cesena dall'Ing. Federico Riva, sono stati estrapolati gli indici strumentali. Il corpo centrale di questa tesi consiste nell'analisi statistica condotta tramite modelli di regressione lineare che mettono in correlazione parametri clinici (acquisiti per mezzo di test e questionari) abitualmente usati in ospedale e indici strumentali.

This study deals with the function and regulation of programmed cell death, or apoptosis, in the development of the embryonic central nervous system of Drosophila melanogaster. The first part provides a description of apoptosis-deficient embryos, which showed that preventing apoptosis does not cause gross morphological defects in the CNS, as it appears well organized despite the presence of too many cells. An analysis of the incidence and pattern of apoptosis over the course of development discloses a partly very orderly pattern suggesting tight spatio-temporal control, but also reveals random apoptotic cells, which suggests a certain amount of plasticity in the embryo. This analysis also allowed precise identification of some of the dying neural cells in the embryo, and establishment of single cell models for studying regulation of segment-specific apoptosis in the embryonic CNS. In the second part of the work, further investigations into mechanisms controlling segment-specific apoptosis revealed the involvement of two Hox genes, Antennapedia (Antp) and Ultrabithorax (Ubx), in this process. Hox genes control the formation of segment-specific structures in their domains of expression, but also regulate organ and tissue morphogenesis. The study presented here shows that Antp and Ubx play antagonistic roles in motoneuron survival in the embryo. Ubx expression in the CNS is strongly upregulated at a late point in development, when most cells have begun to differentiate. This upregulation shortly precedes Ubx-dependent, segment-specific apoptosis of two differentiated motoneurons. It could further be demonstrated that Antp is required for proper development of the NB7-3 lineage and for survival of the NB7-3 motoneuron in the anterior thoracic segments. In segments where Antp and Ubx expression overlaps, Ubx counteracts the anti-apoptotic function of Antp, resulting in cell death. Thus, these two Hox genes play opposing roles in the survival of differentiated neurons in the late developing nervous system. They thereby contribute to establishment of correct connections between outward-projecting neurons and their targets, which is crucial for the assembly of functional neural circuits, as these have to fulfill region-specific locomotion and sensory requirements along the antero-posterior body axis.

La camminata, sia in acqua sia a secco, è un esercizio praticato frequentemente in ambito riabilitativo. Lo scopo di questa tesi è di confrontare variabilità e stabilità del cammino dentro e fuori dall’acqua, al fine di comprendere le caratteristiche dei due tipi di cammino. L’analisi si è focalizzata sulla valutazione di misure di stabilità e variabilità che non erano mai state indagate prima per la camminata in acqua: Harmonic Ratio, Plot di Poincarè, moltiplicatori di Floquet, esponenti di Lyapunov, multiscale entropy, recurrence rate, determinism, averaged diagonal line length, maximum diagonal line length e divergenza. Gli indici sono stati calcolati su camminate di 30 e di 90 passi. Da questa analisi possiamo stimare anche quale parametro è già affidabile a 30 passi e non cambia se analizzato a 90 o se un parametro necessita di più passi per risultare affidabile. Inoltre è stata effettuata indagine statistica tramite test di Kruskal-Wallis. Dall'analisi è emerso che la camminata in acqua è più stabile, meno smooth, c’è un maggiore controllo posturale lungo gli assi antero-posteriore e medio-laterale e che il cammino è condotto in maniera meno automatica.

Cardiovascular disease (CVD) due to atherosclerosis of the arterial vessel wall and to thrombosis is the foremost cause of premature mortality and of disability-adjusted life years (DALYs) in Europe, and is also increasingly common in developing countries.1 In the European Union, the economic cost of CVD represents annually E192 billion1 in direct and indirect healthcare costs. The main clinical entities are coronary artery disease (CAD), ischaemic stroke, and peripheral arterial disease (PAD). The causes of these CVDs are multifactorial. Some of these factors relate to lifestyles, such as tobacco smoking, lack of physical activity, and dietary habits, and are thus modifiable. Other risk factors are also modifiable, such as elevated blood pressure, type 2 diabetes, and dyslipidaemias, or non-modifiable, such as age and male gender. These guidelines deal with the management of dyslipidaemias as an essential and integral part of CVD prevention. Prevention and treatment of dyslipidaemias should always be considered within the broader framework of CVD prevention, which is addressed in guidelines of the Joint European Societies’ Task forces on CVD prevention in clinical practice.2 – 5 The latest version of these guidelines was published in 20075; an update will become available in 2012. These Joint ESC/European Atherosclerosis Society (EAS) guidelines on the management of dyslipidaemias are complementary to the guidelines on CVD prevention in clinical practice and address not only physicians [e.g. general practitioners (GPs) and cardiologists] interested in CVD prevention, but also specialists from lipid clinics or metabolic units who are dealing with dyslipidaemias that are more difficult to classify and treat.

BACKGROUND AND PURPOSE: Daily use of conventional electronic portal imaging devices (EPID) for organ tracking is limited due to the relatively high dose required for high quality image acquisition. We studied the use of a novel dose saving acquisition mode (RadMode) allowing to take images with one monitor unit per image in prostate cancer patients undergoing intensity-modulated radiotherapy (IMRT) and tracking of implanted fiducial gold markers. PATIENTS AND METHODS: Twenty five patients underwent implantation of three fiducial gold markers prior to the planning CT. Before each treatment of a course of 37 fractions, orthogonal localization images from the antero-posterior and from the lateral direction were acquired. Portal images of both the setup procedure and the five IMRT treatment beams were analyzed. RESULTS: On average, four localization images were needed for a correct patient setup, resulting in four monitor units extra dose per fraction. The mean extra dose delivered to the patient was thereby increased by 1.2%. The procedure was precise enough to reduce the mean displacements prior to treatment to < o =0.3 mm. CONCLUSIONS: The use of a new dose saving acquisition mode enables to perform daily EPID-based prostate tracking with a cumulative extra dose of below 1 Gy. This concept is efficiently used in IMRT-treated patients, where separation of setup beams from treatment beams is mandatory.

Traditionally, non-invasive monitoring of tidal volume in infants has been performed using impedance plethysmography analyzed using a one or two compartment model. We developed a new laser system for use in infants, which measures antero-posterior movement of the chest wall during quiet sleep. In 24 unsedated or sedated infants (11 healthy, 13 with respiratory disease), we examined whether the analysis of thoracoabdominal movement based on a three compartment model could more accurately estimate tidal volume in comparison to V(T) measured at the mouth. Using five laser signals, chest wall movements were measured at the right and left, upper and lower ribcage and the abdomen. Within the tidal volume range from 4.6 to 135.7 ml, a three compartment model showed good short term repeatability and the best agreement with tidal volume measured at mouth (r(2) = 0.86) compared to that of a single compartment model (r(2) = 0.62, P < 0.0001) and a two compartment model (r(2) = 0.82, P < 0.01), particularly in the presence of respiratory disease. Three compartment modeling of a 5 laser thoracoabdominal monitoring permits more accurate estimates of tidal volume in infants and potentially of regional differences of chest wall displacement in future studies.

Ornithine transcarbamylase (OTC; EC 2.1.3.3) is a hepatic enzyme involved in ammonia elimination via the urea cycle. Since the sequence of the OTC gene was reported many types of mutations continue to be found in OTC deficiency patients, continuing to increase the already wide mutational spectrum known for this gene. In this study we present the clinical, biochemical and molecular features of thirteen late-onset OTC deficiency patients. Mutations were identified in all these patients, among which six were novel point substitutions (L59R, A137P, L148S, Y176L, L186P, and K210N) and one was a 2-bp deletion at exon 4 (341-342delAA). In addition, a de novo genomic deletion of maternal origin encompassing exons 1 to 5 was also identified by the analysis of LD patterns using intragenic polymorphic markers. This work exemplifies the potential value of population genetic studies for the detection of large deletions.

OBJECTIVE: In transcranial magnetic stimulation (TMS) of the motor cortex, the optimal orientation of the coil on the scalp is dependent on the muscle under investigation, but not yet known for facial muscles. METHODS: Using a figure-of-eight coil, we compared TMS induced motor evoked potentials (MEPs) from eight different coil orientations when recording from ipsi- and contralateral nasalis muscle. RESULTS: The MEPs from nasalis muscle revealed three components: The major ipsi- and contra-lateral middle latency responses of approximately 10 ms onset latency proved entirely dependent on voluntary pre-innervation. They were most easily obtained from a coil orientation with posterior inducing current direction, and in this respect resembled the intrinsic hand rather than the masseter muscles. Early short duration responses of around 6 ms onset latency were best elicited with an antero-lateral current direction and not pre-innervation dependent, and therefore most probably due to stimulation of the nerve roots. Late responses (>18 ms) could inconsistently be elicited with posterior coil orientations in pre-innervated condition. CONCLUSIONS: By using the appropriate coil orientation and both conditions relaxed and pre-innervated, cortically evoked MEP responses from nasalis muscle can reliably be separated from peripheral and reflex components and also from cross talk of masseter muscle activation.

Sequential studies of osteopenic bone disease in small animals require the availability of non-invasive, accurate and precise methods to assess bone mineral content (BMC) and bone mineral density (BMD). Dual-energy X-ray absorptiometry (DXA), which is currently used in humans for this purpose, can also be applied to small animals by means of adapted software. Precision and accuracy of DXA was evaluated in 10 rats weighing 50-265 g. The rats were anesthetized with a mixture of ketamine-xylazine administrated intraperitoneally. Each rat was scanned six times consecutively in the antero-posterior incidence after repositioning using the rat whole-body software for determination of whole-body BMC and BMD (Hologic QDR 1000, software version 5.52). Scan duration was 10-20 min depending on rat size. After the last measurement, rats were sacrificed and soft tissues were removed by dermestid beetles. Skeletons were then scanned in vitro (ultra high resolution software, version 4.47). Bones were subsequently ashed and dissolved in hydrochloric acid and total body calcium directly assayed by atomic absorption spectrophotometry (TBCa[chem]). Total body calcium was also calculated from the DXA whole-body in vivo measurement (TBCa[DXA]) and from the ultra high resolution measurement (TBCa[UH]) under the assumption that calcium accounts for 40.5% of the BMC expressed as hydroxyapatite. Precision error for whole-body BMC and BMD (mean +/- S.D.) was 1.3% and 1.5%, respectively. Simple regression analysis between TBCa[DXA] or TBCa[UH] and TBCa[chem] revealed tight correlations (n = 0.991 and 0.996, respectively), with slopes and intercepts which were significantly different from 1 and 0, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Lacebacks may be used to limit unwanted incisor proclination during initial orthodontic alignment; however, their use has not met with universal approval. This systematic review aims to appraise the evidence in relation to the effectiveness of lacebacks in controlling incisor position during initial alignment. Electronic database searches of published literature (MEDLINE via Ovid, Cochrane Central Register of Controlled Trials, LILACS, and IBECS) and unpublished literature were performed. Search terms used included randomized controlled trial, controlled clinical trial, random allocation, double blind method, orthodontics, and laceback. Data were extracted using custom forms. Risk of bias assessment was made using the Cochrane Collaboration risk of bias tool. The quality of the evidence was also assessed using GRADE. Mean differences in incisor inclination and antero-posterior changes in incisor and molar position during alignment were calculated. Two studies involving 97 participants were found to be at low risk of bias and were included in the quantitative synthesis. The random effects meta-analysis demonstrated that the use of lacebacks was associated with 0.5 mm greater posterior movement of the incisors during alignment; this finding was of limited clinical importance and statistically non-significant [95 per cent confidence interval (CI): -1.25, 0.25, P = 0.19]. Little difference (0.46 mm) was also found between laceback and non-laceback groups with regards to mesial molar movement (95 per cent CI: -0.33, 1.24, P = 0.26). According to the GRADE assessment, the overall quality of evidence relating to the use of lacebacks was high. There is no evidence to support the use of lacebacks for the control of the sagittal position of the incisors during initial orthodontic alignment.

In a patient with staphylococcus lugdunensis prosthetic aortic valve endocarditis and coronary septic embolism accompanied by antero-lateral myocardial infarction, embolic material was successfully aspirated from the bifurcation of the left anterior descending coronary artery and the first diagonal branch. A good angiographic result was documented six months thereafter when the patient presented with a second complication, pulsatile compression of the left main coronary artery by an abscess cavity originating between the aortic and mitral annulus, leading to congestive heart failure. The patient underwent successful surgical replacement of the aortic valve prosthesis with concomitant patch reconstruction of the annulus as well as tricuspid annuloplasty.
